- 0:00You know that feeling when you think you've finally nailed your
- 0:03medication schedule only to realize that Parkinson's had
- 0:07other plans? Yeah, I've been there.
- 0:09And what no one told me early on is that levodopa isn't a long
- 0:13term roommate. It's more like a short term
- 0:15guest. It comes in, it does the job,
- 0:18and then it leaves before dinner.
- 0:20So today I'm going to share the five biggest medication timing
- 0:23mistakes that I've made and what I do now to make sure my meds
- 0:28actually work for me instead of against me.
- 0:31And stick around till the end. Because once you understand how
- 0:34to keep this short term guest happy, your whole day will start
- 0:38to feel oh so much smoother. Welcome to Living with
- 0:42Parkinson's, The Good, the Bad, and the Reality.
- 0:46Real talk, real life, real community with Bryce Perry.
- 0:51Let's begin. Hey everyone, welcome back to
- 0:56the channel. I'm Bryce and I was diagnosed
- 0:58with Parkinson's at the age of 40 and I'm still doing life
- 1:02today, 15 years later. So if you're new here, this
- 1:05channel is all about the good, the bad and the reality of
- 1:07Parkinson's, hopefully with a little bit of humor and a lot of
- 1:10heart. So hit that subscribe button and
- 1:12let's start by looking at what really goes on inside your body
- 1:16every time you take that little yellow pill.
- 1:20And if you have the Parkinson's or love someone that does, you
- 1:23know what I mean by the little yellow pill?
- 1:26Anyway, here's the science. Here's the deal.
- 1:29When you take levodopa, your body starts converting it into
- 1:32dopamine, right? But that process is fast.
- 1:36Within about 90 minutes, half of it's already gone.
- 1:39And by the 3 1/2 to 4 hour mark, you're usually running on empty.
- 1:43Think of it like your phone battery.
- 1:45No matter how good your charger is, by mid afternoon you're back
- 1:49in low power mode. Now if you add an inhibitor to
- 1:53it like Primiprixal, make that battery last a bit longer.
- 1:56But most of us are living on short charge cycles, which means
- 2:00your timing isn't about guessing, it's about charging
- 2:04before it dies. OK now, so now that you know how
- 2:09short that battery life really is, let's talk about what drains
- 2:12it faster, what slows it down, and how you can find your sweet
- 2:16spot. OK, so if you've ever felt your
- 2:19meds running out of steam before your next dose, hit that like
- 2:22button and know that you're definitely not alone.
- 2:26So let's talk about the first mistake.
- 2:29And I call this one chasing the dose.
- 2:31OK, chasing the dose. This one got me for years.
- 2:35You know, you feel your meds wearing off.
- 2:36You know, maybe the stiffness, fatigue, or the mental fog or
- 2:40your symptoms, whatever the symptom is starts to come back
- 2:42and you grab your next pill early, totally by instinct,
- 2:45almost right? The problem is your body hasn't
- 2:47processed the last dose yet. So when you start to stack your
- 2:50pills, you're flooding your system, which leads to spikes,
- 2:54crashes, and actually irritability.
- 2:57So here's what I finally figured out.
- 2:59Instead of reacting, I started tracking patterns.
- 3:02I realized my off times hit at almost the same point every day.
- 3:07So now I take my, my next full dose about 20 minutes before
- 3:11that crash, not early out of panic, but early on purpose.
- 3:15And that's the difference. Not early out of panic, but
- 3:18early out of purpose. OK, That's why you track your
- 3:22Mets. Now, when I say about stacking
- 3:23them, you're going to find yourself in trouble.
- 3:25It's when you're kind of like popping one in between and then
- 3:28keeping that regular cadence going, right?
- 3:30So you're kind of overdoing it. So instead of overdoing it, find
- 3:34that rhythm and adjust. And of course, when I give you
- 3:38when I give these types of tips, always check with your doctor
- 3:411st and your health, you know, whoever would be your
- 3:43neurologist or your movement disorder specialist and, and
- 3:46make sure that you kind of go with it with them.
- 3:48OK, Now do it a little bit different today.
- 3:51Carmen's corner right now coming right up, Carmen's care partner
- 3:53corner, she is going to make a comment about all these
- 3:56different tips. OK.
- 3:58And so here's what Carmen has to say about care partners in this.
- 4:02She said that she used to call those those panic pills, and now
- 4:07she says you're finally driving the bus instead of running
- 4:10behind it. And it's true.
- 4:12Once I stopped chasing and started anticipating, my whole
- 4:15day smoothed out. Now see, when you stack your
- 4:19doses, dopamine spikes too quickly and the brain's
- 4:22receptors, they they down regulate, meaning you get a
- 4:25shorter relief the next time. But when you pace it evenly,
- 4:29your brain finds the rhythm. It's not magic, it's chemistry,
- 4:34and if you're solo, you live in the solo.
- 4:35Don't have a care partner. If you live alone, use your
- 4:38phone and note exactly when your meds were off for a few days and
- 4:41when you see the pattern, that's when you try shifting your next
- 4:44dose just 15 to 20 minutes earlier.
- 4:47It's a game changer, I'm telling you.
- 4:49How about this comment #chaser? If you've ever found yourself
- 4:52topping up too early, because we've all been there.
- 4:56There's no shame in that. Trust me.
- 4:58When you're experimenting, it happens a lot.
- 5:00OK, let's move on to mistake #2 I call this one burning through
- 5:03your dopamine too fast. So if you ever had one of those
- 5:07days where you're on fire, right?
- 5:08You're working in the yard, you're cleaning the house,
- 5:10you're fixing everything, right? That, that everything that's
- 5:13been sitting there for months and then you crash like a brick
- 5:16by mid afternoon. Been there too.
- 5:19I've been there many times. So what's happening is you're
- 5:21burning through dopamine faster. Physical activity, stress,
- 5:25caffeine, even excitement, they all hit the gas pedal on your
- 5:29dopamine usage. So here's the science behind
- 5:33that. Your brain uses dopamine to
- 5:35coordinate movement. So the more you move, the faster
- 5:39you drain your supply. Now you add coffee or tea or and
- 5:43then you accelerate the absorption absorption, which you
- 5:46also then shorten the effect. Same with splitting or crushing
- 5:50pills. They kick in faster but they
- 5:51fade sooner. Even fizzy water can speed up
- 5:55the process because it dissolves the pill, the pill faster in
- 5:59your stomach. So again, think about what I
- 6:01just said there, caffeine, fizzy water breaking up the pill.
- 6:04So if you need to get some dopamine into your system
- 6:06quicker, they all help you get it in quicker.
- 6:09But again, there's no, that little yellow pill doesn't know
- 6:13that's like, hey, I'm going in a little bit soon, so I'm going to
- 6:15last longer. No, it still lasts what it's
- 6:17going to last. So the quicker it gets absorbed
- 6:19in your system, the quicker you're going to be out of it.
- 6:23So you have to remember all that stuff when you're taking your
- 6:25meds. So here's what I do now if I
- 6:28know I've got a busy day, right, like say like home repairs or
- 6:31travel, I shorten my window. And I used to start doing that.
- 6:34I, when I started my medication, I was going in 4 hour doses,
- 6:39right? Take my meds every four hours.
- 6:41Well, when I had those busy days or I was really heavy travel
- 6:44day, I would cut it down about 3 1/2 hours and sometimes three,
- 6:48depends on how much I was going to do.
- 6:50And and I hydrate like it's my job.
- 6:53So really, really, really experiment and figure out what's
- 6:56going on and drink lots of water.
- 6:58And what is Carmen, the care partner corner expert have to
- 7:01say about this? Well, she jokes that she can
- 7:04tell when I've gone full handyman mode, right?
- 7:07She'll say like, you're burning hot today, better top up the
- 7:10tank. And she's usually right.
- 7:13And if you live alone, plan your doses around your activity days.
- 7:17You can't always avoid burning through dopamine, but you can
- 7:20time your pills. You can time your refills
- 7:23better. OK, so how about this drop out
- 7:27an emoji wrench? If that sounds familiar, or if
- 7:30caffeine has ever betrayed you before lunch.
- 7:33You know, that's one thing. That's just caffeine.
- 7:34I love, I love my coffee, but I just have to be careful and
- 7:36watch how I'm doing. Now, be careful.
- 7:38Sorry. Let me back up.
- 7:39I'm going to say that I want to say that, right?
- 7:40I want you to understand what I'm saying.
- 7:41I don't have to be careful, right?
- 7:43I listen. Parkinson's makes you do a lot
- 7:46of things. It makes you study.
- 7:47You do a lot of things. It makes you go tracking things
- 7:50and everything else. So I the last thing I'm going to
- 7:52do is try to figure out, oh, I can't have a coffee now because
- 7:54I have meds coming up. No, that's why I don't plan
- 7:57myself around. I just plan it around, OK?
- 8:00And I understand when I understand what I'm doing and
- 8:03how much I'm going to use and how it affects me and then I
- 8:06adjust accordingly. OK.
- 8:09Again, all with the under the guise of my doctor having been
- 8:12having the ability to experiment with that.
- 8:15OK, how about this mistake #3 and I call this one slowing your
- 8:19system down. Now let's flip it.
- 8:21Just like some things make your meds hit harder and faster,
- 8:24other things put the brakes on it, and not in a good way.
- 8:29Big, heavy meals, especially with protein or fat, they slow
- 8:33down your stomach. Dehydration, same thing.
- 8:36Your meds take the sites, the scenic route instead of the
- 8:40express lane. OK, let's talk about
- 8:43Constipation. Nobody loves it, but it matters,
- 8:46you know, if your gut isn't if your gut isn't moving, your meds
- 8:49aren't either. It's like sending a delivery
- 8:51truck into rush hour traffic. It's plugged jammed.
- 8:56Anyway, here's that. This is a see, levodopa is
- 8:58absorbed in your small intestine.
- 9:00If your food or let's be honest, your plumbing is backed up, your
- 9:04medication literally can't get where it needs to get to on
- 9:07time. So here's what helps me.
- 9:10I never take my meds right after a big meal.
- 9:13I stay hydrated. I, I, I eat lighter during the
- 9:16day and I take my pills with water, sometimes even sparkling
- 9:20water if I want to get that faster kick in.
- 9:24And Carmen says that I that I finally drink like a responsible
- 9:28adult. And she's not wrong.
- 9:29I used to skip water and then wonder why my meds took forever
- 9:33to work again. The more hydrated you are, the
- 9:35better your meds are going to respond.
- 9:37So here's another tip. Keep that bottle of water beside
- 9:40your meds. It's simple, but it works.
- 9:42And if you struggle with Constipation, you need to talk
- 9:44to your doctor. Solving that can feel like
- 9:47upgrading your entire medication plan.
- 9:49It really can. And there is meds to help you.
- 9:51So how about this? Hit that, hit that like if
- 9:54you've ever had your meds take forever to kick in.
- 9:56And tell me in the comments what you've found help speed things
- 10:00up. I'd really like to know that as
- 10:02we move on to mistake #4 inconsistent timing.
- 10:06Parkinson's loves routine. I've told you this and your meds
- 10:09love it too. Missing a dose by even 20 or 30
- 10:11minutes doesn't seem like a big deal.
- 10:13Until it is. And that's when you get those
- 10:16weird midday dips, the unpredictable mood swings, and
- 10:20that awkward. Why am I off already feeling?
- 10:23You see when you delay doses, dopamine levels crash before the
- 10:28next dose hits. Those drops trigger stress
- 10:31hormones like cortisol and which which can make fatigue, anxiety
- 10:36and tremors way worse. So now I keep what I call a
- 10:40metromone timing like every 3 1/2 hours.
- 10:43Well it used to be every 3 1/2 hours.
- 10:45Now it's every two hours and 45 minutes give or take 10 minutes.
- 10:49I dose up and when I do my whole day staying steadier and when
- 10:53the closer I get to that exact time, the better it is.
- 10:59Okay. And from the Carmen Care partner
- 11:01corner, she says this. She says Bryce is more reliable
- 11:04when our oven timer is more reliable than our oven timer
- 11:07now. And that's a compliment, I
- 11:09think. I think that is, yeah.
- 11:11Anyway, she likes it. She likes the reliability,
- 11:13right, Because for her as a care partner, it's a heck of a lot
- 11:16easier to kind of help me right when I'm steady.
- 11:19You know how that goes. Think about, like the think
- 11:22about, you know, you know, if you're when your your children
- 11:24or your grandchildren and when, when they're up and down, up and
- 11:27down, like what they got sugar rush.
- 11:29It's like a sugar rush with kids.
- 11:30Like they go wild and you're like, Oh my God, just like take
- 11:33a pill, like chill out, you know, just for a little bit, you
- 11:36know, and then and then or if they're too down, right, you
- 11:39have to, you're like, oh, what's wrong?
- 11:40What's going on? So you're having to go over care
- 11:42for them, right, and try to figure out what's going on.
- 11:44That's like us, we're like little kids on sugar.
- 11:47Okay. So from a care partners
- 11:49perspective, the more level we are, the easier it is for them
- 11:52to help take care of us. And again, so I hope that was a
- 11:55compliment. Now here's a tip, if alarms
- 11:59drive you nuts, then label your bodies or your bodies label your
- 12:02bottles right with actual times to have like a little scratch
- 12:05pen with a piece of tape, you know, 6301011130.
- 12:08Whatever time you do, it's visual accountability.
- 12:11But I'm telling you that if you haven't heard of this tip
- 12:13before, and I'll tell you again, the pill cap timer, $6 at
- 12:19Amazon, it's a lifesaver. You get 2 in a pack.
- 12:22It's so cheap. Look in the description, you'll
- 12:24see, you'll see a link for it. The one that I use, it's like 6
- 12:27bucks. And I'm telling you, man, I use
- 12:29that thing like it's going out of style and I know exactly when
- 12:32I took my meds last. OK, comment, Metromome.
- 12:37If you've ever missed it, I'm going to tell you how to spell
- 12:39it. If you've ever missed a dose and
- 12:40you instantly regretted it, metromome.
- 12:43All right, let's move on to tip #5 this is huge.
- 12:46Ignoring gut health and hydration.
- 12:49Now, this one doesn't get talked about enough.
- 12:51Your gut is your first pharmacy. If it's not happy, your meds
- 12:56aren't going to be happy either. Again, Constipation,
- 12:59dehydration, or even certain supplements can all affect
- 13:03absorption. I used to think it didn't
- 13:05matter. Then I fixed it and my meds
- 13:08started kicking in faster and lasting longer and I'm like, my
- 13:11God, this works. So what's that science behind
- 13:14that? Well, hydration keeps your
- 13:16stomach empty normally, fiber keeps your intestines moving,
- 13:20and both make levodopa more predictable.
- 13:24The gut and the brain are in constant conversation, and
- 13:27Parkinson's makes that line glitchy.
- 13:30So supporting your gut is supporting your brain.
- 13:35And Carmen says she can tell. When I haven't had enough water
- 13:38because my meds take forever to kick in and work, I get cranky.
- 13:43OK, fair enough. Here's a tip.
- 13:45Add electrolytes or lemon to your water to make it easier to
- 13:48drink. A hydrated brain is a happy
- 13:51brain, and a happy brain use dopamine better.
- 13:55All right, if you needed this reminder, go grab a glass of
- 13:58water right now. Seriously.
- 14:00I'll wait. I'm just kidding.
- 14:04I don't know what I'm doing. OK, So what actually works?
- 14:07Well, here's my rhythm today. OK, I usually take my first dose
- 14:11about 6:30 AM. That's usually what time I get
- 14:13out. And then every two hours and 45
- 14:15minutes, like clockwork, I take my next dose.
- 14:19I shift a bit based on my day, shorter if I'm active, right,
- 14:23normal if I'm if I'm normal, right?
- 14:25And I keep an eye on what speed things up and what slows them
- 14:28down. It's not perfect, but it is
- 14:31actually predictable. Now here's some more science for
- 14:33you. You have to come to realize
- 14:35this, that levodopa, it's it doesn't have a long life, OK?
- 14:41And that's why consistency wins, because steady timing keeps your
- 14:45brain from swinging between feast and famine.
- 14:49Think of it like keeping your phone on a on a slow, constant
- 14:53charge instead of letting it die and sprinting for the charger.
- 14:58Wouldn't that be would not be a change, right?
- 15:01I mean, I'm the worst at that. I am so bad at keep my my phone
- 15:05charged. It's crazy.
- 15:06But one more thing. I want you to realize that
- 15:10Levodopa's peak time after you take it is around 90 minutes,
- 15:14OK, to two hours. That is like the middle, that's
- 15:17the middle. So when you start realizing
- 15:19that, that two hours after you, about two hours and 15 minutes
- 15:23after you take that levodopa, it's already starting to
- 15:25dissipate on your system. You know the these are averages,
- 15:30but the average is for assumption after you take it 20
- 15:34to 30 minutes before it kicks in because it's starting to, it has
- 15:37to get up and going, right? It has to get in there and get
- 15:39in there. OK.
- 15:40And then you realize that an hour later it's already starting
- 15:43to decline down. That's why I, I urge you that
- 15:48you know, if you're, you know, in this journey and you're,
- 15:51you're not taking them, you're not taking your meds, your
- 15:53levodopa based on a time period. If it's just like, hey, one in
- 15:56the morning, 1:00 at night or 1:00 in the afternoon, I'd urge
- 16:01you to talk to your doctor about getting on a regular rhythm
- 16:04because that's where you want to keep yourself.
- 16:06And again, it's better for your brain, it's better for your
- 16:08body. Like I just told you to keep
- 16:09that rhythm. OK, Wow, you're getting
- 16:14technical today, aren't we? Well, if you made it this far, I
- 16:17do have today's Bryces for you. And, and here you go.
- 16:20And I hope you like this one. And this is it with Parkinson's,
- 16:24Your meds are like coffee. It's not about how strong it is.
- 16:28It's about when you drink it. So there you go.
- 16:34Those are the five medication timing mistakes that that can
- 16:37make your Parkinson's worse and more importantly, how to fix
- 16:40them. And remember, Parkinson's might
- 16:42be unpredictable, but your patterns aren't.
- 16:46Learn them, work with them, and life starts to feel a little
- 16:51smoother there. There you go.
- 16:53So hit subscribe. Hey, if you're if you're still
- 16:54watching, you haven't subscribed, you might as well,
