Speaker 1: What's up everyone and welcome to another episode of the
• 0:03
Speaker 1: Epstein Chronicles. In this episode, we're going to take a
• 0:06
Speaker 1: look at the report that was put together by the
• 0:09
Speaker 1: psychiatrist and their staff about Jeffrey Epstein and the events
• 0:15
Speaker 1: leading up to his death. Background information. Jeffrey Epstein was
• 0:19
Speaker 1: 66 years old, white male, who died on August 10th, 2019,
• 0:24
Speaker 1: while housed at the Metropolitan Correctional Complex in New York,
• 0:27
Speaker 1: New York. Redacted, former Acting Assistant Director, Reentry Services Division,
• 0:33
Speaker 1: appointed a team to conduct a psychological reconstruction. The team
• 0:37
Speaker 1: consisted of Redacted, National Suicide Prevention Coordinator, Central Office, Redacted,
• 0:43
Speaker 1: Sex Offender Treatment Program Coordinator, Central Office, Redacted, Mental Health
• 0:48
Speaker 1: Treatment Coordinator, Central Office, and Redacted, Correctional Services Administrator, Northeast
• 0:54
Speaker 1: Regional Office. The reconstruction was established in accordance with Bureau
• 0:58
Speaker 1: of Prisons, Program Statement 53-2408 Suicide Prevention Program. Formal interviews
• 1:05
Speaker 1: were not conducted as part of this reconstruction at the
• 1:08
Speaker 1: direction of the Department of Justice. A copy of the
• 1:11
Speaker 1: video is normally made following a significant incident, but there
• 1:15
Speaker 1: was no such video in this case since the original
• 1:17
Speaker 1: video was confiscated by the Federal Bureau of Investigation prior
• 1:21
Speaker 1: to the beginning of this reconstruction. The absence of these
• 1:25
Speaker 1: two areas of inquiry severely limited the ability to establish
• 1:29
Speaker 1: accurate timelines, confirm subjective reports, establish converging and diverging lines
• 1:35
Speaker 1: of facts, or discover new areas of inquiry. As a result,
• 1:39
Speaker 1: the information typically gathered, reviewed, and consolidated during a reconstruction
• 1:44
Speaker 1: to support actionable findings and recommendations is limited. Social History Mr.
• 1:52
Speaker 1: Epstein did not have a pre-sentence report available at the
• 1:55
Speaker 1: time of reconstruction. Therefore, no official information regarding social history
• 2:01
Speaker 1: was accessible. The following was gathered from publicly available documents. Mr.
• 2:06
Speaker 1: Epstein was born in 1953 and grew up in a
• 2:10
Speaker 1: middle-class family in the neighborhood of Seagate on Coney Island
• 2:14
Speaker 1: with one brother. After early promotion in two grades, Mr.
• 2:18
Speaker 1: Epstein graduated from Lafayette High School in 1969 at the
• 2:22
Speaker 1: age of 16. He attended Cooper Union and New York University,
• 2:27
Speaker 1: but did not graduate from either. Mr. Epstein taught at
• 2:30
Speaker 1: the Dalton School, a private school on the Upper East
• 2:32
Speaker 1: Side of Manhattan, from September 1974 until he was dismissed
• 2:37
Speaker 1: in June of 1976 for inadequate development as a teacher.
• 2:43
Speaker 1: Following that, He held a number of positions in the
• 2:46
Speaker 1: financial industry, to include a position as a limited partner
• 2:50
Speaker 1: at Bear Stearns until he was dismissed for unknown policy
• 2:53
Speaker 1: violations in 1981. He also worked as a financial consultant
• 2:59
Speaker 1: and founded at least two separate companies. Mr. Epstein had
• 3:03
Speaker 1: two significant periods of employment. The rest of these was
• 3:06
Speaker 1: his position as a consultant with Steven Jude Hoffenberg in
• 3:11
Speaker 1: the late 80s. Mr. Hoffenberg was described as his first mentor. Mr.
• 3:16
Speaker 1: Hoffenberg was later convicted and incarcerated of running a large
• 3:19
Speaker 1: Ponzi scheme. He implicated Mr. Epstein in fraudulently diverting company
• 3:24
Speaker 1: funds for his own personal use. Years later, Leslie Wexner, Mr.
• 3:29
Speaker 1: Epstein's sole client at J. Epstein & Company, granted him
• 3:32
Speaker 1: power of attorney over his affairs, despite also being identified
• 3:37
Speaker 1: as Mr. Wexner's mentee. Mr. Epstein was again accused of
• 3:41
Speaker 1: misappropriating funds more than $ 46 million. These large sums are
• 3:46
Speaker 1: believed to be seed money Mr. Epstein used to establish
• 3:49
Speaker 1: his considerable fortune. These events are indicative of Mr. Epstein's
• 3:53
Speaker 1: highly regarded intelligence and charismatic personality. Legal History Mr. Epstein
• 4:00
Speaker 1: had a history of adult criminal charges and convictions. In
• 4:03
Speaker 1: June 2008, he entered into a non-prosecution agreement and pleaded
• 4:07
Speaker 1: guilty to one count of solicitation of prostitution, and one
• 4:11
Speaker 1: count of procuring a person under the age of 18
• 4:15
Speaker 1: for prostitution in the state of Florida. He was sentenced
• 4:19
Speaker 1: to 30 months, 18 months of incarceration, and 12 months
• 4:23
Speaker 1: of probation. He was also mandated to register as a
• 4:26
Speaker 1: sex offender under the National Sex Offender Registry and Notification Act. Mr.
• 4:32
Speaker 1: Epstein served 13 of his 18-month incarceration and then successfully
• 4:36
Speaker 1: completed 12 months of probation. It's unclear whether he followed
• 4:41
Speaker 1: the sex offender registration guidelines in each place. He owned
• 4:45
Speaker 1: a residence. In regard to the pending charges, Mr. Epstein
• 4:48
Speaker 1: was formally charged with sex trafficking conspiracy in violation of U.S.
• 4:52
Speaker 1: Code 18, Section 371, and sex trafficking in violation of U.S.
• 4:57
  Speaker 1: Code 18, Section 1591A, B, 2, and 2 on July 2, 2019. Specifically,
• 5:07
Speaker 1: he was accused of sexually exploiting and abusing minor females
• 5:11
Speaker 1: over the course of several years. Charging documents allege Mr.
• 5:14
Speaker 1: Epstein enticed and recruited minor females to engage in sexual activity.
• 5:20
Speaker 1: The minor females were reportedly compensated with cash following the
• 5:24
Speaker 1: sexual encounters, and some were encouraged to find other minor
• 5:28
Speaker 1: females to accompany them to Mr. Epstein's residences in New
• 5:32
Speaker 1: York or Florida. He pleaded not guilty to the charges
• 5:36
Speaker 1: and was pretrial at the time of his death. In
• 5:39
Speaker 1: a 37-page decision and order remanding the defendant signed by
• 5:43
  Speaker 1: Judge Richard M. Berman on July 18, 2019, 18 pages
• 5:47
Speaker 1: were dedicated to detailing the danger Mr. Epstein posed to
• 5:52
Speaker 1: others and the community. The document also alleged that he
• 5:56
Speaker 1: was a flight risk. As a result, Mr. Epstein's proposed
• 5:59
Speaker 1: bail package was determined to be inadequate. he was denied
• 6:04
Speaker 1: pretrial release and held on remand. This was likely significant
• 6:08
Speaker 1: to Mr. Epstein, as he would lose the physical and
• 6:11
Speaker 1: financial comfort to which he had become accustomed. Furthermore, his
• 6:16
Speaker 1: ability to mount the defense would be limited to the
• 6:19
Speaker 1: parameters of legal visits and legal work that could be
• 6:22
Speaker 1: accomplished in jail. These restrictions and the abundance of witnesses, victims,
• 6:27
Speaker 1: and negative public attention were likely considerable burdens on Mr. Epstein.
• 6:33
Speaker 1: Institutional History On July 6, 2019, Mr. Epstein was arrested
• 6:38
Speaker 1: at Teterboro Airport in New Jersey upon his return from Paris, France.
• 6:43
Speaker 1: It's unknown whether he was anticipating his arrest. He was
• 6:47
Speaker 1: transported to MCC, New York and keyed into Sentry at 9.24 p.m.
• 6:52
Speaker 1: that evening. Mr. Epstein was placed in general population housing
• 6:56
  Speaker 1: for approximately 22 hours. On July 7, 2019, at approximately 7.20 p.m.,
• 7:03
Speaker 1: he was moved to Special Housing Unit, pending reclassification due
• 7:07
Speaker 1: to the significant increase in media coverage and awareness of
• 7:11
Speaker 1: his notoriety among the inmate population. With regard to his
• 7:15
Speaker 1: adjustment to a correctional setting, Mr. Epstein received one incident
• 7:19
Speaker 1: report while in BOP custody for self-mutilation on July 23, 2019.
• 7:23
Speaker 1: As of August 15, 2019, the incident report had been expunged,
• 7:25
Speaker 1: but it's unclear whether Why and whether Mr. Epstein knew this? Also,
• 7:35
Speaker 1: a review of financial transactions associated with Mr. Epstein's prison
• 7:39
Speaker 1: account revealed one of his attorneys was depositing funds into
• 7:43
Speaker 1: his cellmate, inmate Reyes' commissary account for unknown reasons. The
• 7:48
Speaker 1: plot thickens, doesn't it? Notice how nobody talks about that, huh?
• 7:52
Speaker 1: Why the hell was Epstein giving Mr. Reyes money? Healthcare
• 7:58
Speaker 1: and Personality Description BOP electronic medical records indicate Epstein was
• 8:03
Speaker 1: diagnosed with hyperlipidemia, sleep apnea, hypertension, constipation, prediabetes, neuralgia, and
• 8:12
Speaker 1: neuritis unspecified. He was prescribed the following medications. Docusate sodium,
• 8:18
Speaker 1: milk of magnesia, omega-3, methylprednisone, and biscadil, whatever that is. Mr.
• 8:25
Speaker 1: Epstein was also prescribed insulin and the prescription required him
• 8:29
Speaker 1: to go to the institution pharmacy for administration of this medication. However,
• 8:35
Speaker 1: the dates for which it was prescribed have a notation indicating,
• 8:39
Speaker 1: does not indicated, thus it does not appear insulin, was
• 8:42
Speaker 1: routinely medically necessary. The rest of the medications prescribed were self-carry.
• 8:48
Speaker 1: He also had a continuous positive airway pressure machine, which
• 8:52
Speaker 1: is typically used to treat sleep apnea. Mr. Epstein was
• 8:55
Speaker 1: provided with his personal CPAP machine on July 30th, 2019,
• 9:01
Speaker 1: per BEMR. In regard to mental health history and treatment,
• 9:06
Speaker 1: there are no known available records. Certainly, there were records
• 9:10
Speaker 1: kept on Mr. Epstein's incarceration in Florida. However, the records
• 9:14
Speaker 1: were not available for review as of the date of
• 9:17
Speaker 1: this report. With regard to psychology data system, records in BEMR, Dr.
• 9:22
Speaker 1: Carey redacted, forensic psychologist at MCC's New York completed a
• 9:27
Speaker 1: routine intake screening on July 8, 2019. During the screening, Mr.
• 9:33
Speaker 1: Epstein denied any history of mental health problems, substance abuse,
• 9:37
Speaker 1: and treatment. No symptoms of mental illness were observed. He
• 9:41
Speaker 1: was classified as Mental Health Care Level 1 and was
• 9:45
Speaker 1: not diagnosed with mental illness. Following a consultation with Dr. Redacted,
• 9:51
Speaker 1: National Suicide Prevention Coordinator, on July 8, 2019, Dr. Redacted
• 9:56
Speaker 1: chief psychologist at MCC New York, determined Mr. Epstein should
• 10:00
Speaker 1: be preemptively evaluated for suicide risk upon his return from court.
• 10:06
Speaker 1: Primary consideration was given to his various risk factors for suicide,
• 10:11
Speaker 1: such as his being a high-profile case with media attention,
• 10:14
Speaker 1: his pending sex offense charges, pretrial status, and ongoing court proceedings. Mr.
• 10:20
Speaker 1: Epstein returned from court on July 8, 2019, after normal
• 10:25
Speaker 1: business hours. He denied suicidal thoughts at the time, but
• 10:28
Speaker 1: due to the potential for other risk factors listed above,
• 10:32
Speaker 1: the on-call psychologist placed Epstein on psychological observation in one
• 10:37
Speaker 1: of the suicide watch cells until he could be assessed
• 10:40
Speaker 1: in person by a BOP psychologist. On July 9, 2019, Mr.
• 10:45
Speaker 1: Epstein underwent a formal in-person suicide risk assessment with Dr. Redacted.
• 10:51
Speaker 1: She determined that while on suicide watch was not warranted
• 10:54
Speaker 1: at the time, Mr. Epstein should remain on psychological observation
• 10:58
Speaker 1: status out of an abundance of caution. He was removed
• 11:02
Speaker 1: from psychological observation on July 10, 2010. On July 23, 2019, Dr. Redacted,
• 11:10
Speaker 1: the on-call psychologist was notified Mr. Epstein had been found
• 11:13
Speaker 1: in his cell with a piece of orange cloth around
• 11:16
Speaker 1: his neck. Reportedly, he was observed lying in the fetal
• 11:20
Speaker 1: position on the floor with a noose around his neck.
• 11:23
Speaker 1: Medical staff evaluated Mr. Epstein and found friction marks and
• 11:27
Speaker 1: superficial reddening of the neck and one knee. He was
• 11:31
Speaker 1: placed on suicide watch by the operations lieutenant at approximately 1.40 a.m.
• 11:36
Speaker 1: pending a formal in-person suicide risk assessment. Dr. Redacted, staff
• 11:42
Speaker 1: psychologist at MCC, New York assessed Mr. Epstein for risk
• 11:46
Speaker 1: of suicide later in the morning of July 23rd, 2019.
• 11:50
Speaker 1: He determined that he should remain on suicide watch. Mr.
• 11:54
Speaker 1: Epstein denied any knowledge of how he received marks on
• 11:58
Speaker 1: his neck and initially informed staff that he thought that
• 12:03
Speaker 1: his cellmate had attempted to kill him. Special investigative services
• 12:07
Speaker 1: staff opened an investigation to assess Mr. Epstein's safety and
• 12:11
Speaker 1: collect facts surrounding the episode. Despite the investigation, staff were
• 12:15
Speaker 1: unable to determine whether he was assaulted or engaged in
• 12:19
Speaker 1: self-directed violence. Mr. Epstein was removed from Suicide Watch on
• 12:23
  Speaker 1: July 24, 2019, after 31 hours and 5 minutes. Thereafter,
• 12:30
Speaker 1: he remained in the Suicide Watch cell and was placed
• 12:33
Speaker 1: on psychological observation, where he remained housed until July 30, 2019,
• 12:37
Speaker 1: according to PDS-BEMR records. a discrepancy exists regarding when he
• 12:44
Speaker 1: was removed from psychological observation. His cell assignment per century
• 12:49
Speaker 1: indicates that he was transferred back to the special housing
• 12:52
Speaker 1: unit on July 29, 2019, whereas PDS-BEMR indicates that he
• 12:59
Speaker 1: was removed from psychological observation on July 30, 2019 at
• 13:04
Speaker 1: approximately 8.15 a.m. Mr. Epstein attended a court hearing on
• 13:10
Speaker 1: July 31, 2019, And upon his return, the United States
• 13:14
Speaker 1: Marshal Services provided paperwork to receiving and discharge that noted
• 13:19
Speaker 1: suicidal tendencies. Dr. Redacted was notified on August 1st, 2019,
• 13:24
Speaker 1: about the paperwork. She consulted with Dr. Redacted and then
• 13:28
Speaker 1: met with Epstein to conduct a suicide risk assessment. She
• 13:32
Speaker 1: determined suicide watch was not warranted at the time. Epstein
• 13:36
Speaker 1: remained classified as a mental health care level one throughout
• 13:39
Speaker 1: his time at MCC New York. During his contact with
• 13:42
Speaker 1: psychologist Epstein, routinely denied current mental health symptoms, including suicidal ideation,
• 13:49
Speaker 1: and he did not exhibit symptoms of serious mental illness.
• 13:52
Speaker 1: His most frequent complaint was difficulty sleeping. He did not
• 13:56
Speaker 1: have access to his CPAP machine until it was reportedly
• 13:59
Speaker 1: provided to him on July 30, 2019. Mr. Epstein also
• 14:04
Speaker 1: reported he was bothered by noise in the shoe. At times,
• 14:08
Speaker 1: he noted, concerns related to his safety in the shoe
• 14:10
Speaker 1: were or in general population housing unit. On two occasions,
• 14:14
  Speaker 1: July 26, 2019, and July 27, 2019, he described himself
• 14:20
Speaker 1: as a coward and as someone who does not like pain.
• 14:24
Speaker 1: On July 28, 2019, he told Dr. Redacted the toilet
• 14:28
Speaker 1: seat in his cell would not stop flushing for an
• 14:30
Speaker 1: extended period of time, and then he took to sitting
• 14:33
Speaker 1: in the corner with his hands over his ears. Mr.
• 14:37
Speaker 1: Epstein indicated he was agitated following this incident and was
• 14:41
Speaker 1: unable to sleep that night. Alright folks, we're going to
• 14:43
Speaker 1: wrap up right here, and in the next episode dealing
• 14:45
Speaker 1: with the topic we're going to pick up where we
• 14:47
Speaker 1: left off. All the information that goes with this episode
• 14:50
Speaker 1: can be found in the description box. What's up everyone,
• 14:54
Speaker 1: and welcome to another episode of the Epstein Chronicles. In
• 14:58
Speaker 1: this episode, we're going to pick up where we left
• 15:00
Speaker 1: off with the psychological report on Jeffrey Epstein's death. Antecedent
• 15:06
Speaker 1: Circumstances Mr. Epstein entered BOP custody on July 6, 2019
• 15:12
Speaker 1: with a history of convictions for sexual offenses and allegations
• 15:17
Speaker 1: comprised of more serious charges. The current indictment alleged sexual
• 15:22
Speaker 1: crimes against minors and he was facing up to 45
• 15:25
Speaker 1: years in prison. On July 18, 2019, Mr. Epstein's request
• 15:29
Speaker 1: for bail and pretrial release was denied. On July 23, 2019, Mr.
• 15:33
Speaker 1: Epstein was found unresponsive in his cell the motivation and
• 15:38
Speaker 1: context were never fully determined. After 31 hours and 5
• 15:43
Speaker 1: minutes on suicide watch, he was then placed on psychological observation.
• 15:48
Speaker 1: On July 30, 2019, Mr. Epstein was removed from psychological
• 15:52
Speaker 1: observation and needed to be housed with an appropriate cellmate.
• 15:57
Speaker 1: This email was sent to 71 MCC New York staff and,
• 16:00
Speaker 1: as of August 13, 2019, only 27 staff members had
• 16:07
Speaker 1: opened the message. On August 9th, 2019, a federal court
• 16:11
Speaker 1: unsealed approximately 2,000 pages of documents into the public domain.
• 16:17
Speaker 1: These included graphic allegations against Mr. Epstein. Included was a book,
• 16:23
Speaker 1: order receipt for titles such as SM-101, A Realistic Introduction, Slavecraft,
• 16:29
Speaker 1: Roadmaps for Erotic Servitude, and training with Ms. Abernathy, a
• 16:34
Speaker 1: workbook for erotic slaves and their owners. Additional high-profile public
• 16:39
Speaker 1: figures were also named. The documents were part of a
• 16:42
Speaker 1: defamation lawsuit filed by Redacted, a woman who alleged Mr.
• 16:46
Speaker 1: Epstein had victimized her, against a British socialite, Ghislaine Maxwell,
• 16:51
Speaker 1: who was Mr. Epstein's ex-girlfriend, associate, and alleged madam. With
• 16:55
Speaker 1: the release of the information, it became increasingly clear that Mr.
• 16:59
Speaker 1: Epstein's chances of avoiding a lengthy period of incarceration and
• 17:03
Speaker 1: a broader public understanding of his private life were inevitable. Mr.
• 17:08
Speaker 1: Epstein was afforded telephone calls on two different days. No
• 17:12
Speaker 1: records of the call exist, and it's not known with
• 17:15
Speaker 1: whom he was speaking. One occurred on or around July 16, 2019,
• 17:19
Speaker 1: and the other on August 9, 2019. Given the limited
• 17:24
Speaker 1: information known about Mr. Epstein, knowledge of the content of
• 17:28
Speaker 1: the calls would have been crucial to helping staff work
• 17:31
Speaker 1: with him. Following his final telephone call on the evening
• 17:35
Speaker 1: of August 9, 2019, Mr. Epstein was moved into his
• 17:39
Speaker 1: shoe cell. He was single-celled at the time because his cellmate,
• 17:43
Speaker 1: Efren Reyes, did not return from court. The need for
• 17:46
Speaker 1: a cellmate was communicated between day watch and evening watch
• 17:50
Speaker 1: shifts in the shoe, but no cellmate was placed with
• 17:53
Speaker 1: him by the EW staff. BOP staff knew Mr. Epstein's
• 17:57
Speaker 1: cellmate had not returned from court as early as 1.50 p.m.
• 18:01
Speaker 1: that day. A review of the 30-minute round form's indicate
• 18:05
Speaker 1: unit rounds were not completed for the entire MW shift
• 18:08
Speaker 1: on August 10, 2019. However, a memorandum from Lt. Stanley
• 18:13
Speaker 1: Jean indicates Officer Tova Noel and Material Handler Supervisor Michael
• 18:18
Speaker 1: Thomas made a statement after Mr. Epstein's death that they
• 18:21
Speaker 1: did not complete proper 30-minute rounds at 3 a.m. or
• 18:26
Speaker 1: 5 a.m. Description of the scene. A detailed description of
• 18:31
Speaker 1: the scene was unavailable because the officers who discovered Mr.
• 18:34
Speaker 1: Epstein did not write memorandums and could not be interviewed.
• 18:38
Speaker 1: According to the report of incident, on August 10, 2019,
• 18:41
Speaker 1: at approximately 6.33 a.m., while serving the breakfast meal in
• 18:46
Speaker 1: the SHU, Range 9 South, Mr. Epstein was found unresponsive
• 18:50
Speaker 1: in his cell. Staff reportedly called for medical assistance, activated
• 18:54
Speaker 1: the body alarm, and began life-saving measures. Arriving staff alleged
• 18:59
Speaker 1: they brought an automated external defibrillator and stretcher. Cardiopulmonary recitation, CPR,
• 19:06
Speaker 1: reportedly continued while the AED was placed on Epstein. The
• 19:10
Speaker 1: AED reportedly indicated no shock advised and CPR was continued. Mr.
• 19:16
Speaker 1: Epstein was escorted to health services at approximately 6.39 a.m.
• 19:21
Speaker 1: and emergency medical services arrived at 6.43 a.m. He was
• 19:25
Speaker 1: transported to the local hospital At approximately 7.10 a.m., Mr.
• 19:30
Speaker 1: Epstein was pronounced deceased at 7.36 a.m. It was not
• 19:34
Speaker 1: possible to confirm this timeline without reviewing video footage. Conclusions, recommendations.
• 19:41
Speaker 1: A general appreciation of risk factors for suicide with sex
• 19:44
Speaker 1: offenders is necessary when reviewing Mr. Epstein's death. There are
• 19:50
Speaker 1: several common factors that increase risk for suicide in individuals
• 19:54
Speaker 1: with a history of sexual offense. These include stigma due
• 19:57
Speaker 1: to the nature of sexually based crimes both within society
• 20:01
Speaker 1: and the prison system, a disruption of the ability to
• 20:04
Speaker 1: utilize sex as a coping mechanism which can lead to
• 20:07
Speaker 1: increased levels of distress, and negative effect. And grief about
• 20:12
Speaker 1: loss experienced in regards to arrest. The grief may be
• 20:16
Speaker 1: secondary to the loss of former lifestyle, loss of physical items,
• 20:20
Speaker 1: or collection related to sexual offenses, and or loss of
• 20:24
Speaker 1: perceived relationship with victims. Other factors that may increase risk
• 20:29
Speaker 1: for suicide among individuals accused of sex offenses include safety concerns,
• 20:34
Speaker 1: potentially long sentences, and lack of skills, necessary to navigate
• 20:38
Speaker 1: social relationships in prison. Mr. Epstein was a high-profile pre-trial
• 20:43
Speaker 1: detainee awaiting trial on sex trafficking offenses. He had been
• 20:47
Speaker 1: a successful, wealthy businessman with a number of high-profile acquaintances
• 20:52
Speaker 1: that he accumulated through a combination of charisma, charm, and intelligence.
• 20:57
Speaker 1: Despite his many associates, he had limited, significant, or deep
• 21:02
Speaker 1: interpersonal ties. Although Mr. Epstein appeared to cultivate a large
• 21:06
Speaker 1: social and and professional network, he was estranged from his
• 21:09
Speaker 1: only brother. Indeed, his identity appeared to be based on
• 21:13
Speaker 1: his wealth, power, and association with other high-profile individuals. Approximately
• 21:18
Speaker 1: two and a half weeks before his death, Mr. Epstein
• 21:21
Speaker 1: appeared to attempt suicide, but ultimately denied it was a
• 21:24
Speaker 1: suicide attempt. He was convincing in his denial, at the time,
• 21:30
Speaker 1: he was saved because his cellmate notified BOP staff. In
• 21:34
Speaker 1: the weeks before his death, He made statements that he
• 21:36
Speaker 1: was a coward and was having difficulty adapting to his
• 21:39
Speaker 1: diminished circumstances. He also frequently referenced poor sleep and an
• 21:44
Speaker 1: inability to tolerate the noise of prison. On the day
• 21:47
Speaker 1: before his death, a number of documents in his case
• 21:50
Speaker 1: were unsealed, further eroding his previously enjoyed elevated status and
• 21:55
Speaker 1: potentially implicating some of his associates. Given his lack of
• 21:59
Speaker 1: significant interpersonal connection coupled with a complete loss of his
• 22:03
Speaker 1: status in both the community, and among associates, the idea
• 22:07
Speaker 1: of potentially spending his life in prison was likely intolerable. 1. Single-selling.
• 22:14
Speaker 1: It's recommended that all inmates be double-selled unless safety, concerns,
• 22:18
Speaker 1: or an odd number of inmates precludes this. Priority should
• 22:22
Speaker 1: be given to inmates with a history of mental illness,
• 22:24
Speaker 1: self-directed violence, recent stressors. It's recommended that a system of
• 22:29
Speaker 1: control be implemented explaining who will be notified when a
• 22:33
Speaker 1: suicide watch is where psychological observation ends, and how that
• 22:37
Speaker 1: communication will take place. Because this is a life safety issue,
• 22:41
Speaker 1: the system of control, once approved by executive staff, should
• 22:45
Speaker 1: be reviewed in formal meetings such staff recalls, department head meetings,
• 22:49
Speaker 1: and lieutenant meetings. 2. Rounds. 30-minute rounds are required by P5500. 14,
• 22:57
Speaker 1: Correctional Services Procedure Manual. 3. Cellmate Assignments. When Mr. Epstein
• 23:03
Speaker 1: was placed in the shoe on July 7th, 2019, executive
• 23:06
Speaker 1: staff decided Mr. Tartaglione would be his cellmate. As explained
• 23:11
Speaker 1: by Dr. Redacted, input was not sought from psychology services,
• 23:15
Speaker 1: and it's not clear if or how sex offender-specific needs
• 23:19
Speaker 1: and associated risk were incorporated. Mr. Tartaglione was a high-profile inmate,
• 23:25
Speaker 1: an ex-police officer charged in multiple murders. However, he and Epstein...
• 23:30
Speaker 1: did not share risk associated with being sex offender and
• 23:33
Speaker 1: their pairing may have aggravated Mr. Epstein's risk for self-directed violence.
• 23:38
Speaker 1: In an effort to treat Mr. Epstein the same as
• 23:41
Speaker 1: other inmates, a statement repeated by multiple staff, executive staff
• 23:45
Speaker 1: may have inadvertently overlooked the need to consider significant unique
• 23:49
Speaker 1: risk factors associated with individuals who have been charged with
• 23:53
Speaker 1: convicted or convicted of a sex offense. On July 25th, 2019, Dr.
• 24:00
Speaker 1: Redacted sent an email to Redacted Associate Warden explaining a
• 24:04
Speaker 1: consultation between Dr. Redacted and Dr. Redacted, National Suicide Prevention Coordinator.
• 24:11
Speaker 1: In the email, Dr. Redacted reviewed and consulted and recommended
• 24:15
Speaker 1: from the Psychology Services Branch Central Office that Mr. Epstein
• 24:19
Speaker 1: be housed with another inmate who had also been accused
• 24:23
Speaker 1: of committing a sex offense. There is no evidence. This
• 24:26
Speaker 1: information was considered beyond this email and and Mr. Epstein
• 24:29
Speaker 1: was never housed with another sex-offending inmate. It's recommended executive
• 24:34
Speaker 1: staff and correctional services staff include a psychologist in decisions
• 24:38
Speaker 1: about cellmates as a means of incorporating expertise about suicide risk,
• 24:42
Speaker 1: mental health needs, and interventions for psychological stability. 4. Documentation
• 24:49
Speaker 1: Accuracy On July 23, 2019, Mr. Epstein was found unresponsive
• 24:55
Speaker 1: in his cell. He had abrasions on his neck and knee.
• 24:58
Speaker 1: There are inconsistencies between documents describing the circumstances of the scene.
• 25:04
Speaker 1: In a general administrative note, in PDS-BEMR, Dr. Redacted documented
• 25:09
Speaker 1: information received from an operations lieutenant redacted that Mr. Epstein
• 25:14
Speaker 1: was found with a string loosely hanging around his neck.
• 25:18
Speaker 1: In contrast, Officer Wilson-Silva, who responded to the emergency, wrote
• 25:22
Speaker 1: in a memorandum dated July 23, 2019, in the memorandum,
• 25:24
Speaker 1: Officer Wilson-Silva wrote he saw Mr. Epstein laying down near
• 25:30
Speaker 1: his bunk with what appeared to be a piece of
• 25:33
Speaker 1: handmade orange cloth around his neck. It's critical that all
• 25:37
Speaker 1: descriptions of the incident accurately reflect objective evidence. Officer redacted,
• 25:43
Speaker 1: wrote Mr. Epstein, an incident report for self-mutilation on July 23, 2019,
• 25:50
Speaker 1: after he was found unresponsive in his cell. A special
• 25:53
Speaker 1: investigative services threat assessment was completed. On August 2, 2019,
• 25:58
Speaker 1: but results were inconclusive as to whether Mr. Epstein engaged
• 26:02
Speaker 1: in self-directed violence, willingly fought with his cellmate, or was
• 26:06
Speaker 1: assaulted by his cellmate. Although the incident report was written
• 26:10
Speaker 1: in a timely manner, it presumed he engaged in self-directed
• 26:13
Speaker 1: violence and neglected to consider the other options of fighting
• 26:17
Speaker 1: an assault. Since it was unclear what had taken place
• 26:21
Speaker 1: in Mr. Epstein's cell, there should have been three incident
• 26:24
Speaker 1: reports written, one alleging Mr. Epstein engaging in self-mutilation One
• 26:28
Speaker 1: alleging Mr. Epstein participated in a fight or assault without injury,
• 26:33
Speaker 1: and a third alleging Mr. Tartaglione participated in a fault
• 26:38
Speaker 1: or a fight or assault with injury. It's recommended that
• 26:42
Speaker 1: staff remain open to all reasonable explanations for all behavior
• 26:46
Speaker 1: and take appropriate action for all those possibilities until a
• 26:50
Speaker 1: final determination is made. Dr. Redacted entered a psychology service
• 26:55
Speaker 1: intake screening into PDS-BEMR on July 8, 2010. The document
• 27:01
Speaker 1: has three typographical errors. She selected the no sexual offense
• 27:05
Speaker 1: conviction checkbox when in fact Epstein was previously convicted of
• 27:10
Speaker 1: solicitation of prostitution and procuring a person under the age
• 27:14
Speaker 1: of 18 for prostitution. Second, Mr. Epstein was erroneously identified
• 27:19
Speaker 1: as a black male in the document. Finally, there's one
• 27:22
Speaker 1: instance where where he was mistakenly referred to as Mr. Brown. Dr.
• 27:28
Speaker 1: Redacted completed a risk sexual abusiveness document on July 8th, 2019.
• 27:32
Speaker 1: She marked history of prior prison sexual predation in the affirmative.
• 27:37
Speaker 1: This is not accurate. Redacted, mid-level practitioner, completed a history
• 27:42
Speaker 1: and physical on July 9th, 2019. An intake screening should
• 27:46
Speaker 1: have been conducted within 24 hours of his entry into
• 27:50
Speaker 1: Bureau custody. According to P6031. 04, Officer Redacted was responsible for
• 27:58
Speaker 1: observing Mr. Epstein and documenting his behavior while on suicide
• 28:01
Speaker 1: watch on July 23, 2019. Officer Redacted mistakenly used an
• 28:07
  Speaker 1: inmate suicide watch logbook between 1.40 a.m. and 6 a.m.
• 28:12
Speaker 1: on July 23, 2019, when he should have been using
• 28:16
Speaker 1: the staff suicide watch logbook. Inexplicably, Ms. Redacted, drug treatment specialist,
• 28:22
Speaker 1: reportedly noticed this error and subsequently hand-copied all of Officer
• 28:27
  Speaker 1: Redacted's entries from 1.40 a.m. to 6 a.m. into the
• 28:31
Speaker 1: staff suicide watch logbook. She then initialed the entries, and
• 28:35
Speaker 1: this makes it appear as if she was the one
• 28:37
Speaker 1: conducting the watch. This information was discovered and conveyed in
• 28:41
Speaker 1: an email from Ms. Redacted, Associate Warden to Dr. Redacted,
• 28:46
Speaker 1: with a carbon copy to Warden Redacted on August 12, 2019.
• 28:50
Speaker 1: Of note, Ms. Redacted did not make an entry explaining
• 28:53
Speaker 1: why she was making the logbook changes. Additionally, Ms. Redacted
• 28:57
  Speaker 1: then wrote entries for 6.15, 6.30, 6.45, and 7 a.m.
• 29:02
Speaker 1: in the staff suicide watch logbook. These were not part
• 29:05
Speaker 1: of the original entries made by Officer Redacted, nor was Ms.
• 29:08
Speaker 1: Redacted assigned to work suicide watch post. It's recommended that
• 29:13
Speaker 1: if a staff member makes an entry error, the staff
• 29:17
Speaker 1: member should describe the error in the correct logbook. to
• 29:20
Speaker 1: include indicating when they became aware of the error. The
• 29:24
Speaker 1: staff member should then notify the chief psychologist. A review
• 29:27
Speaker 1: of special housing unit records BP-A0292 revealed a number of
• 29:33
Speaker 1: incomplete entries. The document is used to monitor provision and
• 29:37
Speaker 1: receipt of basic services such as recreation, medical rounds, showers, meal, consumption, etc.
• 29:44
Speaker 1: The officer in charge's signature is missing on 10 occasions
• 29:47
Speaker 1: and a medical provider's signature is missing in 7 instances.
• 29:52
Speaker 1: There are 6 instances which it's not clear if Mr.
• 29:56
Speaker 1: Epstein ate his meal. There are nine instances in which
• 29:59
Speaker 1: it's not clear if Mr. Epstein took a shower. There
• 30:01
Speaker 1: are ten in which it's not clear if Mr. Epstein
• 30:04
Speaker 1: was offered recreation. Corrective Service Manual requires accurate and complete
• 30:10
Speaker 1: information on the BP-A0292. A review of psychology observation logbooks
• 30:17
Speaker 1: revealed significant discrepancies from the approved Psychological Observation Procedural Memorandum
• 30:23
Speaker 1: dated April 15, 2019. A correctional officer is required to
• 30:27
Speaker 1: complete hourly rounds and sign the logbook. 179 out of
• 30:32
Speaker 1: 183 round signatures were missing. The lieutenant is required to
• 30:37
Speaker 1: sign the logbook one time per shift and signatures were
• 30:40
Speaker 1: missing in 10 of 23 instances. A physician assistant is
• 30:44
Speaker 1: required to sign one time per shift and 16 of
• 30:47
Speaker 1: 16 instances were missing. It's recommended that a review of
• 30:52
Speaker 1: psychological observation procedure be conducted. And if it can no
• 30:56
Speaker 1: longer be effectively implemented, the program should be discontinued. All right, folks,
• 31:01
Speaker 1: we're going to wrap up right here. And in the
• 31:02
Speaker 1: next episode dealing with the topic, we're going to pick
• 31:05
Speaker 1: up where we left off and finish this bad boy up.
• 31:08
Speaker 1: All the information that goes with this episode can be
• 31:11
Speaker 1: found in the description box. What's up, everyone? And welcome
• 31:15
Speaker 1: to another episode of the Epstein Chronicles. In this episode,
• 31:19
Speaker 1: we're picking up where we left off with the MCC
• 31:22
Speaker 1: psychological department and their explanation As to what happened to
• 31:27
Speaker 1: Jeffrey Epstein. Number six, direct observation. Mr. Epstein was on
• 31:33
  Speaker 1: suicide watch from July 23rd, 2019 until July 24th, 2019.
• 31:35
Speaker 1: While on suicide watch on July 23rd, 2019, Mr. Epstein
• 31:38
Speaker 1: attended an attorney visit from approximately 1240 p.m. until 715 p.m.
• 31:40
Speaker 1: During this time, He was without direct continuous observation by
• 31:56
Speaker 1: a dedicated BOP staff member as required by P5324. 08. While
• 32:02
Speaker 1: on psychological observation, he attended attorney visits on July 24,
• 32:06
  Speaker 1: 2019 for 11 1⁄2 hours, on July 25, 2019 for
• 32:08
  Speaker 1: 11 1⁄2 hours, on July 26, 2019 for 9 1⁄2 hours,
• 32:10
  Speaker 1: on July 27, 2019, 11,. 33 hours. On July 28th, 2019,
• 32:28
Speaker 1: for 10.5 hours. And on July 29th, 2019, for 8 hours.
• 32:36
Speaker 1: On July 30th, 2019, psychology observation was terminated. During these visits,
• 32:42
Speaker 1: continuous observation by a dedicated BOP staff member was not
• 32:46
Speaker 1: maintained as required by MCC New York Procedural Memorandum for
• 32:50
Speaker 1: Psychological Observations. 7 follow-up. Mr. Epstein arrived at MCC, New
• 32:56
Speaker 1: York on Saturday, July 6, 2019, while conducting the 10 p.m.
• 33:02
Speaker 1: institution count that evening, redacted, facilities assistant, reported she observed Mr.
• 33:07
Speaker 1: Epstein in his cell. In an email that she sent
• 33:10
Speaker 1: to Dr. Redacted and Lieutenant Redacted later that evening, she
• 33:16
Speaker 1: described Mr. Epstein as distraught, sad, and a little confused.
• 33:21
Speaker 1: She said that she then asked Mr. Epstein if he
• 33:23
Speaker 1: was okay, and he reportedly said he was. However, she
• 33:27
Speaker 1: noted in her email, she was not convinced of this, adding,
• 33:31
Speaker 1: he seems dazed and withdrawn. She went on to say,
• 33:35
Speaker 1: so just to be on the safe side and prevent
• 33:38
Speaker 1: any suicidal thoughts, can someone from psychology come and talk
• 33:42
Speaker 1: to him? Despite the fact that Lieutenant Redacted opened this email,
• 33:47
Speaker 1: there is no evidence that he contacted the on-call psychologist,
• 33:50
Speaker 1: as is required by P5324. 08 Suicide Prevention Program. Additionally, if Ms.
• 33:58
Speaker 1: Redacted was concerned about suicide risk, P5324. 08 Suicide Program requires
• 34:07
Speaker 1: her to maintain direct, continuous observation of Mr. Epstein. When Dr.
• 34:12
Speaker 1: Redacted opened the email the following Monday morning, Mr. Epstein
• 34:16
Speaker 1: was evaluated by Dr. Redacted at approximately 9.30 a.m. Mr.
• 34:22
Speaker 1: Epstein was denied bail on Thursday, July 18th, 2019. This
• 34:26
Speaker 1: was a significant disappointment for Mr. Epstein and likely challenged
• 34:30
Speaker 1: his ability and willingness to adapt to incarceration. Given the
• 34:35
Speaker 1: potential impact of the judge's decision, a psychologist should have
• 34:38
Speaker 1: assessed Mr. Epstein's mental status upon his return to the institution.
• 34:43
Speaker 1: The BOP developed a sentry assignment of PsyAlert for purposes
• 34:48
Speaker 1: such as this. Specifically, PsyAlert is used to ensure, if
• 34:52
Speaker 1: movement occurs, that all staff consider the special, psychological, and
• 34:58
Speaker 1: management conditions and related risks associated with the inmate. Furthermore, P5324. 07
• 35:06
Speaker 1: Sentry Psychology Alert Function states, when a decision to move
• 35:11
Speaker 1: any PsyAlert inmate occurs, any special psychological needs of the
• 35:16
Speaker 1: inmate are reviewed and considered, by psychology services staff and
• 35:20
Speaker 1: any safety and security concerns are highlighted from non-psychology services staff.
• 35:26
Speaker 1: Psychologists should use the PsyAlert assignment more frequently with high-profile
• 35:30
Speaker 1: cases and with inmates who have a history of charge
• 35:34
Speaker 1: of sex offense. Both of these groups of inmates are
• 35:37
Speaker 1: susceptible to exaggerated or unrealistic fears about correctional settings and
• 35:42
Speaker 1: experience stress associated during movement and periods of transition. e.g.
• 35:49
Speaker 1: cell unit changes, movement to and from court, institutional movement,
• 35:54
Speaker 1: and release of information through the media. Mr. Epstein was
• 35:58
Speaker 1: reportedly in court on July 31, 2019. It's unknown what
• 36:03
Speaker 1: time he returned to MCC, New York, because this information
• 36:07
Speaker 1: was not entered in Sentry. Regardless, upon his return, The
• 36:12
Speaker 1: United States Marshal Service, USMS, provided R & D staff
• 36:16
Speaker 1: with a prisoner of custody alert notice regarding Mr. Epstein.
• 36:20
Speaker 1: The notice indicated Mr. Epstein had MTL, mental concern, suicidal tendencies.
• 36:26
Speaker 1: The USMSS requested R & D staff sign the form,
• 36:31
Speaker 1: and then they departed with the signed copy. On August 1, 2019,
• 36:34
Speaker 1: at 8.46 a.m., Dr. Redacted sent Dr. Redacted an email
• 36:36
Speaker 1: reporting that she had just become aware of the above information.
• 36:45
Speaker 1: In the absence of additional information about this notation, this
• 36:49
Speaker 1: should have been considered a referral to psychology. About a
• 36:53
Speaker 1: potentially suicidal inmate and procedures should have been followed as
• 36:57
Speaker 1: outlined in P5. 324. 08 Suicide Prevention Program. Specifically, when a staff
• 37:05
Speaker 1: member becomes aware an inmate may be thinking about suicide
• 37:08
Speaker 1: during normal working hours, That staff member must contact psychology
• 37:13
Speaker 1: services and maintain the inmate under direct, continuous observation until
• 37:18
Speaker 1: he is placed on suicide watch or seen by a psychologist.
• 37:23
Speaker 1: There is no evidence Mr. Epstein was monitored under these
• 37:26
Speaker 1: conditions from the time he returned from court until he
• 37:29
Speaker 1: was seen by Dr. Redacted for a suicide risk assessment
• 37:33
  Speaker 1: on August 1, 2019 at approximately 1.30 p.m. Number 8.
• 37:40
Speaker 1: Inmate Accountability and Assignment Accuracy. According to a Century Quarters
• 37:45
  Speaker 1: roster generated on August 10, 2010, at 12.51 a.m., there
• 37:50
Speaker 1: were three inmates assigned to Mr. Epstein's shoe cell, Z04-206LAD,
• 37:57
Speaker 1: including him at the time of his death. However, his
• 38:01
Speaker 1: shoe cell was only double occupancy. Inmate Patrick Avila, Inmate
• 38:07
Speaker 1: Gregory Ferrer, and Mr. Epstein were all assigned to the
• 38:10
  Speaker 1: same cell. On August 13, 2019, at 12.06 p.m. and 12.08 p.m.,
• 38:17
Speaker 1: we generated a quarters history roster for inmate Avila and Ferreira, respectively.
• 38:23
  Speaker 1: Inmate Avila's cell assignment was Z04206-LAD from August 5, 2009
• 38:26
Speaker 1: until August 11, 2019, when he was moved to cell 704212-UAD.
• 38:39
  Speaker 1: Inmate Ferrer's cell assignment was Z04206UAD from August 1, 2019
• 38:42
Speaker 1: until August 11, 2019 when he was moved to cell Z04207LAD.
• 38:44
Speaker 1: We then generated a quarters history roster for Mr. Epstein
• 38:46
  Speaker 1: on August 13, 2019 at 9.07 a.m. His cell assignment
• 38:48
  Speaker 1: was Z04-206LAD From July 29, 2019 until August 10, 2019.
• 39:15
Speaker 1: On Monday, August 12, 2019, photographs of name tags on
• 39:19
Speaker 1: shoe cell doors and shoe locator forms were sent to
• 39:22
Speaker 1: the Correctional Service Department in the Northeast Region. The shoe
• 39:26
Speaker 1: locator form is dated August 9, 2019. 2019. It shows inmate
• 39:32
Speaker 1: Ferrer in cell 207L. Sentry states he was moved to
• 39:36
Speaker 1: the cell on August 11th, 2019. Inmate Epstein is in
• 39:43
Speaker 1: cell 220L. Sentry never shows him in this cell along
• 39:46
Speaker 1: with inmate Reyes. The locator shows inmate Copper, and inmate
• 39:51
Speaker 1: Dockery in cell 206. The photo sheets show the cell
• 39:55
Speaker 1: being 220 with inmate Epstein and Reyes identification cards on
• 39:59
Speaker 1: the door. Inmate Reyes, Efren, registration number 85993-054 was in
• 40:04
  Speaker 1: cell Z06-220U from August 5th, 2019 to August 9th, 2019.
• 40:06
Speaker 1: MCC has four suicide watch cells and each is for
• 40:20
Speaker 1: single occupancy use. The suicide watch cells are located in
• 40:25
Speaker 1: health services, each cell is abbreviated with the unit code H01,
• 40:29
