- 0:00Hi, everyone. This is Lucas Voss with Becker's
- 0:01Healthcare. Thanks so much for tuning in to
- 0:03the Becker's Payer Issues podcast series.
- 0:06Today, we're going to be talking about boosting
- 0:08star ratings, the key to health plan
- 0:11success.
- 0:12And joining me for today's discussion, I'm very
- 0:15excited to have her, Whitney Eubanks, VP product
- 0:18management at PointClickCare.
- 0:20Whitney, thanks so much for being here today.
- 0:22It's great to have you. Thanks for having
- 0:24me. Yeah. Excited to be here and just
- 0:26to discuss this topic.
- 0:28Absolutely. Could you just introduce yourself to your
- 0:30audience to our audience really quickly?
- 0:33Sure. Yeah. So like you mentioned, vice president
- 0:35of product management for PointClickCare's
- 0:38acute and payer business where we're really focused
- 0:40on building out the country's largest real time
- 0:43care coordination network.
- 0:45I've been at PointClickCare for a few years
- 0:47now. I've spent nearly twenty years in product
- 0:50marketing and product management roles, primarily in health
- 0:53care technology.
- 0:54Then Then a lot of my time working
- 0:55with health plans and pharmacy benefit managers
- 0:58through the course of my career. So this
- 1:00is a space and a topic near and
- 1:02dear to my heart.
- 1:04And it's great to have you. And we
- 1:05are talking about
- 1:07boosting star ratings. But before we're doing that,
- 1:10we have to talk about
- 1:12why we have to boost them
- 1:15and the recent drop in star ratings for
- 1:18major health plans.
- 1:20What's at stake
- 1:22if don't they don't improve,
- 1:24and why are we seeing those drops?
- 1:27Yeah. Yeah. It's a good question. I mean,
- 1:28the drops are pretty dramatic. So just last
- 1:31year, eighty percent of Medicare Advantage enrollees were
- 1:34in four star or better plans,
- 1:37but now that number is down to sixty
- 1:39four percent.
- 1:40Wow.
- 1:41And and five star plan enrollment fell off
- 1:43a cliff. It went from twenty two percent
- 1:45down to two percent. So, yeah, the question
- 1:47is what what's going on here?
- 1:50And really, it's two main things. First, CMS
- 1:53changed
- 1:54how they calculated the cut points. Essentially,
- 1:57they removed the lowest performing plans from the
- 2:00equation,
- 2:01which Mhmm. Raise the bar really across the
- 2:04board, and I think caught a lot of
- 2:06plans a bit off guard.
- 2:08But the second thing is that the competition
- 2:10is just getting tougher. So if you're not
- 2:12improving year over year, you're actually falling behind.
- 2:16But in terms of of impact and what's
- 2:18at stake, there's real money on the line
- 2:20here. So if you are a plan and
- 2:21you drop below four stars, you're losing
- 2:24quality bonus payments, which can impact the bottom
- 2:26line and really hurt the ability to attract
- 2:29new members as well.
- 2:31I was gonna say, I mean, at the
- 2:33end of the day,
- 2:34name of the game is bottom line for
- 2:36most folks, I would assume. What's the impact
- 2:39on
- 2:40a health plan's
- 2:41bottom line? And then in turn, what are
- 2:44some of those ripple effects that you're seeing
- 2:46that impact patient care and also long term
- 2:49sustainability?
- 2:51Yeah. I mean, in in short, it's a
- 2:53big deal. So a drop from four and
- 2:55a half to three and a half stars
- 2:57can slash a plan's operating income by up
- 2:59to 40%.
- 3:01Mhmm. And when revenue drops, the plans start
- 3:03cutting back on extra benefits that make them
- 3:05competitive in the first place. So you've kinda
- 3:08got this vicious cycle going on. Lower ratings
- 3:11means fewer members,
- 3:12which means less revenue to invest in quality
- 3:15care, and eventually that starts to impact
- 3:18patient outcomes, patient experience, and the plans are
- 3:21are kind of stuck playing catch up.
- 3:23And then you're having to deal with what
- 3:25you've mentioned earlier too. There's more and more
- 3:27competition. Right? So if you're not improving,
- 3:30you're going to be even further caught into
- 3:32that cycle, and it keeps going and going
- 3:35and going.
- 3:36So what steps can health plans take right
- 3:38now to turn declining scores around and and
- 3:40stay ahead in an increasingly competitive landscape?
- 3:44I think most importantly, it's understanding where CMS
- 3:47is going.
- 3:49So they're shifting weight from patient experience to
- 3:52clinical outcomes, and real time is the name
- 3:55of the game. Your best chance to optimize
- 3:58outcomes is actually during some kind of health
- 4:00inflection point. So especially those transitions right after
- 4:04an emergency department or an inpatient encounter.
- 4:08That's when patients are the most vulnerable, but
- 4:11they're also the most receptive to behavior change
- 4:14or
- 4:15taking action on next steps that are required,
- 4:17after those encounters.
- 4:19So that means that measures like TRC, transitions
- 4:22of care, the follow-up after ED visit for
- 4:25high risk patients or what they call FMC,
- 4:28and plan all cause readmissions, PCR, which is
- 4:31actually triple weighted now. Those measures are front
- 4:34and center in star ratings just to to
- 4:36give a couple examples.
- 4:38And if you focus on transitions
- 4:40and and really improving that experience and reducing
- 4:44readmissions around those critical points, you may also
- 4:47boost your patient satisfaction as well, which, of
- 4:50course, can have an indirect impact on CAHPS
- 4:52surveys.
- 4:54And then in turn, like you said, that's
- 4:55what's going to get those stars up. In
- 4:58terms of how we're doing this,
- 5:01how can real time data access and exchange
- 5:04enhanced care coordination between health plans, hospitals, and
- 5:06providers
- 5:08lead to more of a boost in those
- 5:10star ratings and and getting them up?
- 5:13Yeah. It's a game changer. I mean, if
- 5:14you're still using claims data to manage care,
- 5:17you're you're already way too late. Real time
- 5:19clinical data helps care teams act fast right
- 5:22when the patient needs that care the most.
- 5:25Actually, my mother-in-law works as a care manager
- 5:28at a health plan, and she's even, had
- 5:30responsibility
- 5:31for training these care teams. And she's shared
- 5:33with me so many stories
- 5:35about some of the challenges that that those
- 5:37roles face. But what really stuck out to
- 5:39me is,
- 5:41her saying, you know, I have precious few
- 5:43moments with a member. And if I am
- 5:46spending that time
- 5:47asking for information about what happened when you
- 5:49were in the hospital, what were your discharge
- 5:49instructions, then I'm not spending
- 5:51ultimately will improve star ratings, if you're not
- 5:53using that precious time talking about how we
- 5:56can help guide their next steps of care.
- 5:59And it's very difficult to get the outcomes
- 6:01that we all want those outcomes that ultimately
- 6:04will improve star ratings if you're not using
- 6:07that precious time
- 6:08very wisely.
- 6:10So, you know, from my perspective, this is
- 6:12a very solvable problem with real time encounter
- 6:14alerts, real time clinical data. Care teams can
- 6:17jump in immediately after a hospitalization.
- 6:21They can work to close care gaps. They
- 6:23can coordinate with providers or automate
- 6:26intelligence heading over to providers who need to
- 6:28do follow-up care and ultimately reduce readmissions.
- 6:32But at the end of the day, it
- 6:33really just comes down to being proactive
- 6:36instead of reactive, which is exactly what CMS
- 6:38is rewarding now. Yeah. And
- 6:41like you've just talked about, it comes back
- 6:42to full circle what we talked about earlier.
- 6:44Right? It all has ripple effects on patient
- 6:47care. On that long term sustainability that we
- 6:50talked about. It's a very important piece. Whitney,
- 6:52thanks so much for taking the time today
- 6:54and and walking us through this. This is
- 6:55an important topic, and and I'm glad you
- 6:57you spent some time with us today.
- 6:59Yes. Thank you so much for having me.
- 7:01Absolutely. And we also want to thank our
