- 0:00- This is Chris host with the Becker's Healthcare Podcast.
- 0:03I'm thrilled today to be joined by Dr.
- 0:04Mark Bergeron, medical Director
- 0:06for Digital Health transformation,
- 0:08and medical director for the Neonatal Virtual program care
- 0:11program at Children's Minnesota.
- 0:13He joins us today to discuss the health systems,
- 0:15the kids experts at home monitoring program.
- 0:18Dr. Bergeron, thank you so much
- 0:19for being part of the podcast today.
- 0:22- Hey, good morning Chris, and thanks for having me back.
- 0:25- I know you've been on our podcast before,
- 0:26but for those who may not be unfamiliar with you,
- 0:29could you please introduce yourself
- 0:30and tell us a bit about your background?
- 0:33- Yeah, absolutely. So, I, uh, I, like you said,
- 0:36I'm Mark Bergeron.
- 0:37Uh, I'm medical director
- 0:38for Digital Health Transformation at Children's Minnesota,
- 0:41but I'm also a practicing neonatologist
- 0:43and I've been at Children's, uh, Minnesota for, gosh,
- 0:46just about 19 years.
- 0:48Uh, uh, one of my duties is also as medical director
- 0:51of our virtual care programs within
- 0:53the neonatology division.
- 0:56- As I mentioned, we're here to talk about the Kid Experts
- 0:59at Home monitoring program.
- 1:00So can you tell us how it develops and what program
- 1:03or programs, if any, or uses models?
- 1:07- Yeah, absolutely.
- 1:08So just, uh, before we get started,
- 1:09just a little bit about Children's Minnesota.
- 1:11Our neonatology program, uh, is, uh, a vital resource, uh,
- 1:15to our communities and region.
- 1:18Um, and it's one of, it is the largest program in Minnesota
- 1:21with a level four NICU
- 1:23and two level three N ICUs plus, uh,
- 1:27level two special care nurseries, um, making us one
- 1:30of the biggest programs, uh, in the upper Midwest region.
- 1:34Um, the kid Expert at Home Program, uh, kind
- 1:39of came about as, you know, within our, our, uh,
- 1:43our neonatology group, a sense of curiosity about, uh,
- 1:46are there other care models that we should be exploring?
- 1:50Um, and out of that,
- 1:52and a, a real pinch for bed space
- 1:55during respiratory surge months, uh, during winter,
- 1:58we came up with this kind of early discharge program
- 2:02where our kids go home, we are, uh,
- 2:04rounding on them virtually.
- 2:06Um, and they're still receiving, uh,
- 2:08nasal gastric tube feedings administered by their parents.
- 2:11And it, it's great
- 2:12'cause it facilitates earlier discharge than what's
- 2:15traditionally expected from a neonatology unit.
- 2:19Um, and it, uh, gets these kids home where they should be.
- 2:24- Certainly this is something that would need a bit
- 2:27of training for both the
- 2:28staff involved as well as the parents.
- 2:29So what is involved in that process?
- 2:32- Well, uh, from the first day ,
- 2:37a baby is admitted to the neonatal units.
- 2:39We're teaching the parents how to take care
- 2:40of a preterm infant.
- 2:42That doesn't change, but parents have time
- 2:45and time again said that the, the slowest kind
- 2:48of most frustrating part of the neonatal journey, uh,
- 2:52to home is the last few weeks
- 2:55and days when the infant has really no medical problems,
- 3:00but just is slowly learning how to eat.
- 3:03And so we sort of have modified our, uh,
- 3:08discharge teaching for home process
- 3:10to include tube feedings at home.
- 3:12Traditionally, you would wait
- 3:14until the infant had their feeding tube removed from their
- 3:16nose and, and make sure they were gaining weight
- 3:18and then discharge 'em to home.
- 3:20We've sort of cut out that last part, uh,
- 3:23from the hospital experience
- 3:24and offered it to parents to do it at home.
- 3:27So we teach them how to care for that, uh, nasogastric tube,
- 3:31how to administer the feedings using a feeding pump.
- 3:34Uh, we teach 'em how to use a, uh,
- 3:36a monitoring application on a smartphone
- 3:39or a pad to enter their daily weights,
- 3:42their feeding volumes, um, their urine
- 3:45and stool output, all those things
- 3:47that we would be recording in the hospital
- 3:48to make medical decisions.
- 3:49And then we round virtually with the families, uh,
- 3:52to make those decisions about
- 3:55how the feedings are progressing, um,
- 3:57and how soon to remove that, uh, feeding tube,
- 4:00but doing it at home.
- 4:02And then we teach the parents what to do if it comes out,
- 4:04who to call, um, and support them that way.
- 4:09- Gotcha. So can you describe this CASA program has had in
- 4:12the Children's Minnesota community,
- 4:14and are there plans to expand the program
- 4:15beyond the three hospitals it currently serves?
- 4:18- Yeah, absolutely. So I think, you know, we could,
- 4:22it's been a resounding success.
- 4:23It's shortened. Uh, you know, in our pilot study,
- 4:26it shortened the expected hospital stay by about six days.
- 4:30Uh, we saw the feedings, uh, uh, go better at home
- 4:34as we kind of anticipated that they would,
- 4:36and full oral feedings were established quicker at home
- 4:40by several days, um, faster than
- 4:43what we were experiencing traditionally in the hospital.
- 4:45So we think those, those things are a resounding success.
- 4:48We've had very few PE babies have to come back
- 4:51for admission, uh, after discharge in the program.
- 4:54And actually, in looking at those cases, they had nothing
- 4:57to do with feedings and were other issues
- 4:59that prompted the readmission to the hospital.
- 5:01So we think it's successful, we think it's safe,
- 5:04it's effective, and it's been a huge
- 5:05satisfier for our families.
- 5:07And during the height of Covid,
- 5:09there was one patient in particular that struck us all, um,
- 5:13and really characterizes the value of this program
- 5:16that was Finn born at 32 weeks.
- 5:18And, uh, during the height
- 5:20of Covid when we were very restrictive about how many, uh,
- 5:23visitors could come in, uh, uh, we had Finn
- 5:26who was a perfect candidate for this program.
- 5:29Uh, and his father was, uh, ending his, uh, battle with, uh,
- 5:34pancreatic cancer and was not able to come
- 5:36and see Finn in the hospital.
- 5:38And we got Finn home quick on this program, uh,
- 5:42and, uh, into where he should have been
- 5:44with his, his parents.
- 5:46And then, uh, his father ultimately passed away, uh,
- 5:49not long after Finn's feeding Tube was removed,
- 5:51and he was thriving at home.
- 5:53And for, for all of us involved in his care, this was a win.
- 5:57This was just a, a tremendous impact to our families
- 6:01that we serve on, especially fins.
- 6:04- Yeah. Wow. Dr. Bergan,
- 6:06that's certainly a heartbreaking story,
- 6:08but heartwarming in the fact that the father did get
- 6:10to see his son for a little bit before he died.
- 6:12I mean, wow. Yeah. Yeah.
- 6:14Thankfully, yes, there are times when
- 6:16that is considered a victory.
- 6:18- Yeah. And, uh, so we want every family to, to have
- 6:21that experience of getting their, their infant home as soon
- 6:24as possible and, and thrive, uh, and, uh,
- 6:28and we think that this is a great way to do that.
- 6:31- Lastly, Dr. Bergon, it's a bit of a two part question.
- 6:33Sure. So, what do you think is the next evolution
- 6:36for kid experts at home?
- 6:38And what other innovations does Children's Minnesota help
- 6:40hope to launch in 2024 or the next few years?
- 6:44- So we want as many babies to be, uh, suitable for this
- 6:48or the program to be suitable for them as possible.
- 6:51And the limitations of the technology
- 6:53that we had when we launched the program was
- 6:55that it was the, the application was
- 6:57available in English only.
- 6:58So we're now expanding that to Spanish.
- 7:01And we we're working with our app vendor
- 7:03to increase the number of languages offered.
- 7:05Um, we use our own interpreter services for the, um,
- 7:08virtual visits, but we want that app to be in, in, uh,
- 7:12a usable language and format for all of our families.
- 7:15So from an equity lens, that's probably our primary, uh,
- 7:19focus, uh, this year.
- 7:21And then, uh, we want the service to be more, you know, uh,
- 7:25add value to our family's experience.
- 7:27So we are adding virtual, uh, lactation consultations in
- 7:32the kid expert at home program.
- 7:33So one of our pediatric nurse practitioners
- 7:36that works in our special care nursery will have virtual
- 7:38visits, uh, with, um, uh, moms and their babies
- 7:41after discharge to work on the lactation piece.
- 7:45Um, so that we're really sending babies home with, uh,
- 7:48successful, uh, breastfeeding, um, as a goal
- 7:51and, uh, something that they can achieve.
- 7:54And then looking into the future, we kind
- 7:56of see the kid expert at home program as, as kind
- 7:59of an early form of the hospital at home concept
- 8:03that has been so wildly successful in the adult population.
- 8:07And so, you know,
- 8:09I guess in our journey towards a hospital at home care
- 8:12model, this is our, our first baby step,
- 8:14if you will, towards that.
- 8:16Um, and really looking at other patients at Children's
- 8:18Minnesota that could be suitable for, uh, um, kind
- 8:22of a more advanced care at home.
- 8:26- Dr. Berg, thank you so much for being so generous
- 8:28with your time and insights today.
