- 0:31Hello, everyone, and welcome to Becker's Healthcare podcast.
- 0:34I'm Scott King, thrilled today to be joined
- 0:35by very special guest, James Wellman, vice president
- 0:39and chief information officer over at Nathan Littauer
- 0:42Hospital and Nursing Home. James, how are you
- 0:44doing today?
- 0:45Doing very well. How are you doing today,
- 0:46Scott? I'm good. Thanks. You know, it could
- 0:48be a little warmer in Chicago as you
- 0:50may have heard, but but, you know, we'll
- 0:52get through it. And we have a lot
- 0:53of of big topics to get to. But
- 0:55before we do that, I was wondering, James,
- 0:57if you could please just tell us a
- 0:58little bit about your background,
- 1:00in health care.
- 1:01Sure.
- 1:02So I am as stated, I'm the current
- 1:05CIO
- 1:06at Nathan Latower, but I've been in health
- 1:08care,
- 1:09IT for about thirty seven years.
- 1:12Started out in a major university,
- 1:15went to work for
- 1:17for profit groups like McKesson,
- 1:19and ended up in different hospitals around the
- 1:22country where I've kinda
- 1:24focused on cleaning up operations
- 1:27and getting EMRs,
- 1:29kinda back on track. Maybe that didn't have
- 1:32such a great implementation or were struggling a
- 1:34little bit, and that's kinda become a hallmark
- 1:37of my career.
- 1:38Appreciate you sharing your background info, James. And
- 1:41now the the first
- 1:42topic I kinda wanna get to is what
- 1:44opportunities and headwinds do you have your eye
- 1:46on right now in health
- 1:48care? Sure.
- 1:49I I think we're all facing a lot
- 1:51of the same headwinds,
- 1:53with with the way we're waiting on the
- 1:55the final impact, I think, about the one
- 1:58big beautiful bill, what that's doing to us
- 2:00with our,
- 2:01Medicaid and the Medicare and our reimbursement. So,
- 2:04in New York, especially, it's having a major
- 2:06impact on us.
- 2:08You know, the opportunities,
- 2:10we're using this one
- 2:13to really start hyper focusing on the work
- 2:15that we're already doing, making sure that we
- 2:18are getting our full value and reimbursement.
- 2:21And we're also spending a little time at
- 2:23our particular organization,
- 2:25moving forward with a lot of good new
- 2:27technology,
- 2:28some interesting things with Meditech and a few
- 2:31other vendors that we're pretty excited about.
- 2:33How are you thinking about
- 2:35growth and and adding value to your organization
- 2:38right now?
- 2:39So for us, it's, I've been here just
- 2:42a little over two and a half years,
- 2:44and this year is a major transition for
- 2:47us.
- 2:48So from a growth perspective, we're really starting
- 2:50to introduce data
- 2:52to the organization
- 2:53and, like I said, focusing
- 2:56on what the needs are for our community
- 2:58and for our families and residents
- 3:00in our region of the Adirondacks.
- 3:03The to do that, what we're we've done
- 3:06is taken a pragmatic approach to AI,
- 3:09And we think that we have a pretty
- 3:12good four pillar process that we talk about
- 3:15to introduce this and not just introduce a
- 3:18an AI segment, for example, in ambient to
- 3:20listen, that's great, and there's some fantastic products
- 3:23out there. But how do we tie that
- 3:25all the way through the entire process? So
- 3:28we blended that with RCM,
- 3:31with, search and summarization,
- 3:34and gives us a soft audit capabilities going
- 3:36forward to really make sure that
- 3:39we're getting best information for our patients and
- 3:41giving the best diagnostic
- 3:43information to our providers
- 3:45and also getting fully reimbursed for the work
- 3:47that we're doing.
- 3:49And, James, to follow that up, also, once
- 3:50you you find these these uses for AI,
- 3:53you know, after you you, do some research,
- 3:56you implement them, and you work out the
- 3:57kinks, are you already thinking of, like, the
- 3:59next uses
- 4:00for the system you wanna use, or you
- 4:02still kinda wanna see this all run smoothly?
- 4:05So what we're looking at right now is
- 4:07we we spent a lot of time in
- 4:09'25,
- 4:11'24 and '25
- 4:13speaking with multiple vendors about how we wanted
- 4:15to approach this. So we went in kinda
- 4:18all in with one vendor,
- 4:19who's in happened to be a Meditech alliance
- 4:21partner,
- 4:23and we've now started deploying all of these
- 4:25separate projects.
- 4:27So that's that's a big
- 4:29initiative for us. RCM is the one where
- 4:32we think is gonna have the largest return
- 4:35because that's the one that every one of
- 4:38the payers that we're working with, they're already
- 4:40using the AI to to work on denials.
- 4:42So we kinda get ahead of curve is
- 4:44what we're working on.
- 4:46And so that's something that we think is
- 4:49the future for a lot of organizations
- 4:52to rural health organizations, but anyone's gonna benefit
- 4:55from this.
- 4:57What's one risk or investment you think is
- 4:59is worth making in 2026?
- 5:04So to to me, it's absolutely gotta be
- 5:07the RCM, the mid cycle revenue.
- 5:10Where can we make an absolute difference?
- 5:13Rural health is, you know, under
- 5:15amazing
- 5:16pressures that we've never seen before. It's always
- 5:19been a challenge.
- 5:20It's something that, personally,
- 5:23I really resonated with me and I devoted
- 5:25my career to,
- 5:26about twenty years ago.
- 5:29But now we're seeing unprecedented
- 5:30challenges for us. I really think the appropriate
- 5:34use of AI
- 5:36is something that can make a difference.
- 5:38Bringing that data to us, letting us look
- 5:41at information, making sure that we're getting
- 5:45all the money and for everything that we're
- 5:46being fairly represented,
- 5:49that's a major initiative for us. And that's
- 5:52something, you know
- 5:53obviously, it comes with risk.
- 5:56We have to make sure that we don't
- 5:57impact cash flow. We have to make sure
- 6:00that it's an already
- 6:02tenuous situation in reimbursement,
- 6:04but we feel like this can
- 6:06make a very positive transition the other way.
- 6:10Just the first run of data that we're
- 6:12seeing
- 6:13shows that we have a lot of room
- 6:14for improvement, and we can make a a
- 6:16better
- 6:17impact on the bottom line.
- 6:20What are some of the key things you
- 6:21look for in that data to to know
- 6:22that it's working the right way?
- 6:24So one of the things that are challenged
- 6:26for us when you're talking about a small
- 6:28organization
- 6:29is we do not have the ability to
- 6:32have multiple people
- 6:33watching everything that all our providers are doing.
- 6:35So standardization,
- 6:37pulling the data in,
- 6:39improving the way that we can see patients,
- 6:41expanding our schedule,
- 6:44just setting up and creating,
- 6:47chart prep,
- 6:48you know, for a busy schedule on a
- 6:50day and a provider
- 6:52can take an an an enormous amount of
- 6:54time.
- 6:55And we are cutting that back to,
- 6:58in a huge magnitude. Some of the providers
- 7:01that we're talking to, and they're seeing this
- 7:03because we're able to pull all the existing
- 7:04data from our Mediatek expanse, but also our
- 7:07previous systems that we were using, both Mediatek
- 7:10Magic and MedEd for an ambulatory system.
- 7:13And it's in a data lake, and they're
- 7:15able to start they'll be doing a summarization
- 7:17of this data.
- 7:18And that gives them everything that they need
- 7:20so that they can make the appropriate diagnosis.
- 7:23We can actually help with the patient. And
- 7:25then we also understand
- 7:27it looks at our payer contracts, and we
- 7:29know what our reimbursement should be.
- 7:31And this helps us track it.
- 7:33Otherwise, that had been a process that's, you
- 7:36know, always available, and that data is there.
- 7:38But actually running it all effectively,
- 7:41without a large staff has been a huge
- 7:43challenge. This is something we feel will make
- 7:46a a big difference for us.
- 7:48And the last question I have for you,
- 7:49James, where do you see the best opportunities
- 7:52for growth in the future?
- 7:54I think that the thing for us is
- 7:57we're going to go back to the basics
- 7:59and foundations of data,
- 8:01Understand
- 8:02what your what your community needs.
- 8:05We all have a lot of the same
- 8:07standard, you know, diabetic,
- 8:09COPD, something like that that we're tracking.
- 8:13But it's the effective use of that. We
- 8:16wanna make sure that I think if you
- 8:18got a rural health organization and they focus
- 8:21on providing
- 8:22the best possible care and keeping the families
- 8:25close to the patients
- 8:27and not sending them away,
- 8:29that's where we're making our investments. So we're
- 8:32making clinical investments
- 8:34that make a difference for our community. And
- 8:36I think that's one of the things that
- 8:37a lot of people can focus on.
- 8:40Your satisfaction scores go up,
- 8:42and you're doing what's best. And we are
- 8:44integral part of the community. So that's where
- 8:47our focus is right now. Well, James, thanks
- 8:49so much for joining us on the podcast
- 8:51and for a great conversation. Look forward to
- 8:52working with you again soon. Alright. Thank you
- 8:55so much.
