- 0:58- This is Madeline with the Becker CFO and
- 1:00- revenue cycle podcast, and I'm so excited to
- 1:02- be joined by Austin Willowwhite,
- 1:04- CFO of Monument Health. Austin, thanks for being
- 1:07- on with me. Yeah. Thanks for the invite
- 1:09- to the podcast. Excited for our discussion today.
- 1:11- Well, me too. But before we begin, do
- 1:13- you mind sharing with our listeners just a
- 1:15- little bit about your role at Monument Health
- 1:17- and more on your background in health care?
- 1:20- Sure. Yeah. Currently serve as CFO for Monument
- 1:23- Health.
- 1:23- Started with the system about ten years ago
- 1:25- in a vice president of finance role.
- 1:28- Prior to that worked, in healthcare, in the
- 1:30- finance roles, as well as public accounting. So,
- 1:33- it's been a great ten years. Looking forward
- 1:35- to continuing to grow and develop with the
- 1:37- system.
- 1:38- Alright. That's incredible. I appreciate you sharing.
- 1:41- So, obviously, you know, in your time throughout
- 1:43- health care, we've continued to see trends
- 1:46- constantly changing. There's so much going on. But
- 1:48- what would you say is one thing that
- 1:50- you're keeping a real close eye on right
- 1:51- now from the CFO seat, and why is
- 1:53- this trend right now so important to you?
- 1:56- Sure. In our local market, we're really paying
- 1:58- attention to demographics.
- 2:00- Our system is located in Western South Dakota,
- 2:02- very heavily tourist area, Mount Rushmore, the Sturgis
- 2:05- Spike rally,
- 2:06- all of those fun things. But what that
- 2:08- does is it brings a lot of folks
- 2:10- through our communities,
- 2:11- that see,
- 2:13- the communities and get excited and want to
- 2:15- move here. That's a great problem to have,
- 2:17- really good growth.
- 2:18- But sometimes those folks are retirees.
- 2:21- Sometimes they are folks that aren't bringing,
- 2:24- labor force with them in the healthcare space
- 2:26- because they work from home and some other
- 2:28- things. So we've continued to see our governmental
- 2:30- payer mix, rise and rise.
- 2:32- We're more than 50% and not well on
- 2:34- our way to 60%,
- 2:36- with the Medicare program.
- 2:37- So we pay attention to those shifts. We're
- 2:39- looking at ways, to consistently update our service
- 2:42- offerings as well as manage our reimbursement to
- 2:45- handle that kind of long term payer shift.
- 2:48- Yeah. So it sounds like you're quite busy.
- 2:50- We are. We are. Yes. For sure. What
- 2:53- would you say, you know, we're right now
- 2:55- seeing and I personally myself have covered rural
- 2:57- and regional health systems. They're continuing to face
- 3:00- margin pressure.
- 3:01- What would you say are some financial levers
- 3:03- that you're more focused on that that maybe
- 3:05- you weren't even prioritizing a couple years ago?
- 3:08- Sure. Post pandemic, we really focused on growth.
- 3:11- We've been incredibly successful in growing,
- 3:13- both our organization, recruiting physicians,
- 3:16- just really continuing to add services that were
- 3:19- previously not offered in the community.
- 3:21- Well, that strategy will
- 3:22- probably always be there and will continue to
- 3:24- be. We're kind of turning our attention a
- 3:25- little bit more now to the expense side,
- 3:28- really focusing in on per unit type cost
- 3:30- metrics.
- 3:31- We try to learn where we have partnerships.
- 3:34- We have a we recently entered into a
- 3:35- partnership with a national dialysis,
- 3:38- operator, which anybody who's in dialysis knows that
- 3:41- margins are incredibly thin, and they're just relentless
- 3:44- pursuit of reducing cost at the unit level
- 3:46- is is really something that we've latched onto
- 3:49- and we're working toward.
- 3:50- We've always had good benchmarking data and things
- 3:52- like that, but we didn't necessarily use per
- 3:54- unit costs as as a basis and a
- 3:56- reason to make changes.
- 3:58- We're now adopting that approach and just continuing
- 4:00- to push.
- 4:01- You know, we're also making sure that we're
- 4:03- setting a high bar for capital allocation.
- 4:06- In the past, you know, if you had
- 4:07- a project that that looked good, checked several
- 4:09- of the boxes, maybe provided a return just
- 4:12- a little bit in excess of what we
- 4:13- could get off of our investment portfolio, you
- 4:15- were you were good to go. But as
- 4:17- margins tighten up, in the rural health care
- 4:19- space, we just are directing our attention towards
- 4:22- setting that bar higher, making sure the the
- 4:24- right projects, making sure that the ROI is
- 4:26- there,
- 4:27- and that it makes sense to pursue.
- 4:30- Yeah. And you touched on growth. You know,
- 4:32- obviously, that is something that's super important to
- 4:34- a lot of systems. Systems wanna be growing,
- 4:36- but getting factors get into the way right
- 4:38- now, certain challenges. But, however, you guys in
- 4:41- early May just,
- 4:43- acquired rapid Rapid City Medical Center in South
- 4:45- Dakota. So could you share a little bit
- 4:47- about how that integration process has been going
- 4:50- and what's that look like from the finance
- 4:51- side?
- 4:52- Yeah. Absolutely. We've been long term partners, with,
- 4:55- the Rapid City Medical Center physicians and we're
- 4:57- excited when they were were interested in joining
- 4:59- the system.
- 5:01- From the core finance side, they had a
- 5:03- really strong team. They had good processes in
- 5:05- place, which make life a lot easier.
- 5:08- No turf wars or anything like that.
- 5:10- The one things they probably did not have
- 5:12- is maybe as strong of as an of
- 5:14- an ERP system.
- 5:15- We're on a pretty robust ERP, so we're
- 5:17- able to bring them over, you know, in
- 5:19- practice that really looked like just cutting over
- 5:21- one day. We stopped entering data
- 5:24- bills and things like that into their previous
- 5:26- their legacy system, and we then began bringing
- 5:29- them onto our platform. Now having said that,
- 5:31- that's the finance side. The revenue cycle side
- 5:33- is is always more challenging.
- 5:35- We're an epic shop, which, helps,
- 5:38- in several instances, including reducing resistance. Most folks
- 5:41- would prefer to be on epic given the
- 5:43- choice, but that that's not that common in
- 5:45- private practice.
- 5:46- So after
- 5:48- the the merger, we really had to focus,
- 5:50- build a plan, to bring them over onto
- 5:52- Epic. They are still on their current legacy
- 5:54- system, so their teams are are working both
- 5:57- sides of the fence right now. They're actively,
- 5:59- billing and then posting payments and doing all
- 6:01- of the revenue cycle activities in the legacy
- 6:03- system while they're working, to help us build,
- 6:07- an Epic module and systems that look that
- 6:09- work well for them.
- 6:11- You know, if we look back at lessons
- 6:12- learned, I think we we did a great
- 6:14- job of,
- 6:15- building integration teams, including detailed teams both for
- 6:18- finance and the revenue cycle, and that that
- 6:20- just served us incredibly well. It it brought
- 6:22- the teams together, one, to build the relationships
- 6:24- before it was time to do the work.
- 6:27- And now those teams continue, and I'm I'm
- 6:29- not sure. We may we may not discontinue
- 6:30- them at all. We may just continue those
- 6:32- into perpetuity,
- 6:33- to keep working on all the activities that
- 6:35- we wanted to, but merger,
- 6:37- really, really went well. That's incredible to hear.
- 6:40- Do you have any further plans in Monument
- 6:42- Health as you look, you know, towards even
- 6:44- next year for for further acquisitions or growth?
- 6:48- We're always paying attention.
- 6:49- We're we're a system that has historically grown
- 6:51- organically, so that's kind of our highest priority,
- 6:54- but we're certainly always open to options.
- 6:57- And then I suppose final question for you
- 6:59- here,
- 6:59- where do you see, you know, obviously AI
- 7:02- is such a hot topic. When I talk
- 7:03- to CFOs,
- 7:04- they're constantly saying, you know, they're keeping a
- 7:06- close eye on AI,
- 7:08- you know, investing in technologies,
- 7:10- just trying to be smart in that area
- 7:11- as well. But where do you see it
- 7:13- making a real near term difference in health
- 7:16- care finance and the revenue cycle, of course?
- 7:19- Could you share maybe where you think the
- 7:20- hype might still be outpacing this reality?
- 7:23- Sure. Sure. We've adopted several AI related projects.
- 7:26- We're we're not cutting edge. We're probably somewhere
- 7:28- in the middle seeking established technology.
- 7:31- You know, if we look back a few
- 7:32- years, even some basic things,
- 7:34- that we've adopted,
- 7:36- robotic process
- 7:38- organization,
- 7:38- where we've
- 7:39- taken payer portals, the numerous payer portals that
- 7:42- all of our teams have to log into
- 7:43- upload data or pull down data. We're able
- 7:46- to automate that saving several 100 hours. So
- 7:48- that's just one basic level. You know, other
- 7:50- successes we've had really in the ambulance listening
- 7:53- side for our physicians,
- 7:55- that has helped tremendously both from a documentation,
- 7:57- but also a workload perspective. Maybe the ROI
- 8:00- is not as high on that as originally
- 8:02- promised, but the reduction
- 8:04- and stress, for our providers has just been
- 8:06- tremendous.
- 8:08- We've also adopted,
- 8:09- some documentation,
- 8:11- AI tools,
- 8:12- particularly in the inpatient side with our hospitalists
- 8:14- that have paid big dividends. I think for
- 8:16- that, we actually probably did hit a a
- 8:18- two to three to one, ROI. So we're
- 8:20- incredibly excited about how those have gone.
- 8:24- We're also looking at rolling out, some autonomous
- 8:27- coding,
- 8:28- very short order. We're really excited about that.
- 8:30- We know coders are hard to come by,
- 8:33- and we wanna make sure we're coding all
- 8:34- claims accurately for a variety of reasons, not
- 8:37- the least of which is long term financial
- 8:39- stability. So we're excited for that as well
- 8:41- as rolling out our clinical documentation improvement areas,
- 8:45- some tools to enhance their work. You know,
- 8:47- we certainly would like to continue to build
- 8:49- out, that
- 8:50- program,
- 8:51- but it's you it leverages,
- 8:53- RNs to do the work in many cases.
- 8:54- And so we wanna be respectful and make
- 8:56- sure we're using their their efforts to the
- 8:58- top of their license and skill sets. So
- 9:00- we'll be rolling that out in short order.
- 9:02- And, you know, I think early indications are
- 9:04- that those are going well, and we are
- 9:06- excited to reap the rewards over the next
- 9:08- year. Yeah. No. It's incredible to hear all
- 9:11- the success, you know, with with these AI
- 9:13- tools, and so I appreciate you diving a
- 9:15- little bit deeper there.
- 9:17- When you think about Monument Health, even in
- 9:19- the next three to five years, what's that
- 9:21- look like to you? What what does success
- 9:22- mean to you right now, and how do
- 9:24- you plan to get there?
- 9:26- Sure. I mean, success for us is managing,
- 9:29- payer mix that is challenging, continuing to provide
- 9:31- rural health care. You know, we are excited
- 9:34- about the rural health transformation funds. We we've
- 9:36- got several projects and awards that are already
- 9:38- approved that we think will help drive the
- 9:40- health care system
- 9:41- a lot in the digital space, but drive
- 9:43- us forward over the next couple of years.
- 9:45- So we think we're through those funds and
- 9:47- also just the initial investments we're gonna make
- 9:50- in several other areas are are exciting and
- 9:52- and will lead us forward.
- 9:54- Well, I cannot wait to, you know, continue
- 9:56- following Monument Health's path and listening to all
- 9:58- the incredible things you had to say today.
- 10:00- I'm I'm very excited for you guys, and
- 10:02- I, look forward to connecting again sometime soon
- 10:04- to to chat about all your successes. So
- 10:06- thank you for being on, Austin.
- 10:08- Yeah. Thank you. Appreciate the time today.
