- 0:00Hi everyone.
- 0:00Welcome to Eye
- 0:01on Research
- 0:022024.
- 0:03This is our

- 0:08thanks to
- 0:09our sponsors.
- 0:10We are,
- 0:10sponsored by
- 0:11Castle
- 0:11Biosciences
- 0:12as well
- 0:12as IDEAYA
- 0:13Biosciences,
- 0:14Aura
- 0:15Biosciences
- 0:15TriSalus
- 0:16Life Sciences,
- 0:17Immunocore,
- 0:18Delcath
- 0:19and Replimune.
- 0:20So thank you
- 0:20to our
- 0:20pharmaceutical ,Doctor Correa
- 0:22I thank you
- 0:22so much
- 0:22for joining us
- 0:23today.
- 0:23We're so glad
- 0:24to have you
- 0:24and so excited
- 0:25to hear
- 0:25about some of
- 0:26the primary
- 0:26research
- 0:26updates for,
- 0:27primary ocular
- 0:28melanoma.
- 0:29So thanks
- 0:29for joining
- 0:30us.
- 0:31Well,
- 0:31thank you
- 0:31so much
- 0:32for this
- 0:32opportunity.
- 0:33it's really
- 0:34great
- 0:34to interact
- 0:35with this
- 0:35group.
- 0:36You know,
- 0:36so many
- 0:37patients,
- 0:38family members
- 0:39and loved ones
- 0:40that are
- 0:40really
- 0:40interested
- 0:41and involved
- 0:42not only
- 0:43in the care
- 0:44of patients,
- 0:44but also
- 0:45in the
- 0:46advancements
- 0:47of this area.
- 0:48So that
- 0:49brings me
- 0:49great joy
- 0:50to be part of.
- 0:50So I'm
- 0:51going to
- 0:51go ahead
- 0:52and I've just
- 0:53prepared a
- 0:53couple slides
- 0:54just to kind
- 0:55people
- 0:55can follow
- 0:56what I'm
- 0:56talking about
- 0:57and not be
- 0:58a little bit
- 0:59lost in
- 1:00all of this
- 1:00information.
- 1:01I am,
- 1:02an
- 1:02investigator
- 1:03for Castle,
- 1:04IDEAYA,
- 1:04and Aura
- 1:05Biosciences.
- 1:06What I'm here
- 1:06to do
- 1:07today
- 1:07is to talk
- 1:08about
- 1:08general ocular
- 1:09melanoma
- 1:10updates
- 1:11and touch
- 1:11a little bit
- 1:12about
- 1:12potential new
- 1:13developments
- 1:13in neoadjuvant
- 1:14treatments.
- 1:15And finally,
- 1:16update
- 1:16everybody on
- 1:17the radiation
- 1:18retinopathy
- 1:19trial.
- 1:19That's called
- 1:20the protocol
- 1:20AL which
- 1:21is sponsored
- 1:22by the what
- 1:23we call
- 1:23the DRCR Net,
- 1:25which is a
- 1:25diabetic
- 1:26retinopathy
- 1:27network, and
- 1:28and they
- 1:29started out
- 1:30looking
- 1:30at diabetes,
- 1:31and now
- 1:31they have
- 1:32verged into a
- 1:33lot of blood
- 1:33vessel
- 1:34diseases of
- 1:35the retina,
- 1:35and now
- 1:36including
- 1:37radiation
- 1:38retinopathy.
- 1:39So before
- 1:40I get started,
- 1:41I just want
- 1:41to share
- 1:41some wonderful
- 1:42news,
- 1:43is that
- 1:43finally,
- 1:44our paper,
- 1:45sharing the,
- 1:46kind of
- 1:47the validation
- 1:48of the 15
- 1:49gene
- 1:49expression
- 1:50profile
- 1:50and PRAME
- 1:51as an
- 1:52integrated
- 1:52prognostic
- 1:53test for uveal
- 1:54melanoma,
- 1:55was just
- 1:56published
- 1:57the day
- 1:57before
- 1:57yesterday.
- 1:58So,
- 1:58you can see
- 1:59here we're all
- 2:00very excited.
- 2:01This is the first
- 2:02paper
- 2:02by the COOG
- 2:03Two group,
- 2:04and I'm sure
- 2:05when you
- 2:05all browse,
- 2:06you will
- 2:07probably see
- 2:08your ocular
- 2:09oncologist
- 2:09involved
- 2:10there.
- 2:10It was a big,
- 2:11effort on
- 2:12all of us
- 2:13to get
- 2:14this
- 2:14information.
- 2:15We enrolled
- 2:161500 patients,
- 2:17and
- 2:18it really provides
- 2:19now
- 2:20a good
- 2:20framework
- 2:21and a good,
- 2:21valid,
- 2:22prospectively
- 2:23validated
- 2:24framework,
- 2:24and in
- 2:25order to
- 2:26kind of
- 2:26come up with
- 2:28studies
- 2:29going forward,
- 2:29enrollment
- 2:30for adjuvant
- 2:31treatments,
- 2:32etc..
- 2:34So what
- 2:35are the
- 2:35current
- 2:35clinical
- 2:36trials
- 2:36for primary
- 2:37posterior
- 2:37uveal
- 2:38melanoma?
- 2:38Well,
- 2:38first of all,
- 2:39the IDE196.
- 2:42The IDEAYA
- 2:42trial
- 2:43for localized
- 2:44uveal
- 2:44melanoma,
- 2:45the Aura
- 2:47011 trial
- 2:48for the
- 2:49intermediate
- 2:50melanocytic
- 2:51lesions
- 2:51or small
- 2:52choroidal
- 2:52melanomas.
- 2:53There's a
- 2:54talk about
- 2:55a neoadjuvant
- 2:55tebentafusp,
- 2:56for uveal
- 2:57melanoma
- 2:58that is not
- 2:58yet
- 2:58recruiting.
- 2:59And then
- 3:00there's
- 3:00a new player
- 3:01which is the
- 3:02Sitravatinib
- 3:04it's
- 3:04the Sitra,
- 3:05which can be a
- 3:06potential
- 3:06future target.
- 3:08Right now,
- 3:08they're using
- 3:09that as
- 3:10a target
- 3:11for other
- 3:11cancers
- 3:12like lung
- 3:12cancer.
- 3:13They didn't
- 3:13have such
- 3:14a good result,
- 3:15but this
- 3:16medication,
- 3:16probably
- 3:17associated
- 3:18with another
- 3:19medication
- 3:20may be
- 3:20a good
- 3:21potential
- 3:21future target.
- 3:22I'm sure
- 3:23they will
- 3:23look at it
- 3:24first
- 3:24for metastatic
- 3:25disease,
- 3:26and I know
- 3:27that
- 3:27we'll have
- 3:27Doctor
- 3:28Carvajal
- 3:28and his team
- 3:29talking
- 3:29about it
- 3:30a little bit
- 3:30later,
- 3:31but definitely
- 3:32something
- 3:32to be
- 3:32on the lookout.
- 3:33And then
- 3:34finally,
- 3:34I'm going
- 3:34to talk
- 3:35a little bit
- 3:35about that
- 3:36protocol, AL,
- 3:37which is the
- 3:37Intravitreal
- 3:38Faricimab
- 3:39or the
- 3:39Fluocinolone
- 3:41Intravitreal
- 3:42vs Observation
- 3:44to prevent
- 3:44vision loss
- 3:45after
- 3:46radiation
- 3:46treatment.
- 3:47So I know
- 3:49the idea
- 3:50team is going
- 3:50to be
- 3:50talking about
- 3:51their product
- 3:52and
- 3:52what their findings
- 3:53are so far,
- 3:54but I just
- 3:54want to
- 3:55kind of bring
- 3:56to all of you
- 3:57the potential
- 3:57impact
- 3:58of this
- 3:59medication
- 4:00in this
- 4:00treatment.
- 4:01If it proves
- 4:02to be
- 4:02successful,
- 4:03as it's
- 4:03promising to,
- 4:04number one
- 4:05is that
- 4:06possibly
- 4:07will improve
- 4:08lobe salvage
- 4:09tremendously
- 4:10and as
- 4:11a result,
- 4:12limit vision
- 4:13loss of
- 4:13patients
- 4:14associated
- 4:15with radiation
- 4:16treatment,
- 4:16and that
- 4:17will have
- 4:17a huge impact
- 4:18in quality
- 4:19of life
- 4:19and especially
- 4:20for survivors
- 4:21going forward.
- 4:22It could
- 4:23change
- 4:23the systemic
- 4:24outcomes of
- 4:25these patients
- 4:25because
- 4:26targeting
- 4:26GNA11,
- 4:27which
- 4:28we know it is
- 4:29an upstream
- 4:30mutation,
- 4:31it could
- 4:32really improve
- 4:33the rates
- 4:34or decrease
- 4:35the rate of
- 4:36metastatic disease.
- 4:37So it would
- 4:38work
- 4:38both ways.
- 4:39It would be a
- 4:40primary
- 4:40treatment,
- 4:41and
- 4:41it would be
- 4:41also a
- 4:42new adjuvant
- 4:43treatment
- 4:43in some ways.
- 4:44And its
- 4:45downstream
- 4:46effect
- 4:46could
- 4:46potentially
- 4:47improve
- 4:48patients
- 4:48survival, as
- 4:49I mentioned.
- 4:50The other one
- 4:51that we're
- 4:52excited about
- 4:53is the
- 4:53neoadjuvant
- 4:54Tebentafusp
- 4:55we know that
- 4:56the
- 4:56Tebentafusp
- 4:57or Kimmtrak,
- 4:58has been
- 4:58used currently
- 5:00for metastatic
- 5:01disease,
- 5:02but
- 5:02the company
- 5:03is now
- 5:03trying to
- 5:04think about it
- 5:05as a primary
- 5:07neoadjuvant
- 5:07treatment.
- 5:08And you know,
- 5:09and this
- 5:10is something
- 5:11that will
- 5:11probably be
- 5:12launched here
- 5:13in the near
- 5:13future.
- 5:14These are the
- 5:14enrollment
- 5:15criterias,
- 5:15as you see,
- 5:16they're going
- 5:16to start
- 5:17with very
- 5:17large tumors
- 5:18that
- 5:19potentially
- 5:19could not
- 5:20be treated
- 5:21other than
- 5:22by enucleation
- 5:23and they will
- 5:24obviously make
- 5:25sure the
- 5:25patients are
- 5:26all HLA
- 5:27A0201 positive
- 5:29that we know
- 5:29only 50%, give
- 5:31or take, of
- 5:31the population
- 5:32are positive.
- 5:33and they want
- 5:34to enroll
- 5:35as they are
- 5:36validating
- 5:36this potential
- 5:37trial.
- 5:38All the patients
- 5:38that are
- 5:39in good
- 5:39health
- 5:40condition,
- 5:40so that's
- 5:41one thing
- 5:41I tell
- 5:42all of
- 5:42my patients.
- 5:42I always say
- 5:43stay healthy.
- 5:44That's what
- 5:44you can do
- 5:45to improve
- 5:46your odds
- 5:46and whatever
- 5:47we do
- 5:47going forward.
- 5:48And also
- 5:50they want
- 5:50to make sure
- 5:51this person
- 5:51will be able
- 5:52to adhere
- 5:53to the
- 5:53treatment,
- 5:54etc.
- 5:55and as you
- 5:55all recall,
- 5:56this is
- 5:56the data
- 5:57for the
- 5:57Tebentafusp
- 5:58metastatic
- 5:59uveal
- 5:59melanoma
- 6:00setting.
- 6:00And we just
- 6:01think that
- 6:02this
- 6:02could have
- 6:02another
- 6:03huge impact
- 6:04and possibly
- 6:05improving
- 6:05globe salvage,
- 6:06possibly
- 6:07limiting
- 6:08the vision
- 6:08loss
- 6:08associated
- 6:09with the
- 6:10radiation
- 6:10treatment
- 6:11and
- 6:11the downstream
- 6:12effect
- 6:12could
- 6:13potentially
- 6:14improve
- 6:14patient
- 6:14survival
- 6:15and then
- 6:16the question
- 6:16is,
- 6:17will there
- 6:18be selection
- 6:18bias by only
- 6:20using
- 6:20tumor size
- 6:21instead of
- 6:21genomic
- 6:22profile.
- 6:23And this
- 6:23has been
- 6:23something
- 6:24that,
- 6:24you know,
- 6:25this company
- 6:26has been
- 6:26very honed in
- 6:27to other
- 6:29things
- 6:29that are not
- 6:29the genomic
- 6:30profile.
- 6:30Now that
- 6:31we've had
- 6:32the GEP
- 6:32plus Prame
- 6:33validated,
- 6:35it would
- 6:35be kind
- 6:35of interesting
- 6:36to see
- 6:37how
- 6:37it plays out
- 6:38as far as
- 6:40the validation
- 6:41and its usage
- 6:42going forward.
- 6:43Then, the Aura
- 6:44Bioscience
- 6:45trial,
- 6:46this is
- 6:46something that
- 6:47I know
- 6:47Doctor Hopkins
- 6:48will be
- 6:48talking
- 6:49about here
- 6:49pretty soon.
- 6:50I think
- 6:51the exciting
- 6:51thing is that
- 6:52this is
- 6:52a phase
- 6:53three
- 6:53randomized
- 6:54trial.
- 6:55And, you know,
- 6:55we will be
- 6:56seeing how
- 6:57these
- 6:58patients then
- 6:58behave
- 6:59compared to
- 7:00no treatment.
- 7:01I believe that
- 7:02the new
- 7:03suprachoridal
- 7:04injection
- 7:04will
- 7:05definitely
- 7:05optimize
- 7:06the drug
- 7:06delivery,
- 7:07and I'm
- 7:07excited to see
- 7:08these results.
- 7:09And,
- 7:09this is
- 7:10the phase
- 7:10three study
- 7:11schema.
- 7:11The only thing
- 7:13is that
- 7:13it's a very
- 7:14intense
- 7:14treatment.
- 7:15It really
- 7:15needs a buy
- 7:16in from
- 7:16the patient.
- 7:17And
- 7:18understanding
- 7:19that,
- 7:19that you
- 7:19may not
- 7:20be randomized
- 7:21to
- 7:21get the treatment.
- 7:22So you may
- 7:23be randomized
- 7:24to get
- 7:24observation,
- 7:26but
- 7:26obviously you
- 7:27will be taken
- 7:28out of
- 7:28the group
- 7:29of
- 7:29observation.
- 7:30If the tumor
- 7:32keeps on
- 7:32growing
- 7:33and then
- 7:33you will get
- 7:34the radiation
- 7:35or whatever
- 7:35else treatment
- 7:36your physician
- 7:37deems
- 7:37appropriate.
- 7:38Just letting
- 7:39you know that,
- 7:39you know, the,
- 7:40the inclusion
- 7:41criteria
- 7:42is more
- 7:42based on
- 7:43growth
- 7:44and this is
- 7:44something that
- 7:45in the past,
- 7:46I have to
- 7:46confess ,that
- 7:47I was personally
- 7:48a little
- 7:48bit against
- 7:49just having
- 7:49that as
- 7:50a, criteria,
- 7:51but
- 7:52the company
- 7:52is giving
- 7:53us now
- 7:53some freedom
- 7:54to do biopsy
- 7:55the patients
- 7:56before
- 7:57enrolling
- 7:57them,
- 7:58and
- 7:58I think that
- 7:59that will
- 7:59really make
- 8:00the data
- 8:01way more
- 8:01robust.
- 8:02Having that
- 8:03information
- 8:04again,
- 8:05this just
- 8:06kind of shows
- 8:06that not
- 8:07all tumors
- 8:07are created
- 8:08equal
- 8:08and not all
- 8:09patients
- 8:09are equal.
- 8:10So,
- 8:10we have to account
- 8:11for that
- 8:12and
- 8:12I think that
- 8:13that will
- 8:13be really
- 8:14a game changer
- 8:15for
- 8:15those folks
- 8:16that have very
- 8:17small tumors
- 8:18in a location
- 8:19that radiation
- 8:20can be
- 8:20potentially
- 8:21damaging.
- 8:21So, stay tuned
- 8:23because this
- 8:23is going
- 8:24to be
- 8:24very exciting.
- 8:25So, potential
- 8:26impact of
- 8:27this trial
- 8:27is going to be
- 8:28the early
- 8:28treatment of
- 8:30small
- 8:30posterior
- 8:31tumors
- 8:31showing
- 8:31progression
- 8:32over a short
- 8:33period of
- 8:33time, expected
- 8:35low risk
- 8:35for vision
- 8:36loss,
- 8:36and possibly
- 8:38mitigating
- 8:39or eliminating
- 8:40the radiation.
- 8:41But the
- 8:42possibility
- 8:42of radiation
- 8:43if the
- 8:44treatment
- 8:44fails, or
- 8:45if
- 8:45the observation,
- 8:46deems that,
- 8:48you know,
- 8:48the tumor
- 8:49continues
- 8:49to grow.
- 8:50The question
- 8:51is, again,
- 8:52are we testing
- 8:53the efficacy
- 8:53of the
- 8:53treatment
- 8:54for small
- 8:54melanomas or
- 8:55large nevi
- 8:56and
- 8:57I think
- 8:57that now
- 8:57that
- 8:58the company
- 8:58is allowing us
- 8:59to biopsy
- 9:00the patients
- 9:01before
- 9:01enrolling
- 9:02them,
- 9:02I think that
- 9:03will be really
- 9:04a game changer
- 9:05and it's going
- 9:05to make
- 9:06that data
- 9:06a lot
- 9:07more robust.
- 9:08Finally,
- 9:09the protocol
- 9:09AL,
- 9:10the protocol
- 9:11AL by the DRCR
- 9:12Net is
- 9:13going to
- 9:13be very
- 9:14impactful.
- 9:15They're going
- 9:15to test
- 9:16what we call
- 9:16the
- 9:16intravitreal
- 9:17faricimab
- 9:18which is
- 9:19already
- 9:19commercially
- 9:20available
- 9:20as Vabysmo.
- 9:21This medication
- 9:23has been
- 9:23approved
- 9:24for age
- 9:25related
- 9:25macular
- 9:26degeneration,
- 9:27as well
- 9:27as the
- 9:28Fluocinolone
- 9:29acetonide
- 9:29implant, which
- 9:31is called
- 9:31lluvien also
- 9:34by another
- 9:34company, but
- 9:35also
- 9:36already
- 9:36available
- 9:37commercially.
- 9:37So
- 9:38we know both
- 9:39drugs
- 9:39are safe.
- 9:41and then
- 9:42the third
- 9:42group would be
- 9:43the
- 9:43observation
- 9:44for the
- 9:44prevention of
- 9:45visual
- 9:45acuity loss
- 9:46and so
- 9:47they want
- 9:47to see
- 9:48how does
- 9:48that impact
- 9:49the blood
- 9:50vessels
- 9:50around the
- 9:51radiated area.
- 9:52How does that
- 9:52impact
- 9:53the occurrence
- 9:55of blood
- 9:56vessel events,
- 9:57you know,
- 9:57like
- 9:57clogging up
- 9:58the veins
- 9:58and the
- 9:59arteries
- 9:59as a result
- 10:00of the
- 10:00radiation?
- 10:03It's going
- 10:03to be a very,
- 10:05interesting
- 10:06trial
- 10:06because
- 10:07right now
- 10:07we don't have
- 10:08any FDA
- 10:09approved
- 10:10medication
- 10:10for radiation
- 10:11retinopathy,
- 10:12and many of us
- 10:13do use
- 10:14Avastin.
- 10:15Sometimes,
- 10:16Eylea,
- 10:17but we have
- 10:17to jump
- 10:18through a lot
- 10:18of hoops
- 10:19to be able
- 10:19to approve it
- 10:20for
- 10:20the patient,
- 10:21especially if
- 10:22it's a
- 10:22younger
- 10:22patient
- 10:23that doesn't
- 10:24have any
- 10:24other diseases
- 10:25or
- 10:25comorbidities.
- 10:27So having
- 10:27that data
- 10:28will
- 10:28really push
- 10:29the FDA
- 10:30to approve
- 10:31certain
- 10:31medications
- 10:32directly
- 10:33for radiation
- 10:34retinopathy,
- 10:34and we're very
- 10:35excited
- 10:36about that.
- 10:37Here's just
- 10:38the things
- 10:38that we'll
- 10:39be doing,
- 10:39the standard,
- 10:41visual acuity,
- 10:42lab exam,
- 10:43etc., etc,
- 10:44and we will be
- 10:45monitoring
- 10:46patients
- 10:46every three
- 10:47months.
- 10:47We, the group
- 10:49that gets
- 10:49injections
- 10:50will get
- 10:50injections
- 10:51every three
- 10:51months,
- 10:52and the group
- 10:52with the
- 10:53implant
- 10:53will get
- 10:54one implant
- 10:55every
- 10:5524 months.
- 10:56And, the
- 10:57observation
- 10:57group
- 10:58obviously
- 10:59is not going
- 10:59to get
- 11:00any treatment.
- 11:01The downside
- 11:02of this study,
- 11:03I have to
- 11:03alert
- 11:03everybody,
- 11:04is that
- 11:04they are not
- 11:05going to be
- 11:06enrolling
- 11:06any patients
- 11:07with tumors
- 11:08around
- 11:08the macula,
- 11:09so around the
- 11:10focusing point
- 11:11and around
- 11:12the nerve,
- 11:13still, it's
- 11:14going to
- 11:14be useful
- 11:15information.
- 11:16It's not
- 11:16going to be
- 11:17as impactful
- 11:18as we would
- 11:19like,
- 11:19but
- 11:20I think
- 11:20the the
- 11:21idea behind
- 11:22the protocol
- 11:23would be
- 11:23some very
- 11:25striking
- 11:25measurable
- 11:26outcomes,
- 11:27and that is
- 11:28the only way
- 11:29to avoid
- 11:30the bias
- 11:31of the
- 11:32tumor
- 11:33location.
- 11:33So that's
- 11:34that's the
- 11:35whole
- 11:35catch here.
- 11:36So
- 11:38the impact
- 11:38of this
- 11:39protocol
- 11:39will be
- 11:39providing
- 11:40insight
- 11:41into the
- 11:41benefit
- 11:42of
- 11:42preventative
- 11:42medication
- 11:43to avoid
- 11:44or minimize
- 11:44radiation
- 11:45retinopathy.
- 11:46And because
- 11:47of that,
- 11:47improving
- 11:48patient
- 11:49visual
- 11:49outcomes.
- 11:50And hopefully
- 11:51quality
- 11:52of life,
- 11:53especially for
- 11:54those
- 11:54surviving
- 11:55the cancer,
- 11:55determine
- 11:56the success
- 11:57of the
- 11:57treatment
- 11:58for radiation
- 11:58retinopathy
- 11:59and determine
- 12:00the frequency
- 12:00of treatment
- 12:01that
- 12:01right now,
- 12:02you know,
- 12:03we treat
- 12:03kind of
- 12:04by year,
- 12:05not really
- 12:06by year,
- 12:06but by
- 12:07experience
- 12:08and by feeling
- 12:09for how
- 12:10the patient
- 12:11is
- 12:11coming along.
- 12:12But
- 12:12we definitely
- 12:13need
- 12:13a protocol,
- 12:14something
- 12:14that can be followed
- 12:15in, something
- 12:16that like,
- 12:16as I said,
- 12:17can be used
- 12:18to, get
- 12:19insurance
- 12:20approval.
- 12:21So
- 12:21I believe that
- 12:22that's
- 12:22basically
- 12:23my message.
- 12:24Over
- 12:24a century,
- 12:25we had
- 12:25no one change
- 12:26the outcomes
- 12:27of patients
- 12:27with uveal
- 12:28melanoma,
- 12:29and we had
- 12:29limited sight
- 12:30saving
- 12:31therapies
- 12:31and now we're
- 12:32seeing a
- 12:32new era.
- 12:33And it's just
- 12:34so exciting
- 12:35and I think
- 12:35these webinars
- 12:36are very,
- 12:37very helpful.
- 12:38So everybody
- 12:39can stay
- 12:39tuned
- 12:39and understand
- 12:40what's
- 12:40coming down
- 12:41the pipeline,
- 12:42especially
- 12:42for the
- 12:43clinical
- 12:43studies.
- 12:44These clinical
- 12:45trials
- 12:46will
- 12:46likely change
- 12:47how we think
- 12:47when we manage
- 12:48patients,
- 12:48and also
- 12:50will change
- 12:50how patients
- 12:51do after
- 12:52treatment in
- 12:53the long term
- 12:54and this is
- 12:54the first step
- 12:55to improve
- 12:56survival
- 12:56of patients
- 12:57and improve
- 12:57quality
- 12:58of life
- 12:59and their
- 12:59visual
- 13:00outcomes.
- 13:00Thank you
- 13:01so much.
- 13:02We are
- 13:02so thrilled
- 13:03to have
- 13:03you here
- 13:03today, Dr Cole
- 13:04thanks for
- 13:05being here.
- 13:06Well,
- 13:06thank you
- 13:06very much,
- 13:07and I
- 13:07appreciate the
- 13:07opportunity
- 13:08to present
- 13:08what we're
- 13:09doing at
- 13:10IDEAYA
- 13:10Biosciences
- 13:11for uveal
- 13:12melanoma.
- 13:13So
- 13:13this is our Safe
- 13:14Harbor
- 13:14Statement.
- 13:14Just to let
- 13:15you know that
- 13:15everything
- 13:15that I'm about
- 13:16to discuss
- 13:17is
- 13:17investigational,
- 13:18and
- 13:19our molecule
- 13:20is not yet FDA
- 13:21approved for
- 13:22marketing.
- 13:24So I briefly
- 13:25just wanted
- 13:25to,
- 13:26show everyone
- 13:26our pipeline,
- 13:27and the focus
- 13:28of my talk
- 13:28today will be
- 13:29this top part
- 13:30here,
- 13:30Darovasertib.
- 13:31This is what
- 13:32we are
- 13:32developing for
- 13:33uveal
- 13:33melanoma,
- 13:34but we do have
- 13:35a robust
- 13:35pipeline
- 13:36of other
- 13:36molecules
- 13:37that we are
- 13:38developing,
- 13:39for other
- 13:39malignancies.
- 13:42Very quickly.
- 13:43Darovasertib,
- 13:44how does
- 13:45Darovasertib
- 13:46work?
- 13:46As we know
- 13:47most uveal
- 13:48melanoma
- 13:48is driven
- 13:49by mutations
- 13:50in these two
- 13:50genes over
- 13:51here,
- 13:51GNAQ and/or
- 13:53GNA11,
- 13:54and these
- 13:54genes
- 13:55with their
- 13:56mutations
- 13:56propagate
- 13:57the signal
- 13:58through PKC,
- 14:00through RAF
- 14:01and through
- 14:01MEK,
- 14:02which produces
- 14:03our tumor,
- 14:03which produces
- 14:04our
- 14:04malignancy.
- 14:05Darovasertib
- 14:06stops that
- 14:07signaling
- 14:07or inhibits
- 14:08PKC right
- 14:09here,
- 14:10preventing
- 14:10that signaling
- 14:11from moving
- 14:12forward
- 14:12and also
- 14:13preventing,
- 14:14growth of
- 14:14the tumor.
- 14:17So our
- 14:17first study in
- 14:18terms of long
- 14:19primary
- 14:20uveal
- 14:21melanoma,
- 14:22we're looking
- 14:23at
- 14:23Darovasertib
- 14:23in the
- 14:24neoadjuvant
- 14:24setting,
- 14:25Darovasertib
- 14:26in the
- 14:26adjuvant
- 14:26setting,
- 14:27and a new
- 14:28protocol
- 14:29amendment for
- 14:30this trial
- 14:31is looking
- 14:31at the
- 14:32combination
- 14:32of
- 14:32Darovasertib
- 14:33plus
- 14:34Crizotinib
- 14:34in the
- 14:35adjuvant
- 14:35setting.
- 14:38So here's
- 14:38a schema
- 14:39of our trial,
- 14:40and I would
- 14:40say that
- 14:40the overall
- 14:41goal of our
- 14:42trial,
- 14:42we have a few of them
- 14:44But we're
- 14:44divided up
- 14:45into sort of
- 14:45two cohorts
- 14:46if you would.
- 14:47There is the
- 14:47enucleation
- 14:48cohort.
- 14:49The goal
- 14:49of that
- 14:49cohort,
- 14:50to see
- 14:50if
- 14:51Darovasertib
- 14:51can save
- 14:52the eye,
- 14:53the
- 14:53brachytherapy
- 14:54cohort,
- 14:56the idea
- 14:56being
- 14:57Darovasertib
- 14:58can reduce
- 14:59the amount
- 14:59of
- 14:59brachytherapy
- 15:00that's given
- 15:00to the eye.
- 15:01And then,
- 15:02adjuvant
- 15:02setting sort
- 15:03of afterwards,
- 15:03as we say,
- 15:04to save lives.
- 15:06So our
- 15:06primary
- 15:07endpoints
- 15:07in this phase
- 15:08one two trial
- 15:09and phase
- 15:09two trial,
- 15:10eye
- 15:10preservation,
- 15:11decrease
- 15:12in radiation
- 15:13dose,
- 15:13and the
- 15:14secondary
- 15:14endpoints,
- 15:15useful vision
- 15:16meaning
- 15:16improving the
- 15:18visual acuity
- 15:19or not,
- 15:20loss of
- 15:21visual acuity.
- 15:22And I was
- 15:22also seen in
- 15:23relapse free
- 15:24survival.
- 15:27So in our
- 15:27Darovasertib
- 15:29company
- 15:29sponsored
- 15:29trial,
- 15:31very,
- 15:31very
- 15:31preliminary
- 15:32results here,
- 15:32I'm able
- 15:33to present
- 15:33to you,
- 15:34as everyone
- 15:35knows, there
- 15:36a couple of
- 15:36different ways
- 15:36in which
- 15:37we can
- 15:37measure,
- 15:38decrease
- 15:38in size
- 15:39and tumor,
- 15:40whether it be
- 15:40a local high
- 15:41basal diameter
- 15:42and, or
- 15:42the volume,
- 15:43I'm happy
- 15:44to say.
- 15:44And,
- 15:45our initial
- 15:46look at our
- 15:46data,
- 15:47when we look
- 15:47at eight
- 15:47patients
- 15:48who received
- 15:49greater
- 15:50than,
- 15:50or equal
- 15:51to four months
- 15:52of treatment,
- 15:53what
- 15:53stimulated
- 15:54Darovasertib
- 15:54as we see
- 15:55over here,
- 15:56the majority
- 15:56of these
- 15:57patients,
- 15:57we have
- 15:57a median
- 15:58reduction,
- 15:58when we
- 15:59look at this
- 15:59by volume
- 16:00about 72%.
- 16:02And we look at
- 16:02the light
- 16:03blue,
- 16:03these are
- 16:04enucleation
- 16:04patients.
- 16:05We look at
- 16:05the darker
- 16:06blue here,
- 16:07this is our
- 16:07plaque
- 16:08eligible
- 16:08patient.
- 16:09These patients
- 16:10have had
- 16:10a major
- 16:11reduction
- 16:12in the size
- 16:12of their tumors
- 16:13and we take
- 16:14a pause here
- 16:14for a second.
- 16:15The idea
- 16:15behind
- 16:16that is
- 16:17a patient
- 16:17who
- 16:17potentially
- 16:18was scheduled
- 16:19for
- 16:19enucleation
- 16:20now doesn't
- 16:20need
- 16:21enucleation,
- 16:22patient who
- 16:23is scheduled
- 16:23for plaque
- 16:24brachytherapy,
- 16:25Now be getting
- 16:26a smaller
- 16:27amount of that
- 16:27plaque
- 16:28brachytherapy
- 16:29and we'll have
- 16:29some examples
- 16:30of that
- 16:30moving
- 16:30forward.
- 16:32So if we take
- 16:33a look
- 16:33at a couple
- 16:33of case
- 16:34studies now
- 16:35a 50 year
- 16:35old patient
- 16:37uveal
- 16:37melanoma,
- 16:38receiving
- 16:38four months
- 16:39of
- 16:40Darovasertib
- 16:40treatment
- 16:41in a dome
- 16:41shaped tumor.
- 16:43If we look
- 16:43over here
- 16:44at the
- 16:44baseline,
- 16:44their baseline
- 16:45measurements
- 16:46about five
- 16:46millimeters
- 16:47in April,
- 16:47height
- 16:48almost 15mm
- 16:49in the
- 16:50basal
- 16:50diameter.
- 16:51And their
- 16:51volume is
- 16:52shown below.
- 16:53When we look,
- 16:54when we
- 16:54took our
- 16:55measurements,
- 16:56pretty
- 16:56dramatic
- 16:57decrease
- 16:57in the size
- 16:58of their tumor
- 16:58with their
- 16:59apical height,
- 17:00basal
- 17:00diameter,
- 17:01and/or
- 17:01the volume,
- 17:02you know,
- 17:03north of
- 17:0340% decrease
- 17:04or upwards
- 17:06of the greater
- 17:06than 80%
- 17:07decrease
- 17:08total.
- 17:09When we
- 17:09look at
- 17:09another
- 17:10patient
- 17:10who is
- 17:10scheduled
- 17:11for
- 17:11enucleation,
- 17:12the first patient
- 17:12was scheduled
- 17:13for plaque
- 17:14brachytherapy,
- 17:15further on
- 17:15we'll see
- 17:15some more
- 17:16slides about
- 17:16why we're
- 17:17excited
- 17:17about this,
- 17:18but here's
- 17:18a patient
- 17:19who was
- 17:19scheduled
- 17:19for
- 17:20enucleation.
- 17:21We see
- 17:21their initial
- 17:22baseline
- 17:22measurements
- 17:23over here 8
- 17:24millimeters,
- 17:2430mm, and,
- 17:26almost 140
- 17:27cubic
- 17:27millimeters
- 17:28of volume.
- 17:29After four
- 17:29months of
- 17:29treatment,
- 17:30we see
- 17:30pretty
- 17:31dramatic
- 17:31decrease
- 17:32in this,
- 17:33tumor size,
- 17:35whether it be,
- 17:35almost
- 17:36three
- 17:36millimeters
- 17:37in apical
- 17:37height,
- 17:38eight
- 17:38millimeters,
- 17:39and there's
- 17:39a diameter,
- 17:4190 cubic
- 17:42millimeters
- 17:42showing
- 17:43percent change
- 17:44from baseline,
- 17:45almost 70%
- 17:46for the
- 17:46height,
- 17:4640% for
- 17:47the basal
- 17:47diameter,
- 17:48and almost 90%
- 17:49for the
- 17:50volume.
- 17:53when we look
- 17:53at some data
- 17:54that has
- 17:55recently been
- 17:55presented
- 17:56at ASCO
- 17:58and this is
- 17:58data from,
- 18:00Anthony
- 18:00Joshua,
- 18:01this is
- 18:01an
- 18:01investigator
- 18:02initiated
- 18:02trial
- 18:03or an IST,
- 18:05we see that
- 18:06same treatment
- 18:07Darovasertib
- 18:07in the
- 18:07neoadjuvant
- 18:08setting.
- 18:08This is a
- 18:09spider plot
- 18:09over here,
- 18:10and again Dr.
- 18:11Josh was
- 18:11looking at
- 18:12volume two.
- 18:12Over time,
- 18:13we see
- 18:14with these
- 18:1415 patients,
- 18:16all the patients
- 18:16except for
- 18:17maybe one
- 18:17initially
- 18:18from when
- 18:18you took
- 18:19the sample
- 18:19size.
- 18:20Excuse me,
- 18:20when he took
- 18:21the data,
- 18:21all these
- 18:22patients
- 18:22are showing
- 18:23a, decrease
- 18:24in the volume
- 18:25of their
- 18:25tumor sizes.
- 18:26When we look
- 18:27at tumor
- 18:28volume,
- 18:29in terms of
- 18:30eyes
- 18:30preserved, of
- 18:3112 patients,
- 18:3275% of
- 18:33those patients
- 18:34had their eyes
- 18:35preserved.
- 18:35Again,
- 18:36light blue
- 18:36being eyes
- 18:37saved,
- 18:37dark blue
- 18:38the one
- 18:39patient
- 18:39that was
- 18:40enucleated,
- 18:41we see
- 18:41the median
- 18:41reduction
- 18:42again in terms
- 18:42of tumor
- 18:43volumes,
- 18:43about 47%.
- 18:47How does this
- 18:47translate for
- 18:48our plaque
- 18:48brachytherapy
- 18:49patients?
- 18:49Again,
- 18:50more of Dr.
- 18:51Joshua's data
- 18:52that was
- 18:52just recently
- 18:53presented
- 18:53at ASCO.
- 18:54Take a look
- 18:55over here,
- 18:55we have a patient.
- 18:56This over here
- 18:57represents her
- 18:57tumor,
- 18:59critical
- 18:59structures
- 18:59here, optic
- 19:00nerve
- 19:01and phobia.
- 19:02We see this
- 19:02as our
- 19:03baseline
- 19:03measurement
- 19:04and we see
- 19:04how close
- 19:05the tumor is
- 19:06to these
- 19:06critical
- 19:06structures.
- 19:08Again
- 19:08after
- 19:09treatment with
- 19:10Darovasertib,
- 19:10we see a very
- 19:12large increase
- 19:13in the
- 19:13distance
- 19:14of the tumor,
- 19:15to those
- 19:16critical
- 19:16structures,
- 19:16whether it be
- 19:17the fovea
- 19:17and/or
- 19:18the optic
- 19:18nerve.
- 19:19So, we can see
- 19:20how much here
- 19:20the tumor is
- 19:21actually
- 19:21shrunk,
- 19:22greater
- 19:23distance.
- 19:24Why is this
- 19:24important?
- 19:25When we look
- 19:26at the
- 19:26potential,
- 19:26radiation
- 19:27field for
- 19:29plaque
- 19:29brachytherapy
- 19:30treatment
- 19:31baseline,
- 19:32we see
- 19:32how close
- 19:33that radiation
- 19:34field
- 19:34is coming
- 19:34to those
- 19:35critical
- 19:35structures.
- 19:36Whereas after
- 19:37treatment
- 19:38we see that
- 19:38radiation
- 19:39field
- 19:39is much
- 19:39smaller.
- 19:40Over here
- 19:41we have
- 19:41some more
- 19:42specific
- 19:42numbers
- 19:42of that.
- 19:43So if
- 19:44we thought
- 19:44about this
- 19:45patient
- 19:45initially,
- 19:46the dose of
- 19:47the optic
- 19:47nerve
- 19:47and or
- 19:48the dose of
- 19:48the fovea,
- 19:49when we look
- 19:49at the amount
- 19:50of radiation
- 19:50therapy
- 19:51that was about
- 19:51to occur,
- 19:5378 Gys,
- 19:54157 Gys
- 19:55post baseline,
- 19:56this is now
- 19:56decreased
- 19:5713 Gys.
- 19:58That's an
- 19:5883% change
- 20:00to the fovea,
- 20:0159 Gys,
- 20:02which now is
- 20:0262% change.
- 20:04Now
- 20:04when we look
- 20:05at the
- 20:05probability
- 20:05of visual loss
- 20:06from one year
- 20:07and/or
- 20:07three year,
- 20:08through the
- 20:08disease,
- 20:09criteria,
- 20:11we see that
- 20:11at baseline
- 20:12there is
- 20:12almost a
- 20:1370% chance
- 20:14for visual
- 20:14loss
- 20:15and/or 95%
- 20:17chance
- 20:17of visual loss
- 20:18at three years
- 20:19from
- 20:19the initial
- 20:20settings here,
- 20:21our post
- 20:22baseline,
- 20:23after
- 20:23treatment
- 20:23with
- 20:24Darovasertib,
- 20:25that
- 20:25possibility of
- 20:26visual loss
- 20:27at one year
- 20:27is now down
- 20:28to 20%.
- 20:29Three year
- 20:30potential loss
- 20:31is down to 43%
- 20:32and this is
- 20:33where I
- 20:33will end
- 20:34this part
- 20:35of my
- 20:36presentation,
- 20:37because
- 20:37the next part
- 20:38is for
- 20:38what we are
- 20:39doing in,
- 20:41metastatic
- 20:41melanoma.
- 20:42melanoma.
- 20:42we have
- 20:42an update
- 20:43from Doctor
- 20:44Hopkins
- 20:44from Aura
- 20:45Biosciences
- 20:46regarding
- 20:46their CoMpass
- 20:47Trial.
- 20:47Doctor
- 20:48Hopkins,
- 20:48thanks so much
- 20:49for joining
- 20:49us.
- 20:50Tremendous.
- 20:51Thank you
- 20:51so much,
- 20:51Danet.
- 20:52It's,
- 20:52such a
- 20:52pleasure
- 20:53to be here
- 20:53and to
- 20:54see the
- 20:54incredible
- 20:55activity
- 20:55in research
- 20:56and care
- 20:56for patients,
- 20:58with uveal
- 20:58melanoma.
- 20:59So thank you
- 20:59for the
- 20:59opportunity
- 21:00to be here.
- 21:01I'm Jill
- 21:01Hopkins, I'm
- 21:02the chief
- 21:02medical
- 21:03officer
- 21:03at Aura
- 21:03Biosciences
- 21:05and I'll walk
- 21:05you
- 21:05through our
- 21:06current
- 21:07program
- 21:07and our hopes
- 21:09for changing
- 21:11some of
- 21:11the paradigm
- 21:11of care
- 21:12for people
- 21:12living
- 21:12with uveal
- 21:13melanoma.
- 21:14So,
- 21:15disclosures,
- 21:16much like
- 21:16Doctor Cole
- 21:17and IDEAYA,
- 21:17we are
- 21:17an
- 21:17investigational,
- 21:18virus like
- 21:19drug
- 21:19conjugate.
- 21:20We are in
- 21:20clinical
- 21:21trials
- 21:21and are
- 21:22not yet
- 21:22approved for,
- 21:24use.
- 21:26So, Bel-sar or
- 21:27AU-011, but
- 21:29we call it
- 21:29Bel-sar, is
- 21:30a novel
- 21:30viral
- 21:31drug conjugate
- 21:32and this is
- 21:32a very
- 21:33novel
- 21:34technology
- 21:35that uses
- 21:35a virus
- 21:36like particle.
- 21:37So this is,
- 21:37similar
- 21:38to, vaccines
- 21:39using a
- 21:40papilloma
- 21:40virus,
- 21:41but this is a
- 21:42non
- 21:42replicating
- 21:43or empty
- 21:43viral capsid,
- 21:44doesn't
- 21:45contain
- 21:45genetic
- 21:46material
- 21:47and we're able
- 21:47to demonstrate
- 21:48that it has
- 21:49a potential
- 21:49to have
- 21:50multivalent
- 21:51binding
- 21:52to these
- 21:52heparin
- 21:53sulfate
- 21:53proteoglycans
- 21:55on solid
- 21:55tumor cells
- 21:56and
- 21:56this is important
- 21:57because
- 21:57these HSPGs
- 21:59as we call
- 21:59them,
- 22:00are present
- 22:01across a
- 22:01range
- 22:02of tumors.
- 22:02So somewhat
- 22:03agnostic
- 22:03to genetic
- 22:04subtype,
- 22:05solid tumors
- 22:06present
- 22:06these HSPGs
- 22:08on the on
- 22:09the tumor
- 22:09surface,
- 22:10and we're able
- 22:11to see
- 22:11these VLPs
- 22:12bind to them.
- 22:14We then
- 22:14add to that
- 22:15a light
- 22:15activated
- 22:16molecule.
- 22:17We conjugate
- 22:18that on
- 22:18the surface
- 22:19of the VLP
- 22:20and when we
- 22:21activate that
- 22:22with a laser,
- 22:22we're able to
- 22:23see selective
- 22:24binding of
- 22:25our drug
- 22:26to the tumor
- 22:26cell.
- 22:27And additional
- 22:28selectivity
- 22:29brought about
- 22:29by the
- 22:30activation
- 22:30only being
- 22:31at the site
- 22:32of where
- 22:32the laser
- 22:32is
- 22:33administered.
- 22:34So we're
- 22:34able to see
- 22:35very on target
- 22:36tumor effects
- 22:37with a
- 22:38much better
- 22:39looking side
- 22:40effect profile
- 22:41when we look
- 22:42at trying
- 22:42to treat
- 22:42early tumors.
- 22:44So we know
- 22:45that,
- 22:45this allows
- 22:47for a dual
- 22:48mechanism
- 22:48of action
- 22:49within the
- 22:49Bel-sar
- 22:50activity.
- 22:51So we have
- 22:52one mechanism
- 22:53where as
- 22:53we, bind
- 22:54that VDC
- 22:55to the HSPGs
- 22:57on the
- 22:57tumor cells,
- 22:59we see tumor
- 23:00cell necrosis
- 23:00after late
- 23:01activation
- 23:03so the tumor
- 23:03releases
- 23:04these oxygen
- 23:04spaces, it
- 23:05disrupts
- 23:06the cell
- 23:06membrane,
- 23:07and we see
- 23:08tumor cell
- 23:09killing via
- 23:10mechanism one.
- 23:12We secondarily
- 23:13see as
- 23:14mechanism two,
- 23:15that these
- 23:15damaged
- 23:16tumor cells
- 23:16release
- 23:17neoantigens.
- 23:18In these
- 23:18DAMPs,
- 23:19we call
- 23:19them, damage
- 23:20associated
- 23:20molecular
- 23:21patterns,
- 23:22and these
- 23:23bring immune
- 23:24cell reaction
- 23:25into the
- 23:26tumor,
- 23:26locale.
- 23:28So here
- 23:29we're able
- 23:29to see
- 23:30necrosis
- 23:31or cell
- 23:32killing
- 23:32at the
- 23:32time of,
- 23:33of
- 23:33the activation
- 23:34and then the
- 23:35secondary
- 23:35mechanism
- 23:36whereby
- 23:36immune cells
- 23:37come in.
- 23:38And here
- 23:38we're very
- 23:39excited
- 23:39by the
- 23:40possibility
- 23:40that long term
- 23:42adaptive
- 23:43immunity
- 23:44can help
- 23:44with ongoing
- 23:45tumor control.
- 23:50So what
- 23:50are the goals
- 23:51of treatment?
- 23:51Well,
- 23:52as Doctor
- 23:53Correa
- 23:53mentioned,
- 23:53we are in
- 23:54phase three,
- 23:54which means
- 23:55we're in
- 23:55a stage
- 23:56of development
- 23:56that we are
- 23:57hopeful
- 23:57will lead
- 23:58to
- 23:58registration
- 23:59and ultimate
- 24:00access of this
- 24:00therapy
- 24:01for patients.
- 24:02And
- 24:02the hope is,
- 24:03with
- 24:04Bel-sar, is
- 24:05that
- 24:05we'll be able
- 24:05to provide
- 24:06local tumor
- 24:06control,
- 24:07we’ll preserve
- 24:08vision, we’ll
- 24:10move away
- 24:10from the
- 24:11radiation
- 24:11related
- 24:12morbidities
- 24:13that are
- 24:13well known
- 24:13to
- 24:13the community,
- 24:15and that we’ll
- 24:15see,
- 24:15potentially,
- 24:16an opportunity
- 24:16to treat early
- 24:17and reduce
- 24:18the risk
- 24:18of metastases
- 24:20with a side
- 24:20effect profile
- 24:21that will
- 24:21hopefully
- 24:22improve
- 24:22quality
- 24:22of life
- 24:23for patients.
- 24:24So we're
- 24:24very excited
- 24:25by the
- 24:26potential
- 24:27here.
- 24:27We are
- 24:27looking at the
- 24:28delivery
- 24:29of Bel-sar
- 24:30by injection
- 24:31into the eyes.
- 24:32So this is
- 24:32done in
- 24:33the office,
- 24:34do not need
- 24:34to go
- 24:34to the
- 24:35operating room
- 24:35for this.
- 24:36We use
- 24:37a super
- 24:37choroidal
- 24:38injector
- 24:38on the
- 24:38white surface
- 24:39of the eye.
- 24:40We deliver
- 24:41the Bel-sar
- 24:41in that
- 24:42location,
- 24:42and we do
- 24:43two
- 24:43injections,
- 24:44on either
- 24:45side of
- 24:45the tumor.
- 24:46The blue drug,
- 24:47as you
- 24:47see there
- 24:47on the slide,
- 24:48kind of
- 24:48distributes
- 24:49back
- 24:49around
- 24:50the back
- 24:50of the eye,
- 24:50surrounds
- 24:51the tumor
- 24:51and
- 24:51distributes
- 24:52within it.
- 24:53And we
- 24:53then do two,
- 24:54laser
- 24:55treatments.
- 24:56They take
- 24:56about
- 24:56five minutes
- 24:57once the drug
- 24:58has had time
- 24:59to distribute
- 24:59in the tumor.
- 25:00Again,
- 25:01all done
- 25:01in office
- 25:02as Dr Correa
- 25:03mentioned,
- 25:03it's a lot of
- 25:04upfront
- 25:04activity.
- 25:05You do have,
- 25:06three cycles
- 25:06of treatment,
- 25:07at the start
- 25:08of the trial
- 25:08and you’re
- 25:09then followed
- 25:09for an
- 25:10additional
- 25:1015 months,
- 25:12but in-office
- 25:12and as we say,
- 25:13the potential
- 25:14for,
- 25:14side effect
- 25:15profile
- 25:15that allows
- 25:16for
- 25:16preservation
- 25:17of vision.
- 25:20This slide
- 25:20talks
- 25:21about how
- 25:21we're
- 25:21currently
- 25:22managing
- 25:22uveal melanoma
- 25:23and I think,
- 25:23whenever I
- 25:24look at this,
- 25:24and what
- 25:25this webinar
- 25:25absolutely
- 25:26highlights
- 25:26is the
- 25:27activity
- 25:28in research
- 25:29and
- 25:29advancement
- 25:30across the
- 25:30entire
- 25:31spectrum
- 25:31of care
- 25:32for uveal
- 25:32melanoma,
- 25:33which I think
- 25:33is the
- 25:33most exciting
- 25:34thing about
- 25:34this slide.
- 25:36I think
- 25:36where we are,
- 25:37seeing
- 25:37the potential
- 25:38for Bel-sar
- 25:39is in
- 25:39this local
- 25:40or early
- 25:41stage
- 25:41of disease
- 25:42when we have
- 25:42either
- 25:42these
- 25:43indeterminate
- 25:44lesions
- 25:44or the
- 25:44small
- 25:45melanomas
- 25:46where many
- 25:47people are in
- 25:47that watch
- 25:48and wait.
- 25:48So, so many
- 25:49people
- 25:49listening
- 25:50or with
- 25:50family members
- 25:51impacted
- 25:51by this,
- 25:52you may have
- 25:53been told
- 25:53at some time
- 25:54that you have
- 25:54this pigmented
- 25:55lesion
- 25:55in your eye.
- 25:56We're going
- 25:56to watch
- 25:57and wait
- 25:57and see
- 25:57what it does
- 25:58over time,
- 25:59because
- 25:59available
- 26:00therapies
- 26:00are associated
- 26:01with with
- 26:02challenging
- 26:02side effects.
- 26:03It's always a,
- 26:04trade off
- 26:05of sorts
- 26:06to say,
- 26:06do I treat now
- 26:07and know
- 26:08that I'm
- 26:08putting
- 26:08someone on
- 26:09a radiation
- 26:09related series
- 26:10of
- 26:11complications?
- 26:12Or do
- 26:12I observe
- 26:13and make sure
- 26:14I'm seeing
- 26:14growth,
- 26:15or determining
- 26:15that this is,
- 26:16in fact,
- 26:16going to
- 26:16be a lesion
- 26:17that will
- 26:17require
- 26:18treatment?
- 26:19And I think
- 26:19that's a
- 26:19very important
- 26:20point of what
- 26:21Doctor Correa
- 26:21mentioned in
- 26:22her talk.
- 26:22We do, in
- 26:23the trial,
- 26:23want to show
- 26:24evidence
- 26:24of growth,
- 26:25because that
- 26:25is a somewhat
- 26:26irrefutable
- 26:27sign of,
- 26:28change
- 26:29in the tumor.
- 26:29So
- 26:30to make sure
- 26:30we are
- 26:31treating
- 26:31true melanomas
- 26:32rather
- 26:33than nevi,
- 26:33that growth
- 26:34aspect is
- 26:34important
- 26:35and as she
- 26:36mentioned,
- 26:36biopsies prior
- 26:37to entering
- 26:37the trial
- 26:38are absolutely
- 26:38fine to
- 26:39determine,
- 26:39additional
- 26:40information to
- 26:41help support
- 26:41that,
- 26:42but we are,
- 26:43you know,
- 26:44the thought
- 26:45of being able
- 26:45to move
- 26:46into lesions
- 26:46that are
- 26:47earlier,
- 26:48smaller,
- 26:48that might be
- 26:49in that watch
- 26:50and wait,
- 26:50aspect now,
- 26:52with the
- 26:52treatment
- 26:52being explored
- 26:53in phase
- 26:53three, is,
- 26:54is very
- 26:55exciting
- 26:55to us at Aura,
- 26:56and we hope to
- 26:56the community
- 26:57as well.
- 26:58Dr.
- 26:59Cole did a
- 26:59lovely job
- 27:00describing
- 27:00where IDEAYA
- 27:01is in
- 27:02terms of
- 27:02the potential
- 27:02in larger
- 27:03tumors
- 27:04and then the
- 27:04systemic
- 27:05therapy
- 27:05we'll hear
- 27:05about in
- 27:06the next
- 27:06session.
- 27:09I think the,
- 27:10as
- 27:11I mentioned,
- 27:11the high
- 27:11morbidity
- 27:12associated
- 27:12with the
- 27:12current
- 27:13standard
- 27:13of care,
- 27:13which is
- 27:14radiotherapy
- 27:15and has
- 27:15been the case
- 27:16for decades,
- 27:17and it's
- 27:17successful
- 27:18in, in
- 27:19stopping
- 27:19growth of
- 27:20the tumor,
- 27:20the
- 27:21unfortunate
- 27:21side effect
- 27:22for people
- 27:22living with
- 27:23that is that
- 27:23the vast
- 27:24majority
- 27:24of patients
- 27:25treated with
- 27:26radiation
- 27:26do go on
- 27:27to legal
- 27:27blindness
- 27:28over time
- 27:28in the
- 27:29treated eye
- 27:30so we really
- 27:31would,
- 27:31would be,
- 27:33excited to see
- 27:34a paradigm
- 27:34shift
- 27:34that says
- 27:35we can
- 27:35treat early,
- 27:36we don't have
- 27:37the vision
- 27:37loss
- 27:37associated,
- 27:38and we can,
- 27:39look at
- 27:40a shift
- 27:40in how
- 27:40we've been
- 27:41managing
- 27:41patients for
- 27:42many
- 27:42years now.
- 27:45Tell you a
- 27:45little bit
- 27:46about
- 27:46our phase
- 27:46two trial.
- 27:47So
- 27:47the phase two
- 27:48trial
- 27:48is completed
- 27:50and the data
- 27:51from this
- 27:51allowed us
- 27:52to move
- 27:52to our phase
- 27:52three
- 27:53registrational
- 27:53program.
- 27:54In phase two,
- 27:55we again
- 27:56looked at
- 27:56super
- 27:57choroidal
- 27:57administration
- 27:58to the eye,
- 27:58activated
- 27:59with laser
- 28:00and this was
- 28:00a dose
- 28:01escalation
- 28:01trial.
- 28:02So
- 28:02we were able
- 28:02to move
- 28:03quickly
- 28:03through
- 28:04cohorts
- 28:04to show that,
- 28:05increasing
- 28:06either
- 28:06the dose
- 28:07of the Bel-sar
- 28:08or the number
- 28:08of lasers
- 28:09was not
- 28:10associated
- 28:10with any, side
- 28:11effect profile
- 28:12that prevented
- 28:12increasing,
- 28:14and we were
- 28:14able to
- 28:14increase up
- 28:15to our
- 28:15therapeutic
- 28:16regimen,
- 28:16which is what
- 28:17we're
- 28:17exploring in
- 28:17phase three,
- 28:18which is
- 28:19three cycles
- 28:20of treatment,
- 28:20two
- 28:21injections,
- 28:21two lasers.
- 28:22And
- 28:23our endpoints
- 28:24were looking
- 28:24at evidence
- 28:25of tumor
- 28:26progression.
- 28:26So did
- 28:27the tumors
- 28:27grow over
- 28:28time?
- 28:29Was there
- 28:29any loss of
- 28:30visual acuity
- 28:31measured
- 28:31as 15 letters
- 28:32or three lines
- 28:33on the eye
- 28:33chart
- 28:34from baseline,
- 28:35and any change
- 28:36in the rate
- 28:36of growth
- 28:37of the tumor
- 28:37over the
- 28:38course of of
- 28:39the treatment
- 28:39cycle.
- 28:43And what we
- 28:43saw was, three
- 28:45key outcomes
- 28:46that
- 28:46allowed us
- 28:47to move
- 28:47into phase
- 28:47three.
- 28:48First, 80%
- 28:49tumor
- 28:50control rate.
- 28:51So when we
- 28:51looked at our
- 28:51therapeutic
- 28:52regimen,
- 28:53so
- 28:53the three cycles
- 28:54that are
- 28:54being studied
- 28:55in phase
- 28:55three,
- 28:56we had 80% of
- 28:57those patients
- 28:57had a
- 28:58complete
- 28:58cessation of
- 28:59tumor growth
- 29:00at the
- 29:0012 month mark
- 29:02compared
- 29:03to our
- 29:03sub
- 29:03therapeutic.
- 29:04That's not
- 29:04a true
- 29:05sham in
- 29:05the sense
- 29:06that patients
- 29:06did receive
- 29:07some drug,
- 29:08but we know
- 29:08it was
- 29:08a very low
- 29:09level of drug
- 29:10and laser.
- 29:11We're only 20%
- 29:12of those
- 29:12tumors,
- 29:13achieved
- 29:13control.
- 29:14So we had
- 29:15an 80%
- 29:16tumor
- 29:16control rate.
- 29:17Very
- 29:17importantly,
- 29:18we were able
- 29:18to demonstrate
- 29:19that 90%
- 29:20of the
- 29:21patients
- 29:21in the phase
- 29:21two trial
- 29:22maintain
- 29:23their visual
- 29:24acuity
- 29:24so the vision
- 29:25with which
- 29:25they entered
- 29:26the trial
- 29:27was the vision
- 29:28with which
- 29:28they left
- 29:29the trial,
- 29:30and
- 29:30very importantly,
- 29:31these were
- 29:32tumors in 70%
- 29:33of them,
- 29:34were tumors
- 29:35that were
- 29:35very close
- 29:35to the macula
- 29:36or the
- 29:36optic nerve,
- 29:37which, Dr.
- 29:38Cole nicely
- 29:38outlined
- 29:39the importance
- 29:40of those
- 29:40structures
- 29:41in maintaining
- 29:42vision.
- 29:43So even tumors
- 29:44in that high
- 29:45risk location
- 29:46for vision
- 29:46loss,
- 29:47patients
- 29:47were able
- 29:48to preserve
- 29:48vision
- 29:49with the
- 29:49Bel-sar
- 29:49treatment,
- 29:51and then
- 29:51we saw
- 29:51a very low
- 29:52rate of of
- 29:54adverse events
- 29:55in the
- 29:55trial, again,
- 29:56supporting
- 29:57our hypothesis
- 29:58of the dual
- 29:58selectivity of
- 29:59this
- 29:59treatment.
- 30:00We had a little
- 30:01bit of mild,
- 30:02front of
- 30:02the eye
- 30:03inflammation
- 30:03that was
- 30:04managed
- 30:04with topical
- 30:05drops over
- 30:06a few
- 30:06weeks and,
- 30:07resolved
- 30:07without any
- 30:08additional,
- 30:09sequelae
- 30:10or long
- 30:11lasting
- 30:11side effects
- 30:12and no
- 30:13other major,
- 30:13side
- 30:14effects
- 30:15reported.
- 30:15So we were
- 30:15very,
- 30:16encouraged
- 30:17by the
- 30:18favorable
- 30:18safety
- 30:18profile,
- 30:19coupled with
- 30:20the tumor
- 30:20control rate
- 30:21and the
- 30:21preservation
- 30:22of visual
- 30:22acuity coming
- 30:23out of our
- 30:24phase two
- 30:24trial.
- 30:26So this has
- 30:27allowed us
- 30:27to move
- 30:28into the phase
- 30:28three CoMpass
- 30:29trial.
- 30:30We are looking
- 30:30at these
- 30:31small
- 30:32choroidal
- 30:32melanomas
- 30:33or
- 30:33indeterminate
- 30:34lesions,
- 30:35and we are
- 30:35randomizing
- 30:36patients 2
- 30:37to 1 to 2,
- 30:38which means
- 30:38within the,
- 30:40the main
- 30:41comparator
- 30:41groups,
- 30:42we have the
- 30:4280 micrograms,
- 30:43the high dose
- 30:44of Bel-sar
- 30:45compared
- 30:45to the sham
- 30:46control arm.
- 30:47And that is a
- 30:48observation
- 30:49arm.
- 30:49This was very
- 30:50important
- 30:50in our
- 30:51conversations
- 30:51with the FDA
- 30:53and the
- 30:53European
- 30:53Regulatory
- 30:54agencies,
- 30:55that we do
- 30:55have treatment
- 30:57compared
- 30:57to sham
- 30:57or
- 30:58observation,
- 30:58which is for
- 30:59many of these
- 30:59small lesions,
- 31:00what the
- 31:00current
- 31:01standard of
- 31:01care is.
- 31:02And then
- 31:02we have
- 31:03the 40
- 31:03microgram,
- 31:04the lower dose
- 31:05of Bel-sar
- 31:05in there
- 31:06for masking
- 31:07so that
- 31:07patients,
- 31:08and physicians
- 31:08are not aware
- 31:09of which dose
- 31:10they are
- 31:10receiving.
- 31:11This is very
- 31:12important.
- 31:12All
- 31:13that sounds
- 31:13complicated.
- 31:14It's very
- 31:15important
- 31:15in terms
- 31:16of a
- 31:16registrational
- 31:17trial
- 31:17that allows
- 31:18for regulatory
- 31:19approval
- 31:20and access
- 31:21for patients.
- 31:22And we're
- 31:22pleased to say
- 31:23we have
- 31:23both fast
- 31:24track
- 31:24and orphan
- 31:25drug
- 31:25designations.
- 31:26We have
- 31:27a protocol
- 31:27agreement
- 31:28with the FDA,
- 31:29and support
- 31:30from the EMA
- 31:30on our
- 31:31endpoints.
- 31:31so we're
- 31:32looking at
- 31:32enrolling 100
- 31:33participants
- 31:34around
- 31:34the world.
- 31:35We have
- 31:36sites in North
- 31:36America,
- 31:37Europe,
- 31:38Middle East
- 31:38and Asia
- 31:39Pacific,
- 31:39so we are
- 31:40truly global
- 31:41in our
- 31:41approach here,
- 31:4290 sites
- 31:43to enroll
- 31:43100 patients
- 31:44with the hopes
- 31:45that we will
- 31:45have,
- 31:46regulatory
- 31:46approval
- 31:47following
- 31:48enrollment
- 31:48and a
- 31:4815 month,
- 31:49primary
- 31:50efficacy
- 31:50analysis.
- 31:52So ongoing,
- 31:53and this
- 31:53map shows,
- 31:54you can see,
- 31:55both on the,
- 31:55A Cure
- 31:56In Sight
- 31:56website,
- 31:56Thank you,
- 31:57Danet.
- 31:57there's a
- 31:57really
- 31:58nice link
- 31:58to the
- 31:59CoMpass trial
- 31:59and
- 32:00any information
- 32:00you want
- 32:01there.
- 32:01We’re also
- 32:01on
- 32:01ClinicalTrials.gov,
- 32:03and this is
- 32:04a map of the
- 32:04current sites
- 32:05that are open
- 32:05and enrolling
- 32:06in the
- 32:06United States.
- 32:08Still more
- 32:08to come there.
- 32:09We've just
- 32:10been approved
- 32:10in Canada,
- 32:11the United
- 32:11Kingdom
- 32:12and Australia,
- 32:13and we're
- 32:13following,
- 32:14in short order
- 32:14with European
- 32:15sites as well.
- 32:16So a lot of
- 32:17activity on on
- 32:18a key trial,
- 32:19and happy
- 32:20to take
- 32:20any questions.
- 32:21Thank you
- 32:22again
- 32:22for
- 32:22the opportunity
- 32:23to share
- 32:24our goals
- 32:25at Aura
- 32:25to change
- 32:26how patients
- 32:27with melanoma
- 32:27are treated.
- 32:28Thank you.
- 32:29Hey, Danet,
- 32:30how are you?
- 32:31I'm great.
- 32:31How are you?
- 32:33Good.
- 32:33Thank you for
- 32:34having me.
- 32:35Of course.
- 32:36So, Doctor
- 32:36Carvajal,
- 32:37we are
- 32:37so excited
- 32:38to chat
- 32:38about
- 32:38metastatic
- 32:39uveal
- 32:39melanoma.
- 32:40I know this
- 32:40is a hot
- 32:41topic, but,
- 32:43do you have
- 32:43any specific
- 32:44thoughts
- 32:44that you
- 32:44want to
- 32:45give us?
- 32:46You know,
- 32:46kind of going
- 32:47from this
- 32:47last section
- 32:48into our
- 32:48next section
- 32:49Yeah, that's
- 32:49exactly
- 32:50actually what
- 32:50I want
- 32:51to focus on
- 32:51Danet because,
- 32:52I think
- 32:53your point
- 32:54that,
- 32:55you know,
- 32:56we're really
- 32:57in a new place
- 32:58in this ocular
- 32:59melanoma
- 32:59field, and
- 33:01I have to
- 33:01say is,
- 33:01I guess, like,
- 33:02kind of,
- 33:03speaking
- 33:04to patients
- 33:05and other
- 33:05researchers
- 33:06and clinicians
- 33:06now, I am
- 33:07so much
- 33:07more
- 33:08optimistic.
- 33:08I was always
- 33:09optimistic.
- 33:11but,
- 33:12the degree of
- 33:12optimism
- 33:13is just
- 33:13much higher,
- 33:14and it just
- 33:15reminds me,
- 33:16I think
- 33:16some of you
- 33:16who have
- 33:17heard me
- 33:17speak before,
- 33:18I kind of,
- 33:19talk about
- 33:19sometimes
- 33:20where we were
- 33:20in skin
- 33:21melanoma,
- 33:22when I started
- 33:23as an
- 33:24oncologist
- 33:24in 2000, where
- 33:27we really
- 33:27didn't
- 33:27have anything,
- 33:29you know, and
- 33:31fast forward
- 33:3120 years
- 33:32later,
- 33:33with our
- 33:33therapies,
- 33:34where
- 33:34we're carrying
- 33:35patients
- 33:36with
- 33:36metastatic
- 33:36melanoma
- 33:37for not
- 33:38everyone,
- 33:38but 40 to
- 33:3950% of them
- 33:41with these
- 33:41novel
- 33:42immunotherapies
- 33:43and these
- 33:43novel
- 33:43targeted
- 33:44therapies.
- 33:44And as you
- 33:45said, as,
- 33:46as these
- 33:47effective
- 33:48therapies
- 33:48became
- 33:49approved,
- 33:50we had more
- 33:51license
- 33:51in terms of,
- 33:52I would
- 33:53say,
- 33:53creativity
- 33:53in the forms
- 33:54of novel
- 33:54clinical
- 33:55trials, novel
- 33:56combinations,
- 33:57which really
- 33:58just
- 33:58accelerated
- 33:59our ability
- 33:59to improve
- 34:00outcomes.
- 34:01And I think
- 34:01that's where
- 34:01we are now
- 34:02for uveal
- 34:03melanoma,
- 34:03so I'm
- 34:04just I'm
- 34:05really
- 34:05excited.
- 34:08Well, I'm
- 34:08so excited
- 34:09to hear
- 34:09what you guys
- 34:10have,
- 34:11both you
- 34:11and our future
- 34:12guests
- 34:12that we have
- 34:13coming on.
- 34:13So I'm
- 34:13going to
- 34:14go ahead
- 34:14and let
- 34:14you share
- 34:14your slides,
- 34:15and I will
- 34:16step away
- 34:16so that
- 34:17you can
- 34:17present
- 34:18and we will
- 34:18get going.
- 34:19Perfect.
- 34:21So I think so
- 34:22this talk is
- 34:23maybe a little
- 34:24bit different.
- 34:24So what I want
- 34:25to do
- 34:25is maybe,
- 34:28talk a
- 34:28little bit
- 34:29about that
- 34:30bridge,
- 34:30I guess,
- 34:32between,
- 34:33you know,
- 34:33kind of
- 34:33treatment
- 34:34of the
- 34:34primary
- 34:34treatment
- 34:35of metastatic
- 34:35disease,
- 34:36and this is
- 34:36more of
- 34:37a conceptual
- 34:37kind of
- 34:38overview
- 34:39because,
- 34:40again,
- 34:40I do think
- 34:41we're moving
- 34:41towards cure
- 34:43the way
- 34:43we're thinking
- 34:43about this
- 34:44disease
- 34:44and developing
- 34:45therapies.
- 34:46It's
- 34:46much different
- 34:46than the way
- 34:46thinking
- 34:46of things
- 34:46five years
- 34:46ago, and so,
- 34:50this will be
- 34:53a little bit
- 34:57of a
- 34:59different talk
- 35:02than I
- 35:04usually do.
- 35:05Here are
- 35:05my
- 35:06disclosures.
- 35:06I do a little
- 35:08bit of
- 35:09consulting,
- 35:09a little
- 35:09bit of work
- 35:09with IDEAYA
- 35:09and Aura
- 35:09so let me
- 35:10start
- 35:10with this
- 35:11and this is
- 35:11conceptual.
- 35:12if we look at
- 35:15cutaneous
- 35:16melanoma,
- 35:18we have been
- 35:18able to
- 35:19increase
- 35:20the number
- 35:20of patients
- 35:21that we
- 35:21can cure,
- 35:22through
- 35:23kind of three
- 35:24large buckets,
- 35:25I would say.
- 35:26And the left
- 35:27bucket is that
- 35:29with our
- 35:29novel
- 35:30immunotherapies,
- 35:31targeted
- 35:31therapies and
- 35:32so forth,
- 35:33we're
- 35:34actually
- 35:34able to,
- 35:35treat
- 35:36patients with
- 35:37widely
- 35:38metastatic
- 35:38cutaneous
- 35:39melanoma.
- 35:40So
- 35:40this is skin
- 35:40melanoma
- 35:41that's gone
- 35:41through
- 35:41the liver,
- 35:42bone, lungs,
- 35:42wherever
- 35:43brain,
- 35:44and we can
- 35:45treat them
- 35:45with these,
- 35:47immunotherapies
- 35:48and
- 35:48we can just
- 35:48eradicate it.
- 35:50Right.
- 35:50We still have
- 35:51work to do.
- 35:52You know,
- 35:52I want to be
- 35:53able to
- 35:53do that
- 35:53for 100%
- 35:54of people.
- 35:55But,
- 35:56you know,
- 35:5620 years ago,
- 35:57we were
- 35:57carrying maybe
- 35:585% of people
- 35:59with
- 35:59metastatic
- 36:00disease.
- 36:00Now it's up
- 36:00to 50%.
- 36:02Okay.
- 36:03And so one
- 36:03path towards
- 36:04increasing the
- 36:05number
- 36:05of people
- 36:06we can cure
- 36:06with uveal
- 36:07melanoma is
- 36:07just develop,
- 36:09more
- 36:10effective
- 36:10therapies
- 36:11that work
- 36:11in the
- 36:12metastatic
- 36:12setting.
- 36:14Okay.
- 36:14So that's
- 36:14kind of
- 36:15that left
- 36:15bucket.
- 36:16The right
- 36:17bucket is
- 36:20using various
- 36:21tools
- 36:22to prevent
- 36:22the
- 36:22development
- 36:23of cancer
- 36:24in the
- 36:24first place.
- 36:25Right.
- 36:26And so for
- 36:26skin melanoma,
- 36:27which
- 36:27we know
- 36:28is
- 36:28predominantly
- 36:29driven by UV
- 36:30radiation
- 36:31exposure
- 36:31from the sun,
- 36:32if we use
- 36:33sunscreen,
- 36:33if we use sun
- 36:34safe measures,
- 36:35stay out
- 36:36of the sun
- 36:36during kind
- 36:37of noon.
- 36:38wear
- 36:39wide brimmed
- 36:39hats,
- 36:39all of that
- 36:40stuff.
- 36:41We can reduce
- 36:42the risk
- 36:42of developing
- 36:43disease
- 36:43in the
- 36:43first place.
- 36:44And frankly,
- 36:46that's what
- 36:46we have to do
- 36:47for all
- 36:47cancers,
- 36:47right?
- 36:47Prevent it.
- 36:50I would say,
- 36:51you know,
- 36:51where we are
- 36:51for metastatic
- 36:52uveal
- 36:53melanoma,
- 36:53we have
- 36:54some approved
- 36:54effective
- 36:55therapies.
- 36:56We have to
- 36:56enhance
- 36:57what we're
- 36:57doing right,
- 36:58to actually
- 36:59be able
- 36:59to achieve
- 37:00cure and a
- 37:02high number
- 37:02of patients
- 37:04in terms
- 37:04of prevention,
- 37:05we have a bit
- 37:07of work to do
- 37:07because,
- 37:08you know,
- 37:09to prevent
- 37:09cancer, it's
- 37:10helpful
- 37:10for us to know
- 37:11what
- 37:11causes it.
- 37:12And,
- 37:13that's
- 37:14something
- 37:14that we
- 37:14still
- 37:14have work
- 37:15to do.
- 37:16Okay.
- 37:17But for this
- 37:17talk, what
- 37:18I want to
- 37:18focus on
- 37:19is the idea
- 37:19of eliminating
- 37:20what we call
- 37:21micro
- 37:21metastatic
- 37:22disease.
- 37:23Right.
- 37:23And so,
- 37:25can we treat
- 37:27patients?
- 37:27Can we
- 37:28treat people
- 37:28with primary
- 37:29uveal
- 37:30melanoma,
- 37:30not just
- 37:31with
- 37:31enucleation
- 37:32or radiation,
- 37:33but can we
- 37:33use
- 37:34the medicine
- 37:34either
- 37:35before
- 37:36treatment
- 37:36with the eye
- 37:36or after
- 37:37treatment of
- 37:38the eye,
- 37:39to kill
- 37:40any cells
- 37:41that may have
- 37:41escaped.
- 37:42It might be
- 37:42hiding out
- 37:43in the liver.
- 37:44Right,
- 37:44and that's
- 37:45what we call
- 37:45micro
- 37:46metastatic
- 37:46disease
- 37:47and we know
- 37:48in skin
- 37:48melanoma
- 37:49and a lot of
- 37:50other cancers,
- 37:50breast cancer,
- 37:51colon cancer,
- 37:52if we
- 37:53give medicine
- 37:53around
- 37:54the time
- 37:55of surgery
- 37:57either before
- 37:58and or
- 37:58after surgery,
- 38:00we can
- 38:00dramatically
- 38:01decrease
- 38:01the risk of
- 38:02the cancer
- 38:02coming back
- 38:03and in
- 38:03some diseases,
- 38:04help people
- 38:04live longer.
- 38:05We can do that
- 38:06in skin
- 38:07melanoma
- 38:08and I think
- 38:09we're
- 38:09getting close
- 38:10to the point
- 38:10of being able
- 38:11to do that
- 38:11in uveal
- 38:12melanoma.
- 38:14Okay,
- 38:14so that was
- 38:14that was a
- 38:15big overview.
- 38:16So, if
- 38:16we take
- 38:17that kind of
- 38:18middle bucket
- 38:18how do we
- 38:19eliminate
- 38:20micro
- 38:21metastatic
- 38:21disease.
- 38:22There
- 38:22basically
- 38:23three ways
- 38:24that we can
- 38:25think about
- 38:25developing
- 38:26that. Right.
- 38:27And one way
- 38:27we can do that
- 38:28is take
- 38:29what we know
- 38:29works
- 38:30in people
- 38:31with
- 38:31metastatic
- 38:31disease.
- 38:33and then
- 38:33try it
- 38:34either before
- 38:35treatment of
- 38:35the eye
- 38:36or after
- 38:36treatment of
- 38:37the eye
- 38:37or in
- 38:38combination
- 38:40and see
- 38:40if, it can
- 38:41kill off
- 38:42those, rogue
- 38:43cells
- 38:43that might be
- 38:44in the liver,
- 38:45reduce
- 38:45the risk of
- 38:46the cancer
- 38:46coming back
- 38:47in the future
- 38:47and improve
- 38:48outcomes.
- 38:50and I'll
- 38:51talk
- 38:51a little bit
- 38:51about the
- 38:52Tebentafusp
- 38:52story
- 38:53in that space.
- 38:55You know,
- 38:55I'll go
- 38:55through two
- 38:56and three,
- 38:56kind of a
- 38:57little bit
- 38:57later,
- 38:58but so let's,
- 38:58let's focus
- 38:59on this
- 39:01in the
- 39:01metastatic
- 39:02setting.
- 39:03there are
- 39:04kind of two,
- 39:05systemic
- 39:06approaches
- 39:06that we think
- 39:07about, one
- 39:08is
- 39:08Tebentafusp,
- 39:09you’ve
- 39:09heard a bit
- 39:10about this.
- 39:10And that's
- 39:11an
- 39:11immunotherapeutic
- 39:12approach.
- 39:13And the other
- 39:14is the
- 39:14Darovasertib
- 39:15story,
- 39:16which is
- 39:16the targeted
- 39:17therapy
- 39:17approach.
- 39:18I'm not
- 39:19going to talk
- 39:19too much
- 39:20about the,
- 39:21kind of
- 39:21mechanism that
- 39:22Darovasertib,
- 39:22George did
- 39:23a
- 39:24fantastic job
- 39:25about that.
- 39:26I would just
- 39:27say that
- 39:27the targeted
- 39:27therapy
- 39:28approach
- 39:28is basically
- 39:30looking
- 39:30at the
- 39:30signaling
- 39:31pathways
- 39:32within
- 39:33the cancer
- 39:33cell, right.
- 39:34Like the
- 39:35things that
- 39:35are telling
- 39:36that cancer
- 39:36cell to grow
- 39:37and spread
- 39:37and do
- 39:38the things
- 39:38we don't
- 39:38want it to do.
- 39:40and with
- 39:40these pills
- 39:41like
- 39:41Darovasertib,
- 39:42we can
- 39:42shut down
- 39:42those pathways
- 39:43that kills
- 39:44the cancer
- 39:44cell,
- 39:44improve
- 39:45the outcome.
- 39:46Okay.
- 39:46With the
- 39:47immunotherapeutic
- 39:48approaches,
- 39:48what we're
- 39:48trying to do
- 39:49is somehow,
- 39:51increase
- 39:52the ability
- 39:52of our
- 39:53immune system,
- 39:53our immune
- 39:54cells
- 39:54in the body
- 39:55to, recognize
- 39:56and kill
- 39:57off the
- 39:57cancer.
- 39:58And,
- 39:59I don't think
- 39:59we have an
- 40:00Immunocore
- 40:00speaker so
- 40:01I'll talk
- 40:01just briefly
- 40:02about the
- 40:03mechanism
- 40:03about this.
- 40:05It's kind
- 40:05of
- 40:05interesting.
- 40:06Tebentafusp
- 40:07is, in
- 40:08this picture,
- 40:09it's this
- 40:09blue thing
- 40:09here, and here
- 40:11we have
- 40:11the melanoma
- 40:12cell up top
- 40:12and immune
- 40:13cell,
- 40:13they're here.
- 40:14And this
- 40:14blue thing,
- 40:15this tebe
- 40:16thing,
- 40:17it's what
- 40:17we call this
- 40:17bispecific
- 40:18molecule
- 40:19where one
- 40:19half of it
- 40:20binds to
- 40:22this protein
- 40:22gp 100
- 40:23that's present
- 40:24on virtually
- 40:25all uveal
- 40:26melanoma cells
- 40:26in high,
- 40:28concentration.
- 40:30And if it's
- 40:30expressed
- 40:31in a
- 40:31certain way,
- 40:33in the context
- 40:34of what
- 40:34we call
- 40:34HLA-A*02:01,
- 40:36Tebe
- 40:37can bind to
- 40:38that really
- 40:38tightly,
- 40:39so binds
- 40:39to the cancer
- 40:40cell really
- 40:41tightly,
- 40:42and then on
- 40:42the other end,
- 40:43it has
- 40:43this thing
- 40:44that just
- 40:44attracts
- 40:45immune cells
- 40:45that are in
- 40:46that
- 40:46environment
- 40:48and basically
- 40:48all it does
- 40:49then is Tebe
- 40:50brings
- 40:51the immune
- 40:51cells
- 40:52just in close
- 40:52proximity to
- 40:53the cancer
- 40:54cell,
- 40:54and then
- 40:55the cancer
- 40:55cell dies.
- 40:56Right?
- 40:56So that's
- 40:57pretty cool.
- 40:58And it's
- 40:58really cool
- 40:59that this was
- 41:00the first drug
- 41:01that was
- 41:01ever approved
- 41:02for the
- 41:02treatment
- 41:02of uveal
- 41:03melanoma
- 41:03and this is
- 41:04just in
- 41:04January
- 41:05of 2022.
- 41:06Right.
- 41:06Like why
- 41:07am I so
- 41:08optimistic?
- 41:09There was
- 41:10nothing
- 41:10that was ever
- 41:11proven to
- 41:12meaningfully
- 41:12improve
- 41:12outcomes
- 41:13in this
- 41:13disease
- 41:14before that,
- 41:15which is mind
- 41:17blowing to me.
- 41:17Right?
- 41:18That,
- 41:19that was the case,
- 41:20but finally
- 41:20we have,
- 41:21you know,
- 41:22our first
- 41:23approved
- 41:23systemic aid
- 41:24and that
- 41:24really
- 41:24meaningfully
- 41:25improve
- 41:25outcomes
- 41:26in our
- 41:26patients
- 41:27with advanced
- 41:27disease
- 41:28if they are
- 41:29HLA-A*02:01
- 41:30positive.
- 41:32Okay,
- 41:36now I want to
- 41:36kind of
- 41:37shift to
- 41:37how are we
- 41:38applying this
- 41:38and like that
- 41:39prevented it
- 41:40and
- 41:40preventative
- 41:41type setting.
- 41:43And one of
- 41:43the things
- 41:43that we
- 41:44noticed,
- 41:45when
- 41:45we treated
- 41:46patients with
- 41:47metastatic
- 41:48disease
- 41:49is that,
- 41:50the likelihood
- 41:51of
- 41:51significantly
- 41:52shrinking
- 41:52the cancer
- 41:53wasn't
- 41:53that high.
- 41:54And so up
- 41:55top here,
- 41:56this kind
- 41:57of bar thing,
- 41:58this is
- 41:58what we call
- 41:59a waterfall
- 41:59plot.
- 42:00Every patient
- 42:01is a bar.
- 42:02And
- 42:02if the tumor
- 42:02shrinks,
- 42:03the bar
- 42:03goes down.
- 42:04If the tumor
- 42:05grows,
- 42:05it goes up.
- 42:07And you can
- 42:07see that,
- 42:08we have
- 42:08major
- 42:09shrinkage
- 42:10in orange
- 42:10and a small
- 42:11fraction of
- 42:11patients,
- 42:13we have
- 42:13more modest,
- 42:14shrinkage.
- 42:15So
- 42:15a little bit
- 42:15of shrinkage
- 42:16or maybe even
- 42:16a little bit
- 42:17of growth,
- 42:18in the
- 42:18majority
- 42:19of the patients,
- 42:19and then
- 42:20we have major
- 42:20growth in
- 42:21kind of a
- 42:22small subset,
- 42:25in some
- 42:26cancers,
- 42:28drugs that
- 42:29shrink cancer
- 42:30better leads
- 42:32to improve
- 42:32outcomes
- 42:33in terms
- 42:33of having
- 42:34of people
- 42:34live longer.
- 42:35Right?
- 42:35Which is
- 42:36ultimately
- 42:36what we
- 42:36want to do.
- 42:38but for Tebe,
- 42:38we didn't
- 42:39see that
- 42:39relationship,
- 42:40really.
- 42:41So,
- 42:42whether or not
- 42:42you shrunk
- 42:43tumor,
- 42:44the outcomes
- 42:45were kind of
- 42:45equally good
- 42:46and so
- 42:47even if you
- 42:47had minor
- 42:48shrinkage
- 42:48or maybe
- 42:49even just
- 42:49a little bit
- 42:49of growth,
- 42:50there was
- 42:50still that
- 42:51sort of
- 42:51benefit
- 42:52in helping
- 42:52people
- 42:52live longer.
- 42:54and so one of
- 42:55the problems
- 42:56with Tebe
- 42:56then was,
- 42:58well,
- 42:58if size
- 42:59doesn't
- 42:59predict
- 43:00overall
- 43:00outcome,
- 43:01right,
- 43:01if our ability
- 43:01to shrink
- 43:02the cancer
- 43:03doesn't
- 43:03predict
- 43:03overall
- 43:04outcome,
- 43:04is there a
- 43:05better way
- 43:05for us
- 43:05to kind
- 43:06of assess
- 43:07that
- 43:07rather than
- 43:07scans,
- 43:08which is
- 43:08hugely what
- 43:09we do?
- 43:10And so,
- 43:11what we
- 43:12did in the
- 43:12earlier
- 43:13trials is,
- 43:14we collected
- 43:15something
- 43:15called
- 43:15circulating
- 43:16tumor DNA
- 43:18and
- 43:18circulating
- 43:19tumor DNA.
- 43:21basically
- 43:22the DNA is
- 43:23kind of the
- 43:23like the brain
- 43:24of all cells
- 43:25and all of our
- 43:25cells have
- 43:27DNA
- 43:28and DNA is
- 43:29can constantly
- 43:30shed from
- 43:31the cells,
- 43:31and you can
- 43:32detect it
- 43:32in the blood.
- 43:32If we do a
- 43:33blood test,
- 43:33we can find
- 43:34fragments of
- 43:35your
- 43:35normal DNA,
- 43:36but cancer
- 43:38also
- 43:38sheds DNA,
- 43:39and we can
- 43:40we can
- 43:40separate
- 43:41the cancer DNA
- 43:42from normal.
- 43:43And so
- 43:44what was
- 43:45kind of
- 43:45interesting
- 43:46here is
- 43:46that in
- 43:47all of
- 43:47the people
- 43:48we treated
- 43:48with Tebe,
- 43:49if the ctDNA
- 43:51decreased or
- 43:51if they
- 43:52cleared
- 43:52the DNA,
- 43:52if it just
- 43:53disappeared
- 43:53and
- 43:54we couldn't
- 43:54find any
- 43:54anymore,
- 43:55all those patients
- 43:56did well,
- 43:57and so
- 43:58there was
- 43:58a really
- 43:58strong
- 43:59correlation
- 44:00between our
- 44:00ability to
- 44:01get rid
- 44:02of that
- 44:02circulating
- 44:03cancer DNA
- 44:04and improved
- 44:05outcomes.
- 44:07and so
- 44:08based off of
- 44:08that data,
- 44:09the first
- 44:10of the
- 44:10the seven
- 44:11Tebe trials,
- 44:13and adjuvant
- 44:13just means,
- 44:14treatment
- 44:15after
- 44:16treatment
- 44:16with something
- 44:17after
- 44:17treatment
- 44:18with the eye.
- 44:18So the first
- 44:19of these kind
- 44:19of adjuvant
- 44:20trials,
- 44:21came
- 44:21was developed
- 44:22and this is
- 44:22being led by,
- 44:23Mark
- 44:24Middleton,
- 44:25Paul
- 44:25Nathan, and
- 44:25Joe Sacco
- 44:26in the UK.
- 44:28And what
- 44:28they're doing
- 44:28here is,
- 44:30they're
- 44:30identifying
- 44:31individuals
- 44:32with uveal
- 44:33melanoma
- 44:33that's high
- 44:34risk by
- 44:34cytogenetics
- 44:35or gene
- 44:36expression
- 44:37usually
- 44:37cytogenetics
- 44:38and large size
- 44:40who are treated
- 44:42who are HLA
- 44:43A*02:01
- 44:44positive
- 44:44because this
- 44:46drug
- 44:46only works
- 44:47if you are up
- 44:47to a
- 44:48one positive
- 44:49and then
- 44:50they check
- 44:50for ctDNA
- 44:51over time.
- 44:53If they
- 44:53detect ctDNA,
- 44:54then those,
- 44:55folks get six
- 44:56months
- 44:57of Tebentafusp
- 44:58and then
- 44:58they can see
- 44:59if we do that,
- 44:59can we
- 45:00actually clear
- 45:01reduce that
- 45:01the amount
- 45:02of ctDNA
- 45:03under the
- 45:04hypothesis
- 45:05that if we
- 45:05decrease
- 45:05the ctDNA,
- 45:07overall
- 45:07outcomes
- 45:07or the better
- 45:08likelihood of
- 45:09the cancer
- 45:09coming back
- 45:10will be lower?
- 45:11So, this is
- 45:12kind of one
- 45:13kind of
- 45:13ongoing trial.
- 45:14It's ctDNA
- 45:15based.
- 45:16It's kind of
- 45:17it's
- 45:17a big trial,
- 45:18but it's still
- 45:19going to be a
- 45:19little bit
- 45:19hypothesis
- 45:20generating,
- 45:22but what
- 45:23what we
- 45:23wanted to do
- 45:24was really
- 45:25design,
- 45:26a trial
- 45:27that would
- 45:28more
- 45:28definitively
- 45:29answer
- 45:29the question.
- 45:30If we do Tebe
- 45:31after
- 45:32treatment at
- 45:32the primary,
- 45:33can we improve
- 45:34outcomes
- 45:35and so this
- 45:36this is
- 45:37probably 7
- 45:38or 8 years
- 45:38in the making.
- 45:39And, it's
- 45:40finally going
- 45:40to open up
- 45:41this,
- 45:41this Fall,
- 45:43but we are
- 45:44beginning
- 45:45this,
- 45:46trial called
- 45:47the ATOM
- 45:47trial.
- 45:48It's an
- 45:48adjuvant
- 45:48heavy trial
- 45:50where
- 45:50we're going
- 45:50to take
- 45:50individuals
- 45:51with high risk
- 45:52uveal
- 45:53melanoma,
- 45:55A*02:01
- 45:55positive,
- 45:57without
- 45:58evidence
- 45:58of metastatic
- 45:59disease and,
- 46:01they're going
- 46:01to be
- 46:01randomized
- 46:02to either
- 46:02six months
- 46:03of Tebe
- 46:03or
- 46:03observation,
- 46:05and then we're
- 46:05going to
- 46:05follow.
- 46:07There's no
- 46:07placebo here.
- 46:08You know,
- 46:08if you're
- 46:09getting the drug
- 46:09or if
- 46:10you're on
- 46:10observation,
- 46:11observation
- 46:12is the
- 46:12standard
- 46:12of care.
- 46:13Right.
- 46:13So that's why,
- 46:14we feel like
- 46:15this is a very
- 46:16small trial
- 46:16to do
- 46:18and so
- 46:18this will be
- 46:19sponsored
- 46:20by a group
- 46:21called
- 46:21the EORTC,
- 46:22so it's
- 46:22a European
- 46:23group.
- 46:23We're going
- 46:24to open
- 46:24this up,
- 46:26and select
- 46:27sites
- 46:27in the US
- 46:27as well,
- 46:28hopefully up
- 46:29to ten sites,
- 46:30at least five,
- 46:31and,
- 46:31and hopefully
- 46:32those sites
- 46:32will be open,
- 46:34hopefully no
- 46:35later than
- 46:35early
- 46:35next year.
- 46:38I also
- 46:39just want
- 46:40to highlight
- 46:40that we
- 46:41do have
- 46:42what we call
- 46:42regional
- 46:43therapies
- 46:44that,
- 46:44can be
- 46:45very effective
- 46:46in this
- 46:46disease.
- 46:48So,
- 46:48because
- 46:49recurrence
- 46:50tends to occur
- 46:50in the liver,
- 46:51and then in
- 46:52some cases,
- 46:53in many cases
- 46:54it's what
- 46:54we call liver
- 46:55predominant
- 46:55the bulk
- 46:56of the cancers
- 46:56in the liver.
- 46:58A
- 46:58lot of times,
- 46:58what we do is
- 46:59think about
- 46:59is there a
- 47:00treatment
- 47:00we can put
- 47:00right into
- 47:01the liver,that
- 47:03can fight
- 47:03the cancer?
- 47:05And,
- 47:06there are
- 47:06a lot of
- 47:06different ways
- 47:06to do that.
- 47:07there's,
- 47:08one technique
- 47:09called
- 47:09intrahepatic,
- 47:10isolated
- 47:11hepatic
- 47:11perfusion.
- 47:13and there's
- 47:13another one
- 47:14called
- 47:14percutaneous
- 47:15hepatic
- 47:15perfusion.
- 47:16The concept
- 47:17between
- 47:18both of
- 47:18these
- 47:18techniques
- 47:19are kind of
- 47:19the same.
- 47:19Basically
- 47:20we isolate
- 47:21the liver,
- 47:23vasculature
- 47:23for. Right.
- 47:24So you kind
- 47:25of put the
- 47:25liver on like
- 47:25a bypass pump
- 47:27and you
- 47:27put in a
- 47:28lot of
- 47:28chemotherapy
- 47:29into the
- 47:29liver.
- 47:30and because
- 47:31of that
- 47:31bypass pump,
- 47:32the
- 47:32chemotherapy
- 47:33doesn't go
- 47:33into the
- 47:34rest of
- 47:34the body
- 47:34and cause
- 47:35a lot of
- 47:35side effects.
- 47:36and with
- 47:37that high dose
- 47:37chemotherapy,
- 47:39we can see
- 47:39some really
- 47:40nice,
- 47:41tumor
- 47:41shrinkage.
- 47:42And so this,
- 47:43platform
- 47:46by Delcath
- 47:47is the
- 47:48second
- 47:48treatment
- 47:48that was approved
- 47:49for uveal
- 47:49melanoma
- 47:50just
- 47:51in August
- 47:51of last year.
- 47:53Okay.
- 47:53And again,
- 47:54this is
- 47:54another tool
- 47:55I, suspect
- 47:56that we're
- 47:56going to
- 47:57be seeing,
- 47:57trials about
- 47:59can we do this
- 48:00in the
- 48:00preventative
- 48:01setting?
- 48:02to improve
- 48:03outcomes.
- 48:04nothing is,
- 48:07open yet,
- 48:08but there's
- 48:08certainly
- 48:09discussions.
- 48:11Okay.
- 48:12So that's
- 48:12basically,
- 48:14kind of
- 48:14strategy
- 48:14one let's,
- 48:15let's see
- 48:16what works
- 48:16in the
- 48:16metastatic
- 48:17setting
- 48:17and then
- 48:17try it in the
- 48:18preventative
- 48:18setting.
- 48:20Our option
- 48:21two is
- 48:22to perform
- 48:23what I call
- 48:23signal seeking
- 48:24adjuvant
- 48:25trials
- 48:26and then
- 48:27take the
- 48:27most promising
- 48:28therapies
- 48:29forward
- 48:29in
- 48:29confirmatory
- 48:30trials
- 48:31and here
- 48:31I want to talk
- 48:32about IpiNivo.
- 48:33The difference
- 48:35between 1
- 48:36and 2
- 48:36is like it
- 48:37for two here.
- 48:39it may be that
- 48:40we have to
- 48:40take therapies
- 48:41that we
- 48:41have not
- 48:42heavily
- 48:42tested in
- 48:44people
- 48:45with
- 48:45metastatic
- 48:45disease.
- 48:48or maybe
- 48:48we think
- 48:49the treatments
- 48:49will work
- 48:50better
- 48:51when there's
- 48:51only tiny
- 48:52bits of cancer
- 48:53in the body,
- 48:53as opposed
- 48:54to kind
- 48:55a more
- 48:55bulky cancer
- 48:56that I can see
- 48:56on the scans.
- 48:58And so
- 48:58that's what,
- 48:58kind of
- 48:59option two
- 49:00is here.
- 49:01And so,
- 49:03at ASCO,
- 49:04which is
- 49:05the big cancer
- 49:06meeting,
- 49:06just a few
- 49:07months ago,
- 49:07we reported
- 49:09the final
- 49:09results of a
- 49:10second trial
- 49:11of two
- 49:12immunotherapies,
- 49:14one called
- 49:14nivolumab
- 49:15and one
- 49:15called
- 49:15ipilimumab
- 49:16in patients
- 49:17with high
- 49:17risk of
- 49:18melanoma, and
- 49:19so this
- 49:19was the
- 49:20study design.
- 49:21Basically,
- 49:21we took people
- 49:22who had a high
- 49:23risk of
- 49:23the uveal
- 49:24melanoma
- 49:24coming back
- 49:26and all of
- 49:27the patients
- 49:27there got
- 49:28these two
- 49:29immunotherapies
- 49:30IpiNivo,
- 49:31immunotherapies
- 49:32that are
- 49:32commonly used
- 49:33for cutaneous
- 49:33melanoma.
- 49:34This is the regimen
- 49:35that gets us
- 49:35the 45 to 50%
- 49:37cure rate
- 49:37in skin
- 49:38melanoma,
- 49:39and then
- 49:39we followed
- 49:40them.
- 49:41This was not
- 49:42a
- 49:42randomized trial,
- 49:43so
- 49:43we used what
- 49:44we call
- 49:44a real world
- 49:45data set
- 49:46to create
- 49:46a synthetic
- 49:47control arm.
- 49:48So, we kind of
- 49:49we didn't
- 49:49randomize
- 49:50patients,
- 49:50but we use
- 49:51patients
- 49:51who were
- 49:51treated
- 49:52previously,
- 49:53and try to see
- 49:54the patients
- 49:55on this trial
- 49:55do better.
- 49:57I would say
- 49:57that the
- 49:58historical
- 49:58outcomes
- 49:59for IpiNivo
- 50:00in this
- 50:00disease,
- 50:01has been
- 50:02assessed
- 50:02in a couple
- 50:03of small,
- 50:04prospective
- 50:05trials.
- 50:06And we've
- 50:06looked at this
- 50:07in a couple of
- 50:07retrospective
- 50:08trials
- 50:09and the
- 50:10likelihood
- 50:10of major tumor
- 50:11shrinkage
- 50:11isn't that
- 50:12great,
- 50:13you know,
- 50:13maybe 15
- 50:14to 20%
- 50:14at best.
- 50:16The
- 50:16likelihood of,
- 50:17preventing
- 50:19tumor growth,
- 50:20not that
- 50:21great, right?
- 50:22It only seems
- 50:22to control
- 50:22cancer for a
- 50:23few months,
- 50:25but we
- 50:25hypothesized
- 50:26here that
- 50:28if we
- 50:28gave this
- 50:28treatment
- 50:29earlier, that
- 50:30maybe
- 50:30the outcomes
- 50:31would be
- 50:31better.
- 50:32And in fact,
- 50:33this is what
- 50:33we saw
- 50:34if we looked
- 50:34at the
- 50:35patients
- 50:35treated
- 50:36with IpiNivo
- 50:37at three
- 50:37years,
- 50:4070% of
- 50:42the patients
- 50:43were doing
- 50:43well,
- 50:44no evidence
- 50:44of cancer.
- 50:46Whereas if
- 50:46we look at
- 50:47our control
- 50:47arm,
- 50:48it was 43%.
- 50:50Right.
- 50:50So that was a
- 50:51significant
- 50:51difference
- 50:52or improvement
- 50:53for IpiNivo,
- 50:55versus
- 50:56that
- 50:56historical
- 50:56control.
- 51:00and so
- 51:00there are
- 51:03a number of
- 51:03other signals
- 51:04and trials,
- 51:04but I just
- 51:04wanted to
- 51:04highlight
- 51:05that one.
- 51:05That's
- 51:05one of
- 51:06the more
- 51:06recent ones.
- 51:08and then
- 51:08the third
- 51:09thing
- 51:10we've
- 51:10already heard
- 51:10a little
- 51:10bit of
- 51:11or a lot
- 51:11about this
- 51:12from is,
- 51:13maybe
- 51:13we can perform
- 51:14what we call
- 51:15signal seeking
- 51:15neoadjuvant
- 51:16trials
- 51:17and
- 51:17neoadjuvant
- 51:18means
- 51:18let's give a
- 51:19medicine
- 51:19before
- 51:20treatment
- 51:20of the eye,
- 51:22and then take
- 51:22the most
- 51:23promising
- 51:23therapies
- 51:24forward.
- 51:25The benefit
- 51:25of giving
- 51:27a treatment
- 51:27with,
- 51:28a medicine
- 51:29before
- 51:29treatment
- 51:29for the eye
- 51:30is that
- 51:30we can see
- 51:32does it work
- 51:33in shrinking
- 51:34that eye
- 51:34tumor? Right.
- 51:35Because maybe
- 51:36if the
- 51:36treatment
- 51:36actually helps
- 51:37to shrink
- 51:38the tumor,
- 51:39maybe
- 51:39those are
- 51:40the patients
- 51:40who are going
- 51:40to benefit
- 51:41the most,
- 51:41and maybe
- 51:42those are
- 51:42the patients
- 51:43who should be
- 51:43getting more
- 51:44of that
- 51:44treatment
- 51:45after the eye
- 51:46is treated.
- 51:48And so,
- 51:49I'll talk
- 51:49a little
- 51:49bit more
- 51:50about the
- 51:50Daro trial.
- 51:51So again,
- 51:52George spoke
- 51:52a lot
- 51:53about
- 51:53the mechanism
- 51:54so I'm not
- 51:55going to,
- 51:55kind of
- 51:56harp on that.
- 51:57I will say
- 51:57we've been
- 51:58looking
- 51:58at the idea
- 51:59of targeting
- 51:59PKC for
- 52:00a long time
- 52:01in ocular
- 52:02melanoma.
- 52:02So
- 52:03the early papers
- 52:04came out and,
- 52:05you know,
- 52:06to 2010, 2012.
- 52:09And so this
- 52:09has been
- 52:10definitely of
- 52:10high interest.
- 52:11We've done
- 52:12trials
- 52:13looking at
- 52:13PKC inhibition
- 52:14by itself.
- 52:16Again, these
- 52:16these were
- 52:17published in
- 52:172020 and
- 52:182023, but,
- 52:19you know,
- 52:19we were doing
- 52:20these trials
- 52:20a long
- 52:20time ago
- 52:22and the
- 52:23likelihood
- 52:23of major
- 52:24shrinkage
- 52:24for PKC
- 52:25inhibition
- 52:25alone.
- 52:26So that
- 52:26one pill
- 52:27by itself
- 52:28is also pretty
- 52:28modest, like,
- 52:29you know, 3%
- 52:30with this
- 52:31first
- 52:31generation
- 52:31PKC
- 52:32inhibitor, 13%
- 52:33with, Daro,
- 52:35which
- 52:35was called
- 52:35LXS196 before,
- 52:40and there were
- 52:40some toxicity
- 52:41counts too.
- 52:41So,
- 52:44you know,
- 52:44it was a
- 52:44little bit,
- 52:48I would have
- 52:48liked to see
- 52:49more,
- 52:49but as,
- 52:50as George
- 52:51said,
- 52:52IDEAYA
- 52:52has done
- 52:52a really
- 52:53great job
- 52:53looking to see
- 52:55can we combine
- 52:56Daro
- 52:57with other
- 52:57therapies
- 52:58and will
- 52:58that lead
- 52:59to better
- 53:00outcomes?
- 53:00Right.
- 53:02And so I want
- 53:02you to look
- 53:03at these
- 53:03plots,
- 53:04these
- 53:04waterfall
- 53:04plots. Right.
- 53:05Again,
- 53:05the lines
- 53:06going down
- 53:06means
- 53:07the tumor
- 53:07shrank.
- 53:08So here
- 53:09maybe a
- 53:09third line
- 53:10go down, here
- 53:10maybe like
- 53:11a little bit
- 53:12more than
- 53:12half of
- 53:12the lines
- 53:13go down,
- 53:15but
- 53:15this is the
- 53:16waterfall plot
- 53:18when Daro
- 53:19is combined
- 53:19with
- 53:20another
- 53:20pill called
- 53:21Crizotinib,
- 53:21and so we use
- 53:22this
- 53:22combination
- 53:22therapy where
- 53:24look at this,
- 53:24almost
- 53:24every single
- 53:25patient
- 53:25has tumor
- 53:26shrinkage.
- 53:27Right.
- 53:27And then this,
- 53:28this data
- 53:28set, 30%
- 53:30of patients
- 53:30have major
- 53:31shrinkage.
- 53:32That's
- 53:32really
- 53:32durable.
- 53:33Right,
- 53:34and I
- 53:35will say that
- 53:36this sort of
- 53:36waterfall
- 53:37plot,
- 53:37I've been
- 53:37studying
- 53:37this disease
- 53:38for a
- 53:38long time.
- 53:40We've never
- 53:40seen this
- 53:41before.
- 53:42so this
- 53:42this is
- 53:42really,
- 53:42really
- 53:43striking.
- 53:44and so
- 53:45what was also
- 53:45really cool
- 53:46is that in
- 53:47some of the
- 53:47early trials,
- 53:49some of the patients
- 53:49who were treated
- 53:50still had
- 53:51the eye
- 53:51melanoma,
- 53:52the primary
- 53:52lesion
- 53:53in place
- 53:53that was not treated.
- 53:54And so
- 53:55George showed
- 53:55some examples
- 53:56here.
- 53:57You know,
- 53:57here's
- 53:57some other
- 53:58ones. Right.
- 53:58So here's
- 53:59a 70 year
- 53:59old patient
- 54:00who had
- 54:00the primary
- 54:01melanoma there
- 54:02that
- 54:02you can see
- 54:02on PET scan,
- 54:05and just
- 54:06after
- 54:06a couple of weeks
- 54:07you can see
- 54:08it's
- 54:08the brightness
- 54:08is bad.
- 54:09That just
- 54:09means it's
- 54:10fairly active.
- 54:11Here
- 54:11the brightness
- 54:12is much less,
- 54:13right,
- 54:14which shows
- 54:15that it's
- 54:15less active.
- 54:15And it also
- 54:16shrunk down
- 54:17as well.
- 54:18And then
- 54:18here's
- 54:18another
- 54:18patient, 50
- 54:19year old.
- 54:20There's a
- 54:20tumor there.
- 54:21and after
- 54:22five months,
- 54:23significant
- 54:23shrinkage.
- 54:24Right.
- 54:25And so
- 54:25that led
- 54:26Anthony
- 54:26Joshua,
- 54:27to present,
- 54:29his trial.
- 54:30so this is
- 54:31this is
- 54:32a small
- 54:32kind of single
- 54:33seeking trial
- 54:33that he did
- 54:34in Australia
- 54:36called the
- 54:36NADOM trial,
- 54:38and basically
- 54:38what he did
- 54:39was he
- 54:40he found
- 54:40patients who,
- 54:41had a tumor
- 54:42that would
- 54:42require
- 54:43an
- 54:43enucleation.
- 54:44Initially,
- 54:45he would
- 54:45just treat
- 54:46with Daro
- 54:47for a month,
- 54:47do
- 54:48the surgery,
- 54:48and then maybe
- 54:49give more
- 54:50Daro,
- 54:50and then once
- 54:52he showed
- 54:53that was
- 54:53feasible
- 54:53and safe,
- 54:54he, finally
- 54:55found patients
- 54:56requiring
- 54:56enucleation,
- 54:57gave up
- 54:57to six
- 54:58months of
- 54:58Daro,
- 54:59did definitive
- 55:00treatment,
- 55:00and then had
- 55:00the option
- 55:01to do more
- 55:02Daro.
- 55:02And, so
- 55:03this is
- 55:03the slide
- 55:03that George
- 55:04showed
- 55:04you already.
- 55:05You saw this
- 55:07but I would
- 55:07just highlight
- 55:08that,
- 55:09of the
- 55:0912 patients
- 55:11who were
- 55:11treated
- 55:11on that
- 55:11expansion
- 55:12cohort,
- 55:13all 12 of
- 55:13those had
- 55:14a tumor
- 55:15that would
- 55:15require an
- 55:16enucleation.
- 55:18After
- 55:18the treatment
- 55:18course,
- 55:19only three
- 55:20required
- 55:20enucleation,
- 55:21which means
- 55:22nine patients
- 55:22had their
- 55:23eye saved,
- 55:24which is
- 55:25pretty cool
- 55:26I think,
- 55:27and so
- 55:27then that
- 55:28led to
- 55:28the trial
- 55:29that that
- 55:29George
- 55:29spoke about
- 55:30so not
- 55:30gonna spend
- 55:31anymore
- 55:31time here
- 55:33and, but
- 55:33we are really
- 55:34excited
- 55:34about this
- 55:35one.
- 55:37So this
- 55:37is just
- 55:37the landscape
- 55:38of where
- 55:39we are
- 55:39with, recent
- 55:40an ongoing
- 55:41adjuvant in
- 55:42neoadjuvant
- 55:42trials.
- 55:43up top
- 55:44are the,
- 55:45the neoadjuvant
- 55:46trials,
- 55:46including
- 55:47the Anthony
- 55:47Joshua trial
- 55:48in Australia,
- 55:49which, which
- 55:49just completed
- 55:50accrual,
- 55:50the ongoing
- 55:51Daro trial,
- 55:53and then the
- 55:53new adjuvant
- 55:54Tebe trial,
- 55:54which
- 55:55I didn't talk
- 55:55too much about,
- 55:56but that's
- 55:56going to
- 55:56be performed
- 55:57at Jefferson
- 55:58and should
- 55:58open up,
- 55:58shortly.
- 56:00And then
- 56:00we have
- 56:01the
- 56:01preventative
- 56:01adjuvant
- 56:02trials.
- 56:02I spoke
- 56:03about the
- 56:03IpiNivo trial.
- 56:04There's this,
- 56:05vaccine trial
- 56:06that has
- 56:06completed
- 56:07accrual.
- 56:08So I'm
- 56:08looking
- 56:08forward
- 56:08to seeing
- 56:09those results.
- 56:10There's
- 56:10some more data
- 56:11about adjuvant
- 56:12Sutent,
- 56:13alone
- 56:14in combination
- 56:15with another
- 56:15pill called
- 56:16valproic acid.
- 56:18The
- 56:18first cohort
- 56:19results
- 56:19was reported.
- 56:20There's
- 56:21additional
- 56:21data that
- 56:22should be
- 56:22reported
- 56:22shortly.
- 56:24And then
- 56:24we have
- 56:24the TebeMRD
- 56:26trial, the
- 56:27the big
- 56:27adjuvant
- 56:28European ATOM
- 56:29trial,
- 56:30and a few
- 56:31others
- 56:31as well,
- 56:34and so,
- 56:35where we are,
- 56:35overall here
- 56:36strategy
- 56:37one must take
- 56:37what works
- 56:38in metastatic
- 56:38disease
- 56:39treatment
- 56:39preventative
- 56:39setting.
- 56:40I talked
- 56:40about Tebe
- 56:42and so
- 56:42the ATOM trial
- 56:43will evaluate
- 56:44the idea
- 56:45of six months
- 56:46of adjuvant
- 56:46Tebe in a
- 56:47randomized
- 56:48trial.
- 56:48In terms of
- 56:49the signal
- 56:50seeking
- 56:50adjuvant
- 56:51trials,
- 56:51we had
- 56:51IpiNivo,
- 56:52which looked
- 56:52really good
- 56:53in that
- 56:53single arm,
- 56:54trial
- 56:55compared to
- 56:55historical
- 56:56controls.
- 56:57it's driving
- 56:57discussions
- 56:58to see
- 56:59can
- 56:59we actually do
- 57:00a randomized
- 57:01adjuvant
- 57:01IpiNivo trial,
- 57:02which is
- 57:03the only way
- 57:03to really
- 57:04tell,
- 57:04you know,
- 57:05these are
- 57:05preliminary
- 57:06results.
- 57:06Is it real?
- 57:08and then
- 57:08for Daro,
- 57:09we have
- 57:10the signal
- 57:10seeking
- 57:10neoadjuvant
- 57:11trial.
- 57:12I would love
- 57:12to hear
- 57:13George's
- 57:14thoughts
- 57:14about,
- 57:17when we
- 57:17might expect a
- 57:18true adjuvant
- 57:19trial,
- 57:19not to
- 57:20put you on
- 57:20the spot,
- 57:20George.
- 57:22But I think
- 57:23that's,
- 57:24something that
- 57:25should be
- 57:25considered
- 57:26a high
- 57:26priority.
- 57:29and lastly,
- 57:30I wanted to
- 57:31talk
- 57:31just really
- 57:32briefly about
- 57:32the importance
- 57:33of real world
- 57:34data.
- 57:34I showed you
- 57:35that IpiNivo
- 57:36trial
- 57:37and how we use
- 57:38historical
- 57:39data sets,
- 57:40right,
- 57:41to get
- 57:41a sense of,
- 57:43are the
- 57:43results
- 57:43good or bad?
- 57:44And,
- 57:45so generating
- 57:46that sort
- 57:47of data
- 57:47is really,
- 57:48really
- 57:48important.
- 57:50as I think
- 57:51the prior
- 57:52discussion,
- 57:52having
- 57:53the voice
- 57:54of the patient
- 57:55and the
- 57:55caregiver
- 57:55community
- 57:56is super,
- 57:56super
- 57:57important.
- 57:58Right?
- 57:58And so
- 57:59there are
- 57:59a number
- 58:00of efforts
- 58:00that are going
- 58:02to rigorously
- 58:03collect
- 58:03that
- 58:04information.
- 58:06A Cure
- 58:06In Sight,
- 58:07has their
- 58:08rare registry,
- 58:08as you know,
- 58:09so it's
- 58:10a patient
- 58:10powered
- 58:10registry
- 58:11where patients
- 58:12will go online
- 58:13consent and
- 58:14provide
- 58:14their own,
- 58:15data.
- 58:17the Melanoma
- 58:18Research
- 58:18Foundation,
- 58:20Cure OM Group
- 58:22has another
- 58:23patient
- 58:23powered
- 58:23registry,
- 58:24which is
- 58:25ongoing.
- 58:28Bill Harbor
- 58:28leads a group
- 58:29called the
- 58:29Collaborative
- 58:30Ocular
- 58:30Oncology
- 58:31Group, so
- 58:31that's a group
- 58:32of ocular
- 58:32oncologists
- 58:33and medical
- 58:33oncologist,
- 58:35putting data
- 58:36in a database
- 58:37so that
- 58:37we can learn,
- 58:39and we also
- 58:39have this
- 58:40International
- 58:41Ocular
- 58:41Melanoma
- 58:41Natural
- 58:42History Study,
- 58:42the Omni
- 58:43study,
- 58:43which is
- 58:44a global,
- 58:46effort that I
- 58:47worked on
- 58:48with Anthony
- 58:48Joshua
- 58:49and Joe Sacco
- 58:50where we have
- 58:51nearly
- 58:51a thousand,
- 58:52patients,
- 58:53enrolled.
- 58:54and I think,
- 58:56we need
- 58:56other avenues
- 58:57to learn
- 58:57beyond
- 58:58prospective
- 58:58clinical trials
- 58:59and this is
- 59:00a mechanism
- 59:00to do that
- 59:02and using
- 59:03these kind
- 59:04of registries,
- 59:05I think
- 59:05we're going
- 59:05to be able
- 59:05to answer
- 59:06questions
- 59:07regarding,
- 59:08are we
- 59:08actually
- 59:09educating
- 59:10patients
- 59:11caregivers
- 59:12right away?
- 59:13Are we doing
- 59:13the shared
- 59:13decision
- 59:14making that
- 59:14we should be
- 59:15doing?
- 59:16what are additional
- 59:16concerns
- 59:17that we
- 59:17may not
- 59:18be addressing
- 59:19right.
- 59:19From the
- 59:19clinician
- 59:20registries?
- 59:21We can
- 59:21look at,
- 59:21you know,
- 59:22the various
- 59:22correlations
- 59:23that,
- 59:24are hard
- 59:25to do
- 59:25with these
- 59:25single center
- 59:26databases.
- 59:28and when
- 59:29if we
- 59:29ever combine
- 59:30these data
- 59:30sets,
- 59:31combining
- 59:31patient
- 59:32preference
- 59:32with clinical
- 59:33outcomes,
- 59:34I think
- 59:34could be
- 59:34really, really
- 59:35powerful.
- 59:37So, I'll
- 59:37leave it
- 59:37at that.
- 59:38I
- 59:38do want to
- 59:39again,
- 59:40just thank,
- 59:41thank
- 59:41all of you
- 59:41for,
- 59:42listening.
- 59:43Danet,
- 59:43I'd like to
- 59:44thank you
- 59:44for putting
- 59:44this together.
- 59:45I think
- 59:46this is
- 59:46really
- 59:46critical
- 59:47we're a team,
- 59:48and I want to
- 59:49work with you,
- 59:50to fix
- 59:50this problem,
- 59:52so I will
- 59:52stop there.
- 59:53Dr Carvajal,
- 59:54thank you
- 59:54so, so much.
- 59:55This was
- 59:56so exciting
- 59:57to hear
- 59:57and I hope
- 59:58that everybody
- 59:59who's
- 59:59listening
- 59:59is hearing
- 1:00:00and
- 1:00:01understanding
- 1:00:01your excitement
- 1:00:02just around
- 1:00:03the
- 1:00:03information
- 1:00:03that you guys
- 1:00:04are seeing,
- 1:00:04the data
- 1:00:05that
- 1:00:05we're seeing
- 1:00:06and just
- 1:00:06the potential
- 1:00:07that we have
- 1:00:07for some
- 1:00:08of these newer
- 1:00:08treatments,
- 1:00:09and some of
- 1:00:10the ones
- 1:00:10that
- 1:00:10we're just
- 1:00:10seeing used
- 1:00:11in new ways.
- 1:00:12let's go ahead
- 1:00:12and move
- 1:00:13on to, Doctor
- 1:00:15Cole again
- 1:00:16from Ideaya
- 1:00:17Biosciences is
- 1:00:18back again
- 1:00:18to discuss
- 1:00:19the Daro
- 1:00:20Crizotinib
- 1:00:20combination
- 1:00:21trial.
- 1:00:21And,
- 1:00:22just give us
- 1:00:23a little more
- 1:00:23of an update
- 1:00:24on where
- 1:00:24the metastatic
- 1:00:25treatment of
- 1:00:26the IDE196
- 1:00:27is going.
- 1:00:28Absolutely.
- 1:00:29Let me,
- 1:00:29share
- 1:00:30my screen here
- 1:00:31All right,
- 1:00:31everyone,
- 1:00:32let me know
- 1:00:32if here
- 1:00:33if
- 1:00:34the screen
- 1:00:34here is
- 1:00:35blocking
- 1:00:36some things.
- 1:00:36It
- 1:00:36didn't do this
- 1:00:37the last time.
- 1:00:39and again,
- 1:00:39thank you
- 1:00:40for that
- 1:00:40wonderful
- 1:00:40introduction
- 1:00:41and my
- 1:00:42goodness,
- 1:00:42Richard,
- 1:00:43thank you
- 1:00:44very much.
- 1:00:44I hope to be
- 1:00:45able to answer
- 1:00:46some of
- 1:00:46your
- 1:00:46questions.
- 1:00:47If not,
- 1:00:47I will
- 1:00:48definitely
- 1:00:48be back
- 1:00:49in touch
- 1:00:49with you.
- 1:00:50But let's
- 1:00:50take a look
- 1:00:51at some of
- 1:00:51the things
- 1:00:51that, IDEAYA
- 1:00:52and
- 1:00:53Darovasertib
- 1:00:54is doing,
- 1:00:55in the
- 1:00:55metastatic
- 1:00:56uveal
- 1:00:56melanoma,
- 1:00:57setting.
- 1:00:58Again, I'm
- 1:00:58going to give
- 1:00:59some
- 1:00:59information
- 1:01:00which,
- 1:01:00Richard
- 1:01:00already showed
- 1:01:01a little
- 1:01:01bit of
- 1:01:02in terms
- 1:01:03of our phase
- 1:01:03one two
- 1:01:04clinical trial
- 1:01:05combination
- 1:01:05of
- 1:01:05Darovasertib
- 1:01:07plus
- 1:01:07Crizotinib,
- 1:01:08and again,
- 1:01:09as was
- 1:01:10mentioned
- 1:01:10before, it's
- 1:01:11very important
- 1:01:11to
- 1:01:11keep in mind
- 1:01:12the one I'm
- 1:01:12about to
- 1:01:13show you
- 1:01:14in some of
- 1:01:14the later
- 1:01:15slides after,
- 1:01:15we look
- 1:01:16at some of
- 1:01:16this data.
- 1:01:18This is
- 1:01:18single agent
- 1:01:19data, so phase
- 1:01:20one,
- 1:01:20phase two,
- 1:01:21not phase
- 1:01:22three.
- 1:01:22So when
- 1:01:23we look at,
- 1:01:25our patient
- 1:01:25characteristics
- 1:01:26in this trial
- 1:01:27and this data
- 1:01:28was presented
- 1:01:29last year
- 1:01:30at ESMO,
- 1:01:31important
- 1:01:32thing to keep
- 1:01:32in mind
- 1:01:32in terms
- 1:01:33of our phase
- 1:01:34two trial
- 1:01:34here,
- 1:01:35when we look
- 1:01:35at the
- 1:01:35severity
- 1:01:36of disease
- 1:01:36in our patients.
- 1:01:37So when
- 1:01:37we look
- 1:01:38at one of our
- 1:01:38markers, LDH,
- 1:01:4060% of
- 1:01:41our patients
- 1:01:42in any line
- 1:01:43cohort
- 1:01:43in patients
- 1:01:44who are not
- 1:01:44frontline
- 1:01:46had a
- 1:01:46increased LDH,
- 1:01:47you know,
- 1:01:48greater than
- 1:01:4860% of
- 1:01:49those patients
- 1:01:49had an
- 1:01:50increased,
- 1:01:50LDH,
- 1:01:51whereas
- 1:01:51our patients
- 1:01:52in the
- 1:01:52frontline
- 1:01:53cohort,
- 1:01:5350% of
- 1:01:54those patients
- 1:01:55had
- 1:01:55an increase
- 1:01:56in LDH.
- 1:01:57If we look
- 1:01:57at our tumor
- 1:01:58sizes
- 1:01:59down here,
- 1:01:59the largest
- 1:02:00metastatic
- 1:02:00lesion,
- 1:02:01the majority
- 1:02:01of the lesions
- 1:02:02in both of
- 1:02:03our cohorts
- 1:02:03were between
- 1:02:043 to 8cm.
- 1:02:06And also,
- 1:02:06if we look at
- 1:02:07the location
- 1:02:07of the
- 1:02:07metastases,
- 1:02:08greater
- 1:02:09than 50%
- 1:02:09of the
- 1:02:10patients in
- 1:02:10both of
- 1:02:11our arms
- 1:02:12had both liver
- 1:02:13and extra
- 1:02:14hepatic
- 1:02:15or outside
- 1:02:15liver,
- 1:02:16metastatic
- 1:02:17disease.
- 1:02:18So
- 1:02:18these patients
- 1:02:18have a
- 1:02:19pretty high
- 1:02:19severity
- 1:02:20of disease,
- 1:02:20so we're
- 1:02:21in our phase
- 1:02:21one two trial
- 1:02:22and they're
- 1:02:22looking at
- 1:02:23our phase two
- 1:02:23data
- 1:02:24right now.
- 1:02:25So again
- 1:02:26this is
- 1:02:27a, waterfall
- 1:02:28plot
- 1:02:28that we've seen
- 1:02:29before.
- 1:02:29But this one
- 1:02:30sort of
- 1:02:30divided up
- 1:02:30into two
- 1:02:31different
- 1:02:32areas.
- 1:02:34and again,
- 1:02:34we want to
- 1:02:35see bars down,
- 1:02:36not bars up
- 1:02:37and so by
- 1:02:38looking
- 1:02:38at these
- 1:02:39two things,
- 1:02:39we have a lot
- 1:02:39of bars down
- 1:02:40and that's a
- 1:02:40good thing.
- 1:02:41The plus signs
- 1:02:43talk about
- 1:02:43confirmed
- 1:02:44response
- 1:02:45in a single
- 1:02:46arm trial,
- 1:02:46we confirm
- 1:02:47response with
- 1:02:48two Cat scans.
- 1:02:49So two cycles,
- 1:02:51these patients
- 1:02:51had two Cat
- 1:02:52scans
- 1:02:52confirming
- 1:02:53that response.
- 1:02:55Top waterfall
- 1:02:56plot is front
- 1:02:57line, bottom
- 1:02:58waterfall plot
- 1:02:58any line.
- 1:02:59So, patients
- 1:03:00this is a
- 1:03:00combination of
- 1:03:01front line
- 1:03:01and patients
- 1:03:03who have
- 1:03:03more than one
- 1:03:04line
- 1:03:04of therapy.
- 1:03:05The confirmed
- 1:03:06overall
- 1:03:06response
- 1:03:07for our
- 1:03:07front line
- 1:03:08patients
- 1:03:08about 45%
- 1:03:09and
- 1:03:10importantly
- 1:03:10our disease
- 1:03:11control rate
- 1:03:11was about 90%.
- 1:03:13So disease
- 1:03:14control rate
- 1:03:14is a criteria
- 1:03:15that we use
- 1:03:16that
- 1:03:16encompasses,
- 1:03:17stable
- 1:03:17disease.
- 1:03:18Another thing
- 1:03:19that I want
- 1:03:19to say
- 1:03:20when we
- 1:03:20take a look
- 1:03:21at tumors
- 1:03:21that had,
- 1:03:22greater
- 1:03:22than 30%
- 1:03:23shrinkage
- 1:03:24overall,
- 1:03:2412 out of 20
- 1:03:25and it was
- 1:03:2560% of
- 1:03:26the patients,
- 1:03:27but what's
- 1:03:28exciting to me
- 1:03:29is our disease
- 1:03:29control rate
- 1:03:30here in the
- 1:03:31single arm
- 1:03:31study,
- 1:03:32about 90%
- 1:03:33stable
- 1:03:33disease,
- 1:03:34as we've now,
- 1:03:35I would say,
- 1:03:36modern
- 1:03:36oncology
- 1:03:37to have
- 1:03:37developed
- 1:03:37modern
- 1:03:38thinking.
- 1:03:38Stable
- 1:03:39disease is not
- 1:03:40a bad thing.
- 1:03:41It truly means
- 1:03:41that we
- 1:03:42still have
- 1:03:42that tumor
- 1:03:43under control.
- 1:03:44When we look
- 1:03:44at our 80 line
- 1:03:45patients
- 1:03:46down here
- 1:03:46again,
- 1:03:46patients
- 1:03:47that are
- 1:03:47more heavily,
- 1:03:48treated
- 1:03:49confirmed
- 1:03:49overall
- 1:03:50response
- 1:03:50rates,
- 1:03:50about 30%, but
- 1:03:52our disease
- 1:03:52control
- 1:03:53rate, again
- 1:03:53is about
- 1:03:54the same
- 1:03:54as those
- 1:03:54frontline
- 1:03:55patients.
- 1:03:55About 90%,
- 1:03:56again,
- 1:03:58very
- 1:03:58encouraging
- 1:03:59single line
- 1:04:00data.
- 1:04:01And again
- 1:04:02down here
- 1:04:02where we're
- 1:04:03looking at
- 1:04:03this are
- 1:04:03heavily
- 1:04:04pretreated
- 1:04:04patients.
- 1:04:0562% of
- 1:04:06these patients
- 1:04:06in this
- 1:04:07waterfall plot
- 1:04:08that we're
- 1:04:08taking a look
- 1:04:09at down here
- 1:04:10at two
- 1:04:10or greater,
- 1:04:11prior lines
- 1:04:12of therapy.
- 1:04:15So I'm
- 1:04:16sorry
- 1:04:16about this on
- 1:04:17the side here.
- 1:04:18I couldn't
- 1:04:18get that
- 1:04:18to shrink,
- 1:04:19but what we're
- 1:04:20trying to
- 1:04:20show here
- 1:04:21is that
- 1:04:21even in our
- 1:04:22trial,
- 1:04:23we are
- 1:04:23HLA-A2
- 1:04:24agnostics.
- 1:04:25So we had both
- 1:04:27HLA-A2
- 1:04:27Negative
- 1:04:27patients
- 1:04:28and HLA-A2
- 1:04:29Positive
- 1:04:29patients
- 1:04:30enrolled
- 1:04:31in this trial.
- 1:04:32And what's
- 1:04:32interesting
- 1:04:33for us is
- 1:04:33when we
- 1:04:34take a look
- 1:04:34at our
- 1:04:34response
- 1:04:35rates again,
- 1:04:36single arm
- 1:04:36data,
- 1:04:37the response
- 1:04:38rate for
- 1:04:38our frontline
- 1:04:39patients
- 1:04:39about 42%
- 1:04:41and when we
- 1:04:41take a
- 1:04:42look at,
- 1:04:42our HLA-A2
- 1:04:44positive
- 1:04:45patients,
- 1:04:45frontline
- 1:04:46patients,
- 1:04:46there's a 62%
- 1:04:47response rate.
- 1:04:48Both patients
- 1:04:49have 100%
- 1:04:49disease
- 1:04:50control rate.
- 1:04:51When we look
- 1:04:51at any line
- 1:04:52patients,
- 1:04:53HLA2 Negative,
- 1:04:54we have
- 1:04:5429% or 30%,
- 1:04:57response rate
- 1:04:58with about
- 1:04:58a 94%
- 1:04:59control rate.
- 1:05:01And when
- 1:05:01we look
- 1:05:01at any line
- 1:05:02in the HLA2
- 1:05:03Positive
- 1:05:03patients,
- 1:05:04but a 32%,
- 1:05:05response
- 1:05:06rate with an
- 1:05:0684% disease
- 1:05:08control rate,
- 1:05:10when we look
- 1:05:11at our, I'll
- 1:05:12say molecular
- 1:05:13responses
- 1:05:13with the
- 1:05:14circulating
- 1:05:15tumor DNA
- 1:05:17again,
- 1:05:17when we,
- 1:05:19and this
- 1:05:19is
- 1:05:19regardless of
- 1:05:20BAP1 mutation,
- 1:05:21you get
- 1:05:21another
- 1:05:22mutation,
- 1:05:22which,
- 1:05:23which portends
- 1:05:24to more
- 1:05:25aggressive
- 1:05:25disease
- 1:05:26or disease
- 1:05:27or bad
- 1:05:27tumor markers.
- 1:05:28We like
- 1:05:28to say,
- 1:05:29we're showing
- 1:05:29pretty robust
- 1:05:30responses
- 1:05:31here.
- 1:05:31Again,
- 1:05:32single agent,
- 1:05:33single arm
- 1:05:33trial
- 1:05:34with patients,
- 1:05:35whether
- 1:05:36they are
- 1:05:36first line
- 1:05:36patients
- 1:05:37and or
- 1:05:37pretreated
- 1:05:38patients.
- 1:05:39And again,
- 1:05:40keep in mind
- 1:05:41this is our
- 1:05:41combination
- 1:05:42Darovasertib
- 1:05:42and
- 1:05:43Crizotinib.
- 1:05:44And
- 1:05:44so one thing
- 1:05:45that we have
- 1:05:45to remember
- 1:05:45with our uveal
- 1:05:46melanoma
- 1:05:47patients,
- 1:05:48as Richard
- 1:05:48showed,
- 1:05:48and as
- 1:05:49I showed
- 1:05:49a little bit
- 1:05:50before,
- 1:05:50Darovasertib
- 1:05:51is really
- 1:05:52shutting down,
- 1:05:52signaling
- 1:05:53through PKC
- 1:05:55another route
- 1:05:56of signaling
- 1:05:57with a
- 1:05:57uveal melanoma
- 1:05:58is through
- 1:05:58MET and
- 1:06:00most of
- 1:06:00our patients
- 1:06:01another
- 1:06:01oncogene.
- 1:06:02Most of
- 1:06:03our patients
- 1:06:03are showing
- 1:06:04increased
- 1:06:04signaling
- 1:06:05through MET
- 1:06:06and Crizotinib
- 1:06:07stops
- 1:06:07signaling
- 1:06:08through MET,
- 1:06:10and in
- 1:06:10Richard’s
- 1:06:10slide, he
- 1:06:11showed
- 1:06:11that aspect
- 1:06:12of the MET
- 1:06:12signaling in.
- 1:06:13So what
- 1:06:14we like
- 1:06:14to think
- 1:06:14in terms
- 1:06:15of this
- 1:06:15combination,
- 1:06:16we’re
- 1:06:16shutting down
- 1:06:17all signaling
- 1:06:18that is
- 1:06:18driving
- 1:06:19this tumor.
- 1:06:21So when we
- 1:06:22look at,
- 1:06:23progression
- 1:06:23free survival,
- 1:06:24in these
- 1:06:25patients
- 1:06:26and again,
- 1:06:26it's
- 1:06:27any line,
- 1:06:28when we
- 1:06:28look at
- 1:06:28front line
- 1:06:29patients
- 1:06:29and or
- 1:06:30any line
- 1:06:30patients
- 1:06:31about the
- 1:06:31same amount
- 1:06:32of progression
- 1:06:32free survival,
- 1:06:33about
- 1:06:33seven months
- 1:06:34frontline,
- 1:06:357.11
- 1:06:36specifically,
- 1:06:37any line,
- 1:06:376.8 months
- 1:06:39for patients
- 1:06:39who only have,
- 1:06:40liver
- 1:06:41disease, it's
- 1:06:41about
- 1:06:4211 months.
- 1:06:43the treatment
- 1:06:44duration
- 1:06:44for this,
- 1:06:4550% of
- 1:06:46the patients
- 1:06:46had treatment
- 1:06:47greater
- 1:06:47than six
- 1:06:48months,
- 1:06:48and about
- 1:06:4830% of
- 1:06:49the patients
- 1:06:49had treatment
- 1:06:50for greater
- 1:06:50than one year.
- 1:06:54When we
- 1:06:54look at our,
- 1:06:55Kaplan-Meier
- 1:06:55curves
- 1:06:56and again,
- 1:06:56we have to
- 1:06:57take this
- 1:06:57slide
- 1:06:58a little
- 1:06:58bit of
- 1:06:58grain of salt
- 1:06:59because the,
- 1:07:01Darovasertib/Crizotinib
- 1:07:03data is
- 1:07:03not phase
- 1:07:04three data,
- 1:07:05but we're
- 1:07:05trying to see
- 1:07:06where
- 1:07:06this falls
- 1:07:07into place
- 1:07:07with some
- 1:07:08other phase
- 1:07:08three data.
- 1:07:09And I
- 1:07:09would say
- 1:07:10when we look
- 1:07:11at this,
- 1:07:11Kaplan-Meier
- 1:07:12curve
- 1:07:13compared to
- 1:07:13some other
- 1:07:13curves,
- 1:07:14this gave us
- 1:07:15the confidence
- 1:07:16and the
- 1:07:16wherewithal
- 1:07:17to conduct
- 1:07:18our phase
- 1:07:18three trial.
- 1:07:20But we do
- 1:07:20see, the
- 1:07:21PFS curve
- 1:07:22and I think
- 1:07:23the most
- 1:07:23important
- 1:07:23thing to
- 1:07:24keep in mind
- 1:07:25from
- 1:07:25this slide,
- 1:07:25if we look at
- 1:07:26that control
- 1:07:27arm where
- 1:07:27the Daro in
- 1:07:28the arm is.
- 1:07:31So again,
- 1:07:32when we
- 1:07:33look at this
- 1:07:33line again,
- 1:07:34we're not
- 1:07:34comparing
- 1:07:35cross,
- 1:07:35but we're just
- 1:07:36taking a look
- 1:07:36at
- 1:07:36some numbers,
- 1:07:37and we see
- 1:07:38this slide
- 1:07:38along
- 1:07:39with the,
- 1:07:39curve that
- 1:07:39I just showed
- 1:07:40you gave us
- 1:07:41that
- 1:07:41confidence
- 1:07:43to move
- 1:07:43forward
- 1:07:43with our phase
- 1:07:44three trial.
- 1:07:45So when we
- 1:07:46look at our,
- 1:07:46potential,
- 1:07:47tolerability,
- 1:07:48safety,
- 1:07:48something
- 1:07:49that was
- 1:07:49mentioned
- 1:07:49before,
- 1:07:50when we look
- 1:07:51at our
- 1:07:51grade greater
- 1:07:51than or equal
- 1:07:53to three,
- 1:07:53AEs that we're
- 1:07:54drug related.
- 1:07:55About a third
- 1:07:55of the
- 1:07:55patients
- 1:07:56in our
- 1:07:57phase, one,
- 1:07:58two trial,
- 1:07:59when we
- 1:07:59look at
- 1:07:59tumor
- 1:08:00shrinkage
- 1:08:00with a
- 1:08:01robust
- 1:08:01amount of
- 1:08:02tumor
- 1:08:02shrinkage
- 1:08:03when we
- 1:08:03look at
- 1:08:04ORR again,
- 1:08:05frontline
- 1:08:05about 45%
- 1:08:06patients,
- 1:08:07and some
- 1:08:08had a
- 1:08:08confirmed,
- 1:08:09overall
- 1:08:09response rate,
- 1:08:10about 30%
- 1:08:12had a
- 1:08:13confirmed
- 1:08:13overall
- 1:08:13response rate.
- 1:08:14Again,
- 1:08:14this is by
- 1:08:15our RECIST
- 1:08:151.1.
- 1:08:16And when we
- 1:08:17look
- 1:08:17at our PFS,
- 1:08:18we know about
- 1:08:18seven months.
- 1:08:22So what is our
- 1:08:23phase
- 1:08:23two three
- 1:08:23trial
- 1:08:24look like.
- 1:08:24This trial is
- 1:08:25currently
- 1:08:25open,
- 1:08:25currently
- 1:08:26running
- 1:08:27and I can say
- 1:08:28we are
- 1:08:28HLA restricted
- 1:08:30to HLA
- 1:08:30to negative
- 1:08:31patients only,
- 1:08:32and that's
- 1:08:33because,
- 1:08:33you know,
- 1:08:33having good
- 1:08:34therapy
- 1:08:35is already
- 1:08:35approved
- 1:08:36for HLA2
- 1:08:36positive
- 1:08:37patients.
- 1:08:38So this is
- 1:08:42the schema
- 1:08:43of our phase
- 1:08:44two three
- 1:08:44trial.
- 1:08:45We are currently
- 1:08:46in the phase
- 1:08:47two portion
- 1:08:48right now.
- 1:08:49what we're
- 1:08:49doing now
- 1:08:50and because of
- 1:08:51some, guidance
- 1:08:51or recent
- 1:08:52guidance
- 1:08:53from the FDA,
- 1:08:54the phase
- 1:08:55two portion
- 1:08:55of this trial
- 1:08:56is sort
- 1:08:56of divided
- 1:08:56into two
- 1:08:57parts, phase
- 1:08:57two A
- 1:08:58and phase
- 1:08:58two B.
- 1:09:00We're having
- 1:09:00a dose
- 1:09:01optimization
- 1:09:02part of
- 1:09:02this trial.
- 1:09:03At first,
- 1:09:03figure out
- 1:09:04the
- 1:09:04appropriate
- 1:09:04dose for
- 1:09:05Darovasertib
- 1:09:05in combination
- 1:09:07with
- 1:09:07Crizotinib
- 1:09:07I think
- 1:09:08we have
- 1:09:08an idea
- 1:09:08what that dose
- 1:09:09should be,
- 1:09:10but in order
- 1:09:10to make sure
- 1:09:11that we tested
- 1:09:11appropriately,
- 1:09:12we're testing
- 1:09:13two doses.
- 1:09:13We plan
- 1:09:14on having
- 1:09:15a meeting
- 1:09:15with our
- 1:09:16independent
- 1:09:16data review
- 1:09:17committee
- 1:09:18and that
- 1:09:18meeting, in
- 1:09:19a blinded
- 1:09:20fashion,
- 1:09:20will help
- 1:09:20us determine
- 1:09:21what is the
- 1:09:22best dose.
- 1:09:23We're looking
- 1:09:23at both
- 1:09:23efficacy
- 1:09:24and safety
- 1:09:24to do that,
- 1:09:25once
- 1:09:26we have confirmed
- 1:09:27what our dose
- 1:09:28is going
- 1:09:28to be,
- 1:09:29we're going
- 1:09:29to continue
- 1:09:30to enroll
- 1:09:31patients at
- 1:09:31that next
- 1:09:32level
- 1:09:34to
- 1:09:34complete the
- 1:09:34phase two
- 1:09:35portion,
- 1:09:35patients
- 1:09:36that were at
- 1:09:37a different
- 1:09:37dose level
- 1:09:38and will
- 1:09:38then be
- 1:09:39allowed to
- 1:09:39move up
- 1:09:40to, that
- 1:09:41other dose
- 1:09:41level,
- 1:09:42or down,
- 1:09:43and as
- 1:09:44that dose
- 1:09:44level
- 1:09:44was chosen,
- 1:09:46our goal is
- 1:09:47to have
- 1:09:47accelerated
- 1:09:48approval
- 1:09:49and rapid
- 1:09:49phase
- 1:09:50two portion,
- 1:09:50that's
- 1:09:51going to
- 1:09:51be a PFS.
- 1:09:51After that,
- 1:09:52we are
- 1:09:52simultaneously
- 1:09:53enrolling
- 1:09:53patients
- 1:09:54in the phase
- 1:09:55three portion,
- 1:09:56where the
- 1:09:57primary
- 1:09:58endpoint
- 1:09:58is going to be
- 1:09:59overall
- 1:09:59survival
- 1:10:00and similar
- 1:10:01to, Dr.
- 1:10:02Correa,
- 1:10:03I mentioned
- 1:10:03before,
- 1:10:04we do have
- 1:10:04fast track
- 1:10:05designation
- 1:10:06for, our,
- 1:10:06combination
- 1:10:07with the
- 1:10:07Darovasertib
- 1:10:08plus
- 1:10:08Crizotinib
- 1:10:08in metastatic
- 1:10:09uveal
- 1:10:09melanoma.
- 1:10:12And so this is
- 1:10:12our overall,
- 1:10:14clinical
- 1:10:14development
- 1:10:14strategy.
- 1:10:16our plan
- 1:10:16is to, yes,
- 1:10:17move forward
- 1:10:18in the
- 1:10:18neoadjuvant
- 1:10:19setting
- 1:10:20with both,
- 1:10:20HLA2 Positive
- 1:10:21and HLA2
- 1:10:22Negative
- 1:10:22patients.
- 1:10:24Yes, we
- 1:10:24do have a,
- 1:10:25adjuvant plan.
- 1:10:27I could say,
- 1:10:28Richard with
- 1:10:28a big smile
- 1:10:29and I'm
- 1:10:29sure
- 1:10:29I'll be on
- 1:10:30to talk
- 1:10:30with you
- 1:10:30the next couple
- 1:10:31of weeks.
- 1:10:31But, since
- 1:10:32ASCO, myself
- 1:10:34and my,
- 1:10:34lead
- 1:10:35statistician
- 1:10:36have been,
- 1:10:36vigorously
- 1:10:37designing,
- 1:10:39a adjuvant
- 1:10:40trial,
- 1:10:41and over here,
- 1:10:41we see what
- 1:10:42we're doing
- 1:10:43right now
- 1:10:44in terms
- 1:10:44of metastatic
- 1:10:46melanoma.
- 1:10:46The trial
- 1:10:47that I just
- 1:10:47presented,
- 1:10:48data for
- 1:10:48and spoke
- 1:10:49about,
- 1:10:50we've just
- 1:10:50amended that
- 1:10:51clinical
- 1:10:51trial,
- 1:10:52actually
- 1:10:52to enroll
- 1:10:53an arm
- 1:10:54with an HLA2
- 1:10:55Positive
- 1:10:56patients.
- 1:10:59And that ends
- 1:11:00this portion
- 1:11:00of my
- 1:11:00presentation
- 1:11:01We
- 1:11:01are going to
- 1:11:02bring on Dr.
- 1:11:03Vukovic
- 1:11:03from the
- 1:11:04Delcath
- 1:11:04Systems
- 1:11:05company,
- 1:11:05and they're
- 1:11:06going to talk
- 1:11:06a little
- 1:11:06about the FDA
- 1:11:07approval of
- 1:11:08Hepzato kit.
- 1:11:08So thank
- 1:11:09you, Doctor
- 1:11:09Vukovic, for
- 1:11:10being here.
- 1:11:11I thank you
- 1:11:12very much
- 1:11:12for
- 1:11:13inviting me,
- 1:11:13and happy
- 1:11:14to be here
- 1:11:14to have
- 1:11:15the
- 1:11:15opportunity
- 1:11:16to, tell
- 1:11:16a couple
- 1:11:17of things
- 1:11:17about,
- 1:11:18Hepzato and,
- 1:11:19focus trial
- 1:11:20and what's
- 1:11:21next.
- 1:11:21So I'll start
- 1:11:22by sharing
- 1:11:23my screen.
- 1:11:24So, again,
- 1:11:25I'm going
- 1:11:26to tell
- 1:11:26a couple
- 1:11:27of things
- 1:11:27about
- 1:11:27Hepzato kit,
- 1:11:29and my name
- 1:11:30is Voji
- 1:11:30Vukovic
- 1:11:31and I’m chief
- 1:11:31medical
- 1:11:31officer
- 1:11:32at Delcath
- 1:11:33Systems.
- 1:11:34Doctor
- 1:11:35Carlisle has
- 1:11:35already kindly
- 1:11:36introduced,
- 1:11:37so I'm
- 1:11:37going to go
- 1:11:38relatively
- 1:11:38quickly
- 1:11:39to the
- 1:11:39first couple
- 1:11:40of slides.
- 1:11:41The Hepzato
- 1:11:42kit is
- 1:11:43basically
- 1:11:43a drug device
- 1:11:44combination
- 1:11:44and the
- 1:11:45relatively
- 1:11:46complex schema
- 1:11:47shown here
- 1:11:48on the right
- 1:11:49shows
- 1:11:49it's really
- 1:11:49essentially
- 1:11:50a system
- 1:11:51of catheters
- 1:11:52that's
- 1:11:52inserted into
- 1:11:53the patient's
- 1:11:53blood vessels
- 1:11:54to isolate
- 1:11:56the
- 1:11:56circulation
- 1:11:57and deliver
- 1:11:58and, to allow
- 1:12:00for high dose
- 1:12:01chemotherapy
- 1:12:01to be
- 1:12:02administered
- 1:12:03just
- 1:12:04to the liver.
- 1:12:05And then
- 1:12:06the blood
- 1:12:06that flows
- 1:12:07out of the
- 1:12:07liver
- 1:12:08is isolated
- 1:12:10and, taken
- 1:12:11to
- 1:12:11extracorporeal
- 1:12:12circuit
- 1:12:13to filters,
- 1:12:14which remove
- 1:12:15close to 90%
- 1:12:16of the chemo
- 1:12:17that was
- 1:12:17given, meaning
- 1:12:19we are
- 1:12:19achieving,
- 1:12:20regional
- 1:12:21focused
- 1:12:22high dose
- 1:12:23chemotherapy
- 1:12:23of the liver,
- 1:12:24which of
- 1:12:25course,
- 1:12:25in patients
- 1:12:26with
- 1:12:26metastatic
- 1:12:27melanoma is of
- 1:12:28extreme
- 1:12:28importance
- 1:12:29because 90%
- 1:12:30of patients
- 1:12:31with
- 1:12:31metastatic
- 1:12:31disease
- 1:12:32will have
- 1:12:32liver lesions
- 1:12:34and actually
- 1:12:34half of,
- 1:12:36but
- 1:12:36essentially
- 1:12:37all melanoma
- 1:12:37patients
- 1:12:38have liver
- 1:12:38lesions only.
- 1:12:40And,
- 1:12:40unfortunately,
- 1:12:41most of
- 1:12:42the patients
- 1:12:42will succumb
- 1:12:43to
- 1:12:43the disease,
- 1:12:44do so
- 1:12:45because of
- 1:12:45progression
- 1:12:46to liver
- 1:12:46that cannot
- 1:12:47be controlled
- 1:12:47with
- 1:12:47systemic therapy
- 1:12:49and that is
- 1:12:49the reason why
- 1:12:50local regional
- 1:12:51therapies,
- 1:12:52such as
- 1:12:53percutaneous
- 1:12:54hepatic
- 1:12:54perfusion,
- 1:12:56which is
- 1:12:57basically
- 1:12:57enabled
- 1:12:58with the
- 1:12:58Hepzato kit,
- 1:12:59the other
- 1:13:00component of
- 1:13:00the kit
- 1:13:01is the drug
- 1:13:01Melphalan.
- 1:13:02It's an
- 1:13:02old drug.
- 1:13:03It's actually
- 1:13:04older by
- 1:13:05about
- 1:13:05two years,
- 1:13:06and that's
- 1:13:06something.
- 1:13:07but old
- 1:13:07doesn't
- 1:13:08mean bad.
- 1:13:08The reason
- 1:13:09why all drugs,
- 1:13:11chemotherapy
- 1:13:11is banned
- 1:13:12in favor of,
- 1:13:13modern drugs
- 1:13:14and new
- 1:13:14therapies,
- 1:13:14is that
- 1:13:15not the
- 1:13:16lack of
- 1:13:16efficacy.
- 1:13:17Chemotherapy
- 1:13:18is a very
- 1:13:18efficacious
- 1:13:20problem,
- 1:13:20is toxicity
- 1:13:21and toxicity
- 1:13:22means
- 1:13:23that you're
- 1:13:23not only
- 1:13:23hitting
- 1:13:24the tumor,
- 1:13:24which is the
- 1:13:25desired
- 1:13:25effect,
- 1:13:26but
- 1:13:26unfortunately,
- 1:13:27chemotherapy
- 1:13:28being given
- 1:13:28systemically
- 1:13:29will also,
- 1:13:31hit, targets
- 1:13:33such as
- 1:13:33our organs,
- 1:13:35blood vessels
- 1:13:36and the
- 1:13:36whole body.
- 1:13:37Basically,
- 1:13:37it's
- 1:13:37making the
- 1:13:37patients sick.
- 1:13:39So if you
- 1:13:39had a
- 1:13:40means of
- 1:13:40delivering
- 1:13:41high dose
- 1:13:41chemotherapy
- 1:13:42specifically
- 1:13:43to the
- 1:13:43tumor site
- 1:13:44without
- 1:13:44damaging
- 1:13:45too much,
- 1:13:45the rest
- 1:13:46of the body,
- 1:13:46chemotherapy
- 1:13:47could really
- 1:13:48be quite
- 1:13:48effective
- 1:13:49and that's
- 1:13:50what I'm going
- 1:13:50to show
- 1:13:51you in
- 1:13:52the next
- 1:13:52slides.
- 1:13:53So
- 1:13:55the,
- 1:13:56Hepzato kit
- 1:13:57is utilized
- 1:13:58with a team
- 1:13:58of specialists
- 1:13:59on the left.
- 1:14:00You see,
- 1:14:01the team
- 1:14:01of specialists
- 1:14:02is conducting
- 1:14:02the procedure,
- 1:14:04PHP or Hepatic
- 1:14:05Perfusion
- 1:14:05Procedure,
- 1:14:06which is
- 1:14:07led by
- 1:14:07an
- 1:14:07interventional
- 1:14:08radiologist,
- 1:14:09the
- 1:14:09anesthesiologist,
- 1:14:10and
- 1:14:10perfusionist,
- 1:14:11and in
- 1:14:12addition
- 1:14:12to that,
- 1:14:13of course,
- 1:14:13is the
- 1:14:13treatment team
- 1:14:15led by
- 1:14:15the oncologist
- 1:14:16and other
- 1:14:16people
- 1:14:17who help,
- 1:14:19prepare, treat
- 1:14:20and manage
- 1:14:21to the patient
- 1:14:22and then
- 1:14:22also manage,
- 1:14:23the patient
- 1:14:24between
- 1:14:24treatments
- 1:14:25with Hepzato
- 1:14:26kit.
- 1:14:28The three
- 1:14:28steps of
- 1:14:29the procedure
- 1:14:29are the
- 1:14:30isolation,
- 1:14:31followed
- 1:14:32by a local
- 1:14:32administration
- 1:14:33of the high
- 1:14:34dose chemo
- 1:14:35and then
- 1:14:35finally
- 1:14:36filtration
- 1:14:36to remove,
- 1:14:38the vast
- 1:14:39majority
- 1:14:39of the chemo
- 1:14:41so that the
- 1:14:41systemic
- 1:14:42circulation
- 1:14:43receives
- 1:14:43only ten, 15%
- 1:14:45of the
- 1:14:45administered
- 1:14:46dose.
- 1:14:49The product
- 1:14:50was approved
- 1:14:51by the FDA
- 1:14:52in August 23,
- 1:14:54as mentioned
- 1:14:55earlier,
- 1:14:55by Dr.
- 1:14:56Carvajal
- 1:14:57And the
- 1:14:58rationale,
- 1:14:59the basis for
- 1:15:00the approval
- 1:15:01was the
- 1:15:01pivotal
- 1:15:01focus trial,
- 1:15:03which was
- 1:15:03published,
- 1:15:04and you see
- 1:15:04the reference,
- 1:15:05publication
- 1:15:06below.
- 1:15:08And,
- 1:15:10the history
- 1:15:11of the trial,
- 1:15:12it was
- 1:15:12designed
- 1:15:12originally
- 1:15:13as a phase
- 1:15:13three,
- 1:15:14randomized,
- 1:15:15multicenter,
- 1:15:15prospective,
- 1:15:16trial,
- 1:15:17where patients
- 1:15:18would be
- 1:15:18randomized
- 1:15:19to receive
- 1:15:19treatment
- 1:15:20with,
- 1:15:20the
- 1:15:21Melphalan/HDS
- 1:15:22or Hepzato kit
- 1:15:23or the best
- 1:15:24available care
- 1:15:25investigator
- 1:15:26choice,
- 1:15:26which included
- 1:15:27another
- 1:15:28regional
- 1:15:28therapy called
- 1:15:29TACE/Pembrolizumab,
- 1:15:29Ipilimumab, or
- 1:15:31Dacarbazine.
- 1:15:33About 85
- 1:15:34patients
- 1:15:35into the trial,
- 1:15:36we noticed
- 1:15:36a significant
- 1:15:37slowdown
- 1:15:38in the
- 1:15:38treatment,
- 1:15:39because,
- 1:15:40patients
- 1:15:40who were
- 1:15:41randomized
- 1:15:41to the
- 1:15:42control arm
- 1:15:42started,
- 1:15:44withdrawing
- 1:15:45consent
- 1:15:46to that level
- 1:15:47that
- 1:15:47jeopardized
- 1:15:48the
- 1:15:48randomization
- 1:15:49and the
- 1:15:49statistical
- 1:15:50design.
- 1:15:50So we met
- 1:15:51with the FDA,
- 1:15:51discussed
- 1:15:52with them
- 1:15:52the problem,
- 1:15:53and the
- 1:15:53FDA proposed
- 1:15:54that we amend
- 1:15:55the protocol
- 1:15:55to make it
- 1:15:56a one
- 1:15:57arm trial,
- 1:15:58single
- 1:15:58on trial
- 1:15:58to drop the
- 1:15:59control arm,
- 1:16:00because
- 1:16:00otherwise
- 1:16:01the integrity
- 1:16:01of the
- 1:16:01trial couldn’t
- 1:16:02be preserved.
- 1:16:02So that,
- 1:16:03happened
- 1:16:03with
- 1:16:04amendment,
- 1:16:05and the study
- 1:16:05has been
- 1:16:06completed
- 1:16:06as a
- 1:16:06single arm
- 1:16:07study
- 1:16:08where all
- 1:16:09enrolled
- 1:16:09patients
- 1:16:10with the
- 1:16:10receive
- 1:16:10the treatment
- 1:16:11with
- 1:16:11Melphalan/HDS
- 1:16:13and the
- 1:16:14treatment
- 1:16:14cycles are
- 1:16:15administered
- 1:16:15typically 6
- 1:16:16to 8 weeks
- 1:16:17apart.
- 1:16:20Inclusion
- 1:16:21criteria,
- 1:16:22basically,
- 1:16:24as for
- 1:16:25the extent
- 1:16:25of disease,
- 1:16:26both patients
- 1:16:27with liver
- 1:16:28only and
- 1:16:28extrahepatic
- 1:16:29disease
- 1:16:29as well.
- 1:16:30The eligible
- 1:16:31is support
- 1:16:32when 50%
- 1:16:32or less of
- 1:16:33the liver
- 1:16:34be occupied
- 1:16:34by the tumors
- 1:16:35to be
- 1:16:36measurable,
- 1:16:37and patients
- 1:16:37have to be,
- 1:16:38in good
- 1:16:38perform
- 1:16:39status.
- 1:16:40If the liver,
- 1:16:41was
- 1:16:41compromised
- 1:16:42by cirrhosis
- 1:16:43or portal
- 1:16:44hypertension,
- 1:16:44that was the
- 1:16:45exclusion
- 1:16:46criteria,
- 1:16:47and because
- 1:16:47the procedure
- 1:16:48is conducted
- 1:16:49under general
- 1:16:49anesthesia,
- 1:16:50the patients
- 1:16:50have to be
- 1:16:51fit enough
- 1:16:51to be able
- 1:16:52to, undergo
- 1:16:53general
- 1:16:54anesthesia
- 1:16:54procedure,
- 1:16:56and patients
- 1:16:56will,
- 1:16:57on
- 1:16:58immunosuppressant
- 1:16:58drugs
- 1:16:58for infections
- 1:16:59were not
- 1:17:00eligible
- 1:17:00because
- 1:17:01Melphalan
- 1:17:02would suppress
- 1:17:03the bone
- 1:17:03marrow
- 1:17:03and make
- 1:17:04patients
- 1:17:04more likely to
- 1:17:05experience
- 1:17:05infections.
- 1:17:09So, the
- 1:17:09primary
- 1:17:10endpoint of
- 1:17:11the study was,
- 1:17:12response
- 1:17:12rate
- 1:17:12confirmed,
- 1:17:13independently
- 1:17:14reviewed,
- 1:17:15a
- 1:17:15response rate
- 1:17:16and a
- 1:17:17historical
- 1:17:17control,
- 1:17:18requested by
- 1:17:19the FDA.
- 1:17:20was a meta
- 1:17:21analysis
- 1:17:22of
- 1:17:22contemporary,
- 1:17:23trials
- 1:17:24where
- 1:17:24immunotherapy,
- 1:17:25ipilimumab,
- 1:17:26nivolumab
- 1:17:27or the
- 1:17:27combination
- 1:17:27thereof, was,
- 1:17:29used to treat
- 1:17:30patients with
- 1:17:30metastatic
- 1:17:31disease
- 1:17:32based on the
- 1:17:32meta analysis
- 1:17:33results,
- 1:17:34the response
- 1:17:35rate in the
- 1:17:35historical
- 1:17:36control was 5%
- 1:17:37approximately.
- 1:17:39And response
- 1:17:39rate in
- 1:17:40the focused
- 1:17:40trial was 36,
- 1:17:41which was,
- 1:17:42highly
- 1:17:42statistically
- 1:17:43significant.
- 1:17:44Equally
- 1:17:45important
- 1:17:45is not just
- 1:17:46the large
- 1:17:47number
- 1:17:47of patients
- 1:17:47who responded
- 1:17:48to the
- 1:17:48treatment.
- 1:17:49It's actually
- 1:17:50the duration
- 1:17:50of response.
- 1:17:52The median
- 1:17:52duration was
- 1:17:5314 months,
- 1:17:54which is,
- 1:17:55depending on
- 1:17:55which trial
- 1:17:56you look at
- 1:17:56between 3
- 1:17:57and 5
- 1:17:57times longer
- 1:17:58than what
- 1:17:59you typically
- 1:18:00see acceptable
- 1:18:00long term
- 1:18:01survivors, in
- 1:18:02the therapy.
- 1:18:05Here's the
- 1:18:06complete
- 1:18:07data set.
- 1:18:08What is
- 1:18:08probably,
- 1:18:09very
- 1:18:10interesting
- 1:18:10for this
- 1:18:10group,
- 1:18:11all these
- 1:18:12patients,
- 1:18:12both treatment
- 1:18:13naive and,
- 1:18:14treated
- 1:18:15patients,
- 1:18:16we saw
- 1:18:17some
- 1:18:17responders,
- 1:18:19patients
- 1:18:19who had
- 1:18:20complete
- 1:18:20response,
- 1:18:21and I'll
- 1:18:22show you
- 1:18:22in graph
- 1:18:22in a moment
- 1:18:24that,
- 1:18:24and actually,
- 1:18:24whether what
- 1:18:25I call
- 1:18:25the right away
- 1:18:27on the
- 1:18:27vertical plot,
- 1:18:28you see that
- 1:18:29the majority
- 1:18:30of the
- 1:18:30patients
- 1:18:30respond
- 1:18:31with the tumor
- 1:18:31shrinkage,
- 1:18:33but when you
- 1:18:33look to
- 1:18:34the far right,
- 1:18:35you see that,
- 1:18:36total patients
- 1:18:36have a
- 1:18:37complete
- 1:18:37disappearance
- 1:18:37of the
- 1:18:38target
- 1:18:38lesions,
- 1:18:39seven of
- 1:18:39which
- 1:18:40were
- 1:18:40classified
- 1:18:40as CR,
- 1:18:41that means
- 1:18:41there was
- 1:18:41no tumor
- 1:18:42left in
- 1:18:42the body,
- 1:18:43and five were
- 1:18:44not classified
- 1:18:44as CR, meaning
- 1:18:46it was
- 1:18:46still extra
- 1:18:47disease
- 1:18:47leftover.
- 1:18:48That really
- 1:18:49says a lot
- 1:18:51about
- 1:18:51the efficacy
- 1:18:52of the
- 1:18:52treatment in
- 1:18:52the liver.
- 1:18:54but what's
- 1:18:55really also
- 1:18:55very important
- 1:18:56is on the
- 1:18:56previous
- 1:18:57slide,
- 1:18:57I will just
- 1:18:57go back to
- 1:18:58for a moment,
- 1:18:59is the disease
- 1:19:00control rate
- 1:19:00so close
- 1:19:01to 74%
- 1:19:03of patients
- 1:19:04achieved
- 1:19:05disease
- 1:19:05control,
- 1:19:07which is,
- 1:19:07as Dr.
- 1:19:08Cole mentioned
- 1:19:08previously, is
- 1:19:09really
- 1:19:10important.
- 1:19:10The
- 1:19:11controlling
- 1:19:12the disease,
- 1:19:13having
- 1:19:13a potential
- 1:19:14prolonged
- 1:19:14life is really
- 1:19:15what you're
- 1:19:15trying
- 1:19:16to accomplish
- 1:19:17in this,
- 1:19:17setting,
- 1:19:18in the
- 1:19:18setting of
- 1:19:18metastatic
- 1:19:19disease.
- 1:19:21So that's the
- 1:19:22efficacy data.
- 1:19:24In addition
- 1:19:24to that,
- 1:19:24I'm showing
- 1:19:25you also
- 1:19:25progression
- 1:19:26free survival,
- 1:19:27which is
- 1:19:27nine months,
- 1:19:28which is
- 1:19:29I think,
- 1:19:30very,
- 1:19:31very good.
- 1:19:31I don't know
- 1:19:32of any
- 1:19:32published data
- 1:19:33that shows
- 1:19:34longer PFS
- 1:19:35in metastatic
- 1:19:36uveal
- 1:19:36melanoma.
- 1:19:37The overall
- 1:19:38survival is
- 1:19:39also,
- 1:19:40very
- 1:19:40encouraging,
- 1:19:40more than
- 1:19:4120 months.
- 1:19:42Obviously,
- 1:19:42the limitations
- 1:19:43are that
- 1:19:44this is not a
- 1:19:44randomized trial.
- 1:19:45So
- 1:19:46we cannot make
- 1:19:46definitive
- 1:19:47statements,
- 1:19:47but it's
- 1:19:48encouraging
- 1:19:49to see,
- 1:19:50both
- 1:19:50progression
- 1:19:51free survival
- 1:19:51and overall
- 1:19:52survival,
- 1:19:53trending in
- 1:19:54the right
- 1:19:54direction.
- 1:19:58Here's
- 1:19:59a safety
- 1:19:59summary, and
- 1:20:00I'll show you
- 1:20:00a more
- 1:20:01detailed slide
- 1:20:02as well.
- 1:20:03In the focus
- 1:20:04trial,
- 1:20:05the median
- 1:20:05number of
- 1:20:06treatment
- 1:20:06cycles
- 1:20:07was four.
- 1:20:08We planned
- 1:20:08up to six
- 1:20:09treatment
- 1:20:09cycles,
- 1:20:10And about
- 1:20:1037% of
- 1:20:11patients
- 1:20:12completed
- 1:20:13all six
- 1:20:13treatment
- 1:20:13cycles.
- 1:20:15We didn't
- 1:20:15record
- 1:20:15any deaths
- 1:20:16related to
- 1:20:17study
- 1:20:17treatments,
- 1:20:18which is
- 1:20:18very
- 1:20:18important,
- 1:20:19because,
- 1:20:20the previous
- 1:20:21method used
- 1:20:21to deliver
- 1:20:22high dose
- 1:20:23chemotherapy
- 1:20:23for the liver,
- 1:20:24called IHP
- 1:20:26or isolated
- 1:20:26hepatic
- 1:20:27perfusion,
- 1:20:28had a
- 1:20:28mortality rate
- 1:20:29about
- 1:20:295%
- 1:20:30historically.
- 1:20:31Obviously,
- 1:20:32PHP is
- 1:20:32a less
- 1:20:33invasive,
- 1:20:33repeatable
- 1:20:34procedure,
- 1:20:35and it's
- 1:20:36better
- 1:20:36tolerated.
- 1:20:37Approximately
- 1:20:3841% of
- 1:20:39patients
- 1:20:39had serious
- 1:20:40adverse
- 1:20:40events,
- 1:20:40which is,
- 1:20:41very typical
- 1:20:42for pivotal
- 1:20:43studies
- 1:20:44in advanced
- 1:20:45metastatic
- 1:20:45disease.
- 1:20:46Typically,
- 1:20:47patients,
- 1:20:48we put around
- 1:20:5040 to 50% SAE,
- 1:20:51so
- 1:20:51this is in
- 1:20:52line with
- 1:20:54other phase
- 1:20:54two trials.
- 1:20:56The
- 1:20:56predominant
- 1:20:56toxicity
- 1:20:57experienced
- 1:20:58by patients
- 1:20:58with transient
- 1:20:59non-cumulative
- 1:21:00hematological
- 1:21:01toxicities.
- 1:21:01Melphalan,
- 1:21:03is a drug
- 1:21:03that will
- 1:21:04suppress the
- 1:21:04bone marrow.
- 1:21:05And although
- 1:21:05we are able
- 1:21:06to filter out,
- 1:21:07more than 80%
- 1:21:08to 90% of
- 1:21:09the
- 1:21:09administered
- 1:21:10Melphalan,
- 1:21:10there's
- 1:21:10still some
- 1:21:11residual
- 1:21:11Melphalan
- 1:21:12which is
- 1:21:12causing this
- 1:21:13transient,
- 1:21:14non-cumulative
- 1:21:15hematological
- 1:21:15toxicity.
- 1:21:16However,
- 1:21:17hematological
- 1:21:17toxicity
- 1:21:18is something
- 1:21:18that, doctors
- 1:21:20are very
- 1:21:20experienced
- 1:21:21with managing
- 1:21:22and it
- 1:21:23responds
- 1:21:23well to
- 1:21:23standard,
- 1:21:24supportive
- 1:21:24care therapy.
- 1:21:2680% of
- 1:21:27patients had
- 1:21:27AE that led
- 1:21:28to treatment
- 1:21:28discontinuation,
- 1:21:31but this
- 1:21:31is again,
- 1:21:32it's very
- 1:21:32strict
- 1:21:33definition,
- 1:21:33so all
- 1:21:34the patients
- 1:21:35who
- 1:21:35discontinue
- 1:21:35due to AE,
- 1:21:37were planned
- 1:21:38to receive
- 1:21:39six treatment.
- 1:21:40So that's
- 1:21:40why the rate
- 1:21:41is a
- 1:21:41little bit
- 1:21:41higher
- 1:21:42than what
- 1:21:42you would
- 1:21:42normally
- 1:21:42expect.
- 1:21:44And, what's
- 1:21:45important,
- 1:21:46although
- 1:21:46we give
- 1:21:47the treatment
- 1:21:47directly
- 1:21:47to the liver,
- 1:21:48people
- 1:21:48ask, well,
- 1:21:49is this
- 1:21:49high dose
- 1:21:50chemotherapy
- 1:21:50toxic
- 1:21:51to the liver?
- 1:21:51And the answer
- 1:21:52is no
- 1:21:53because less
- 1:21:53than 10%
- 1:21:54of patients
- 1:21:54experienced
- 1:21:55transient
- 1:21:55Grade 3/4
- 1:21:56liver AEs,
- 1:21:57which were
- 1:21:57predominantly,
- 1:21:59increases in
- 1:22:00liver
- 1:22:00transformations,
- 1:22:01enzymes that,
- 1:22:03indicate
- 1:22:03potential
- 1:22:04damage
- 1:22:04to the liver.
- 1:22:08In terms
- 1:22:08of specific,
- 1:22:11treatment,
- 1:22:11emergent
- 1:22:12serious
- 1:22:12adverse events
- 1:22:13and events,
- 1:22:14related
- 1:22:15and unrelated
- 1:22:16to the
- 1:22:17treatment,
- 1:22:18you can see
- 1:22:19here on
- 1:22:19the left,
- 1:22:20the table,
- 1:22:21Thrombocytopenia,
- 1:22:22Neutropenia,
- 1:22:23Leukopenia,
- 1:22:24are events
- 1:22:25that happened,
- 1:22:26most
- 1:22:26frequently,
- 1:22:27we had
- 1:22:27three patients
- 1:22:28with cardiac
- 1:22:29arrest,
- 1:22:30none of them
- 1:22:30fatal and none
- 1:22:31of them
- 1:22:32related
- 1:22:32treatment.
- 1:22:34Some of
- 1:22:34the side
- 1:22:35effects
- 1:22:36happened
- 1:22:36during the
- 1:22:36treatment.
- 1:22:37For example,
- 1:22:38Hypoxia,
- 1:22:38Pulmonary
- 1:22:39Edema,
- 1:22:40because
- 1:22:40you have
- 1:22:40to remember,
- 1:22:41these are
- 1:22:41patients
- 1:22:42who are,
- 1:22:42in general
- 1:22:43anesthesia,
- 1:22:43and hypoxia,
- 1:22:44pulmonary
- 1:22:45edema happens
- 1:22:46in a small
- 1:22:46percent of
- 1:22:47patients,
- 1:22:48and it's
- 1:22:48related to the
- 1:22:49anesthesia
- 1:22:50procedure.
- 1:22:51And, then
- 1:22:52after the
- 1:22:52procedure at
- 1:22:53the bottom
- 1:22:54right table,
- 1:22:54you see again,
- 1:22:56the mostly
- 1:22:57hematological
- 1:22:58toxicity,
- 1:22:59as previously
- 1:22:59mentioned.
- 1:23:02So that's
- 1:23:03the focus
- 1:23:03trial
- 1:23:04and people ask
- 1:23:05the question,
- 1:23:06okay,
- 1:23:06what's next?
- 1:23:07What's next
- 1:23:08for Hepzato
- 1:23:08kit patients
- 1:23:09with uveal
- 1:23:10melanoma
- 1:23:11and, we,
- 1:23:12do have plans.
- 1:23:13I think,
- 1:23:15the
- 1:23:15most imminent,
- 1:23:16projects
- 1:23:17that we're
- 1:23:17currently
- 1:23:17working on
- 1:23:18are
- 1:23:18combinations
- 1:23:19with
- 1:23:19immunotherapies
- 1:23:21in patients
- 1:23:21with
- 1:23:21metastatic
- 1:23:22disease.
- 1:23:23combining
- 1:23:24chemo with,
- 1:23:25immunotherapy
- 1:23:26has proven
- 1:23:27very
- 1:23:27successful
- 1:23:28in many
- 1:23:29solid tumors.
- 1:23:30So,
- 1:23:30we were hoping
- 1:23:32to see,
- 1:23:33similar effect
- 1:23:34in patients
- 1:23:35with
- 1:23:35metastatic
- 1:23:36melanoma
- 1:23:36and I'll
- 1:23:36show you
- 1:23:371 or 2 slides,
- 1:23:38how this is,
- 1:23:39shaping up.
- 1:23:41And then,
- 1:23:41again,
- 1:23:42as mentioned
- 1:23:42by Doctor
- 1:23:43Carvajal
- 1:23:43earlier,
- 1:23:44we're also
- 1:23:45interested in
- 1:23:46looking at
- 1:23:46the efficacy
- 1:23:47of the
- 1:23:47treatment
- 1:23:48in patients
- 1:23:49with high risk
- 1:23:50metastatic
- 1:23:51disease
- 1:23:51profile,
- 1:23:51meaning
- 1:23:52patients
- 1:23:53were
- 1:23:53successfully
- 1:23:54treated
- 1:23:54for the
- 1:23:55primary tumor
- 1:23:56but
- 1:23:56unfortunately
- 1:23:57have a high
- 1:23:57risk of
- 1:23:58metastatic
- 1:23:58disease.
- 1:24:00There's lots
- 1:24:00of trials
- 1:24:00underway,
- 1:24:01and we believe
- 1:24:02that treatment
- 1:24:04with Hepzato
- 1:24:04kit,
- 1:24:05has a really
- 1:24:07high potential
- 1:24:08to mitigate
- 1:24:10the risk of
- 1:24:10metastasis.
- 1:24:12And,
- 1:24:13we are
- 1:24:13discussing
- 1:24:14some
- 1:24:14proposals,
- 1:24:15about
- 1:24:15launching
- 1:24:16such studies,
- 1:24:16but that's
- 1:24:16something
- 1:24:17we'll probably
- 1:24:17discuss
- 1:24:18next time
- 1:24:19in a forum
- 1:24:19like this.
- 1:24:20But
- 1:24:21in the meantime,
- 1:24:21for
- 1:24:22conclusion,
- 1:24:22let me show
- 1:24:23you how
- 1:24:24the
- 1:24:24combination
- 1:24:24therapies,
- 1:24:25may look
- 1:24:26like,
- 1:24:27in patients
- 1:24:27with
- 1:24:27metastatic
- 1:24:28disease.
- 1:24:29Study called
- 1:24:30the CHOPIN
- 1:24:31Trial
- 1:24:32was initiated
- 1:24:32in the
- 1:24:33Netherlands
- 1:24:33about
- 1:24:34four years ago
- 1:24:35as a phase 1b
- 1:24:37phase
- 1:24:37two study.
- 1:24:38Currently,
- 1:24:39the phase two
- 1:24:39is completing
- 1:24:40enrollment,
- 1:24:41and we
- 1:24:41expect
- 1:24:41the results
- 1:24:42from the phase
- 1:24:43two randomized
- 1:24:43trial
- 1:24:44to be
- 1:24:44available
- 1:24:45sometime
- 1:24:45next year.
- 1:24:47But,
- 1:24:47what was
- 1:24:48published
- 1:24:48about
- 1:24:48two years
- 1:24:49ago,
- 1:24:49are results
- 1:24:49from the
- 1:24:50phase 1b,
- 1:24:51the dose
- 1:24:52finding part
- 1:24:52because PHP
- 1:24:54or Hepzato
- 1:24:55kit
- 1:24:55was combined
- 1:24:57with
- 1:24:57ipilimumab
- 1:24:58and nivolumab,
- 1:24:59the two
- 1:25:00most effective
- 1:25:01checkpoint
- 1:25:01inhibitors
- 1:25:02thus far.
- 1:25:04And you
- 1:25:05see here
- 1:25:06the spider
- 1:25:06plot basically
- 1:25:07how the
- 1:25:07tumor size
- 1:25:09changes
- 1:25:09following
- 1:25:10the treatment.
- 1:25:11It's very
- 1:25:11encouraging
- 1:25:12to see that
- 1:25:14the vast
- 1:25:14majority,
- 1:25:15although it's
- 1:25:15a small number
- 1:25:15of patients.
- 1:25:16So with
- 1:25:16some patients,
- 1:25:17we see
- 1:25:18definitely,
- 1:25:19very
- 1:25:19encouraging
- 1:25:20signs of
- 1:25:20tumor
- 1:25:20shrinkage.
- 1:25:22And on
- 1:25:22the next
- 1:25:23slide,
- 1:25:25we see
- 1:25:26a very
- 1:25:27interesting,
- 1:25:27intriguing
- 1:25:28signal of
- 1:25:28efficacy
- 1:25:29and response
- 1:25:30rates.
- 1:25:30You know,
- 1:25:31very high,
- 1:25:32as shown here.
- 1:25:33Again,
- 1:25:33a caution,
- 1:25:34this is a
- 1:25:34small number
- 1:25:34of patients,
- 1:25:35only seven
- 1:25:35patients
- 1:25:36and the
- 1:25:36larger trial
- 1:25:37that is
- 1:25:37currently
- 1:25:37completing
- 1:25:38enrollment,
- 1:25:39will hopefully
- 1:25:39provide
- 1:25:40a much
- 1:25:40more
- 1:25:40conclusive
- 1:25:41data set.
- 1:25:42What is also
- 1:25:43very
- 1:25:43intriguing,
- 1:25:44it's shown
- 1:25:44in the table
- 1:25:45on the right.
- 1:25:45And that's
- 1:25:46the
- 1:25:46progression
- 1:25:46free survival.
- 1:25:48If you recall,
- 1:25:48if you use
- 1:25:49systemic
- 1:25:49treatments,
- 1:25:50typically
- 1:25:50you'll see
- 1:25:51three months
- 1:25:51progression
- 1:25:51free survival
- 1:25:53or maybe
- 1:25:53with some
- 1:25:53of the
- 1:25:54novel drugs
- 1:25:54like the
- 1:25:55IDEAYA drug,
- 1:25:56maybe
- 1:25:56seven months.
- 1:25:57Here
- 1:25:57we have seen,
- 1:25:58in this
- 1:25:59very small
- 1:26:00initial trial,
- 1:26:0029 months,
- 1:26:02again,
- 1:26:02this is
- 1:26:03just something
- 1:26:04to, intrigue
- 1:26:07our curiosity
- 1:26:08and then
- 1:26:09really
- 1:26:09make us,
- 1:26:12wait for the
- 1:26:13randomized
- 1:26:14system data,
- 1:26:14which will be
- 1:26:15available
- 1:26:15next year.
- 1:26:16So that's,
- 1:26:17all the,
- 1:26:19technical sets
- 1:26:20that I had
- 1:26:20here,
- 1:26:21then I
- 1:26:21thank you
- 1:26:21very much.
- 1:26:22thanks to
- 1:26:23the audience.
- 1:26:23Thank you, Dr.
- 1:26:24Vukovic!
- 1:26:25So you guys
- 1:26:25have
- 1:26:25questions,
- 1:26:25please drop
- 1:26:26them in
- 1:26:26the Q&A
- 1:26:26and we'll
- 1:26:27get to them
- 1:26:27after
- 1:26:27our final
- 1:26:28presenter.
- 1:26:29I'm going
- 1:26:29to go ahead
- 1:26:29and bring on
- 1:26:30our next
- 1:26:30speaker
- 1:26:30from Replimune
- 1:26:31to talk about
- 1:26:32their, data
- 1:26:33that was
- 1:26:33presented
- 1:26:34at ASCO
- 1:26:34this year.
- 1:26:35Dr.
- 1:26:36Olsson-Brown,
- 1:26:36thank you
- 1:26:36so much
- 1:26:37for being
- 1:26:37with us.
- 1:26:39Thank you
- 1:26:39so much
- 1:26:39for
- 1:26:39the invite.
- 1:26:40I'll just
- 1:26:40share
- 1:26:40my screen.
- 1:26:45So, as I said
- 1:26:46thank you
- 1:26:46so much
- 1:26:46for the invite
- 1:26:47time there.
- 1:26:48I’m Anna
- 1:26:49Olsson-Brown,
- 1:26:49I'm
- 1:26:49a oncologist,
- 1:26:50medical
- 1:26:50oncologist
- 1:26:50based in
- 1:26:51the UK.
- 1:26:52I'm working
- 1:26:52directly on
- 1:26:54a number
- 1:26:54of that,
- 1:26:55that oncolytic
- 1:26:55virus therapy.
- 1:26:56So I'm just
- 1:26:56going to give
- 1:26:56a little bit
- 1:26:57of background
- 1:26:57around
- 1:26:58oncolytic
- 1:26:58viruses
- 1:26:59in general,
- 1:27:00and then,
- 1:27:00talk about
- 1:27:01a study
- 1:27:02that was
- 1:27:02presented at
- 1:27:03ASCO this
- 1:27:03year.
- 1:27:11Here’s my
- 1:27:11disclosures.
- 1:27:13So obviously,
- 1:27:14throughout
- 1:27:14this entire
- 1:27:15and really,
- 1:27:15frankly
- 1:27:16amazing,
- 1:27:17webinar,
- 1:27:17we've heard
- 1:27:17about all
- 1:27:18different
- 1:27:18types
- 1:27:18of therapeutic
- 1:27:19approaches
- 1:27:20to managing
- 1:27:20ocular
- 1:27:21uveal
- 1:27:21melanoma,
- 1:27:23and I think
- 1:27:23one of
- 1:27:23the things
- 1:27:24that's one
- 1:27:24was really
- 1:27:24important
- 1:27:25is that
- 1:27:25we understand
- 1:27:26and have this
- 1:27:27sort of
- 1:27:27moment
- 1:27:28of reflection
- 1:27:28on how quickly
- 1:27:29this field
- 1:27:30has expanded,
- 1:27:30particularly
- 1:27:31thinking about
- 1:27:32how we sort
- 1:27:32of diversify
- 1:27:33and, harness
- 1:27:34the power of
- 1:27:34the immune
- 1:27:35system
- 1:27:36to try
- 1:27:36and treat
- 1:27:37in the,
- 1:27:37as we've
- 1:27:38heard,
- 1:27:38the
- 1:27:38neoadjuvant,
- 1:27:39adjuvant,
- 1:27:39and metastatic
- 1:27:40setting
- 1:27:41and oncology
- 1:27:41viruses
- 1:27:42are one of
- 1:27:42those
- 1:27:42subgroups
- 1:27:43of therapies
- 1:27:44that we
- 1:27:44are working
- 1:27:44very heavily
- 1:27:45on to try
- 1:27:46and understand
- 1:27:47where
- 1:27:47they might
- 1:27:47have a role
- 1:27:48in the
- 1:27:49management of,
- 1:27:50multiple
- 1:27:50different
- 1:27:50malignancies.
- 1:27:51That actually
- 1:27:52includes uveal
- 1:27:53melanoma
- 1:27:54and when we
- 1:27:55think about
- 1:27:55how all kinds
- 1:27:56of viruses
- 1:27:57work,
- 1:27:58essentially
- 1:27:58they
- 1:27:58actually do
- 1:27:59a number
- 1:27:59of things.
- 1:28:00So, the virus
- 1:28:02I'm going to
- 1:28:02talk about
- 1:28:03is called
- 1:28:03the virus.
- 1:28:05but it's
- 1:28:05based on
- 1:28:06the principle
- 1:28:06that,
- 1:28:07oncology
- 1:28:07viruses
- 1:28:08can infect,
- 1:28:10malignant
- 1:28:10cells
- 1:28:10and cause
- 1:28:11direct,
- 1:28:12cellular
- 1:28:12destruction.
- 1:28:13But also that
- 1:28:14leads to a
- 1:28:15series of,
- 1:28:16of the
- 1:28:17downstream
- 1:28:17events,
- 1:28:18which
- 1:28:18essentially
- 1:28:19leads
- 1:28:19to recruitment
- 1:28:20and efficacy
- 1:28:21of, T cells,
- 1:28:23probably
- 1:28:24amongst
- 1:28:25the
- 1:28:25immune cells
- 1:28:26that we can
- 1:28:26also cause,
- 1:28:27at optimal
- 1:28:29recruitment
- 1:28:29by the,
- 1:28:30by the
- 1:28:30introduction
- 1:28:31of, of an
- 1:28:32all kinds
- 1:28:32of virus
- 1:28:33generally
- 1:28:33speaking,
- 1:28:34at the moment
- 1:28:34and certainly
- 1:28:34the virus I'm
- 1:28:35going to talk
- 1:28:35about
- 1:28:35is directed
- 1:28:36that is directly
- 1:28:37injected into
- 1:28:38a area
- 1:28:39of tumor.
- 1:28:40and it causes
- 1:28:41these
- 1:28:41localized
- 1:28:42effects
- 1:28:43and certainly
- 1:28:44the,
- 1:28:44the viruses
- 1:28:45that we've
- 1:28:45been working
- 1:28:46on have
- 1:28:46been used
- 1:28:47in combination
- 1:28:48with
- 1:28:48checkpoint
- 1:28:48inhibitors
- 1:28:49in the aim
- 1:28:50to optimize
- 1:28:50our immune
- 1:28:51response.
- 1:28:51So, the virus
- 1:28:52sort of allows
- 1:28:53direct effects
- 1:28:54on the
- 1:28:54tumor cells,
- 1:28:55but also
- 1:28:55T-cell
- 1:28:56recruitment
- 1:28:57with the
- 1:28:57principle
- 1:28:57that if we add
- 1:28:58a checkpoint
- 1:28:58inhibitor,
- 1:28:59we actually
- 1:28:59have an
- 1:29:00upregulated,
- 1:29:02a much less,
- 1:29:02inhibited,
- 1:29:04t cell that
- 1:29:05that arrives
- 1:29:06and is much
- 1:29:07more able
- 1:29:07to be
- 1:29:08efficacious
- 1:29:09in the,
- 1:29:09in the
- 1:29:09tumor
- 1:29:09microenvironment.
- 1:29:11So there are
- 1:29:11multiple
- 1:29:12different,
- 1:29:13downstream
- 1:29:13effects
- 1:29:14of the
- 1:29:14oncolytic
- 1:29:14virus
- 1:29:15as well as the
- 1:29:16sort of
- 1:29:16the viral,
- 1:29:17the viral
- 1:29:17effect
- 1:29:18itself,
- 1:29:19which is
- 1:29:19quite,
- 1:29:20quite amazing.
- 1:29:20And,
- 1:29:21this has
- 1:29:21been trialed
- 1:29:22in a number
- 1:29:22of different,
- 1:29:23settings
- 1:29:24so far
- 1:29:24in this
- 1:29:24direction.
- 1:29:25Recognition
- 1:29:25looking at
- 1:29:26using the Aki
- 1:29:26one virus,
- 1:29:27which
- 1:29:27showed that
- 1:29:28it was
- 1:29:28very,
- 1:29:28very, useful
- 1:29:30in the
- 1:29:30setting of
- 1:29:32Pt one failed
- 1:29:33treated
- 1:29:34cutaneous
- 1:29:34rendering
- 1:29:35when we saw
- 1:29:36so many
- 1:29:36promising
- 1:29:37signs
- 1:29:37of efficacy.
- 1:29:39and so
- 1:29:39the question
- 1:29:39was, could we,
- 1:29:40could we
- 1:29:41put that
- 1:29:41some
- 1:29:41introduced
- 1:29:42into,
- 1:29:42you know,
- 1:29:42melanoma.
- 1:29:43So as we've
- 1:29:44talked about,
- 1:29:45throughout
- 1:29:45the day,
- 1:29:46the kind
- 1:29:46of the
- 1:29:46complexities
- 1:29:47of the
- 1:29:47melanoma
- 1:29:48and
- 1:29:48malignancy,
- 1:29:49particularly
- 1:29:50in the
- 1:29:50metastatic
- 1:29:50setting,
- 1:29:51we know that
- 1:29:51it's a
- 1:29:52very
- 1:29:52challenging
- 1:29:53to treat,
- 1:29:53and it doesn't
- 1:29:54respond in
- 1:29:54quite the
- 1:29:55same way
- 1:29:55as it's
- 1:29:55cutaneous
- 1:29:56and
- 1:29:56counterpart.
- 1:29:58But also
- 1:29:58there were
- 1:29:58there were
- 1:29:59different
- 1:29:59immunological,
- 1:30:01issues with
- 1:30:02uveal
- 1:30:02melanoma.
- 1:30:03metastatic
- 1:30:03setting
- 1:30:04is going to
- 1:30:05cold.
- 1:30:05Tumor is
- 1:30:06quite
- 1:30:06difficult
- 1:30:06to get.
- 1:30:072000
- 1:30:08other immune
- 1:30:08cells
- 1:30:08into the
- 1:30:09microenvironment.
- 1:30:10and also
- 1:30:11understanding
- 1:30:11whether
- 1:30:11these T-cells
- 1:30:12are
- 1:30:12efficacious
- 1:30:13once they get
- 1:30:13there is
- 1:30:14also a
- 1:30:15challenge.
- 1:30:15So to try
- 1:30:16and to sort
- 1:30:17of address
- 1:30:17some of those
- 1:30:18issues,
- 1:30:19has led
- 1:30:19to the
- 1:30:19development
- 1:30:20of a series
- 1:30:20of oncolytic
- 1:30:21viruses,
- 1:30:21but this one
- 1:30:22is called
- 1:30:22the RB
- 1:30:23T virus.
- 1:30:24It's a,
- 1:30:24genetically
- 1:30:25modified HSV
- 1:30:26one virus,
- 1:30:28that has got
- 1:30:29a number
- 1:30:29of different
- 1:30:29mechanisms
- 1:30:30by which
- 1:30:31you can
- 1:30:31sort of
- 1:30:32and optimize
- 1:30:32it.
- 1:30:33It is
- 1:30:33a minimal
- 1:30:34modulation,
- 1:30:35and that
- 1:30:35includes with
- 1:30:35the RBt
- 1:30:36two virus.
- 1:30:37this sort
- 1:30:38of anti
- 1:30:39for antibody
- 1:30:40like molecule
- 1:30:40which allows
- 1:30:41some
- 1:30:42initial
- 1:30:43checkpoint,
- 1:30:44antagonism
- 1:30:46as part
- 1:30:47of the viral
- 1:30:47at the
- 1:30:47viral cascade.
- 1:30:49so the hope
- 1:30:49is that
- 1:30:50this would,
- 1:30:50this would
- 1:30:51should be seen
- 1:30:52to show
- 1:30:52some efficacy
- 1:30:54in the
- 1:30:54metastatic
- 1:30:54setting
- 1:30:55to thinking
- 1:30:55about
- 1:30:55patients,
- 1:30:56with,
- 1:30:57with their
- 1:30:58metastatic
- 1:30:58disease.
- 1:30:59this is,
- 1:31:00going on
- 1:31:01to form
- 1:31:02part of a
- 1:31:02bigger,
- 1:31:03body of work,
- 1:31:04the vaccine,
- 1:31:04and taking
- 1:31:05a different,
- 1:31:07virus is
- 1:31:07different,
- 1:31:07different
- 1:31:08versions of it
- 1:31:09at the same
- 1:31:09base.
- 1:31:10looking in
- 1:31:11various
- 1:31:11different,
- 1:31:12different
- 1:31:12settings.
- 1:31:13And there is,
- 1:31:14a planning
- 1:31:14underway to do
- 1:31:15a bigger
- 1:31:16for melanoma,
- 1:31:17study.
- 1:31:18But this is,
- 1:31:18this is this
- 1:31:19is this
- 1:31:19of the data
- 1:31:20from the phase
- 1:31:20one trial,
- 1:31:21which was a
- 1:31:22solid tumor,
- 1:31:23trial,
- 1:31:24but looking
- 1:31:25we were going
- 1:31:25to look
- 1:31:26specifically
- 1:31:26at the,
- 1:31:27uveal
- 1:31:27melanoma
- 1:31:27cohort,
- 1:31:29within
- 1:31:29that trial.
- 1:31:29And this
- 1:31:30was presented
- 1:31:30to asked
- 1:31:30for this year
- 1:31:31by my
- 1:31:31colleague, Dr.
- 1:31:32Saket.
- 1:31:33So if we think
- 1:31:34about
- 1:31:34the design
- 1:31:34of this today
- 1:31:35and there were
- 1:31:36two parts
- 1:31:36to the study,
- 1:31:37one was
- 1:31:38a monotherapy,
- 1:31:39dose
- 1:31:39escalation
- 1:31:40and so
- 1:31:41selection,
- 1:31:42part.
- 1:31:43And then that
- 1:31:43went on to
- 1:31:44a second
- 1:31:45this year,
- 1:31:45which divided
- 1:31:46into two
- 1:31:46cohorts A
- 1:31:48and B,
- 1:31:49and really
- 1:31:50the difference
- 1:31:50between the
- 1:31:51two parts
- 1:31:52of the,
- 1:31:52of the second
- 1:31:52part of
- 1:31:53the city
- 1:31:53was when
- 1:31:54you would
- 1:31:54introduce the,
- 1:31:55the checkpoint
- 1:31:56inhibitor, did
- 1:31:57you need
- 1:31:57that upfront
- 1:31:58or or do you,
- 1:31:59do you,
- 1:32:00can you,
- 1:32:00can you
- 1:32:01introduce it
- 1:32:01later on
- 1:32:01with the same,
- 1:32:02the same outcome.
- 1:32:03And we're
- 1:32:03still
- 1:32:03answering
- 1:32:04that question.
- 1:32:04The city's
- 1:32:05an ongoing
- 1:32:06so our P
- 1:32:07two, as I
- 1:32:07said, is
- 1:32:08an oncology
- 1:32:08virus.
- 1:32:08It's directly
- 1:32:09administered
- 1:32:10into
- 1:32:10the tumor,
- 1:32:11and can be
- 1:32:12and
- 1:32:13administered
- 1:32:13either
- 1:32:13subcutaneous
- 1:32:14or superficial
- 1:32:15lesions
- 1:32:15or to deep
- 1:32:16and visceral
- 1:32:16lesions
- 1:32:17using image
- 1:32:18guidance.
- 1:32:18And
- 1:32:18that was IDed
- 1:32:19by, CT guided
- 1:32:21injection
- 1:32:22into the liver
- 1:32:23predominantly
- 1:32:23or ultrasound
- 1:32:24guided
- 1:32:25injection
- 1:32:25to into
- 1:32:26any lesions
- 1:32:27in the liver.
- 1:32:28Patients
- 1:32:28were included
- 1:32:29if
- 1:32:29they were over
- 1:32:3018, if
- 1:32:30they had
- 1:32:31advanced
- 1:32:31or metastatic
- 1:32:32non
- 1:32:32neurological
- 1:32:33solid tumors
- 1:32:33including the
- 1:32:34melanoma
- 1:32:35cohort.
- 1:32:36and that they
- 1:32:37had progressed
- 1:32:37or couldn't
- 1:32:38tolerate
- 1:32:38standard
- 1:32:39therapy.
- 1:32:40They had to
- 1:32:40have at
- 1:32:40least one
- 1:32:41measurable
- 1:32:41and injectable
- 1:32:42tumor of more
- 1:32:43than or equal
- 1:32:43to one
- 1:32:44centimeter.
- 1:32:44And they had
- 1:32:45to have
- 1:32:45an equal
- 1:32:46performance
- 1:32:46data set equal
- 1:32:47to one.
- 1:32:48Patients
- 1:32:48were excluded
- 1:32:49if they had
- 1:32:49prior
- 1:32:50treatment
- 1:32:50with oncolytic
- 1:32:51viruses,
- 1:32:52if they had a
- 1:32:52history of
- 1:32:53hepatitis
- 1:32:55B C or
- 1:32:56HIV infection,
- 1:32:57or if they had
- 1:32:58any active
- 1:32:58significant
- 1:33:00infections
- 1:33:00or prior
- 1:33:01complications
- 1:33:01with HSV one,
- 1:33:03if they had
- 1:33:03active brain
- 1:33:04metastases
- 1:33:05or,
- 1:33:06meningitis
- 1:33:06from a
- 1:33:07secondary
- 1:33:07to malignancy,
- 1:33:08or if they had
- 1:33:09major surgery
- 1:33:10in less
- 1:33:11than two weeks
- 1:33:11prior
- 1:33:11to starting
- 1:33:12institutions
- 1:33:13and the
- 1:33:13endpoints.
- 1:33:14The primary
- 1:33:14endpoint was
- 1:33:15the safety
- 1:33:15and
- 1:33:15tolerability
- 1:33:16of the
- 1:33:16virus, plus
- 1:33:17or minus
- 1:33:17the checkpoint
- 1:33:18inhibitor,
- 1:33:18in this case
- 1:33:19nivolumab.
- 1:33:19that was
- 1:33:20tracked as a
- 1:33:22standard in
- 1:33:22the case.
- 1:33:24And the
- 1:33:24overall
- 1:33:24response to
- 1:33:25the virus
- 1:33:26plus or minus
- 1:33:26the nivolumab
- 1:33:27secondary
- 1:33:28endpoints
- 1:33:28were duration
- 1:33:29of response,
- 1:33:29complete
- 1:33:30response rate,
- 1:33:31disease
- 1:33:31control rate
- 1:33:32and
- 1:33:32progression
- 1:33:32free survival,
- 1:33:33and the one
- 1:33:34and two rate
- 1:33:34for survival.
- 1:33:35And then we
- 1:33:36as part
- 1:33:37of the trial,
- 1:33:37we wanted
- 1:33:37to understand
- 1:33:38the biomarker,
- 1:33:39elements
- 1:33:40of this
- 1:33:40is that
- 1:33:41there was
- 1:33:41a large
- 1:33:41translational
- 1:33:42component
- 1:33:42to this
- 1:33:43with multiple
- 1:33:44biopsies
- 1:33:45taken as part
- 1:33:45of
- 1:33:46the process.
- 1:33:46And there were
- 1:33:46optional
- 1:33:47biopsies
- 1:33:47as well
- 1:33:48to try
- 1:33:48and understand
- 1:33:49what what
- 1:33:49changes that
- 1:33:50the treatment
- 1:33:51was affecting
- 1:33:52in
- 1:33:52to
- 1:33:52microenvironment.
- 1:33:53And I'll talk
- 1:33:54a little bit
- 1:33:54more
- 1:33:54about that
- 1:33:54towards
- 1:33:55the end.
- 1:33:56So in terms
- 1:33:57of the
- 1:33:57at the
- 1:33:58demographics,
- 1:33:58I think that,
- 1:33:59you know, they
- 1:33:59were they were
- 1:34:01the IBT
- 1:34:02monotherapy
- 1:34:02in those
- 1:34:03three patients
- 1:34:03with uveal
- 1:34:04melanoma
- 1:34:05and then
- 1:34:05the RPt
- 1:34:052% volume up.
- 1:34:06second phase
- 1:34:08that was that
- 1:34:0814 patients,
- 1:34:10essentially
- 1:34:11the the
- 1:34:12monotherapy
- 1:34:13to, to, to
- 1:34:14phase two,
- 1:34:15there weren't
- 1:34:15any really
- 1:34:16notable
- 1:34:17differences.
- 1:34:17But it's
- 1:34:18really
- 1:34:18I think
- 1:34:18the thing
- 1:34:19that's
- 1:34:19really key
- 1:34:20is the fact
- 1:34:20that most
- 1:34:21patients
- 1:34:21had had
- 1:34:22prior
- 1:34:23anti-PD-1 and
- 1:34:25TLR four
- 1:34:26in their,
- 1:34:26in their
- 1:34:27treatment
- 1:34:28prior
- 1:34:28to being given
- 1:34:29this
- 1:34:30treatment
- 1:34:30incumbent
- 1:34:30in the
- 1:34:31volume up
- 1:34:31in combination
- 1:34:32with the
- 1:34:33the RBP to
- 1:34:35an overall
- 1:34:3617.6%
- 1:34:37the patients
- 1:34:38had received
- 1:34:39were more
- 1:34:39or equal
- 1:34:40to three lines
- 1:34:41of therapy.
- 1:34:42So these were
- 1:34:42heavily
- 1:34:43pretreated
- 1:34:43patients.
- 1:34:45and
- 1:34:45the majority
- 1:34:46had also
- 1:34:46received
- 1:34:47this
- 1:34:47combination of
- 1:34:48PD1 and TCT.
- 1:34:49And I thought
- 1:34:51if we
- 1:34:52look at the
- 1:34:52clinical
- 1:34:52activity,
- 1:34:53which is
- 1:34:53essentially
- 1:34:53we were
- 1:34:53looking for
- 1:34:54early,
- 1:34:55early markers
- 1:34:55of clinical
- 1:34:56activity
- 1:34:56in this
- 1:34:57patient group,
- 1:34:58what did we see
- 1:34:59any was really
- 1:35:00the
- 1:35:00fundamental
- 1:35:00question.
- 1:35:01and
- 1:35:02interestingly,
- 1:35:03we saw an
- 1:35:03overall
- 1:35:03response rate
- 1:35:04at 29.4%
- 1:35:06or partial
- 1:35:07responses,
- 1:35:08and a disease
- 1:35:09control rate
- 1:35:09of 58.8%
- 1:35:11at the median
- 1:35:12duration
- 1:35:12of response
- 1:35:13was 11.5
- 1:35:14months,
- 1:35:15with a
- 1:35:15range of 2.8
- 1:35:16to 21.2
- 1:35:17months.
- 1:35:18And
- 1:35:18you can see
- 1:35:19you can
- 1:35:19see that
- 1:35:19written
- 1:35:20down here
- 1:35:20in the table.
- 1:35:21the responses
- 1:35:22were and
- 1:35:23interestingly,
- 1:35:24we found
- 1:35:25we observed
- 1:35:26both HLA
- 1:35:26to positive
- 1:35:28and negative
- 1:35:29patients
- 1:35:29and obviously
- 1:35:30quite a lot
- 1:35:30of the work
- 1:35:31that we're
- 1:35:31doing
- 1:35:31in the
- 1:35:32immune space
- 1:35:32in the
- 1:35:33and leave
- 1:35:34your melanoma
- 1:35:35is focused
- 1:35:35on the HLA
- 1:35:36to positive
- 1:35:37group.
- 1:35:37So
- 1:35:38so we wanted
- 1:35:39to see
- 1:35:39whether
- 1:35:40actually
- 1:35:40you can
- 1:35:40see activity
- 1:35:41in both of
- 1:35:41these groups.
- 1:35:41there
- 1:35:42was in fact
- 1:35:42activity seen.
- 1:35:44And this
- 1:35:46just breaks it
- 1:35:46down in
- 1:35:46terms of
- 1:35:47visuals
- 1:35:47for you.
- 1:35:47So you can see
- 1:35:48that there
- 1:35:48were,
- 1:35:49five patients
- 1:35:50that showed
- 1:35:50a partial
- 1:35:51response,
- 1:35:52and a
- 1:35:53further five
- 1:35:53that showed,
- 1:35:54stable
- 1:35:54disease,
- 1:35:55over time.
- 1:35:58And actually
- 1:35:58what we're
- 1:35:58really
- 1:35:59looking for
- 1:35:59is whether
- 1:35:59we see
- 1:36:00a response
- 1:36:01in the
- 1:36:01injected,
- 1:36:02the,
- 1:36:02an injected,
- 1:36:03lesions.
- 1:36:04And this is
- 1:36:04a really
- 1:36:05this is a
- 1:36:05really key
- 1:36:06question
- 1:36:06for us
- 1:36:07with, virus
- 1:36:08therapy.
- 1:36:08Can we
- 1:36:09essentially
- 1:36:10show,
- 1:36:10antigen
- 1:36:11presentation.
- 1:36:12that leads
- 1:36:13to further
- 1:36:14T-cell
- 1:36:14recognition
- 1:36:15of other
- 1:36:15areas, that,
- 1:36:18that will
- 1:36:18lead to
- 1:36:18response, in,
- 1:36:20in the non
- 1:36:20injected
- 1:36:21lesions.
- 1:36:21And you can
- 1:36:21see in these
- 1:36:22and in this,
- 1:36:23in this
- 1:36:23patient here
- 1:36:24who had a
- 1:36:25partial
- 1:36:25response,
- 1:36:26you can see
- 1:36:26responses
- 1:36:27in both
- 1:36:27both T
- 1:36:28and injected.
- 1:36:28the injected
- 1:36:29lesions
- 1:36:30which is
- 1:36:30important to,
- 1:36:32to notice
- 1:36:32that we do
- 1:36:33see this
- 1:36:33sort of
- 1:36:34more upscale
- 1:36:35effect.
- 1:36:36and
- 1:36:38and again,
- 1:36:39you can
- 1:36:40see that
- 1:36:40as well.
- 1:36:43So what are
- 1:36:43we thinking
- 1:36:43about?
- 1:36:44the clinical
- 1:36:45responses.
- 1:36:46We also
- 1:36:46want to see
- 1:36:46what's
- 1:36:47happening
- 1:36:47at a more,
- 1:36:48more baseline,
- 1:36:48translational
- 1:36:49level,
- 1:36:50because
- 1:36:50that will help
- 1:36:50us understand
- 1:36:51how we can
- 1:36:51then
- 1:36:52essentially
- 1:36:53coordinate
- 1:36:53and, and,
- 1:36:54build on
- 1:36:55these results.
- 1:36:56So what we did
- 1:36:56here is
- 1:36:57we took
- 1:36:57biopsies,
- 1:36:58at baseline
- 1:36:59screening
- 1:37:00and at day 43
- 1:37:01and we
- 1:37:02stained
- 1:37:02for CD8,
- 1:37:03which is
- 1:37:04a marker
- 1:37:04of T-cell
- 1:37:05presence,
- 1:37:06and a
- 1:37:07marker of
- 1:37:07immune
- 1:37:07activity.
- 1:37:09and you can
- 1:37:09see that
- 1:37:09these change
- 1:37:10over time
- 1:37:10that
- 1:37:11the tumors
- 1:37:11become
- 1:37:12more
- 1:37:12infiltrate it,
- 1:37:14in response
- 1:37:14to the
- 1:37:15injections.
- 1:37:15And also
- 1:37:16there's a
- 1:37:16higher, higher
- 1:37:17expression of
- 1:37:17PD one,
- 1:37:18which would
- 1:37:18suggest that
- 1:37:19there is a,
- 1:37:21a more sort
- 1:37:21of a need
- 1:37:22and immune
- 1:37:23environment,
- 1:37:24interestingly,
- 1:37:24new selected
- 1:37:25T cell
- 1:37:26receptor
- 1:37:27sequencing
- 1:37:28and within
- 1:37:28pbmcs
- 1:37:29within the
- 1:37:29with skin
- 1:37:30and peripheral
- 1:37:30blood
- 1:37:31looking to see
- 1:37:31if we
- 1:37:32could see
- 1:37:32a response.
- 1:37:33in the sort
- 1:37:34of the
- 1:37:34circulating
- 1:37:35immune
- 1:37:37so
- 1:37:38repertoire,
- 1:37:39as a, as a
- 1:37:40response
- 1:37:40to treatment.
- 1:37:41And we can see
- 1:37:41that there are
- 1:37:42significant
- 1:37:43expansion
- 1:37:43and new
- 1:37:44clones.
- 1:37:45there was very
- 1:37:46there was
- 1:37:46some claims
- 1:37:47that got
- 1:37:47smaller,
- 1:37:48but actually
- 1:37:49there are,
- 1:37:49as you
- 1:37:49can see,
- 1:37:49a lot of blue
- 1:37:50blobs on that.
- 1:37:51On
- 1:37:51these curves,
- 1:37:52it showed that
- 1:37:52numerous
- 1:37:53clones
- 1:37:53exhibited
- 1:37:54expansion,
- 1:37:55that were
- 1:37:56first
- 1:37:56identified on
- 1:37:57day 43.
- 1:37:58So after the,
- 1:37:59the sort
- 1:38:00of the
- 1:38:00implementation
- 1:38:01of treatment.
- 1:38:02So
- 1:38:02we can see
- 1:38:03that these,
- 1:38:03this
- 1:38:04therapeutic,
- 1:38:05combination
- 1:38:06is leading to
- 1:38:06mean
- 1:38:07activation.
- 1:38:07and and
- 1:38:09actually
- 1:38:09seeing
- 1:38:09benefit.
- 1:38:10One of the
- 1:38:10things that
- 1:38:12was
- 1:38:12interesting is
- 1:38:13we didn't
- 1:38:13necessarily
- 1:38:14see that
- 1:38:14this clonal
- 1:38:14expansion
- 1:38:15correlates to
- 1:38:16clinical
- 1:38:16response.
- 1:38:17So that
- 1:38:17that means
- 1:38:18that there's
- 1:38:18probably
- 1:38:19more work
- 1:38:19that we
- 1:38:19could do
- 1:38:20with
- 1:38:20additional
- 1:38:21agents
- 1:38:21or
- 1:38:22a different,
- 1:38:22trial design
- 1:38:23to actually
- 1:38:24understand
- 1:38:24how that will,
- 1:38:25that will
- 1:38:25lead itself
- 1:38:26to, to,
- 1:38:27to leading
- 1:38:27to clinical
- 1:38:28clinical
- 1:38:28response,
- 1:38:29you know,
- 1:38:29but actually
- 1:38:30just seeing
- 1:38:30the fact
- 1:38:30that we see
- 1:38:31those changes
- 1:38:31means
- 1:38:32that these,
- 1:38:32these
- 1:38:32therapies,
- 1:38:33therapies are
- 1:38:34made immune,
- 1:38:35generally
- 1:38:35active,
- 1:38:36which is
- 1:38:36really
- 1:38:36reassuring
- 1:38:37in this phase,
- 1:38:37in this
- 1:38:38setting phase
- 1:38:39on trial,
- 1:38:40the safety
- 1:38:41profile
- 1:38:41is also
- 1:38:41quite
- 1:38:42interesting.
- 1:38:42So as you can
- 1:38:43see here,
- 1:38:43there were
- 1:38:44there were
- 1:38:44some patients
- 1:38:45who who got
- 1:38:46toxicity.
- 1:38:46the most were
- 1:38:48common
- 1:38:48were grade
- 1:38:49one away
- 1:38:49two in
- 1:38:51in groups
- 1:38:52were directed
- 1:38:52towards
- 1:38:53fatigue
- 1:38:53and
- 1:38:53hypertension.
- 1:38:54And some
- 1:38:54people
- 1:38:55also got it.
- 1:38:56But process.
- 1:38:57there were
- 1:38:58two cases
- 1:38:59of grade
- 1:39:00three
- 1:39:00hypertension,
- 1:39:01but both
- 1:39:01were transient
- 1:39:02and mostly
- 1:39:02managed
- 1:39:03with fluid
- 1:39:03therapy alone.
- 1:39:04And there were
- 1:39:05no grade
- 1:39:054 or 5,
- 1:39:07a which
- 1:39:08again,
- 1:39:08is quite,
- 1:39:08quite
- 1:39:09relevant.
- 1:39:09Interestingly,
- 1:39:10given the fact
- 1:39:11that the
- 1:39:11majority
- 1:39:12of the eight
- 1:39:12of the
- 1:39:12patients
- 1:39:13in the RPE,
- 1:39:14I'll be
- 1:39:14treated
- 1:39:15as nivolumab
- 1:39:15trial
- 1:39:16also didn't
- 1:39:17experience,
- 1:39:18any particular
- 1:39:20I mean, when
- 1:39:20I set
- 1:39:21this event
- 1:39:21toxicities
- 1:39:22is really
- 1:39:23quite
- 1:39:23noticeable.
- 1:39:24and and
- 1:39:25and suggesting
- 1:39:27a lower rate
- 1:39:27than
- 1:39:27we would do
- 1:39:28with
- 1:39:28the volume of,
- 1:39:29therapy,
- 1:39:30is a single
- 1:39:30agent,
- 1:39:30which is
- 1:39:31quite
- 1:39:31interesting
- 1:39:31at this point.
- 1:39:33So in
- 1:39:33conclusion,
- 1:39:34we see
- 1:39:34the optimal
- 1:39:35therapy
- 1:39:36in P2.
- 1:39:36Plasma volume
- 1:39:37therapy
- 1:39:38resulted in
- 1:39:39an overall
- 1:39:39response
- 1:39:39rate of 29.4%,
- 1:39:42and
- 1:39:42the disease
- 1:39:42control rate
- 1:39:43at 58.8%.
- 1:39:44In a cohort
- 1:39:45of patients
- 1:39:46with
- 1:39:46metastatic
- 1:39:47melanoma,
- 1:39:48who have
- 1:39:48been heavily
- 1:39:49pretreated,
- 1:39:50and
- 1:39:51they showed
- 1:39:51durable
- 1:39:51responses
- 1:39:52and a
- 1:39:52favorable
- 1:39:53safety
- 1:39:53profile.
- 1:39:55We saw
- 1:39:55this activity,
- 1:39:56regardless of
- 1:39:56HLA status,
- 1:39:58I think is
- 1:39:58very
- 1:39:58reassuring
- 1:39:59for those
- 1:39:59for that
- 1:39:59patient group
- 1:40:00that don't
- 1:40:00have a
- 1:40:01need to
- 1:40:01positivity.
- 1:40:03and
- 1:40:03it doesn't
- 1:40:03seem to
- 1:40:03be specific
- 1:40:04to a priori
- 1:40:05PSI regime.
- 1:40:06Essentially,
- 1:40:06patients
- 1:40:07could have had
- 1:40:07already
- 1:40:08been treated
- 1:40:08with
- 1:40:08an anti-PD-1,
- 1:40:09and we would
- 1:40:10still CMT,
- 1:40:11associate the
- 1:40:12benefit
- 1:40:12in this
- 1:40:13in this
- 1:40:13setting.
- 1:40:14And we saw,
- 1:40:15responses in
- 1:40:16both
- 1:40:16the hepatic
- 1:40:17and that
- 1:40:17to testes
- 1:40:18and injected
- 1:40:19in yet
- 1:40:19to
- 1:40:19the lesions.
- 1:40:20So we're
- 1:40:21seeing quite
- 1:40:21a lot
- 1:40:21of activity
- 1:40:22across
- 1:40:22the
- 1:40:22metastatic,
- 1:40:24repertoire in
- 1:40:25even in
- 1:40:26those patients
- 1:40:26who've
- 1:40:26had only
- 1:40:27a small number
- 1:40:28of lesions
- 1:40:28injected,
- 1:40:29we do see an
- 1:40:30increase in
- 1:40:31the infiltration
- 1:40:32and we see
- 1:40:32increased
- 1:40:33Pd-l1
- 1:40:33expression
- 1:40:34in tumors.
- 1:40:35and we see
- 1:40:35also there's
- 1:40:36changes
- 1:40:36in the
- 1:40:36peripheral
- 1:40:37T-cell
- 1:40:37repertoire,
- 1:40:38which is
- 1:40:38useful
- 1:40:38in
- 1:40:39understanding
- 1:40:39the fact
- 1:40:39that we see
- 1:40:40different
- 1:40:40T cells
- 1:40:41being,
- 1:40:41the expressed
- 1:40:42and expanded,
- 1:40:43but also
- 1:40:43we can
- 1:40:44we have a
- 1:40:44peripheral
- 1:40:45marker of
- 1:40:45potential,
- 1:40:46I mean
- 1:40:47activation,
- 1:40:48which is,
- 1:40:48which is
- 1:40:48useful in
- 1:40:49the patient
- 1:40:49group
- 1:40:50in patients
- 1:40:50generally.
- 1:40:51I might
- 1:40:52possibly have
- 1:40:52some clinical
- 1:40:53translate
- 1:40:53ability
- 1:40:54further down
- 1:40:54the line.
- 1:40:55This suggests
- 1:40:56that, this,
- 1:40:57intramural
- 1:40:58gated
- 1:40:58community
- 1:40:58therapy,
- 1:40:59within
- 1:41:00in this
- 1:41:00expression
- 1:41:01of this
- 1:41:01antigenic
- 1:41:02four molecule
- 1:41:03as part of
- 1:41:03its genetic
- 1:41:03modification,
- 1:41:04in combination
- 1:41:05with this
- 1:41:05checkpoint
- 1:41:06inhibitor,
- 1:41:06has a
- 1:41:07potential for
- 1:41:07clinically
- 1:41:08meaningful
- 1:41:08benefit
- 1:41:09and a
- 1:41:09favorable
- 1:41:09toxicity
- 1:41:10profile.
- 1:41:11And therefore,
- 1:41:12we're going
- 1:41:12to directly,
- 1:41:13you know,
- 1:41:13taking these
- 1:41:14forward
- 1:41:14and developing
- 1:41:15a more
- 1:41:16advanced phase
- 1:41:17two trial.
- 1:41:18so essentially
- 1:41:19we see
- 1:41:20we see
- 1:41:20really notable
- 1:41:21objective
- 1:41:22response rates
- 1:41:22and disease
- 1:41:23control rates
- 1:41:24and patients,
- 1:41:25and don't
- 1:41:25appear to
- 1:41:26exhibit,
- 1:41:26particularly
- 1:41:27concerning
- 1:41:28safety
- 1:41:29profiles.
- 1:41:30and obviously,
- 1:41:31we will be
- 1:41:32more than
- 1:41:32happy to
- 1:41:33take any,
- 1:41:34any for
- 1:41:35information
- 1:41:35and that,
- 1:41:36that emails
- 1:41:36at the bottom
- 1:41:37of this slide,
- 1:41:38I'm
- 1:41:38going to stop.
- 1:41:41Thank you
- 1:41:41so much.
- 1:41:42this
- 1:41:43was amazing.
