- 0:30- This is Andrew Cash with the Becker's Healthcare
- 0:32- Podcast. I'm thrilled to be joined today by
- 0:34- Amanda De Los Reyes,
- 0:35- vice president of revenue cycle at Renown Health.
- 0:38- Amanda, it's a pleasure to have you on
- 0:39- the podcast. Thank you for having me.
- 0:42- Before we begin,
- 0:44- can you tell us a little bit about
- 0:45- your background?
- 0:46- Absolutely. So I've been in health care for
- 0:49- the past twenty four years. My career began
- 0:51- at Legacy Health where I spent twenty one
- 0:54- years.
- 0:55- I started out in hospital operations and moved
- 0:58- to finance and then finally revenue cycle.
- 1:02- After that, I took a role at Valleywise
- 1:04- in Phoenix, Arizona as the vice president of
- 1:07- revenue cycle,
- 1:09- and I just joined Renown as the vice
- 1:11- president of revenue cycle in January of this
- 1:14- year.
- 1:15- Yeah. And so you're still fairly new to
- 1:18- your role at Renown. And so how are
- 1:20- the first few months of your job going?
- 1:21- And as a leader, what do you think
- 1:23- are the keys to settling into a new
- 1:25- organization?
- 1:27- Well, it's going fantastic.
- 1:29- I've received a great warm welcome from everyone
- 1:32- at Renown, so that's been great. Of course,
- 1:34- it's been extremely busy starting to learn,
- 1:37- to, you know, a new organization and that
- 1:40- type of thing.
- 1:41- So just kind of getting all of the
- 1:43- knowledge has been,
- 1:45- has been a little bit overwhelming, but has
- 1:47- also been great.
- 1:49- As far as kind of how to settle
- 1:50- into a new role, my philosophy really is
- 1:53- to kind of understand the organization and the
- 1:55- team and how things are currently operating before
- 1:58- just coming in and making, you know, massive
- 2:00- changes or anything like that,
- 2:02- and really gaining the team's perspective and buy
- 2:05- in on any changes that will be made,
- 2:07- but not just to come in and start
- 2:09- making changes, of course, unless there's a compelling
- 2:12- reason to do so, which that can happen
- 2:14- from time to time.
- 2:15- But really trying to build the credibility with
- 2:18- the team and get to know them. And,
- 2:20- of course, you know, revenue cycle are big
- 2:23- on data and analytics and KPIs, so really
- 2:26- understanding,
- 2:27- the current kind of situation of where the
- 2:30- KPIs are, what's working really well, and where
- 2:33- there could be some potential opportunities.
- 2:36- Yeah.
- 2:37- And so what would you say are three
- 2:39- of the top trends that you're following in
- 2:41- health care today?
- 2:42- Oh, okay. So I would say AI and
- 2:45- automation. I think there's a lot of opportunity
- 2:48- there.
- 2:49- With, you know, with caution, of course.
- 2:51- But there's some really exciting things that are
- 2:54- already in place and on the horizon.
- 2:56- One of the other issues is the increasing
- 2:59- number of denials and really the tension between
- 3:02- the payers and providers that seems like it's
- 3:04- at an all time high. And, you know,
- 3:06- as soon as we're able to work through
- 3:09- certain denials, another one pops up and it's
- 3:11- just like plain whack a mole. So that's
- 3:13- definitely something,
- 3:15- that I'm following.
- 3:16- And then,
- 3:17- HR one and how that will impact different
- 3:19- health systems. You know, it has different,
- 3:22- kind of outcomes based on the type of
- 3:24- health system where you're located geographically,
- 3:26- if you are, you know, in a Medicaid
- 3:28- expansion state,
- 3:30- the type of state directed payments that you
- 3:32- have. And so that's something that I'm watching
- 3:34- very closely over the next six months and
- 3:36- really trying to make sure that my team's
- 3:38- prepared for some of the things that will
- 3:40- directly impact us, you know, right off the
- 3:43- bat such as,
- 3:44- the Medicaid redeterminations
- 3:46- being every six months and some of the
- 3:48- things that we can put into place to
- 3:49- help with our patients, you know, to make
- 3:52- sure that they can continue to have coverage
- 3:54- so that they're not concerned about, you know,
- 3:56- receiving a large bill and those types of
- 3:58- things.
- 4:00- Mhmm.
- 4:00- And so what would you say is the
- 4:02- biggest revenue cycle headwind you're facing right now
- 4:05- and how are you, addressing that challenge?
- 4:07- Yeah. I it's kind of a little bit
- 4:09- of a play on the things that I'm
- 4:10- following, which I guess makes perfect sense.
- 4:13- So revenue cycle specifically,
- 4:16- the escalating friction and the more opaque denials
- 4:19- driven by payer automation,
- 4:21- you know, we'll we'll receive denial sometimes that
- 4:24- are obvious that there is some type of
- 4:26- automation because it's the wrong denial type.
- 4:29- But, you know, it seems that the pay
- 4:31- you know, most of the time, the payers
- 4:33- have more money to invest and spend on
- 4:35- some of the automation.
- 4:37- And from my perspective, I think some of
- 4:40- the operational folks don't really understand,
- 4:43- that we work with directly at the payers.
- 4:46- Some of the service reps don't understand even
- 4:49- some of the automation that their plan is
- 4:52- using, and so it can be really difficult.
- 4:55- And it can be really difficult to be
- 4:57- able to talk through those items
- 4:59- and work through how to overcome those things,
- 5:02- especially when they're not correct. And then sometimes
- 5:05- it's hard to determine what even the reason
- 5:08- is for the denial and the lack of
- 5:10- information that the payers are providing about those
- 5:12- denials.
- 5:13- I think the second thing I've get again,
- 5:15- and this is more global for hospitals in
- 5:18- general is with HR one and, you know,
- 5:20- kinda going back to the redeterminations
- 5:22- and things like that. But there's a lot
- 5:24- of other things that are on the horizon
- 5:26- from a regulatory
- 5:28- standpoint
- 5:29- with looking at hospital outpatient departments and the
- 5:32- potential sunsetting some of those programs. And so
- 5:35- I think it's just really important to keep
- 5:37- a a close pulse on those types of
- 5:40- things right now.
- 5:41- Yeah. And so what are you most excited
- 5:44- about right now?
- 5:45- Well,
- 5:46- going back to automation, I think the shift
- 5:48- in what automation can do. And I know,
- 5:51- you know, sometimes automation can be scary for
- 5:53- frontline staff because they may feel that their
- 5:55- job's going away and really talking to them
- 5:59- about, you know, what the automation can do
- 6:01- for them and how they can enhance their
- 6:03- skill set to be more specific and, you
- 6:06- know, at a higher level than what it
- 6:08- was currently because we can
- 6:10- hopefully resolve a lot of the low hanging
- 6:12- fruit with, some of the automation and have
- 6:14- really more specificity and and detailed, kind of
- 6:18- thinking around some of the more complex types
- 6:21- of things. And so they can work at
- 6:22- a higher level.
- 6:24- I just think there's a lot of opportunity
- 6:26- with automation, but you have to be,
- 6:28- cautious with it because if you turn something
- 6:31- on and it's wrong, it can be wrong
- 6:33- 500 times where the person could have been
- 6:36- wrong maybe 50 times. And so just making
- 6:38- sure that you do have the checks and
- 6:39- balances in place as you continue to kinda
- 6:42- move forward down that path.
- 6:44- Yeah. And would you say that overall,
- 6:47- you're getting, good buy in with your team
- 6:49- on,
- 6:50- on starting to use some automation?
- 6:53- I I know that there is obviously some
- 6:55- hesitation with some staff as you've mentioned.
- 6:58- Yeah. We're just kind of dipping our toe
- 6:59- in at Renown right now, and I think
- 7:01- the most important thing is just transparency.
- 7:04- I think if people understand what the tool
- 7:07- is and what it can do and how
- 7:09- they fit into it,
- 7:10- it's a lot easier to obtain their buy
- 7:13- in. And it it usually goes much further
- 7:15- than just saying, oh, we're gonna do this
- 7:17- and not really a good explanation behind the
- 7:20- why. So I think that's probably the most
- 7:22- important thing with,
- 7:24- is transparency
- 7:25- in general, but especially
- 7:27- as it you're introducing new technology.
- 7:31- Mhmm.
- 7:32- And, the last question I have for you
- 7:33- today is how are you thinking about growth
- 7:35- over the next twelve to twenty four months?
- 7:38- Well, we actually have a lot of really
- 7:40- exciting things happening at Renown.
- 7:43- We have a really unique partnership,
- 7:45- that we just joined with Kaiser Permanente,
- 7:48- and that will allow a lot of additional
- 7:50- access to care in Northern Nevada,
- 7:53- as well as California. And so that's really
- 7:56- exciting. And so we're continuing to move move
- 7:59- forward with our current initiatives
- 8:01- and scale,
- 8:02- our new, you know, workflows to be able
- 8:04- to grow,
- 8:05- having a lot of growth at the system
- 8:08- And just, again, providing additional access where folks
- 8:10- didn't have access before, is really exciting. But
- 8:14- we're growing in all of our specialty care,
- 8:16- which is fantastic for the community because
- 8:19- folks were having to, travel outside of the
- 8:21- community to receive certain types of care, and
- 8:24- so now they can be within their own
- 8:25- community. So I'm excited for kind of the
- 8:29- the partnership to take off here within the
- 8:31- next twelve months and also the continued growth
- 8:34- with our different specialties at Renown.
- 8:37- Yeah. That's great. And,
- 8:39- like I said, that's all the questions I
- 8:40- had for you today. Thank you for joining
- 8:42- us today, Amanda. It's been a pleasure speaking
- 8:44- with you, and I look forward to talking
- 8:45- with you again soon. Thank you very much.
