- 0:00This is Andrew Cass with the Becker's Healthcare
- 0:01Podcast. I'm thrilled to be joined today by
- 0:04Kellen Piddle, CFO at People Incorporated. Kellen, it's
- 0:07a pleasure to have you on the podcast.
- 0:09Yeah. No. Thanks, Andrew. I'm excited to be
- 0:10here.
- 0:12Before we begin, can you tell us a
- 0:14little bit about your background and, your organization?
- 0:17Absolutely. So, yeah, like Andrew mentioned, I'm Kellen
- 0:21Tittle. I'm the chief financial officer
- 0:23at People Incorporated.
- 0:25I've been here almost thirteen years in a
- 0:27variety of roles and cross functional,
- 0:30roles as well.
- 0:31Currently, I'm in charge
- 0:34of all of revenue cycle,
- 0:35accounting, data enterprise,
- 0:37new ventures,
- 0:39strategy,
- 0:40and also I also get a an operational
- 0:42canvas long term residential that I'm in charge
- 0:45of as well.
- 0:46Seen revenue grow from $35,000,000
- 0:49to currently almost $75,000,000.
- 0:51So we've been a growing agency ever since
- 0:54I got here, and we're very excited
- 0:56about the future.
- 0:58Yeah. Speaking of the future,
- 1:00I guess, the present there, what are three
- 1:02of the top trends you're following in health
- 1:04care today?
- 1:05Yeah. No. From a from a mental health
- 1:07standpoint,
- 1:08the top three that our agency and myself
- 1:10are very, very focused on are really shifting
- 1:13from access to outcomes.
- 1:16One thing that we look for at People
- 1:17Incorporated
- 1:18is a really integrated model.
- 1:20Care coordination is always top of mind for
- 1:22our agency.
- 1:24We know that of our 13,000
- 1:26individual unique clients,
- 1:27that the more we're able to talk to
- 1:29them, the more we're able to connect with
- 1:31them, the more we're able to follow-up and
- 1:33making sure not only are they aware of
- 1:35our services, but they're attending them and we're
- 1:37getting follow-up, the better off that not only
- 1:40their outcomes are, but the better off it
- 1:42is for the general community and schools as
- 1:44a whole. So we know that working with
- 1:46our payers, working with our funders,
- 1:49that not only is access important,
- 1:52but also reaching for those outcomes is just
- 1:54as important and critical as well.
- 1:56Our second thing that I'm looking for from
- 1:58a trend standpoint is workforce. I mean, when
- 2:01you look at our our web pages of
- 2:04opening positions,
- 2:05at any given time, we have 75 to
- 2:0785 open positions.
- 2:09Burnout is the thing. Turnover is consistent.
- 2:13Right now, we have a lot of documentation
- 2:14to make sure that we're hitting not only
- 2:17from a billable standpoint,
- 2:18but from a client share standpoint in our
- 2:20system.
- 2:21I mean, we're only gonna be able to
- 2:23do this as a whole and continue to
- 2:25grow
- 2:26in our communities
- 2:27by having the right people in the right
- 2:29places, but we know that we're not gonna
- 2:31be able to hire our way out of
- 2:32storage. We really have to redesign that care.
- 2:35And People Incorporated
- 2:36was one of the seven demo
- 2:38CCBHCs,
- 2:40which is certified
- 2:41community behavioral health clinics.
- 2:43And what they've done in the state of
- 2:45Minnesota
- 2:46is really focusing on that integrated care. They
- 2:49wanna know what can you do from an
- 2:52integrated care model,
- 2:53and you tell us how much it's gonna
- 2:55cost you, and we'll work to give you
- 2:56that reimbursement rate. Now it's not perfect, but
- 2:59at the same time, as we're trying to
- 3:01redesign that care model and try to make
- 3:03sure how do we do more for the
- 3:05client with less staff because they're just not
- 3:07available, we're really working towards an integrated model
- 3:10to make sure that our workforce not only
- 3:12can sustain itself but also grow at a
- 3:16consistent model as well.
- 3:18And for our third one that I'm really,
- 3:20really curious about, I'm sure it's on every
- 3:22agency's mind, is AI. I mean, it's the
- 3:25buzzword right now. It's going around all over
- 3:27the place. We're really focused on
- 3:30the one most recent that we've re that
- 3:31we're rolling out is our,
- 3:34client documentation.
- 3:35How can we make sure that our clinic
- 3:37documentation
- 3:39on our clients
- 3:40is not only
- 3:41robust
- 3:42and accurate, but also make sure that we're
- 3:44using AI to kinda filter things through because
- 3:47our payers right now have their AI models,
- 3:49and they're denying claims. If they deny claims
- 3:52versus because our documentation
- 3:54wasn't correct in their eyes. We gotta make
- 3:56sure that our AI tools can match up
- 3:58to their AI tools
- 4:00and get our claims processed in not only
- 4:02a a quick way, but also an efficient
- 4:04way to make sure we're getting reimbursement quickly.
- 4:07And now since you brought up both workforce
- 4:09and AI, do you see AI as a
- 4:11tool that can help automate the workforce as,
- 4:14you know,
- 4:15there are some challenges filling roles?
- 4:18Absolutely.
- 4:19I mean, not only from our applicant tracking
- 4:22system, which we're using AI to kinda filter
- 4:24through,
- 4:25resumes, but also on the back end, yes,
- 4:27through client documentation, working through our electronical
- 4:31medical record,
- 4:32really trying to figure out how do we
- 4:34become more efficient, but also make sure that
- 4:37that our clinicians if they're able to cut
- 4:39down 20% from a documentation time, that means
- 4:42they're able to spend that much more time
- 4:43directly impacting our clients and impacting our communities.
- 4:47So, yeah, AI is a big a big
- 4:49thing for not only our agency, but I'm
- 4:51sure the rest of mental health and physical
- 4:53health in our spaces.
- 4:55Yes. And so how do you feel your
- 4:57AI journey is going, so far overall?
- 5:00I would say we're pretty we're still in
- 5:02our infancy. We're we're we haven't historically been
- 5:05an early adapter. I would say we want
- 5:07there's so many regulations.
- 5:09There's so many rules in health care that
- 5:11we wanna make sure that we're partnering
- 5:13with our electronic medical record
- 5:16that not only they're making sure their systems
- 5:18are working very well, but for our staff
- 5:21that we're rolling it out on a distinct,
- 5:23manner. So we know, like, we can get
- 5:26the buy in from the staff, but also
- 5:28making sure that we're not overwhelming
- 5:30our staff without clear documented policies and procedures
- 5:33and making sure that our training is not
- 5:36only sustained but also able to get everyone
- 5:38on board at the same time. So right
- 5:40now, we're kind of working on those policies
- 5:42and procedures. We're working on those training plans
- 5:45and really trying to roll out that AI
- 5:46for the clinical documentation
- 5:48over the next three to four months. And,
- 5:50hopefully, if all goes well, by the end
- 5:52of this year, we'll see some really significant
- 5:54gains from our clinical documentation,
- 5:57reducing that time so they can spend time
- 5:59with our clients.
- 6:01I'm shifting gears a bit. What are you
- 6:02most excited about right now in health care?
- 6:06I mean, right now, me personally and People
- 6:08Incorporated is really that integration for the whole
- 6:11person. I mean, care coordination has been around
- 6:14for a long time, and we're still working
- 6:16on how do we continue
- 6:18to build the right skill sets and build
- 6:21the right team of clinicians
- 6:23to make sure our clients are getting all
- 6:25their needs met. So we work in schools.
- 6:27We're working in our communities. We're working in
- 6:29clinics. We're out wherever our clients are to
- 6:32meet them where they are,
- 6:34and having that care coordination, having that care
- 6:36team
- 6:37really focused on meeting our clients' needs
- 6:40is so critical. That integration piece is what's
- 6:43gonna make our outcomes
- 6:45even more strong as we continue to grow
- 6:47our CCBHC
- 6:49and as we can continue to grow our
- 6:50impact in our communities. But, yeah, integration is
- 6:54so critical. We used to historically, there'd be
- 6:56silos. Like, you'd have your residential,
- 6:58you'd have your clinics.
- 7:00Now everything is integrated in our systems that,
- 7:03hey. If somebody's being discharged from one of
- 7:05our residential claims, we know we have a
- 7:08clinic work worker waiting for them to discharge
- 7:10so they can get them into our other
- 7:12services.
- 7:12We want them to be able to access
- 7:14our
- 7:15services
- 7:16as quickly and as integrated as possible because
- 7:19we know if they just charge for one
- 7:20of our programs, we may not see them
- 7:22again. So we have to have that soft
- 7:24handoff between all of our programs to make
- 7:27sure our clients are consistently being seen to
- 7:29help them.
- 7:31And, how are you thinking about growth over
- 7:33the next twelve to twenty four months?
- 7:36Oh, growth. I mean, in a perfect world,
- 7:39like, it's one of those weird businesses. Mental
- 7:41health, our phones ring all the time. On
- 7:44any given day, we're getting three to 500
- 7:46calls. So from a business standpoint,
- 7:49we have almost unlimited demand.
- 7:51But from a growth standpoint,
- 7:54even if we do everything right, Andrew, we're
- 7:56lucky to get a 2% margin.
- 7:59So in order for us to grow sustainably
- 8:01and also fiscally responsibly,
- 8:04we need to be very, very sure that
- 8:07what we're doing is not only going to
- 8:09be
- 8:10full but also reimbursed in the future. I
- 8:12don't know if you've seen, but Minnesota recently,
- 8:15there's been a lot of fraud allegations.
- 8:17I mean, Minnesota has been in the news
- 8:19for a lot of different reasons.
- 8:21Frankly, People Incorporated, we have nothing to worry
- 8:23about. We run a very, very
- 8:25high integrity agency.
- 8:27But being a nonprofit in Minnesota, sometimes you
- 8:29get roped into those those conversations.
- 8:32For us to continue to grow,
- 8:35we need to have ourselves
- 8:37in the right places, in the right areas.
- 8:39I would say
- 8:40between the metropolitan areas, they're a little bit
- 8:43saturated. I think our growth and our sustainable
- 8:45growth is gonna be in the the first
- 8:47or second ring suburbs of the Minneapolis, Saint
- 8:50Paul areas and really continuing to grow our
- 8:53mission, which is the safety net, and align
- 8:55that growth with sustainability.
- 8:57So we need to make sure that our
- 8:59staffing models make sense. We need to make
- 9:01sure that our our supervisory
- 9:03from a standpoint of can we have staff
- 9:05to supervise
- 9:06are there as well. We know we're not
- 9:08graduating enough,
- 9:10licensed professionals in the state of Minnesota.
- 9:12There's been a worker shortage for as long
- 9:14as I've been in this business. So in
- 9:16order for us to grow, not only do
- 9:18we need to make sure it's fiscally responsible
- 9:20and sustainable from a client, but, also, we
- 9:23need to make sure we can staff it
- 9:25appropriately. So for People Incorporated, we're always looking
- 9:28to grow, but we know that in order
- 9:30to grow, we need to take on some
- 9:32debt, but also make sure that our staffing
- 9:35is aligning with our sustainable growth.
- 9:37Those are all the questions I had for
- 9:39you today, Kelvin. Thank you so much for
- 9:40joining us. It's been a pleasure speaking with
- 9:42you, and I hope we can talk again
- 9:44soon.
- 9:45Yeah, Andrew. No. I appreciate it. I just
- 9:47wanna leave maybe one last lasting impression. It's
- 9:50just we know that the future of nonprofit
- 9:52mental health care isn't just about kind of
- 9:54expanding access. It's really about looking at those
- 9:57outcomes,
- 9:59supporting our clinicians and the rest of our
- 10:01staff, and making sure that our systems in
- 10:03place can really actually sustain both of those
- 10:06models, not only physically, but also from a
- 10:08standpoint
- 10:09clinically as well. So, yeah, we have we're
- 10:12very excited for what we've done. We're gonna
- 10:14continue to grow here at People Incorporated.
- 10:16And, yeah, I appreciate you letting me talk
- 10:18about not only our agency, but also what
- 10:20we're trying to do with mental health of
- 10:21Minnesota.
- 10:23Of course. Oh, it's a pleasure.
