- 0:00Speaker 1: What's up, everyone, and welcome to another episode of the
- 0:03Speaker 1: Epstein Chronicles. In this episode, we're going to take a
- 0:06Speaker 1: look at the report that was put together by the
- 0:09Speaker 1: psychiatrist and their staff about Jeffrey Epstein and the events
- 0:15Speaker 1: leading up to his death. Background information Jeffrey Epstein was
- 0:19Speaker 1: sixty six years old, white mel who died on August tenth,
- 0:22Speaker 1: twenty nineteen, while housed at the Metropolitan Correctional Complex in
- 0:27Speaker 1: New York, New York. Redacted. Former acting Assistant Director re
- 0:31Speaker 1: Entry Services Division appointed a team to conduct a psychological reconstruction.
- 0:37Speaker 1: The team consisted of redacted National Suicide Prevention Coordinator, Central Office,
- 0:42Speaker 1: redacted Sex Offender Treatment Program Coordinator, Central Office, redacted Mental
- 0:47Speaker 1: Health Treatment Coordinator, Central Office, and redacted Correctional Services Administrator
- 0:53Speaker 1: Northeast Regional Office. The reconstruction was established in accordance with
- 0:58Speaker 1: Bureau of Prisons Program Statement five three to four zero
- 1:02Speaker 1: eight Suicide Prevention Program. Formal interviews were not conducted as
- 1:07Speaker 1: part of this reconstruction at the direction of the Department
- 1:09Speaker 1: of Justice. A copy of the video is normally made
- 1:13Speaker 1: following a significant incident, but there was no such video
- 1:16Speaker 1: in this case, since the original video was confiscated by
- 1:19Speaker 1: the Federal Bureau of Investigation prior to the beginning of
- 1:23Speaker 1: this reconstruction. The absence of these two areas of inquiry
- 1:27Speaker 1: severely limited the ability to establish accurate timelines, confirm subjective reports,
- 1:33Speaker 1: establish converging and diverging lines of facts, or discover new
- 1:37Speaker 1: areas of inquiry. As a result, the information typically gathered, reviewed,
- 1:42Speaker 1: and consolidated during a reconstruction to support actionable findings and
- 1:46Speaker 1: recommendations is limited. Social history mister Epstein did not have
- 1:53Speaker 1: a pre sentenced report available at the time of reconstruction. Therefore,
- 1:58Speaker 1: no official information regarding social history was accessible. The following
- 2:03Speaker 1: was gathered from publicly available documents. Mister Epstein was born
- 2:07Speaker 1: in nineteen fifty three and grew up in a middle
- 2:10Speaker 1: class family in the neighborhood of Seagate on Coney Island
- 2:14Speaker 1: with one brother. After early promotion in two grades, mister
- 2:18Speaker 1: Epstein graduated from Lafayette High School in nineteen sixty nine
- 2:22Speaker 1: at the age of sixteen. He attended Cooper Union and
- 2:25Speaker 1: New York University, but did not graduate from either. Mister
- 2:29Speaker 1: Epstein taught at the Dalton School, a private school on
- 2:32Speaker 1: the upper East side of Manhattan from September nineteen seventy
- 2:35Speaker 1: four until he was dismissed in June of nineteen seventy
- 2:39Speaker 1: six for inadequate development as a teacher. Following that, he
- 2:44Speaker 1: held a number of positions in the financial industry, to
- 2:47Speaker 1: include a position as a limited partner at Barr Stearns
- 2:51Speaker 1: until he was dismissed for unknown policy violations in nineteen
- 2:55Speaker 1: eighty one. He also worked as a financial consultant and
- 2:59Speaker 1: found it out at least two separate companies. Mister Epstein
- 3:03Speaker 1: had two significant periods of employment. The rest of these
- 3:06Speaker 1: was his position as a consultant with Stephen Jude Hoffenberg
- 3:10Speaker 1: in the late eighties. Mister Hoffenberg was described as his
- 3:14Speaker 1: first mentor. Mister Hoffenberg was later convicted and incarcerated of
- 3:18Speaker 1: running a large Ponzi scheme. He implicated mister Epstein in
- 3:23Speaker 1: fraudently diverting company funds for his own personal use. Years later,
- 3:28Speaker 1: Leslie Wexner, mister Epstein's sole client at Jay Epstein in Company,
- 3:32Speaker 1: granted him power of attorney over his affairs. Despite also
- 3:36Speaker 1: being identified as mister Wexner's mentee. Mister Epstein was again
- 3:40Speaker 1: accused of misappropriating funds more than forty six million dollars.
- 3:44Speaker 1: These large sums are believed to be seed money mister
- 3:47Speaker 1: Epstein used to establish his considerable fortune. These events are
- 3:52Speaker 1: indicative of mister Epstein's highly regarded intelligence and charismatic personality.
- 3:57Speaker 1: Legal history, mister Epps seen at a history of adult
- 4:01Speaker 1: criminal charges and convictions. In June two thousand and eight,
- 4:04Speaker 1: he entered into a non prosecution agreement and pleaded guilty
- 4:08Speaker 1: to one count of solicitation of prostitution and one count
- 4:12Speaker 1: of procuring a person under the age of eighteen for
- 4:15Speaker 1: prostitution in the state of Florida. He was sentenced to
- 4:19Speaker 1: thirty months eighteen months of incarceration and twelve months of probation.
- 4:24Speaker 1: He was also mandated to register as a sex offender
- 4:28Speaker 1: under the National Sex Offender Registry and Notification Act. Mister
- 4:32Speaker 1: Epstein served thirteen of his eighteen month incarceration and then
- 4:36Speaker 1: successfully completed twelve months of probation. It's unclear whether he
- 4:41Speaker 1: followed the sex offender registration guidelines in each place he
- 4:44Speaker 1: owned a residence. In regard to the pending charges, mister
- 4:48Speaker 1: Epstein was formally charged with sex trafficking conspiracy in violation
- 4:52Speaker 1: of US Code eighteen, Section three seventy one and sex
- 4:56Speaker 1: trafficking in violation of US Code eighteen, Section fifteen ninety
- 4:59Speaker 1: one V one A, B, two and two. On July second,
- 5:04Speaker 1: twenty nineteen. Specifically, he was accused of sexually exploiting and
- 5:09Speaker 1: abusing minor females over the course of several years. Charging
- 5:13Speaker 1: documents alleged mister Epstein enticed and recruited minor females to
- 5:18Speaker 1: engage in sexual activity. The minor females were reportedly compensated
- 5:22Speaker 1: with cash following the sexual encounters, and some were encouraged
- 5:26Speaker 1: to find other minor females to accompany them to mister
- 5:30Speaker 1: Epstein's residences in New York or Florida. He pleaded not
- 5:34Speaker 1: guilty to the charges and was pre trial at the
- 5:37Speaker 1: time of his death. In a thirty seven page decision
- 5:40Speaker 1: an order remanding the defendant signed by Judge Richard M.
- 5:43Speaker 1: Berman on July eighteenth, twenty nineteen. Eighteen pages were dedicated
- 5:49Speaker 1: to detailing the danger mister Epstein posed to others and
- 5:53Speaker 1: the community. The document also alleged that he was a
- 5:56Speaker 1: flight risk. As a result, mister Epstein's proposed bellpackage was
- 6:00Speaker 1: determined to be inadequate. He was denied pre trial release
- 6:05Speaker 1: and held on remand this was likely significant to mister Epstein,
- 6:09Speaker 1: as he would lose the physical and financial comfort to
- 6:12Speaker 1: which he had become accustomed. Furthermore, his ability to mount
- 6:17Speaker 1: the defense would be limited to the parameters of legal
- 6:20Speaker 1: visits and legal work that could be accomplished in jail.
- 6:24Speaker 1: These restrictions, and the abundance of witnesses, victims, and negative
- 6:28Speaker 1: public attention were likely considerable burdens on mister epstein institutional history.
- 6:35Speaker 1: On July six, twenty nineteen, mister Epstein was arrested at
- 6:38Speaker 1: Teterborough Airport in New Jersey upon his return from Paris, France.
- 6:43Speaker 1: It's unknown whether he was anticipating his arrest. He was
- 6:47Speaker 1: transported to mcc New York and keaden the sentry at
- 6:51Speaker 1: nine twenty four pm that evening. Mister Epstein was placed
- 6:54Speaker 1: in general population housing for approximately twenty two hours. On
- 6:59Speaker 1: July seventh, twenty nineteen, at approximately seven to twenty pm,
- 7:03Speaker 1: he was moved to special housing unit pending reclassification. Due
- 7:07Speaker 1: to the significant increase in media coverage and awareness of
- 7:11Speaker 1: his notoriety among the inmate population. With regard to his adjustment,
- 7:16Speaker 1: to a correctional setting. Mister Epstein received one incident report
- 7:19Speaker 1: while in BP custody for self mutilation on July twenty third,
- 7:24Speaker 1: twenty nineteen. As of August fifteenth, twenty nineteen, the incident
- 7:30Speaker 1: report had been expunged, but it's unclear why and whether
- 7:33Speaker 1: mister Epstein knew this. Also, a review of financial transactions
- 7:37Speaker 1: associated with mister Epstein's prison account revealed one of his
- 7:41Speaker 1: attorneys was depositing funds into his cellmate, inmate Rayu's commissary
- 7:45Speaker 1: account for unknown reasons. The plot dickens, Doesn't it notice
- 7:50Speaker 1: how nobody talks about that? Huh? Why the hell was
- 7:53Speaker 1: Epstein giving mister rays money? Health Care and Personality description
- 8:01Speaker 1: BOP electronic medical records indicate Epstein was diagnosed with hyper lipidemia,
- 8:06Speaker 1: sleep apnea, hypertension, constipation, pre diabetes, neurologia, and neritis unspecified.
- 8:14Speaker 1: He was prescribed the following medications docusate, sodium, milk of magnesia,
- 8:19Speaker 1: Omega III, methyl pretozone, and biscodeal whatever that is. Mister
- 8:25Speaker 1: Epstein was also prescribed insulin and the prescription required him
- 8:29Speaker 1: to go to the Institution Pharmacy for administration of this medication. However,
- 8:35Speaker 1: the dates for which it was prescribed have a notation
- 8:38Speaker 1: indicating does not indicated, Thus it does not appear insulin
- 8:42Speaker 1: was routinely medically necessary. The rest of the medications prescribed
- 8:47Speaker 1: were self carry. He also had a continuous positive airway
- 8:50Speaker 1: pressure machine, which is typically used to treat sleep apnea.
- 8:55Speaker 1: Mister Epstein was provided with his personal seapap machine on
- 8:58Speaker 1: July thirtieth to thouhgh per b e MR. In regard
- 9:04Speaker 1: to mental health history and treatment, there are no known
- 9:07Speaker 1: available records. Certainly, there were records kept on mister Epstein's
- 9:11Speaker 1: incarceration in Florida. However, the records were not available for
- 9:15Speaker 1: review as of the data this report. With regard to
- 9:19Speaker 1: Psychology Data System records in b MR, doctor Kerry redacted,
- 9:23Speaker 1: forensic psychologists at MCC's New York completed a routine intake
- 9:28Speaker 1: screening on July eighth, twenty nineteen. During the screening, mister
- 9:33Speaker 1: Epstein denied any history of mental health problems, substance abuse
- 9:37Speaker 1: and treatment. No symptoms of mental illness were observed. He
- 9:41Speaker 1: was classified as Mental Health Care Level one and was
- 9:45Speaker 1: not diagnosed with mental illness. Following a consultation with doctor redacted,
- 9:51Speaker 1: National Suicide Prevention Coordinator. On July eighth, twenty nineteen, Doctor redacted,
- 9:56Speaker 1: chief psychologist at mcc New York, determine mister Epstein should
- 10:01Speaker 1: be preemptively evaluated for suicide risk upon his return from court.
- 10:06Speaker 1: Primary consideration was given to his various risk factors for suicide,
- 10:11Speaker 1: such as his being a high profile case with media attention,
- 10:14Speaker 1: his pending sex offense charges, pre trial status, and ongoing
- 10:18Speaker 1: court proceeding. Mister Epstein returned from court on July eighth,
- 10:23Speaker 1: twenty nineteen, after normal business hours. He denied suicidal thoughts
- 10:27Speaker 1: at the time, but due to the potential for other
- 10:30Speaker 1: risk factors listed above, the on call psychologist placed Epstein
- 10:35Speaker 1: on psychological observation in one of the suicide watch cells
- 10:39Speaker 1: until he could be assessed in person by a BOP psychologist.
- 10:43Speaker 1: On July ninth, twenty nineteen, mister Epstein underwent a formal
- 10:47Speaker 1: in person suicide risk assessment with doctor Adactid. She determined that,
- 10:52Speaker 1: while on suicide watch was not warranted at the time,
- 10:55Speaker 1: mister Epstein should remain on psychological observation status out of
- 10:59Speaker 1: an abundance of caution. He was removed from psychological observation
- 11:03Speaker 1: on July tenth, twenty ten. On July twenty third, twenty nineteen,
- 11:08Speaker 1: Doctor redacted, the on call psychologist, was notified mister Epstein
- 11:13Speaker 1: had been found in his cell with a piece of
- 11:15Speaker 1: orange cloth around his neck. Reportedly, he was observed lying
- 11:19Speaker 1: in the feudal position on the floor with a noose
- 11:22Speaker 1: around his neck. Medical staff evaluated mister Epstein and found
- 11:26Speaker 1: friction marks and superficial reddening of the neck and one knee.
- 11:31Speaker 1: He was placed on suicide watch by the Operations lieutenant
- 11:34Speaker 1: at approximately one forty am, pending a formal in person
- 11:38Speaker 1: suicide risk assessment. Doctor adacted staff psychologist at mcc New
- 11:44Speaker 1: York assessed mister Epstein for risk of suicide. Later in
- 11:47Speaker 1: the morning of July twenty third, twenty nineteen. He determined
- 11:51Speaker 1: that he should remain on suicide watch. Mister Epstein denied
- 11:55Speaker 1: any knowledge of how he received marks on his neck
- 12:00Speaker 1: and initially informed staff that he thought that his cellmate
- 12:03Speaker 1: had attempted to kill him. Special Investigative Services staff opened
- 12:08Speaker 1: an investigation to assess mister Epstein's safety and collect facts
- 12:11Speaker 1: surrounding the episode. Despite the investigation, staff were unable to
- 12:16Speaker 1: determine whether he was assaulted or engaged in self directed violence.
- 12:21Speaker 1: Mister Epstein was removed from suicide Watch on July twenty fourth,
- 12:25Speaker 1: twenty nineteen, after thirty one hours and five minutes. Thereafter,
- 12:30Speaker 1: he remained in the suicide Watch cell and was placed
- 12:33Speaker 1: on psychological observation, where he remained housed until July thirtieth,
- 12:37Speaker 1: twenty nineteen. According to pds B E MR records, a
- 12:42Speaker 1: discrepancy exists regarding when he was removed from psychological observation.
- 12:47Speaker 1: His cell assignment per century indicates that he was transferred
- 12:51Speaker 1: back to the special housing unit on July twenty ninth,
- 12:53Speaker 1: twenty nineteen, whereas PDS B E MR indicates that he
- 12:59Speaker 1: was removed from psychological observation on July thirtieth, twenty nineteen,
- 13:04Speaker 1: at approximately eight fifteen am. Mister Epstein attended a court
- 13:09Speaker 1: hearing on July thirty one, twenty nineteen, and upon his return,
- 13:13Speaker 1: the United States Marshall Services provided paperwork to Receiving and
- 13:17Speaker 1: Discharge that noted suicidal tendencies. Doctor redacted was notified on
- 13:22Speaker 1: August first, twenty nineteen about the paperwork. She consulted with
- 13:27Speaker 1: doctor redacted and then met with Epstein to conduct a
- 13:30Speaker 1: suicide risk assessment. She determined suicide watch was not warranted
- 13:34Speaker 1: at the time. Epstein remained classified as a mental health
- 13:38Speaker 1: care Level one throughout his time at mcc New York.
- 13:41Speaker 1: During his contact with psychologist, Epstein routinely denied current mental
- 13:45Speaker 1: health symptoms, including suicidal ideation, and he did not exhibit
- 13:50Speaker 1: symptoms of serious mental illness. His most frequent complaint was
- 13:54Speaker 1: difficulty sleeping. He did not have access to his seapat
- 13:57Speaker 1: machine until it was reportedly provided to him on July thirtieth,
- 14:01Speaker 1: twenty nineteen. Mister Epstein also reported he was bothered by
- 14:05Speaker 1: noise in the shoe at times. He noted concerns related
- 14:09Speaker 1: to his safety in the shoe or in general population
- 14:12Speaker 1: housing unit. On two occasions, July twenty sixth, twenty nineteen
- 14:17Speaker 1: and July twenty seventh, twenty nineteen, he described himself as
- 14:21Speaker 1: a coward and as someone who does not like pain.
- 14:24Speaker 1: On July twenty eighth, twenty nineteen, he told doctor redacted
- 14:27Speaker 1: the toilet seat in his cell would not stop flushing
- 14:30Speaker 1: for an extended period of time, and then he took
- 14:32Speaker 1: to sitting in the corner with his hands over his ears.
- 14:36Speaker 1: Mister Epstein indicated he was agitated following this incident and
- 14:40Speaker 1: was unable to sleep that night. All right, folks, we're
- 14:43Speaker 1: gonna wrap up right here, and in the next episode
- 14:45Speaker 1: dealing with the topic, we're gonna pick up where we
- 14:47Speaker 1: left off. All the information that goes with this episode
- 14:50Speaker 1: can be found in the description box. What's up, everyone,
- 14:54Speaker 1: and welcome to another episode of the Epstein Chronicles. In
- 14:58Speaker 1: this episode, we're going to pick up where we left
- 15:00Speaker 1: off with the psychological report on Jeffrey Epstein's death at
- 15:06Speaker 1: the sedents circumstances. Mister Epstein entered BOP custody on July six,
- 15:12Speaker 1: twenty nineteen, with the history of convictions for sexual offenses
- 15:16Speaker 1: and allegations comprised of more serious charges. The current indictment
- 15:21Speaker 1: alleged sexual crimes against minors, and he was facing up
- 15:24Speaker 1: to forty five years in prison. On July eighteenth, twenty nineteen,
- 15:28Speaker 1: mister Epstein's request for bell and pre trial release was denied.
- 15:32Speaker 1: On July twenty three, twenty nineteen, mister Epstein was found
- 15:35Speaker 1: on a responsive in his cell. The motivation and context
- 15:39Speaker 1: were never fully determined. After thirty one hours and five
- 15:43Speaker 1: minutes on suicide watch, He was then placed on psychological observation.
- 15:48Speaker 1: On July thirtieth, twenty nineteen, mister Epstein was removed from
- 15:52Speaker 1: psychological observation and needed to be housed with an appropriate
- 15:55Speaker 1: cell mate. This email was sent to seventy one on
- 16:00Speaker 1: mcc New York staff, and as of August thirteenth, twenty nineteen,
- 16:05Speaker 1: only twenty seven staff members had opened the message. On
- 16:09Speaker 1: August ninth, twenty nineteen, a federal court unsealed approximately two
- 16:13Speaker 1: thousand pages of documents into the public domain. These included
- 16:18Speaker 1: graphic allegations against mister Epstein. Included was a book order
- 16:23Speaker 1: receipt for titles such as SM one oh one, A
- 16:26Speaker 1: Realistic Introduction slave Craft, road Maps for Erotic Servitude and
- 16:31Speaker 1: Training with Miss Abernathy, a workbook for erotic slaves and
- 16:36Speaker 1: their owners. Additional high profile public figures were also named.
- 16:41Speaker 1: The documents were part of a defamation lawsuit filed by redacted,
- 16:45Speaker 1: a woman who alleged mister Epstein had victimized her against
- 16:49Speaker 1: a British socialite Glaion Maxwell, who was mister Epstein's ex girlfriend,
- 16:53Speaker 1: associate and alleged madam. With the release of the information,
- 16:57Speaker 1: it became increasingly clear that mister Epstein's chance of avoiding
- 17:01Speaker 1: a lengthy period of incarceration and a broader public understanding
- 17:05Speaker 1: of his private life were inevitable. Mister Epstein was afforded
- 17:10Speaker 1: telephone calls on two different days. No records of the
- 17:13Speaker 1: call exist, and it's not known with whom he was speaking.
- 17:17Speaker 1: One occurred on or around July sixteenth, twenty nineteen, and
- 17:20Speaker 1: the other on August ninth, twenty nineteen. Given the limited
- 17:25Speaker 1: information known about mister Epstein, knowledge of the content of
- 17:28Speaker 1: the calls would have been crucial to helping staff work
- 17:31Speaker 1: with them. Following his final telephone call on the evening
- 17:35Speaker 1: of August ninth, twenty nineteen, mister Epstein was moved into
- 17:39Speaker 1: his shoe cell. He was single cells at the time
- 17:42Speaker 1: because his cell mat, Ephren Reyus, did not return from court.
- 17:46Speaker 1: The need for a cell mate was communicated between day
- 17:49Speaker 1: watch and evening Watch ships in the shoe, but no
- 17:52Speaker 1: cell mat was placed with them by the e W staff.
- 17:55Speaker 1: BOP staff knew mister Epstein's cellmate had not returned from
- 17:59Speaker 1: court as as early as one fifty pm that day.
- 18:02Speaker 1: A review of the thirty minute round forms indicate unit
- 18:05Speaker 1: rounds were not completed for the entire MW shift on
- 18:08Speaker 1: August tenth, twenty nineteen. However, a memorandum from Lieutenant Stanley
- 18:13Speaker 1: Jean indicates officer Toavin Nowell and Material Handler Supervisor Michael
- 18:18Speaker 1: Thomas made a statement after mister Epstein's death that they
- 18:21Speaker 1: did not complete proper thirty minute rounds at three AM
- 18:26Speaker 1: or five am description of the scene. A detailed description
- 18:31Speaker 1: of the scene was unavailable because the officers who discovered
- 18:34Speaker 1: mister Epstein did not write memorandums and cannot be interviewed.
- 18:38Speaker 1: According to the report of incident, on August tenth, twenty nineteen,
- 18:42Speaker 1: at approximately six thirty three am, while serving the breakfast
- 18:45Speaker 1: meal in the Shoe Range nine South, mister Epstein was
- 18:49Speaker 1: found on a responsive in his cell. Staff reportedly called
- 18:52Speaker 1: for medical assistance, activated the body alarm, and began life
- 18:56Speaker 1: saving measures. Arriving staff alleged they brought an automated external
- 19:01Speaker 1: defibrillator and stretcher. Cardio pulmonary recitation CPR reportedly continued while
- 19:08Speaker 1: the AED was placed on Epstein. The AED reportedly indicated
- 19:12Speaker 1: no shock advised and CPR was continued. Mister Epstein was
- 19:16Speaker 1: escorted to health services at approximately six thirty nine am
- 19:21Speaker 1: and emergency medical services arrived at six forty three am.
- 19:25Speaker 1: He was transported to the local hospital at approximately seven
- 19:28Speaker 1: ten am. Mister Epstein was pronounced deceased at seven thirty
- 19:32Speaker 1: six am. It was not possible to confirm this timeline
- 19:36Speaker 1: without reviewing video footage. Conclusions recommendations. A general appreciation of
- 19:42Speaker 1: risk factors for suicide with sex offenders is necessary when
- 19:46Speaker 1: reviewing mister Epstein's death. There are several common factors that
- 19:51Speaker 1: increase risk for suicide and individuals with a history of
- 19:54Speaker 1: sexual offense. These include stigma due to the nature of
- 19:58Speaker 1: sexually based crimes, both within society and the prison system,
- 20:03Speaker 1: a disruption of the ability to utilize sex as a
- 20:05Speaker 1: coping mechanism, which can lead to increased levels of distress
- 20:09Speaker 1: and negative effect, and grief about loss experienced in regards
- 20:14Speaker 1: to arrest. The grief may be secondary to the loss
- 20:17Speaker 1: of former lifestyle, loss of physical items or collection relating
- 20:21Speaker 1: to sexual offenses, and or loss of perceived relationship with victims.
- 20:27Speaker 1: Other factors that may increase risk for suicide among individuals
- 20:30Speaker 1: accused of sex offenses include safety concerns, potentially long sentences,
- 20:36Speaker 1: and lack of skills necessary to navigate social relationships in prison.
- 20:41Speaker 1: Mister Epstein was a high profile pre trial detainee awaiting
- 20:44Speaker 1: trial on sex trafficking offenses. He had been successful, wealthy
- 20:48Speaker 1: businessman with a number of high profile acquaintances that he
- 20:52Speaker 1: accumulated through a combination of charisma, charm, and intelligence. Despite
- 20:57Speaker 1: his many associates, he had limited significant or deep interpersonal ties.
- 21:03Speaker 1: Although mister Epstein appeared to cultivate a large social and
- 21:07Speaker 1: professional network, he was estranged from his only brother. Indeed,
- 21:11Speaker 1: his identity appeared to be based on his wealth power
- 21:14Speaker 1: and association with other high profile individuals. Approximately two and
- 21:19Speaker 1: a half weeks before his death, mister Epstein appeared to
- 21:21Speaker 1: attempt suicide, but ultimately denied it was a suicide attempt.
- 21:27Speaker 1: He was convincing in his denial at the time. He
- 21:30Speaker 1: was saved because his cellmate notified BOP staff. In the
- 21:34Speaker 1: weeks before his death, he made statements that he was
- 21:36Speaker 1: a coward and it was having difficulty adapting to his
- 21:39Speaker 1: diminished circumstances. He also frequently referenced poor sleep and an
- 21:44Speaker 1: inability to tolerate the noise of prison. On the day
- 21:47Speaker 1: before his death, a number of documents in his case
- 21:50Speaker 1: were unsealed, further eroding his previously enjoyed elevated status and
- 21:55Speaker 1: potentially implicating some of his associates. Given his lack of
- 21:59Speaker 1: significant interpersonal connection, coupled with the complete loss of a
- 22:03Speaker 1: status in both the community and among associates, the idea
- 22:07Speaker 1: potentially spending his life in prison was likely intolerable. One
- 22:12Speaker 1: single selling. It's recommended that all inmates be double celled,
- 22:16Speaker 1: unless safety concerns or an odd number of inmates precludes this.
- 22:21Speaker 1: Priorities should be given to inmates with the history of
- 22:23Speaker 1: mental illness, self directed violence, recent stressors. It's recommended that
- 22:29Speaker 1: a system of control be implemented, explaining who will be
- 22:32Speaker 1: notified when a suicide watch or psychological observation ends and
- 22:36Speaker 1: how that communication will take place. Because this is a
- 22:40Speaker 1: life safety issue, the system of control, once approved by
- 22:44Speaker 1: executive staff, should be reviewed in formal meetings such staff recalls,

- 23:02Speaker 1: When mister Epstein was placed in the shoe on July seventh,
- 23:04Speaker 1: twenty nineteen, executive staff decided mister Tartaglioni would be a cellmate.
- 23:10Speaker 1: As explained by doctor redacted, input was not sought from
- 23:14Speaker 1: Psychology Services, and it's not clear if or how sex
- 23:18Speaker 1: offenders specific needs and associated risk were incorporated. Mister Tartaglioni
- 23:24Speaker 1: was a high profile inmate, an ex police officer charged
- 23:27Speaker 1: in multiple murders. However, he and Epstein did not share
- 23:31Speaker 1: risk associated with being sex offender, and their pairing may
- 23:34Speaker 1: have aggravated mister Epstein's risk for self directed violence. In
- 23:39Speaker 1: an effort to treat mister Epstein the same as other inmates,
- 23:41Speaker 1: a statement repeated by multiple staff, executive staff may have
- 23:45Speaker 1: inadvertently overlooked the need to consider significant, unique risk factors
- 23:50Speaker 1: associated with individuals who have been charged with, convicted, or
- 23:54Speaker 1: convicted of a sex offense. On July twenty fifth, twenty nineteen,
- 23:59Speaker 1: doctor adacted sent an email to redacted Associate Warden explaining
- 24:04Speaker 1: a consultation between doctor redacted and doctor redacted National suicide
- 24:09Speaker 1: Prevention Coordinator. In the email, doctor redacted reviewed and consulted
- 24:14Speaker 1: and recommended from the Psychology Services Branch Central Office that
- 24:18Speaker 1: mister Epstein be housed with another inmate who had also
- 24:22Speaker 1: been accused of committing a sex offense. There is no
- 24:25Speaker 1: evidence this information was considered beyond this email, and mister
- 24:29Speaker 1: Epstein was never housed with another sex offending inmate. It's
- 24:33Speaker 1: recommended executive staff and correctional services staff include a psychologist

- 25:00Speaker 1: between documents describing the circumstances of the scene. In a
- 25:04Speaker 1: general administrative note in pdsp E, Mr Doctor redacted documented
- 25:09Speaker 1: information received from an operations Lieutenant redacted that mister Epstein
- 25:14Speaker 1: was found with a string loosely hanging around his neck.
- 25:18Speaker 1: In contrast, Officer Wilson Silva, who responded to the emergency,
- 25:22Speaker 1: wrote in a memorandum dated July twenty third, twenty nineteen.
- 25:26Speaker 1: In the memorandum, Officer with Silva wrote he saw mister
- 25:29Speaker 1: Epstein laying down near his bunk with what appeared to
- 25:32Speaker 1: be a piece of handmade orange cloth around his neck.
- 25:36Speaker 1: It's critical that all descriptions of the incident accurately reflect
- 25:40Speaker 1: objective evidence. Officer adacted wrote mister Epstein an incident report
- 25:46Speaker 1: for self mutilation on July twenty third, twenty nineteen, after
- 25:50Speaker 1: he was found on responsive in his cell. A Special
- 25:53Speaker 1: Investigative Services threat assessment was completed on August two, twenty nineteen,
- 25:58Speaker 1: but results were inconclusive as to whether mister Epstein engaged
- 26:02Speaker 1: in self directed violence, willingly fought with the cell mate,
- 26:06Speaker 1: or was assaulted by a cell mate. Although the incident
- 26:09Speaker 1: report was written in a timely manner, it presumed the
- 26:12Speaker 1: engaged in self directed violence and neglected to consider the
- 26:16Speaker 1: other options of fighting an assault. Since it was unclear
- 26:20Speaker 1: what had taken place in mister Epstein's cell, there should
- 26:23Speaker 1: have been three incident reports written, one alleging mister Epstein
- 26:26Speaker 1: engaging in self mutilation, one alleging mister Epstein participated in
- 26:31Speaker 1: a fight or assault without injury, and a third alleging
- 26:34Speaker 1: mister Tartaglione participated in a fault or a fight or
- 26:40Speaker 1: assault with injury. It's recommended that staff remain open to
- 26:43Speaker 1: all reasonable explanations for all behavior and take appropriate action
- 26:47Speaker 1: for all those possibilities until a final determinations made. Doctor
- 26:52Speaker 1: adacted entered a Psychology Service intake screening into PDS B
- 26:57Speaker 1: E m R on July eighth, twenty ten. The document
- 27:01Speaker 1: has three typographical errors. She selected the no sexual offense
- 27:05Speaker 1: conviction checkbox when in fact Epstein was previously convicted of
- 27:10Speaker 1: solicitation of prostitution and procuring a person under the age
- 27:14Speaker 1: of eighteen for a prostitution. Second, mister Epstein was erroneously
- 27:18Speaker 1: identified as a black mail in the document. Finally, there's
- 27:22Speaker 1: one instance where he was mistakenly referred to as mister Brown.
- 27:27Speaker 1: Doctor redacted completed a risk sexual abusiveness document on July eighth,
- 27:31Speaker 1: twenty nineteen. She marked history of prior prison sexual predation
- 27:36Speaker 1: in the affirmative this is not accurate. Redacted mid level

- 27:45Speaker 1: An intake screening should have been conducted within twenty four
- 27:48Speaker 1: hours of his entry into bureau custody. According to P
- 27:52Speaker 1: six zero three one to zero four, Officer adacted was
- 27:57Speaker 1: responsible for observing mister Epstein and documenting his behavior while
- 28:01Speaker 1: on suicide watch. On July twenty third, twenty nineteen, Officer
- 28:05Speaker 1: redacted mistakenly used an inmate suicide watch log book between
- 28:09Speaker 1: one forty am and six AM on July twenty third,
- 28:13Speaker 1: twenty nineteen, when he should have been using the staff
- 28:16Speaker 1: suicide watch logbook. Inexplicably, misredacted drug treatment specialist reportedly noticed
- 28:23Speaker 1: this error and subsequently hand copied all of Officer redacted's
- 28:27Speaker 1: entries from one forty am to six am into the
- 28:31Speaker 1: staff Suicide Watch logbook. She then initialed the entries, and
- 28:35Speaker 1: this makes it appear as if she was the one
- 28:37Speaker 1: conducting the watch. This information was discovered and conveyed in
- 28:41Speaker 1: an email from misredacted associate Warden to doctor redacted with
- 28:46Speaker 1: a carbon copied a Warden redacted on August twelfth, twenty nineteen.
- 28:50Speaker 1: Of note, misredacted did not make an entry explaining why
- 28:53Speaker 1: she was making the logbook changes. Additionally, misredacted then wrote
- 28:57Speaker 1: entries for six fifteen, six thirty, six forty five and
- 29:01Speaker 1: seven am in the staff Suicide Watch logbook. These were
- 29:05Speaker 1: not part of the original entries made by Officer redacted,
- 29:08Speaker 1: nor was misredacted assigned to work suicide watch post. It's
- 29:12Speaker 1: recommended that if a staff member makes an entry error,
- 29:16Speaker 1: the staff members should describe the error in the correct logbook,
- 29:20Speaker 1: to include indicating when they became aware of the error.
- 29:23Speaker 1: The staff members should then notify the chief Psychologist. A

- 31:18Speaker 1: this episode, we're picking up where we left off with
- 31:21Speaker 1: the mcc Psychological Department and their explanation as to what
- 31:26Speaker 1: happened to Jeffrey Epstein Number six direct observation. Mister Epstein
- 31:33Speaker 1: was on suicide watch from July twenty third, twenty nineteen,
- 31:37Speaker 1: until July twenty fourth, twenty nineteen, while on suicide watch.
- 31:41Speaker 1: On July twenty third, twenty nineteen, mister Epstein attended an
- 31:46Speaker 1: attorney visit from approximately twelve forty pm until seven to
- 31:50Speaker 1: fifteen PM. During this time, he was without direct continuous
- 31:55Speaker 1: observation by a dedicated BOP staff member as required by P.
- 31:59Speaker 1: Five five three two four dot zero eight. While on
- 32:02Speaker 1: psychological observation, he attended attorney visits on July twenty fourth,
- 32:07Speaker 1: twenty nineteen, for eleven and a half hours, on July

- 32:27Speaker 1: for ten and a half hours, and on July twenty ninth,
- 32:31Speaker 1: twenty nineteen, for eight hours. On July thirtieth, twenty nineteen,
- 32:37Speaker 1: psychology observation was terminated. During these visits, continuous observation by
- 32:43Speaker 1: a dedicated BOP staff member was not maintained as required
- 32:47Speaker 1: by mcc New York procedural Memorandum for Psychological Observation seven
- 32:53Speaker 1: follow up. Mister Epstein arrived at mcc New York on Saturday,
- 32:58Speaker 1: July six, twenty nineteen. While conducting the ten pm institution
- 33:02Speaker 1: count that evening, redacted facility as assistant reported she observed
- 33:07Speaker 1: mister Epstein in itself. In an email that she sent
- 33:10Speaker 1: to doctor redacted and redacted and Lieutenant redacted later that evening,
- 33:16Speaker 1: she described mister Epstein as distraught, sad, and a little confused.
- 33:21Speaker 1: She said that she then asked mister Epstein if he
- 33:23Speaker 1: was okay, and he reportedly said he was. However, she
- 33:27Speaker 1: noted in her email she was not convinced of this,
- 33:30Speaker 1: adding he seems dazed and withdrawn. She went on to say, so,
- 33:35Speaker 1: just to be on the safe side and prevent any
- 33:38Speaker 1: suicidal thoughts, can someone from psychology come and talk to him.
- 33:43Speaker 1: Despite the fact that Lieutenant redacted open this email, there's
- 33:47Speaker 1: no evidence that he contacted the on call psychologist as
- 33:50Speaker 1: is required by P. Five three two four dot zero
- 33:54Speaker 1: eight Suicide Prevention Program. Additionally, if misredacted was concerned about
- 34:00Speaker 1: suicide risk, P. Five three two four dot zero eight
- 34:05Speaker 1: Suicide Program requires her to maintain direct, continuous observation of
- 34:10Speaker 1: mister Epstein when Doctor Redacted opened the email. The following
- 34:14Speaker 1: one day morning, mister Epstein was evaluated by Doctor redacted
- 34:18Speaker 1: at approximately nine thirty am. Mister Epstein was denied bail
- 34:23Speaker 1: on Thursday July eighteenth, twenty nineteen. This was a significant
- 34:27Speaker 1: disappointment for mister Epstein and likely challenged his ability and
- 34:31Speaker 1: willingness to adapt to incarceration. Given the potential impact of
- 34:35Speaker 1: the judge's decision, a psychologist should have assessed mister Epstein's
- 34:39Speaker 1: mental status upon his return to the institution. The BOP
- 34:44Speaker 1: developed a century assignment of sy alert for purposes such
- 34:48Speaker 1: as this. Specifically, cy alert is used to ensure, if
- 34:52Speaker 1: movement occurs, that all staff consider the special psychological and
- 34:58Speaker 1: management conditions and related risks associated with the inmate. Furthermore, p.

- 35:16Speaker 1: reviewed and considered by psychology services staff, and any safety
- 35:21Speaker 1: and security concerns are highlighted from non psychology services staff.
- 35:26Speaker 1: Psychologists should use the sy alert assignment more frequently with
- 35:30Speaker 1: high profile cases. And with inmates who have a history
- 35:33Speaker 1: of charge of sex offense. Both of these groups of
- 35:36Speaker 1: inmates are susceptible to exaggerated or unrealistic fears about correctional
- 35:41Speaker 1: settings and experience stress associated during movement and periods of
- 35:46Speaker 1: transition e g. Cell unit changes, movement to and from court,
- 35:52Speaker 1: institutional movement, and release of information through the media. Mister
- 35:57Speaker 1: Epstein was a reportedly in court on July third, twenty nineteen.
- 36:02Speaker 1: It's unknown what time he returned to mcc New York,
- 36:06Speaker 1: because this information was not entered in century. Regardless, upon
- 36:11Speaker 1: his return, the United States Marshall Service us MS provided
- 36:15Speaker 1: R and D staff with a Prisoner of Custody Alert
- 36:18Speaker 1: notice regarding mister Epstein. The notice indicated mister Epstein had
- 36:22Speaker 1: MTL mental concern suicidal tendencies. The us MSS requested R
- 36:29Speaker 1: and D staff sign the form and then they departed
- 36:32Speaker 1: with the signed copy. On August first, twenty nineteen, at
- 36:36Speaker 1: eight forty six am, Doctor adacted sent Doctor Adacted in
- 36:40Speaker 1: email reporting she had just become aware of the above information.
- 36:45Speaker 1: In the absence of additional information about this notation, this
- 36:49Speaker 1: should have been considered a referral to psychology about it.
- 36:53Speaker 1: Potentially suicidal inmate, and procedures should have been followed as
- 36:57Speaker 1: outlined in p. Five three two four dot zero eight
- 37:01Speaker 1: Suicide Prevention Program. Specifically, when a staff member becomes aware
- 37:06Speaker 1: an inmate may be thinking about suicide during normal working hours,
- 37:11Speaker 1: that staff member must contact psychology services and maintain the
- 37:15Speaker 1: inmate under direct continuous observation until he is placed on
- 37:19Speaker 1: suicide watch or seen by a psychologist. There's no evidence
- 37:24Speaker 1: mister Epstein was monitored under these conditions from the time
- 37:27Speaker 1: he returned from court until he was seen by doctor
- 37:30Speaker 1: redacted for a suicide risk assessment on August first, twenty nineteen,
