- 0:00Welcome everyone to the Becker's Healthcare podcast series.
- 0:02I'm Mariah Muhammad, writer and moderator with Becker's
- 0:05Healthcare. And I'm thrilled to have with me
- 0:07today Steve Tringale, president of Mass General Brigham
- 0:10health plan. Steve, welcome to the podcast. We're
- 0:12very excited to have you join us today.
- 0:14To To get us started, would you mind
- 0:15please introducing yourself and telling us a bit
- 0:17about your background?
- 0:18First, thank you for having me, today.
- 0:21I'm Steve Tringale. I'm president of the Mass
- 0:24General Brigham Health Plan.
- 0:26We're a health plan of approximately
- 0:28400,000
- 0:30members
- 0:31across commercial Medicare Advantage
- 0:35and Medicaid,
- 0:37product lines.
- 0:38We also will be entering,
- 0:41the dual eligible product line, so we have
- 0:43a full product portfolio.
- 0:45And we are focused
- 0:48really on
- 0:49building a model which
- 0:52takes advantage
- 0:54of our integrated health care system
- 0:57in using
- 0:59the incredible resources at Mass General Brigham
- 1:02to provide our members with
- 1:04a better experience,
- 1:06a better clinical outcome, and quite frankly,
- 1:09a better product
- 1:11than other health insurers
- 1:13and and really take advantage of that combination
- 1:16of world class care
- 1:18and comprehensive coverage that we can provide.
- 1:21Yeah. Yeah. Wonderful. Thank you so much for
- 1:23giving us that background. So
- 1:25health care in hospitals and health plans, health
- 1:28care overall is really, changing rapidly right now
- 1:32as I'm sure you're, well aware. What are
- 1:34some of the biggest issues you're following in
- 1:36health care this year so far?
- 1:38Well, it is, in fact,
- 1:41both the fragility and the volatility of of
- 1:44both the delivery systems and insurers at the
- 1:47same time
- 1:49is something I have not seen
- 1:52very often in my career.
- 1:55So when we look at that level of
- 1:59volatility
- 2:00in the marketplace,
- 2:02when we look at,
- 2:04the level of accelerated
- 2:06change that you usually get
- 2:09in the first year of a new administration
- 2:11and a new congress,
- 2:12and you put all of those things together,
- 2:16keeping
- 2:17our plan
- 2:19focused
- 2:20on our North Stars,
- 2:22which are essentially
- 2:24providing that member
- 2:26focus so that they're having a better experience
- 2:28and a better outcome
- 2:30and
- 2:32taking advantage
- 2:33of our integrated delivery system.
- 2:35We try and stay focused on that, but
- 2:37at the same time,
- 2:40be flexible and agile
- 2:42to be able to pivot to respond to
- 2:45both expected and unexpected challenges and opportunities
- 2:50that are gonna be created by what is
- 2:52a rapidly changing and a rapidly consolidating industry
- 2:55on
- 3:01Yeah. Yeah. Absolutely.
- 3:02And,
- 3:03you know, saying this type of change being
- 3:05something you haven't seen much in your in
- 3:07your career, is this something that's kinda making
- 3:09you nervous? And if not, is there anything
- 3:11that you are most excited about,
- 3:14whether in tech, whether,
- 3:16something that is changing overall or anything else
- 3:18that's making you nervous?
- 3:21So
- 3:22I don't know that I'd use the word
- 3:24nervous as much as
- 3:27focused.
- 3:28There are a number of areas that require,
- 3:31an extraordinary
- 3:32level of focus right now.
- 3:34Access is a huge issue and access from
- 3:37a financial basis,
- 3:39which means making sure that the products can
- 3:41stay affordable,
- 3:43and access
- 3:45from a physical basis, which means
- 3:47dealing with the primary
- 3:49care shortage, which is affecting
- 3:51both delivery systems and health plans across The
- 3:54United States, and we're no exception to that.
- 3:57So coming up with build to purpose,
- 3:59solutions on the primary care side
- 4:03and using
- 4:04AI,
- 4:06not just
- 4:07at the the health plan level in terms
- 4:10of
- 4:11managing our claims,
- 4:13volumes and managing,
- 4:16denials and prior approvals,
- 4:19using AI
- 4:20to improve the member experience,
- 4:23to enhance the ability
- 4:25to
- 4:27help the member navigate and move to a
- 4:29very complex
- 4:31delivery system
- 4:32to be at the right place, to get
- 4:34the right care at the right time,
- 4:36to make sure that we're providing the member
- 4:39with the highest level of
- 4:43digitally enhanced
- 4:45responses to some of
- 4:47the clinical challenges, whether it be
- 4:51in the area of of
- 4:53high end imaging,
- 4:55or whether it be in using
- 4:58AI to help our analytics to focus on
- 5:02what are the right mix of services
- 5:06to provide to a member so that when
- 5:07we look at issues like the lifestyle challenges
- 5:11between diabetes and obesity,
- 5:14that
- 5:14we're not solely focused on
- 5:17do, we approve
- 5:20the provision
- 5:21of a GLP one, but what are the
- 5:23right
- 5:25mix of services
- 5:28that we should wrap around the provision of
- 5:30that GLP one
- 5:32that's going to help that member
- 5:35succeed over the long term
- 5:37to have
- 5:38better health outcomes and not just have
- 5:41some weight loss, to change the way they
- 5:44look at some of their lifestyle decisions,
- 5:46to change the way that they
- 5:49approach some of their day to day activities,
- 5:52whether it be,
- 5:53food based, whether it be exercise based, etcetera.
- 5:57In having,
- 5:58using AI to build analytics to help us
- 6:01on those issues
- 6:02as much as some of
- 6:04the core insurance issues, which have quite frankly
- 6:07gotten some negative press
- 6:09in in terms of how other companies are
- 6:11using it. We wanna use AI
- 6:14to improve the member experience and to improve
- 6:17the member outcome.
- 6:19Yeah. Yeah. Absolutely. Thank you so much for
- 6:21sharing that. And access overall is definitely something
- 6:24I've heard other techs talk about in different
- 6:26ways as well.
- 6:27And,
- 6:28going with this kind of theme of change
- 6:30and implementing different things,
- 6:33in your opinion, what would the most effective
- 6:35health care leaders need in order to be
- 6:37successful in the next two to three years,
- 6:39do you think?
- 6:41A couple of things.
- 6:42I think the biggest challenge, quite frankly, in
- 6:45the short term is leadership.
- 6:47There is so much going on
- 6:49that you need to keep your teams
- 6:51focused
- 6:53on the business of the business
- 6:55and at the same time,
- 6:58flexible and agile
- 7:00to, as I said, to take
- 7:03advantage of opportunities,
- 7:05not just to respond to some of the
- 7:07specific challenges out there.
- 7:09In terms of the focus on the business,
- 7:11you know, we're really proud that over the
- 7:13last two years, we've won
- 7:16recognition
- 7:17as from JD Power
- 7:19as the most trusted
- 7:22plan
- 7:23in the commercial marketplace in Massachusetts,
- 7:26a market which has some very strong competitors,
- 7:28and we've won that two years in a
- 7:30row.
- 7:31That's an example of staying focused.
- 7:33At the same time, an example of staying
- 7:37flexible and agile.
- 7:39You've all written an enormous amount about some
- 7:42of the challenges that this
- 7:45health system all over the country faced
- 7:48when,
- 7:49Stewart Healthcare went into bankruptcy.
- 7:52Our team,
- 7:53stepped up and immediately
- 7:55looked at
- 7:57every member that we had, every individual member
- 8:01that was in a steward facility to make
- 8:03sure that we could maintain continuity of care
- 8:06and a care regimen for patients who are
- 8:08in active cancer treatment, active cardiac treatment, etcetera.
- 8:12And they did that while we were running
- 8:14the the rest of, you know, the plan
- 8:16activities on a day to day basis.
- 8:19That's the kind of response that I think
- 8:22you're gonna need from good health plans
- 8:24to continue to operate in a system
- 8:28which is almost guaranteed to have
- 8:31a lot more bumps in the road, a
- 8:33lot more change,
- 8:34a lot more consolidation over the next two
- 8:36to three years. What we're seeing go on
- 8:38right now is not a one year
- 8:41interlude.
- 8:42There is a a a significant sea change
- 8:45going on in delivery system right now.
- 8:48So whether it's,
- 8:50digital innovation,
- 8:52it's whether working with our delivery system
- 8:57to improve integrated models of care, which differentiate
- 9:00us
- 9:01in the market,
- 9:03whether it's the blocking and tackling of
- 9:06customer service across very different product lines, whether
- 9:10it be commercial,
- 9:11Medicaid,
- 9:13geriatrics with Medicare Advantage.
- 9:16We need to
- 9:17to continue to improve
- 9:20what I think has been
- 9:22rather good performance, quite frankly.
- 9:25So,
- 9:26you know, the focus on continual improvement in
- 9:28a period of rapid change
- 9:31is a challenge from a leadership perspective.
- 9:34So the last piece of that is I
- 9:35need to challenge my leadership team,
- 9:39to be great two way communicators.
- 9:41The leadership teams have a pretty good window
- 9:44on what's going on
- 9:46at the policy level, at the state level,
- 9:48at the federal level, at the national level.
- 9:51But we need to be teaching,
- 9:54explaining,
- 9:55and be transparent about both the market dynamics
- 9:59and the political dynamics
- 10:01that are driving some of that change.
- 10:04And at the same time, we need to
- 10:05be sitting down with our workforce
- 10:07and be active listeners
- 10:09and make sure we understand how this is
- 10:12impacting them in the way they have to
- 10:14undertake their duties on a day to day
- 10:16basis.
- 10:17While this is a huge challenge for leadership,
- 10:19sometimes I think we forget
- 10:22how daunting some of this change can be
- 10:23for the overall workforce.
- 10:25So I think in many ways, the biggest
- 10:27challenge
- 10:28for senior executives, for senior leaders,
- 10:31for myself,
- 10:33is to be,
- 10:34as I said, sort of that great two
- 10:37way communicator
- 10:38because
- 10:39people will respond to challenges if they understand
- 10:42why they need to do it. It's a
- 10:45lot harder to respond to those challenges,
- 10:48and it's a lot more it's a lot
- 10:50more stressful to respond to those challenges
- 10:53if you really don't understand the underlying dynamics
- 10:55about what's going on.
- 10:58Yes. Yes. Absolutely. Thank you so much for
- 11:00those final thoughts. This has definitely been an
- 11:02informative discussion. And, again, I wanna thank you
- 11:04so much for coming on Becker's HealthCare,
- 11:07especially for the first time, and I look
- 11:09forward to connecting with you again soon.
- 11:11Sure. Thank you very much.
