- 0:04>> Dr. Terry Simpson: Hey, welcome back to 4Q. Today we are launching a
- 0:07new series about longevity and health span. Now,
- 0:11longevity is easy to measure because there's a
- 0:13date on every tombstone. But health span is
- 0:16different. Healthspan we define as the time when
- 0:19you're healthy, mobile, curious, and capable
- 0:21before disease or aging slows you down. That's the
- 0:26part we want to stretch out, because living longer
- 0:28isn't the same as living well. Now, the moment you
- 0:32step into the world of longevity, you meet a
- 0:35parade of gurus, pouters, and podcasts. There's
- 0:38some serious scientists in the field, like the
- 0:41founder of the whole field, Leonard Hayflick, who
- 0:43recently died at age 93. He was the one who
- 0:46defined cellular aging in the first place. Or Dr.
- 0:50Eric Topol, who actually advance his medicine. But
- 0:54then there are those who have turned longevity
- 0:56into a luxury brand. Take Peter Attia. In his many
- 1:01iterations of himself, he finally settled on
- 1:03building an empire where he can charge $150,000 or
- 1:07more for a patient to become a patient of his. And
- 1:11what do you get for that? You get a handful of
- 1:13basic lab tests. You get a VO2 max run. You get to
- 1:17hike with him with a rucksack, a weighted
- 1:19backpack. You learn a few yoga poses, and you get
- 1:22some lectures about how you should sleep better.
- 1:24Some of this you can get from your local primary
- 1:26care doctor and a local gym for about 30 bucks a
- 1:29month and copay. So remember, this is a man who
- 1:31never finished a surgical residency, but
- 1:34billionaires pay him because they can. He
- 1:36positioned himself exceedingly well. The man who
- 1:40has gone from being all pro keto to let's do all
- 1:43the exercise we can. By the way, he's also an
- 1:47investor in something called AG1, that bright
- 1:50green drink sold as nutrition in a scoop. And of
- 1:54course, there's Andrew Huberman. He is another AG1
- 1:57investor. He podcasts between studio lights that
- 2:00are bright enough to give you a tan, and he seems
- 2:02to always wear black. Now, both of these guys are
- 2:05smart, they are accomplished, but let's be honest,
- 2:08they live where marketing meets medicine. And
- 2:12sometimes the marketing wins. There's the other
- 2:16longevity star. You've probably heard of Dr. David
- 2:18Sinclair. He's a Harvard professor, and he turns
- 2:21something called nad, which is a supplement
- 2:24almost, into a religion. And the precursor
- 2:27molecules, these are initials you're going to
- 2:29hear. Nmn, M and nr. They sound like secret
- 2:33passwords to eternal health. You know David
- 2:36Sinclair's early work on aging and sirtuins? It
- 2:39was brilliant. But somewhere between Harvard and
- 2:42Hollywood, the science and the sales pitch,
- 2:44started to share a toothbrush. He even formed a
- 2:47company. And he tried to patent NMN as a drug.
- 2:50Now, when you try and patent a supplement as a
- 2:52drug, the FDA does what they would always do. They
- 2:55pulled it from the supplement market. Now this
- 2:57sparked all of these rhettic panics and these
- 2:59headlines that the government was hiding. The
- 3:01fountain of youth. You can buy NMN now. But that
- 3:04fiasco showed just how thin the line is between
- 3:07research and retail and, and how desperate people
- 3:10are for that fountain of youth. Now, no longevity
- 3:15carnival would be complete without the shirtless
- 3:17salesmen of supplements and scams, the ones who
- 3:20promise eternal youth. Everything from protein
- 3:22powder and ancestral vibes. Take Paul Saladino.
- 3:26Hasn't seen a patient in years, discovered that
- 3:29fruit is good for you, even after declaring steak
- 3:32with salvation. Or the liver King, his erstwhile
- 3:35partner, who discovered that, well, his secret
- 3:37wasn't liver, but it was ever he was injecting.
- 3:40Then, of course, no claim would be complete
- 3:43without mentioning Gary Breca, the guy who says
- 3:46he's a human biologist, whatever that is, and can
- 3:49predict the date of your death. Of course, all you
- 3:52have to do is buy some of his fancy amino acids,
- 3:54his hydrogen water bottles, and all of his scams
- 3:57in between. Those people aren't scientists.
- 4:01They're basically services in human forms. Now
- 4:04here at 4Q, we're not selling miracle molecules or
- 4:07$150,000 wellness plans. We're bringing you the
- 4:11real science. What we know, what we don't, and how
- 4:15to tell the difference. Because understanding
- 4:17longevity shouldn't require six figure copay or a
- 4:20discount code. And as you will learn, you do not
- 4:24have to be a billionaire to learn about it and to
- 4:26implement it.
- 4:27But today, we're going to start with nad, which
- 4:31stands for nicotinamide adenine dinucleotide. This
- 4:36is the so called molecule of life. Now, does it
- 4:39actually work in humans? We're going to find out.
- 4:45I'm Dr. Terry Simpson, your chief medical
- 4:47explanationist, and this is Fork U Fork
- 4:51University, where we bust myths, make sense of the
- 4:53madness, and teach you a little bit about food and
- 4:56medicine. NAD is something that every freshman
- 5:03medical student learns in basic biochemistry about
- 5:07how the body works. It responsible for fueling
- 5:10energy production. It's in the Krebs cycle. It's
- 5:13responsible for DNA repair. And as we age, NAD
- 5:17levels drop, metabolism slows, inflammation rises,
- 5:21cells get sloppy, and blood, like Leonard Hayflick
- 5:23pointed out in the 1960s, senescent. So scientists
- 5:27ask the most obvious of all, can we top it up?
- 5:31Meaning can we Give external NAD or one of its
- 5:34precursors and kind of turn these cells back into
- 5:37powerhouses again, make those mitochondria shine.
- 5:41Well, it turns out in mice, the answer is yes.
- 5:45Boosting NAD in mice by using precursors such as
- 5:49NR or NMN extends the lifespan of the mice by 5 to
- 5:5415%. That translates to just a few months for a
- 5:57mouse. But if you're thinking about a human mouse,
- 6:01years in humans might work out to a couple of
- 6:03years. But in mice, we found that the mice moved
- 6:07better, they burned energy better, and as far as
- 6:11we could tell, they aged a little slower. But, uh,
- 6:14here's the catch. Human beings, we're not just
- 6:17mice with mortgages. Here's the thing. You cannot
- 6:21eat nad, but you can eat its precursors,
- 6:25tryptophan and niacin, which is vitamin B3. These
- 6:29are found in fish, poultry, beans, milk, and whole
- 6:34grains. Does this sound familiar? Yeah, we call
- 6:38that the Mediterranean diet. So before you drop 90
- 6:42to 100 bucks on a bottle of NMN or NR, maybe stop
- 6:47at the grocery store instead. The big supplement
- 6:50players, NR and NMN, can raise NAD levels and are
- 6:54generally safe up to about 2 grams a day. There's
- 6:57no major side effects beyond the occasional flush,
- 7:00but they don't reverse aging, and they don't
- 7:05replace real medicine. Now, a meta analysis shows
- 7:09that Some of these NAD precursors, especially
- 7:11niacin, can lower LDL cholesterol by about 8 to
- 7:1412%. That's a nice lab number, but let's compare
- 7:18that to, let's say, Crestor, which is
- 7:20rosuvastatin, which is the LDL lowering
- 7:23cholesterol agent. I use 5 milligrams, lowers
- 7:26cholesterol by about LDL cholesterol by 45%. 10
- 7:31milligrams, 52%, 20 milligrams. What? I take 55%,
- 7:3540 milligrams, same. So that's not a blip. That's
- 7:40the miracle of modern medicine. And no NAD
- 7:42supplement has ever been shown to prevent heart
- 7:44attacks or strokes. Statins have again and again
- 7:49shown that they can do that. Now, that's the
- 7:51difference between some interesting biochemistry
- 7:54and some actual medicine. But if we're looking at
- 7:59this, who might actually benefit from taking some
- 8:02form of nad? Well, maybe older adults with
- 8:04metabolic syndrome or prediabetes. The. Those
- 8:07small improvements in lipids or inflammation may
- 8:09be helpful. But imagine this. My bottle of Crestor
- 8:13cost me about 75 cents for a month. That bottle of
- 8:16NAD is going to be out 100 bucks a month, and the
- 8:18Crestor is going to do a little bit more for me.
- 8:22Now, there has been a hint of potential in
- 8:24patients with early Alzheimer's or Parkinson's
- 8:27disease, but that data is tiny and for all intents
- 8:30and purposes is statistically meaningless. Now, if
- 8:34you're young and healthy, the only thing NAD will
- 8:37reliably do is reduce your bank account. Safety
- 8:40wise, there have been no major interactions that
- 8:43have been found, although there are theoretical
- 8:45issues with certain anti cancer or DNA repair
- 8:47drugs. And even if your gut microbiome helps
- 8:50process NAD precursors, another reason to feed it
- 8:53well instead of flood it with pills, it may not
- 8:55actually help you that much. So what actually
- 8:58works NAD does drop. As we age, our cells enter
- 9:02into their cellular retirement. Or as Hayflick
- 9:05described it years ago, senescence. And whether
- 9:08topping NAD keeps them working longer is still
- 9:10really unknown. But what we do know is how to
- 9:14support your body's own NAD production on a plate,
- 9:17not in a capsule. The Mediterranean diet, which is
- 9:21rich in fish, legumes, olive oil, whole grains,
- 9:23naturally fuels the NAD pathway in your body and
- 9:27protects your heart in a dozen other ways. You
- 9:30don't need a subscription. You don't need a magic
- 9:33powder. You don't need this bougie price tag. You
- 9:36just need to eat better. Now, uh, here's the
- 9:40thing. If you want to learn a lot more about
- 9:43longevity the right way, we are going to actually
- 9:46work on, we're working on it now. A longevity
- 9:48cruise in the Mediterranean in the summer of 2026,
- 9:51next summer, maybe the second week of August. Our
- 9:54plan is to have world class physician, scientist,
- 9:57a food network chef with real conversations about
- 10:00living longer and better, all while sailing in the
- 10:02Mediterranean with shore excursions that will drop
- 10:06your socks off. You're not going to be fitted with
- 10:08a weighted rucksack. You're not going to have to
- 10:10pay a six figure fee. Um, unlike Peter Attia's
- 10:13program, this one includes shore excursions, wine
- 10:15and actual fun. And for the record, Yeah, I drink
- 10:19AG1, but. But not because I have an affiliate
- 10:21code. I don't get a kickback, I don't get stock on
- 10:24it. I just like how gentle it is on my stomach,
- 10:26which is why I take it. But I'm always looking for
- 10:29a less pricey, less bougie way to get in my
- 10:31vitamins. So come on. If you're interested, serve
- 10:36that time and consider about thinking about
- 10:38healthspan under the Tuscan sun, where we found 70
- 10:42years ago, people lived longer and lived better by
- 10:45eating simpler and developing immunity. This has
- 10:52been fork you, fork University researched and
- 10:54written by me, Dr. Terry Simpson, your chief
- 10:56medical explanationist. All things audio and
- 11:00editing were done by my friends at Simpler Media
- 11:02and the pod God himself who spent X amounts of
- 11:05time with me today and my new fancy equipment. For
- 11:09references and show notes, visit4q.com or

- 11:14substack@drsimpson.com now remember, I'm a board
- 11:18certified physician, but I am not your physician.
- 11:21If you have medical concerns, please see a real
- 11:23board certified doctor, not a chiropractor, not a
- 11:26wellness influencer. And please, you don't need to
- 11:28spend $150,000. This podcast is for education and
- 11:32entertainment, not personal medical advice. And
- 11:35until next time, stay curious, stay skeptical, and
- 11:39keep your diet Mediterranean. Have a great week,
- 11:42everyone.
- 11:53All right, Evo, we're getting old enough that our
- 11:55nad levels are dropping. I'm thinking that we
- 11:58should replace them. I just don't know how. More
- 12:02sunshine. An ocean. Happy? Uh, to hear your ideas.
- 12:11>> Speaker B: Well, I may be getting older, but I'm still
- 12:14perpetually 8 years old in my brain thinking about
- 12:17how my nads are dropping as I get older.
