- 0:04>> Dr. Terry Simpson: This is sort of episode two of our miniseries about
- 0:07food isn't a prescription pad. We've already
- 0:09debunked in episode one the idea that food is
- 0:12medicine. You may remember that Hippocrates never
- 0:15said that, no matter how many times a wellness
- 0:17influencer might report that or RFK
- 0:20Jr saying it. Well, again, if RFK
- 0:23Jr is your fact checker, you are in a world of
- 0:26hurt. But here's the thing. Food is
- 0:29powerful. Food is empowering.
- 0:31Lifestyle changes continue to be
- 0:34the cornerstone upon which physicians
- 0:37try to help you. The right foods can
- 0:40actually lower your blood pressure. And there's an entire diet
- 0:43that was designed for it called the DASH diet. And it
- 0:46works, just not as well as actual medicine.
- 0:49So today we're going to talk about bananas, beans, leafy
- 0:51greens, but why they might not replace
- 0:54your hydrochlorothiazide or lisinopril or beta
- 0:57blocker.
- 1:01I'm your Chief Medical Explanationist, Dr. Terri Simpson,
- 1:04and this is Fork U Fork University,
- 1:07where we bust a few myths, make sense of the
- 1:10madness, and teach you a little bit about food
- 1:13and medicine.
- 1:17DASH stands for Dietary Approaches to Stop
- 1:20Hypertension. It's often called America's version
- 1:23of the Mediterranean diet. Why? Because when
- 1:26researchers were shaping the Mediterranean diet
- 1:28guidelines, another group asked narrower question.
- 1:31What foods and what food groups could directly lower blood
- 1:34pressure? And here's what made dash
- 1:37different. Researchers didn't just hand out
- 1:39pamphlets. They cooked all the food for the
- 1:42participants. Every meal, every snack,
- 1:45every day. That level of control is
- 1:48expensive and hard to do, but it's why the
- 1:51DASH sodium studies are so powerful.
- 1:54They could say with confidence, this diet
- 1:57lowered blood pressure. The DASH diet
- 1:59emphasizes fruit, vegetables, especially
- 2:02potassium rich fruits like bananas,
- 2:04beans, leafy greens, whole grains, lean
- 2:07proteins, fish, poultry, and low fat dairy.
- 2:10And more importantly, cutting back on sodium
- 2:13sweets and cutting back on red meat.
- 2:16And DASH works because potassium, magnesium and
- 2:19calcium relax blood vessels and balance
- 2:21sodium. The leafy greens and things like beets
- 2:24actually help your body produce more nitrous oxide.
- 2:28Fiber helps too. So when you
- 2:30flood your body with these nutrients, your arteries get
- 2:33less stiff and your blood pressure will come down.
- 2:37In trials, the DASH diet lowered
- 2:39systolic blood pressure. That's the top number, by
- 2:42about 11 millimeters of mercury.
- 2:45That's about the same thing as taking some blood pressure
- 2:48pills. We actually have Talked about the DASH diet
- 2:51previously in episode 46 of 4U, and it's
- 2:54worth repeating. This is one of the most
- 2:57studied diets out there. In the original
- 3:00DASH trial, blood pressure dropped within
- 3:03two weeks. That's Quick. Other
- 3:06studies following it confirmed the same thing.
- 3:09An 8 to 11 millimeter reduction in systolic and a
- 3:125 to 6 millimeter reduction in the diastolic or
- 3:14lower number. Here's the kicker. It
- 3:17worked best when paired with low sodium
- 3:20intake. This was one of the first diets to
- 3:23clearly show that less salt meant lower
- 3:26blood pressure. Now there's people who think that
- 3:29unlimited salt is just fine. They're
- 3:32wrong. Excess sodium is
- 3:34directly tied to high blood pressure or heart
- 3:37disease, stroke, and even stomach
- 3:40cancer. Now let's tackle
- 3:43the exercise angle. A lot of folks
- 3:46buy salty electrolyte drinks, then think they
- 3:49need them every time they work out. For most people
- 3:51who exercise less than an hour, you don't need
- 3:54electrolytes. Water is fine. Where
- 3:57electrolytes matter is prolonged or high
- 4:00intensity exercise, usually more than one or
- 4:02two hours, especially in the heat or with heavy sweating.
- 4:06Think marathon runners in Phoenix, Arizona in
- 4:09the summer. That just is torture. But it actually
- 4:12happens. But that's when sodium helps prevent
- 4:15hyponatremia, which is a dangerous
- 4:17condition where over drinking plain water
- 4:20dilutes your body's sodium. But here's a
- 4:23catch. Many sports drinks don't even have enough
- 4:26sodium to replace what you've lost. In those cases,
- 4:29salty snacks or actual food work better.
- 4:31And after exercise, if you're eating a normal meal, plain
- 4:34water plus food is usually enough for
- 4:37recovery. And now the scam.
- 4:41You've seen them shirtless salesmen on
- 4:44Instagram selling overpriced mango flavored
- 4:46salt packets as if they're miracle electrolyte
- 4:49replacements, hawking them on streets as people go
- 4:52off to the shore to try and enjoy a day in the
- 4:55sun. It's just nonsense on
- 4:57stilts. The Dash trials proved
- 5:00beyond argument that excess sodium
- 5:03raises blood pressure and with it, the risk
- 5:06of heart disease and stroke. So while these
- 5:09bros tell you salt is the fountain of life,
- 5:12what they're really selling is a faster trip to your
- 5:15cardiologist. Here's the
- 5:17electrolyte replacement has a role, but in
- 5:20very specific settings.
- 5:22Prolonged endurance exercise,
- 5:25heavy sweating, extreme heat. For almost
- 5:28everyone else, it's unnecessary. And for
- 5:31some, it's dangerous. So
- 5:34who's most at risk? People with high blood pressure?
- 5:36People with heart failure? People with kidney disease because more
- 5:39salt means more fluid retention and more
- 5:42complications. Women, smaller
- 5:45body size and lower sweat rates make
- 5:48them more likely to over hydrate and develop
- 5:51hypon. Older
- 5:54athletes and those with low body mass, the
- 5:56margins are even slimmer. Those dudes might
- 5:59actually be hurting themselves. And recreational athletes,
- 6:02where most of us as weekend warriors,
- 6:05we need to not be following that outdated advice
- 6:08to drink as much as possible. Those are the ones who
- 6:11end up over hydrated, not the pros.
- 6:14But even in ultra endurance events,
- 6:17sodium supplementation does not reliably
- 6:20prevent hyponatremia if you drink more than
- 6:22you sweat out. In fact, it can make the
- 6:25problem worse. The Wilderness Medical
- 6:28Society and the American College of Sports Medicine
- 6:31both say the same thing. Sodium should only
- 6:34be added when sweat losses are high. And fluids should
- 6:37be guided by thirst, not by some
- 6:39influencer's affiliate code. So yes,
- 6:42if you're running a hundred mile ultramarathon in the
- 6:45desert, sodium may have a role.
- 6:48But buying the mango flavored salt packets for a
- 6:51half hour jog? You're not an athlete. You're
- 6:54a customer.
- 6:55So what does a DASH look like in real life? On your plate, Getting
- 6:58away from the shirtless salesman of supplements and scams.
- 7:01Breakfast. A bowl of oatmeal and berries and a banana
- 7:04pan full of unsalted nuts. Lunch. Black
- 7:07beans, lentil soup, veggie salad, vinaigrette,
- 7:10grilled salmon, side of leafy greens, maybe brown
- 7:13rice, and maybe a little bit of extra low fat
- 7:16yogurt for some calcium. That's a day on the DASH
- 7:19diet. Did you notice? No salty canned soup, no
- 7:22fast food fries, no processed snack. And here's the challenge.
- 7:25It's not about eating one banana. It's about
- 7:28changing your entire eating pattern.
- 7:32Now let's compare it to medication.
- 7:34One pill, like an ACE inhibitor, calcium channel
- 7:37block, or diuretic can lower your systolic blood pressure by
- 7:4020 to 40 millimeters of mercury, which is double
- 7:43or triple what dash can do. And here's the other difference.
- 7:46Meds don't depend upon whether you ate, ah, your kale salad today.
- 7:50They aren't concerned if you decided to have some french
- 7:52fries that were extra salty. They work consistently
- 7:55every time you take them. So, yes, DASH
- 7:58helps, but it's not going to replace your beta blocker, your
- 8:01ACE inhibitor, your calcium channel blocker, your diuretic.
- 8:04Here's a takeaway. The DASH diet's
- 8:07fantastic. I endorse it completely. It
- 8:10lowers blood pressure, improves overall health, and pairs
- 8:12beautifully with medicine. But if you're
- 8:15trying to control high blood pressures with bananas and beans
- 8:18alone, you're setting yourself up for
- 8:21disappointment. Food helps,
- 8:23medicine saves. And please
- 8:26do not buy into the myth that unlimited salt is
- 8:29healthy or that you should get the pink
- 8:31Himalayan salt. High sodium raises
- 8:34blood pressure. DASH studies proved it. Electrolyte
- 8:37drinks, save them for marathoners, not, uh,
- 8:39Morning walkers. And the sweet
- 8:42spot is always food and
- 8:45medicine. So when you see that shirtless salesman
- 8:48of supplement and scam standing on the side of road saying, hey, you want
- 8:51some of these electrolyte drinks? Let me give you some for free. You can buy it on
- 8:53Amazon. Walk on by. Enjoy
- 8:56your bottled water.
- 9:01This has been episode two of, uh, Food is in a Prescription Pad.
- 9:04Next time, when salmon is in a stent, how, uh, the
- 9:06Mediterranean diet lowers risk but still doesn't open
- 9:09blocked arteries. I'm Dr. Terry Simpson, your
- 9:12chief medical explanationist. Remember, bananas are great, but they're
- 9:15not beta blockers. Fork U is
- 9:17produced, researched and written by me, Dr. Terri
- 9:20Simpson. It is distributed by our friends at Simpler Media
- 9:23and the pod God, Mr. Evotera. For references

- 9:43talk to your own doctor and registered dietitian
- 9:46before making changes to your health. Especially if you're going
- 9:49to buy one of those silly electrolyte packets because you
- 9:52actually harm yourself. Get to know your
- 9:55doctor. Not a chiropractor, not an eastern shame.
- 9:58Alright, everybody, have a good week.
- 10:07You know, Ivo, some folks think that if they eat bananas, beans
- 10:10and kale, they can toss out their blood pressure meds. That's like saying you don't need
- 10:13a seatbelt. If you drive carefully, it works till the crash.
- 10:16And the same goes for those mangroves flavored salt packet
- 10:19sold by the shirtless Instagram Bros. It's not
- 10:22performance science, that's performance scam. I'll
- 10:25stick with my meds, my bananas, and keep the show going
- 10:28because we're only on episode two of this series
- 10:31and we've got like a hundred to go
- 10:34and. No, the pension plan
- 10:36is we just die with our boots on, buddy.
- 10:41Aw.
- 10:41>> Speaker B: Uh, damn. Oh, well, I get to save a lot
- 10:44of money on those sugary sports drinks, I guess. Though
- 10:47I might spend them all on bananas. That's bananas.
- 10:50See what I did there? Yeah.
