- 0:04>> Dr. Terry Simpson: Here's a question for you. Is obesity a
- 0:06lifestyle choice? Short answer,
- 0:09Absolutely not. Long answer. That's
- 0:12what this podcast is about.
- 0:14Buckle up, because today we're going to unravel the
- 0:17complex, fascinating, and occasionally
- 0:19ridiculous way our brains, ultra
- 0:22processed food, and a little hormone called
- 0:24GLP1 are running the show when it comes to
- 0:27obesity. Oh, and spoiler alert.
- 0:30Calling obesity a, uh, lifestyle choice is not only
- 0:33incorrect, it is also fat shaming.
- 0:36So let's put that nonsense to
- 0:39rest.
- 0:42I am your Chief Medical Explanationist, Dr. Terry
- 0:45Simpson, and this is Forku
- 0:48Fork University, where we bust myths, make
- 0:51sense of the madness, and learn a little bit about food
- 0:54and medicine.
- 0:59Let's start with the oldest, most tired
- 1:02advice in the world. Just eat less
- 1:04and move more. It's the go to slogan of
- 1:07diet culture, printed on motivational posters
- 1:10and yelled by personal trainers everywhere.
- 1:13And guess what? It's as useful as
- 1:16telling a person with depression to just cheer
- 1:19up. If that advice worked, this
- 1:21podcast would be about something way more fun,
- 1:25like how to make a perfect sourdough bread.
- 1:28The reality is, obesity isn't just about
- 1:31calories in and calories out. It's a
- 1:33complex interplay of your brain, your
- 1:36biology, and the world around you. A
- 1:39world stuffed to the gills with ultra
- 1:41processed foods engineered to make you eat
- 1:44more. I'm talking about foods so refined
- 1:47they could pass as a contestant on the
- 1:49Bachelor. But why do we keep eating them? This
- 1:52brings me to my next point. Your brain is not
- 1:55your BFF when it comes to food.
- 1:59And here's the thing. Your brain has one job when it
- 2:01comes to food. To keep you alive. And
- 2:04it's stuck in the caveman mode,
- 2:07constantly afraid you're going to starve.
- 2:10So when you eat that bag of chips or that tub of ice cream,
- 2:13your brain throws a party. Why? Because
- 2:16these foods are packed with calories. And your brain
- 2:19sees calories as survival. The
- 2:21problem. Ultra processed foods
- 2:24hack to the system. They're designed to hit
- 2:27reward centers of your brain harder than a toddler hits a
- 2:30pinata. They combine sugar, fat, and
- 2:33salt into an unholy trinity of
- 2:35deliciousness that keeps you coming back for more.
- 2:38Your brain doesn't stand a chance. It's like playing
- 2:41poker against a deck full of aces. And you're going to lose
- 2:44every time. Oh, and let's not
- 2:47forget the lovely hormone Ghrelin. The
- 2:50I'm hungry hormone. Think of Ghrelin as
- 2:53your annoying co worker who keeps reminding
- 2:56you it's lunchtime even though you just ate. And guess
- 2:59what? Ultra processed foods can
- 3:01Amplify ghrelin's effects, making you
- 3:04feel hungrier and eat more. It's a
- 3:07vicious cycle.
- 3:09Now let's talk about the hero of our story, modern
- 3:12medicine and the GLP1s.
- 3:14GLP1 is a hormone that helps you regulate your
- 3:17appetite. And it's key to understanding why
- 3:19obesity isn't just a willpower issue. You
- 3:22see, GLP1 tells your brain, hey, you're full. You can stop
- 3:25eating now. But for people with
- 3:28obesity, that system is out of whack.
- 3:31Enter GLP1. Like semaglutide
- 3:34or Mounjaro. These medications mimic
- 3:37the effect of GLP1, essentially whispering to the brain,
- 3:40hey, chill out. You don't need another slice of pizza. And the
- 3:42results? People lose a significant amount
- 3:45of weight not just because they suddenly develop
- 3:48superhuman willpower, but because their biology
- 3:51is finally working with them instead of
- 3:54against them. And here's the kicker.
- 3:57These drugs have shown us that obesity isn't about
- 4:00eating too much. It's a disease. A
- 4:03complex chronic condition influenced by your
- 4:06genes, your hormones, and, yes, that sneaky brain
- 4:09of yours. And when you treat it like a
- 4:11disease, guess what happens? People get
- 4:14better.
- 4:16Now, uh, let's tackle the elephant in the room. The idea that
- 4:19obesity is a lifestyle choice. I'm just going to say it.
- 4:22This is one of the most damaging myths out there. Calling
- 4:24obesity a lifestyle choice is like calling an
- 4:27asthmatic a bad breathing decision.
- 4:30It's not just wrong, it's harmful.
- 4:33When we frame obesity as a lifestyle choice, we're
- 4:35essentially saying, if you're overweight, it's your fault.
- 4:38That kind of thinking leads to the stigmata, shame, and
- 4:41discrimination. And guess what? Shame is not a good
- 4:44motivator for change. If it were, the
- 4:47gym would be packed with people who got roasted at
- 4:50Thanksgiving dinner. Let's be clear. When
- 4:52lifestyle factors like diet and exercise contribute to
- 4:55obesity, they're not the sole cause. Genetics plays a
- 4:58massive role, as does the food environment we live
- 5:01in. Blaming someone for their obesity is like blaming
- 5:04someone for living in a city with terrible air quality. Sure,
- 5:07they can try and breathe better, but the odds are stacked against
- 5:10them. And let's take a moment to appreciate how
- 5:12diabolical ultra processed foods really
- 5:15are. These aren't just foods.
- 5:18They're science experiments designed to keep you
- 5:21eating. They're hyper palatable, calorie
- 5:23dense, often stripped of fiber and nutrients.
- 5:26It's like the evil twin of a home cooked
- 5:29meal. Here's a fun fact. Studies have shown
- 5:32that people eat more when they consume ultra
- 5:35processed foods. About 500
- 5:37calories a day more. And guess what?
- 5:40Those calories add up over time. That's
- 5:43how you end up with weight gain, metabolic issues, and
- 5:46a lifelong addiction to Doritos.
- 5:49In my case, maybe it's Reese's. Ultra
- 5:52processed foods aren't just bad for your waistline,
- 5:55they're bad for your brain. They can hijack your
- 5:58dopamine system, making you crave more and more.
- 6:01It's like that one X. You can't quit texting at
- 6:042am you know it's bad for you, but you do it
- 6:06anyway. So what have we learned today? To
- 6:09recap, obesity is a disease. It has
- 6:12been diagnosed as a disease since 2013.
- 6:15It is not a lifestyle choice. Blaming people
- 6:18for their obesity is not only wrong, it's unhelpful
- 6:21and cruel. Two, your brain and hormones
- 6:24are driving the bus and ultra processed foods are out there slashing
- 6:27the tires. Three GLP1 drugs,
- 6:30uh, are game changers. They've shown us that treating obesity
- 6:33as a medical condition can lead to real society, sustainable
- 6:35results. The bottom line, we
- 6:38need to stop shaming people for their weight and start
- 6:41addressing the root causes of obesity, starting with the food environment
- 6:44and the biologic factors that drive
- 6:47overconsumption. And here's one final
- 6:49kicker. You know your body makes its own
- 6:52GLP1. Did you know that ultra
- 6:54processed food starts
- 6:57feeding bacteria in your
- 6:59microbiome that actually destroy the
- 7:02cells that make GLP1? So imagine
- 7:05this. Ultra processed foods not
- 7:08only hack your brain into wanting to eat more, I
- 7:10mean, you can't just eat one, but
- 7:13they also promote a microbiome or the bacteria in your
- 7:16gut that destroy the cells that make your own
- 7:19endogenous GLP1. Oh, by the
- 7:22way, is there something you can do to help
- 7:25your cells make more GLP1 and
- 7:28support a healthy microbiome? Yeah, turns out
- 7:31eating more fiber, specifically a kind of fiber called
- 7:33inulin, can do that. Well, that's
- 7:36all for today's episode of Fork youk. If you've learned one thing, let it
- 7:39be obesity isn't about bad choices. It's about bad
- 7:42systems. And now we understand it better,
- 7:45we can start treating it better.
- 7:46Until next time, I'm Dr. Terry Simpson, your chief medical
- 7:49explanationist, reminding you to love your food, love your body,
- 7:52and remember, it's not about the fork in your
- 7:55hand. It's about what's at the end of it. This
- 7:58episode was written and researched by me, Dr. Terry Simpson.
