- 0:29Hello, and welcome to the Becker Payers Issues
- 0:30podcast
- 0:31recorded live at the third annual spring payer
- 0:33issues roundtable here in Chicago.
- 0:36I'm joined today by Mark Rich, until recently
- 0:38the president and current board member of Stewart
- 0:40Healthcare System.
- 0:42Mark, thank you so much for joining us
- 0:43today. Thank you. I appreciate the opportunity.
- 0:46Well, the pleasure is all ours. Would you
- 0:48please take a moment to introduce yourself and
- 0:49tell us a little bit about, what you
- 0:51do? Sure.
- 0:52Sure. So I'm a a health care executive
- 0:55with forty years of experience,
- 0:58thirty of it in c suites,
- 1:01but I'm from the provider side and the
- 1:03ACO side. So a little bit different view
- 1:05than some of the people you may have
- 1:06been talking to from the plan side. You
- 1:08know, I've served as a a part a
- 1:10partner in a firm and as the CFO,
- 1:13the chief administrative officer, the president of a
- 1:16of a number of providers,
- 1:18and, I'm kind of a turnaround crisis guy
- 1:21these days.
- 1:22Okay. So balancing affordability and quality is a
- 1:25constant challenge for health plans. How is your
- 1:28organization innovating to manage the cost of care
- 1:30while maintaining or improving member outcomes?
- 1:33That's a great question. I really appreciate it.
- 1:35And, you know, I could share from the
- 1:36provider side, the ACO side, what I think
- 1:39the the solution is.
- 1:40And I think we've heard about it at
- 1:42the conference today here and in the other
- 1:44side.
- 1:45So
- 1:46I think you have to define affordability
- 1:48and quality.
- 1:50I'm not sure we always do that. And
- 1:52and if does affordability mean,
- 1:54cheaper
- 1:55premium for the patient
- 1:57or the customer?
- 1:59And quality, as you know, is all performance
- 2:01metrics. It's rearview stuff. It's did you give
- 2:03a glucose test. It's not really outcome based,
- 2:06and that's because that's very difficult to do.
- 2:08I'm not it's what we've got, and we're
- 2:10working with it. But, if you could define
- 2:13those two and just put that aside,
- 2:15I'm a big
- 2:16believer in ACOs
- 2:17and,
- 2:18having come from just one of the biggest
- 2:20in the country.
- 2:21And
- 2:22I think that you have to
- 2:25answer this question in context of the overall
- 2:28strategy.
- 2:29And for me, our strategy has been value
- 2:32based care and embracing value based care. I've
- 2:34given it a big bear hug. I'm bullish
- 2:37on it.
- 2:38It it works. You know, it's gonna do
- 2:40$3,600,000,000,000
- 2:41over ten years in the total commercial space.
- 2:45It's a,
- 2:463.1
- 2:47to 5,000,000,000 in '23 just for Medicare,
- 2:50and that's the MSSP program, which is the
- 2:52hardest kind of of managed care in that
- 2:55bit in that side of the business, because
- 2:57you you it works on attribution and not
- 2:59as and not directly chosen PCP.
- 3:02So I think you have to embrace that.
- 3:03And then
- 3:04if you do that, then you need to
- 3:06think about the core things you need, and
- 3:08and ACOs are really simple. Once you have
- 3:10the physicians,
- 3:12you it's really IT,
- 3:14then it's data management,
- 3:16because you want to you wanna be able
- 3:18to use the data for predictive outcomes and
- 3:20everything that we're trying to do these days.
- 3:22So big data warehouse.
- 3:24And the thing you need for,
- 3:26an ACO
- 3:28or forming an ACO, value based care company's
- 3:30money. Mhmm. Mhmm. All those things cost a
- 3:32lot of money. We need to have reserves.
- 3:35I think there's plenty of money in the
- 3:36system right now. I just don't think it's
- 3:38evenly distributed. We could talk about that if
- 3:40we want. Mhmm. Yeah. Mhmm. So in an
- 3:42ever evolving regulatory
- 3:44landscape,
- 3:44what best practices or tools does your organization
- 3:47rely on to keep the quality of care
- 3:49at the forefront?
- 3:50Well,
- 3:52again, you know, I think quality metrics are
- 3:55performance metrics,
- 3:57so we measure those.
- 3:58We give feedback. We try to keep it
- 4:00simple.
- 4:01When we measure data,
- 4:04I'm a big believer that the only data
- 4:05that should be supplied to anybody in the
- 4:07organization
- 4:08is is actionable immediately actionable
- 4:11data that they could turn into information and
- 4:13move. And that could be because I'm from
- 4:15a crisis centered chaos centered world. Mhmm. But
- 4:19clouding everybody's inbox with a bunch of random
- 4:22data
- 4:23basically just gives people the
- 4:26opportunity to admire the problem. Mhmm. They're not
- 4:28really working. Right. So we wanna get the
- 4:30right data in the right hands. And so
- 4:32we measure all that. We measure quality. We
- 4:34measure performance up to that. We have, pharmacists
- 4:38that go to go to the home to
- 4:40make sure that the patients are taking their
- 4:41drugs, and they're not, you know, after the
- 4:43shoulder surgery, not up swinging a golf club
- 4:45two days later. Right. Right. So you try
- 4:47to get a lot of that in. And
- 4:49then,
- 4:50I think value based care becomes a big
- 4:53big way to do this. Mhmm. It it
- 4:55it clearly saves money. It clearly works.
- 4:58And the beauty of it is better for
- 5:00the patient. Mhmm. I mean, if you think
- 5:01about it,
- 5:02value based care, we make a big deal
- 5:04about it. But if you think about it,
- 5:06it's what the patient thinks is happening anyway.
- 5:08Right? They think that you're the PCP. Right.
- 5:10You're talking to me, this surgeon, and giving
- 5:13me notes because Right. I'm getting personalized care.
- 5:16And we know that that is difficult to
- 5:18do, and it doesn't really happen
- 5:20until you're able to,
- 5:22get a standard EMR and bring people under
- 5:24the umbrella. Mhmm. And you have to you
- 5:26have to line the economics or they won't
- 5:28come under the umbrella, and that's really the
- 5:30key. Right. Well, looking ahead, what do you
- 5:32see as the biggest opportunity for payers to
- 5:34lead the charge in transforming health care delivery
- 5:36and driving better outcomes, for all stakeholders?
- 5:39And how can leaders take a step in
- 5:41this direction right now? I think I think
- 5:43commercial insurers,
- 5:45can play a big part, in two ways.
- 5:47One, they need they need to take a
- 5:49look at their systems, and they need to
- 5:50embrace value based care a little bit. We've
- 5:52run into problems as Stewart went across the
- 5:54country
- 5:55with with the value based care that that
- 5:57we were ready. We had the infrastructure and
- 5:59the ACO in place, but we didn't have
- 6:01an insurance partner who had the IT infrastructure
- 6:04to handle that. And it's unique, and you
- 6:06gotta you're gonna have to make the the,
- 6:08you know, the investment, and it's a big
- 6:10number. But I think, that's where we're going.
- 6:13There's 608,000
- 6:15clinicians in value based care right now,
- 6:17and it's growing at an 8% CAGR over
- 6:20the next ten years. So Mhmm. It's gonna
- 6:22be big, and it's gonna be here.
- 6:24But,
- 6:25the cash is out there. I think we
- 6:27need to look at relook at risk based
- 6:29capital, which I know is the rail the
- 6:31rail.
- 6:32But when you have 800%
- 6:34of the minimum required
- 6:36minimum required risk based capital
- 6:39and you're a nonprofit plan, like a Blue
- 6:41Cross plan, it's not tax paying, I mean,
- 6:43isn't that just isn't that just excess premium
- 6:45that people have paid to you over the
- 6:46years that you maybe could have lowered the
- 6:49rate if you invested that capital?
- 6:51So there's plenty of cash there. There's a
- 6:53trillion dollars sitting in private equity on the
- 6:55sideline. Now not all that's for health care,
- 6:57but that's a big pretty big pool to
- 6:59choose up to pull pull from. And the
- 7:02government is gonna continue to make investments in
- 7:05value based care. Medicare is is all of
- 7:07this. I think the I think the new
- 7:09secretary,
- 7:11even had meeting special meetings on value based
- 7:12care, like, last week as soon as it
- 7:14works out again.
- 7:16Well, Mark, I really appreciate you, taking the
- 7:18time to join us today on the Becker's
- 7:19Perry issue podcast.
- 7:21Before I let you go, do you have
- 7:22any final thoughts you'd like to leave with
- 7:23our listeners? I think that
- 7:25if I had a final thought, it would
- 7:27be the solution to this problem and really
- 7:29bending the cost curve is having to work
- 7:31together. Mhmm. Payer and provider. That's been a
- 7:34very big success for Stewart with Blue Cross
- 7:36of Mass. Started that in 02/2010.
- 7:39That's where the money's gonna come from. That's
- 7:41where we can think about capital. We can
- 7:43think about new capital structure.
- 7:45And I don't I'm not a big provider
- 7:47insurer kind of base guy. I think providers
- 7:50do what they do, and insurers do what
- 7:52they do. And never the twain shall meet,
- 7:55but Mhmm. There is a good opportunity to,
- 7:57work together. Right. Right. Well, Mark, thank you
- 8:00again for joining us today, and I hope
- 8:02you have a lovely rest of your day.
- 8:03Thank you. I appreciate it. My pleasure.
