- 0:30Today we are talking about pain, specifically the physiological theater
- 0:35of the absurd that occurs inside your body every time
- 0:38you stub your toe or suffer a paper cut. It
- 0:41is a truly fascinating, if not entirely exhausting, mechanism of
- 0:45biological alarm bells. According to online sources, pain is essentially
- 0:50a sophisticated, albeit highly annoying, protective system designed to keep
- 0:55us from falling apart. The process kicks off with transduction.
- 0:59This is the moment your body decides that a mechanical,
- 1:02thermal or chemical insult like stepping on a stray lego,
- 1:05is worthy of attention. Your damaged tissues and immune cells
- 1:09start throwing a chemical party, releasing a cocktail of substances
- 1:13like histamine and prostaglandins. These molecules tickle your NO susceptors,
- 1:18which are basically the body's drama queen sensory neurons, forcing
- 1:22them to generate an electrical signal. Once this signal is born,
- 1:26we move into transmission. Think of this as the world's
- 1:29most high stakes game of telephone. The signal travels along
- 1:33a three neuron relay race. First, the NO susceptor carries
- 1:37the message to your spinal cord's dorsal horn. From there,
- 1:41second order neurons shuttle it up to the brain stem

- 3:57localized throb in your skin, bones, or joints. It is
- 4:01the honest, straightforward misery of a stubbed toe or an
- 4:04aching knee. Then we have visceral pain, which is far
- 4:08more elusive and annoying, originating from your internal organs. It
- 4:13is diffuse and maddeningly difficult to pinpoint, much like trying
- 4:17to find meaning in a modern art exhibit. According to
- 4:20various digital compendiums, visceral distress is often a vague, deep
- 4:25seated ache that defies your attempts to locate its exact source.
- 4:30Then there is neuropathic pain, which is the nervous system
- 4:34experiencing a total glitch in its firmware. It does not
- 4:38require an actual injury to occur. Instead, your nerves decide
- 4:42to start firing off electrical signals for no reason at all,
- 4:45creating that delightful sensation of shooting, burning, or pins and needles.

- 6:59code for a spy agency, but is actually just a
- 7:01way to force us to speak coherently while we are
- 7:04busy suffering. O is for onset, because apparently we need
- 7:08a time stamp on our agony. P stands for provoking
- 7:12or alleviating factors, which is a polite way of asking
- 7:15what you did to make the situation worse. Q is quality,
- 7:19where you are invited to describe your pain as if
- 7:22you were auditioning for a role in a Gothic tragedy.
- 7:26Is it stabbing, burning, or perhaps just a dull existential ache.
- 7:31R is for region and radiation. Tracking the migration patterns
- 7:35of your discomfort. S is severity, the classic zero to
- 7:39ten scale that assumes we all have a standardized internal
- 7:42metric for misery, which is quite the humorous leap of faith. Finally,
- 7:47TEA is timing, capturing whether your suffering has evolved into
- 7:50a long term, unwonted companion. Of course, we cannot rely
- 7:55on a simple number for everyone. Internet accessible documentation suggests
- 7:59that that medicine has developed an array of scales to
- 8:02accommodate those who find the numeric approach insufficient or impossible,
- 8:07we have descriptive scales for the verbose, and for those
- 8:10who cannot or will not use words, we resort to
- 8:13picture based systems like the Wongbaker Faces scale. There is
- 8:16something deeply ironic about asking a patient to point at
- 8:19a cartoon face to define their level of torment. It
- 8:23is a strange, whimsical, and slightly surreal theater. We also
- 8:27have behavioral scales for those who are nonverbal, essentially turning
- 8:30the assessment of human anguish into a game of observation
- 8:33and guessing. From the perspective of clinical data, these tools
- 8:37are our best attempt at quantifying the unquantifiable, a task
- 8:41that remains inherently paradoxical. We try to pin down the chaotic,
- 8:45subjective reality of pain with these rigid, structured frameworks, creating
- 8:50a bureaucratic dance that is as necessary as it is
- 8:53perpetually prone to failure. Given our collective obsession with quantifying
- 8:58the ethereal nature of discomfort, we eventually arrive at the
- 9:01management phase, a realm where modern medicine tries to play
- 9:05god with a prescription pad and a yoga maat. It
- 9:09is a delightfully absurd spectacle. Really, we attempt to dampen
- 9:13the firing of neurons with a cocktail of chemicals, while
- 9:16simultaneously hoping that a bit of aromatherapy or a deep
- 9:20tissue massage might convince the brain to stop screaming. According
- 9:24to various public medical archives, the pharmacological arsenal is quite
- 9:29the grab bag. We start with the basics, those non
- 9:32opioids like acetaminofin or aspirin, which act as the polite
- 9:36request for the pain to kindly vacate the premises. When
- 9:39that inevitably fails, we escalate to the heavy hitters, the opioids,
- 9:44which essentially tell the brain to stop caring about the agony,
- 9:47a strategy that is as effective as it is fraught
- 9:50with its own brand of dark comedy. Then we have
- 9:54the so called adjuvants. These are the medications that weren't
- 9:57invited to the pain party, like antidpress or anti epileptics,
- 10:02yet find themselves pressed into service to modulate the neurological chaos.
- 10:07It is truly a masterclass in medical improvisation. But wait,
- 10:12we cannot forget the non pharmacological theater. Internet based clinical
- 10:16overviews suggests that while we are busy suppressing signals with pills,
- 10:21we are also engaging in what can only be described
- 10:25as a ritualistic dance of comfort. We use heat and cold,
- 10:28therapeutic touch, or even acupuncture, which is essentially the practice
- 10:33of inviting someone to pokey with needles in the hopes
- 10:36that your body decides the original pain is no longer
- 10:39the priority. It is a surreal juxtaposition, isn't it. We
- 10:43rely on high tech pharmacology while simultaneously dabbling in ancient,
- 10:48holistic traditions, hoping that some combination of the two will
- 10:52finally quiet the somatosensory cortex. From the vantage point of
- 10:56current health care literature, the strategy is often to grow
- 11:00everything at the wall and see what sticks, creating a
- 11:03multimodal approach that acknowledges the sheer, messy complexity of being human.
