- 0:46Hello, everyone, and welcome to the Becker's Healthcare
- 0:49Podcast.
- 0:50I'm Mackenzie Bean, associate vice president and managing
- 0:54editor of Becker's Hospital Review.
- 0:56Today, I am so thrilled to be joined
- 0:58by Dave Rehia, who is the president of
- 1:01Endeavor Health Northwest Community Hospital.
- 1:04Hi, Dave. Thanks for joining the podcast. How
- 1:06are you doing today? Doing well, Mackenzie. Thanks
- 1:09for having me on. Appreciate it. Of course.
- 1:11I mean, it's a pleasure to have you
- 1:12back. I know you've been on the podcast
- 1:14before.
- 1:15For any listeners who might be hearing you
- 1:17for the first time, I was hoping you
- 1:18could just tell us a little bit more
- 1:20about yourself, your career, and then, of course,
- 1:23Northwest Community Hospital.
- 1:26Yeah. Thanks, Mackenzie.
- 1:27Dave Rahim. I've, been part of Endeavor Health
- 1:30now for seventeen years.
- 1:32Just, as an overview, Endeavor Health,
- 1:35is
- 1:36a health system in the Chicago Suburbs and
- 1:39Chicagoland, including Chicago,
- 1:41consisting of nine hospitals over 300 ambulatory sites
- 1:44of care. We take care of,
- 1:46almost 1.4,
- 1:48million patients a year,
- 1:50with our incredible team of 27,000
- 1:54team members.
- 1:55I've,
- 1:56Endeavor Health is relatively
- 1:58a newer health system,
- 2:00by name. It is, actually coming together for
- 2:05successful
- 2:05health systems over the last five years. North
- 2:08Shore University Health Systems and health system in
- 2:11the Northern Suburbs Of Chicago,
- 2:13Swedish Hospital in, the North
- 2:16part of the city, Northwest Community Hospital in
- 2:19the Northwest Suburbs Of Chicago,
- 2:21and Edward Elmhurst
- 2:23Health at,
- 2:24which is in the Western Suburbs Of Chicago.
- 2:28We are we came together,
- 2:30a really like minded, not for profit, community
- 2:33based
- 2:33health system dedicated to excellence,
- 2:36and, serving our communities.
- 2:39And it's been a great ride.
- 2:40I've, as I mentioned, I've been part of
- 2:42the health system for seventeen years,
- 2:46starting at Glenbrook Hospital, started our Orthopedic and
- 2:49Spine Specialty Hospital in Skokie,
- 2:51president of operations of,
- 2:54the Endeavor Medical Group, as we brought together
- 2:57the four
- 2:58employed medical groups. And I have now, the
- 3:00privilege and pleasure,
- 3:02almost a year now serving at Northwest Community
- 3:05Hospital, which is our biggest of our nine
- 3:07hospitals,
- 3:08licensed for 500 beds, multiple off sites,
- 3:12and it's really been,
- 3:14a pleasure,
- 3:15and, I guess, that privilege
- 3:17of serving this community.
- 3:20Absolutely. And as you said, you're,
- 3:22almost a year into your new president role.
- 3:25As you reflect back on the last twelve
- 3:28months, what would you say is the most
- 3:30important initiative that you've led? Can you tell
- 3:32us more about it, and what sort of
- 3:34results have you seen so far?
- 3:37Sure. Yeah, Mackenzie. As I think about where
- 3:39we are in the evolution of our health
- 3:41system, we really are challenging ourselves as a
- 3:44health system to come together,
- 3:46and not just be a collection of hospitals
- 3:48or collection of provider practices,
- 3:51like most health systems across the country,
- 3:53but really challenging ourselves to be a truly
- 3:56integrated system of care across the continuum of
- 3:59care.
- 4:01Northwest Community obviously is a big part of
- 4:03that just given its size.
- 4:05Each of the hospital presidents within Endeavor also,
- 4:08are executives over clinical service lines for all
- 4:12of Endeavor.
- 4:13Working with our physician or singular physician executives
- 4:16for each service line,
- 4:19minor surgical services across all of Denver,
- 4:22neurosciences,
- 4:23and rehab.
- 4:24To answer your question, Mackenzie, really working on
- 4:27bringing together,
- 4:29integrating, and creating consistent and seamless
- 4:33seamless care,
- 4:37service lines across all of Endeavor, across all
- 4:39of Chicagoland. I would say that is a
- 4:42lot of work. It takes a lot of
- 4:44bringing,
- 4:45stakeholders together,
- 4:46but really challenging everyone to,
- 4:49put your best practices on the table.
- 4:52Let's do best practice research, but at the
- 4:54end of the day, how do we reduce
- 4:56unnecessary clinical variation
- 4:58and really settle on a singular
- 5:01Endeavor Health Way,
- 5:03best practice
- 5:04across all of our service lines. And so
- 5:06that has been a lot of the work
- 5:07that, we've been doing,
- 5:10this past year and I would say with,
- 5:12with very good success.
- 5:13It's
- 5:14been really gratifying to see all of our,
- 5:18clinical experts, our leaders, our team members coming
- 5:20together
- 5:21really focused with the patient in the center
- 5:23on designing,
- 5:25what does seamless,
- 5:27care safe, seamless, and personal care look like
- 5:30across,
- 5:31the entire continuum of care from the office
- 5:33to the hospital,
- 5:35to post acute if if necessary,
- 5:38back to the office,
- 5:39and how do we do that in a
- 5:41seamless fashion?
- 5:43It's,
- 5:45it's a big body of work, but, it
- 5:47potentially
- 5:48is gonna really differentiate us in the Chicago
- 5:51market.
- 5:52Yeah. No small task like you said, but
- 5:55but so so crucial to really achieve, like
- 5:57you said, that true integration,
- 6:00especially across,
- 6:02those specialties that you're overseeing.
- 6:05I'm curious, how are you gonna carry this
- 6:06work ahead in 2026?
- 6:09What's coming down the pipeline as you also
- 6:11think about some of the headwinds that health
- 6:13systems nationwide are facing right now?
- 6:16Sure. Yeah. I I would think,
- 6:18to that end, relative to integration, our single
- 6:21biggest project within Endeavor is, taking three instances
- 6:25of Epic
- 6:26and going to a singular instance of Epic
- 6:29for all of Endeavor Health.
- 6:31All nine hospitals, all of our offsites,
- 6:33our entire enterprise,
- 6:36working off a singular platform.
- 6:38That is really gonna be an enabling
- 6:40a significant enabling step
- 6:43to really allow us to,
- 6:45really take our integration
- 6:47and differentiation
- 6:48to the next level.
- 6:50And,
- 6:51as you can imagine, anyone who's gone through
- 6:53an EMR conversion,
- 6:56this is a major major project, but,
- 6:59one that the investment of time, resources, and
- 7:02energy,
- 7:04should produce,
- 7:05significant results.
- 7:08To your question, Mackenzie, relative to headwinds, obviously,
- 7:10The US health care industry is going through,
- 7:13a very tumultuous,
- 7:15period,
- 7:16especially relative to financing.
- 7:19Just like the rest of the country, we
- 7:21are preparing for
- 7:24patients in our communities losing,
- 7:26coverage whether that is, through Medicaid, whether that
- 7:28is not having access
- 7:30or ability to buy plans on,
- 7:34the Affordable Care Act exchanges,
- 7:36or other means.
- 7:38We are committed to our communities,
- 7:40as a not for profit health system. So
- 7:42really,
- 7:44working with our partners, working,
- 7:46as a system, and and using all of
- 7:48our assets,
- 7:49to ensure that we continue
- 7:53to, serve and take care of everyone in
- 7:55our community,
- 7:56but at the same time continue to differentiate
- 7:58and grow.
- 8:01That is gonna require very much a system
- 8:03approach,
- 8:05and making probably difficult,
- 8:07but, necessary decisions,
- 8:10to ensure that we not only survive this
- 8:12period but continue to thrive.
- 8:14Mhmm. I know. You know, so many systems
- 8:17are focused, especially on access right now, acknowledging
- 8:20a lot of those,
- 8:21you know, federal headwinds as well. And, you
- 8:25know, I think that goes hand in hand
- 8:26with thinking through
- 8:28strategic integration. So I'm curious to, like, hear
- 8:31from you and your perspective, specifically honing in
- 8:33on, you you know, you said your your
- 8:34work in surgical services where we're also seeing,
- 8:37you know, heightened demand for inpatient care and
- 8:39surgical surgical,
- 8:40demands as well.
- 8:42How are you thinking about that specifically?
- 8:46Yeah. There's no doubt,
- 8:48as more and more surgical procedures are moved
- 8:50off the inpatient only list,
- 8:52we're gonna see,
- 8:55especially from our, physicians that are aligned with
- 8:58us through independent practice, potentially pull cases,
- 9:02to, ambulatory surgery centers where they're aligned.
- 9:05We are looking at our, surgical platform to
- 9:08ensure that our cost structure,
- 9:11is ready for site neutrality,
- 9:14and reducing our cost structure,
- 9:16with the assumption that we're just gonna get
- 9:17paid less for the work that we do.
- 9:19So to continue to
- 9:23deliver the high quality care, invest in technology,
- 9:26utilize it where the evidence,
- 9:29shows that it produces the benefit.
- 9:31But then looking at how do we,
- 9:34using technology, using AI,
- 9:37reduce our cost structure knowing that we're just
- 9:39gonna get paid less for the work that
- 9:40we do.
- 9:42It's important to look at synergies,
- 9:44and really looking at not just the operating
- 9:47room, but the entire,
- 9:49perioperative system, everything from SPD,
- 9:52to pre and post PACU,
- 9:55and really thinking about how do you optimize
- 9:57those,
- 9:57very
- 9:58big and expensive assets,
- 10:02and and optimize it to, ultimately reduce the
- 10:05total cost.
- 10:07We've been spent a lot of time working
- 10:09with our care teams and our clinicians,
- 10:12ensuring that patients that, are not in an
- 10:15inpatient status, they have to go home,
- 10:18safely,
- 10:19and be discharged
- 10:20from our surgical,
- 10:24ASU environment.
- 10:25And so, we've already been, working towards that,
- 10:29knowing what's coming down the pike, and I
- 10:31think we're gonna be well positioned.
- 10:34That's great. Great to hear. And I appreciate
- 10:37you outlining that plan. You know, you also
- 10:40mentioned,
- 10:41the big,
- 10:42transition of taking three instances of Epic to
- 10:45one. Can you just speak a little bit
- 10:46more about,
- 10:48where Endeavor is in that journey? And maybe
- 10:49from your perspective,
- 10:51what you're most excited for,
- 10:54once that, integration is complete?
- 10:57Yeah. We're well into that work with the
- 11:00with, about half of our hospitals going live,
- 11:03in 2026,
- 11:06and about the other half going live in
- 11:08November
- 11:092026.
- 11:10So, we've
- 11:11been seating multidisciplinary
- 11:13teams now for,
- 11:16almost two years,
- 11:18preparing for this.
- 11:19What it's gonna allow us to do, it's
- 11:20gonna allow us to,
- 11:23allow our patients and our clinicians to work
- 11:25off a singular platform.
- 11:26From an access standpoint, it's gonna be easier
- 11:29to go to market with the singular service
- 11:31line,
- 11:32with singular decision trees built in epic
- 11:35to get patients to the right provider in
- 11:37the right location, the right time, and have
- 11:39access to the entire
- 11:41network of Endeavor Health.
- 11:44In addition for patients,
- 11:47Natalie, it will allow access,
- 11:50to be more seamless. It opens up, providers
- 11:53options to providers across our system, especially subspecialists
- 11:57that otherwise would be hard to you'd have
- 11:59to go,
- 12:00navigate,
- 12:03and potentially, it might be a little harder
- 12:05to find. It's gonna allow our clinicians also
- 12:07to, really work collaboratively and more seamlessly.
- 12:11All of our care teams will have access,
- 12:14to anything that is done with Endeavor, whether
- 12:16that is,
- 12:18in an outpatient site in Plainfield or hospital
- 12:21in Evanston,
- 12:22or a surgery in Arlington Heights. And,
- 12:26that singular platform,
- 12:28allows our care teams to coordinate, collaborate,
- 12:31and ensure that we're providing that seamless care
- 12:34across the continuum of care.
- 12:36The US health care system obviously is known
- 12:38to be fragmented,
- 12:40and so this is a giant step forward,
- 12:44within our health system to make it more
- 12:46seamless.
- 12:48And it's exciting work.
- 12:49Obviously, EMRs don't treat patients, but it's a
- 12:52huge enabling step,
- 12:54to allow us to,
- 12:56deploy evidence based pathways, evidence based,
- 13:00order sets,
- 13:02evidence based,
- 13:03research
- 13:04in a big swoop,
- 13:06without having to,
- 13:08make modifications in multiple places.
- 13:10I think this is gonna be a big
- 13:11step forward,
- 13:13in,
- 13:14the seamlessness
- 13:15of our care that we provide to our
- 13:17communities.
- 13:19For sure. A huge step towards,
- 13:21that seamless, highly coordinated care, like you said.
- 13:25I wanna end here, taking an opportunity to
- 13:28look at,
- 13:30how you're thinking about growth in 2026 and
- 13:33beyond. I'm curious from your perspective, what do
- 13:35you see as some of the best opportunities
- 13:37for organizational
- 13:38growth, or how are you really defining growth
- 13:40right now?
- 13:42Yeah. No. We are very much,
- 13:44focused on growth. I I think growth, though,
- 13:47is gonna be,
- 13:48defined on the system level and on the
- 13:50service line level,
- 13:52instead of each individual
- 13:54hospital per se. Again, we've,
- 13:57we've moved to singular clinical service lines, singular
- 14:01institutes,
- 14:02across all of Endeavor Health
- 14:04across Chicagoland
- 14:05and then,
- 14:06ensuring that our strategic and growth plans
- 14:09are aligned with optimizing each service line across
- 14:13all of Chicagoland.
- 14:15And so that, is forcing us to really
- 14:18think and make strategic decisions on not what
- 14:21is best for any individual hospital campus,
- 14:24but what is best for,
- 14:26a particular institute or service line,
- 14:29and by extension,
- 14:31the patients in the that are getting cared
- 14:33for in those institutes and service lines across
- 14:35all of Endeavor Health.
- 14:37And so,
- 14:38we are focused on those systemness
- 14:41decisions,
- 14:43and that starts with growing our front doors,
- 14:45making sure that our patients have access,
- 14:48to primary care, immediate care, virtual care,
- 14:51to get into
- 14:52our system and then growing subspecialty access
- 14:56across the service lines across all of endeavor.
- 14:59That should set us up, to be successful
- 15:02not only in a fee for service world,
- 15:04but also as we continue to take on
- 15:06more risk in a value based world as
- 15:09well. So our strategy is well aligned,
- 15:12for the headwinds and what the future of
- 15:15US health care is gonna
- 15:17was gonna come down the pike. But, as
- 15:19you know, Mackenzie, Chicago is very competitive.
- 15:22There's a lot of very good providers, but
- 15:24we are setting ourselves up to be differentiated
- 15:26in the Chicago land market,
- 15:28and offer a higher level, more coordinated, seamless,
- 15:34coordinated care, across our clinical service lines that,
- 15:39really will be world class and allow patients
- 15:42to get care in their communities close to
- 15:44home.
- 15:45Mhmm. I'm curious. Is there a specific
- 15:48decision,
- 15:49that you've made recently that you talked about
- 15:51of necessarily maybe what's not best best for
- 15:53the hospital, but is best for the service
- 15:55line,
- 15:56that you could share to help sort of
- 15:58paint a picture for the listeners of how
- 16:00you're thinking about this and weighing it out?
- 16:04Sure. Yeah.
- 16:05We've opened,
- 16:07across Endeavor,
- 16:09several centers of excellence. I mentioned earlier
- 16:12that,
- 16:13one of the one of the projects I
- 16:14led was starting an orthopedic and spine specialty
- 16:17hospital
- 16:21based in Skokie. We made the decision to
- 16:23pull orthopedics
- 16:24out of several hospitals,
- 16:28within the Endeavor system, then the North Shore
- 16:30University Health System,
- 16:32and centralized care into the specialty hospital.
- 16:36That was a key decision.
- 16:38It helps that our p and l accountability
- 16:41is on the horizontal service line level,
- 16:44because if it was on the vertical,
- 16:46hospital level, no hospital president would want orthopedics
- 16:49pulled out of their hospital,
- 16:50given it's a higher margin service line.
- 16:54So that enabling step of aligning,
- 16:57where the accountability is from a p and
- 16:59l
- 17:01allowed us to then make,
- 17:03more strategic decisions on, how do we
- 17:06develop subspecialized orthopedic care similar,
- 17:10to our cardiovascular
- 17:11care. We have two
- 17:13large,
- 17:15cardiovascular pavilions,
- 17:16Heart Hospital at Edward Hospital and the Cardiovascular
- 17:19Institute Pavilion at Glenbrook Hospital.
- 17:22While most of our cardiac care is distributed,
- 17:25high volume,
- 17:26across all of our Endeavor sites,
- 17:29We have centralized open heart surgery,
- 17:32TAVRs,
- 17:34high complexity, low volume cardiac procedures to those
- 17:38those two sites.
- 17:39Again,
- 17:42difficult decisions, but when what it allows us
- 17:45to do, it allows our teams to get
- 17:47more subspecialized.
- 17:48We can bring in
- 17:49the latest and greatest,
- 17:51technology,
- 17:52and really double down on doing those procedures,
- 17:55those high complex, low volume
- 17:58procedures better than anyone else.
- 18:00So those would be two tangible examples of
- 18:03how we have,
- 18:05really elevated the care,
- 18:07for our communities.
- 18:09Great examples. Thanks for sharing those. I think
- 18:11it's always helpful for listeners
- 18:13to get an inside look in in some
- 18:15of those strategy decisions.
- 18:17Dave, it's been so interesting speaking with you
- 18:20and hearing,
- 18:21about some of the work that you've been
- 18:22doing at Endeavor, how you're thinking thinking about
- 18:24growth, your significant integration efforts. I'm excited to
- 18:28follow along and see how this work continues
- 18:30in 2026.
- 18:31So thank you so much for joining us
- 18:32on the podcast.
- 18:34Thank you, Mackenzie. Appreciate the opportunity.
