Transcript
- 0:00You ever walk out of your neurologist's office feeling
- 0:02like you nailed it? You sat there nodding, taking it
- 0:05all in, and then 5 minutes later, you're in the parking lot
- 0:07thinking, wait, what just happened in there?
- 0:10Didn't even ask the one question I needed answered.
- 0:13Yeah, I've been there. I did that for three years.
- 0:16And you know what? It cost me hundreds of hours of
- 0:19feeling good that I didn't have to lose.
- 0:21So today we're going to fix that.
- 0:23We're talking about the five medication questions that could
- 0:26totally change your Parkinson's treatment.
- 0:28Because when you ask the right questions, you get better
- 0:31results, better understanding, and better days and stick around
- 0:35till the end. Because I've got 2 braces in
- 0:37this one. One's funny with some science
- 0:39behind it, and the other, well, let's just say it hits you in
- 0:44the heart. Welcome to Living with
- 0:47Parkinson's, the good, the bad, and the reality.
- 0:51Real talk, real life, real community with Bryce Perry.
- 0:56Let's begin. Well, welcome back friends.
- 1:00If you're new here, my name is Bryce and this is Living with
- 1:03Parkinson's, The Good, the bad, and the Reality.
- 1:05Now this podcast is all about bringing strength, hope, and
- 1:08community to people living with Parkinson's and their care
- 1:11partners with a mix of real talk, science made simple, and
- 1:15let's be honest, hopefully a few laughs along the way.
- 1:18So if that sounds like something you need, hit that subscribe
- 1:21button and join us. Now before we jump into the
- 1:24questions, let's talk about what what's actually happening inside
- 1:28your body. OK?
- 1:30Levodopa is basically your brains stand in for dopamine.
- 1:35It's the lead singer of the Parkinson's band.
- 1:38The problem is it's it's half life is about 90 minutes,
- 1:42meaning it burns through your system faster than your first
- 1:45cup of coffee on a Monday morning.
- 1:48Your brain converts it into dopamine, but only after it gets
- 1:52through your stomach, your intestines, your blood, and
- 1:55finally crosses that cross crosses the blood brain barrier,
- 1:59which is like airport security for your neurons.
- 2:02And if your stomach's busy digesting breakfast or if you
- 2:05got protein and iron cutting in line, your poor levodopa stuck
- 2:09waiting with his shoes off and a belt in a tray.
- 2:13All right, let's get into the questions that actually matter,
- 2:16the ones your doctors wishes you'd ask.
- 2:19Here's question #1 how do I know if my dose is still right or if
- 2:25my brain is just changing? OK, when I was first diagnosed,
- 2:29I thought, OK, they'll give me a dose and that's my dose forever.
- 2:34Spoiler alert, you know this. Nope.
- 2:38For years I thought my meds had stopped working.
- 2:41My stiffness came back earlier in the day.
- 2:43My tremor crept in mid afternoon and I thought, well, there it
- 2:46is. I'm it's, I'm just getting
- 2:48worse. It's Parkinson's.
- 2:51But I wasn't getting worse. My brain was changing.
- 2:55So here's the science part. As Parkinson's progresses, your
- 2:59brain stores less dopamine. Imagine like a sponge, a sponge
- 3:04that a sponge that used to hold a cup of water, but now it only
- 3:07holds like 1/2 a cup of water. The medicine's the same, the
- 3:12sponge just can't soak up as much.
- 3:15So what used to last five hours now maybe only last three.
- 3:20Now that's not failure, just chemistry.
- 3:23And once I learned that, my doctor and I adjusted the
- 3:25timing. Not more pills, just smaller,
- 3:28smaller spacing. It's like tuning a guitar.
- 3:31Same strings, just a tighter rhythm.
- 3:35And that made all the difference.
- 3:38So if you've been thinking that your meds don't work anymore,
- 3:41don't panic. Ask your doctor.
- 3:43This. Has my brain changed or does my
- 3:46schedule need to? That question can save you a ton
- 3:51of frustration. OK, question #2 could any of my
- 3:56other meds or supplements be messing with my Parkinson's
- 4:00meds? OK, now this one's a game
- 4:02changer, and it's one that hardly anyone asks now.
- 4:07I used to take my levodopa, my iron supplement, and my morning
- 4:11protein shake all at once. Basically, I was throwing AI was
- 4:15like throwing a block party in my stomach and then wondering
- 4:17why nobody was getting through the door.
- 4:20I thought just maybe the bouncers were keeping them out.
- 4:21But here's what's happening. Levodopa has to hitch a ride on
- 4:27the same transport truck as amino acids and the and and iron
- 4:31to get absorbed. So if your breakfast is full of
- 4:35protein or you're taking iron, it's like 15 trucks trying to
- 4:39merge into a one lane highway. Dopamine sitting there in
- 4:42traffic honking like let me, let me go, let me in.
- 4:45Even caffeine can play games with dopamine, with metabolism.
- 4:49It speeds it up, which is why you might burn through your meds
- 4:52faster on those high energy days and high coffee days.
- 4:57And certain antidepressants and blood pressure meds can
- 5:00interfere too. So best advice, just bring your
- 5:04full list of prescriptions, supplements, even over the
- 5:07counter stuff to your neurologist.
- 5:09Don't guess because sometimes it's not that your levodopa is
- 5:13broken, it's just being bullied by your vitamin shelf.
- 5:18And I know I remember this other time.
- 5:20Cough medicine, I don't know if you know this or not, but like
- 5:24there's different anyway. There's different types of
- 5:26things that you need to to to kind of regulate with levodopa.
- 5:30You can take with a lot of things, but there are things
- 5:32that mixing up the timing of it with the other meds can be
- 5:36concerning, not concerning, but just make your levodopa not work
- 5:39as well. So what I do is, you know, if
- 5:42I'm got cough medicine or I've been going through that or if
- 5:45my, my general doctor, my family doctor has prescribed me
- 5:49something, I make sure I take it with me to my appointments every
- 5:52time just so he, my my neurologist can see it, He can
- 5:55go through it, look at it and he can kind of advise me if there's
- 5:58any concerns that or if concerns on timing or anything like that.
- 6:02So that's a big tip. That's a power tip.
- 6:05All right. And if you have ever, ever,
- 6:08ever, ever left a doctor's office and thought, what an
- 6:13idiot, I totally forgot that maybe you're not as hard on
- 6:15yourself as I am. But what a what a silly, what a
- 6:17silly. But you know what?
- 6:19I just can't believe I didn't ask that question.
- 6:21Me too in the comments, please. Yes, you know me.
- 6:24It's all about me making myself feel not so bad.
- 6:27Anyway, let's move on to question #3 what can I do when
- 6:31my meds start wearing off early or are taking longer to kick in?
- 6:36Now this one hit me hard. For years, I'd feel my meds
- 6:39wearing off and I'd immediately pop another pill like I was
- 6:42rebooting a frozen laptop. I didn't even realize there was
- 6:45a better way, you know? And I told you a lot about this
- 6:48in the last episode, but here's the deal.
- 6:50When you start noticing the off periods where the symptoms creep
- 6:53back in before your next dose, and that's your cue to talk to
- 6:57your doctor, it might mean adding adding an inhibitator or
- 7:01enhancer or just in timing or splitting doses.
- 7:05Now here's some science. Wearing off happens because your
- 7:10body clears levodopa faster as dopamine buffering decreases.
- 7:16Think of it like a bathtub that used to hold hot hold water for
- 7:19hours, but now drains faster. You're refilling it more often,
- 7:24not because it's you're broken, it's just because the plumbing's
- 7:27changing. Now the delayed on the delayed
- 7:32on issue, you know, that's usually digestion.
- 7:35If your stomach's busy or you've eaten, eaten a big meal, your
- 7:39pills just sitting there thinking anytime now.
- 7:42And on those days, I'll wait longer after eating or take my
- 7:45meds with fizzy water. Carbonation helps move it along
- 7:48faster or break it up, bunch it up, smash it up.
- 7:51It gets in there quicker. And then there's Carmen.
- 7:54She's turned this whole process into an Olympic sport of
- 7:57coordination. She's got alarms on her phone,
- 8:01backup alarms on mine. And if she's out, I'll get texts
- 8:05all the time saying take your, take your pills now, take your
- 8:07pills now. It's teamwork.
- 8:10And Speaking of that, here's something Carmen wanted me to
- 8:13read to you as part of Carmen's Care Partner Corner.
- 8:18You know, I asked Carmen what she wanted other care partners
- 8:20to know about helping with medications, and she actually
- 8:23wrote this down for me to share with you.
- 8:26When it comes to helping Bryce with his meds, I've learned it's
- 8:30not about being in control, it's about being In Sync.
- 8:33Early on, I thought I could just remind him here and there, but
- 8:38Parkinson's doesn't care about reminders.
- 8:40It's all about rhythm. So now I've got alarms on my
- 8:44phone too when his goes off. Mine does too.
- 8:48And if I'm not at home, I'll send him a quick text.
- 8:51Hey, meds, meds, honey, meds, honey.
- 8:54Sometimes he already has. Sometimes he's about to and
- 8:58sometimes, well, let's just say Parkinson's brain had other
- 9:02plans. The point is, we tag team it.
- 9:06It's not about babysitting. It's about backing each other
- 9:09up. And here's the other part,
- 9:12Grace, so much grace. There are days when the timings
- 9:16off or you forgot or I forget, or we both think the other ones
- 9:21got it covered. You can't get mad at that.
- 9:24It's not, it's, it's not forgetfulness, it's Parkinson's.
- 9:28So if you're a care partner, my advice is this, don't just set
- 9:31reminders, build systems, don't just manage symptoms, build
- 9:36patients and celebrate the winds.
- 9:40If you get, if you both get through a full day of meds on
- 9:43time, that's a high 5 moment. Awesome.
- 9:48Thank you, Carmen. That's Carmen.
- 9:50Practical and funny and somehow always right.
- 9:56All right, here's question #4 how will I know if what I'm
- 10:00feeling is dyskinesia and what can we do about it?
- 10:04Yes, the bonus dance feature. You know, the first time it
- 10:09happened, I thought, well, look at me, I'm getting my cardio in
- 10:12without even leaving the chair. That joke wore off really quick.
- 10:17Here's the science. Dyskinesia isn't your medicine
- 10:20breaking you, it's your brain getting too much dopamine at
- 10:23once. It's like turn, it's like
- 10:25turning up the volume to high. The song is still good, but now
- 10:29the speakers are shaking and distorted.
- 10:32This is where your doctor can smooth it out by splitting
- 10:35doses, adding slower release options, or adjusting timing.
- 10:40That's really important and actually leads me into what I,
- 10:42you know, I have a prices right here.
- 10:44So this is your first prices. You get a bonus 1 today.
- 10:46This is an early one. There's another one coming
- 10:48later, but here it is. Levodopa doesn't cause
- 10:50Parkinson's to progress, but the fear of levodopa can cause your
- 10:55life to stop progressing. That's a shot out to someone
- 11:00who's delaying their medication starting.
- 11:04OK. And that like that one hits me
- 11:06personally, I was scared of dyskinesia.
- 11:10Like when I, when I, when I heard about it, I was like, Oh
- 11:13my gosh, but I'd already started my meds.
- 11:15So I'm like, oh man, what did I do here?
- 11:17Did I start too soon? Did I, did I not start too soon?
- 11:20Oh man, I was freaking out. I was like, what's happening?
- 11:23And when I finally realized that taking my my meds early and
- 11:31right off the bat, I was going to save myself wasting time,
- 11:35wasting good years waiting to feel bad enough to start my
- 11:39meds. So like in everything else, it's
- 11:43time to be proactive, not reactive.
- 11:45Don't wait until you feel you're bad enough to take levodopa at
- 11:51all. So ask your ask your doctor now,
- 11:54not later. How do we know when to adjust
- 11:57for dyskinesia before it gets worse?
- 12:00And I promise you, you'll be glad you did.
- 12:02I'm not even going to answer that.
- 12:03I'm going to let that sit there for your doctor, OK?
- 12:07And question #5 what should I be watching for?
- 12:11That tells us it's time to adjust my whole plan, not just
- 12:15my meds. Now, this one's the most
- 12:19important and the easiest to miss.
- 12:23There was a time when I thought I had everything figured out.
- 12:26Perfect Med schedule, diet, exercise.
- 12:30I was like the model patient. And then I started feeling
- 12:33garbage. Turns out I wasn't sleeping, I
- 12:37was dehydrated, and my stress was through the roof.
- 12:40My meds weren't failing. My maintenance plan was.
- 12:45Here's the thing most of us don't realize, dopamine isn't
- 12:50just movement, it's mood, motivation, and even digestion.
- 12:54And when those systems get off, everything feels off.
- 12:59So if you're on periods are shorter or you're feeling more
- 13:03anxious or fatigued, don't just ask about the pills, ask what
- 13:09else could be throwing my dopamine balance off.
- 13:12Your doctor might look at a sleep hygiene, hydration, gut
- 13:16health, you know, even cortisone levels.
- 13:19It's like fine tuning a whole orchestra now, not just the
- 13:22violin. And for those of you managing
- 13:25the solo, I see you. You're juggling meds.
- 13:29You're juggling meals, life and probably a dozen alarms.
- 13:33And you're still here and you're still showing up.
- 13:36And that takes grit. So here's your challenge this
- 13:41week. At your next appointment, take
- 13:43one of these questions. Just one and ask it, write it
- 13:48down and say, I heard this on a video, on a path, on a podcast
- 13:52and I really want to understand it better.
- 13:55That's one question. That one question could
- 13:57completely train change your treatment journey.
- 14:01So which one of these five questions are you going to ask
- 14:04at your next appointment? Drop number one to five in the
- 14:07comments. And care partners, what's your
- 14:10best system for managing medication chaos?
- 14:15And here we go, you made it. You did it again.
- 14:18Here is your final braces for the day with Parkinson's.
- 14:22Your doctor has half the picture.
- 14:25You hold another half. The goal is to start coloring it
- 14:29in together because Parkinson's might be unpredictable.
- 14:35But how we manage it doesn't have to be all right.
- 14:40You know the drill. Like the video?
- 14:43Share it with somebody you think might need this.
- 14:46Thank you for everything you're doing.
- 14:49And if you're new here and you made it this far, you might as
- 14:51well subscribe. You're already one of the crew,
- 14:54all right? And until next time, keep
- 14:55showing up, keep being real, keep laughing, and you keep
- 14:59doing life today. Parkinson's may be part of your
- 15:05life, but it doesn't get to define it.
