- 0:00This is Laura Diredo with the Becker's Healthcare
- 0:02podcast. I'm thrilled today to be joined by
- 0:04Callie Miller, hospital chief executive at Presbyterian Santa
- 0:07Fe Medical Center. Callie, it's a pleasure to
- 0:09have you on the podcast today.
- 0:11Yes. Thanks for having me, Laura. Happy to
- 0:13be here. Absolutely. Now I'm looking forward to
- 0:15our conversation and just learning a little bit
- 0:17more about everything you're doing at Presbyterian Santa
- 0:20Fe Medical Center because I know it's such
- 0:21a fascinating time in health care. Certainly,
- 0:24a lot a lot to be thinking about
- 0:26and sharing across the industry, and so I'm
- 0:28looking forward to that. But before we dig
- 0:30in, can you tell me a little bit
- 0:31more about yourself and the hospital?
- 0:34Yeah. Sure. Absolutely. I'm Kelly Miller. I'm the
- 0:36hospital CEO at Presbyterian Santa Fe Medical Center
- 0:39in Santa Fe, New Mexico.
- 0:41Presbyterian is a large integrated health care system
- 0:44in New Mexico, so we have both a
- 0:46traditional delivery system of hospitals, clinics, ambulance service,
- 0:51plus we have our own health plan. So
- 0:52we serve one in three New Mexicans through
- 0:54care delivery or health coverage in the state,
- 0:56which is really unique.
- 0:58Like I said, I'm at our Santa Fe
- 0:59hospital, which is an acute care facility within
- 1:02our Northern New Mexico operations. I've been here
- 1:04now.
- 1:05I'm going on three years in Santa Fe
- 1:07as a whole, going on almost two years
- 1:09in this particular role. So
- 1:12That's fantastic. And, you know, I think especially
- 1:14when you look at, everything that's going on
- 1:17in health care, it's been an impactful,
- 1:18two years that you've been at the helm.
- 1:21And so can you tell us a little
- 1:23bit more about some of the trends that
- 1:24you're watching currently?
- 1:26Yeah. Yeah. I think you you hit on
- 1:28it. It's an it's an interesting time in
- 1:29health care, so certainly a lot of trends
- 1:31to be keeping up with. I would say,
- 1:34one of the top trends I'm always watching
- 1:36is workforce engagement. You know, engagement amongst our
- 1:40health care teams is probably more critical than
- 1:42it has ever been.
- 1:43I think we all know, but, you know,
- 1:45an engaged team really drives quality improvement, of
- 1:47course, drives retention and decreased turnover.
- 1:51So, within workforce engagement too, I'm specifically watching
- 1:54manager development and capability,
- 1:57especially around our frontline managers. You know, we're
- 1:59seeing that
- 2:00this is usually their first management role, and
- 2:02so their onboarding and then their ongoing development
- 2:05and growth is is really critical to our
- 2:07success as a facility.
- 2:09You know, when your frontline managers are successful,
- 2:11that really drives improvement and engagement for the
- 2:14whole facility.
- 2:15I think, you know, the next trend most
- 2:17people are watching regardless of the industry they're
- 2:19in is, of course, AI.
- 2:21You know, in health care, we're really looking
- 2:23closely at how it's reducing the burden on
- 2:25our physicians and APCs, and
- 2:28it's really amazing that five, six years ago,
- 2:30the Ambient Learning was not as robust as
- 2:33it is now. So we're seeing wins with
- 2:35the Ambient AI. Our medical group has rolled
- 2:37out to our physicians and APCs here, and,
- 2:40you know, I'm hearing directly from physicians that
- 2:42has made a significant difference for them in
- 2:44their practice, and that's huge.
- 2:46I think third trend is, of course, the
- 2:48constantly changing financial landscape. You know, as a
- 2:50health care executive, you really have to stay
- 2:52up to date and informed in this area
- 2:54at all times.
- 2:56Watching a lot of the changes at the
- 2:58federal and state level and how that impacts
- 3:00facilities like Santa Fe Medical Center. It's important
- 3:03that we
- 3:04really can be financially sustainable without compromising
- 3:07on our mission or our impact in the
- 3:09community.
- 3:10I love that. I think all of those
- 3:12trends are so important and very much top
- 3:14of mind for many leaders across the country.
- 3:16And I think, especially when you look at
- 3:18workforce engagement,
- 3:20in how valuable
- 3:21every person is on the team and in
- 3:23care delivery or administrative side.
- 3:26And from your perspective, what are some of
- 3:28the things that you're doing really to, improve
- 3:31retention and just, keep that workforce engaged, especially
- 3:34with some of the uncertainties going through the
- 3:36industry right now?
- 3:37I think, yeah, keeping, the workforce engaged has
- 3:40been, you know, critical for us here,
- 3:44and it's probably one of the things I'm
- 3:45most proud of. You know, I'm really proud
- 3:47of the culture we're building here at Santa
- 3:49Fe Medical Center. So we're actually the newest
- 3:51hospital within Presbyterian.
- 3:53We opened in 2018 and then, of course,
- 3:56shortly after that had a pandemic in 2020.
- 3:58So,
- 3:59you know, coming out of that pandemic and
- 4:01trying to stabilize, we've really spent the last
- 4:03few years laying the groundwork for the hospital
- 4:06we wanna be and the culture we wanna
- 4:07have. And so,
- 4:08it takes time to build that culture. It
- 4:10really doesn't happen overnight. So I'm proud to
- 4:12say that we've we've been doing that.
- 4:15We've seen our engagement scores for all staff
- 4:18and then our position in APC group,
- 4:20increase based on the year on the surveys
- 4:22we had in 2025,
- 4:24you know, which is great, and it takes
- 4:26a lot of effort to do that. So,
- 4:28it's important to me that staff feel and
- 4:30know they can escalate ideas, concerns, or anything
- 4:33else, and we're really seeing that in our
- 4:35engagement score. So I'm I'm proud of that.
- 4:38That's great to hear. It's really impressive to
- 4:40have that type of engagement and,
- 4:43really involvement by the staff and how the
- 4:45health of the organization is going. And to
- 4:48your other point too in looking at technology,
- 4:50AI, and,
- 4:52you know, really leveraging that to its fullest
- 4:54abilities, I I think things are moving so
- 4:55quickly with technology,
- 4:57and there are a lot of,
- 5:00changes happening with AI in particular when you
- 5:02think about, all the different opportunities out there,
- 5:05but then, you know, it it can also
- 5:07bring some additional risks or challenges. And so
- 5:10when you look at AI, from your organizational
- 5:13standpoint, what are the conversations like with your
- 5:15leadership team, and how are you really keeping
- 5:17things focused and resources focused in the areas
- 5:20that make the most sense, through this digital
- 5:22transformation process?
- 5:24Yeah. I think it's a great question. I
- 5:26mean, I think, you know, really,
- 5:28where we've seen Presbyterian
- 5:29as a system really invest is, like I
- 5:31mentioned in our, position at APC groups and
- 5:34really trying to decrease the burden on them,
- 5:37the the documentation burden, which we all know
- 5:39is there. And so,
- 5:41you know, to be able to keep and
- 5:42retain your physicians and APCs, taking some of
- 5:45that burden off of them has been critical.
- 5:48So,
- 5:49you know, again, proud to say that Presbyterian
- 5:51as a whole has really rolled that out
- 5:52to our physicians and APCs, and we're seeing
- 5:55that momentum there. I think next steps are,
- 5:57you know, organizational wide. There's,
- 6:00you know, opportunities to look at, you know,
- 6:02other areas where we can have wins like
- 6:04that, things that we can automate, things that
- 6:06are administrative in nature that we can, take
- 6:08off people's plates so that we can really
- 6:10focus on quality and the patient experience. So
- 6:13excited to see where that kinda takes us
- 6:15in the future.
- 6:17I love it. I I think that's so
- 6:19helpful,
- 6:19to really understand where your head's at and
- 6:21and to where are some of the things
- 6:23that are the biggest opportunities for you moving
- 6:25forward. And, you know, speaking of that, what
- 6:27are you most focused on and excited about?
- 6:29And and can you tell us a little
- 6:30bit as well about something you're proud of
- 6:32from the last six to eighteen months or
- 6:34so?
- 6:35Yeah. It's a great question. You know, like
- 6:37I mentioned, really proud of the culture we're
- 6:39building here at Santa Fe Medical Center.
- 6:41You know, we've, like I said, built built
- 6:43this culture slowly year over year, and so,
- 6:47you know, that's that's our team coming together
- 6:49to really do that. And so, like I
- 6:50mentioned, we're seeing that in our engagement scores.
- 6:53So we saw an increase in our engagement
- 6:55scores for all staff and then physician and
- 6:57EPCs
- 6:58over 2024 performance. So we saw that in
- 7:002025. Of course, we will survey everybody again
- 7:03here in the fall, and so, again, expecting
- 7:05to,
- 7:06really continue to increase engagement. So something I'm
- 7:09really proud of there. What we're gonna be
- 7:11focused on and excited about in 2026
- 7:13is, you know, some of our key quality
- 7:15initiatives.
- 7:16Were really laying the groundwork as a system,
- 7:18for our journey to high reliability, which is
- 7:20great.
- 7:21And then, Santa Fe Medical Center really saw
- 7:23some record growth for key services last year,
- 7:26and the plan is to continue to grow
- 7:28those services as well as grow the overall
- 7:30acuity of the facility.
- 7:32And that's gonna allow us to expand the
- 7:33patients we're able to care for in Santa
- 7:35Fe. So I'm really excited about that.
- 7:38That's cool to hear. And, I I think
- 7:40it's so critical to have that, clinical team
- 7:43in place that really is able to serve
- 7:45the community and adapt to whatever the community
- 7:47needs are, at any time. And, you know,
- 7:49that journey to high reliability is not an
- 7:51easy one. I know it takes a lot
- 7:53of planning and time and then, you know,
- 7:55effort from the team in order to get
- 7:57there.
- 7:58So, as you're going through this journey, has
- 8:00there been anything that,
- 8:02has surprised you so far about it, or
- 8:03or where do you see some of the
- 8:04spaces where your team is just most excited
- 8:06to dig in?
- 8:08I I don't think there's any,
- 8:10anything that's come out, you know,
- 8:13so far. We're really kind of at the
- 8:14beginning of this journey, and so, I know,
- 8:18really this entire week,
- 8:20we have, partners here with Crestani really helping
- 8:23us on our high reliability journey. So right
- 8:26now, we're really in kind of a fact
- 8:27finding mode and and kind of in that,
- 8:29you know, talking to frontline staff. So really
- 8:32excited about this week. They'll be at Santa
- 8:33Fe on, Friday. And so,
- 8:36that's really a chance for our frontline staff
- 8:38to, you know, talk about quality, talk about
- 8:40their processes, talk about things that maybe they
- 8:43see as an opportunity. So, you know, just
- 8:45grateful that the staff is really plugging into
- 8:47that work and that they're excited about it.
- 8:49So it's yeah. Excited about that journey this
- 8:51year.
- 8:52Absolutely. That sounds great. And definitely, we'll be
- 8:55looking forward to learning more as you get
- 8:57further along within that journey. Now before we
- 8:59wrap up here, I wanted to talk about
- 9:01leadership as well. What advice would you give
- 9:03to evolving leaders, especially those who are trying
- 9:06to,
- 9:07just build their career in emerging in the
- 9:09space?
- 9:11I think, two pieces of advice that I
- 9:13would give. I think first, you know, be
- 9:15open to opportunities. I think career opportunities such
- 9:18as promotions or the chance to, you know,
- 9:20be over a new service line or book
- 9:22a business
- 9:23sometimes come when you least expect it, and
- 9:25those can be the best opportunities to really
- 9:27grow and learn. So,
- 9:29just embrace it. And then I think second
- 9:31piece of advice is
- 9:32really take the time to invest in physician
- 9:34relationships.
- 9:36You know, as a health care executive, you
- 9:38really cannot be successful without the trust and
- 9:41buy in from your physician. So it's important
- 9:43to invest early on and really continuously build
- 9:46those relationships with them. So those are those
- 9:48are my two pieces of advice.
- 9:51I love it. Kelly, thank you so much
- 9:52for joining us on the podcast today. This
- 9:54has been such an insightful conversation,
- 9:56inspiring to hear about what you've been up
- 9:58to, and looking forward to continuing this conversation
- 10:01in the future.
- 10:02Absolutely. Thanks for having me, Laura.
