- 0:02You're listening to the
- 0:03Play Therapy Parenting Podcast with Dr. Brenna Hicks.
- 0:08Hi, I'm Dr. Brenna Hicks, The Kid Counselor.
- 0:11This is the Play Therapy Parenting Podcast where
- 0:13I give you insight awareness and
- 0:15enlightenment about your parenting and your relationship with your kids.
- 0:19In today's episode, I am asking a question
- 0:21that was actually submitted by a therapist on behalf of a parent.
- 0:26So I know I have a lot of carryover
- 0:29listeners between my parenting podcast and my Play Therapy Podcast.
- 0:33So I so appreciate that therapists are
- 0:36referring to parents and parents are listening
- 0:39to the therapist podcast and I think
- 0:41there's a really cool crossover that's happening.
- 0:43So
- 0:44a therapist emailed on behalf of a parent
- 0:47and it's specifically about
- 0:49a six year old girl with some body image issues.
- 0:53So we'll,
- 0:55I'll read parts of the question and I'll hit some highlights for you and
- 0:58then we'll kind of dive into some thoughts specifically on how to best handle
- 1:04a child that is really struggling with body image and,
- 1:09and eating and all of those types of concerns.
- 1:12So
- 1:13the question that I got is
- 1:16there's a six year old girl who is already struggling with body image issues,
- 1:20for example, saying that her tummy is fat saying that other kids.
- 1:23Look at her because of this.
- 1:25Mom is concerned that she is eating to soothe big emotions.
- 1:30Mom has incorporated a lot of play therapy skills into her
- 1:33parenting and we are working through this with child-centered play therapy.
- 1:37Thank you for any information.
- 1:39I really appreciate your podcast. So
- 1:41thank you for and I'm actually not using
- 1:44this therapist name just to protect confidentiality.
- 1:47So
- 1:48know that I'm intentionally not telling you who it is,
- 1:51but the email came from Kansas. So I will let you know that.
- 1:54So
- 1:55the therapist is really kind of actively working with this child.
- 2:00And then mom is struggling to know how to handle this. And
- 2:03this is something that is becoming much more prominent. I think that
- 2:07with social media
- 2:08and with kids being online all the time and having the expectations of the
- 2:13perfect life and the perfect body and the perfect image and the perfect photo
- 2:17and, and all of that, that has been driven by the social media
- 2:22craze.
- 2:24I think that younger and younger we are seeing these types of struggles.
- 2:27And so this is something that we need to not only be aware of,
- 2:29but we also need to make sure that we understand
- 2:31it well and then therefore that we're handling it effectively.
- 2:35So I really appreciate the, the email and we're going to dive in with some thoughts,
- 2:40the path to calm, confident and in control parenting starts now.
- 2:50All right. So let's let's go back to
- 2:53some previously discussed topics, but it's worth revisiting because
- 2:57with the
- 2:592.0 version or season two, if you will, of the Play Therapy Parenting Podcast,
- 3:03I know some of you are
- 3:05new listeners and you have not gone and listened to all of the earlier episodes. So
- 3:10one of the things that we've talked about a lot
- 3:12is that children do not really have control over much in their lives.
- 3:17So
- 3:18when we look at the things over which children do actually have control,
- 3:22it's limited to things you can fit on one hand.
- 3:25So as a general rule, kids can control their, eating
- 3:29their sleeping, their toilet
- 3:31and their obedience.
- 3:34So
- 3:35it's been very clear and obvious
- 3:38that when a child is fighting for control
- 3:41because they feel that they don't have much control.
- 3:44It's usually going to default to one of those four issues
- 3:48or sometimes more than one. But eating sleeping toilet and obeying are, are the four
- 3:54typical areas with which the child will
- 3:58want power and they will realize that they actually can take,
- 4:01take control in those scenarios.
- 4:04So since eating is one of those,
- 4:05we can immediately understand that this is a need for control
- 4:10and then that begs the question,
- 4:14she is trying to control
- 4:17this area of her life.
- 4:18So
- 4:19the curiosity then is what has felt out of control in her life
- 4:25and that can be a variety of things.
- 4:27So it can be big changes. It can be
- 4:31struggles academically,
- 4:33it can be social inadequacy, it can be emotional immaturity,
- 4:37it can be family or situational, a divorce, a separation,
- 4:42a death,
- 4:43a move.
- 4:45What are the scenarios that
- 4:48have felt very out of this child's control?
- 4:52And if you can rule out
- 4:54those types of issues.
- 4:56So if, if mom is looking at the scenario going,
- 5:00I really don't see that there have really been any kind of big changes or any
- 5:04areas over which she felt like things kind of got really out of her control.
- 5:09If that is ruled out, then we can almost always tie it to very high levels of anxiety.
- 5:15And here's why
- 5:17when a child is extremely anxious,
- 5:21the way that they deal with their anxiety
- 5:24is trying to take control
- 5:27in another area of their life
- 5:29so that they feel powerful and it's not scary.
- 5:34So when you have a very anxious child,
- 5:36they will often grasp at control.
- 5:39So let's think through eating disorders really quickly.
- 5:42And I don't want to imply that I think this is where this is going.
- 5:46So I just want to be very clear.
- 5:47I don't feel that this six year old girl is moving toward an eating disorder.
- 5:51I just want to help you understand that there's a tie in here
- 5:55because again, eating is a need for control and
- 6:00high levels of anxiety are almost always associated with eating disorders.
- 6:04So let's unpack this.
- 6:07You have
- 6:09someone who feels that their world is completely
- 6:13out of control. Spiraling. They don't have any power over anything
- 6:18that feels so uncomfortable and so miserable and so scary.
- 6:23Well,
- 6:24I'm going to grasp at any measure of power and control that I can take.
- 6:27So I will either eat too much or I will not eat enough
- 6:32because that is something over which I do have control.
- 6:35And
- 6:36we very, very consistently, I've worked with a lot of
- 6:41clients, the older clients that have had eating disorders
- 6:44and we very,
- 6:45very consistently see that the super high levels of anxiety are tied into that.
- 6:51And so
- 6:52if we recognize control and power,
- 6:56that gives us the why.
- 6:59So for this little girl,
- 7:01instead of seeing it as an eating issue, which it really isn't,
- 7:04it's a power and control issue
- 7:07and or it's an anxiety issue.
- 7:10So
- 7:11with that as the foundation of root cause,
- 7:15now here are some things that we can do to help
- 7:18the child with that scenario.
- 7:21As she develops more self esteem,
- 7:24she will naturally feel better about herself
- 7:28regardless of whether or not she
- 7:31likes her body yet or not.
- 7:34In other words,
- 7:34she doesn't have to get to a point where she's
- 7:37happy with her body or content with her body.
- 7:40For the increase in self esteem to influence the way that she thinks about herself.
- 7:45And one of the universal outcomes of child-centered
- 7:47play therapy is an increase in self esteem.
- 7:50So as she feels more confident. And as she knows that she has self worth,
- 7:56she will naturally feel better about herself,
- 7:58whether or not her body changes at all.
- 8:02So, self esteem is a big factor in this.
- 8:04Secondly,
- 8:06as she develops an emotional vocabulary,
- 8:09she will handle her big feelings more effectively.
- 8:14So the anxiety will not have the power that
- 8:16it once had over her because she will understand
- 8:19her feelings better and she will be able to
- 8:21accurately identify them and then accurately and effectively communicate them
- 8:25because you can have your needs met if you
- 8:27know how to identify and communicate your feelings,
- 8:30which is what we talked about in the reflecting feelings episode, right?
- 8:33So
- 8:35as mom, you and therapist are both reflecting her feelings to her,
- 8:40she's developing an emotional vocabulary.
- 8:43Mom and therapist, as you are giving her choices,
- 8:48you are helping her build a sense of self esteem.
- 8:51As you are using encouragement phrases with her,
- 8:55you are helping her build her self confidence.
- 8:58So this is all tied together. You see that.
- 9:01And then finally,
- 9:03as she develops self control and self regulation,
- 9:06she will have better coping strategies.
- 9:09She will be more resilient
- 9:11and she will be able to better manage
- 9:14the circumstances in which she finds herself.
- 9:17So even if other kids look at her because she has a pudgy tummy,
- 9:22or even if she thinks that her tummy is fat,
- 9:25or even if she is so consumed by these big
- 9:29feelings that she doesn't know what to do with them.
- 9:31The self control and the self regulation that she develops
- 9:35will naturally even all of that out.
- 9:39Because that's one of the universal outcomes of
- 9:42the child's center play therapy process is self regulation
- 9:46and that's regulation across all areas of life.
- 9:51So
- 9:52as those three things continue to grow for her and mom,
- 9:56if you are using those techniques at home and therapist,
- 9:59if you are continuing to use those in the playroom,
- 10:02it will naturally sort itself out.
- 10:04And the fact that she's eating to soothe her big emotions,
- 10:08that's just another indication that she has really big feelings,
- 10:12doesn't have coping, doesn't have self control,
- 10:15doesn't have an emotional vocabulary
- 10:17and doesn't have self esteem.
- 10:19This all is connected,
- 10:21but it's very easy to look at this scenario and think this is a food issue.
- 10:27But the beauty of the play therapy process, specifically,
- 10:30the child-centered approach
- 10:32is
- 10:33we understand the undercurrent, we understand the why
- 10:38we understand what's driving
- 10:40the behavior because the behavior is just the cue
- 10:44into where the child is struggling or hurting.
- 10:48Let me say that again,
- 10:49the behavior is just the cue
- 10:51to where the child is struggling or hurting.
- 10:54So
- 10:56what she's saying and what she's doing
- 10:59are not actually the issues,
- 11:02that's just what gives us the clue
- 11:04as to what's really going on.
- 11:06And in this case, it's a lack of self esteem, a lack of emotional vocabulary,
- 11:11a lack of self regulation
- 11:13and either super high levels of anxiety
- 11:16or a huge need for power and control.
- 11:20So in context of that, the whole process changes,
- 11:23not that we adjust anything from the skills that we're using,
- 11:27but we can see it in a different light and we're
- 11:30no longer trying to address the food and the eating.
- 11:34We're getting to all of the other stuff that's underneath that.
- 11:38So mom and therapist in Kansas,
- 11:40I hope that that's super helpful for you and for those of you that
- 11:45maybe have that same kind of issue with your own child,
- 11:47you notice that they're super picky with food,
- 11:49they will only eat certain types of things.
- 11:51They'll only eat
- 11:53at certain times of the day.
- 11:55They refuse most things, they
- 11:59eat too much, they hide food in their bedrooms. I mean, I've, I've heard it all right.
- 12:03This is traditional kid behavior. You don't have to teach it.
- 12:06They just, they just know it right.
- 12:08So if you are struggling in any capacity with your child and their food
- 12:13and then as a related aside or their sleeping or their toilet or their obedience,
- 12:18be aware that there is a root cause
- 12:21of power and control.
- 12:23And if you can sort that out,
- 12:25that will equip you with how you can better address it. But remember
- 12:29your go to are your four pillars.
- 12:32You reflect the child's feelings, you give them choices,
- 12:35you encourage them and if necessary, you set limits.
- 12:39So the the pillars and the approach don't change.
- 12:42But understanding the why always does give us better
- 12:45perspective and insight about how best to handle things.
- 12:49So,
- 12:50thank you, therapist and mom in Kansas. I appreciate that question.
