- 0:38This is Scott Becker with the Becker's health
- 0:41care podcast.
- 0:42We're thrilled today
- 0:43to to be joined by Ewan Condon.
- 0:46Ewan is an editor in chief the editor
- 0:48in chief of Becker's health care.
- 0:50Brilliant leader, brilliant thinker about health care and
- 0:53health care system issues.
- 0:55Alan, I'm gonna tee it up and ask
- 0:57you to take it away and tell us
- 0:58what are a few of the stories that
- 1:00you're watching currently in health care. Again, Scott
- 1:03Becker with you with Becker's Healthcare.
- 1:05Ellen Condon, editor in chief. What are a
- 1:07couple of the stories you're watching closely currently?
- 1:10Yeah. Pleasure to be to be back on
- 1:12after a couple weeks away, Scott. But, I
- 1:15think
- 1:15coming to the January
- 1:17now, of course, we're starting to see some
- 1:20of the
- 1:21health system q four slash full year 2025
- 1:25financial reports come through and also on the
- 1:27payer side as well. So just today,
- 1:29HCA read the results. Her earnings call a
- 1:32little bit later on this morning.
- 1:34So just kinda to to give readers, audience,
- 1:36a bit quick update on where HCA stands
- 1:38as today. Fantastic system, huge for profit system,
- 1:42and some really, stellar financial results as it
- 1:45relates to 2025.
- 1:46So,
- 1:49HCA overall,
- 1:50reported a net income for 2025
- 1:54of around about $6,800,000,000.
- 1:56And to give you some context, that's,
- 1:59about an 8% increase year over year from
- 2:01what from the net margin, the net income
- 2:03they reported in the previous year.
- 2:05And just as hone in on the revenue
- 2:07growth,
- 2:09grew in revenue by about $5,000,000,000
- 2:12in revenue this year.
- 2:14Now it's about a $75,000,000,000
- 2:16company, $75,000,000,000
- 2:18health system.
- 2:20Revenue out outpaced expenses by quite a bit.
- 2:23Revenue grew by $5,000,000,000.
- 2:25Expenses grew by about $3,700,000,000.
- 2:27So really, a substantial performance on the operating
- 2:30side, financial side.
- 2:32And just to give some context as to
- 2:34what we're expecting, what HCA is expecting for
- 2:372026,
- 2:38issued some some some guidance granted. It's only
- 2:41the start of the year now, but HCA
- 2:43similarly
- 2:44expects to grow by about another $5,000,000,000
- 2:47in revenue for 2026.
- 2:49Now a lot a lot still up in
- 2:50the air, a lot can change, but still
- 2:52forecasting some impressive revenue growth and figures
- 2:55for this year as well. And then also
- 2:58has earmarked about $5,000,000,000
- 3:01for the entire company for acquisitions,
- 3:03for a capital expenditures,
- 3:05new hospital bills, new ASCs, outpatient
- 3:08growth, and whatnot. So
- 3:10no certainly no signs of slowing down in
- 3:13terms of HCA growth, fantastic leadership, but really,
- 3:16really impressive performance across the board from that
- 3:18for profit health system, Scott.
- 3:21Thank you. And and they keep on sort
- 3:23of chugging along, and it seems like some
- 3:25of the larger
- 3:27not for profit systems
- 3:29are doing pretty well as well. But HCA,
- 3:31at least profitability
- 3:32wise, seems to outpace everybody year after year.
- 3:35And they're also one of the few large,
- 3:37large health systems
- 3:39for profit health systems that have stuck to
- 3:41really being a hospital system or at least
- 3:44as much as anything a hospital system, whereas
- 3:47Tenet
- 3:48and others
- 3:50have moved to heavily outpatient as well as
- 3:53being a health system.
- 3:54I mean, it's fascinating what HCA has accomplished
- 3:56and how they continue to do so.
- 3:59Yeah. Absolutely. I think,
- 4:01it is really interesting, Scott, to your point
- 4:03as you as you look at where HCA
- 4:04stands, where Tenet stand, some diverging strategies over
- 4:07the last couple years.
- 4:09HCA, no doubt, it's quite a substantial outpatient
- 4:12footprint, but nowhere near has made that investment.
- 4:15The likes of tenant is doing, the likes
- 4:17that or even see Ascension, somewhat doing on
- 4:19the for profit side with their potential
- 4:21acquisition of AmSurge, which is, likely to close
- 4:24maybe this this quarter as well. But I
- 4:26think HCA very very much still in the
- 4:28hospital game. I believe about a 190 hospitals
- 4:30in The US,
- 4:32compared to about 2,500
- 4:33ambulatory
- 4:35care sites and whatnot.
- 4:37But but kinda ties nicely into the next
- 4:39the story that I wanted to touch on,
- 4:41Scott. When staying on the line of for
- 4:43profit health systems here,
- 4:45community health systems, the other for profit system,
- 4:48hasn't quite reported its fourth quarter or full
- 4:51year twenty twenty five results yet. We believe
- 4:53they're gonna come either next week, so pay
- 4:56close attention to there. But,
- 4:59had the the pleasure of speaking with my
- 5:01colleague, Kelly Gooch. We we interviewed the CFO
- 5:04and the CEO
- 5:06of Community Health Systems. That is Kevin Hammonds,
- 5:08the former CFO, and their new CFO, Jason
- 5:11Johnson. So just to really dive into the
- 5:14strategy behind CHS ongoing turnaround,
- 5:18with their new CEO, new new CFO leader.
- 5:20So
- 5:21really ongoing divestiture program here. CHS is selling
- 5:25this year five hospitals for a combined about
- 5:28a billion dollars.
- 5:30So those five hospitals are in Alabama. They're
- 5:32in Tennessee.
- 5:33And, notably,
- 5:34three of the hospitals it's selling are in
- 5:36Pennsylvania, and those three hospitals has been trying
- 5:39to get over the the line for quite
- 5:40some time, had one deal with a a
- 5:43a nonprofit health system that fell through, collapsed
- 5:45in November
- 5:462024,
- 5:47now trying to sell that same three hospital
- 5:49health system in Pennsylvania,
- 5:51Commonwealth Health,
- 5:53and and that would signify its exit from
- 5:56the state of Pennsylvania.
- 5:58So so looking to sell hospitals in Alabama,
- 6:00Tennessee, and Pennsylvania
- 6:02as part of its ongoing divestiture program, those
- 6:05five hospital sales in those three different markets,
- 6:08again, will be a combined $1,000,000,000,
- 6:10in capital for,
- 6:12CHS. So
- 6:14I think the last quick kinda note here
- 6:16just in terms of,
- 6:18speaking with Kevin and and Jason at CHS,
- 6:21essentially, what they told us is that this
- 6:23divestiture plan really for Exosystems'
- 6:26broader strategy of redefining its portfolio
- 6:29around those core core markets and hospitals that
- 6:32it wants to focus on and continuing
- 6:34to reduce its debt. So
- 6:37since 2039,
- 6:39I believe, CHS has paid down more than
- 6:41$3,000,000,000
- 6:42in debt. And, again, more of these hospital
- 6:45sales will acquire more liquidity, more capital again
- 6:47to pay down some of its debts, really
- 6:49hone in on some of its core markets.
- 6:51But
- 6:52different different end of the spectrum when we
- 6:54see what's going on at CHS,
- 6:56ongoing ongoing revamp of its operate operating model,
- 7:00paying down some of its debts, ongoing turnaround,
- 7:02and we look at the likes of your
- 7:04HCA and your tenants, really, really strong returns,
- 7:06continue to grow and strong margins there.
- 7:09No. It it is remarkable
- 7:11what CHS
- 7:12HSHS,
- 7:14COD Community Health Services, has done over the
- 7:15last decade to get themselves back into the
- 7:18type of fiscal shape they need to be
- 7:20in. They'd at one point overextended themselves
- 7:23in the effort to chase HCA
- 7:25and be the largest
- 7:27owner of hospitals
- 7:28in the country,
- 7:30that was under a couple CEOs go. And
- 7:33in doing so, they'd highly leverage themselves. Then
- 7:35they've spent the last five to ten years
- 7:37trying to
- 7:38trim themselves back down and put themselves in
- 7:40great fiscal shape, and they've done a great
- 7:42job of it. But it is fascinating to
- 7:43see that divergences between HCA,
- 7:46community health systems, and tenant health care and
- 7:48where they've each gone in terms of being
- 7:50some of the major for profit health system
- 7:52chains in the country.
- 7:54Mhmm. Yeah. Absolutely.
- 7:56And I think
- 7:57last last one on that note, I think
- 7:59since we're on the topic of of mergers
- 8:01and acquisitions, hospital sales, divestitures,
- 8:03I think we we should call out,
- 8:07Providence, CommonSpirit, too big through the largest nonprofits,
- 8:10Catholic faith based systems,
- 8:12also going through looking to divest and sell
- 8:15some hospitals in certain markets as they look
- 8:17to really kind of focus on some of
- 8:19their core markets, really take a harder look
- 8:21at where they're operating, and also look to
- 8:22get a bit of an injection of capital
- 8:25as well to focus on some of their
- 8:27their ongoing turnaround efforts. Both systems do expect
- 8:29a big, big financial hit this year in
- 8:31terms of, some of the Medicaid, federal cuts
- 8:34coming down the pipeline late twenty twenty six,
- 8:362027,
- 8:37and everything along with the ACA subsidies as
- 8:39well up in the air, of course, accounting
- 8:40for that. So really kinda try to get
- 8:43ahead of the game in terms of offloading
- 8:45some hospitals that make sense in certain markets.
- 8:47And CommonSpirit
- 8:48is in talks to sell
- 8:51seven hospitals,
- 8:52at the moment.
- 8:53Four hospitals will be sold in North Dakota
- 8:57to local health system, Altru Health System, and
- 9:00then it also is aiming to sell three
- 9:02hospitals
- 9:03in Ohio.
- 9:05It's, to UPMC
- 9:07Health. So, essentially, that would really
- 9:09UPMC really make it expand deeper and deeper
- 9:12into Ohio,
- 9:14with that three hospital sale that was expected
- 9:16to close, I believe, in the second quarter
- 9:18of this year.
- 9:20Yeah. It's not just the for profit systems
- 9:22and the that are also making some of
- 9:23these divestitures. We're also seeing it likely the
- 9:25same. We saw Ascension, of course, on the
- 9:27nonprofit side really revamped its hospital portfolio, consolidate
- 9:31hospitals, a lot of divestitures in key markets.
- 9:34CommonSpirit and Providence now doing some of the
- 9:36same as we think about some of the
- 9:38financial challenges that are coming down the pipeline
- 9:40late this year and into next, Scott.
- 9:43Elon, literally just fascinating.
- 9:45Thank you so much for taking the time
- 9:46with us today on the Beckers HealthCare podcast.
- 9:50The the the amount of efforts at these
- 9:52major, major not for profits, I mean, Ascension
- 9:56is doing a fascinating thing to get into
- 9:57the ASC business with
- 9:59acquiring AMPSURG.
- 10:02CommonSpirit,
- 10:03like Community Health Systems, trying to trim their
- 10:05portfolio of hospitals a little bit. But it's
- 10:07fascinating to see the different strategic moves that
- 10:10some of these large systems are making to
- 10:11stay competitive and stay financially thriving. Just fascinating.
- 10:16Alan,
- 10:17thank you so much for joining us as
- 10:18always on the Veterans Healthcare Podcast. You're both
- 10:20a remarkable journalist and, more importantly, a remarkable
- 10:24leader. Thank you so much for joining us.
- 10:26Always a pleasure. Thank you so much, Scott.
- 10:28Cheers.
