- 0:34Hello, everyone, and welcome to Becker's Healthcare Podcast.
- 0:36I'm Scott King. Thrilled today to be joined
- 0:38by a very special guest, Christy Spencer, vice
- 0:41president of provider partnerships
- 0:43with Elevent's Health. Christy, how are you? Thanks
- 0:45so much for joining the podcast.
- 0:47Yeah. I'm doing great. It's great to be
- 0:48here with you today. You know, we have
- 0:50a lot of big topics, new year, but
- 0:52some some new topics and some old ones
- 0:54to get into
- 0:55in health care and with health plans. But
- 0:57before I kinda jump into those, Christy, I
- 0:59was wondering if you could please just share
- 1:00a little bit about your background and your
- 1:02career journey.
- 1:04Yeah. Absolutely.
- 1:05So as you said, I'm the vice president
- 1:07for provider partnerships at EleventHealth.
- 1:10In that role, I lead our national strategy
- 1:12for provider partnerships and value based care across
- 1:16our Medicare, Medicaid, and commercial lines of business.
- 1:19So really focusing on improving cost, quality, and
- 1:23our member experience.
- 1:24I I bring a unique perspective to this
- 1:27work because I've spent my career in health
- 1:29care
- 1:30viewing health care from multiple different angles. So
- 1:32I've held leadership positions at hospital systems,
- 1:36primary care organizations,
- 1:38health plans.
- 1:39I've run value based physician partnerships with full
- 1:42P and L accountability.
- 1:44I've worked directly with providers as they have
- 1:47transitioned from fee for service into downside risk
- 1:50models.
- 1:51So that really that experience really shapes how
- 1:53I approach the role.
- 1:55So I've seen firsthand that value based care
- 1:59only works when it's practical,
- 2:01when incentives are aligned,
- 2:03data is actionable, and providers are genuinely
- 2:06supported and not overwhelmed.
- 2:08So my focus at Allavance is on enabling
- 2:11provider success,
- 2:13reducing friction,
- 2:14and building partnerships that deliver measurable outcomes for
- 2:18providers,
- 2:19members, and for the business.
- 2:21Thank you, Christy. And I appreciate you sharing
- 2:23all that background info. And, yeah, we're you
- 2:24definitely have a unique perspective. And not only
- 2:27are you gonna lean on that for your
- 2:28with you, today in the podcast, but we're
- 2:30really looking forward to having you speak at
- 2:31our event, in April to spring payer issues
- 2:33roundtable. So excited to have you there. And,
- 2:36you you know, the first question I wanna
- 2:38get to from your perspective
- 2:40and something we hear about at our conferences
- 2:42especially
- 2:43is how relationships
- 2:45are
- 2:45changing a bit, evolving a little bit with
- 2:48health plans and providers.
- 2:50How are your relationship with providers changing as
- 2:52both sides face, you know, right now, cost
- 2:54pressure
- 2:55and workforce shortages?
- 2:57Yeah. That's such a great question. Those challenges
- 2:59are real.
- 3:00Provider relationships
- 3:02are becoming more pragmatic
- 3:04and interdependent.
- 3:05As we look at the landscape costs,
- 3:08workforce challenges, and just overall industry
- 3:11complexity,
- 3:12it's forcing us to shift away from transactional
- 3:16contracting
- 3:17to true partnerships.
- 3:20At a high level, success from my provider
- 3:23partnership background,
- 3:24it really requires
- 3:25three things working
- 3:27together, effective provider engagement,
- 3:30well designed incentives,
- 3:32and connected actionable data.
- 3:35When those elements are aligned, you can really
- 3:38begin to create a connected care ecosystem.
- 3:42So what's changing most is the conversation.
- 3:46Providers don't need more measures or reporting requirements.
- 3:51What they need is help removing friction in
- 3:53their day to day operations,
- 3:55as well as clarity on what actually drives
- 3:58outcomes.
- 3:59So that means designing incentives that are focused
- 4:03that focus attention on
- 4:05actions that matter most
- 4:07and invest
- 4:08in dedicated engagement teams that can meet providers
- 4:11where they are and delivering data through unified
- 4:15platforms that providers can actually use.
- 4:18When you combine that with innovative use of
- 4:21technology
- 4:22and artificial intelligence,
- 4:24you can really start to lower cost, improve
- 4:27outcomes,
- 4:28and reduce burden all at the same time.
- 4:31Our strongest provider relationships today are the ones
- 4:35that are built on trust and shared accountability
- 4:38and a real commitment to making it easier
- 4:41to deliver care, not making it more complex.
- 4:45Absolutely. I I think those are a lot
- 4:46of things that'll definitely strengthen that provider,
- 4:50relationship with with health plans. And, Christy, do
- 4:52you have is do you have an example
- 4:54of of you you know, I I think
- 4:56it was a great point you made. Do
- 4:57you have an example of, like, an action
- 4:58driven
- 4:59incentive
- 5:00that that you're trying to incorporate or have
- 5:02incorporated?
- 5:04Yeah. One of the things that we're doing
- 5:05is really focusing around quality outcomes
- 5:09and looking at how do we reward providers
- 5:13for closing quality care gaps. So we in
- 5:16in this year are beginning to
- 5:19expand our reach
- 5:20of inviting providers in to participate in our
- 5:24pay for quality program
- 5:26that really incentivizes
- 5:27providers to
- 5:29close care gaps and aligns and simplifies the
- 5:32work for them to actually do that. So
- 5:34we're super excited about being able to expand
- 5:37that program this year.
- 5:39Where do you see the biggest gap
- 5:41right now between payer strategy and operational
- 5:44execution?
- 5:46Yeah. I I don't know that I would
- 5:47describe it necessarily
- 5:49as a gap,
- 5:50maybe more as a moment of transition for
- 5:53the industry.
- 5:54Okay. The the future of health care really
- 5:56has to be built around outcomes. We've focused
- 5:59so much on transactions
- 6:01in the past. And moving forward, I think
- 6:03we really have to focus on
- 6:05aligning around incentives that improve health
- 6:08versus just
- 6:10just focusing on delivery of services.
- 6:12Value based care for Elevance Health, it isn't
- 6:15a pilot. It's not a side initiative.
- 6:18It's truly how we operate.
- 6:20And you can see that embedded in our
- 6:22provider
- 6:22partnerships,
- 6:23in the way that we're designing our products,
- 6:26in how we're engaging our consumers.
- 6:29And the results that we're seeing are real.
- 6:33In 2024,
- 6:3565.5%
- 6:37of our total medical
- 6:39spend was aligned to value based care, and
- 6:4237%
- 6:43of that was in shared risk, which is
- 6:45an increase over the prior year.
- 6:47And the impact,
- 6:49of that, the outcomes are really impactful.
- 6:52In our, Medicare Advantage plans, we're seeing that
- 6:55value based providers
- 6:57demonstrated
- 6:583.9%
- 7:00lower MLR
- 7:01compared to non value based providers,
- 7:05and they've outperformed
- 7:06year over year on nearly 74%
- 7:09of our STARS quality measures.
- 7:12Additionally,
- 7:1376%
- 7:14of our value based practices improved their overall
- 7:17stars performance
- 7:18in that same period.
- 7:20So the strategy is right, yet it it
- 7:22really only works
- 7:24when it shows up simply and clearly. Ideally,
- 7:28it's directly within the provider's work flow
- 7:31where it can can really reduce complexity.
- 7:34And so that's where the next phase of
- 7:37operational excellence
- 7:38really matters.
- 7:40Along the same lines of reducing complexity, Christy,
- 7:43what's one investment or initiative you believe will
- 7:46most reshape how health plans operate over the
- 7:48next two to three years?
- 7:51That's a great question. As I stated in
- 7:53my intro, I've worked across
- 7:55multiple,
- 7:56components of the health care ecosystem, hospitals,
- 8:00physician groups, health plans.
- 8:02And that's that perspective has reinforced one thing
- 8:05for me. Partnerships have been done well or
- 8:07one of the most powerful levers that we
- 8:09have.
- 8:10Many providers,
- 8:12particularly smaller and independent groups,
- 8:16are operating in constrained environments.
- 8:18They have the ability to succeed in value
- 8:21based care, but they often just don't have
- 8:23the scale, the infrastructure, or the support that's
- 8:26needed
- 8:27to make that transition effectively.
- 8:30That's where our national value partners play a
- 8:33really critical role. So we work with these
- 8:36partners to extend capabilities
- 8:39to smaller provider groups,
- 8:41and they help those smaller provider groups standardize
- 8:45data
- 8:46flows, close care gaps, and prioritize
- 8:49interventions that actually move outcomes at a clinical
- 8:53level.
- 8:54The goal isn't
- 8:56to favor larger providers. It's really about lifting
- 8:59performance
- 9:00across the overall ecosystem
- 9:03to make value based care accessible and sustainable
- 9:06for providers of all sizes.
- 9:08So when when payers and these national
- 9:11value partners
- 9:13team up with our local providers and share
- 9:16knowledge
- 9:17and operate in an integrated system,
- 9:20everyone benefits, providers get the tools and support
- 9:23they need,
- 9:24health plans see better cost and quality performance,
- 9:28and members receive more coordinated and proactive care.
- 9:31So it's a true partnership model that's focused
- 9:34on enabling.
- 9:35That's really what's gonna help reshape
- 9:38how health plans operate over the next few
- 9:41years.
- 9:42Yeah. I really like that point about lifting
- 9:44performance across the board and kind of focusing
- 9:46there and not just, you know, it's just
- 9:47not just about paying attention to the big
- 9:49ones. I think, yeah, everyone benefits from that,
- 9:51and that it's such a great, you know,
- 9:52use of time and effort to focus on
- 9:54that.
- 9:55If you could change one
- 9:57regulatory or industry practice tomorrow to improve affordability
- 10:00and access,
- 10:01what would it be and why?
- 10:04Yeah. That's another great question. Yeah. I think
- 10:06there's such a need for simplification
- 10:09in the industry. So I would simplify
- 10:12and standardize administrative requirements across the industry,
- 10:16particularly
- 10:17around two areas, prior authorization
- 10:19and quality reporting.
- 10:22Traditionally, health plans have focused on paying for
- 10:24care, managing costs, and ensuring quality.
- 10:28Today, though, we recognize that most of the
- 10:30drivers of health happen outside of the clinical
- 10:33setting.
- 10:34In
- 10:35excessive administrative burden, it really pulls time and
- 10:39resources away from where it matters most.
- 10:42At Elevent's Health, we're using advanced digital tools
- 10:46and artificial intelligence to make health care simpler
- 10:50and more affordable and more personalized.
- 10:53This empowers our care providers to enhance health
- 10:56outcomes.
- 10:57So, for example,
- 10:59we're using digital tools to help identify care
- 11:03gaps quickly
- 11:04and support treatment decisions,
- 11:06and that really is helping improve health outcomes.
- 11:09We're also appropriately
- 11:11leveraging
- 11:13digital technologies
- 11:14for tasks like
- 11:16claims review and provider data updates,
- 11:19and it's helping us to ensure smoother and
- 11:22faster service.
- 11:24So as a result,
- 11:25AI enabled tools are cutting down on time
- 11:28consuming tasks
- 11:30so care providers can focus
- 11:32more time with patients.
- 11:34So my hope is that a more holistic
- 11:37and streamlined approach becomes the norm,
- 11:40one where digital advances are used to personalize
- 11:43care and eliminate friction, not not add to
- 11:46it. We have enough adding to friction in
- 11:49the industry.
- 11:50So so again, success really, really comes back
- 11:53to those three key elements working together in
- 11:55concert.
- 11:57Effective provider engagement,
- 11:59well designed incentives,
- 12:00and connected data working together.
- 12:03So the big picture is then a connected
- 12:06care ecosystem.
- 12:08Yeah. I think digital tools are obviously making
- 12:10a huge difference for health plans right now
- 12:13in so many regards.
- 12:14What digital tool do you think has helped
- 12:16the LVNs the most?
- 12:18Yeah. I think that that we're really focused
- 12:21on there's there's several digital tools that we're
- 12:24work working through. One of them is that
- 12:26we are using digital technologies to automate
- 12:30our post call,
- 12:32wrap up summaries,
- 12:33and we're incorporating
- 12:35issue detection and smart routing into each of
- 12:38those calls that which really helps to improve
- 12:41our associates' ability to manage large call volumes
- 12:45and enhance the customer experience.
- 12:48So each month, we're averaging about 1,000,000
- 12:51post call
- 12:52wrap up summaries, which translates into really meaningful
- 12:56time savings and better experience.
- 12:58And then we're using those same capabilities
- 13:01externally
- 13:02to help providers spend less time on administrative
- 13:05work and more time with patients. So that
- 13:08whether it's
- 13:09time spent reviewing claims or processing provider data
- 13:12updates or identifying care gaps, the goal is
- 13:16really the same. We're simplifying how we work,
- 13:19we're
- 13:20personalizing
- 13:21experience,
- 13:22and really empowering
- 13:24providers to enhance health outcomes.
- 13:26That's incredible. Not only think about how much
- 13:28time was is saved with with that, but
- 13:30what people can do with that time. That's
- 13:33I think it's phenomenal.
- 13:34The last question I have for you, Christy,
- 13:37what issue is putting the most pressure
- 13:39on health plan margins right now, and how
- 13:42are you responding differently or how will you
- 13:44respond differently in in 2026
- 13:46here? Yeah. And we we touch on that
- 13:48a little bit. Yeah.
- 13:49Health care costs, they're shaped by many forces.
- 13:52Hospital prices, pharmaceutical
- 13:54costs, health insurance costs,
- 13:57advances in technology,
- 13:59changing regulations.
- 14:00We we all have a role to play,
- 14:03across the entire health care ecosystem
- 14:06to help make sure that members are receiving
- 14:08the care that they need at the most
- 14:10affordable cost. It's it's a really important
- 14:12focus for Elevance.
- 14:15In 2026,
- 14:16my provider partnership team is focused on
- 14:19optimizing our value based portfolio
- 14:22and scaling
- 14:24risk where providers are ready and investing in
- 14:27enablement to really truly drive health care,
- 14:30outcomes and affordability.
- 14:32And as we've talked about previously,
- 14:35technology and AI are such a critical part
- 14:38of that.
- 14:39And it's not about creating tech for tech's
- 14:41sake. The main goal really is about improving
- 14:44the health care experience
- 14:45for both providers and members.
- 14:48So I talked a little bit about internal
- 14:50automation and how much time we're able to
- 14:53spend
- 14:54or how much time we're able
- 14:56to save by removing those manual tasks and
- 14:59allowing greater focus, for our members and for
- 15:02their experiences.
- 15:04And we're deploying those,
- 15:06same capabilities
- 15:07for our care providers to really do the
- 15:09same and and really in enabling them to
- 15:13focus their time with patients.
- 15:16So margin improvement,
- 15:18from my view, comes from
- 15:20enabling the right partnerships
- 15:23at the right level of risk with the
- 15:25right support
- 15:26and measuring success by outcome and not activity.
- 15:31So again, success comes from
- 15:34those three elements working together:
- 15:36effective provider engagement,
- 15:38well designed incentives, and connected actionable data. When
- 15:42those elements are aligned,
- 15:44you can really start to create a connected
- 15:46care ecosystem
- 15:48that produces better outcomes and affordability for our
- 15:51members.
- 15:53Christy, thanks so much for joining the podcast.
- 15:55It was a great conversation. I look forward
- 15:56to working with you again soon.
- 15:58Likewise. Thank you so much for your time
- 16:00and for having me today.
