Transcript
- 0:00Hello, hello, welcome back everybody.
- 0:01Welcome back to the podcast. So glad to be back recording
- 0:07episodes with you guys. So I have some updates and some
- 0:13really good information to give you guys because I have a slew
- 0:17of brand new episodes for the month of October.
- 0:20OK, so I am bringing you some really good hack full
- 0:25information on this podcast. And if you haven't subscribe to
- 0:30the YouTube channel because there's more content coming to
- 0:32the YouTube channel as well. So let me kind of run through
- 0:34some updates really quick. OK, so like I mentioned with the
- 0:37YouTube channel, I am going to be posting here soon, probably
- 0:42this week. So make sure you follow on
- 0:44social, make sure you're on my e-mail list so you can get the
- 0:47exact date on when I'm dropping this course, I am going to be
- 0:54teaching and uploading a video on a patient centering course
- 0:58because that seems to be a topic that's really, really hot is
- 1:02patient centering. So I'm going to give you some
- 1:05really good tips and I'm going to go into depth on patient
- 1:08centering and why that matters. OK?
- 1:10So make sure you stay tuned for that.
- 1:12That's going to be uploaded on the YouTube channel later on
- 1:14this week. So make sure you're on the
- 1:16e-mail list. Make sure you follow on TikTok
- 1:18and Instagram because I will be releasing the date on that.
- 1:22OK, also make sure you guys follow on TikTok because I
- 1:26dropped some really good informational stuff when it
- 1:29comes to CT on TikTok because that is my main platform.
- 1:32This platform I love to post on, so make sure you do that as
- 1:36well. Now I've also had a backup page
- 1:38on TikTok. That's when my main page did get
- 1:42hacked, but it was restored to me a few months ago.
- 1:45So I have just kind of kept that second page dormant.
- 1:49Well, I've decided to rebrand it and I'm going to be talking
- 1:53about more life content, sharing more pieces about my life as a
- 1:57mom, being divorced, relationships, doing feeling
- 2:01inner work and just childhood, you know, inner shit trauma.
- 2:07If you want to follow that page and see all the good stuff on
- 2:10that, I'll have a link in the show notes so you can follow
- 2:12that page on TikTok. OK.
- 2:15That one also has its own dedicated podcast and YouTube
- 2:18channel. So I'll put all that stuff in
- 2:19the show notes so you guys can follow along.
- 2:22So got some really good episodes coming for the Cat Scan Guru
- 2:25podcast. Just some things that we're
- 2:27going to talk about and we're going to dial this shit up a
- 2:29little bit. OK?
- 2:30So in today's episode, we're going to talk into depth about
- 2:35CT 10. What what you weren't taught.
- 2:38OK, I know I've gone through CT 101 episodes before, but this is
- 2:43going to be spicier with some hotter takes and some
- 2:47information that you really should know.
- 2:49So if you're new here, make sure you subscribe to the podcast and
- 2:54make sure you like share, you know, with a technologist that's
- 2:57fucking needs to hear this shit. OK, so real CT training didn't
- 3:01come from a program or from your first job.
- 3:05I know that's going to be a hot take for everybody.
- 3:07This is the episode that's going to give you some foundation so
- 3:10you can stop guessing when you're doing your CT training,
- 3:13especially as a new technologist or if you're at a new facility,
- 3:17right? So let's be real.
- 3:20OK, Let's have a reality check for a second.
- 3:23Your program, you know, in CT or the minimal training that you
- 3:28received when you were just starting out, you know, taught
- 3:31you just enough. It didn't teach you everything,
- 3:33but it taught you just enough. But that enough wasn't good
- 3:36enough. OK, Now I'm going to be talking
- 3:39about some things here on what I think you should understand and
- 3:42what are the foundations of CT, OK?
- 3:45Now, not knowing your anatomy, your protocols, contrast,
- 3:50timing, all those things are very important, OK?
- 3:53Because there are certain landmarks, there are certain
- 3:55anatomy that we have to cover depending on the exam that we're
- 3:58scanning, depending on the scan that is asked for.
- 4:02And sometimes patients are very complicated and you can't get
- 4:05like an optimal scan and you have to do the best that you can
- 4:08knowing your basic protocols. Listen, NCT, there's a shit ton
- 4:14of protocols, OK? The protocol book is thicker
- 4:17than I don't know what. It is physically impossible for
- 4:20you to remember all those protocols in your freaking
- 4:23brain, OK? That's why I encourage texts to
- 4:25make sure that they take a notebook or something with them
- 4:28so they can write shit down South.
- 4:29So protocols there are, you know, basic protocols that you
- 4:33should know in your head without contrast.
- 4:34Your PE study, your abdomen, pelvis with contrast.
- 4:37You know, your C, spine, facial bones, things that you do every
- 4:40day, all day that gets ordered through the ER consistently and
- 4:44that we're freaking bored of scanning.
- 4:47Those are the things you need to know.
- 4:49Contrast time because certain exams have different contrast
- 4:52timing. So for example, a PE study has a
- 4:55different contrast timing than an abdomen and pelvis with
- 4:59contrast. If you're looking for abdominal
- 5:01pain, the timing is very different.
- 5:03And those are the things that you need to know.
- 5:05Now, this is one of the pieces that I stress in every podcast
- 5:08episode and I am going to continue to tell you until I'm
- 5:12blue in the face until you get it.
- 5:14Patient positioning, Why? This is the foundation of it all
- 5:18because optimal scans and optimal dose start out with
- 5:23correct patients positioning and centering.
- 5:25Let me say that again, optimal exams and optimal scanning start
- 5:31from proper patient positioning and centering.
- 5:34It's the foundation of everything.
- 5:35That's how you get your dose. You know, under the limits that
- 5:38you're supposed to, making sure you're administering the correct
- 5:41dose because you need to remember your scalp calculates
- 5:44how much dose is going to be delivered to the patient and the
- 5:47system calculates that. So if you're way off, it's going
- 5:50to calculate either something that's way too low and the tube
- 5:54is starving, or it's way too high where it's exposing in the
- 5:57patient with too much radiation. Now, I know there are certain
- 6:00instances where you get contracted patients, patients
- 6:03who are combative, patients who are just a hot mess.
- 6:06What I say to that is to do the best you can, you could still
- 6:09position and center properly. Even if they're contracted, even
- 6:12if they're a hot mess, you can still do it, right?
- 6:15OK. And then your landmarks that
- 6:17matter. OK, So we know for a CT of the
- 6:20head, you're going to center at your OM.
- 6:22You know, for your chest, you're going to center at the sternal
- 6:24notch. You know for the abdomen and
- 6:26pelvis, you're going to Center for the xiphoid tip.
- 6:29Now with systems like Siemens and Phillips, you have to center
- 6:32higher. And this is one of the things
- 6:34that I taught one of the texts that is brand new to our
- 6:37department where I work at is on the Phillips machine that we
- 6:40have in the department. You actually have to center
- 6:42higher because the table starts moving before those tube
- 6:45exposes. So and I said just watch it when
- 6:49we when it gives you the green light to go, just watch the
- 6:52table and listen to the exposure.
- 6:55And sure enough, the table started moving first before the
- 6:59exposure hit. So that's why you have to center
- 7:01higher. OK, and then mastering your
- 7:03protocols. Now, I don't expect you to know
- 7:06every single freaking protocol there is to man, OK, I expect
- 7:10you to know the basic ones because those are the ones that
- 7:14you're going to, you know, get repetition on.
- 7:17You're going to be consistent on and you're going to build
- 7:19knowledge on because once you are consistent, there's
- 7:23repetition and you have knowledge on all the basic
- 7:25protocols you're going to fucking then just kill it with
- 7:28the other ones and you're going to know exactly what to do and
- 7:31you know what you're doing. OK, so you have to master those
- 7:34protocols. Plus you also need to master on
- 7:37what you're doing in that room with the patient.
- 7:40It's not all about pressing buttons guys, because you know
- 7:42what? I get some text in my comments
- 7:44in on TikTok that they feel that it's just pressing a button.
- 7:49I'm telling you, CT is more complex than that and radiology
- 7:51is more than that OK than pressing a damn button.
- 7:55It's not all about pressing buttons, your actually making an
- 7:58impact in that patient's life and trying to figure out
- 8:01clinically what is wrong with them so doctors can treat them.
- 8:04OK Now you can't be confident if you don't know the basics.
- 8:09Period, end of story. OK like you need to know your
- 8:11basics. You need to know how to position
- 8:14your patient properly. You need to know how to utilize
- 8:18the buttons, how to put them on the table correctly, how to, you
- 8:21know, maneuver the table up and down, where to select the
- 8:24protocols. You know on your screen how to
- 8:27execute that contrast timing. Making sure your four's are good
- 8:31and they're checked properly, making sure that your contrast
- 8:35is injector is primed and ready to go, making sure you're
- 8:38checking labs, checking the orders, checking the chart for
- 8:41allergies and all that stuff. So you have to know the basics.
- 8:45And that's why I give you an 8 week checklist, which you can
- 8:48find in the show notes on things that I feel that you should know
- 8:51when you're starting out in CT for the very first time.
- 8:54And especially if you're a new technologist, you really need a
- 8:56guide to know how to execute those things.
- 8:59OK, now it's not your fault that you weren't taught correctly,
- 9:04OK? But it is your fault if you
- 9:06choose to stay stuck there and not do anything about it.
- 9:09I can't tell you how many times I have worked at different
- 9:13institutions where techs fail to improve, you know, their
- 9:18scanning abilities to learn the new trends, learn the latest and
- 9:22greatest, to get more education, to go to the seminars, to, you
- 9:26know, learn something because they say, oh, well, this is the
- 9:30way it's done. This is how we're going to do
- 9:31it. And I don't need to know
- 9:33anything else. Now.
- 9:34Our technology is constantly evolving.
- 9:36It is constantly changing and things change, especially with,
- 9:41you know, faster scanners, different kinds of protocols,
- 9:44protocols being added, new things being added, you know,
- 9:47things that are getting taken away from X-ray and coming to
- 9:51CT. OK, When was the last time you
- 9:53saw an IVP done? OK, the last time I saw an IVP
- 9:56done was probably 20 plus years ago when I was working in X-ray.
- 10:01OK, I've been in tech for 23 years, so it was probably about
- 10:0420 years ago. CT has taken that on.
- 10:07We do urograms, we do histograms, so we have taken
- 10:10that on. When was the last time you saw a
- 10:12mandible, an X-ray done? Now, I'm not saying that they
- 10:15aren't done, but how often are they done?
- 10:18You know why? Because CT has taken that over.
- 10:20So it's really important to invest in your continuing
- 10:24education, in your knowledge and staying up to date with the
- 10:27latest trends. I can't tell you how many
- 10:30technologists I've met over my entire career that choose to
- 10:33stay stuck. And then they wonder why they
- 10:37can't advance into anything because you're not willing to
- 10:41put yourself out there. Invest in yourself to learn
- 10:43something and you really should. That is the name of the game.
- 10:48Like if you want to stay stuff, that is a personal choice.
- 10:51But I know there's a lot of room for growth and for improvement
- 10:55and to move on to excel and to things that you have a passion
- 11:00for or things that might light you up.
- 11:03OK, what lights me up is doing this podcast.
- 11:06What lights me up is teaching. What lights me up is going on
- 11:09TikTok and giving you that educational information that you
- 11:13need to know because it actually helps people.
- 11:15Can't tell you how many times in my DMS and in my comments about
- 11:18people who appreciate and praise that I give out this information
- 11:23because it's so helpful. So think about it like that, OK?
- 11:27It is your fault if you choose to stay stuck and not, you know,
- 11:31leave room for growth, OK? It's not your fault that you
- 11:34weren't taught correctly because there are texts that are
- 11:36training the newer ones coming in and the training is shit.
- 11:41And I hate to say that, but that's the truth.
- 11:43So we're going to be honest on this podcast and there is room
- 11:47for improvement on that. And us as a community need to
- 11:52just, you know, get involved in improve the situation, OK,
- 11:57because that's what it takes to make this community just
- 12:00wonderful. And there's a lot of room for
- 12:03growth and just elevate things so that we are seeing.
- 12:07OK, So those are my talking points for this podcast episode.
- 12:12Like I said, everything that I discussed and as far as where to
- 12:15find things, everything will be in the show notes as far as any
- 12:18courses that I whole that I that Ioffer that I sell.
- 12:24Sorry, where you can follow me on social media TikTok.
