- 0:00Good morning, Good morning, good morning, and welcome to Ask the Doctor
- 0:03with doctor Carrie Williams with Med Club, Bartlesfel. How are we today?
- 0:07We're good? How are you well? You know I'm great because I've got
- 0:10a great doctor. By the way, transparency, Carrie Williams is my doctor
- 0:15direct primary care. Tell us about this particular model. This particular model is
- 0:20somewhat new in the United States. It's a practice model where patients make direct
- 0:26contact contracts with physicians primary care physicians to provide them services. At mid Club.
- 0:34We have a monthly charge depending on your age, and what that covers
- 0:39is my time, my consultations care for chronic diseases and things like physicals,
- 0:53but we do med checks. Extra things are extra labs, prescriptions and we
- 1:00offer those at med Club and a discounted rate. Whi's really great. It's
- 1:04kind of like a one stop deal there. And you're not finding yourself.
- 1:08You know it's your choice, but you're not finding yourself, you know,
- 1:11going all over town to get things done. You can get most everything you
- 1:15need done right there at med Club. Artiles Well, yes, sir,
- 1:19and I genuinely more doing them. You are now the advantage of course,
- 1:25of being a member of a med club. You got plenty of But let's
- 1:30take a look at you. Well, first of all, through transparency,
- 1:34you know how much you're going to pay every month. It's not changing.
- 1:37You know, get unexpected bills. Wait a minute, this is medical stuff
- 1:41and we're not getting unexpected bills. No, and this is usually less expensive.
- 1:46The average patient saves over twenty percent if they use versus traditional healthcare interns
- 1:55as far as they're overall cost to the statistics, and we cut out the
- 2:02middle man. There's not an insurance company with their hands out. And the
- 2:10statistics show that if you are a patient of a direct primary care practice,
- 2:16you have, as a general rule, there is a sixty five percent less
- 2:22chance that you will end up in the emergency room on an emergency room visit.
- 2:25Well, that's great that those are expensive. They are. It's a
- 2:30it's not a day at Disney World, that for sure, but it may
- 2:34cost as much. Oh my, yes, yes, And what I like
- 2:39is the accessibility I have to the doctor. Yes, we try to do
- 2:45same day or next day appointments. Some people like to book out further,
- 2:49but we do that. And for instance, if you've had an unexpected hospitalization
- 2:53and you need to be seen quickly after you've been discharged from the hospital,
- 2:58we'll try to make room for you to be seen and a lot of times
- 3:05word the number one advantage is is that you get explanations of what's happened.
- 3:10Because the medical field can be very subservient. You don't know what's going on
- 3:15when you're in the hospital, So having somebody to give you a play by
- 3:20play or they did this procedure for that reason, and that's what that's providing
- 3:29that patient understanding gets through. It gets rid of a lot of the fear,
- 3:35and fear is misery, and so if we can help a patient get
- 3:40comfortable with their medical care or what's been done and what to expect, then
- 3:46we've really done our job. One thing I like is that the appointments are
- 3:52waiting in a waiting room for an hour, can be seen for five minutes.
- 3:55That's correct. Not to make any one way and limit the size of
- 4:03the practice. When I was in a traditional medical practice, I had over
- 4:06three thousand patients. At med Club, we limit our patients just seven hundred,
- 4:14so I have time to get to know you and most of the appointments
- 4:17are at least a half hour, and you know, some patients want that,
- 4:24some patients don't. But the explanation going back, the accessibility is a
- 4:31huge thing. Being accessible to a natural physician's huge. Well. You get
- 4:39more personalized care, and for a lot of people that goes a long way,
- 4:45because we used to kind of being shuffled around and you might only get
- 4:49to see your doctor three to four minutes and the tops yes, and then
- 4:54it's in out by sea, here's your bill, yes, and it's not
- 4:57that way there. I get to ask questions. You encourage that. Yes,
- 5:02I encourage my patients to bring a list with them to the appointment written
- 5:06please, because you're not going to remember them. That's exactly right. And
- 5:13getting their questions answered again takes takes the guesswork out by the patient. You
- 5:21can go and read the computer look it up, but getting through the vocabulary
- 5:29is a lot of time, very confusing, and so many things mean the
- 5:32same thing and understanding oh they did this that means sad and I have to
- 5:44anticipate this coming at me. Yes, every patient is different, but the
- 5:50disease process is kind of with most act similar. One thing I like is
- 5:58that you'll actually bring out a physical book if needed. Absolutely, I showed
- 6:02the patient exactly what is going on and why so that we understand it.
- 6:09And that's something that I've never witnessed is a patient I am my entire life
- 6:15until being a member of med Club article well, providing that patient explanation and
- 6:21understanding, understanding how that doctor's thinking about you, Understanding this is where this
- 6:29particular organ lives, and showing the patient a picture and saying when you talk
- 6:35to the doctor, they're going to be talking left right and to your post
- 6:41here, and how that doctor determines what that means. Then the vocabulary is
- 6:48not so bad. The way that doctor talks to you is not that confusing.
- 6:54But bringing that knowledge level up. Also, if I send a patient
- 6:58to a specialist, I want them to understand what the specialist is going to
- 7:02be thinking about, oh yeah, and what to anticipate and how not give
- 7:10them any answers, but how to answer the questions that have been asked by
- 7:15the specialist. Therefore, the communication is automatically approved by tenfold yes, and
- 7:20my patient will be looked at as a cooperative patient, someone they want to
- 7:29have back in their office. And that makes doctors are no different than anyone
- 7:34else. If they like you, they'll spend more time with you. Now,
- 7:39the met Club bartlepill, we talked about that, the monthly fee.
- 7:43It covers the primary care and consultation. Now we did kind of mention labs
- 7:47and another thing, tell us more about labs and other things. You okay,
- 7:53we offer lab basic labs through our office, blood work, you're other
- 8:00things, and we have discounted prices in comparison to the national labs. We're
- 8:07not adding on what the insurance company would add or what that middleman for processing
- 8:13is. You know what your lab that's going to be. And also with
- 8:18medications, I am in a buying group with other direct primary care practices and
- 8:24we get discounted Hotel medicines. Oh yes, you do a lot of times
- 8:30it will cost you less to purchase it at med Club. It will be
- 8:35cost you what your insurance will cover. Very good. This is all really
- 8:39pretty cool. Now when we take a look at this for IRS purposes,
- 8:45and I know some people do, I know I have to these days because
- 8:50the IR sings likely what is there some kind of coding or something that we
- 8:54have to kind of go through here we talked about I think called a health
- 8:58plan a health plan. The IRIS does not allow individuals to detect the monthly
- 9:05costs from their taxes. That should and that's basically the only intervention that the
- 9:13IRIS has in this. Now when we go through a costing payments from med
- 9:18Club, there is the monthly fee. And also there's a lot of great
- 9:22things that come along with this because like you said, it's not like every
- 9:26time you visit it's a monthly feed. You're going to visit as much as
- 9:30you need to visit. Correct, So if I need some screening, I
- 9:33can just come in with an appointment and we get that taken care of it.
- 9:39Like say I got an acre of pain. A week later, I
- 9:41can call up and say, hey Doc, yes, and it's still one
- 9:45monthly fee. Yes, huh that here. We also text and email my
- 9:52patients and I communicate a lot. Even though you may not see me every
- 9:56month, there's a lot of communication that goes on behind behind the scenes.
- 10:03Your labs are sent to you, I have looked at them. I'll make
- 10:05comments on them and or X rays or other tests. Patients have questions,
- 10:16you know my medicine that I started as doing this is this normal or is
- 10:20this not? And also there's access, emergency access to me after hours and
- 10:26on the weekends. So that's that cuts down on a lot of the er
- 10:33visits. It does, Yeah, absolutely does we keep you well, We'll
- 10:39try. And I put an enormant amount of time into the patients figuring out
- 10:43where they're at, what needs to be done, what they've done, what
- 10:48they haven't done, and get them caught up on their screens, figure out
- 10:52their medicines. And sometimes I've got patients that are on medicines that don't agree
- 10:58together and adjust things and figure out more than anything, figure out what that
- 11:05patient's goals are. You know, you said meet your patients where they are.
- 11:11I know a fellow who hadn't been to a doctor for twenty years until
- 11:16he visited you, and he raves about how fast you caught up and how
- 11:24fast everything came into play simply because there was communication and properties and knowing we're
- 11:30a patient, what their goals are, how they plan on living, what
- 11:33their goals are. You know, everyone wants a healthy life, but that's
- 11:39not realistic and setting the realistic expectations. Also, it's a little known fact
- 11:46doctors are not reimbursed for reading past medical records. In the office, and
- 11:52there are patients who come with hundreds of pages worth of past medical records and
- 11:58that's where doctor just find their discrepancies, but they it takes hours to go
- 12:03through them. And my theory on that if it's important enough to hit my
- 12:07office, this is important enough for me to read and understand and see what's
- 12:13been done. And a lot of the costs of a patient are cut down
- 12:18in that step right there, because patients don't remember if they've had a test,
- 12:24but those records will indicate yay or name that is correct and usually the
- 12:28result yes, that is correct, and in understanding that and being able to
- 12:33document that is a big deal. Also through traditional healthcare, Medicare and Medicaid,
- 12:39there's something called medical necessity, which is a big nichiance. From time
- 12:45to time you have to prove as a physician why that patient needs that test,
- 12:50and a lot of times there's a lot of hoops to jump through.
- 12:54If I have old medical records that I am familiar with, sometimes I can
- 13:00buying medical necessity in those now. Sometimes you're going to need health care insurance
- 13:07with direct primary care sometimes tell us about them. Sometimes we sometimes are on
- 13:13a catastrophic events, things that you don't anticipate surgeries, heart attacks, cancers.
- 13:20These are devastating to patients in their financial wellbeing because it's easy to run
- 13:28up tens of thousands of dollars worth of medical bills. Also, if you've
- 13:35got cancer, some of those drugs are exorbitantly expensive, and so a high
- 13:41deductible health care insurance plan is what we recommend to go with this because in
- 13:48direct primary care we can catch most of these many of the health care needs
- 13:54ahead of time and possibly ward things off or or catch them early before they
- 14:01are a catastrophic event. Very good. How do we become a patient of
