- 0:00This is Laura Diet with Becker healthcare care
- 0:02podcast. I'm thrilled today to be joined by
- 0:04Lindsey Horn, manager of nurse residences at Nova
- 0:07Health. Lindsay is a pleasure to have you
- 0:08on the podcast today.
- 0:10Thanks for having me. No. I'm really looking
- 0:12forward to our discussion and digging a bit
- 0:15deeper into what you're doing at Nova in
- 0:17terms of building out in our pipeline and,
- 0:19really training nurses of the future. I know
- 0:21such an important aspect of health care today,
- 0:24And really a lot of organizations trying to
- 0:27make sure they have a strong pipeline going
- 0:28forward. So I'm I'm excited to dive in.
- 0:31But before we do, I was wondering could
- 0:33you tell us a little bit more about
- 0:34yourself and your background? Or Sure. Be happy
- 0:37to. So I am Lindsay Horn and and
- 0:40the manager of the residency
- 0:42program here at Nova Health and Never health
- 0:45is a large Multi
- 0:47organization, spans both North and South Carolina,
- 0:50and we employ Christ 40000
- 0:53team members. And so I'm really proud to
- 0:56say that I've spent my entire career at
- 0:58Nova Health and various nursing roles
- 1:02from, you know, labor delivery, bedside nurse to
- 1:06nurse educator and and, you know, most currently,
- 1:09my current role, but the manager of the
- 1:12nurse residency program, which I think has been
- 1:14my favorite thus far. And so throughout my
- 1:18nursing journey, I have spent many many hours,
- 1:22preset and onboarding, new nurses, see the profession
- 1:26and and really no matter what role I
- 1:28was in. It just has have has been
- 1:30my passion. And so
- 1:32it it all boils down to my clinical
- 1:35experiences that I had a student at Nova
- 1:38Health. It really was the driving factor on
- 1:41where I wanted to work as a nurse.
- 1:43And so
- 1:44it it has been really great for me
- 1:47thus far. But I will say that transition
- 1:50into nursing practice, my very first year in
- 1:52the profession was probably the the hardest of
- 1:55my my life. And and... But, thankfully, I
- 1:58had such a great experience at Nova Hill.
- 2:01That it really helped me settle into the
- 2:03clinical setting and really just,
- 2:06you know,
- 2:07reiterated that, you know, having that... Supportive and
- 2:10welcoming environment is really key for new clinicians
- 2:13as they're their they're learning kind of the
- 2:15complexities of health care. And so I I
- 2:19will say in 20 16, I was presented
- 2:21the opportunity to build and
- 2:24begin, from the ground up, a a nurse
- 2:27residency program for Nova Health, and it has
- 2:30been some of my best work, I think
- 2:33today and and work that I'm so proud
- 2:35of as we we now recruit nurse residents
- 2:39or brand new nurses from all over the
- 2:41country. We have an accreditation with distinction that
- 2:45we have maintained, and and we're really proud
- 2:48of that
- 2:49and we we have a team that just
- 2:51find so much joy in making this a
- 2:53great experience for people that are joining our
- 2:56organization. And so... Next month, I will actually
- 2:59graduate our twentieth cohort
- 3:01of brand new nurses at Nova Health, and
- 3:04so that's that equals out to about 4300
- 3:07nurses that I've had the opportunity to work
- 3:10with since 20 16. So it has been...
- 3:14An absolute fun journey, and I've learned a
- 3:16lot around you know, along the way, and
- 3:19and I'm excited to hear a little bit
- 3:22about, kinda half how our program works.
- 3:26Well, that's amazing here in really impressive 4300
- 3:29nurses,
- 3:30don't you know, coming through your program and
- 3:32being able to,
- 3:33touch their career journeys and set them off
- 3:35for success. I can imagine that's a huge
- 3:38undertaking, but also something,
- 3:40should be really excited and proud of.
- 3:43Yes. It is. It's it's... It has been
- 3:45really on work.
- 3:47Excellent. Well, I'm wondering first and foremost, what
- 3:50makes the nurse residency
- 3:52program at Health unique. What really do you
- 3:54do that sets you apart?
- 3:58Well, I think we have you know, several
- 4:00pieces that that are unique about our residency
- 4:03program. I think first foremost we're a homegrown
- 4:06program.
- 4:06You know, most large multi site
- 4:09organizations really do, you know, have to rely
- 4:12a lot of vendors and things like that
- 4:14to operationalize these just very large,
- 4:18I programs, but we have... We've really built
- 4:20ours from the ground up with a special
- 4:23emphasis on what we we know and we
- 4:25want for our organization, which is extremely competent
- 4:30and competent new nurses. And so
- 4:32some of the unique pieces
- 4:35that that I think we offer is we
- 4:37are a 12 month program. You were gonna
- 4:38see a lot across the country about our
- 4:416 months some that are 3 months. But
- 4:43we really feel strongly, and that 12 months
- 4:46is is is what new nurses need, to
- 4:50feel confident
- 4:51at the bedside. And so it is a
- 4:53massive investment for our organization. It can it
- 4:56can can be expensive
- 4:59but it really has been a
- 5:02a win win for not only the nurses
- 5:04joining our team, but the unit,
- 5:07that are looking to fill vacancies on their
- 5:09floor. So some other pieces that I think
- 5:12make us unique and have really contributed to
- 5:14our success are that we are a centralized
- 5:18residency program.
- 5:20And with a centralized
- 5:21that model. So we do not hire new
- 5:24nurses directly to any of our acute care
- 5:26units. They come into the residency program,
- 5:29and we oversee their experience for that very
- 5:32first year. And so this allows the residency
- 5:35leaders to work very closely with not only
- 5:38the residents but also the unit to make
- 5:41sure that this onboarding experience is smooth. We
- 5:44make sure that they are on the right
- 5:47units and that they are set up for
- 5:49success, with their predecessor,
- 5:51their
- 5:52education,
- 5:53etcetera. And so having a team that is
- 5:55dedicated,
- 5:56and do that first year, for new nurses
- 6:00has been key in not only the... How
- 6:03the residents are feeling about it, but also
- 6:06in our overall retention. We're able to quickly
- 6:08move residents out of areas that maybe aren't
- 6:11the right fit and and try other areas
- 6:13within the organization that might be
- 6:17just, you know, better suited for that particular
- 6:19resident and their skill level. And so that,
- 6:22we we have really
- 6:25you know, been proud of as part of
- 6:26our program.
- 6:27I think the the next 1 that comes
- 6:29to mind is that we allow residents to
- 6:32rotate through different units,
- 6:34within,
- 6:35our system. So that... We don't have them
- 6:39when they are hired on,
- 6:41committed to 1 specific unit at 1 specific
- 6:44facility.
- 6:45If they are a critical care resident, they
- 6:47might get to try a medical. I you
- 6:50and a neuro Icu and a cardiac Icu.
- 6:53So we really give them a lot to
- 6:55the additional experiences that first year,
- 6:58to help them see really what kind of
- 7:00nurse they're meant to be. It also helps
- 7:03our unit leaders see which new grads or
- 7:05the right fit for their unit and their
- 7:07team. So from a culture standpoint,
- 7:10this has has been
- 7:12a huge,
- 7:14success of our program because we are able
- 7:17to get residents in in the right homes
- 7:19where they will hopefully be Novo nurses for
- 7:21many years to come. So those are probably,
- 7:24I think the big ones that we get
- 7:25a lot of comments about from applicants and
- 7:28from unit leaders is
- 7:31the ability to rotate and having that centralized
- 7:34leadership model
- 7:35have been
- 7:37have been big keys to our success.
- 7:41Oh, that's great to hear. You know? And
- 7:42I really appreciate you talking about that
- 7:4612 month program you have at Nova, and
- 7:48it seems really like a fascinating opportunity to
- 7:50have the ability rotating through different departments and
- 7:53and finding the right units
- 7:55for for each different
- 7:57training nurse resident, a or from your perspective,
- 7:59when you look at building something like this
- 8:01and, bringing in
- 8:03talented people to be part of the residency
- 8:05program,
- 8:06are they typically pretty ex... Excited about it
- 8:09or or, you know, nervous about this being
- 8:11different from what most programs are like, How
- 8:13do you really sell that to the nurses
- 8:15coming in? And then, like you said, it's
- 8:17a big investment from the health system side.
- 8:19So really showing, you know, the rest of
- 8:22your, the leadership team, what difference it makes
- 8:24to be able to of this type of
- 8:27program even though it is a bigger investment.
- 8:30Yeah. So you know, thankfully, the the program...
- 8:33The way that we market it, it it's
- 8:35it's pretty easy to market. You know, what
- 8:37we know new graduates, what they're looking for,
- 8:39it's lots of support. While they are very
- 8:41excited about their brand new nursing role.
- 8:44They also are very nervous.
- 8:47Social media, you know,
- 8:49in today's time, doesn't always paint nursing
- 8:52units as the most supportive places to
- 8:55to begin. And so we we have our
- 8:57work cut out for us there. And I
- 8:59think that's 1 of the areas that we
- 9:01do well on the front end or pre
- 9:03hire if we have many touch points with
- 9:05them there to reassure that they are in
- 9:07good hands. Here's all the things that we
- 9:09have planned for you.
- 9:11We we do a lot of of just
- 9:13touch points
- 9:15before they even began to kinda hopefully settle
- 9:18some of those nerves that they might have
- 9:20about starting their new role and they get
- 9:22to meet the... Residency leaders very early on
- 9:25in the process. So they're very used to
- 9:28us
- 9:29before they even begin on the nursing unit.
- 9:31And so our managers, I think,
- 9:34you know, they they see their excitement when
- 9:37they come to the floor.
- 9:38They also know the caliber of candidates that
- 9:41a residency program,
- 9:42recruits. And so I think that excites them
- 9:45to see people that are excited about nursing.
- 9:48And that are,
- 9:49you know, being followed very closely by the
- 9:52nurse residency team. So there's a partnership that
- 9:55we've formed
- 9:56with the unit leaders that we're gonna work
- 9:58together to make this a good experience and
- 10:00to make sure that you're getting nurses
- 10:03that are good fit for your units.
- 10:05And we're also gonna be helping the residents,
- 10:07making sure that they're landing in areas that
- 10:10they're really happy about and feel good about.
- 10:13And so with all of the options of
- 10:15our program and the flexibility to rotate units,
- 10:18we are seeing our new hires or come
- 10:22in not quite as anxious,
- 10:25as as we did before the program because
- 10:27they know that they're sent they're gonna have
- 10:30a full 12 months to figure it out
- 10:32and figure out what they like and really
- 10:34where they're meant to be and what type
- 10:36of nursing is really right for them.
- 10:39That's great to hear. Now, how do you
- 10:41measure success for the program?
- 10:44Yeah. So that's something we're constantly looking at,
- 10:47you know, with... I know I'm in it
- 10:48prior, but residency programs are a massive investment
- 10:51for healthcare organizations and quite expensive. And so
- 10:55We have lots of metrics that we we
- 10:57keep close eye on
- 10:58to make sure that we are seeing the
- 11:01success that that we're looking for. And so
- 11:03come some of the... The key metrics that
- 11:06that we we wanna see is is participant
- 11:09feedback. So we... Every graduating cohort, we... We
- 11:13receive feedback from candidates that or residents that
- 11:16have been in the program.
- 11:18We want to know what was their overall
- 11:20satisfaction of the program. We have scores there
- 11:23we look closely at. What we also look
- 11:25at, is this a program they would recommend
- 11:27to others.
- 11:29With cohorts as large as ours, it's the
- 11:31best recruitment tool to... Have residency graduates that
- 11:35wanna go out and tell their friend, hey,
- 11:37this is where you wanna be as a
- 11:38new nurse. And so we have found that
- 11:40if we watch those numbers very closely. We
- 11:43have a really good idea on on what,
- 11:45you know, what they're saying about their experience.
- 11:48And so that's that's 1 thing that we
- 11:50keep a close By. We also are looking
- 11:53at our nurse leader
- 11:55feedback. From senior leaders, to nursing directors, By
- 11:59as well as our unit leaders? We wanna
- 12:01know what is their overall satisfaction
- 12:03with the program? Are they filling vacancies through
- 12:06this pipeline, are they getting the caliber of
- 12:08candidates
- 12:09that they're looking for. And overall, do they
- 12:11feel like it's a good investment for Nova
- 12:13help. So I do think these are the
- 12:16key the key ones that we want to
- 12:18keep our eyes on to make sure that
- 12:21they are
- 12:22extremely positive,
- 12:24because we know that if we have
- 12:26you know, residents that or feeling good about
- 12:29their overall experience and willing to recommend it
- 12:31to others, and we have unit leaders that
- 12:34feel like this is a good investment and
- 12:35our and are filling vacancies with this pipeline
- 12:38that we are moving in the right direction.
- 12:41So
- 12:42you know, in addition to those things,
- 12:45we do like to look at retention data.
- 12:47That's a big big metric as well, and
- 12:49then, our overall recruitment number. So I would
- 12:51say,
- 12:52those those are the big ones that that
- 12:54every year are on my radar and that
- 12:57I I get to report out to our
- 12:58executive team. And in senior leaders. But but,
- 13:01yeah. So thankfully, all of that has been
- 13:04going very well for us and and and
- 13:06and really gives us a good glimpse on
- 13:08the health of our program
- 13:09that's amazing to hear. And I think really
- 13:11smart as you said, to be able to
- 13:12evaluate not only the nurse residents going through
- 13:15the program, but leaders who are working with
- 13:17those residents in understanding, you know, how they're
- 13:20viewing new residents coming through in each given
- 13:23class. So that's really cool to hear. And
- 13:25definitely
- 13:26seems like a smart way to also share
- 13:29the success with the broader leadership team. Now
- 13:32before we wrap up here, I'm wondering, how
- 13:33are you thinking about growth and
- 13:36experience going forward.
- 13:38Sure. So that that definitely is is on
- 13:40our radar. I will tell you 20 23
- 13:42and 20 24 have been our biggest recruitment
- 13:46years to date,
- 13:47which is excellent. Right? We we have a
- 13:49a very strong pipeline of new graduate nurses.
- 13:53We we are hoping that our retention efforts
- 13:56sick and that we maybe can get those
- 13:58numbers down a little bit for the network
- 14:01next year. So while we wanna, you know,
- 14:03certainly grow the program in, you know, programmatic
- 14:06pieces, we we would like to maybe not
- 14:09not need the same Nurse numbers that that
- 14:11we've needed in 23 and 20 24 and
- 14:14really be able to shift that focus
- 14:16to the recruitment of experienced nurses, because we
- 14:19know the value that they bring at the
- 14:21unit level. And so
- 14:23we have, you know, just so many coming
- 14:25on onboard, this year that we hope that
- 14:29with the residency program and all the components
- 14:31we have in place that that retaining, you
- 14:34know, all of those... Or at least most
- 14:36of those numbers will get us to a
- 14:38great place from a contract labor standpoint
- 14:40and a unit vacancy standpoint that we will
- 14:44you know, not need the same numbers, and
- 14:46that just makes our program more competitive
- 14:48and and allows us to to have, you
- 14:52know, a little bit more stronger focused on
- 14:54the ones currently in the program. As far
- 14:56as member experience, 1 of the things that
- 14:59we're focused on this year is our pre
- 15:01preset.
- 15:02So anytime, you know, we have, you know,
- 15:04large volumes of of new nurses coming in.
- 15:07It is it is pretty taxing from the
- 15:09pre preset standpoint
- 15:11So we are, you know, really working hard
- 15:13and keeping a close eye that we're not
- 15:15burning out. Our our veteran nurses on the
- 15:17unit. We wanna make sure our onboarding experiences.
- 15:20Positive ones. And so 1 of the new
- 15:23things that we've done this year is create
- 15:25what we call a lead pre role on
- 15:27the unit.
- 15:28For And this is a seasoned nurse that
- 15:31has been a pre sector on this unit
- 15:34for, you know, an extended period of time.
- 15:37And if able to not only support the
- 15:39unit leaders and how we plan out
- 15:43orientation, but also working with maybe those newer
- 15:46pre sectors on the floor that are new
- 15:48to having an orient and helping supporting them
- 15:51in their growth as they're learning how to
- 15:54orient someone to the floor. So we... We
- 15:56are already seeing a lot of really
- 15:59good good outcomes from this change. And so
- 16:03I think that you know, continuing to focus
- 16:05on that that pre preset role on the
- 16:08unit is gonna be key in in member
- 16:11experience, not only, you for the residents coming
- 16:14through the program, but any new hire that
- 16:16they... When they arrive to the floor that
- 16:18they're feeling supported, and they have
- 16:20that are excited about the role and
- 16:23are ready to teach and show them the
- 16:25wrote at the bedside.
- 16:30That's amazing here. And you know, definitely is
- 16:32exciting to see the opportunities ahead. Lindsay, thank
- 16:35you so much for joining us on the
- 16:36podcast today. This has been really informative and
- 16:39exciting to hear about your residency program, and
- 16:41I look forward to connecting with you again
- 16:42soon.
- 16:44Alright. Thanks for having me.
