- 0:04>> Dr. Terry Simpson: In the topic about heart disease, you're going to
- 0:06hear a lot about cholesterol. And it
- 0:09gets confusing, because you're going to hear about good cholesterol,
- 0:12bad cholesterol, LDL, hidl,
- 0:15apob, small size, large
- 0:17size, what does it all mean?
- 0:20You're going to hear people state that LDL isn't important, but
- 0:23that the size of particle or the oxidation is important.
- 0:26And while all of this gets obscure,
- 0:29today we're going to make sense of the madness
- 0:32and show you how size isn't
- 0:35important.
- 0:38I am, your chief medical explanationist, doctor Terry Simpson,
- 0:41and this is fork U forked
- 0:44university, where we make sense of the madness. Busting
- 0:47a few myths and teach you a little bit about food
- 0:49is not always medicine.
- 0:55Maybe you heard about LDL being the bad
- 0:58cholesterol and HDl being the good cholesterol. And we
- 1:01have to check your HDl triglyceride level
- 1:04on and on and on. It really gets
- 1:07confusing. Stripping away the good and bad
- 1:10labels, we know that cholesterol, first of all, doesn't just
- 1:13float around in your blood. Cholesterol is oil based
- 1:16or waxy. And since your blood is water
- 1:18based, we have to carry the cholesterol in
- 1:21something. And so the cholesterol is placed in these
- 1:24little packages of protein, and
- 1:27those proteins are classified by their weight in a
- 1:29centrifuge. And over time, we found that these
- 1:32low density lipoproteins, that those
- 1:35packages are responsible for
- 1:38leaving deposits in the walls of your
- 1:41arteries. And those deposits are called
- 1:43atherosclerosis. And if you get a bunch of them together,
- 1:46it's called a plaque. And those
- 1:48plaques, when they rupture, lead to a heart attack
- 1:51or to a stroke, or if they impinge on the
- 1:54artery, meaning they start decreasing the flow of the
- 1:57artery to the heart, they lead to heart ischemia
- 2:00or angina. So
- 2:03LDL isn't a one size fits all villain.
- 2:06All the LDL particles vary in size, density,
- 2:09oxidation, et cetera. So we're going to sort
- 2:11of strip away one of the more common myths that I
- 2:14heard, that you have to worry about
- 2:17LDL, particle size. As if you couldn't hear
- 2:20enough. We're going to strip that away because you don't
- 2:23need to care about the size anymore. Size of the
- 2:26LDL particle isn't important. And you don't have to
- 2:29remember these old terms, like pattern a, large fluffy
- 2:32particles are good. Pattern B, small dense
- 2:35particles, are bad. Because the recent
- 2:37research has shifted focus from the LDL particle
- 2:40size to one of the lipoproteins
- 2:43that surrounds the cholesterol, called apolipoprotein B,
- 2:46which we will call ApoB.
- 2:49Think of it this way. If you want to
- 2:52not have messy tea, you
- 2:55put your loose tea in something
- 2:58like a teabag. Think about the
- 3:00teabag as being partially held
- 3:03together by one of these apob molecules,
- 3:06right? So every single teabag, or
- 3:09LDL particle carries one of
- 3:12these little proteins, that Apop protein. Think of the tea
- 3:15as the cholesterol. But that protein is a
- 3:18passport that allows that
- 3:20teabag to enter into the cell
- 3:23wall of the artery,
- 3:26not just passively, but it's actively
- 3:29transported in. If there's an LDL particle, it's actively
- 3:31transported into this cell wall.
- 3:34And if there are too many LDL, particles in
- 3:37that cell to be metabolized by that
- 3:39cell, it tosses the LDL particles
- 3:42out the backside of the cell into the wall of the
- 3:45artery between the first and second layer.
- 3:49That's where they build up, that's where they oxidize,
- 3:52and that's where they can ultimately get so large
- 3:55that they impinge on the artery wall, leading
- 3:58to less blood flow to, like, the heart
- 4:01artery, or less blood flow to the brain,
- 4:03which can cause ischemia, a heart attack, or stroke.
- 4:07The key is how many apob
- 4:10particles there are. Now, we used
- 4:13to think that that was the answer. The size
- 4:16of the particle. We used to think small particles.
- 4:19Wow. They're the bad ones, because when we looked at the research,
- 4:22we saw small particles had more atherosclerosis. Is this
- 4:25confusing you? But think about this. The more small
- 4:28particles you have, you're gonna have more
- 4:30apob, because if you bake a lot of little tea
- 4:33bags, you gotta sew it together with that apolipoprotein
- 4:35b, and that's a problem.
- 4:38The particle number gives us a count of
- 4:41how many of these atherogenic particles there are.
- 4:44We call them atherogenic because the ApOB
- 4:46particle is the thing, the particle that
- 4:49leads to atherosclerosis. Doesn't matter
- 4:52how large, how much tea is in the
- 4:55teabag, how much cholesterol
- 4:58is in the LDL particle. What matters
- 5:01is how many of them are circulating in your blood.
- 5:04Now, I've, heard people say, well,
- 5:07cholesterol's important. It is. Cholesterol's
- 5:10used in every cell. It is. Cholesterol's used to
- 5:13make steroid hormones, like testosterone, estrogen.
- 5:16It is. Cholesterol's important to your brain. It
- 5:19is. But too much cholesterol, like too much
- 5:21water or too much salt, or too much blood
- 5:24sugar, is bad. Too much
- 5:26cholesterol leads to atherosclerosis.
- 5:30Some people say God doesn't make mistakes. Well, I don't know if God
- 5:33made a mistake or not. We're not going to live forever. But if
- 5:36you want to live longer, you want to have less
- 5:39atherosclerosis. There have been
- 5:42a number of articles that have shown this, that it is
- 5:45the number of apob particles,
- 5:47nothing, not the size of
- 5:50the molecule. And there's two ways of looking at this.
- 5:53If you have a bunch of little tiny molecules of ldl, which
- 5:56have small particles, and they get in and they're tossed out
- 5:59to the back, well, it's going to take a lot of those
- 6:02to build one large cholesterol particle which is
- 6:04equally actively transported in. So
- 6:07you have a lot of large, fluffy particles, you'll have a
- 6:10faster buildup. Now, what are things that increase
- 6:13apob in your body? One of them
- 6:16is sugar. That's why
- 6:19diabetics need to be under better control, because
- 6:21more glucose in your blood, the more spikes you have, the more
- 6:24insulin resistance you have, the more diabetes you have, you're making
- 6:27more APOB particles. The more apob particles, the more
- 6:30atherosclerosis, the higher your risk of heart disease.
- 6:33That's all. That's as simple as that.
- 6:36So here's the key. This month,
- 6:39when you go to your doctor, APOB is the
- 6:42test you want to ask for. They'll give you a standard
- 6:45cholesterol panel. It'll have your hdl, your
- 6:47ldl, your total cholesterol, your
- 6:50triglycerides. Ask to get an
- 6:53APOb test. And while you're at it, also ask to get a
- 6:56PSA test, because this month, this month,
- 6:59we want you to do cancer screening. So, American
- 7:02Cancer Society, what's my number? Screen me,
- 7:04etcetera. So there's been lots of
- 7:07meta analysis of participants showing how APOB is a stronger
- 7:10predictor of heart events than cart particle
- 7:13size. And I won't bore you with it, but if you want to find out more of
- 7:16the detail and more of those for the nerdy bunches of
- 7:19you, and most of you are nerds, go to the blog associated with
- 7:21this@yourdoctorsorders.com or four
- 7:24q.com.
- 7:26now, how can diet and lifestyle affect the
- 7:29apob number? We already talked about refined grains,
- 7:31but saturated fat like red
- 7:34meat increases apob
- 7:37particles. Lean meat,
- 7:40plant based fats like olive oil and
- 7:42avocado don't. They might actually
- 7:45decrease APOb number. So changing your diet from
- 7:48saturated fats to less saturated fats when you get
- 7:51below 9%, we see that change.
- 7:53I know the low carb people never like this because
- 7:56they want to wave off cholesterol, because every
- 7:59article out there shows that low carb and keto diets lead to
- 8:02higher ldl numbers,
- 8:05increasing the fiber in your diet. Soluble fiber, like
- 8:07foods found in oats, beans, and fruits,
- 8:10reduce the number of ldl and apob particles in your
- 8:13bloodstream. How do they do that? Because here's
- 8:16what happens when your liver
- 8:19makes ldl. Because this cholesterol is made and you're packaged
- 8:22in your liver, what happens is it doesn't
- 8:24just throw it into your bloodstream. It makes
- 8:27all these ldl particles and puts it into your bile.
- 8:30Your bile is secreted by your liver and goes into your
- 8:33intestine, and from your intestine, it goes into your
- 8:36bloodstream. Now, when you eat more
- 8:39fiber, the fiber binds
- 8:42to those LDL particles, and those fibers
- 8:45that bind to the LDL particles
- 8:47passively go out of your backside
- 8:50into the toilet. Right.
- 8:53Diets that are high in processed carbs and sugar raise apob
- 8:56levels. And so those diets also have less
- 8:58fiber. So increasing the fiber in your diet not only makes
- 9:01you feel full longer, it decreases your
- 9:04apob particles. I'll give you another
- 9:07example. Whole grains, brown
- 9:09rice, quinoa,
- 9:12oats, corn,
- 9:14all have fiber that
- 9:17decrease apob. Refined grains don't have
- 9:20fiber. Lead to sugar spikes lead to more
- 9:23manufacturing of apob. That's why refined
- 9:26grains aren't good. That's why whole grains are.
- 9:29Okay? Okay, so to wrap
- 9:32this up, while we used to think the size of the
- 9:35LDL particle was important, it really is apob.
- 9:38Ask your doctor for an apob. That's how we
- 9:41get a better picture of your heart. What should your levels be? In the
- 9:43United States, we want your apob levels to be under 100.
- 9:47In Europe, we want them under 70. We'll probably go to under
- 9:4970. Why do we keep changing this every
- 9:52year? Because we have discovered over time that the lower
- 9:55your total cholesterol number is, the healthier
- 9:58you are. Now, there are m those people who say, well, you know,
- 10:01long live people have higher ldls.
- 10:04Okay? If you get to be old, you can have a
- 10:06higher ldl. But remember, there are many other things that
- 10:09influence this. Meaning this. I'll
- 10:12give you one example. There's a genetic
- 10:15predisposition to people called the PCSK nine
- 10:18transport system. And if you have a genetic
- 10:21defect in that, you can have high ldl,
- 10:24but you also have high ldl
- 10:26receptors. Because you have high ldl receptors, you
- 10:29don't have that much ldl in your body. It's cleared very
- 10:32quickly. And those people live
- 10:34long. Remember, longer lived people,
- 10:37still die of heart disease, still die of cancer, still
- 10:40have diabetes, still have cognitive decline. They
- 10:43just put it off for a while.
- 10:45So get your ldl down. Is there a
- 10:48lower level? Well, we know below. I think it's 53. You're probably
- 10:51not going to have heart disease. What do kids
- 10:54need? What do you need to maintain the amount of
- 10:57testosterone and keep your brain well? Remember your lDl.
- 11:00Your brain makes its own cholesterol. It doesn't rely on your
- 11:03liver. Your gonads
- 11:05make your own testosterone and estrogen. They don't rely on your
- 11:08liver. So don't listen to people who say, that's
- 11:11all I terribly important because too much cholesterol, like
- 11:14too much salt, like too much blood sugar is
- 11:17deadly.
- 11:17All right, go to my blog, yourdoctorsorders.com, or
- 11:20fork u.com, to see all
- 11:23of the references and you can look them up for yourself. This
- 11:26episode was written and researched by me, doctor Terry Simpson.
- 11:29And while I am a doctor, I'm not your doctor. Please
- 11:32see your doctor, a western trained, non
- 11:34chiropractic, non eastern trained
- 11:37physician. And if you're curious about, what
- 11:40your LDl APOB number is, please see your regular doctor for
- 11:43that. Podcast is distributed by our friends
- 11:45at Simpler Media than the pod God, Mister Evo
- 11:48Terra. Our production is done by producer Girl Productions, who
- 11:51insist on my hair being perfect even though we're not
- 11:54doing video today.
- 11:57Until next time, stay curious. And
- 12:00remember, food can be a powerful influence on your
- 12:03health.
- 12:10This month is cancer screening month for
- 12:12prostate cancer. See, we want you to know your PSA
- 12:15number, just like I know my APOb number, which is 63.
- 12:19My PSA number is three. We want it under four.
- 12:22So I hope this month you go to the American
- 12:24Cancer Society, get screened, find out your
- 12:27psa number. Because after all, min 50
- 12:30and over, we need to know the number. And I hope
- 12:33yours is less than four. And be sure and check
- 12:36out your apopee number. Also.
- 12:40>> Speaker B: 2.97, I'm happy to report. And
- 12:43yes, that is checked on a regular and frequent basis with
- 12:46me. And thanks for the information on the
- 12:48APOB. I'm never going to drink tea in a
- 12:50teabag again.
