- 0:00Hi, everyone. This is Arianna Portallatin with the
- 0:02Becker's Dental and DSO review podcast. I'm thrilled
- 0:05to be joined today by doctor Peraz Edelat,
- 0:07the CEO of Clark Street Dental Partners. Doctor
- 0:09Edelat, thank you so much for being here
- 0:11today. It's great to have you. Thank you
- 0:12so much for having me. I'm excited to
- 0:14do this.
- 0:15Of course. Yes. To start us off, can
- 0:17you introduce yourself for our listeners and tell
- 0:19us a little bit about your background as
- 0:20well?
- 0:21Sure. My name is Faraz. I'm an orthodontist
- 0:24in, Southern California.
- 0:26I went to, USC for dental school, so,
- 0:29go Trojans and, UOP
- 0:31up north for my ortho residency.
- 0:35About five years ago, we started
- 0:37our company with our first practice,
- 0:40and
- 0:41now we've grown to six, soon seven practices.
- 0:45And,
- 0:46you know,
- 0:47during COVID, we sort of saw an opportunity
- 0:50where,
- 0:51you know, people were slowing down or wanting
- 0:53to get out. So, you know, we took
- 0:56it, and
- 0:57we're excited to be here.
- 0:59Yeah. Thank you so much for that introduction.
- 1:01Looking forward to getting into our conversation today.
- 1:04My first question for you here is, what
- 1:05are some of the biggest issues that you're
- 1:07following in the dental industry this year?
- 1:09Of course.
- 1:11I mean, I think one of the biggest
- 1:12things right now is,
- 1:14increases in
- 1:16not only
- 1:17sort of supplies, but also
- 1:20just on the margins in terms of, marketing
- 1:22dollars. You know, the cost per click has
- 1:25gone up, and
- 1:26the cost of, patient acquisition has gone up.
- 1:30So there is a, you know, sort of,
- 1:33a nibbling on the on the margins of
- 1:36the you know, from the top line.
- 1:39But also in terms of,
- 1:41there's a few legislative issues and some, regulatory
- 1:45things that are coming down the pike, especially,
- 1:47where we practice in California that, you know,
- 1:50is gonna throw a wrench into some things,
- 1:51but
- 1:53isn't anything that, you know, we're not prepared
- 1:55for, and we,
- 1:57haven't been putting our ducks in a row
- 1:59to handle.
- 2:00Yeah. Yeah. Definitely. I'd love to to get
- 2:03a little bit more detail on the legislative,
- 2:05proposals that you're following as well. Can you
- 2:06give me some more insight into what you're
- 2:08keeping track of there? Yeah. Of course. So,
- 2:10July 1, they're
- 2:12slated to make some changes to Dentical, which
- 2:15is,
- 2:16you know, the the, government subsidized
- 2:21air
- 2:22that the residents of California can receive.
- 2:25They're cutting
- 2:26a few benefits from adults,
- 2:28some to kids. It's it's essentially
- 2:31a
- 2:33a clawback
- 2:34of proposition 56,
- 2:36which was the, tobacco
- 2:38tax that was passed,
- 2:40I believe, in 2014. So,
- 2:42you know, Dennis, she was, seeing
- 2:45a bit of that money go to them.
- 2:47But I think as less people smoke,
- 2:49there's less money. Well, so I guess this
- 2:51is a good thing. Mhmm. But,
- 2:54but,
- 2:55again, these are things where if,
- 2:57if we can apply a little bit of
- 2:59foresight
- 3:00and prepare for,
- 3:02us as operators shouldn't see a huge
- 3:05drop in revenue or, services offered to patients.
- 3:09Okay. Yeah. I I know you mentioned also
- 3:12not putting all your ducks in a row.
- 3:13Can you tell me a little bit more
- 3:14about what that looks like at Clark Street?
- 3:16How are you adapting?
- 3:18What Of course. Any changes you're making? Anything
- 3:20that you need to keep the same?
- 3:23Yeah. So,
- 3:24I think payer mix is huge. I think,
- 3:27from day one, I've had really, really great
- 3:30mentors who,
- 3:31have told me, you know, don't put your
- 3:33eggs in one basket. So you don't wanna
- 3:35have too much exposure to one payer versus
- 3:38another. And, you know, this is true for
- 3:39PPOs either. You know, PPOs are
- 3:41sort of the holy grail in
- 3:44in, dentistry. We get excited when we see
- 3:46our PPO patients, but, you know, you don't
- 3:48wanna overexpose yourself theirs. And same thing goes
- 3:51for, you know, government subsidized patients.
- 3:54So we've done as good of a job
- 3:56as we could have done in terms of,
- 3:58you know, balancing out our,
- 4:00payer mixes
- 4:01and,
- 4:03and offering our services to as many people
- 4:05as we can so that there isn't
- 4:07any one
- 4:09payer type that
- 4:11you know, should they go away, we
- 4:13we see,
- 4:14a huge drop in revenue. But the other
- 4:17thing is thinking outside the box and offering
- 4:19maybe cash plans to patients or in house
- 4:22plans to patients where,
- 4:24you know, they they can become part of
- 4:26our membership group, and
- 4:28they don't they don't lose out on any
- 4:30of the benefits that their
- 4:33insurance was providing them prior to, you know,
- 4:36July 1.
- 4:38Alright. Yeah. Well, very interesting. Thank you so
- 4:39much for sharing a bit more about that.
- 4:42I'm also curious to know, what are you
- 4:43most excited about when it comes to dentistry
- 4:45right now, and what is making you nervous
- 4:47about the industry as well?
- 4:49Yeah.
- 4:50So I think the nervous probably just went
- 4:51over. Right? So, you know, they're cutting huge,
- 4:54or or some of the portions of our
- 4:56revenue.
- 4:57But, you know, I think you know, like
- 4:59any business, I don't think dentistry is immune
- 5:01from AI and
- 5:04the things that it can bring and how
- 5:05it can shape not just, you know, back
- 5:08office and actually a clinical
- 5:11delivery of dentistry chairside, because we've seen that
- 5:13with,
- 5:14OverJED
- 5:15and different AI tools that we have where
- 5:17it helps,
- 5:18with diagnosis and sort of,
- 5:21hunkering down and making sure that the diagnosis
- 5:24is, a, accurate,
- 5:26b, correct for what the patient needs, and
- 5:29sort of and and fits our
- 5:31rubric of how we wanna treat our patients,
- 5:34but also the front office.
- 5:36And,
- 5:38at the intake or the call center level,
- 5:40I've seen
- 5:41really, really cool,
- 5:44tools in terms of
- 5:46AI. The company we just partnered with called
- 5:48Flossy,
- 5:49where
- 5:50they're able to monitor our, call center agents
- 5:54and help us train them on
- 5:57different aspects of what makes a call
- 6:00successful. You know, and a successful call is
- 6:02a call that leads to a patient who
- 6:03shows up. So they're able to
- 6:06gauge for
- 6:07empathy at the AI level where
- 6:10they've gone through
- 6:12so many phone calls and, you know, they've
- 6:14they've ran enough
- 6:16verbiage by an algorithm that they know what
- 6:18can get a call center agent in or
- 6:21a a patient in from a call center
- 6:22agent.
- 6:23And we've been able to use this to
- 6:25sit, you know, side by side and have
- 6:27one on ones with our call center agents.
- 6:29Hey.
- 6:31You know, Liana,
- 6:32you did great on this call, and this
- 6:34is why this patient came in. So good,
- 6:36you know, good on you. Keep it keep
- 6:38it up. Or,
- 6:39hey, call center agent. You know, let's work
- 6:41on using this phraseology to make sure that
- 6:44we convert these phone calls and we bring
- 6:46the patients in.
- 6:48Yeah. Super interesting. Thank you. Yeah. Really cool.
- 6:50I yeah. Yeah. And I'd love to talk
- 6:53more specifically about DSOs as well. In your
- 6:56opinion,
- 6:57you know, your predictions for this year, what
- 6:59will the biggest challenges for DSOs be, in
- 7:022026?
- 7:03I think the biggest biggest thing that we'll
- 7:05see from,
- 7:07DSOs and, you know, I think we're sort
- 7:09of
- 7:10hopefully, getting on the other side of that
- 7:12is is interest rates. Right? You know, as,
- 7:14as the DSOs, we're only
- 7:17you know, our value comes from our ability
- 7:18to grow and scale. So
- 7:22to be able to acquire debt from a
- 7:24lender,
- 7:26if your interest rates are too high, then
- 7:27it might not debt service. So you so
- 7:29it does hinder growth a little bit.
- 7:33Another thing that becomes important
- 7:35for DSOs and DSO operators is having a
- 7:37reserve.
- 7:38We've been blessed enough where we're actually able
- 7:41to do a few of our deals with
- 7:42cash. So we've
- 7:44been able to sort of navigate this minefield
- 7:46of
- 7:48of the current debt environment.
- 7:50And, you know, we have our fingers crossed
- 7:52and, you know,
- 7:53think that it's gonna get better. So maybe
- 7:55we can go back to a little bit
- 7:56more of a traditional lending model outside of
- 7:58just, you know, using our own cash to
- 8:00grow.
- 8:01Yeah. It it's interesting you say that too.
- 8:03I think I've heard that from a few
- 8:04other DSO leaders, that they are optimistic, but
- 8:08maybe more so, like, cautiously optimistic just because
- 8:10of what the debt environment and inflation kinda
- 8:13looks like right now and some of the
- 8:14other economic challenges.
- 8:16I know
- 8:17the
- 8:17ADA recently released its economic report for the
- 8:20fourth quarter, and it kinda shows that,
- 8:23when you're looking ahead at the next six
- 8:25months or so, dentist economic confidence is kind
- 8:28of declining a little bit more.
- 8:31Are you remaining cautiously optimistic, or are you
- 8:33more optimistic than maybe some of the other
- 8:35DSO executives are?
- 8:37I think so. I think,
- 8:39inflation is slowly getting under control.
- 8:42I think,
- 8:44you know, the Fed shoots for a 2%
- 8:46rate, but I think, you know, given what's
- 8:49transpired in the past,
- 8:51I guess, six years now,
- 8:532% might might be a tough goal to
- 8:56reach. But, you know, if you can be
- 8:58somewhere between 22.7%
- 9:01of inflation,
- 9:02then,
- 9:03lenders feel better about lending.
- 9:06And then
- 9:07depending on your relationship with the banks as
- 9:09well, it shouldn't be an impediment to growth.
- 9:13Alright.
- 9:14And last question for you here. What will
- 9:15the most effective dental leaders need to be
- 9:18successful in the next two to three years?
- 9:21I think foresight is very important. I think
- 9:23as I grow in this industry and as
- 9:26I sort of, you know, take my slings
- 9:28and arrows, I realized that,
- 9:31you know, you gotta keep your eye on
- 9:32the ball as to what's currently happening in
- 9:35your business and in the industry. But what's
- 9:37coming down the pike is also very, very
- 9:39important. And not just the bad things, not
- 9:40just, you know,
- 9:42cuts from the insurance, but also the good
- 9:43things that are, you know, coming down the
- 9:45pike in terms of,
- 9:47you know, job reports being high. So there's,
- 9:50you know, higher higher employment in in certain
- 9:52areas, so more people have access to insurance.
- 9:55So,
- 9:56they can patronize our practices.
- 9:58Or, like we were just talking about, AI
- 10:00and how to help streamline and bring efficiencies
- 10:03to a DSO where,
- 10:05previously,
- 10:06you know, it it would take three to
- 10:08four people to process a claim.
- 10:11Now, you know, as long as you have
- 10:13one solid person overseeing it, a lot of
- 10:15this can happen,
- 10:17you know, digitally through AI and through AI
- 10:20tools.
- 10:21Alright. Well, those are the questions I have
- 10:23for you today, doctor Edelab. Thank you so
- 10:25much for joining us. It's been a pleasure
- 10:26speaking with you on our podcast, and I'm
- 10:28looking forward to connecting with you again in
- 10:29the future. Of course. Thank you so much.
