- 0:00Good morning, everyone, and welcome to another episode of Morning Medical with Doctor Chow.
- 0:04I am your host, Doctor Chi-Ming Ming Chow.
- 0:08As always, We are here to bring you the latest updates that could have a significant and let's
- 0:25dive into today's medical news.
- 0:31Our first story today comes from a recent study conducted by Harvard University which has been
- 0:36published in JAMA Network open.
- 0:38This study sheds light on the benefits of the Mediterranean diet and its potential to help
- 0:43women live longer.
- 0:44The Mediterranean diet has long been associated with numerous health benefits, including a
- 0:49reduced risk of heart disease, cancer, and obesity.
- 0:53Chow this new study has revealed a very specific advantage for women, a 23% lower risk
- 0:59of death from any cause.
- 1:02The study included data collected from 25,115 healthy women who participated in the women's
- 1:09health study over a period of 25 years from April 1993 to January 1996.
- 1:16The average age of the participants was 54.
- 1:20Researchers analyzed the women's blood samples biomarker measurements and self reported
- 1:24dietary information from June 2018 to November 2023.
- 1:29The findings were quite compelling.
- 1:31Women who adhered to the heart healthy plant focused, meditating, and diet were found to
- 1:36have a 23% lower risk of death from any cause.
- 1:40So what exactly is the Mediterranean diet?
- 1:44It is a plant based nutrition plan that mimics the regional cuisines of the countries along
- 1:49the Mediterranean Sea, such as Italy and Greece.
- 1:53Primary foods in the diet include whole vegetables, whole grains, fruits, nuts, seeds,
- 2:00and herbs, and spices.
- 2:02Olive oil is the primary source of added fat.
- 2:06Other foods, including fish, poultry, and dairy, can be incorporated in moderation.
- 2:11The diet limits red meat, sweets, butter, and sugary drinks.
- 2:16The health benefits of the Mediterranean diet stem from limiting saturated fats, refined
- 2:22carbohydrates, and sodium, while promoting healthy unsaturated fats fiber and
- 2:27antioxidants.
- 2:29Experts have long highlighted the benefits of the Mediterranean diet.
- 2:34Doctor Mark Siegel, a clinical professor of medicine at NYU Langony Medical Center, and a
- 2:40Fox News medical contributor was not involved in the study, but commented on its findings.
- 2:46He stated that the study confirms what is already known.
- 2:49That the Mediterranean diet decreases mortality.
- 2:53Doctor Isegal pointed out that monounsaturated fats and antioxidants which are abundant in the
- 2:59Mediterranean diet, decrease inflammation, and are heart healthy.
- 3:03He contrasted this with saturated fats, salts, and sugars, which are not heart healthy and
- 3:09increase the risk of obesity, high blood pressure, diabetes, and cancer.
- 3:15New Jersey based registered dietitian, Aaron Polinski Wade, also reflected on the longevity
- 3:21boosting benefits of the Mediterranean diet.
- 3:24She explained that one significant reason, the Mediterranean diet can improve overall health
- 3:29and reduce mortality risk is its impact on visceral fat or belly fat.
- 3:35High levels of fat in the mid section have been found to worsen insulin resistance and increase
- 3:41the risk of diseases such as type 2 diabetes, cancer and heart disease.
- 3:47By making dietary changes to reduce visceral fat, which align with the principles of the
- 3:53Mediterranean diet.
- 3:54It is possible to improve insulin sensitivity and reduce disease risk.
- 4:00The Mediterranean diet is also rich in antioxidants and phytonutrients, which have
- 4:05been shown to reduce inflammation in the body.
- 4:07This can help to reduce the risk of future diseases such as type 2 diabetes and
- 4:12cardiovascular disease.
- 4:14Additionally, the diet has a positive impact on blood sugar balance, mood, and even sleep, all
- 4:20of which can help to reduce stress levels and improve overall quality of life.
- 4:25When mood and sleep improve, many individuals find it much easier to be more physically
- 4:30active, which can play a large role in reducing mortality risk as well.
- 4:35However, the study did have some limitations.
- 4:38The participants were middle aged and older, well educated female health professionals who
- 4:44were predominantly non Hispanic white individuals.
- 4:47This may limit the generalizability of the findings.
- 4:51Additionally, because the women's dietary intake was self reported through
- 4:56questionnaires, there was some potential for misrepresentation.
- 5:01Another limitation is that the researchers only had access to the blood samples provided at
- 5:06baseline as follow-up samples were not collected.
- 5:10There is also the possibility that other factors such as high blood pressure and high
- 5:14body mass index could have impacted the outcomes.
- 5:19Despite these limitations, the study adds to the growing body of evidence supporting the
- 5:23Mediterranean diet as a powerful tool for improving health and longevity.
- 5:28Future studies should examine other pathways that could potentially mediate the
- 5:32Mediterranean diet associated lower mortality as well as examine cause specific mortality.
- 5:38This is an exciting area of research that holds promise for further understanding how dietary
- 5:43choices can impact long term health outcomes.
- 5:47Our next topic revolves around a question that many patients and health care providers are
- 5:53grappling with.
- 5:54How to find the right balance between telemedicine and in person care.
- 5:59Telemedicine has gained significant traction especially in the wake of the COVID 19
- 6:04pandemic.
- 6:05But the question remains, when should you opt for a virtual visit, and when is it essential
- 6:10to see your doctor face to face?
- 6:13Telemedicine generally refers to diagnosing and treating patients remotely, often through a
- 6:18secure video connection.
- 6:20This can be done using a smartphone, tablet, or computer.
- 6:24Telemedicine can also involve telephone calls or exchanging secure messages with your
- 6:29doctor's office to discuss test results or follow-up steps after an appointment.
- 6:34It can be an effective way to diagnose new health problems and monitor existing conditions
- 6:39such as diabetes.
- 6:411 of the primary advantages of telemedicine is convenience.
- 6:45Virtual visits can save time and offer patients a broader selection of health care providers.
- 6:51This is particularly beneficial for those living in areas with limited access to in
- 6:55person care or for patients who cannot easily take time off work or lack transportation.
- 7:01Telemedicine's popularity surged during the COVID 19 pandemic.
- 7:06And while it has declined somewhat since then, it remains more utilized than it was pre
- 7:11pandemic, especially in fields like dermatology and mental health care.
- 7:16However, the effectiveness of telemedicine hinges on a few key factors.
- 7:20First, ensure that your device's audio and video functions are working correctly before
- 7:25the visit begins.
- 7:27You might need a few moments to adjust your device settings.
- 7:30It is also crucial to be in a private location, particularly for sensitive consultations such
- 7:36as therapy sessions.
- 7:38Avoid using public wifi or being in a noisy environment like a work cubicle, library, or
- 7:44restaurant.
- 7:45Additionally, do not drive, walk, or eat during your telemedicine appointment as these
- 7:52activities can be distracting and unsafe.
- 7:55Despite its many benefits, telemedicine does have limitations.
- 8:00A reliable fast internet connection is essential.
- 8:04And not all patients or health care providers may have access to the necessary technology.
- 8:11Physical exams which are sometimes crucial for accurate diagnosis cannot be performed
- 8:16virtually.
- 8:17For example, a patient seeking help for a urinary tract infection which could be managed
- 8:22through telemedicine might have underlying gallbladder issues that require an in person
- 8:28ultrasound.
- 8:29Additionally, receiving telemedicine from out of state doctors can be complicated due to
- 8:35varying state regulations.
- 8:37Particularly now that pandemic emergency declarations facilitating such practices have
- 8:43ended.
- 8:44So how do you find the right balance between telemedicine and in person visits?
- 8:49It often depends on the patient's comfort level with telemedicine and the specific care they
- 8:54require.
- 8:55In some cases, a balance may not be possible due to a lack of in person options or
- 9:00difficulty scheduling appointments.
- 9:02If feasible, Doctor E Jay Lee, a family physician who conducts both in person and
- 9:09virtual visits recommends an initial in person visit followed by telemedicine for subsequent
- 9:14checkups.
- 9:16This initial face to face meeting is vital for establishing a rapport and ensuring both the
- 9:21doctor and patient feel comfortable working together.
- 9:25Ultimately, and in person care will vary from patient to patient.
- 9:32It is essential to assess your specific health care needs and consult with your health care
- 9:37provider to determine the most appropriate approach.
- 9:41As telemedicine continues to evolve, it promises to remain a valuable component of
- 9:46modern health care.
- 9:48Complimenting traditional in person visits and enhancing overall access to medical services.
- 9:55Chow let us move on to our next segment, which addresses a common concern that many people
- 10:00experience hand swelling.
- 10:02If you notice your hands are swelling, it could be both uncomfortable and unsettling.
- 10:07But in most instances, the cause could be situational.
- 10:12Factors such as temperature, exercise, diet, medications you are taking, or certain health
- 10:18conditions can all contribute to hand swelling.
- 10:21You may notice your rings are not fitting correctly, among other things.
- 10:25To find out more about this condition, Fox News Digital asked 2 doctors what causes hand
- 10:31swelling and what can be done to treat it.
- 10:34Doctor Amy Kiel, a board certified rheumatologist with St.
- 10:37John's physician partners in Santa Monica, California, explains that most commonly hand
- 10:43swelling is due to fluid buildup in the soft tissue.
- 10:47Soft tissue swelling or edema is not always a cause for concern.
- 10:52And can occur with weather changes or fluid shifts in the body, said doctor Raquel.
- 10:58She notes that fluid shifts can occur with too much salt intake which can lead to increased
- 11:04hand or foot swelling.
- 11:06Most patients observe some degree of soft tissue swelling or edema with hot weather and
- 11:12with exercise.
- 11:13Additionally, certain medications such as blood pressure medications like amlodipine steroids
- 11:19such as prednisone or medrol or hormone treatments can contribute to soft tissue
- 11:25swelling in the hands or feet.
- 11:27However, there are more serious causes of hand swelling that require medical attention.
- 11:33Doctor Kale advises that if a person notices an increase in hand or foot swelling that is not
- 11:40reversible or intermittent, or if the hand swelling is also accompanied by leg swelling
- 11:45with marked elevations in blood pressure or shortness of breath, medical attention should
- 11:50be sought.
- 11:51A person should ensure they have had proper medical evaluation to assess both kidney and
- 11:55cardiac function as hand or foot swelling can be an indicator of kidney dysfunction.
- 12:00Protein loss in the urine or cardiac dysfunction she cautioned.
- 12:05Your doctor may order tests to assess your kidney function urine studies, or in some
- 12:10cases, an echocardiogram.
- 12:12Diuretics may be needed to remove the excess fluid if it is related to cardiac dysfunction.
- 12:19Another possible cause of fluid buildup is lymphedema, which can occur if the patient has
- 12:24had surgery.
- 12:25Such as a lymph node resection for breast cancer treatment according to doctor.
- 12:31This may be treated with local lymphatic drainage treatments arm elevation, compression
- 12:36sleeves, and exercise.
- 12:38The doctor said.
- 12:39Patients should seek medical attention right away.
- 12:42If new and asymmetric swelling and pain develop.
- 12:46Individuals should also be cognizant of other symptoms.
- 12:50Certainly, if the hand swelling is associated with joint pains, this may indicate an
- 12:55underlying inflammatory arthritis, such as rheumatoid arthritis or psoriatic arthritis.
- 13:01Doctor Kale told Fox News Digital.
- 13:04Again, an examination may be warranted to distinguish soft tissue swelling from articular
- 13:09or joint swelling.
- 13:11Patients should also be aware if the swelling is asymmetric.
- 13:15This can be a sign of something more serious, such as a blood clot, doctor Cal warned.
- 13:27If a patient notices hand swelling and association with facial or lip swelling, this
- 13:32warrants immediate medical attention, she said.
- 13:35If the hand swelling is deemed to be due to benign causes, this is typically reversible,
- 13:41doctor Ron's Kale noted.
- 13:43For instance, to reduce hand swelling after exercising, elevating the hands, and performing
- 13:49arm circles may be helpful to increase the blood flow to the hands.
- 13:53She said, Similarly, it may be necessary to review your medication list with your medical
- 13:58provider to identify any pharmaceutical culprits.
- 14:02Typically, with discontinuation of the offending medication, the swelling should
- 14:06subside.
- 14:07Doctor Kale said.
- 14:12Diet should also be monitored.
- 14:15Staying well hydrated can help keep fluids circulating.
- 14:18Maintaining the proper balance of salt and water is important and reducing salt intake and
- 14:23following a low sodium diet may be advisable for some patients.
- 14:27The expert noted, lifestyle changes such as movement can also be incorporated to help
- 14:33alleviate the swelling.
- 14:36Stretch both arms up, move your hands and wrists, and make fist several times.
- 14:42Said Doctor Beth Ollar, a family physician with Rooks County Health Center in Stockton, Kansas.
- 14:48Running your hands under cool water may help.
- 14:51She said, and taking off jewelry or constricting clothes while exercising can also
- 14:56help prevent swelling due to temperature changes.
- 15:00Staying well hydrated can also help keep fluids circulating.
- 15:04Doctor Ollar added.
- 15:06Pregnancy is another common cause of swelling.
- 15:10In that case, doctors Mueller said that moving the extremities can help.
- 15:15If swelling and pregnancy occurs suddenly or is extreme, Talk to your physician immediately as
- 15:21this can be a symptom of elevated blood pressure.
- 15:24She also said.
- 15:26As we conclude today's episode, let's talk about an important topic that affects many, but
- 15:31is often overlooked.
- 15:32Men's mental health.
- 15:35June is men's mental health awareness month, and it is a crucial time to shed light on the
- 15:40mental well-being of men and boys.
- 15:43The Centers for Disease Control And Prevention CDC has highlighted some alarming statistics
- 15:49that underscore the need for greater awareness and support.
- 15:54According to the CDC in 2020, women were diagnosed with depression twice as much as men.
- 16:01However, men died by suicide at a rate 4 times higher than women.
- 16:06This stark contrast points to a significant issue.
- 16:10Men are less likely to seek help or express their emotions, often due to societal
- 16:16expectations and stigmas.
- 16:18Phrases like don't cry, be a man, and man up are commonly heard, but they can have a
- 16:25detrimental impact on men's mental health by discouraging emotional expression.
- 16:32Christie Edwards, the CEO of Centers and a therapist, emphasizes the importance of
- 16:36recognizing that emotions are normal and that expressing them is healthy.
- 16:40She points out that many men have been raised to suppress their feelings Chi-Ming that doing
- 16:45so makes them more manly.
- 16:47This mindset can prevent men from seeking the help they need exacerbating mental health
- 16:52issues.
- 16:54The statistics are indeed concerning.
- 16:56From 2000 to 2021, the suicide rate among men increased by 29%.
- 17:02Men account for 79% of all suicides amounting to roughly 105 men dying by suicide every day.
- 17:11Edwards explains that the methods men use to attempt suicide are often more lethal, such as
- 17:16firearms, and that men may act more impulsively especially under the influence of alcohol.
- 17:23In the Permian Basin, Edwards has observed a significant number of men seeking help.
- 17:28From June 1st, 2023 to June 1st, 2024, 679 men visited centers for therapy, with 3 166 being
- 17:38under 181313 being over 18.
- 17:43She notes that having an outlet to talk about their feelings can be incredibly relieving for
- 17:47these men.
- 17:49Many report that acknowledging and expressing their emotions has made them better individuals
- 17:54and improved their relationships with their families.
- 17:58Men's mental health awareness month is an opportunity to break the stigma surrounding
- 18:03men's mental health and encouragement to seek help.
- 18:08If you or someone you know is struggling with mental health issues, it is vital to reach out
- 18:13to a trusted friend family member, or healthcare professional.
- 18:17Remember, expressing emotions is a sign of strength, not weakness, and seeking help can
- 18:24lead to a healthier, more fulfilling life.
- 18:27That wraps up today's episode of Morning Medical with Doctor Chow.
- 18:32Thank you for joining us, and I hope you found today's topics insightful and informative.
- 18:38Remember to take care of your health both physical and mental, and we will see you next
- 18:43time.
- 18:43Have a wonderful day.
