Transcript
- 0:00SPEAKER_00: A new glaucoma drug is approaching FDA approval.
- 0:03SPEAKER_00: Can it lower eye pressure better than today's best drops?
- 0:06SPEAKER_00: Evidence reviewed through September 10, 2026.
- 0:09SPEAKER_00: Bottom line NCX 470, now called K911, with the proposed generic name Bimataprost Grenode, has been accepted for Food and Drug Administration Review.
- 0:20SPEAKER_00: The new drug application was submitted by COA on June 30, 2026, accepted for review on August 27, 2026, and assigned a Prescription Drug User Fee Act decision date of April 30, 2027.
- 0:36SPEAKER_00: It is not yet approved.
- 0:38SPEAKER_00: COA FDA Acceptance Announcement.
- 0:41SPEAKER_00: The clinical evidence shows that NCX470 lowers intraocular pressure by approximately 8 to 10 millimeters of mercury in patients starting with pressures around 26 to 28 millimeters of mercury, produces roughly 0.5 to 0.8 millimeters of mercury, more reduction than latanoprost in the strongest published phase 3 dataset, performs better than latanoprost at several individual time points, but the average advantage is modest rather than dramatic.
- 1:08SPEAKER_00: Has more conjunctival hyperemia or eye-redness than latanoprost, has not been directly compared with the bioprost, latanoprostine bunod, rocklatin, or cosopht in a pivotal head-to-head trial.
- 1:21SPEAKER_00: Has not been adequately studied in patients starting with pressures in the mid-teens or in patients who need targets near 10 to 12 millimeters of mercury.
- 1:31SPEAKER_00: My overall judgment, NCX470 appears to be a meaningful incremental improvement over latanaprost, but not a breakthrough that is likely to replace combination therapy, selective laser trabeculoplasty, or glaucoma surgery.
- 1:45SPEAKER_00: Current regulatory status.
- 1:48SPEAKER_00: For the full table, please open this article on visualfieldtest.com.
- 1:53SPEAKER_00: FDA acceptance means the application was considered complete enough for formal review.
- 1:58SPEAKER_00: It does not mean that approval is guaranteed.
- 2:00SPEAKER_00: COA product pipeline.
- 2:02SPEAKER_00: COA FDA announcement.
- 2:04SPEAKER_00: The application is supported principally by the Mont Blanc and Dinali Phase III trials.
- 2:09SPEAKER_00: A small Phase 3B study called Whistler examined the drug's effect on aqueous humor outflow, but was exploratory and was not required for the new drug application.
- 2:18SPEAKER_00: NYCOX pre-new drug application update.
- 2:21SPEAKER_00: What is NCX470?
- 2:23SPEAKER_00: NCX470 is a nitric oxide donating version of bimatoprost.
- 2:28SPEAKER_00: After the molecule is exposed to esterases in the eye, it releases bimatoprost-related prostaglandin activity, which primarily increases uveoscleral outflow.
- 2:37SPEAKER_00: Nitric oxide, which is intended to relax the trabecular meshwork and improve drainage through the conventional trabecular meshwork schlem canal pathway.
- 2:46SPEAKER_00: In simplified terms, bematoprost helps fluid leave through the uveoscleral root.
- 2:52SPEAKER_00: Nitric oxide attempts to improve the eye's conventional drainage system.
- 2:56SPEAKER_00: This is conceptually similar to latanoprosin bunod, marketed as vasulta, which combines a prostaglandin effect with nitric oxide donation.
- 3:05SPEAKER_00: The difference is that vasulta releases a latanoprost-derived prostaglandin, whereas NCX470 releases bimatoprost-related activity.
- 3:15SPEAKER_00: NCX470 mechanism and preclinical research, Mont Blanc publication, the Pivotal Clinical Trials Mont Blanc Phase III trial.
- 3:23SPEAKER_00: The Mont Blanc study was a randomized, double-masked, multi-center phase III trial conducted at 56 United States sites and one site in China.
- 3:32SPEAKER_00: A total of 691 patients were randomized.
- 3:35SPEAKER_00: Mont Blanc Publication Clinicaltrials.gov record treatment groups.
- 3:39SPEAKER_00: NCX470 0.065% once daily, NCX470-0.1% once daily.
- 3:45SPEAKER_00: Latanopost 0.05% once daily.
- 3:48SPEAKER_00: The 0.065% arm included 30 initial patients and was stopped after the planned interim analysis.
- 3:54SPEAKER_00: The final efficacy comparison involved NCX 470 0.1%, 328 patients, Latanoprost 0.05%, 333 patients.
- 4:04SPEAKER_00: The drops were administered to both eyes, generally in the evening.
- 4:08SPEAKER_00: Baseline intraocular pressure.
- 4:10SPEAKER_00: The final treatment groups had very similar baseline values.
- 4:14SPEAKER_00: For the full table, please open this article on VisualFieldTest.com.
- 4:18SPEAKER_00: These patients had relatively high baseline pressures.
- 4:21SPEAKER_00: They were required to have untreated or washed-out pressure of at least approximately 26 millimeters of mercury at 8 a.m., 24 at 10 a.m., and 22 at 4 p.m.
- 4:34SPEAKER_00: ClinicalTrials.gov Results.
- 4:36SPEAKER_00: Mont Blanc responder analysis, primary endpoint.
- 4:40SPEAKER_00: The primary endpoint was the reduction from time-matched baseline pressure at 8 a.m.
- 4:45SPEAKER_00: and 4 p.m., week 2, week 6, month 3.
- 4:48SPEAKER_00: The primary statistical goal was non-inferiority to Latanoprost.
- 4:53SPEAKER_00: A secondary analysis examined whether NCX 470 was statistically superior.
- 4:58SPEAKER_00: Montblanc Actual Numerical Results.
- 5:01SPEAKER_00: The table below uses the reported mean reductions from baseline.
- 5:05SPEAKER_00: Percent reductions are calculated from the published baseline values.
- 5:08SPEAKER_00: For the full table, please open this article on VisualFieldtest.com.
- 5:12SPEAKER_00: The reductions were statistically significant compared with baseline at every evaluated time point.
- 5:18SPEAKER_00: NCX470 was numerically better than Latanoprost at all six time points and statistically better at four of them.
- 5:26SPEAKER_00: Clinical Trials.gov Numerical Results.
- 5:29SPEAKER_00: Mean Diurnal Pressure Results.
- 5:31SPEAKER_00: The secondary mean diurnal pressure analysis showed.
- 5:34SPEAKER_00: For the full table, please open this article on VisualFieldTest.com.
- 5:38SPEAKER_00: This suggests that the average incremental benefit over Litanoprost was approximately 0.5 to 0.8 millimeters of mercury, with an average across these visits of about 0.64 millimeters of mercury.
- 5:51SPEAKER_00: That is a real difference.
- 5:53SPEAKER_00: However, it is not the type of difference that usually transforms a patient from needing several medications to needing none.
- 6:01SPEAKER_00: Denali phase 3 trial.
- 6:03SPEAKER_00: Dinali was the second pivotal phase 3 trial.
- 6:06SPEAKER_00: It randomized 696 patients at 90 sites in the United States and China.
- 6:10SPEAKER_00: NCX 470.1%, 348 patients.
- 6:14SPEAKER_00: Litanoprost 0.5 cents, 348 patients.
- 6:19SPEAKER_00: Treatment was once daily in both eyes.
- 6:21SPEAKER_00: All patients were followed through approximately six months, while a subset contributed safety data through 12 months.
- 6:27SPEAKER_00: Dinali Trial Summary, Clinicaltrials.gov Dinali Record, Baseline Pressure.
- 6:32SPEAKER_00: For the full table, please open this article on visualfieldtest.com.
- 6:37SPEAKER_00: Reported efficacy across the six pre-specified efficacy time points.
- 6:41SPEAKER_00: NCX470 reduced pressure by 7.9 to 10.0 mm of Mercury.
- 6:47SPEAKER_00: Latina Prost reduced pressure by 7.1 to 9.8 mm of Mercury.
- 6:52SPEAKER_00: NCX 470 was numerically better at 5 of 6 time points.
- 6:56SPEAKER_00: The difference favored NCX 470 by as much as 0.8 mm of Mercury.
- 7:01SPEAKER_00: Statistical superiority was demonstrated at 3 of 6 time points, but the overall superiority endpoint was not achieved.
- 7:09SPEAKER_00: Thus, Dinali confirmed the general pattern seen in Mont Blanc.
- 7:13SPEAKER_00: NCX470 is at least as effective as Latanoprost and sometimes modestly better, but the average advantage is not large.
- 7:21SPEAKER_00: Dinali topline results, comparison with other glaucoma medications.
- 7:26SPEAKER_00: Cross-trial comparisons must be interpreted cautiously.
- 7:29SPEAKER_00: The studies use different entry pressures, populations, follow-up periods, statistical methods, and treatment schedules.
- 7:37SPEAKER_00: A drug that appears stronger in one trial may have been tested in patients with higher starting pressure.
- 7:42SPEAKER_00: Quantitative comparison.
- 7:44SPEAKER_00: For the full table, please open this article on VisualFieldTest.com.
- 7:48SPEAKER_00: Sources include the first-line glaucoma medication network meta-analysis, Visulta Clinical Review, Rocket Netarseutal Trials, Rocket 4, Mercury Rock Late and Pooled Analysis, and Fixed Combination Medication Meta Analysis, NCX 470 versus Latanoprost.
- 8:06SPEAKER_00: This is the best established comparison.
- 8:08SPEAKER_00: NCX470 appears to provide approximately 0.5 to 0.8 millimeters of mercury more mean diurnal reduction in Mont Blanc, approximately 0 to 0.8 mm of Mercury more reduction in Denali.
- 8:22SPEAKER_00: A somewhat stronger morning effect, but the difference at 8 a.m.
- 8:26SPEAKER_00: was inconsistent and small, ranging from 0.22 to 0.58 millimeters of mercury in Mont Blanc.
- 8:33SPEAKER_00: The advantage is therefore statistically credible but clinically modest.
- 8:37SPEAKER_00: NCX470 versus Bimatoprost.
- 8:40SPEAKER_00: This question is central because NCX470 releases bimatoprost-related activity.
- 8:45SPEAKER_00: NCX470 has not been directly compared with marketed bimatoprost in a large pivotal human trial.
- 8:52SPEAKER_00: A network meta-analysis of first-line drugs estimated that bimatoprost reduced pressure by approximately 5.61 mm of mercury compared with 4.85 mm of mercury for latanoprost, a difference of approximately 0.776 mm of mercury.
- 9:09SPEAKER_00: Network meta-analysis.
- 9:11SPEAKER_00: That is important because the average NCX470 advantage over latanoprost, roughly 0.5 to 0.8 mm of mercury, is similar in size to the historical advantage often reported for bimatoprost over latanoprost.
- 9:26SPEAKER_00: Therefore, the pivotal trials do not prove that nitric oxide adds a large benefit beyond bimatoprost itself.
- 9:33SPEAKER_00: Some of the observed difference may come from the bimatoprost component rather than the nitric oxide component.
- 9:39SPEAKER_00: NCX470 versus Visalta, Visalta, or Latanoprostin Bunod, is the closest mechanistic competitor because it also combines a prostaglandin effect with nitric oxide donation.
- 9:50SPEAKER_00: In the Apollo and lunar trials, mean baseline diurnal pressure was approximately 26.7 mm of mercury for latanoprostin bunod and 26.5 mm of mercury for timolol.
- 10:03SPEAKER_00: Three-month mean pressure was approximately 17.8 mm of mercury with latanopostin bunaud and 19.1 mm of mercury with timolol.
- 10:14SPEAKER_00: The implied reduction was approximately 8.9 mm of mercury with latanopostin bunaud compared with approximately 7.4 mm of mercury with timolol.
- 10:25SPEAKER_00: These values are broadly similar to NCX470's mean reduction in Mont Blanc.
- 10:30SPEAKER_00: But the trials were not head-to-head, so it is not scientifically valid to claim that NCX470 is better than Vizalta.
- 10:38SPEAKER_00: My interpretation NCX470 is not a wholly new therapeutic concept.
- 10:44SPEAKER_00: It is a potentially stronger or differently optimized version of the same general strategy.
- 10:50SPEAKER_00: Its real clinical differentiation will depend on whether its bimatoprost component produces better results than latanoprostine Bunod's latanoprost component and whether that difference survives real-world use.
- 11:02SPEAKER_00: NCX470 versus netarsudal.
- 11:04SPEAKER_00: Netarseudal has a different mechanism.
- 11:06SPEAKER_00: It inhibits rho kinase and improves conventional outflow.
- 11:10SPEAKER_00: In Rocket IV, patients received either netarseutal once daily or timolol twice daily.
- 11:16SPEAKER_00: Nidarcidal, netarcidal 3.9 to 4.7 mm of mercury, timolol 3.8 to 5.2 mm of mercury.
- 11:24SPEAKER_00: Nitarsidol was not inferior to timolol in the studied population, but its major weakness was redness.
- 11:30SPEAKER_00: Conjunctival hyperemia occurred in approximately 48 to 54% of netarseudal treated patients in phase 3 studies.
- 11:37SPEAKER_00: Timolol associated hyperemia was approximately 8% to 11%.
- 11:43SPEAKER_00: NCX470 appears substantially less hyperemic than netarcudil, although it is more hyperemic than latanoprost.
- 11:50SPEAKER_00: Rocket 1 and rocket 2 rocket 4, NCX470 versus Rocklatan.
- 11:54SPEAKER_00: Rocklatan combines latanoprost with netarcidal.
- 11:57SPEAKER_00: In pooled Mercury 1 and Mercury II results, baseline mean diurnal pressure was approximately 23.6 millimeters of mercury with rock latan.
- 12:06SPEAKER_00: Month 3 mean diurnal pressure was approximately 15.8 millimeters of mercury.
- 12:12SPEAKER_00: The calculated reduction was approximately 7.8 millimeters of mercury, or about 33%.
- 12:18SPEAKER_00: Latanoprost alone reduced pressure from approximately 23.5 to 17.3 mm of mercury, or about 6.2 millimeters of mercury, approximately 26%.
- 12:29SPEAKER_00: Rocklaton therefore produced approximately 1.6 millimeters of mercury more reduction than latanoprost in that pooled analysis, larger than the average NCX-470 advantage over latanoprost.
- 12:41SPEAKER_00: However, Rocklatin caused much more redness, approximately 41% nitarsudil alone, approximately 53% with the fixed combination, approximately 14% with latanoprost alone.
- 12:53SPEAKER_00: This creates a practical trade-off.
- 12:55SPEAKER_00: Rocklatin may lower pressure more, but NCX470 may be easier to tolerate.
- 13:00SPEAKER_00: The drugs have not been directly compared.
- 13:02SPEAKER_00: Mercury pooled analysis.
- 13:04SPEAKER_00: NCX470 versus timolol and COSOPT.
- 13:07SPEAKER_00: Timolol alone generally lowers pressure less than prostaglandin therapy.
- 13:12SPEAKER_00: A network meta-analysis estimated a mean three-month reduction of approximately 3.7 mm of mercury.
- 13:19SPEAKER_00: First line treatment network meta-analysis.
- 13:21SPEAKER_00: COSOPT, the fixed combination of dorzolamide and timolol, is more powerful than either component alone.
- 13:28SPEAKER_00: A pooled meta-analysis found approximate mean diurnal reductions of dorzolamide timolol, 7.41 mm, brinzolamide, 8.33 mm, latanoprost timolol, 8.85 mm, bromatoprost timolol, 8.40 mm, bromonidine timolol, 6.55 mm.
- 13:52SPEAKER_00: These are not direct comparisons with NCX470 and come from older trials with different populations.
- 13:59SPEAKER_00: Still, they suggest that NCX470 monotherapy may approach the pressure reduction of some fixed combinations, but it has not been shown to replace combination therapy in patients who need very low targets.
- 14:11SPEAKER_00: Fixed combination meta-analysis, morning, daytime and nocturnal pressure control.
- 14:16SPEAKER_00: Morning effect, NCX470's strongest relative advantage over Litanoprost, appeared at some 4 p.m.
- 14:23SPEAKER_00: measurements, but there was also a modest morning benefit.
- 14:26SPEAKER_00: Week 2 at 8 a.m, 0.58 mm of mercury.
- 14:31SPEAKER_00: Week 6 at 8 a.m, 0.36 mm of mercury.
- 14:36SPEAKER_00: Month 3 at 8 a.m, 0.22 mm of mercury.
- 14:41SPEAKER_00: The absolute morning reductions with NCX 470 were approximately 9.4 to 9.7 mm of mercury or about 33% to 34% from baseline.
- 14:52SPEAKER_00: The 8 a.m measurement is useful because it occurs after overnight drug exposure, but it is not the same as measuring pressure during sleep or in the supine position.
- 15:02SPEAKER_00: Daytime profile Mont Blanc measured pressure at 8 a.m, 10 a.m and 4 p.m.
- 15:08SPEAKER_00: The drug showed a fairly consistent effect across the daytime schedule.
- 15:13SPEAKER_00: The published primary results however do not provide a full 24 hour pressure curve.
- 15:18SPEAKER_00: Nocturnal evidence there is currently no convincing direct nocturnal pressure dataset for NCX470.
- 15:25SPEAKER_00: The pivotal trials did not measure midnight pressure, supine pressure, early morning sleep associated pressure peaks, full 24-hour pressure curves.
- 15:36SPEAKER_00: Studies of bimatoprost and latanapost have examined 24-hour pressure, but those findings cannot be assumed to represent NCX470.
- 15:45SPEAKER_00: 24-hour bimatoprost versus latanaprost study for patients whose greatest pressure problem occurs at night, NCX470 remains an interesting hypothesis, not a proven solution.
- 15:56SPEAKER_00: Responder data and clinically important pressure targets.
- 16:00SPEAKER_00: Reduction of at least 10 mm of mercury.
- 16:03SPEAKER_00: A Mont Blanc responder analysis reported the proportion of eyes achieving at least a 10 mm reduction from baseline.
- 16:11SPEAKER_00: For the full table please open this article on Visualfieldtest.com.
- 16:15SPEAKER_00: These results suggest that more patients received a large pressure reduction with NCX470.
- 16:21SPEAKER_00: However, the all visits numbers represent observed time points across visits, not necessarily the proportion of patients who maintain the response at every visit.
- 16:32SPEAKER_00: A rough time point based calculation gives 9.5 percentage points of additional responders, approximately 11 patients treated for one additional responder, 11.3 percentage points of additional responders, approximately 9 patients treated for one additional responder.
- 16:50SPEAKER_00: These are not formal patient level numbers needed to treat.
- 16:54SPEAKER_00: They should not be interpreted as proof that treating 9 patients with NCX470 will prevent one patient from needing another medication.
- 17:02SPEAKER_00: Mont Blanc responder analysis Responder poster with numerical table, reaching 1815, 12, or 10 millimeters of mercury.
- 17:12SPEAKER_00: The Mlanc responder analysis found statistically more NCX 470 treated eyes reaching mean diurnal pressure thresholds of 18 mm of mercury or lower, 17 or lower, 16 or lower, 15 or lower, 14 or lower.
- 17:28SPEAKER_00: However, the publicly available poster provides the most detailed information as graphs rather than a complete numerical table.
- 17:35SPEAKER_00: The trial did not provide robust published responder data for 12 mm of mercury or lower, 10 mm or lower.
- 17:45SPEAKER_00: This is a major limitation for advanced glaucoma.
- 17:48SPEAKER_00: Starting from a baseline near 27, average treatment with NCX470 brings pressure to around 18 mm of mercury, not 10 to 12.
- 17:58SPEAKER_00: Does nitric oxide add meaningful outflow what the mechanism studies show.
- 18:03SPEAKER_00: The Whistler Phase 3B study included only 18 healthy volunteers or people with ocular hypertension.
- 18:10SPEAKER_00: Baseline pressure was approximately 16.6 millimeters of mercury in the NCX470 group and 16.9 millimeters of mercury in the placebo group.
- 18:20SPEAKER_00: After eight days, aqueous humor flow changes trended toward significance with a reported probability value of 0.072.
- 18:28SPEAKER_00: Outflow facility favored NCX470 at 1 pm with a probability value of 0.081.
- 18:35SPEAKER_00: Outflow facility was statistically significant at 3 pm with a probability value of 0.001.
- 18:42SPEAKER_00: Diurnal outflow was statistically significant with a probability value of.004.
- 18:48SPEAKER_00: Uvioscleral outflow and pressure reduction were statistically significant.
- 18:53SPEAKER_00: Episcleral venous pressure did not show a meaningful trend.
- 18:57SPEAKER_00: These findings support the idea that NCX470 affects both conventional and uveoscleral outflow.
- 19:03SPEAKER_00: Whistler results preclinical experiments also found that NCX470 increased both conventional outflow facility and uveoscleral outflow more than bimatoprost alone in some models.
- 19:15SPEAKER_00: Those findings are pharmacologically encouraging but cannot substitute for a large human trial.
- 19:21SPEAKER_00: Preclinical aqueous humor dynamics study what remains unproven the human phase III trials compared NCX470 mainly with latanoprost, not with bimatoprost.
- 19:32SPEAKER_00: That means the trials show NCX470 is modestly better than latanopost.
- 19:38SPEAKER_00: They do not definitively show nitric oxide adds a specific large amount of pressure lowering beyond bimatoprost.
- 19:44SPEAKER_00: To isolate the nitric oxide contribution the most informative trial would compare NCX470 versus equimolar bimatoprost similar formulation and dosing similar baseline pressure conventional outflow measurements pressure outcomes in people with glaucoma that definitive comparison has not yet been reported.
- 20:06SPEAKER_00: Safety analysis For the full table please open this article on Visualfieldtest.com The different hyperemia values reflect different methods of collecting and coding redness.
- 20:16SPEAKER_00: The important clinical message is consistent.
- 20:19SPEAKER_00: NCX470 caused more redness than Latanoprost, but the absolute rate was far lower than rates reported with netarseutal containing therapy.
- 20:28SPEAKER_00: Montblanc Clinical Trials.gov Safety Results, Nicox MontBlanc Summary, Denali, through the reported 12-month follow-up, for the full table, please open this article on VisualFieldtest.com.
- 20:40SPEAKER_00: There were no ocular serious adverse events considered related to NCX 470 and no treatment related non-ocular serious adverse events in the reported top line results.
- 20:51SPEAKER_00: Denali results in a United States subgroup reported at the 2026 American Glaucoma Society meeting the most common events through the longer follow-up included conjunctival hyperemia 14.9% with NCX 470 versus 6.9% with Latanoprost.
- 21:08SPEAKER_00: Punctate keratitis 5.9% versus 3.2% eyelash growth 5.6% versus 0.4%.
- 21:16SPEAKER_00: Discontinuation because of an adverse event 1.1% versus 0.4%.
- 21:20SPEAKER_00: Discontinuation for lack of efficacy 2.2% versus 1.4%.
- 21:25SPEAKER_00: These subgroup data are useful but should not be mistaken for the complete international denali population.
- 21:30SPEAKER_00: American Glaucoma Society 2026 Denali subgroup poster iris pigmentation periorbital fat atrophy and inflammation.
- 21:38SPEAKER_00: Because NCX470 releases bimatapost related activity clinicians will watch for prostaglandin associated effects such as iris pigmentation, eyelid or periocular skin darkening, eyelash growth, periorbital fat changes, ocular surface irritation, inflammation.
- 21:55SPEAKER_00: However the publicly released pivotal summaries do not provide reliable numerical rates for Pigmentation or periorbital fat atrophy.
- 22:03SPEAKER_00: The absence of a reported signal is not the same as proof that these effects cannot occur.
- 22:08SPEAKER_00: Systemic nitric oxide effects.
- 22:10SPEAKER_00: Nitric oxide can cause vascular relaxation in other parts of the body, raising theoretical concerns about low blood pressure, headache, dizziness, flushing, cardiovascular effects.
- 22:22SPEAKER_00: The reported phase 3 summaries did not identify a treatment-related systemic safety signal or treatment-related non-ocular serious adverse event pattern.
- 22:32SPEAKER_00: However, detailed systemic nitric oxide pharmacokinetic and blood pressure data have not been publicly presented in enough detail to eliminate every theoretical concern.
- 22:42SPEAKER_00: The most accurate conclusion is systemic nitric oxide effects appear uncommon in the reported trials, but the available public data are not detailed enough to quantify them precisely.
- 22:54SPEAKER_00: Is NCX470 useful for patients with low starting pressure?
- 22:59SPEAKER_00: This is one of the most important unanswered questions.
- 23:02SPEAKER_00: The pivotal Mont Blanc and Denali trials primarily enrolled patients with baseline pressure around 26 to 28 millimeters of mercury.
- 23:11SPEAKER_00: They were not designed around patients starting at 1820 or 22 mm of mercury.
- 23:17SPEAKER_00: Neacox has reported a post-hoc Mont Blanc analysis suggesting that NCX470 maintained a relatively consistent reduction in patients with baseline pressure below 28 mm of mercury, while the Latana Prost response appeared more dependent on starting pressure.
- 23:34SPEAKER_00: However, below 28 in this setting does not mean normal or low pressure.
- 23:39SPEAKER_00: Many of these patients still had substantial ocular hypertension.
- 23:43SPEAKER_00: The Whistler study included people with baseline pressures near 16.6 mm of mercury, but it was a small mechanistic study and was not designed to prove glaucoma treatment efficacy in low pressure patients.
- 23:57SPEAKER_00: Therefore, evidence at the low end of the Mont Blanc and Denali eligibility range, suggestive.
- 24:03SPEAKER_00: Evidence in normal tension glaucoma, insufficient.
- 24:06SPEAKER_00: Evidence for reliably reaching 10 to 12 mm of mercury with monotherapy, absent.
- 24:13SPEAKER_00: Could NCX470 reduce combination therapy?
- 24:16SPEAKER_00: Possibly, but probably only for a subset of patients.
- 24:20SPEAKER_00: The average advantage over Latanaprost was approximately 0.5 to 0.8 millimeters of mercury.
- 24:26SPEAKER_00: That may be enough for a patient who is just above target pressure.
- 24:29SPEAKER_00: It may also help some patients achieve a threshold such as 18 millimeters of mercury without immediately adding another medication.
- 24:37SPEAKER_00: But for patients who need pressure in the low teens, a 40-50% reduction from baseline, control of large pressure fluctuations, protection in advanced visual field loss, NCX 470 monotherapy is unlikely to be sufficient for most patients.
- 24:55SPEAKER_00: There are no pivotal trials showing that NCX 470 reduces the number of additional medications required, the rate of selective laser trabeculoplasty, the rate of minimally invasive glaucoma surgery, the rate of trabeculectomy or tube shunt surgery, visual field progression.
- 25:13SPEAKER_00: The pivotal trials also excluded patients with advanced glaucoma, including those with very advanced optic nerve damage.
- 25:20SPEAKER_00: Consequently, claims that NCX470 will delay surgery remain speculative.
- 25:26SPEAKER_00: Is it useful in prostaglandin nonresponders?
- 25:29SPEAKER_00: There is a plausible mechanism, but no convincing clinical proof.
- 25:33SPEAKER_00: If a patient responds poorly to a prostaglandin because the uvioscleral pathway is insufficient, adding nitric oxide activity could theoretically provide another route for pressure reduction through the trabecular meshwork.
- 25:46SPEAKER_00: However, NCX470 was not adequately tested in a population specifically defined as true bimatoprost non-responders, true Litanoprosta nonresponders, patients who failed multiple prostaglandins, patients with documented severe trabecular outflow resistance despite prostaglandin treatment.
- 26:06SPEAKER_00: A patient who appears to be a prostaglandin nonresponder may also have poor adherence, incorrect administration, preservative intolerance, an inaccurate target, or pressure spikes that are missed during office measurements.
- 26:18SPEAKER_00: Thus, NCX470 may eventually have a role in selected prostaglandin nonresponders, but this is currently a mechanistic hypothesis rather than an evidence-based indication.
- 26:29SPEAKER_00: Is NCX470 genuinely superior to Visolta?
- 26:33SPEAKER_00: That has not been established.
- 26:35SPEAKER_00: Both drugs are once-daily nitric oxide donating prostaglandin therapies, are designed to improve uviosclerol and conventional outflow, have shown reductions of roughly 8 to 10 millimeters of mercury in trials involving patients with baseline pressures in the mid-20s.
- 26:52SPEAKER_00: Potential reasons NCX470 might differ include bumatoprost-related prostaglandin activity rather than latanopost-related activity, different nitric oxide donor chemistry, different release characteristics, potentially different duration of action, potentially different tolerability, but no large direct NCX470 versus Visalta study has been reported.
- 27:16SPEAKER_00: Any claim that NCX470 is superior to Visalta is therefore premature.
- 27:21SPEAKER_00: At present, NCX470 should be viewed as a competing nitric oxide prostaglandin strategy, not a proven replacement for Visalta.
- 27:30SPEAKER_00: Expected additional pressure reduction and number needed to treat versus Latanoprost.
- 27:35SPEAKER_00: Based on Mont Blanc and Denali, best supported estimate, 0.5 to 0.8 mm of mercury, additional reduction.
- 27:42SPEAKER_00: Conservative overall estimate across the program, approximately 0.4 to 0.7 millimeters of mercury.
- 27:50SPEAKER_00: Morning advantage, approximately 0.2 to 0.6 millimeters of mercury.
- 27:55SPEAKER_00: Maximum reported pointwise advantage, approximately 1.0 mm of mercury in Mont Blanc and 0.8 in Denali.
- 28:02SPEAKER_00: Versus bimatoprost.
- 28:04SPEAKER_00: There is no reliable direct estimate.
- 28:06SPEAKER_00: The advantage of bimatoprost over latanopost in previous evidence is already approximately 0.7 to 0.8 millimeters of mercury, similar to NCX470's advantage over latanoprost.
- 28:18SPEAKER_00: Therefore, the extra nitric oxide contribution beyond bimatoprost could be small, clinically variable, meaningful in some patients, not detectable as an average effect in others.
- 28:30SPEAKER_00: A reasonable evidence-based conclusion is that the additional benefit over marketed bimatoprost is unknown and may be less than 1 mm of mercury.
- 28:38SPEAKER_00: Number needed to treat.
- 28:41SPEAKER_00: For continuous pressure outcomes, a formal number needed to treat is not naturally defined.
- 28:46SPEAKER_00: For a responder threshold of at least a 10 millimeter reduction, the Mont Blanc results suggest a time point-based number needed to treat of approximately 9 to 11 at the most favorable measured time points, approximately 20 at the least favorable reported time point.
- 29:04SPEAKER_00: No advantage at the month 3.8 a.m.
- 29:07SPEAKER_00: Time point in the displayed responder analysis.
- 29:10SPEAKER_00: These calculations are exploratory because the analysis is not the same as a patient-level sustained response.
- 29:17SPEAKER_00: Probability of food and drug administration approval.
- 29:21SPEAKER_00: This is necessarily an estimate, not an official prediction.
- 29:24SPEAKER_00: My estimate, approximately 75%.
- 29:27SPEAKER_00: Reasons supporting approval.
- 29:29SPEAKER_00: The new drug application was accepted for formal review.
- 29:33SPEAKER_00: Two large phase 3 trials were completed.
- 29:35SPEAKER_00: Both trials met their primary non-inferiority objectives against Letanoprost.
- 29:40SPEAKER_00: The safety profile was broadly consistent across studies.
- 29:44SPEAKER_00: The drug has a clear mechanism and an established therapeutic class.
- 29:48SPEAKER_00: The Food and Drug Administration reportedly considered the proposed application package generally acceptable at the pre-new drug application meeting.
- 29:56SPEAKER_00: Reasons not to assume approval is nearly certain.
- 30:00SPEAKER_00: FDA approval can be delayed by manufacturing, chemistry, controls, or labeling issues.
- 30:06SPEAKER_00: Full Denali peer-reviewed data are not yet available.
- 30:09SPEAKER_00: Hyperemia and longer-term prostaglandin-related effects remain relevant.
- 30:14SPEAKER_00: The clinical advantage over Latanoprost is relatively small.
- 30:18SPEAKER_00: The public data do not yet include the complete regulatory safety and manufacturing package.
- 30:23SPEAKER_00: A reasonable range would be 65 to 85%, with the estimate increasing if the review proceeds without major chemistry, manufacturing, or inspection issues.
- 30:34SPEAKER_00: Likely pricing and clinical positioning.
- 30:37SPEAKER_00: No United States launch price has been announced.
- 30:40SPEAKER_00: If approved, NCX 470 will probably be positioned as a premium-branded once-daily prostaglandin therapy, a potential alternative to Latanoprost by Mataprost and Vazolta.
- 30:51SPEAKER_00: A treatment for patients who need slightly more pressure lowering than Latanoprost, but want to avoid the high redness rate of netarsudal.
- 31:00SPEAKER_00: A medication likely subject to insurance step therapy requiring trial of generic latanoprost first.
- 31:06SPEAKER_00: Its commercial success will depend heavily on the final label Insurance Coverage Copay Assistance.
- 31:12SPEAKER_00: Whether physicians view the extra half to three-quarters of a millimeter as worth the additional cost, whether the final price is closer to generic prostaglandins or to premium drugs such as Visalta and Rocklatin.
- 31:25SPEAKER_00: It is unlikely to be positioned as a replacement for surgery in advanced glaucoma.
- 31:31SPEAKER_00: Realistic place in the glaucoma treatment algorithm.
- 31:34SPEAKER_00: If approved, a reasonable clinical role would be first-line therapy for selected patients who want a once-daily branded prostaglandin and can tolerate redness.
- 31:43SPEAKER_00: An alternative for patients who obtain an incomplete response to latanoprost.
- 31:48SPEAKER_00: A possible choice for patients who need modest additional reduction but do not tolerate netarsudal.
- 31:54SPEAKER_00: A potential alternative to Visalta, though not proven superior.
- 31:58SPEAKER_00: A possible component of combination therapy when pressure remains above target.
- 32:03SPEAKER_00: It is unlikely to replace generic latanoprost as the usual low-cost starting medication.
- 32:10SPEAKER_00: Selective laser trabeculoplasty, rocklatan, when a larger medication-based pressure reduction is needed, COSOPT or other combinations when multiple mechanisms are required.
- 32:20SPEAKER_00: Glaucoma surgery for advanced disease or very low target pressures.
- 32:24SPEAKER_00: Is this a meaningful upgrade or just another prostaglandin drop?
- 32:28SPEAKER_00: My scorecard.
- 32:30SPEAKER_00: For the full table, please open this article on VisualFieldTest.com.
- 32:34SPEAKER_00: Final judgment NCX470 is more than just another prostaglandin pharmacologically, but clinically, it currently looks like an incremental upgrade rather than a breakthrough.
- 32:45SPEAKER_00: The nitric oxide mechanism appears real.
- 32:48SPEAKER_00: Mechanistic studies show improved conventional outflow, and the phase 3 trials show a small but reproducible advantage over latanoprost.
- 32:56SPEAKER_00: However, the pivotal evidence does not yet prove that nitric oxide adds a major benefit beyond bimatoprost itself.
- 33:04SPEAKER_00: The most defensible expectation is about half to three-quarters of a millimeter more lowering than latanoprost.
- 33:11SPEAKER_00: A higher chance of achieving large reductions in some patients, more redness than latanoprost, much less redness than netarseutal-containing treatments.
- 33:20SPEAKER_00: No proven ability to replace combination therapy, laser, or surgery, no established benefit for very low target pressures or prostaglandin non-responders.
- 33:30SPEAKER_00: If approved on April 30th, 2027, NCX470 could become a useful additional option, especially for patients who need a little more pressure lowering from a once-daily drop.
- 33:42SPEAKER_00: But it is unlikely to redefine glaucoma treatment by itself.
- 33:46SPEAKER_00: Overall verdict promising, differentiated, and clinically useful, but not revolutionary.
