- 0:29This is Haley Recker with the Becker's Payer
- 0:31podcast. And today, we are recording live at
- 0:33the Becker's third annual spring payer issues roundtable.
- 0:36I am thrilled today to be joined by
- 0:38Angie Kalousek,
- 0:40senior director of clinical strategy and programs at
- 0:42Blue Shield of California.
- 0:44Angie, thank you so much for being with
- 0:46me today. Can you go ahead and introduce
- 0:48yourself and share a little bit about your
- 0:50background?
- 0:51Absolutely.
- 0:52So,
- 0:53like you said, Angie Kalousek. I'm with Blue
- 0:55Shield of California. I've actually been with Blue
- 0:57Shield for about fifteen years.
- 0:59So I had a really
- 1:01great long tenured career there and have done
- 1:04a lot of different things.
- 1:06Right now, I'm working on our clinical strategy
- 1:09and programs, which is really an effort
- 1:12to prioritize
- 1:14clinical priorities,
- 1:16based on,
- 1:18need. So what is the prevalence? What is
- 1:21the cost drivers?
- 1:22And it's been really interesting
- 1:24work to dive into that.
- 1:27Previously,
- 1:28I was the wellness,
- 1:30program director at Visa, and I also have,
- 1:34a fitness background. I was a personal trainer
- 1:37and,
- 1:38helped others,
- 1:40get to their best health as well.
- 1:42Well, thank you so much for that introduction.
- 1:45Now I'd like to get things started by
- 1:47asking you about balancing affordability and quality.
- 1:50So this is a constant challenge for health
- 1:53plans. So how is your organization
- 1:55innovating to manage the cost of care
- 1:57while maintaining or improving member outcomes? Yeah. It's
- 2:00a absolutely great question, and it really is
- 2:02a double edged sword between affordability
- 2:05and quality. But I really do think you
- 2:07can have both.
- 2:09One of the ways that we are tackling
- 2:11this conundrum
- 2:13is by really doubling down on
- 2:15value based care. So we are working with
- 2:19a lot of our providers across our network
- 2:21as well as vendor contracts,
- 2:24and sort of the,
- 2:25clinical
- 2:26program contracts as well
- 2:28to always
- 2:30create
- 2:31a payment structure that incentivizes
- 2:34the provider
- 2:35to deliver outcomes. And then when you're aligning
- 2:38the cost with the outcomes,
- 2:41you are then getting to a place where
- 2:44your care is more affordable
- 2:46and also higher quality.
- 2:49I think the other thing that we can
- 2:50really take a look at and focus on
- 2:52is, does our medical policy
- 2:54align
- 2:55with reducing
- 2:57unnecessary
- 2:58procedures, reducing waste?
- 3:01You know, you think about,
- 3:02people who are
- 3:04going getting an injury and going straight to
- 3:07surgery instead of doing the physical therapy first.
- 3:11You know, we see up to, like, fifty
- 3:13percent reduction
- 3:14in surgery if people are doing physical therapy
- 3:17first. So, again, just aligning that medical policy
- 3:20so that,
- 3:21the best clinical care
- 3:23is
- 3:24utilized.
- 3:26I think the final thing, and I think
- 3:27this one's really important, is really focusing on
- 3:31prevention,
- 3:33prevention,
- 3:34early screenings,
- 3:35and lifestyle based programs. So if all health
- 3:39plans were working to
- 3:41optimize the health of their members,
- 3:44then all members, no matter which health plan
- 3:46they went to, would show up at at
- 3:49the lowest possible risk.
- 3:51Getting ahead of chronic conditions like
- 3:54obesity, type two diabetes, hypertension,
- 3:57these are things that downstream
- 3:59can
- 4:00drive a lot of the cost in the
- 4:02system.
- 4:03And if we can get ahead of it,
- 4:05that's certainly the best approach.
- 4:07Well, that was just fascinating. Thank you so
- 4:09much for going into detail there.
- 4:11I'd like to pivot the conversation a little
- 4:13bit since there's a lot of talk about
- 4:15imperative to reduce costs, improve quality, and advance
- 4:18health equity,
- 4:20none of which are small tasks. So how
- 4:22do you approach aligning these priorities in your
- 4:24organization's
- 4:25strategic vision?
- 4:26Yeah. Well, our our mission is to deliver
- 4:28affordable health care to our, members
- 4:32in a way that is worthy of our
- 4:33family and friends. And so,
- 4:36I always kind of keep that at heart.
- 4:38One of the things that we are doing
- 4:41right now is we are shifting our approach
- 4:44to a very specific line of business approach.
- 4:47And I was talking about how I'm beginning
- 4:49to work on some of the clinical priorities
- 4:52and, strategizing on how we do that.
- 4:55The truth is depending on the line of
- 4:57business,
- 4:58the priorities might be a little different. So
- 5:00when we look at prevalence and we look
- 5:02at high cost drivers,
- 5:04there are some of the usual suspects that
- 5:06show up kind of across all the lines
- 5:08of business, like cancer, often MSK,
- 5:12and, of course, cardiovascular.
- 5:14But beyond that,
- 5:16priorities change. For example, Medi Cal may have
- 5:20a higher maternity need.
- 5:22There might be more in the digestive health
- 5:24space
- 5:25with,
- 5:27our our commercial membership.
- 5:29Medicare may have its own considerations because we're
- 5:32dealing with the senior population.
- 5:34So I think it's it's crucial to
- 5:38align the the action that you're taking and
- 5:41the focus that you're you're driving towards with
- 5:44the actual
- 5:45line of business. And I think by doing
- 5:47this and being more targeted and more precise
- 5:48in how we're approaching these lines of business,
- 5:51we are also able to address health equity.
- 5:54As I mentioned, you know,
- 5:56the Medi Cal line of business has its
- 5:58own unique
- 5:59needs,
- 6:00based on some of the underserved populations. And
- 6:03so we're able to address health equity by
- 6:06being precise in how we deliver clinical care.
- 6:09Looking ahead, what do you see as the
- 6:11biggest opportunity for payers to lead the charge
- 6:14in transforming care delivery and driving better outcomes
- 6:16for all stakeholders?
- 6:18And how can leaders take a step in
- 6:19this direction now?
- 6:21Yeah. I think,
- 6:23digital care and virtual care are here to
- 6:26stay. And when we when we talk about
- 6:28the care delivery
- 6:30kind of vehicle or how we're delivering care,
- 6:33if we can inject virtual care and digital
- 6:36care,
- 6:38more upstream
- 6:39and early on in the intervention,
- 6:42we can save costs by reducing the point
- 6:45of service,
- 6:47expense. Right? And and so I I think
- 6:50delivering
- 6:51on virtual care and digital care in the
- 6:53right way,
- 6:55is gonna be critical to transforming
- 6:57care delivery.
- 6:59I also believe that health plans are notorious
- 7:02for
- 7:03not being super transparent
- 7:06and,
- 7:07treating health care like it's this very confusing
- 7:11topic, and
- 7:13most consumers are very confused by health care.
- 7:16So I think we could all do a
- 7:18much better job of treating
- 7:21the member or the patient
- 7:23like a consumer. You know? What would a
- 7:26a normal consumer
- 7:28retail site do to engage the customer,
- 7:32drive them in, help them understand how to
- 7:34use the site? And if we could sort
- 7:36of mimic that in the health care space,
- 7:38we'd go a long way to improving the
- 7:40way that we deliver care.
- 7:42Alright. And just to wrap up here, I'd
- 7:44like to address health equity, which has become
- 7:47a critical focus for many health plans.
- 7:49Can you share an overview of a key
- 7:51initiative here that you're involved in or particularly
- 7:54excited about, and what are you hoping to
- 7:55achieve?
- 7:56Yeah. I mean, we have a lot of
- 7:58health equity initiatives, and so I'll just maybe
- 8:01pull out two or three that we're working
- 8:03on. One that we have sort of ongoing
- 8:06all the time is for our Medi Cal
- 8:08line of business.
- 8:09We are
- 8:11focused in the San Diego and Los Angeles
- 8:13County area,
- 8:14and so we have,
- 8:17a number of community resource centers
- 8:20that are available to not only our members,
- 8:24not only our Medi Cal members, but all
- 8:26members of that community. And at those community
- 8:29resource centers,
- 8:30you can get help with advocacy for your
- 8:32health plan. Again, we're talking about how confusing
- 8:35health care is. So you can go there
- 8:37and get help on
- 8:39how do you utilize your plan. But there's
- 8:41also health educators there that are doing coaching
- 8:44and classes. There's fitness classes,
- 8:46cooking classes,
- 8:48child activities. So a lot of different things
- 8:51for the community that we have across
- 8:53San Diego and,
- 8:55Los Angeles County.
- 8:57We're also,
- 8:58implementing
- 8:59doulas for the medical population. That has become
- 9:02a benefit that has really helped improve,
- 9:05maternal health.
- 9:07And,
- 9:08finally, we are doing a pilot also in
- 9:11the counties surrounding our Medi Cal line of
- 9:14business area, San Diego and Los Angeles,
- 9:17where we're partnering with the local Walmarts. And
- 9:19so,
- 9:20the members can go to the Walmart. They
- 9:23can get their blood pressure checked. They can
- 9:25get nutrition counseling,
- 9:27and they can get
- 9:29rewards
- 9:30to shop for healthy foods at Walmart. So
- 9:33kind of tackling it from all different angles.
- 9:36There are many more, but it those are
- 9:37just a few that I'd like to highlight.
- 9:39Well, Angie, thank you so much for having
- 9:42this conversation with me today. Again, this is
- 9:44Haley Recker with the Becker's Payer Podcast recorded
- 9:47live at the Becker's third annual spring payer
- 9:50issues roundtable.
