- 0:00SPEAKER_00: Lowering eye pressure, the Lumigan versus Roklanda Showdown.
- 0:04SPEAKER_00: Open angle glaucoma and ocular hypertension are conditions where intraocular pressure is higher than normal, threatening vision.
- 0:11SPEAKER_00: Eye drops that reduce IOP are the first line defense.
- 0:15SPEAKER_00: In this showdown, we compare Lumigan, brand of bimatoprost, and rockland, called rocklatan in the U.S., a fixed-dose combo of latanoprost plus netarsudal, explaining how each works, how well it lowers pressure, side effects, dosing, cost, and which patients might benefit most.
- 0:32SPEAKER_00: Mechanism of Action.
- 0:33SPEAKER_00: The two drops work differently.
- 0:35SPEAKER_00: Lumigan bimatoprost is a prostaglandin analog.
- 0:39SPEAKER_00: Prostaglandin analogs act mainly by increasing the drainage of fluid out of the eye, especially via the uvioscleral outflow pathway.
- 0:46SPEAKER_00: Bimatoprost is converted in the eye to a form that boosts outflow of aqueous humor, reducing pressure.
- 0:53SPEAKER_00: For example, laboratory studies show bimatoprost lowers resistance in the trabecular meshwork by about 26%.
- 1:00SPEAKER_00: Clinically, bimatoprost and other prostaglandins like latanopost share this main effect.
- 1:05SPEAKER_00: More fluid drain leads to lower IOP.
- 1:08SPEAKER_00: Roklandda, latanoprost plus netarseutal, combines two actions.
- 1:12SPEAKER_00: The latanoprost part is another prostaglandin, working like bimatoprost to improve outflow.
- 1:17SPEAKER_00: On top of that, netarseudal is a rokinase rock inhibitor, a different class.
- 1:23SPEAKER_00: Nitarseudal's unique action is to relax the eye's conventional drainage meshwork, trabecular meshwork, and lower the pressure needed to push fluid out.
- 1:32SPEAKER_00: It also has secondary effects.
- 1:35SPEAKER_00: It can reduce pressure in the episcleral veins and even slightly reduce fluid production.
- 1:40SPEAKER_00: In short, while both drugs share the prostaglandin mechanism, increasing uvioscleral drainage, netarsutal adds a tug at the trabecular outflow route and venous side of things.
- 1:51SPEAKER_00: Efficacy.
- 1:52SPEAKER_00: Clinical trials show both lumagan and the netarseudal latanoprost combo can dramatically lower IOP, but the combo generally drops pressure further than latanoprost alone, and thus more than a single prostaglandin alone.
- 2:05SPEAKER_00: Lumigan monotherapy typically reduces IOP by a large percentage.
- 2:10SPEAKER_00: In trials of bimatoprost 0.03%, average drops of about 6-8 mmH, roughly 25 to 35% from baseline were seen.
- 2:18SPEAKER_00: For example, one analysis noted an average reduction around 7.7 mmHg.
- 2:24SPEAKER_00: This means from a starting IOP in the mid-20s, many patients on lumagin drop into the high teens.
- 2:29SPEAKER_00: In fact, bimatoprost has consistently shown strong pressure lowering.
- 2:33SPEAKER_00: Multiple studies and meta-analyses found it often lowers IOP slightly more than other prostaglandins like latanoprost.
- 2:41SPEAKER_00: For Roklanda rochlatin, netarseudal plus latanoprost, phase 3 trials, the Mercury studies, found even bigger drops compared to each drug alone.
- 2:50SPEAKER_00: In those trials, adding netarseudal to latanoprost produced an extra 1.5 to 3 mmherazerin reduction in IOP beyond latanoprost alone.
- 2:59SPEAKER_00: In concrete terms, patients on the combination averaged IOPs around 15-16 mmG at follow-up, compared to about 17 to 18 mmHz on latanoprost alone.
- 3:10SPEAKER_00: Notably, at three months, one study reported 42% of patients on the combo hit a mean pressure 15 mmg, while only about 16 to 18% did so on latanoprost or netarseudal alone.
- 3:22SPEAKER_00: In pooled analyses, Roklanda patients had mean daytime IOP 15 to 16 mmHzg versus 17 to 18 mmHzgram on latanoprost alone.
- 3:32SPEAKER_00: There are no direct head-to-head trials comparing lumagan versus the netarsutal-latanoprost combo.
- 3:37SPEAKER_00: The recent Mercury 3 trial compared the combo to bimatoprost timolol rather than bimatoprost alone.
- 3:43SPEAKER_00: Thus we rely on separate data.
- 3:45SPEAKER_00: Prostaglandin drops like lumigan give a strong single-agent effect, while the fixed combo can add on extra pressure reduction for those needing it.
- 3:53SPEAKER_00: The combined drug is designed for patients not at target on a prostaglandin alone.
- 3:59SPEAKER_00: Side effects and tolerability.
- 4:01SPEAKER_00: Side effects of these drops are mostly well-known ocular reactions.
- 4:06SPEAKER_00: Prostaglandin analogues, Lumigan's bimatoprost and Roclanda's Latanoprost, commonly cause mild eye redness, conjunctival hyperemia, longer or thicker eyelashes, and gradual darkening of the iris color and surrounding skin.
- 4:20SPEAKER_00: These changes come from the drug's pharmacology.
- 4:23SPEAKER_00: For example, Lumigan's prescribing information notes pigmentation changes, iris, eyelid lashes, and eyelash growth.
- 4:30SPEAKER_00: In practice, redness on Lumigan 0.01% affects about 30% of patients.
- 4:36SPEAKER_00: A higher 0.03% dose had more.
- 4:40SPEAKER_00: Eyelash growth and iris darkening are not dangerous and can even be cosmetically welcome.
- 4:45SPEAKER_00: They gave rise to Latisse eyelash formulation.
- 4:48SPEAKER_00: However, the pigmentation changes tend to be permanent, and some patients dislike puffiness or deepening of the eye folds, sunken eyes seen with prostaglandins.
- 4:57SPEAKER_00: Overall, prostaglandin side effects are usually mild to moderate, but they lead many patients to notice their eyes look a bit different.
- 5:05SPEAKER_00: Nedarseudal, by contrast, has a distinctive side effect profile.
- 5:10SPEAKER_00: The most common issue is also redness, in fact, even higher than with prostaglandins.
- 5:15SPEAKER_00: Clinical data report redness in over 50% of netarseutal-treated eyes.
- 5:20SPEAKER_00: Nedarseudal can also cause conjunctival hemorrhages, tiny red blood spots on the white of the eye, and corneal verticillata, whirl-like deposits on the cornea, in up to 10 to 20% of patients.
- 5:33SPEAKER_00: For example, Repress's label notes Bosh 53% hyperemia and about 20% cornea deposits or hemorrhages.
- 5:40SPEAKER_00: In rock latin trials, about 59% of combo patients had some hyperemia, and 11% had conjunctival bleeding and 15% had corneal deposits.
- 5:49SPEAKER_00: Latanoprost's own side effects, modest pigment lash changes, can add to this but are relatively minor compared to the big difference in redness between netarseudal versus prostaglandin.
- 6:00SPEAKER_00: In summary, Lumigan's typical adverse effects, eye redness, hyperemia, very common, 31% on 0.01%, eyelash and eyelid changes, growth darkening, and possible eyelid fat loss, iris color darkening, permanent.
- 6:14SPEAKER_00: Roclanda's common adverse effects, netarseudalatinoprost, hyperemia redness, very common, subconjunctival hemorrhage, small at 11%, corneal verticillata deposits, choice 15%, eyelash or pigmentation changes from the latinoprost, generally milder than with bimatoprost.
- 6:36SPEAKER_00: These side effects affect tolerability.
- 6:39SPEAKER_00: In practice, up to 40 to 45% of patients on a prostaglandin drop report some adverse event, and redness is often the main complaint.
- 6:47SPEAKER_00: The higher redness rate with netarsidal combinations can be uncomfortable.
- 6:51SPEAKER_00: Many patients describe mild stinging or a bloodshot eye that typically fades over time.
- 6:57SPEAKER_00: Such effects do influence real-world use.
- 7:00SPEAKER_00: Patients bothered by redness or irritation may skip doses or switch drugs.
- 7:05SPEAKER_00: Interestingly, in the netarcidal vs.
- 7:07SPEAKER_00: Lahanopros studies, some reported the red eye of netarcidal tended to decrease after a month of nightly dosing.
- 7:14SPEAKER_00: Both drugs are usually given at bedtime in part to minimize noticing these effects during waking hours.
- 7:20SPEAKER_00: Dosing and convenience.
- 7:22SPEAKER_00: Both lumagin and rockland are simple once-daily drops, usually one drop in the affected eyes each evening.
- 7:28SPEAKER_00: Lumagumin 0.01 to 03% is instilled nightly, as is rocklatatin.
- 7:34SPEAKER_00: Netarsudal 0.2%, latanoprost 0.05%.
- 7:39SPEAKER_00: So neither requires multiple doses per day.
- 7:41SPEAKER_00: A convenience point.
- 7:42SPEAKER_00: Roklanda provides two active ingredients in one bottle.
- 7:45SPEAKER_00: If a patient needs both a prostaglandin and netarsudal, using the fixed combo means only one dropper instead of two separate bottles.
- 7:53SPEAKER_00: For example, someone already on Latanoprost who adds netarsudal could use rocklaton and simplify their regimen.
- 8:01SPEAKER_00: By contrast, if a patient on lumagin needs extra therapy, they would typically add a second bottle, another drug.
- 8:08SPEAKER_00: The single bottle combination can improve adherence for some patients by reducing the number of separate medications to juggle.
- 8:16SPEAKER_00: Cost, availability, and approval.
- 8:18SPEAKER_00: Lumagan, Bimatoprost, is a well-established medication.
- 8:22SPEAKER_00: In the U.S., it was originally approved in 2001 as.03% and later.01% and has parent patent expiration spun off generics.
- 8:32SPEAKER_00: Generic versions of Bimatoprost.pyro1% were approved around 2025.
- 8:37SPEAKER_00: In practice, generic Bimatoprost drops are now widely available, making this therapy relatively inexpensive with insurance or discount programs.
- 8:45SPEAKER_00: By contrast, the brand Lumagan Without Insurance can cost hundreds of dollars for a bottle, though coupons often reduce this.
- 8:52SPEAKER_00: Roklanda, Latanoprost Natarsudl is newer.
- 8:56SPEAKER_00: The European Medicines Agency, EMA, gave approval in January 2021.
- 9:01SPEAKER_00: It is marketed in the U.S.
- 9:02SPEAKER_00: as Rock Latin, FDA approval 2019.
- 9:05SPEAKER_00: As a brand name combination, it is pricier.
- 9:08SPEAKER_00: There are currently no generic versions of the Natarsudal Latanoprost combo.
- 9:12SPEAKER_00: Natarsudal itself is proprietary.
- 9:15SPEAKER_00: Without insurance, Rocklatan's cost is several hundred dollars for a month's supply.
- 9:20SPEAKER_00: Many insurance plans require patients to try simpler therapy first, for example, requiring failure of a prostaglandin drop before covering the combo.
- 9:29SPEAKER_00: Regional differences apply.
- 9:30SPEAKER_00: In Europe, Rockland would be covered by national health systems similarly to other secondline glaucoma meds.
- 9:37SPEAKER_00: Ideal patient profiles lumigan, bimataprost monotherapy, is often used as a first-choice eye drop for open-angle glaucoma or ocular hypertension because it's very effective and simple.
- 9:50SPEAKER_00: It suits a patient whose target IOP is modestly low and who tolerates prostaglandin side effects.
- 9:56SPEAKER_00: Many patients starting glaucoma therapy will see significant IOP reduction on lumagan alone.
- 10:03SPEAKER_00: It's also useful for long-term maintenance if the drop controls pressure without intolerable side effects.
- 10:09SPEAKER_00: The 0.01% formulation was specifically introduced to improve tolerability.
- 10:14SPEAKER_00: Studies showed it maintains similar efficacy with less redness, improving adherence.
- 10:20SPEAKER_00: Roklanda rockladen is typically used for patients who need more lowering than a prostaglandin alone can provide.
- 10:27SPEAKER_00: For example, someone already on letanoprost or bimatoprost, whose pressure remains above target, or someone starting therapy at a very high IOP, might benefit from the extra kick of adding netarsudal.
- 10:39SPEAKER_00: The combination is specifically indicated for patients in whom monotherapy, either a prostaglandin or netarseutal, does not sufficiently reduce IOP.
- 10:48SPEAKER_00: In practice, Roklanda might be favored for patients who need an extra 2 to 3 mm drop or a higher chance to reach mid-teens intraocular pressure, and for those who can accept a higher chance of redness.
- 11:01SPEAKER_00: In summary, both drugs are authoritative glaucoma therapies, but fit slightly different needs.
- 11:07SPEAKER_00: Lumigan is a proven first-line prostaglandin drop, simple, once daily, with well-known side effects.
- 11:14SPEAKER_00: Roklanda is a newer combination option for more aggressive lowering or as a next step if a prostaglandin alone isn't enough.
- 11:21SPEAKER_00: Deciding between them depends on how low the eye pressure must go, previous response to medications, side effect sensitivity, and cost insurance factors.
