- 0:32Hi, everyone. This is Elizabeth Caslow with the
- 0:34Becker's Payer Issues podcast here at the April
- 0:372026 Becker's Payer Issues Roundtable.
- 0:39I'm so excited today to be joined by
- 0:41Ellen Sexton, executive vice president and chief growth
- 0:43officer at Blue Shield of California. Ellen, thank
- 0:45you so much for joining us today. Thanks,
- 0:47Elizabeth, for having me. It's great to be
- 0:49here. Yeah. So I would love to hear
- 0:51a bit more about your role at Blue
- 0:52Shield of California and your health care career
- 0:54background, where you're at now.
- 0:56Absolutely.
- 0:57So I am executive vice president and chief
- 1:00growth officer for Blue Shield of California.
- 1:03We cover 6,000,000
- 1:05members,
- 1:06both inside and outside California. We have a
- 1:08million members actually outside of California,
- 1:11and we cover employer group, Medicare,
- 1:14Medicaid,
- 1:15and individual exchange, which is called Covered California.
- 1:19So we have a really diverse group of
- 1:20membership.
- 1:22And
- 1:23I have been in health care what seems
- 1:25like, a long time now and all within
- 1:28health care payers. So I started at UnitedHealthcare
- 1:31and just got to do really fabulous things
- 1:33there,
- 1:34like roll out part d, which was twenty
- 1:36some years ago now. So thinking about, you
- 1:39know, prescription drug coverage for Medicare members.
- 1:42I also was able to run a Medicaid
- 1:44plan for them and really learn about how
- 1:46do you have to deliver for complex members.
- 1:49I also work for Humana, and I ran
- 1:51their specialty and group benefits.
- 1:53And then I also spent,
- 1:55a little bit of time at a startup
- 1:57called Curative out of Austin, Texas.
- 2:00And we were trying to do really innovative
- 2:02things around employer group coverage,
- 2:04getting people in a preventative care by giving
- 2:07them a baseline visit and then zero co
- 2:09pay, zero deductible.
- 2:11And so really great to see sort of
- 2:13the nimbleness of a start up and how
- 2:15you get members actually into care. So,
- 2:18I love the challenges of health care and
- 2:20that's why I think I've just stayed within
- 2:22it for so long, and we've got more
- 2:24to go, and I'm ready to roll up
- 2:26my sleeves and do that. Well, thank you
- 2:28so much for that background, Ellen. And so
- 2:30from your point of view, what is the
- 2:32biggest
- 2:33operational challenge that payers are dealing with right
- 2:35now, and what are you doing to address
- 2:37it?
- 2:38I think from that standpoint for health plans,
- 2:41really getting that affordability
- 2:43and figuring out how do we actually bend
- 2:46that cost curve.
- 2:47And do we do it with providers, but
- 2:49also do it within sort of how do
- 2:51we run our operations more efficiently,
- 2:54whether that's through AI,
- 2:56whether that's through just better process and technology.
- 2:59And we know that health care in America
- 3:02has to change from the trajectory of health
- 3:05care costs. Employer groups can't afford it. Most
- 3:08families are just one health care crisis away
- 3:10from bankruptcy.
- 3:12And to think about that as a charge
- 3:14from a leadership standpoint
- 3:16just is really impactful.
- 3:18And so I think there's a blend of
- 3:20how we work with providers and value based
- 3:22care and bring things to market where we
- 3:24actually
- 3:25help them select the right care for members.
- 3:28We help members engage with providers that deliver
- 3:31high quality
- 3:33as well as how do we make
- 3:35the complexities
- 3:36of health care go away. And that's not
- 3:38an easy thing to do and as a
- 3:39journey we're on and one that I'm proud
- 3:41to be part of.
- 3:42So I wanna talk a little bit more
- 3:44about that cost curve. So there's a real
- 3:46tension between keeping costs down and also
- 3:50ensuring a good member. So
- 3:52that is something that we see a lot
- 3:53of with care coordination, but how is your
- 3:55organization balancing cost containment with improving that experience
- 3:59and also thinking about quality outcomes, and where
- 4:02are you seeing the biggest opportunity for impact?
- 4:05The first thing I would say is that,
- 4:07you know, member experience
- 4:09and making sure that people have access to
- 4:11care, that is table stakes and the first
- 4:13thing that we're always really focused on, and
- 4:16that can't come at the expense of anything
- 4:18else. And so thinking about how do we
- 4:20introduce digital tools that help members access care?
- 4:23How do we think about behind the scenes,
- 4:26like, prior auth tools with providers so that
- 4:29we make care more seamless? And in many
- 4:31cases, when somebody's undergoing treatment, not having to
- 4:34wait for approvals and things like that.
- 4:37And why that matters is is if you
- 4:39get people into care sooner, then their conditions
- 4:42can actually be addressed, and you don't have
- 4:45the same sort of pervasive, like, issues that
- 4:48people end up in the ER with or
- 4:50back with a readmit. And so how we
- 4:52get after that is really important.
- 4:54You know, one of the things that we've
- 4:56have is virtual blue, and that is our
- 4:59online
- 5:00virtual care. And we're in our third year
- 5:02anniversary.
- 5:03We've had a 150,000
- 5:05members access it, and the first thing it
- 5:08brings to the table is the ability to
- 5:10get into an appointment much sooner. So on
- 5:12the primary care and mental health, people are
- 5:15able to access and get into appointment in
- 5:18one day versus ten days. And on the
- 5:20specialty side, within three days versus fifteen. So
- 5:24imagine you're a patient
- 5:26and you have something ailing you. And instead
- 5:28of waiting for a number of days, you
- 5:30can get in much sooner and get whatever
- 5:32it is addressed. And so we've seen that
- 5:35not only improve the experience, but also a
- 5:37lot less ER visits because people don't have
- 5:40to go to the ER.
- 5:42So 10%
- 5:43less ER visits. And so overall,
- 5:46we're seeing a member savings of $468
- 5:49per member, which is huge. Right? So better
- 5:52experience as well as lowering costs at the
- 5:55same time.
- 5:56So my next question was about technology as
- 5:58an AI that you're prioritizing right now,
- 6:03Virtual Blue and what's to come there. So
- 6:06what are you prioritizing within Virtual Blue? What's
- 6:08the next step now that you've had that
- 6:10three year landmark? Yeah. And where are you
- 6:12seeing meaningful ROI and better engagement with members?
- 6:16Definitely. So, again, it's about helping us as
- 6:19members access care, and I think we're all
- 6:22frustrated too. There's lots of experiences we have
- 6:25outside health care that are way more seamless
- 6:28that we're now comparing to, you know, whether
- 6:30it's through an app, whether it's how we,
- 6:32you know, talk to,
- 6:34you know, our doctor. There's a ton of
- 6:36things that people are just waiting for that
- 6:39we can bring to the table. And so
- 6:41continuing to roll out Virtual Blue is a
- 6:44big focus point. Also, online appointments in in
- 6:47just in general. And so we work with
- 6:49Zocdoc to put online appointment capabilities. So just
- 6:53like when you reserve an airline ticket or
- 6:56a hotel, you don't have to make a
- 6:58call. Right? You're able to do it through
- 6:59an app. Same thing for your doctor and
- 7:01just makes it much easier with all of
- 7:03our busy schedules.
- 7:05The other thing that we're rolling out is,
- 7:08check
- 7:09Rx script, and what that allows you to
- 7:11do is when you're in the doctor's office,
- 7:14get the ability to understand,
- 7:17could you have gotten that prescription through a
- 7:19generic or through biosimilar,
- 7:21and have a lot less,
- 7:23cost to your bottom line? And what's important
- 7:26about that is it's not when you're at
- 7:27the pharmacy and can't make a decision. You
- 7:30can talk through with your doctor, like, how
- 7:32could this impact me? And so for instance,
- 7:34maybe you have something,
- 7:36that has a generic, but it could make
- 7:38you drowsy. Your doc can talk that through
- 7:40with you and can say, well, yes. For
- 7:42me, the ability to, like, have to pay
- 7:44less, and I'm okay with maybe that as
- 7:46a side effect, or maybe you're not. But
- 7:48it puts you more in that decision phase
- 7:50and helps you understand what you can do.
- 7:53So all these things are are well underway,
- 7:56and we've just seen great impact. And our
- 7:58members really love it, and we're gonna continue
- 8:00to look for things like this to disrupt
- 8:02the status quo.
- 8:04It's really interesting that you're using new technology
- 8:06and those types of innovations
- 8:08for building agency with the members. Mhmm.
- 8:12But just more broadly speaking, how do you
- 8:14see AI supporting member facing interactions while still
- 8:17maintaining trust and quality, and what advice do
- 8:19you have for other carriers that are looking
- 8:21for advice in that? Yeah. I don't think
- 8:23I go through a day where I don't
- 8:25talk about AI,
- 8:26either internally with teams,
- 8:28with providers,
- 8:30or even with other health plans on how
- 8:32they're using it because it's certainly gonna, like,
- 8:34I think, improve a lot of things. It's
- 8:36gonna bring data to the table,
- 8:38and help with decision points that we never
- 8:41had before and make it much more real
- 8:43time as well. Instead of waiting for claims
- 8:45or waiting for a a phone call,
- 8:47we can use AI.
- 8:48With all of that, though, you're right. You
- 8:51have to build the trust and the guardrails
- 8:53around it to make sure that it's gonna
- 8:55help improve the experience
- 8:58as well as make sure sure that it's
- 8:59gonna be accurate and help really that member
- 9:02and provider. And so those are the layers
- 9:04that we're instituting
- 9:06over it and within it to make sure
- 9:08that we can get sure make sure that
- 9:10that's, the right way to handle it. And
- 9:13I think as we think about it, it's
- 9:15just gonna grow more and more, and it
- 9:16is gonna be able to cut out a
- 9:18lot of costs, which then means we can
- 9:21invest that in the benefits
- 9:23and the things that really improve health care
- 9:25overall.
- 9:26Well, Ellen, any last thoughts as we wrap
- 9:28up our conversation today?
- 9:30It's been just so delightful to talk with
- 9:33you. I'm really excited about this entire conference
- 9:36because I think, again, it brings ideas to
- 9:38the table, and no one organization is gonna
- 9:41be able to solve this alone. So how
- 9:43we work together and solve health care challenges
- 9:46is what these kinds of venues help us
- 9:48do. That's great to hear. We're so happy
- 9:51to have you here with us at the
- 9:52event.
- 9:53And to our listeners, if you wanna listen
- 9:55to more podcasts from Becker's, you can go
