- 0:30- This is Laura Dedo with the Becker's Healthcare Podcast.
- 0:33I'm thrilled today to be joined by Robert Crane, co-founder
- 0:35and CEO of ry, the national mental health practice
- 0:39that provides in-network psychiatry and therapy.
- 0:42Robert, it's a pleasure to have you on the podcast today.
- 0:45- Laura. Thank you so much for
- 0:47having me. I really appreciate it.
- 0:48- Now, I'm looking forward to our conversation
- 0:50because mental health
- 0:51and therapy is really on the forefront of a lot
- 0:54of the conversations happening, um,
- 0:56on the national scale today,
- 0:58and so important for healthcare as well as the wellbeing,
- 1:01um, of everyone, uh, within the, the country.
- 1:04But before we dive into that discussion,
- 1:06can you tell me a little bit more
- 1:07about yourself and your background?
- 1:10- Sure. Yeah. I mean, listen, my,
- 1:11my background is mostly in finance.
- 1:13Uh, I never set out to be an entrepreneur
- 1:15or specifically in the mental health space.
- 1:18Um, and, you know, the start of
- 1:20how I got involved really stems from a,
- 1:21a traumatic experience I had back in 2016.
- 1:24I had a home invasion in my apartment, uh, in New York City,
- 1:27and my wife, now girlfriend at the time, uh,
- 1:29was scared to leave the apartment after that.
- 1:31Um, and so, you know, obviously it was, it was tough.
- 1:34And, uh, a good friend of mine, his wife is a psychiatrist,
- 1:37and she recommended I go and speak to someone.
- 1:39Um, and very quickly found,
- 1:41found out it was almost impossible to find a psychiatrist,
- 1:44um, who accepts insurance.
- 1:45Um, and even more difficult to find the right one.
- 1:49You email 20 people and whoever
- 1:50responds is the person you go and see.
- 1:51And it's very high likelihood that
- 1:53that's not the right doctor for you,
- 1:54and you're not the right patient for that doctor.
- 1:58And so really try to understand, you know,
- 2:00why is this an issue?
- 2:01And, and, you know, you, you pull the string and, and more
- 2:03and more unravels you hear that, you know,
- 2:05insurers don't wanna pay for behavioral healthcare,
- 2:07which is not, not accurate at all.
- 2:09Um, and just saw really kind of, uh, interesting, um,
- 2:12opportunity in the space to solve a really big problem
- 2:14that I went through myself, uh, and had to pay out of pocket
- 2:17because the psychiatrist didn't accept insurance.
- 2:19Um, and, uh, you know, in 2019, uh, I, I left, uh,
- 2:24my previous role, um, I met my co-founder, Dr.
- 2:26Georgia Guvera, who's a triple board certified psychiatrist.
- 2:30Um, and together we, we founded psychiatry
- 2:32and saw our first patient in April of 2020,
- 2:36and we are fastly approaching our 1000000th visit.
- 2:40So it's been a, it's been a, a very exciting journey
- 2:43and a very rewarding one as well.
- 2:47- That's amazing to hear, you know,
- 2:48and definitely it seems like you've got, um, a,
- 2:52a great story in how this company was founded
- 2:54and really such an important mission.
- 2:57Now, what opportunities do you see as well as headwinds
- 3:00that you have your eye on, um, within the mental health
- 3:02and psychiatry space?
- 3:05- Yeah, I mean, listen, I think, I think there's a lot
- 3:07of opportunities, um, from a care coordination standpoint.
- 3:11Um, you know, psychiatry, um, really views ourselves.
- 3:15Um, I know like Switzerland, to be, to be totally honest
- 3:18with you, I think there's a lot
- 3:19of people in the behavioral health space,
- 3:21whether they're doing, you know, therapy
- 3:23or different types of specialized therapy, uh,
- 3:26or mental wellness or in the EAP market, if you will, right?
- 3:30There's, there's a lot of people, you know, who are,
- 3:32you know, either working with employers
- 3:34or insurers or health systems.
- 3:37And I think that, um, you know, what's,
- 3:39what's a really great opportunity in the space is
- 3:42to collaborate on care coordination for these patients.
- 3:44So regardless of how they, you know,
- 3:47seek behavioral healthcare
- 3:48or, you know, um, come in contact with a provider, you know,
- 3:51can we get them to the right place?
- 3:52You know, and RY is not the right place for everyone.
- 3:55And we've, we've collaborated on care coordination with,
- 3:58you know, other digital beha, behavioral health companies,
- 4:01like no cd for example.
- 4:02We're, we're announcing a, a, a higher acuity, um,
- 4:04care coordination partnership, um, in the, in the next week
- 4:07or so here as well, but also really major health
- 4:09systems that are nationwide.
- 4:11Um, and, uh, you know,
- 4:14RY provides care on an in-network basis.
- 4:15We provided, you know, in, in 43 states today, we
- 4:18provided care in 48 states.
- 4:20And so I see a really big opportunity
- 4:21to coordinate on patient's care, um,
- 4:24and help them move, um, up
- 4:25or down the acuity spectrum, whether that's, you know, um,
- 4:28exiting an intensive outpatient program
- 4:30and stepping down to a place like ry, whether that's,
- 4:33you know, they're in therapy currently
- 4:34and they, they need medication management,
- 4:36and how are we making sure that, um, we make, we, we,
- 4:38we ensure that that patient gets access
- 4:40to care in a timely manner.
- 4:41So I think that that's a big area
- 4:43that we're really looking forward to in kind of partnering
- 4:45with other nationwide, whether it's primary care groups,
- 4:47health systems, uh, EAP programs, mental wellness apps, um,
- 4:52but also, uh, therapy companies.
- 4:54Um, and I think that that really works for everyone, uh,
- 4:57for the benefit of patients,
- 4:58but also being able to provide, uh, information back, um,
- 5:02on quality measures
- 5:03and track those quality measures throughout, um,
- 5:05the acuity level back to payers at the end of the day.
- 5:07So super excited to continue to expand on that.
- 5:12- That's great to hear it, you know, and,
- 5:13and certainly, um, encouraging that there are so many ways,
- 5:16you know, that you've been able to really coordinate care,
- 5:19been able to make things a little bit more targeted
- 5:22and personalized, which I know patients, um,
- 5:24really appreciate and lots of opportunities ahead.
- 5:27Um, which, which is great to hear as well.
- 5:30How are you thinking about growth
- 5:31and adding value to, um, psychiatry overall?
- 5:35What does that look like for you?
- 5:38- Yeah, I mean, psychiatry's a really large employer
- 5:40of psychiatrists across the country.
- 5:42Um, that's our core expertise.
- 5:44Um, we're, we're focused on, you know,
- 5:46hiring clinicians on a W2 basis.
- 5:49And so, uh, whether that's, um, psychiatrist, we also have,
- 5:52uh, a therapy practice here at, at psychiatry, and,
- 5:54and we use that for patients
- 5:55who are already psychiatric patients of psychiatry and, and,
- 5:58and need a, um, a more closely coordinated, uh,
- 6:01level of care, if you will.
- 6:02Um, but we collaborate again, like I mentioned, most
- 6:05of our patients get therapy elsewhere,
- 6:06and we wanna be able to collaborate with that.
- 6:08So focused on continuing to grow our physician base,
- 6:11um, across the country.
- 6:13Uh, we do have a focus. We represent already every
- 6:15subspecialty in psychiatry,
- 6:17but really excited to continue to grow them.
- 6:18We announced earlier this year, our child
- 6:20and adolescent, uh, program specifically,
- 6:23and my co-founder, of course, is a, a board certified child
- 6:25and adolescent psychiatrist as well
- 6:27as an addiction specialist.
- 6:28Um, and we have a separate intake, uh, for patients, uh,
- 6:32who are, who are minors, for example,
- 6:34longer visit durations, proxy portals, you know, the ability
- 6:36to have multiple people on a televisit.
- 6:38So, you know, expanding into other, um,
- 6:40subspecialties in psychiatry, uh,
- 6:42or expanding upon them as we already have them, is something
- 6:45that we're, we're really
- 6:46excited, you know, to continue to do.
- 6:49- Yeah, absolutely.
- 6:50I, it feels like every, um, where I turn, I, I,
- 6:53I see the need for, especially as you said, more, um,
- 6:56mental health services for adolescents
- 6:58and children in the pediatric population, um,
- 7:01which could really, you know, uh, be helpful for, for them
- 7:04as they're navigating just, uh, a lot of
- 7:06of things going on right now in the world.
- 7:08So, um, I, I can imagine there'll be a lot of need in,
- 7:11in a lot of, uh, excitement
- 7:13around being able to dig deeper there.
- 7:16- Yeah, exactly. We're, we're super excited about it.
- 7:19- Fantastic. Well, you know,
- 7:21I know this past year especially has been challenging
- 7:24for many organizations in healthcare and
- 7:26otherwise, um, you know, whether it's financially or,
- 7:29or trying to make sure you have your resources matching in,
- 7:33in, um, spending, you know, the right type
- 7:35of resources in the right areas, um,
- 7:38but still is important to grow and innovate.
- 7:40From your perspective, what is one risk
- 7:42or investment that's still worth making this year
- 7:45or in 2024 as well?
- 7:47- Yeah, definitely. I mean, psychiatry, um,
- 7:50certainly has a focus, um,
- 7:52and continues to invest in, in quality
- 7:54and measuring outcomes.
- 7:55Um, you know, we, we have a,
- 7:57a clinical quality function at psychiatry,
- 7:59you know, led by a psychiatrist.
- 8:01Um, it's a big, big focus of ours.
- 8:03Um, we're gonna be coming out
- 8:05with a study here very shortly, uh, that's published, um,
- 8:08on, uh, uh, our patient population that we did.
- 8:11We, we looked at over 1800 patients at pry that came in
- 8:15with varying levels of, um, severity.
- 8:17And so we're really excited, um, to continue
- 8:20to invest in proving outcomes as well as connecting it
- 8:23to total cost of care, right.
- 8:25Pry, uh, is really looking to shift towards more
- 8:29of a value-based, um, component, if you will.
- 8:32And so we want to identify the total cost of care
- 8:35of our patients on the physical side
- 8:37and the behavioral health side,
- 8:38and be able to demonstrate, um, that the outcomes, uh, are,
- 8:42are not only best in class,
- 8:44but also resulting in significant cost savings
- 8:47for our payer partners, which of course trickles down
- 8:49to cost savings for patients at the end of the day.
- 8:51So we continue to have a big focus on investing in those
- 8:54programs, um,
- 8:55and going at risk, uh, for, um, some of the, uh, payments
- 8:59that we receive and establishing those programs, uh, with,
- 9:03with our large payer partners.
- 9:06- I love it. That's great to hear.
- 9:08Well, before we wrap up our conversation,
- 9:09where do you see some of the best, um, opportunities
- 9:12for growth in the future yourself?
- 9:16- Yeah, I mean, again, I would go back to, you know,
- 9:19we see a lot of growth in partnership opportunities in terms
- 9:21of collaborating on patient care,
- 9:23and they don't just have
- 9:24to come from the behavioral health space.
- 9:25We see a lot of opportunity in really large health systems
- 9:28partnering with psychiatry
- 9:29for outpatient psychiatry referrals.
- 9:32We see a, a, a great opportunity
- 9:33to give them the information back, of course,
- 9:35with patient consent in terms of how that patient's doing,
- 9:38what did we diagnose them with, what is the care plan
- 9:40to allow primary care providers
- 9:42to effectively follow up better on that patient care, um,
- 9:45and make sure that we're coordinating really effectively.
- 9:47So on the partnership side, you know, we're,
- 9:49we're incredibly excited.
- 9:50You know, we think, um, you know, the world is moving
- 9:53to not just access, but access and quality.
- 9:56Um, and a lot of these partners, if you will, you know,
- 9:58and what quality looks like is not only delivering best,
- 10:01you know, great outcomes themselves for what they do,
- 10:04but having much better care coordination
- 10:05and a lot of com, uh, a large portion of patients, you know,
- 10:08they, they, they need medication management at some point.
- 10:12Uh, not all of them, of course,
- 10:13but if you do need that, you know, it's super difficult
- 10:15to find, um, psychiatrists,
- 10:17if you will, but accept insurance.
- 10:19Um, and that's where psychiatry really stands out as kind
- 10:22of a nationwide partner to help collaborate
- 10:24on those, on those patients.
- 10:25Um, you know, not really in a,
- 10:26in a competitive nature at all,
- 10:27but more on a, on a collaborative, um, component.
- 10:30And so, you know, we're, we're really excited for that
- 10:32and we see a big opportunity, uh, to kind of grow those, um,
- 10:36those partnerships, uh, in the coming year.
- 10:40- That's amazing to hear. I love it.
- 10:42Robert, thank you so much
