- 0:51- Hi, everyone. This is Elizabeth Caslow with the
- 0:53- Becker's Payer Issues podcast.
- 0:55- I'm really excited today to be joined by
- 0:57- Margaret Anderson, president of Health Alliance Plan by
- 1:00- Henry Ford Health. Margaret, thank you so much
- 1:02- for taking the time. Thank you. Thank you
- 1:04- for having me. Of course. So before we
- 1:07- jump in, I would love to hear a
- 1:08- bit more about your career background in health
- 1:10- care and also your current role.
- 1:13- Sure. So I've been in health care for
- 1:14- the majority of my career,
- 1:16- both on the provider side and then in
- 1:18- this in the payer space most recently.
- 1:20- And, I would say very excited to be
- 1:22- here right now leading the health alliance plan.
- 1:25- We are a regional plan
- 1:27- owned by Henry Ford Health. We serve about
- 1:29- 510,000
- 1:31- members in Southeast Michigan, actually throughout the entire
- 1:34- Lower Peninsula.
- 1:35- Prior to joining here, I had roles at
- 1:36- Blue Cross Blue Shield of Western New York,
- 1:39- Blue Shield of California, and WellCare Health Plans.
- 1:42- All all different plans that I had the
- 1:44- opportunity
- 1:45- to lead, teams primarily in the sales and
- 1:48- account management area as well as marketing and
- 1:51- business planning and strategy.
- 1:53- So I know one motivator for this conversation
- 1:55- is some relatively recent news coming from the
- 1:57- plan,
- 1:58- the rebrand to Health Alliance Plan by Henry
- 2:00- Ford Health, making sure that Henry Ford Health
- 2:03- piece is right on there. So can you
- 2:05- walk me through what motivated that change, and,
- 2:08- why now? Why now do you think it's
- 2:10- important?
- 2:11- Yeah. We're at a really interesting time in
- 2:13- the health care sector. And when we looked
- 2:16- at HAP Health Alliance Plan, we were called
- 2:17- HAPT before, which which was what the acronym
- 2:20- that we used.
- 2:21- We really looked at about eighteen months worth
- 2:23- of data and research
- 2:25- to think through, like, we really wanna get
- 2:27- our constituents and our key stakeholders input. You
- 2:29- know, what really resonates
- 2:31- with people about our brand and how important
- 2:33- the brand is because it's not just the
- 2:35- name, it's really what we stand for and
- 2:37- how we think about being part of a
- 2:39- larger enterprise.
- 2:40- And so
- 2:41- Henry Ford Health has owned Health Alliance Plan
- 2:44- for forty years. We're celebrating our fortieth anniversary
- 2:46- of being one family in July.
- 2:48- And it's just been something that we've kinda
- 2:50- danced around and said, we really wanna say
- 2:53- put a stake in the ground and be
- 2:54- a part of Henry Ford Health. We wanna
- 2:56- be all thanks to everybody. We wanna think
- 2:58- about other providers that we do business within
- 3:00- Michigan that aren't part of the Henry Ford
- 3:02- Health family. Well, we really tested and asked
- 3:05- the question.
- 3:06- There was so much more alignment
- 3:08- and affinity where people really resonated with the
- 3:10- fact that we were part of something big.
- 3:12- And so it's that was part of the
- 3:14- motivation. The second piece is we really are
- 3:16- on a journey to affordability,
- 3:18- thinking through how we can make health care
- 3:20- more affordable together
- 3:21- when we provide care and coverage to our
- 3:23- members and patients.
- 3:25- So this journey,
- 3:26- the path to affordability really also looked at
- 3:29- how can we take waste out of the
- 3:30- system, what can we do to reduce costs,
- 3:33- and we thought to ourselves, you know what?
- 3:34- We can put a stake in the ground
- 3:35- and say we are part of something bigger.
- 3:37- And it also has helped us from a
- 3:39- national perspective just in terms of name recognition,
- 3:42- things along those lines. And Health Alliance plan
- 3:44- was in the top 10 for Medicare Advantage
- 3:46- growth in 2026.
- 3:49- And that's pretty big for a regional plan.
- 3:52- So we really thought that this was the
- 3:53- time was right for us to say let's
- 3:56- be part of a bigger system and be
- 3:58- part of Henry Ford Health.
- 4:00- So I I know, you know, regional plans,
- 4:02- a lot of them happen, seeing some growth.
- 4:03- So I just kind of wanna ask as
- 4:05- a follow-up, what are you excited for,
- 4:07- as a regional plan operating in this market
- 4:09- over the next few years?
- 4:12- We have so much positive momentum. I'm so
- 4:14- excited about, our leadership team and our ability
- 4:17- to be intentional about being in the community.
- 4:19- So being a regional plan, we really do
- 4:21- commit to making sure that we are focused
- 4:24- on the health and well-being of the communities
- 4:25- that we serve.
- 4:27- So this year in particular, every single 800
- 4:29- team members that we have here at the
- 4:31- plan,
- 4:31- have a goal to do two community events
- 4:33- that we are represented in here in Michigan.
- 4:36- And you really can see the Brandon Act
- 4:38- and seeing people connect with folks in the
- 4:41- communities, you know, volunteering at key events, volunteering
- 4:44- where police people really need our help.
- 4:46- So really that being out there and being
- 4:48- visible has been incredibly important for us. What's
- 4:51- exciting is that we actually are launching products
- 4:54- and programs that are impacting the cost of
- 4:56- health care, which is such a big topic
- 4:58- of discussion these days. And so we are
- 5:01- really committed to thinking through how can we
- 5:03- do things differently.
- 5:04- So one of the things that's super exciting
- 5:06- for me
- 5:07- is we launched a product last year called
- 5:09- Henry Ford Health Select.
- 5:10- And it's a high performing network that people
- 5:13- have access to. That's just the Henry Ford
- 5:15- Health Network.
- 5:16- It includes 13 hospitals, five fifty sites of
- 5:19- care, and throughout Southeast Michigan. And we really
- 5:22- saw a significant
- 5:24- uptick where people were excited about learning more
- 5:26- about this product. We've got over
- 5:28- 18,000
- 5:29- members
- 5:30- that are in a Henry Ford Health Select
- 5:32- product across Medicare,
- 5:34- small business. A lot of small business owners
- 5:36- are using that product as well as our
- 5:38- large group business. So it's exciting to introduce
- 5:40- new products and new services
- 5:42- to the community and to see how much
- 5:44- they're embracing it and looking for an alternative
- 5:46- and something that's different.
- 5:48- One thing that I've been noticing just in
- 5:50- the space more broadly,
- 5:52- with payviders and health systems that have health
- 5:54- plans is there's been some pullback, and then
- 5:57- there's also been some parties that are just
- 5:59- totally leaning into it right now. So,
- 6:01- could you walk me through what you're seeing
- 6:03- in that space,
- 6:05- and what's next for your level of integration?
- 6:07- I know you just touched on that network,
- 6:09- but what's next, when it comes to that
- 6:10- integration with Henry Ford Health more broadly?
- 6:13- Yeah. I think what's exciting is,
- 6:15- you know, it's been a journey. I've been
- 6:17- here for just over seven years, and we
- 6:19- certainly weren't always, as integrated and connected as
- 6:23- we are today. But it's something that we
- 6:24- committed to from our CEO, our CFO, and
- 6:27- and everyone here at the plan. And we
- 6:29- really you know what? If we are gonna
- 6:31- be relevant, if we're going to make an
- 6:33- impact, we need to lean into integration. So
- 6:36- our strategic plan actually has our number one
- 6:39- priority as we think enterprise first. So if
- 6:41- things are going to benefit the health care
- 6:43- ecosystem
- 6:44- and Henry Ford Health, we're gonna lean into
- 6:46- those priorities first. And we're really seeing that,
- 6:49- show up in many different ways, and it's
- 6:51- exciting to really see it in action.
- 6:54- The one thing I'll say is what's happening
- 6:55- across the industry,
- 6:57- A lot of, my colleagues that do run
- 6:59- provider sponsored plans, they might not have the
- 7:01- same sense of integration, and they don't have
- 7:04- the same alignment at the leadership level on
- 7:06- the executive teams. And you really need that
- 7:08- alignment and that shared vision. And once we
- 7:10- have that shared vision, it's much easier to
- 7:13- start articulating what we want our goals to
- 7:15- be. And so we've we've, really grown a
- 7:18- tremendous amount, and it's fun. Growth is exciting.
- 7:21- We just had a two day off, like,
- 7:23- leadership meeting,
- 7:25- and it's fun to talk about growth. It's
- 7:27- great to think about how we can do
- 7:28- things differently. And so,
- 7:30- really, really looking forward to leading the plan
- 7:32- as we continue to grow. We're marking toward,
- 7:35- 07/50000
- 7:36- members as our goal. And so having that
- 7:39- aligned goal with Henry Ford Health and with,
- 7:41- you know, them by our side, it's really
- 7:43- been exciting.
- 7:44- So with growth in this space, I know
- 7:46- one concern that some plans have is discipline
- 7:49- and making sure they're being able to meet
- 7:50- all of those different needs
- 7:52- as, the population gets bigger. So could you
- 7:55- talk about what's on your mind with regard
- 7:57- to that?
- 7:58- Yes. We are very committed to looking at
- 8:01- our administrative spend, and so I'm proud to
- 8:03- say that our administrative loss ratio was is
- 8:06- less than 10.5%,
- 8:08- the lowest it's been since 2013.
- 8:10- So I think it's really important as we
- 8:12- grow, we need to hold the tension around
- 8:14- capacity
- 8:15- and adding too much capacity to the plan.
- 8:18- And so we really are focused on
- 8:21- optimizing
- 8:22- technology where we can, on thinking through, you
- 8:24- know, what makes the most sense for our
- 8:27- members. One thing we did, and this might
- 8:29- sound counter, counter to what I just shared
- 8:31- on the admin side, is we've moved our
- 8:33- customer service centers back onshore.
- 8:36- And the reason for that was it was
- 8:37- actually costing us more money when you looked
- 8:39- at the complaints we were getting in, you
- 8:41- know, some potential compliance concerns,
- 8:44- and that's created a differentiator for us where
- 8:46- people can talk to,
- 8:48- somebody that actually lives in the state of
- 8:50- Michigan, can actually understand what their needs are
- 8:52- and and how they might be unique and
- 8:53- different. And so that's something that's important.
- 8:56- I would also say the one thing that's
- 8:58- that, certainly is concerning is the increase in
- 9:01- rise in gene therapies.
- 9:03- Concerning from a cost standpoint, it's wonderful, and
- 9:06- these are definitely therapies that we need our
- 9:08- patients and members to have access to, but
- 9:10- there isn't a mechanism
- 9:12- to fund these gene therapies.
- 9:14- And so you're talking anywhere from 3 to
- 9:16- $4,000,000
- 9:18- just for one person to be treated with
- 9:20- these therapies. And so thinking through
- 9:23- as we look at things nationally, should there
- 9:25- be risk pools to help offset those those
- 9:28- costs? What could we be doing differently as
- 9:30- an industry as we lean into the that
- 9:32- work? It's very important, critical,
- 9:34- but we also need to think about how
- 9:35- we fund it.
- 9:37- Would you mind going into a little bit
- 9:38- more detail? I'm just curious with the risk
- 9:40- pool idea there. I mean, I'm imagining it's
- 9:42- a pretty small population of people that would
- 9:45- need these therapies, but I'm, you know, I
- 9:47- don't write about this all every day, so
- 9:49- I'm I'm curious to hear what you think.
- 9:51- Yeah. Well, it's small until, I think there
- 9:53- are close to over there's over a thousand,
- 9:56- new gene therapies that are in the pipeline.
- 9:58- One of them is for diabetes. So So
- 10:00- when you think about how prevalent diabetes is
- 10:02- in this country,
- 10:03- and you think about the cost of that,
- 10:05- it's just going to be exorbitant. And so,
- 10:08- essentially, what happens is right now, we could
- 10:10- have a someone who's at an employer group,
- 10:13- and they need a gene therapy for muscular
- 10:15- dystrophy, for example.
- 10:16- So they might, you know, they come in,
- 10:18- they present, they meet all the criteria, we
- 10:20- go ahead, we approve it, we pay for
- 10:22- it, and then that employer could choose to
- 10:23- go to another health plan the following year.
- 10:25- So you're kind of sitting there, you know,
- 10:27- you've just paid for all this, but yet,
- 10:29- you know, there's there's no way we can
- 10:30- follow that patient. There's nothing that we can
- 10:32- do to really impact their care going forward.
- 10:34- So the thought process is that,
- 10:36- health plans would be
- 10:38- essentially,
- 10:39- responsible for I'll make up a number. The
- 10:41- first million dollars of care, and then there
- 10:43- would be a risk pool that would have
- 10:44- some type of reinsurance on top of it
- 10:46- that would that would fund the remaining cost
- 10:49- for that.
- 10:50- Got it. And then going back to bigger
- 10:52- picture here, I know you talked a little
- 10:53- bit about peers, with the health system on
- 10:56- plans before. What advice do you have for
- 10:59- players in that space right now?
- 11:01- Yeah. I I think you really need to
- 11:02- focus on making sure that you've got aligned
- 11:04- goals. There's a natural misalignment when you think
- 11:07- about providers and plans, And so you have
- 11:09- to think about what are some shared goals
- 11:11- that you can rally around. Things like Medicare
- 11:13- Advantage star rating. You know, that's something that
- 11:15- we can all lean in on together. People
- 11:17- get better outcomes, better access to quality care.
- 11:20- It helps the provider side. It helps the
- 11:22- plan side. So thinking about having shared goals,
- 11:25- I think is critical.
- 11:26- I also think it's really important to develop
- 11:28- relationships
- 11:29- with the provider between the provider and the
- 11:31- plan across the enterprise. It's so important, it's
- 11:34- so critical
- 11:35- to have those conversations,
- 11:36- to be able to solve problems together.
- 11:39- But you really think about innovation in a
- 11:40- different way.
- 11:42- I I just think it's critical. I mean,
- 11:43- you have to have that alignment in goals
- 11:45- and in strategy.
- 11:47- Anything else that you wanna share while I
- 11:48- have you here?
- 11:50- No. I think this has been fabulous. I
- 11:51- really appreciate you taking the time to talk
- 11:53- with me. I would just say that, you
- 11:54- know, we
- 11:55- are excited about the year coming up ahead,
- 11:58- and we look forward to, you know, continuing
- 12:00- to grow in Michigan and excited to be
- 12:03- part of the Henry Ford Health family as
- 12:04- we celebrate our fortieth year anniversary.
- 12:07- Well, thank you so much, Margaret. I really
- 12:09- appreciate you taking the time. Thank you. Have
- 12:11- a great afternoon. Yeah. You too. And to
