Transcript
- 0:00SPEAKER_00: You lost the weight and now your face looks 10 years older than it did before you started.
- 0:04SPEAKER_00: People call this Ozempic face.
- 0:07SPEAKER_00: When weight comes off fast, two different changes happen to a face and they need opposite treatments.
- 0:12SPEAKER_00: Treat the wrong problem, and you can spend a lot of money without actually fixing it.
- 0:16SPEAKER_00: Hi, I'm Dr.
- 0:17SPEAKER_00: Dan Gould.
- 0:17SPEAKER_00: I'm a board certified plastic surgeon in Beverly Hills with a PhD in bioengineering, which gives me a very different way of looking at how the face changes over time.
- 0:25SPEAKER_00: I focus on regenerative techniques and have treated thousands of faces over the last 10 years.
- 0:29SPEAKER_00: I prescribe semaglutide inside a structured weight loss program, so I'm not here to talk you off these drugs.
- 0:34SPEAKER_00: I'm actually here because I rebuild the faces of people who take them.
- 0:37SPEAKER_00: By the end, you'll know which of these two changes you're looking at and what actually helps.
- 0:41SPEAKER_00: And the quick little test that you can do at home today to find out before you book anything.
- 0:45SPEAKER_00: Start with what actually happened to your face when the weight came off.
- 0:48SPEAKER_00: The fat on your face isn't one sheet.
- 0:50SPEAKER_00: It sits in separate pockets.
- 0:51SPEAKER_00: There's a pad under your eye, a pad on your cheek, one along the temple, one down by the jaw.
- 0:55SPEAKER_00: When you were younger, those pockets were full and they sat right up against each other, so the surface of your face read as one smooth curve.
- 1:02SPEAKER_00: But when you lose weight, your body doesn't ask you which pocket it's allowed to take from.
- 1:06SPEAKER_00: It takes from all of them.
- 1:07SPEAKER_00: And it usually takes from the face early.
- 1:08SPEAKER_00: That's true whether the weight came off with a GLP1, with surgery, or with two years of hard work in the gym.
- 1:14SPEAKER_00: What's different about the medication is the speed.
- 1:16SPEAKER_00: Weight that used to take years to come off now comes off in months.
- 1:19SPEAKER_00: And your skin and the deeper tissue underneath it don't get the same amount of time to catch up.
- 1:24SPEAKER_00: So the face pockets empty out, but they also move.
- 1:26SPEAKER_00: Your face is held up by ligaments, small, tough, and they anchored the deep tissue of your cheek to the bone underneath it.
- 1:32SPEAKER_00: Over the years they stretch.
- 1:33SPEAKER_00: When they stretch, the tissue they were holding slides kind of downward and it collects along the jaw and the fold beside your mouth.
- 1:39SPEAKER_00: That's happening to everybody in their 40s and 50s, weight loss or not.
- 1:43SPEAKER_00: What the weight was doing was hiding it.
- 1:44SPEAKER_00: A full cheek sits on top of a sagging one and covers it up.
- 1:47SPEAKER_00: If you take the fullness away that quickly and you're seeing for the first time where everything actually is.
- 1:52SPEAKER_00: So when you look in the mirror after losing a lot of weight and you think that you've aged a decade, some of what you're seeing is those emptier pockets, and some of it is aging that was already there and just became uncovered.
- 2:02SPEAKER_00: Those two need opposite treatments.
- 2:03SPEAKER_00: Filler adds, what it can't do is moving something back up to where it started because it's not a lifting material, it's a filling one.
- 2:10SPEAKER_00: If you put it into a cheek that's dropped, you haven't lifted the cheek, you're just gonna make that drop cheek bigger.
- 2:14SPEAKER_00: That's the overfilled look that women are telling me they're most afraid of.
- 2:18SPEAKER_00: So which one do you have?
- 2:19SPEAKER_00: You can figure out at home in your bathroom.
- 2:21SPEAKER_00: And it's close to what I do when somebody sits down in front of me.
- 2:23SPEAKER_00: Find a photo of yourself from before the weight came off and hold it up next to your face in the mirror.
- 2:27SPEAKER_00: Ignore how full you look.
- 2:29SPEAKER_00: Look at where things sit.
- 2:30SPEAKER_00: Start at the corner of your mouth in the old photo.
- 2:32SPEAKER_00: Follow the line from there down to your jaw.
- 2:34SPEAKER_00: Is that stretch still smooth or is there a small pad of tissue sitting next to the jawline now that wasn't sitting there before?
- 2:40SPEAKER_00: When you look at your cheek against your eye, is the highest part of the cheek still near the outer corner of your eye, or is the fullest part now sitting down lower and closer to your nose?
- 2:48SPEAKER_00: If the shapes are all in the same places and they just look emptier and flatter, then that's volume and that's the one you can add to.
- 2:54SPEAKER_00: But if the same tissue is lower on your face than it used to be, that's position.
- 2:57SPEAKER_00: And nothing you inject changes position.
- 2:59SPEAKER_00: The jawline is where this shows up first and where you'll see it most clearly.
- 3:03SPEAKER_00: It's the thing I look at before anything else, because a clean jawline usually means everything above it is still sitting where it belongs.
- 3:08SPEAKER_00: If you want somebody to look at your face and tell you which of the two you're dealing with, that's what a consultation is for.
- 3:13SPEAKER_00: My practice is in Beverly Hills.
- 3:14SPEAKER_00: There's a link in the description to reach out to us.
- 3:16SPEAKER_00: Now back to it.
- 3:17SPEAKER_00: Say it's volume.
- 3:18SPEAKER_00: Then adding is the right answer and filler can work.
- 3:21SPEAKER_00: So does fat crafting, which is your own fat move back into your face.
- 3:24SPEAKER_00: And for someone who's lost a lot of weight, it usually holds a better shape than filler does.
- 3:27SPEAKER_00: The rule I work by is to restore what's actually gone, where it's actually gone, and to stop there, not to refill the whole face until it's full again.
- 3:34SPEAKER_00: If you refill a face that has also dropped and you end up with proportions that don't belong to any age, you'll see that it'll look full in places a young face was never full and still heavy along that jaw.
- 3:44SPEAKER_00: And how much you need depends on where you started.
- 3:46SPEAKER_00: Somebody who's very thin now needs real volume put back, and that could be two or three sessions, not one appointment.
- 3:52SPEAKER_00: Somebody who came into the weight loss heavier has the opposite problem.
- 3:55SPEAKER_00: There's still soft tissue there.
- 3:56SPEAKER_00: So the work is mostly about removing that soft tissue rather than adding to it.
- 4:00SPEAKER_00: Same medication, same amount of weight on two completely different plans can be the case.
- 4:04SPEAKER_00: That's why I won't tell you on the internet what your face needs.
- 4:06SPEAKER_00: Say it's position.
- 4:07SPEAKER_00: Then somebody has to put the tissue back where it was, and that's surgery.
- 4:11SPEAKER_00: That's what I do.
- 4:12SPEAKER_00: I open, I look at where things were, and I put them back.
- 4:14SPEAKER_00: I'm not trying to make anybody look like a different person, and I don't move the target past where it started.
- 4:19SPEAKER_00: Whether that's the right step for you is a separate conversation and plenty of people aren't ready for it, but nothing injected will do that job.
- 4:25SPEAKER_00: And there's one more thing that's specific to the medications that has nothing to do with your face directly.
- 4:30SPEAKER_00: Losing weight quickly on a GLP1 usually means losing a lot of muscle with that fat.
- 4:34SPEAKER_00: And it means eating a lot less protein because you're not hungry.
- 4:36SPEAKER_00: That shows up in the quality of the tissue and it shows up in how you heal.
- 4:39SPEAKER_00: If you're on one of these and you're talking to anybody about your face, you should definitely mention it to them because it changes what I would recommend and it changes when I'd recommend it.
- 4:47SPEAKER_00: So before you book anything, go find the photo.
- 4:49SPEAKER_00: Decide if it's emptying or movement or both.
- 4:52SPEAKER_00: If it's volume, you can add, and you should do this carefully.
- 4:54SPEAKER_00: If it's position, adding won't get you there.
- 4:56SPEAKER_00: And knowing that today is worth two years and a lot of money.
- 4:59SPEAKER_00: The other thing people ask me is about the neck, because when the weight comes off, the neck usually goes before the face does.
- 5:04SPEAKER_00: I made a whole video on this and what actually tightens a loose neck and what doesn't, and that's the next one that I would watch.
- 5:10SPEAKER_00: You didn't lose your face.
- 5:11SPEAKER_00: Some of it emptied and some of it moved, and those two get fixed in completely different ways.
- 5:15SPEAKER_00: Go find the photo and hit subscribe on your way out.
