- 0:00Hello, everyone. This is Jacob Emerson with the
- 0:02Becker's Pay Your Issues podcast. Thrilled today to
- 0:04be joined by Jake Pascaglio, who is the
- 0:07president of commercial business at Harvard Pilgrim Health
- 0:10Care. Jake, thank you so much for taking
- 0:11the time to be with me on the
- 0:13podcast today.
- 0:14Great to be with you, Jacob. Thank you.
- 0:16Yeah. Absolutely. And before we dive into everything
- 0:18we wanna talk with you about, can you
- 0:19tell us a little bit more about yourself,
- 0:21your background in health care, and what it
- 0:23is that you do today at Harvard Pilgrim?
- 0:26Certainly.
- 0:27Again, I'm Jacob Viscaglio. Currently serve as a
- 0:29president of commercial markets and specialty products for
- 0:33Harvard Pilgrim Health Care, which is a subsidiary
- 0:37company of point thirty two Health. We're based
- 0:40in Massachusetts
- 0:41and serve
- 0:42New England markets,
- 0:44Massachusetts,
- 0:45Maine, New Hampshire, and Rhode Island.
- 0:48And through that and in my role, I'm
- 0:50responsible for sales, sales operations, product strategy innovation,
- 0:54strategic business development, and the overall financial performance
- 0:57of the commercial and specialty businesses.
- 1:01I've been in health care the majority of
- 1:03my career
- 1:04with
- 1:05other organizations
- 1:06like Cigna, Anthem,
- 1:08Blue Cross Blue Shield of Massachusetts. Been with
- 1:11Irving Pilgrim Health Care for about seven months
- 1:14now.
- 1:15And,
- 1:16I also have a stint in financial services
- 1:19at Prudential Financial, and it's great to be
- 1:21back in health care.
- 1:23Fantastic. Well, we're glad to have you with
- 1:24us. A long career in the industry,
- 1:27Jake.
- 1:27And we're and we're here specifically to talk
- 1:29with you a little bit about Harvard Pilgrim
- 1:31Health Care expanding its partnership with Progyny Health
- 1:35to support your members for a full year
- 1:37after NICU or special care nursery discharge. So
- 1:41let's dive into that a little bit because,
- 1:43as you know, neonatal care, it's getting increasingly
- 1:46complex
- 1:47as the year goes on. Coverage of these
- 1:49services is increasingly getting more complex. And and
- 1:52the challenges,
- 1:53that these families face after NICU discharge,
- 1:56are daunting. So
- 1:57how do you see programs like this, between
- 2:00Harvard Pilgrim and Progyny Health shaping the future
- 2:03of post acute care management and improving those
- 2:06long term outcomes for the families that you
- 2:08serve?
- 2:09It is complicated as you mentioned, and I
- 2:12think you touched on a really important aspect
- 2:14of this, which is the family aspect of
- 2:17care. So certainly,
- 2:18the
- 2:19the child, the newborn that's in receiving care
- 2:22is the one who's receiving care, but really
- 2:24it's impacting the family
- 2:26across the board. So it's not only about
- 2:29caring for the the infant,
- 2:31but it's also really taking care of the
- 2:34the family,
- 2:35at large.
- 2:36And I'll I'll share a little bit of
- 2:38the perspective in terms of why we think
- 2:40this expansion is so important, but I'll also
- 2:43give my own, family story because it's, something
- 2:46we've we experienced now. Yeah. It's been a
- 2:48while since my children are older, but, firsthand
- 2:51experience
- 2:51with,
- 2:52twins,
- 2:53that were
- 2:54premature
- 2:55and, spent some time in the NICU. So
- 2:57for me and my family, it's very personal.
- 3:00So what we're doing here is we're expanding
- 3:02a program that we started,
- 3:04with our great partner, Progyny.
- 3:06Initially, we're sixty days after
- 3:09birth,
- 3:10to care for the newborn,
- 3:12in the family.
- 3:13I should say newborns or or more plural
- 3:16there.
- 3:17And,
- 3:18really expanding that to the full year.
- 3:21And just a little bit about the program
- 3:23around Progyny. So their team includes board certified
- 3:27physicians, nurses, social workers,
- 3:29and to date, they've worked with more than
- 3:312,500
- 3:32of our members.
- 3:34And I think as most know,
- 3:36there are specific life challenges that families face
- 3:39following a NICU stay.
- 3:41In addition to assistance with discharge planning, education,
- 3:45screening for postpartum depression,
- 3:47the Progyny Health team and their case managers
- 3:50connect families to important resources they may
- 3:53need,
- 3:54things like transportation, healthy food, financial support for
- 3:58utilities and rent, and even help members find
- 4:00access to free items they need
- 4:02for their babies like cribs, car seats, diapers,
- 4:04and baby formula. So certainly, the
- 4:08the care in the hospital and the coordination
- 4:10there,
- 4:11for the newborn or newborns, the the mom,
- 4:14the dad, etcetera,
- 4:15but also really beyond that and the impacts
- 4:18that,
- 4:20someone, a family that's going through,
- 4:23this is dealing within all aspects of what's
- 4:26needed
- 4:27to really
- 4:28launch,
- 4:29the the new family,
- 4:31on a path, is really important. So as
- 4:33I mentioned,
- 4:34my wife and I are fortunate to have
- 4:36a set of twins,
- 4:38that spent time in the NICU and,
- 4:41programs like this were not available to us
- 4:43at that time. Fortunately, we had a great
- 4:45support system of family members,
- 4:47friends, great doctors, nurses, etcetera. But as I
- 4:50reflect back on that time,
- 4:53we were just
- 4:54trying to survive day to day. We actually
- 4:56had, a 20 old at the time, so
- 4:59trying to juggle across all that,
- 5:01and care for our newborns,
- 5:04and just make sure we were on top
- 5:06of everything.
- 5:07Looking back, was a daunting challenge. So a
- 5:10program like this, would have been something that
- 5:12would have been very much appreciated,
- 5:14for for my family at the time. So
- 5:16that's why for me, it's personal and something
- 5:19that I'm really proud that our organization
- 5:21stands behind for our clients, for our members,
- 5:24and making this more broadly available.
- 5:28Certainly. So you really understand these issues firsthand,
- 5:31these issues that are also facing your members.
- 5:34In that vein, Jake, what are some of
- 5:36the broader implications of expanding this kind of
- 5:38care management from the sixty days to a
- 5:40full year for neonatal cases?
- 5:43And how do you think this shift, in
- 5:45care strategy is reflecting the industry evolving the
- 5:48industry's evolving approach towards more,
- 5:51member centered health plans?
- 5:54Yeah. Absolutely. It's, it's an important
- 5:57aspect of
- 5:59how we really want to surround our members
- 6:03with
- 6:03services that are going to be helpful
- 6:07over time.
- 6:08And it it just makes good sense to
- 6:10support the the families
- 6:13with a program like this.
- 6:15Additionally, as we know, the first year of
- 6:17an infant's life is crucial for their ongoing
- 6:20health. No surprise there.
- 6:22Then a NICU stay can further compound some
- 6:24of the challenges that families face during
- 6:27that baby's first year.
- 6:29One of the things we saw in the
- 6:31data is,
- 6:32more than ninety percent of our Harvard Pilgrim
- 6:35members who participate in the program have reported
- 6:37high satisfaction.
- 6:39We saw the opportunity to do even more
- 6:41for this population.
- 6:43And I'd love to share an example of
- 6:44one of our members from New Hampshire,
- 6:47was a mother of premature twins born at
- 6:4933,
- 6:51and
- 6:52the daughters were in the NICU. They had
- 6:54a toddler at home, sounds familiar,
- 6:57and the the mom had just started a
- 6:58new job and had no paid maternity time
- 7:00off. So certainly a challenge there just to
- 7:03start.
- 7:04Add a complication
- 7:05of
- 7:06her grandmother passing away and that she had
- 7:08contracted COVID and couldn't visit the twins in
- 7:10a hospital for an extended period of time.
- 7:13Progyny stepped in with gift cards to help
- 7:16with travel expenses, connected the mom with resources,
- 7:19helped manage the cost of formula to help
- 7:21her fortify her breast milk due to the
- 7:24twins' prematurity.
- 7:25The Progyny team also worked closely with the
- 7:27hospital to clarify the twins milestones and provided
- 7:30reassurances
- 7:31to the mom and her husband when needed.
- 7:34So the members are not just getting help
- 7:36and support,
- 7:37but advocacy.
- 7:39And by expanding the program, we can assure
- 7:41ensure that happens. Right? There is not a
- 7:43timeframe of sixty days or ninety days.
- 7:46Sometimes that's sufficient, but sometimes,
- 7:49that first year is longer. So for us,
- 7:51this isn't really a shift in strategy,
- 7:53but rather it's an expansion of a program
- 7:55that was
- 7:56critically needed by this subset of our membership.
- 8:00And for us, it was an easy decision
- 8:01to expand the partnership to help our vulnerable
- 8:04members and their families.
- 8:07Absolutely. Well, that's a great real world example.
- 8:09So appreciate you sharing that with us and
- 8:10and some amazing satisfaction rates that you're you're
- 8:13seeing through this.
- 8:16Jake, we we hear from health plans,
- 8:18all over the country about beyond Harvard Pilgrim
- 8:20about,
- 8:21NICU admissions and preterm births remaining a significant
- 8:25concern for for their poll how they how
- 8:27they cover things. And so I wonder if
- 8:30you have advice for,
- 8:32our our listeners in terms of, how to
- 8:34integrate more comprehensive
- 8:36maternity and infant care management
- 8:38into into their health plans.
- 8:41Yeah. Absolutely. Let me start maybe with some
- 8:43of the the data
- 8:45in terms of what's happening industry wide around
- 8:48the, you know, the specific need around
- 8:51NICU admissions and the this topic that we've
- 8:54been talking about. So the stats paint a
- 8:57a pretty dire picture. In The United States,
- 8:59we have one of the highest maternal mortality
- 9:01rates in developed world and the rates are
- 9:04on the rise.
- 9:05According to the March of Dimes twenty twenty
- 9:07four report card, preterm birth rate in The
- 9:09US remains historically high at ten point four
- 9:12percent
- 9:13and NICU births require specialized attention to ensure
- 9:16the best outcomes during an infant's first year.
- 9:19And even a short stay in the NICU
- 9:21can delay family bonding, impact breastfeeding adoption.
- 9:24And once an infant is discharged from the
- 9:26hospital, the risk of a readmission back to
- 9:29the hospital is significant without case management support
- 9:31for the infant and family. Additionally, perinatal depression,
- 9:36which is a condition that can have devastating
- 9:38consequences, affects up to one in seven women
- 9:41in The US. And some studies suggest that
- 9:43this number can be even higher.
- 9:45So we've seen the first hand of positive
- 9:47results
- 9:48of this type of program with Progyny Health,
- 9:51and the impact it has on babies and
- 9:53the families, broadly.
- 9:55So as it relates to other,
- 9:58insurance
- 9:59leaders that are thinking about these programs, I
- 10:01really think about it in a couple of,
- 10:04important manners. One is really understanding
- 10:07the needs of
- 10:08the
- 10:09memberships
- 10:10that a membership that is covered,
- 10:13by the plans
- 10:14and really understanding
- 10:16with those needs, what's the best way to
- 10:19deliver it,
- 10:20that care, that coaching, that program, whatever the
- 10:22case may be,
- 10:24in determining
- 10:25what is the best approach to enable
- 10:28that program
- 10:29or the that those care pathways, etcetera.
- 10:32And as we think about this, across all
- 10:34aspects of our business, we really think about,
- 10:37is this something that
- 10:39we can do ourselves?
- 10:41Is there a way to do it through
- 10:43a partner relationship that has that specialized
- 10:47expertise?
- 10:48That's so critical.
- 10:49And,
- 10:50certainly with
- 10:52NICU,
- 10:53we saw the opportunity to
- 10:55work
- 10:56alongside Progyny Health, which is, you know, very
- 10:59focused across,
- 11:00this dimension of enabling
- 11:03support and care,
- 11:04for the most vulnerable members. And, again, very,
- 11:07very pleased with the
- 11:09results that we've delivered as our our members,
- 11:12which is most important to us.
- 11:14Certainly. And it's such a good call out
- 11:16and a reminder for our listeners in terms
- 11:18of the maternal and infant,
- 11:20issues and care that that the country still
- 11:22faces.
- 11:24Before we go, Jake, what else are we
- 11:25missing? Any final thoughts that you wanna share
- 11:28with our listeners while you have the ears
- 11:30of a lot of health plan leaders,
- 11:32from all over the country right now? Yeah.
- 11:34I appreciate the opportunity.
- 11:36Yeah. So first and foremost,
- 11:38Progyny Health, as I mentioned, a significant impact
- 11:41positively on our members, and we're very proud
- 11:43of this collaboration.
- 11:45For us, this collaboration is not just a
- 11:47one off program at Harvard Pilgrim Health Care.
- 11:50We are always looking to sort support our
- 11:52members through their entire health care journey.
- 11:56Related,
- 11:57obviously, is women's health, which is a large
- 11:59area of focus for us. We've
- 12:02we had a number of partnerships with,
- 12:04organizations like Maven and Innovia Health that really
- 12:08helps us to provide women with 24 access
- 12:11to a variety of specialists,
- 12:13personalized resources,
- 12:14coaches,
- 12:16lactation experts, behavioral health experts, etcetera, etcetera,
- 12:19that really help us provide much needed information,
- 12:23access to providers, to the right providers,
- 12:26and care across the board,
- 12:29for
- 12:30our members,
- 12:32who are
- 12:33navigating
- 12:34challenging life transitions
- 12:36really end to end.
- 12:37Additionally, something that we're really proud of that
- 12:39connects in well here is
- 12:42a program that was launched, which we refer
- 12:45to as our behavioral health service navigation program.
- 12:48This program provides personalized one to one assistance
- 12:52that helps members find a behavioral health provider.
- 12:55And we know finding behavioral
- 12:57health providers, can be a challenge at times.
- 13:00Scheduling those appointments once you find
- 13:03that provider
- 13:04can be a challenge at times. So our
- 13:06navigators really take that burden off of the
- 13:09individual and work side by side with our
- 13:12members to
- 13:14enable
- 13:15awareness,
- 13:16access to the right
- 13:18provider, and
- 13:19then scheduling,
- 13:20to make sure that they can get what
- 13:22they need. And then we follow-up to make
- 13:24sure they're satisfied with their care, and then
- 13:26we take those learnings and continue to iterate
- 13:28and improve upon our programs.
- 13:30Why that's really important,
- 13:32as we're talking about the topic of,
- 13:35NICU care,
- 13:36for for the birth mom, for the children,
- 13:39etcetera, is the connection certainly between,
- 13:42behavioral health needs,
- 13:44in this context,
- 13:46making sure that we're able to provide that
- 13:49access that's so important
- 13:51to remove any barriers,
- 13:52for for the moms and all the members,
- 13:55quite frankly, who are looking to access vital
- 13:57mental
- 13:59health services when they need them.
- 14:02Last thing I'll add is,
- 14:04women's health month will end soon.
- 14:07The yet the focus on the maternal health
- 14:09crisis needs our year round attention and action.
- 14:11And it's really my hope that we can
- 14:13work together, broadly across the system
- 14:16of health care to ensure every woman has
- 14:18the opportunity
- 14:19to experience a healthy pregnancy,
- 14:21safe delivery, and a thriving happy life with
- 14:24their baby or babies, for that matter. It's
- 14:27a really important topic, and we're very focused
- 14:29as I know many of,
- 14:31the other insurance leaders are as well.
- 14:34Absolutely. Well, I I think that's a great
- 14:36place to leave things. So, Jake, I wanna
- 14:38thank you for taking the time to sit
- 14:40down with us and for sharing
- 14:42about the impactful work going on under your
- 14:44leadership at Harvard Pilgrim. We really appreciate it.
- 14:48Thank you, Jake. Appreciate the opportunity to join
- 14:50you today.
