- 0:01Good morning, Good morning, good morning, and welcome to Ask the Doctor
- 0:04with doctor Carrie Williams with Med Club, Bartlesfail. How are we today?
- 0:08We're good? Are you well? You know I'm great because I've got a
- 0:11great doctor. By the way, transparency, Carrie Williams is my doctor direct
- 0:16primary care. Tell us about this particular model. This particular model is somewhat
- 0:21new in the United States. It's a practice model where patients make direct contact
- 0:27contracts with physicians primary care physicians to provide them services at mid Club. We
- 0:35have a monthly charge depending on your age, and what that covers is my
- 0:41time, my consultations care for chronic diseases and things like physicals, but we
- 0:54do medchecks. Extra things are extra labs, prescriptions, and we offer those
- 1:02at med Club and at discounted rate. Wh's really great. It's kind of
- 1:06like a one stop the deal there. And you're not finding yourself. You
- 1:10know it's your choice, but you're not finding yourself, you know, going
- 1:12all over town to get things done. You can get most everything you need
- 1:17done right there at Med Club. Artiles Well, yes, sir, and
- 1:19I genuinely doing them. You are now the advantage, of course of being
- 1:26a member of a med club, and you got plenty of them. But
- 1:30let's take a look at it. You Well, first of all, through
- 1:34transparency, you know how much you're going to pay every month. It's not
- 1:37changing. You're getting unexpected bills. Wait a minute, this is medical stuff
- 1:42and we're not getting unexpected bills. No, and this is usually less expensive.
- 1:47The average patient saves over twenty percent if they use versus traditional healthcare interns
- 1:56as far as they're overall cost according to the statistics, and we cut out
- 2:04the middle of the man. There's not an insurance company with their hands out.
- 2:10And the statistics show that if you are a patient of a direct primary
- 2:16care practice, you have, as a general rule, there is a sixty
- 2:21five percent less chance that you will end up in the emergency room on an
- 2:25emergency room visit. Well, that's great that those are expensive. They are.
- 2:30It's a it's not a day at Disney World and for sure, but
- 2:34it may cost as much. Oh my yes, yes, And what I
- 2:39like is the accessibility I have to the doctor. Yes, we try to
- 2:45do same day or next day appointments. Some people like to book out further,
- 2:49but we do that. And for instance, if you've had an unexpected
- 2:53hospitalization and you need to be seen quickly after you've been discharged from the hospital.
- 2:58We'll try to make room for you to be seen, and a lot
- 3:05of times, the number one advantage is is that you get explanations of what's
- 3:09happened. Because the medical field can be very subservient. You don't know what's
- 3:15going on when you're in the hospital, So having somebody to give you a
- 3:21play by play or they did this procedure for that reason, and that's what
- 3:27that's providing. That patient understanding gets gets rid of a lot of the fear,
- 3:36and fear is misery, and so if we can help a patient get
- 3:42comfortable with their medical care or what's been done and what to expect, then
- 3:46we've really done our job. One thing I like is that the appointments are
- 3:52waiting in a waiting room for an hour, can be seen for five minutes,
- 3:57that's correct. We try not to make any one way, and we
- 4:02limit the size of the practice. When I was in a traditional medical practice,
- 4:08I had over three thousand patients. At med Club, we limit our
- 4:12patients just seven hundred, so I have time to get to know you and
- 4:16most of the appointments are at least a half hour. And you know,
- 4:23some patients want that, some patients don't. But the explanation going back,
- 4:29the accessibility is a huge thing. Being accessible to a natural physician is huge.
- 4:40Well, you get more personalized care and for a lot of people that
- 4:45goes a long way because we used to kind of being shuffled around and you
- 4:49might only get to see your doctor three to four minutes and tops, yes,
- 4:54and then it's in out by sea, here's your bill. Yes,
- 4:57And it's not that way there. As a patient, I get to ask
- 5:00questions. You encourage that. Yes, I encourage my patients to bring a
- 5:04list with them to the appointment written please, because you're not going to remember
- 5:09them. That's exactly right. Hey, getting their questions answered again takes takes
- 5:20the guesswork out by the patient. You can go and read the computer look
- 5:26it up, but getting through the vocabulary is a lot of time, very
- 5:30confusing, and so many things mean the same thing and understanding, oh they
- 5:39did this, that means that, and I have to anticipate this coming at
- 5:46me. Yes, every patient is different, but the disease process is kind
- 5:53of with most acts similar. One thing I like is that day you'll actually
- 6:00bring out a physical book if needed. Absolutely, I show the patient exactly
- 6:04what is going on and why so that we understand it. And that's something
- 6:12that I've never witnessed is a patient in my entire life until being a member
- 6:17of med Club ARTICLM well providing that patient explanation and understanding, understanding how that
- 6:25doctor's thinking about you, Understanding this is where this particular organ lives, and
- 6:31showing the patient a picture and saying when you talk to the doctor, they're
- 6:36going to be talking left right and to your post here, and how that
- 6:43doctor determines what that means. Then the vocabulary is not so bad. The
- 6:50way that doctor talks to you is not that confusing, but bringing that knowledge
- 6:56level up. Also, if I send a patient to special I want them
- 7:01to understand what the specialist is going to be thinking about, yeah, and
- 7:06what to anticipate and how not give them any answers, but how to answer
- 7:13the questions that have been asked by the specialist. Therefore, the communication is
- 7:17automatically approved by tenfold yes, and my patient will be looked at as a
- 7:27cooperative patient, someone they want to have back in their office. And that
- 7:32makes doctors are no different than anyone else. If they like you, they'll
- 7:36spend more time with you. Now, the met Club bartletpill, we talked
- 7:42about that, the monthly fee. It covers the primary care and consultation.
- 7:46Now we did kind of mention labs and another thing, tell us more about
- 7:50labs and other things. You okay, we offer lab basic labs through our
- 7:58office, bloodwork or other things, and we have discounted prices in comparison to
- 8:05the national labs. We're not adding on what the insurance company would add or
- 8:11what that middleman for processing is. You know what your lab bill's going to
- 8:16be. And also with medications. I am in a buying group with other
- 8:22direct primary care practices and we get discounted hotel medicines. Oh yes, you
- 8:28do a lot of times it will cost you less to purchase it at med
- 8:33Club, will be cost you what your insurance will cover. Very good.
- 8:39This is all really pretty cool. Now when we take a look at this
- 8:43for IRS purposes, and I know some people do, I know I have
- 8:48to these days because the IRS seems to like me. What is there some
- 8:54kind of coding or something that we have to kind of go through here we
- 8:56talk about I think called a health plan and health plan. The IRIS does
- 9:01not allow individuals to dedebt the monthly costs from their taxes, and that's basically
- 9:11the only intervention that the IRIS has in this. Now when we go through
- 9:16a cost and payments from med Club, there is the monthly fee. And
- 9:20also there's a lot of great things that come along with this, because like
- 9:24you said, it's not like every time you visit it's a monthly fee.
- 9:28You're going to visit as much as you need to visit. Correct, So
- 9:33if I need some screening, I can just come in with an appointment and
- 9:37we get that taken care of it. Like say I got an acre of
- 9:41pain. A week later, I can call up and say, hey Doc,
- 9:43yes, and it's still one monthly fee. Yes, hu that here.
- 9:50We also text and email my patients and I communicate a lot. Even
- 9:56though you may not see me every month, there's a lot communications that goes
- 10:01on behind behind the scenes. Your labs are sent to you. I have
- 10:05looked at them. I'll make comments on them, and or X rays or
- 10:09other tests. Patients have questions, you know, my medicine that I started
- 10:18is doing this? Is this normal or is this not? And also there's
- 10:24access, emergency access to me after hours and on the weekends. So that's
- 10:31that cuts down on a lot of the er visits. It does, yeah,
- 10:37absolutely does we keep you well, We'll try. And I put an
- 10:41enormant amount of time into the patients figuring out where they're at, what needs
- 10:46to be done, what they've done, what they haven't done, and get
- 10:50them caught up on their screenings, get to figure out their medicines. And
- 10:54sometimes I've got patients that are on medicines that don't agree together and adjust things
- 11:03and figure out more than anything, figure out what that patient's goals are.
- 11:09You know, you said meet your patients where they are. I know a
- 11:13fellow who hadn't been to a doctor for twenty years until he visited you,
- 11:18and he raves about how fast you caught up and how fast everything came into
- 11:26play simply because there was communication and propertiestic and knowing we're a patient, what
- 11:31their goals are, how they plan on living, what their goals are.
- 11:35You know, everyone wants a healthy life, but that's not realistic and setting
- 11:41the realistic expectations. Also, it's a little known fact doctors are not reimbursed
- 11:50for reading past medical records in the office, and there are patients who come
- 11:54with hundreds of pages worth of past medical records, and that's where doctors find
- 12:01their discrepancies, but they it takes hours to go through them. And my
- 12:07theory on that if it's important enough to hit my office, this is important
- 12:09enough for me to read and understand and see what's been done. And a
- 12:15lot of the costs of a patient are cut down in that step right there,
- 12:20because patients don't remember if they've had a test, but those records will
- 12:26indicate yay or name that is correct, and usually the result yes, that
- 12:30is correct, And in understanding that and being able to document that is a
- 12:35big deal. Also with traditional healthcare, Medicare and Medicaid, there's something called
- 12:41medical necessity, which is a book nuisance. From time to time you have
- 12:46to prove as a physician why that patient needs that test, and a lot
- 12:52of times there's a lot of hoops to jump through. If I have old
- 12:56medical records that I am familiar with, some times I can find medical necessity
- 13:01in those. Now sometimes you're gonna need health care insurance with direct primary care
- 13:09sometimes tell us about them. Sometimes we sometimes are on a catastrophic events things
- 13:16that you don't anticipate, surgeries, heart attacks, cancers. These are devastating
- 13:22to patients in their financial well being because it's easy to run up tens of
- 13:30thousands of dollars worth of medical bills. Also, if you've got cancer,
- 13:37some of those drugs are absorbitedly expensive and so a high deductible health care insurance
- 13:46plan is what we recommend to go with this because in direct primary care we
- 13:50can catch most of these many of the health care needs ahead of time and
- 13:58possibly warding off or catch them early before they are a catastrophic event. Very
- 14:05good. How do we become a patient of doctor Carry Williams at bed Club
