- 0:00Hello, everyone. This is Jacob Emerson with the
- 0:02Becker's Pay Your Issues podcast. Thrilled today to
- 0:04be joined by doctor Sariya Cicoccio, who is
- 0:07the chief medical officer at Essence Healthcare.
- 0:10Doctor Cicoccio, thank you so much for taking
- 0:12the time to be with me on the
- 0:13podcast today. Yeah. Jacob, this is gonna be
- 0:16a lot of fun. Looking forward to it.
- 0:18Likewise. And and, Sareya, before we dive into
- 0:20everything we wanna talk with you about, can
- 0:22you first tell us a little bit more
- 0:23about yourself, your background in health care, and
- 0:26what it is that you do today at
- 0:28Essence?
- 0:29You bet. So in my role as the
- 0:31CMO for Essence, I'm responsible for quality and
- 0:35ensuring that our members receive an excellent experience
- 0:39every time in every encounter.
- 0:41So this includes population health management, care management,
- 0:45certainly clinical
- 0:47utilization
- 0:48of the services we have and overall cost
- 0:50of care. And as a family physician by
- 0:53training, I've worn a few hats in the
- 0:54past from being in solo practice, academics,
- 0:57a CMO on the health system side to
- 0:59truly understand how health care is delivered in
- 1:03our communities, and, of course, on the health
- 1:05plan space, which has been so enjoyable and
- 1:08an opportunity to integrate all of these perspectives.
- 1:11It can defocus on the patient, the members
- 1:14that we serve.
- 1:15Wonderful. Well, Sareal, like I said, we really
- 1:17appreciate you taking the time to chat with
- 1:19us today. I wanted to get started, by
- 1:21talking a little bit about Medicare Advantage star
- 1:24ratings. Of course, we're right in the middle
- 1:26of the open enrollment period. Star ratings are
- 1:28now out. Essence did fantastic. Once again, yeah,
- 1:32you've got 80% of your members in four
- 1:34and a half star plans for 2026.
- 1:36So talk to us and our audience a
- 1:39little bit about some of the strategies that
- 1:40have been most effective internally for achieving that,
- 1:43for maintaining
- 1:44those high star ratings,
- 1:46particularly given the fact that you operate in
- 1:48multiple competitive markets like St. Louis, Chicago, Louisville.
- 1:52What's what's gone into those ratings?
- 1:55You bet. And, Jacob, I'll just say we
- 1:58are so very proud. I am particularly proud
- 2:01of our health plan that can consistently deliver
- 2:04high quality care. So thank you for the
- 2:06call out and the opportunity
- 2:08to represent
- 2:10why we do what we do. And most
- 2:11importantly here is how we do it.
- 2:14So how we do this is with strong
- 2:17collaboration
- 2:18and relationships.
- 2:19That is critical.
- 2:21Whether we're working with a multi
- 2:24health system group that reaches across The US
- 2:27or we're dealing with
- 2:29local
- 2:30independent physician
- 2:32advisory or or association,
- 2:34it's critical that we understand how they deliver
- 2:38care.
- 2:39First and foremost, aligning with processes and workflows
- 2:42they have in place,
- 2:43that's so important.
- 2:45I call this the we see you piece
- 2:47of the work that we do and understanding
- 2:49that relationship.
- 2:51We're one of many health plans out there,
- 2:53and we will be ultimately successful if we
- 2:56find ways to align with the best practices
- 2:58that are already in play.
- 3:00Now what we bring to the table as
- 3:02a Medicare Advantage health plan is real time
- 3:05data access.
- 3:06We have partners
- 3:08that have full risk.
- 3:10And, of course, in a full risk relationship,
- 3:13we're accountable for transparency
- 3:15and that real time data for those gaps
- 3:18in care to close
- 3:19and sharing those best practices to help them
- 3:21achieve those goals.
- 3:23Because listen, at at the same time, they
- 3:25care about the same thing we do, and
- 3:28that's the patient receiving high quality of care.
- 3:30So we embrace that. We take that forward
- 3:33with a multidisciplinary
- 3:34team on our end. So we know that
- 3:38STARS is more than just HEDIS. It's more
- 3:40than just the quality, the prevention,
- 3:42the chronic disease that's important, but it's also
- 3:45the medications that our members take and ensuring
- 3:48that they have access to those when they
- 3:50need them, how they need them. Health outcomes,
- 3:53these are the other pieces of the puzzle
- 3:56of what includes their exercise, their mental health,
- 4:00and all the other pieces of that health
- 4:02outcome survey that matters to us and matters
- 4:05to our partners and certainly to our members.
- 4:07And then finally with operations and improvement. And
- 4:10I mentioned each one of these components within
- 4:14STARS
- 4:14because you can't do just one well. You
- 4:17can't just do quality.
- 4:19You have to take care of the entire
- 4:21experience
- 4:22and help these members navigate their path to
- 4:25create the the highest quality of life possible.
- 4:29So you can't do member care without the
- 4:32members at the center,
- 4:33and this is a complex
- 4:40significant role in health care across the board,
- 4:43and that includes member outreach.
- 4:45But we're not the only ones as a
- 4:47health plan. We ensure that we partner
- 4:50with those health care providers
- 4:52so that we're not duplicating messaging, but we're
- 4:55aligning messages together. That includes a systematic and
- 4:59also personalized
- 5:00outreach in the work that we do. So
- 5:02and finally, what I'll say in this section
- 5:04of what helped us be most effective in
- 5:07our higher star ratings
- 5:09is aligning
- 5:10the benefits with the needs of our members.
- 5:12If you don't understand the communities in which
- 5:15they live and where you're serving,
- 5:18it creates a challenge in ensuring that the
- 5:20benefits match
- 5:21what the members need. And that's what we've
- 5:24done really well.
- 5:25Absolutely. And I I appreciate the detail there,
- 5:27Sariyah, in terms of it's it's really understanding
- 5:30the member. It's those strong provider partnerships and
- 5:32then those really regimented processes,
- 5:35that you talked about to really achieve these
- 5:37ratings, which for any of our audience, our
- 5:38audience that might not be familiar, you know,
- 5:40a four and a half star rating or
- 5:42having the majority of your members in in
- 5:44such high rated plans is is very difficult,
- 5:46as you know. There's really only a handful
- 5:49of of health plans around the country that
- 5:50have achieved this.
- 5:52And so I wonder because it's been getting
- 5:55more difficult year after year to maintain and
- 5:57achieve
- 5:58such high ratings,
- 5:59what what are some of the biggest challenges
- 6:01right now,
- 6:02either this year or or as you think
- 6:04about, years to come in achieving these ratings?
- 6:07Because I know that's already starting and in
- 6:09place.
- 6:10What are some of the biggest challenges Essence
- 6:12is navigating in this space right now?
- 6:14Well, first and foremost, I could just say
- 6:17repeat
- 6:18when it comes to the challenges. So the
- 6:19areas where we excel with the strong relationships
- 6:23and the multidisciplinary
- 6:25teams and that specialized message. We have to
- 6:27continue to evolve and get better at the
- 6:29work that we do and why we continuously
- 6:32learn from what we did last year. What
- 6:34we did for our work plan for 2025
- 6:37has changed significantly
- 6:39in 2025,
- 6:40and our team knows in 2026,
- 6:43it will continue
- 6:44to look different. And that's because not only
- 6:47are we striving for perfection, that's what's important
- 6:50here, Every member deserves a 100% quality of
- 6:54care and the experience that they deserve it,
- 6:57and we'll have to keep changing. Now that
- 6:59being said, everybody else is as well. That
- 7:02means the cut points change on stars. This
- 7:04we know, And that can be a frustrating
- 7:07point, which I understand,
- 7:08and I always reflect back to our board
- 7:10and to our team members that this is
- 7:13a good thing. Right? I I think we
- 7:15would all agree that it's critical
- 7:17that we continue to perform better. And not
- 7:20only Essence Healthcare, all Medicare Advantage plans, and
- 7:23in fact, every health plan and every provider,
- 7:27it is imperative that we continue to look
- 7:30for the best ways to deliver that care
- 7:31because that's what our members deserve at the
- 7:33end of the day. Absolutely. And speaking of
- 7:36good things, I mean, these high ratings means
- 7:38more funding for you to invest back into
- 7:41better benefits.
- 7:42So what does that mean moving forward? How
- 7:45how does achieving these ratings,
- 7:47so high, how does that influence benefit design
- 7:50and member engagement moving forward for you all?
- 7:53Well, I appreciate the insight, and you're absolutely
- 7:55right. Year over year should indeed build on
- 7:58top of a previous successful experience for members.
- 8:02Now that does require money to invest back,
- 8:05which means we've gotta put the right care
- 8:07in the right place
- 8:08at the right time.
- 8:10Because in order to invest back in, you've
- 8:13gotta be able to meet the the margins,
- 8:15frankly.
- 8:16Now that being said, what I am thrilled
- 8:19about for 2025
- 8:20for particularly
- 8:22our low income subsidized members
- 8:24is we went back and we asked the
- 8:26physicians, physicians,
- 8:28what do your patients need?
- 8:31And what we heard loud and clear is
- 8:33transportation
- 8:34matters,
- 8:35food matters, social determinants of health across the
- 8:37board are what make a difference. So we
- 8:40have dialed in with our care management team,
- 8:43our social workers
- 8:45to ask them, what are you hearing? What
- 8:47are you experiencing?
- 8:48And what are the members need? And I
- 8:50think this will become more complex
- 8:52as we hear. I mean, you can't
- 8:54avoid listening regardless of if it's social media
- 8:57or the news or wherever you get your
- 8:59information.
- 9:01People are struggling
- 9:02and for many reasons. We need to make
- 9:04sure they have food in their bellies because
- 9:06they shouldn't have to make the decision
- 9:08between medications
- 9:09and their food because food is also medicine.
- 9:12That means we also have to think about
- 9:14cost effective
- 9:15pharmacies that work and community engagement.
- 9:18So, yes, we focus on official benefits, and
- 9:21I named a few just a moment ago,
- 9:23But we have connected
- 9:25more deeply with our mission,
- 9:28Jacob. We are so focused on the communities
- 9:32in which our patients
- 9:33and our members live. And for that reason,
- 9:36we have identified community partners that are helping
- 9:39us understand
- 9:40how these members within their neighborhoods
- 9:43receive the best care. And the best care,
- 9:45as I mentioned, is not just medicine.
- 9:48It's also food. It's shelter. It's security across
- 9:52the board.
- 9:53So those are things that we don't always
- 9:55talk about, but I'm seeing more and more
- 9:57in a very positive way that my peer
- 10:00health plans are doing the same. Let's care
- 10:02about community
- 10:03and serve up an environment that is most
- 10:05beneficial for our members.
- 10:08Absolutely. Addressing those social determinants among your members
- 10:11is is clearly crucial here, and it obviously
- 10:13reflects in your ratings.
- 10:15I wonder, Sariya, I I wanna ask your
- 10:17perspective given that you've been in this industry
- 10:20in different parts,
- 10:22and leading such large organizations
- 10:24or or in parts of them, how do
- 10:26how are you thinking about the relevance
- 10:30of star ratings right now in terms of
- 10:32truly reflecting health plan quality? Do you think
- 10:35that the existing methodology
- 10:37is is effectively capturing the complexities of of
- 10:40care delivery, of coverage,
- 10:42especially for such vulnerable populations that you serve?
- 10:46What's what's your perspective on on the star
- 10:48ratings process right now?
- 10:49Well, thank you for this question, Jacob. And,
- 10:52frankly, we could do a whole podcast
- 10:55on this alone. I'm considering it.
- 10:59Right? How was it calculated and what we
- 11:01shoot for here?
- 11:02But it boils down to this for me.
- 11:05And what I
- 11:07try to inspire
- 11:08my team members to consider and those that
- 11:11we support,
- 11:12at the end of the day, what CMS
- 11:15is doing here and, frankly, other health plans
- 11:18including us, is let's do what matters.
- 11:21And you could take any of these measures
- 11:24of all the components and relate it back
- 11:26to why it matters for health.
- 11:29Do I always agree with, gosh,
- 11:31missing a star by minus
- 11:33point four zero four? No. But I do
- 11:36understand why we have to have cut points.
- 11:38I do understand why we focus on prevention
- 11:41of cancer.
- 11:42It's a significant cost of care, but it
- 11:45is ultimately an impact on the quality of
- 11:47life that we have on this earth. In
- 11:50chronic disease management for the same reason, if
- 11:53you know or have diabetes yourself, you know
- 11:55that that is a challenging disease that it
- 11:58it is incumbent upon us to make sure
- 12:00that we're delivering comprehensive care.
- 12:03Now that being said, the experience is just
- 12:06as important and every member
- 12:08matters.
- 12:09And I oftentimes think of my own mother.
- 12:12I call her Mimi because that's what my
- 12:14kids call her, and she is in her
- 12:16seventies. She is the prime Medicare Advantage patient.
- 12:20And I often ask her, Mimi, what does
- 12:23this mean to you? How would you respond
- 12:25to this type of situation?
- 12:28When you go to speak to your family
- 12:30doctor, what do you ask? What do you
- 12:32need to know? What do you need to
- 12:33hear from them? And it has given me
- 12:36an incredible
- 12:39and realistic view
- 12:41of how difficult it is to navigate care.
- 12:43Because every one of us is different, and
- 12:45not all patients with diabetes respond the same,
- 12:48need the same care plan. And we could
- 12:50walk through health equity for the same reason.
- 12:53Every member voice matters, and what's critical for
- 12:56us is that
- 12:58we do focus on every voice that comes
- 13:00in. But that's what CMS wants to do.
- 13:03It's challenging to do population health when at
- 13:05the same time, it's critical to personalize that
- 13:08care.
- 13:09But at the end of the day, I
- 13:11think that's what you would want for your
- 13:13care. That's what you would want for family
- 13:14and what I certainly expect for mine.
- 13:17Absolutely. And I appreciate you sharing that personal
- 13:19anecdote, with your mother,
- 13:21Soria. It's a it's a great example. And
- 13:23I and I think that leads me into
- 13:25my my last question for you in terms
- 13:27of,
- 13:29how are you perceiving members' actual awareness and
- 13:32utilization
- 13:33of these ratings when they're selecting these plans?
- 13:36Because as you just explained,
- 13:38the the methodology, the the cut points, for
- 13:40example, it's it's very important to somebody in
- 13:42a role like yours, but that, you know,
- 13:44the person on the ground, the Medicare Advantage
- 13:46enrollee likely doesn't know, anything about cut points
- 13:49and how that trickles down to a final
- 13:51star rating. So what are what are you
- 13:53hearing out there about how members are engaging
- 13:55with these ratings today?
- 13:57Well well said. We have to bring it
- 13:59back to the language that matters to them.
- 14:01If you straight out give a star performance,
- 14:03that doesn't mean a whole lot. But when
- 14:05you can reflect back in your communication of
- 14:08why it matters,
- 14:09that your diabetes is controlled and we're able
- 14:12to save your eyesight, when we can focus
- 14:14on
- 14:15your blood pressure and why that matters and
- 14:17finding a health plan that also believes that
- 14:20that's important. We know that we've reduced heart
- 14:22attacks and strokes, and everybody understands the impact
- 14:26of cancer because you've either had it yourself
- 14:28or you know someone personally
- 14:30who has been on that journey.
- 14:31So this is part of understanding what the
- 14:34individual expects
- 14:36and what members are wanting.
- 14:38I have tuned in very closely to our
- 14:40members at this time of year as, of
- 14:42course, we're in the annual enrollment period, and
- 14:45they're telling us. They told us what they
- 14:47care about pharmacy plans. They wanna know that
- 14:49they're remaining stable.
- 14:51When it comes to the benefits available to
- 14:53them or let's say a flex card, they
- 14:54wanna know that it's gonna be consistent next
- 14:56year. And as we see tremendous
- 14:59disruption
- 15:00across the market
- 15:02during this annual enrollment period,
- 15:05it's because
- 15:06members have become more educated
- 15:08and know what they want. And, frankly, they
- 15:10know what they've had most recently.
- 15:13And whenever we pull back benefits,
- 15:15it pullbacks pulls back the benefit to the
- 15:18member.
- 15:19And that's what we need to be mindful
- 15:21of, being stable because that's what they want
- 15:23from us. I don't believe members choose
- 15:26to change plans every year, but their experience
- 15:29is guiding their way. So let's keep it
- 15:31simple in language they understand
- 15:34and language that we understand together and drive
- 15:37back to what impacts their quality of life
- 15:40and their ease of accessing their benefits.
- 15:44Well, Sareya, but before we go, you've got
- 15:46the ears of a lot of other industry
- 15:48leaders listening in from all over the country.
- 15:51Any final thoughts for them? Final bits of
- 15:53advice you wanna share as they navigate,
- 15:57similar challenges
- 15:58in the year ahead?
- 16:00Sure thing. And I'll keep it simple.
- 16:02At the end of the day,
- 16:05people want to be heard,
- 16:07and it's incumbent upon us
- 16:09to listen
- 16:11and then to respond appropriately.
- 16:14And so as I've joined SSENSE and it
- 16:16brings a huge smile to my face when
- 16:18I hear repetitively
- 16:20across departments now, we use the language,
- 16:23you spoke, we listened, here's what's happening.
- 16:27Hearing people out,
- 16:29listening and processing what matters most, and then
- 16:32reflecting back what's changed because of their input
- 16:35creates the strongest relationships
- 16:38and what we're committed to. And I'm very
- 16:40proud of our team for that. It takes
- 16:42the entire team. We also say teamwork truly
- 16:45makes that dream work. So I'm grateful for
- 16:48our teams
- 16:49for the opportunity to celebrate their success
- 16:53and share that message with our members as
- 16:55well.
- 16:56A simple message, but a very impactful
- 16:59one. Doctor Sacoccio, thank you so much for
- 17:01taking the time to join us on the
- 17:03podcast today and for sharing your insights with
- 17:06our listeners. We really appreciate it.
- 17:08My pleasure. And thank you, and be well.
- 17:11You as well. And to our listeners, if
