- 0:53payer issues roundtable.
- 0:54I'm currently joined by Krisha Winwright, who is
- 0:57the president of Medicare Advantage at Blue Cross
- 1:00Blue Shield of Michigan. So, Krisha, thanks for
- 1:02being here. We'd love to have you start
- 1:04off by introducing yourself and explaining a little
- 1:06bit more about your role. Thanks for having
- 1:08me, Grace. It's so good to be here
- 1:09and all the excitement. It's,
- 1:11energetic just to walk through this this event.
- 1:14Thanks for having me. As you mentioned, I'm
- 1:16the president of Medicare Advantage for Blue Cross
- 1:19Blue Shield of Michigan, and we are the
- 1:217th largest payer in the nation for Medicare
- 1:24Advantage.
- 1:25And we cover Michigan, several states that we
- 1:27have partnerships with other Blues plans to really
- 1:30drive performance in Medicare Advantage. So that's my
- 1:33that's my role within the Blue System. I've
- 1:35been in health care for
- 1:37many, many years working in a variety of
- 1:39roles across the health care continuum,
- 1:42and this is a really great spot to
- 1:44see the action in Medicare Advantage these days.
- 1:47So glad to be here.
- 1:49Wonderful. Well, thank you for taking the time.
- 1:51And let's start our conversation today around growth
- 1:54goals. So from improving member experience
- 1:57and expanding value based care to controlling costs,
- 2:00payer executives do have ambitious growth goals for
- 2:03the rest of this year as well as
- 2:04we're looking into 2025.
- 2:06So in your role, what is your top
- 2:08priority, and how are you planning to get
- 2:09there? You know, it's interesting that you mentioned
- 2:12the focus on growth. We know that the
- 2:14Medicare Advantage market is growing, and it's growing
- 2:17nationally.
- 2:19Millions of Medicare Advantage members will come into
- 2:22the pipeline in the coming decades, and
- 2:24that is,
- 2:26causing a a market reality right now in
- 2:28Medicare Advantage and a market shift overall. And
- 2:30so it's interesting when you mentioned that payers
- 2:33have high growth goals. I would probably rephrase
- 2:35that a little bit that we are facing
- 2:38growth,
- 2:39but it's also a moment when Medicare Advantage
- 2:42is going through an inflection point. And so
- 2:44what we're doing right now and what is
- 2:46top of mind is how do we create
- 2:48a sustainable
- 2:50solution for Medicare Advantage and for seniors that
- 2:53is really true to our mission, that doubles
- 2:55down on that which we care a lot
- 2:57about, which is access to excellent quality affordable
- 3:00health care for seniors and do that in
- 3:02a way that
- 3:03is taking advantage of the market context, understand
- 3:06the market context, but really being true to
- 3:08who we are. I imagine that payers nationally
- 3:10are doing that right now. It is, a
- 3:12market shift that's happening.
- 3:15Increased utilization
- 3:16coming out post pandemic is causing pressure on
- 3:20payers nationally
- 3:21that is well above anything that we would
- 3:23have imagined pre COVID nor seen.
- 3:26And at the same time, the,
- 3:28CMS program is contracting funding to cope with
- 3:31the increased surge of Medicare Advantage lives, and
- 3:34that's causing quite a moment for Medicare Advantage.
- 3:37And so as we look ahead to positive
- 3:38growth, we also wanna do that in a
- 3:40way that's sustainable,
- 3:41that we can be counted on by seniors
- 3:44to continue to serve them in the manner
- 3:45that we have for many, many years. And
- 3:47so it's a very exciting time for sure.
- 3:49Absolutely. Well, thank you for those insights. And
- 3:52kind of going off of that in the
- 3:54industry level, how would you describe the biggest
- 3:57barriers to effectively
- 3:59serving and engaging members? And what opportunities do
- 4:02you see for large scale improvements, and how
- 4:04are these being applied to your current strategies?
- 4:07It's so interesting that you asked me that
- 4:09question on today of all days, which is
- 4:11election day nationally.
- 4:13And I would,
- 4:14hearken to that when I think about seniors
- 4:17that are navigating the health care environment. It
- 4:20is an incredibly noisy environment. Much like our
- 4:23election day and leading up to it, just
- 4:25even cutting through the noise has been very
- 4:27difficult. And we know that that's happening nationally
- 4:30just as a result of this event today
- 4:32with voting day being so critically important to
- 4:35our nation today. Well, very similarly,
- 4:38it's tough to cut through the noise for
- 4:40seniors. And,
- 4:41seniors are telling us that they want more
- 4:44alignment. They want easier selection.
- 4:48They have a lot of choices to make
- 4:49in the market, and sometimes they're not fun
- 4:51to make. They're hard to navigate.
- 4:53The
- 4:54health care landscape
- 4:56continues to be complex
- 4:58due to lots of factors, but innovation is
- 5:00happening in the market. And that causes even
- 5:03more fantastic solutions for health care. It's also
- 5:06more confusing at times for seniors. And so
- 5:08one of the things that we're doing to
- 5:10get through those barriers of noise and concern
- 5:13and, confusion
- 5:15is to really get aligned with providers. We
- 5:18have fantastic
- 5:19partnerships with providers in our market.
- 5:22And, that's true of Blue Cross companies nationally.
- 5:25Health care is very, very local, and the
- 5:28partnerships that we build with providers tend to
- 5:30be critically important. They're important more than ever
- 5:33now.
- 5:34The seniors that we talk to want us
- 5:36to be on the same page with their
- 5:37provider. They don't want confusion. They don't want
- 5:39different answers. They want alignment, and that's what
- 5:41we're working very, very hard to do. We
- 5:44have enjoyed
- 5:45success in partnering with providers. Our blueprint for
- 5:48affordability program is among the leading in the
- 5:51nation where we are coming alongside
- 5:53providers in our market, partnering together to make
- 5:56things better, more affordable, sustainable
- 5:58for everyone in the market, including seniors. And
- 6:01we're so proud of that work. Over 22
- 6:03physician organizations
- 6:04have joined us in that quest.
- 6:06And, so I would say that that's probably
- 6:08the number one thing that we're doing right
- 6:09now to overcome those barriers with consumers is
- 6:12doubling down on our partnership with providers.
- 6:16Absolutely.
- 6:17And shifting our conversation
- 6:19toward digital innovation,
- 6:21what role is technology currently playing in your
- 6:24organization's growth strategies?
- 6:26And can you share any specific examples with
- 6:28us? Sure can. The Medicare Advantage market
- 6:32ends up serving members with a lot of
- 6:34complex chronic disease, and that's the cause of,
- 6:38as we mentioned, not only,
- 6:40a lot of care being delivered in that
- 6:41market, confusion, but also,
- 6:44is fraught with
- 6:47opportunities and challenges. Technology
- 6:49helps us get precise in how we work
- 6:52with seniors.
- 6:53It also helps us get more precise in
- 6:56how we work with providers so that we're
- 6:58working in the context of someone's illness burden
- 7:01and their unique situation.
- 7:03It's not just about the the clinical challenges
- 7:05that you have. It's about your social
- 7:08environment, your context in which you journey through
- 7:10that. And health care is so much more
- 7:12than what we've given it, credit for in
- 7:15the past, and we wanna do a really
- 7:16good job and get more precise as we're
- 7:18journeying with seniors in context.
- 7:21Technology,
- 7:22especially
- 7:23artificial intelligence, generative AI, new solutions that are
- 7:26hitting the market are table stakes to be
- 7:27able to provide that type of care precision
- 7:30and, be able to be there for members
- 7:33in new ways and in new more meaningful
- 7:35ways that are tailored specifically to where they're
- 7:38at, meeting them where they're at. As it
- 7:40turns out, the CMS program for Medicare Advantage
- 7:44also rewards our doing that on a member
- 7:46by member basis. It's very unique
- 7:48in that way. And so it's a really
- 7:50good fit. Technology innovation and contemporary technology that's
- 7:54in market right now allows you to do
- 7:57exactly what we need to do right now,
- 7:58which is get more precise for members, align
- 8:00better with the CMS Medicare Advantage Program, and
- 8:03that's what we're doing.
- 8:05And how is technology supporting value based care
- 8:08or member focused initiatives at your organization, and
- 8:11what is most exciting to you here?
- 8:14I think the most exciting thing is the
- 8:16conversations that we're able to have with providers
- 8:19to
- 8:20understand that we're really after many of the
- 8:22same things, but we have different sources of
- 8:25information.
- 8:26Providers are working at the point of care,
- 8:28and the insurer and the payer is seeing
- 8:31the longitudinal care across every
- 8:34provider and facility that that member might be
- 8:36seeing. They they see all of the,
- 8:39pharmaceuticals, the drugs that they might they might
- 8:41be on and take. And we also see
- 8:43the benefits
- 8:44in the environment that the senior is working
- 8:47in with their products. And so those perspectives
- 8:49are very helpful when they're working together. And
- 8:52so the most exciting work that we're doing
- 8:54right now is in partnership again with providers
- 8:57to see the whole person
- 8:58and to be able to participate in that
- 9:01positive conversation
- 9:02with providers to make sure that we're moving
- 9:04the ball down the field and that we're
- 9:06getting where the senior wants to go and
- 9:08helping them live their very best life.
- 9:11And shifting toward quality, in an ever
- 9:14evolving regulatory environment, what tools or key strategies
- 9:18have helped your organization improve quality and clinical
- 9:21outcomes for members while staying compliant?
- 9:25Embrace the reality
- 9:27of the program
- 9:29and
- 9:30look at the intent
- 9:32of the regulatory
- 9:33guidance
- 9:34and still stay true to the mission that
- 9:37your organization
- 9:38is,
- 9:40attempting to implement.
- 9:41For us,
- 9:43we want to be senior centric, and we
- 9:45will always be senior centric. We're gonna be
- 9:47local. We're gonna be, pro provider. Those things
- 9:50are unchanged. They work very well in their
- 9:52changing regulatory environment. But there are challenges.
- 9:56Changing regulatory makes you more, need to be
- 9:58more nimble. It makes you need to,
- 10:02operate in a way that's very responsive. That's
- 10:04been hard for health care organizations to do,
- 10:08and we're continuing to chase that as well.
- 10:10How do you get to be a more
- 10:12responsive, nimble organization? We're working really hard at
- 10:15that, making some fantastic strides in doing that.
- 10:17And so I would say it's those few
- 10:19handful of things. It's making sure that you're
- 10:21embracing the intent, you are committed to be
- 10:23compliant, but you're also on mission. And that
- 10:26drives you to do incredibly inspiring things on
- 10:29behalf of the members that you serve.
- 10:32Absolutely. Well, Krisha, before we wrap up here,
- 10:34are there any parting words you'd like to
- 10:36share on the podcast today?
- 10:38I'm just, inspired to be a part of
- 10:41the conversation right now. The market in health
- 10:43care is very, very,
- 10:46challenging. But with that challenge also comes opportunity.
- 10:49And, Blue Cross Blue Shield of Michigan is
- 10:52a part of that right now, and we
- 10:54couldn't be more thrilled
- 10:56to be, creating solutions for members that we
- 10:59believe are gonna last for a very, very
- 11:01long time. And so I would just leave
- 11:03the the,
- 11:04the listeners with that. Be true to your
- 11:06mission, double down on it, and be excited
- 11:09about the impact that we can have in
- 11:10this moment.
- 11:12Wonderful. Well, thank you so much for taking
- 11:14the time to be here today and join
- 11:15me on the Becker's Healthcare podcast. Again, we
- 11:17are live at the 2024 payer issues roundtable.
