- 0:00This is Madeline Ashley with the Becker CFO
- 0:02and Revenue Cycle podcast, and I'm excited to
- 0:05be joined today by John Melia, interim CFO
- 0:08of Methodist Le Bonheur Healthcare.
- 0:10John, thanks for joining me.
- 0:12Thanks for having me.
- 0:13So I'm so excited to dive into our
- 0:15conversation. Before we kick things off, do you
- 0:17mind sharing just a little bit about yourself,
- 0:20your background, and and more about your organization?
- 0:23Sure.
- 0:24You know, as you mentioned, I'm interim CFO
- 0:26at Methodist Le Bonheur in Memphis.
- 0:29We're a six hospital system
- 0:31generating about 2,000,000,000 in net revenue annually. We
- 0:34have a nationally recognized children's hospital, of course,
- 0:37in Le Bonheur.
- 0:38We have
- 0:39a academic medical center here and four other
- 0:42great hospital facilities as well as many fantastic
- 0:45outpatient sites in and around Greater Memphis.
- 0:49We recently went live with Epic and Workday
- 0:51just last year, so that obviously has been
- 0:54a major transformational
- 0:55change for us across the system.
- 0:58As far as myself, before coming to Memphis
- 1:00here, I served
- 1:02as a an interim capacity in New York,
- 1:05both Upstate and in Brooklyn.
- 1:07So I've seen a wide range of markets
- 1:09and and environments, payer environments.
- 1:12But now I'm proud to serve MLH,
- 1:14Midwestern accent and all, and they're alongside a
- 1:17very dedicated team here in Memphis.
- 1:20Wonderful. Thanks so much for sharing. I I
- 1:22too have the Midwestern accent. So,
- 1:25well,
- 1:26just to kick us off, and and as
- 1:27you mentioned, you've served in in different areas,
- 1:30different markets.
- 1:31I would be curious to hear maybe some
- 1:32of the broader challenges that you were, you
- 1:35know, following closely right now in that in
- 1:37the health care industry from a financial perspective
- 1:39and and why these challenges are, you know,
- 1:42these industry pain points.
- 1:44Sure. Yep. There's a bunch, as you know.
- 1:47You know, just off the top of my
- 1:48head,
- 1:49workforce pressures really have not gone all the
- 1:52way down, you know, since coming out of
- 1:54the the, the pandemic.
- 1:56Some of them have shifted around a little
- 1:58bit,
- 1:59but labor costs continue to be elevated.
- 2:02Allocating,
- 2:03market increases is is critically important for recruitment
- 2:06and retention.
- 2:08And this is true with staff and physicians,
- 2:10and and increasingly, APPs,
- 2:12in fact, advanced practice providers.
- 2:16That's becoming,
- 2:17you know, continues to be a a tough
- 2:19spot for us.
- 2:20Medicare Advantage, we're seeing higher administrative burdens, higher
- 2:24denials,
- 2:25and meaningfully
- 2:26lower yield compared with traditional Medicare, and that
- 2:29that continues.
- 2:31As we look ahead, we're looking at potential
- 2:33reductions in our $3.40 b savings.
- 2:37That's a big deal for us. That's a
- 2:38big deal for any safety net.
- 2:41And then, of course, OBBA,
- 2:43OB three. I've heard it referred to a
- 2:45number of different ways. Mhmm. Possible reduction
- 2:49of the,
- 2:50ACA exchange subsidies the subsidies, the eliminations
- 2:54that'll put more people,
- 2:56make more people may lose coverage,
- 2:59which increases bad debt and shifts payer mix.
- 3:01So that's that's a challenge that we're looking
- 3:03at.
- 3:04And then just long term pressure on directed
- 3:06payments specifically for safety nets.
- 3:10That is become, you know, a pretty, integral
- 3:12part of our reimbursement
- 3:14and our strategy going forward. So
- 3:16understanding the impacts of that in in future
- 3:18years and any potential mitigation strategies that that
- 3:22might come,
- 3:23is important to us.
- 3:25Yeah. And I wanna circle back a little
- 3:26bit to the the one big beautiful bill
- 3:28act. You know, that's something that we've heavily
- 3:31covered here at Becker's and something that we're,
- 3:33you know, seeing a lot of attention drawn
- 3:34to from from finance leaders.
- 3:37With so much unknown around that right now,
- 3:40how are you just kind of keeping a
- 3:42quote maybe like a pulse on this and
- 3:43and working with your finance team to be
- 3:45ready for, you know, the the unknowns?
- 3:48Yeah. So we're putting together,
- 3:50long term financial forecast.
- 3:52And as we head into this budget year,
- 3:55year one, is really gonna be our budget
- 3:57for next year. So I think the the
- 3:59most,
- 4:01the closest things that we have to look
- 4:02out for is how those exchange plans are
- 4:05going to impact us, the the reduction in
- 4:08the subsidies. So that's the nearest term thing.
- 4:12As we look out to the future,
- 4:14it it does appear that there will be
- 4:16some reduction in our, directed payments.
- 4:19In Tennessee here, we are a non expansion
- 4:22state. So the floor on the reduction right
- 4:24now as it stands is a 110%
- 4:26of Medicare, which
- 4:28is still better than our Medicaid rates, but
- 4:30also much less than we're currently getting. So
- 4:33we're trying to model those out, understand what
- 4:36shifts we have to make in our financial
- 4:37strategy going forward,
- 4:39to mitigate the, the impact of losing that
- 4:42reimbursement.
- 4:44And,
- 4:45one final question for you here. You know,
- 4:47some CFOs that I've talked to just honed
- 4:49in on the the importance of working with
- 4:52your with your CEO,
- 4:54to to really develop strategy and and sometimes
- 4:56have those tough
- 4:58conversations.
- 4:59Can you kinda talk a little bit about
- 5:00that working relationship that you have and and
- 5:03just kind of, you know, how you guys
- 5:04have maybe even grown together during this time?
- 5:07Absolutely. Yeah. And there's a lot of things
- 5:09to talk about. So, Michael Lubecki, our our
- 5:12CEO here, and I,
- 5:14you know, have a lot of these conversations
- 5:16about how to plan,
- 5:17because a lot of our strategy is currently
- 5:19driven around the fact that that we do
- 5:21get these reimbursement streams.
- 5:24And, you know, three forty b,
- 5:27in particular
- 5:27is is a key area for for growth,
- 5:30just pharmacy in general.
- 5:32So we stay, you know, very close in
- 5:35in conversation about the potential impacts of these
- 5:38things, how it changes our strategy, how we
- 5:40look at, you know, potential impacts of, site
- 5:43neutrality,
- 5:45as we look at expansion,
- 5:47for our hospital outpatient departments.
- 5:50So, yeah, that constant communication, staying in touch
- 5:52with the story both together
- 5:54with our external partners and our board is
- 5:56is really important. Yeah. Definitely the key there.
- 6:00I wanna shift,
- 6:02just the discussion a little bit and see
- 6:04if you could share with with myself and
- 6:06the listeners a little bit about how you're,
- 6:09deciding right now where to invest in the
- 6:11revenue cycle. If you could just share some
- 6:13thoughts there as well.
- 6:14Sure.
- 6:15You know, as I mentioned, we went live
- 6:17with Epic
- 6:18Yeah. Which is, pretty transformational
- 6:21for us. But we've also locked in our
- 6:23non epic cost of collections
- 6:26and, are accelerating the full revenue cycle performance
- 6:29with our partnership with Ensemble Health Partners,
- 6:33which just started this year.
- 6:35So other than increasing yield and accelerating cash
- 6:38flow, which, you know, the the primary goals
- 6:40of the relationship also include treating them as
- 6:43a co innovation partner.
- 6:45You know, as I mentioned, we've locked in
- 6:46the the cost to collect. So,
- 6:49you know, benefits that the r and d
- 6:51that they put into,
- 6:53automation, we wanna make sure that it it
- 6:55truly improves cash and yield, that benefits both
- 6:59sides of the partnership, obviously.
- 7:01But we really wanna reduce the friction for
- 7:03patients and staff,
- 7:05as part of
- 7:06the evaluation of these innovations and and the
- 7:08investments,
- 7:10and, of course, improving the patient financial experience,
- 7:12which
- 7:13can, as you know, be rocky.
- 7:16And we're trying to mitigate as much friction
- 7:18as as possible on that. And, really, when
- 7:21we're talking about on the automation,
- 7:23other than improving the the patient financial experience
- 7:26and reducing that friction, we really wanna make
- 7:28sure it just stays focused where,
- 7:30it really improve improves the, the yield and
- 7:33the cash.
- 7:34Can you share,
- 7:35with us a little bit about everything that
- 7:37went into this epic integration,
- 7:39you know, how long,
- 7:41this this took and and kind of what
- 7:43things are looking like right now?
- 7:45Sure.
- 7:46You know, I could spend a a whole
- 7:47podcast on on that as well. You know,
- 7:50we
- 7:51we we went through the the journey like
- 7:53a a lot of other organizations. We went
- 7:55live, in October, so just about fourteen months
- 7:58ago.
- 7:59October
- 8:002024.
- 8:01Yep. October 2020, actually. Yep. And we we,
- 8:05doubled down. We went live on Workday
- 8:08in in, in July
- 8:102024.
- 8:10So there was a lot of change
- 8:13very quickly.
- 8:14And and the organization now, I think, is
- 8:16really getting its legs underneath us, understanding the,
- 8:20all the benefits that we're we're,
- 8:22able to to get from it, all the
- 8:24the,
- 8:26the the, things we're able to see that
- 8:28we hadn't been able to see before,
- 8:30the the reduction in the amount of, I
- 8:33would say, manual work and manual interfaces,
- 8:35having people just take information out of one
- 8:37system and putting into another. A lot of
- 8:39that's automated now. And we're just really starting
- 8:42to hit our stride. You know, our AR
- 8:44days are down to, less than they were
- 8:48pre go live,
- 8:49and, you know, our coding is is improving.
- 8:52So we're hitting our stride on a lot
- 8:53of those key points right now.
- 8:55Well, thank you for diving a little bit
- 8:57deeper there.
- 8:58Final question I have for you here. You
- 9:00know, as the the industry continues to evolve,
- 9:03ebb and flow, you know, like we talked
- 9:04about, some of the uncertainties we're seeing. Can
- 9:06you share how the CFO role has been
- 9:09maybe forced to evolve beyond just just crunching
- 9:12numbers, just the finance side of things, and
- 9:14maybe some of the most important leadership skills
- 9:16you're seeing in this role right now?
- 9:18Yeah. You know, it it's it that's a
- 9:19good question. And and
- 9:21as I think about, really, the the latter
- 9:24part of my career, how it's evolved,
- 9:27it's so much less about numbers now in
- 9:29the CFO role, and more about telling a
- 9:32compelling story. I mean Yeah. The story has
- 9:35to be a true story.
- 9:36Mhmm. And you have to be able to
- 9:39influence it in a positive way as as
- 9:41much as possible. But being able to tell
- 9:44it in a clear, consistent manner,
- 9:46internally and externally,
- 9:48that folks can easily understand and that resonates.
- 9:52And, really, it has to stay focused on
- 9:54the future.
- 9:55You know, we spend a lot of time
- 9:57telling,
- 9:58our our boards and external stakeholders about what
- 10:00happened historically. But I think it's really important
- 10:03to say, hey. This is what we're anticipating
- 10:05going forward. These are the headwinds that we're
- 10:07facing.
- 10:09It it's it's our responsibility
- 10:11to outline all of the knowns and give
- 10:13the best guidance we can
- 10:15about the the known unknowns.
- 10:18Yeah. And then we also have to acknowledge
- 10:20the fact that there are always unknown unknowns,
- 10:23I think, is,
- 10:24is Donald Rumsfeld put it. You know, recent
- 10:27history
- 10:28from the change health care cyber incident to
- 10:31the pandemic.
- 10:32Stuff happens.
- 10:33And while we can't really be prepared for
- 10:35things like that, we have to keep in
- 10:37mind that they do happen. So Right. I
- 10:39would say, you know, communicating simply and consistently,
- 10:43keeping the focus forward,
- 10:46interpreting uncertainty, which we've talked a lot about
- 10:48already today, without creating anxiety,
- 10:52and then just, you know, really leading with
- 10:54steadiness,
- 10:55even when those variables change.
- 10:57And could you share too, how long have
- 10:59you been in your interim role?
- 11:01So I've yep. I've been in at, MLH
- 11:04since July.
- 11:05Yeah.
- 11:06You know, and and then previous to that,
- 11:07as I mentioned, I was working in, two
- 11:10different facilities and interim capacities in New York
- 11:12since, 2019.
- 11:15What does the and this might be maybe
- 11:17an obvious question, but what does that interim
- 11:19period that interim role kind of bring to
- 11:21the table that might be different from, you
- 11:23know,
- 11:24a regular CFO role.
- 11:25Maybe what are some things that you've learned
- 11:27during this time? Sure.
- 11:29So my background is in consulting,
- 11:32and it kinda morphed into interim management later
- 11:35in my career, which, I'm I'm really
- 11:38enjoying. But, you know, interim,
- 11:41CFOs
- 11:41are a little bit different in that they
- 11:44come during a time of, uncertainty
- 11:47transition. Sometimes it's as simple as keeping the
- 11:49seat warm in between,
- 11:52the former CFO and the known CFO that
- 11:55will be coming in at a a certain
- 11:57date. Other times, they're kind of date uncertain.
- 12:00So Mhmm. You know, that's part of the
- 12:02the,
- 12:03the challenge is to make sure that while
- 12:05you have that interim tag, you also lead
- 12:07with purpose and
- 12:09and some semblance of consistencies
- 12:11because there can be some,
- 12:13worry about the the lack of,
- 12:16you know, tenure and and consistency
- 12:18with that. But I I I guess to,
- 12:22you know, circle back on on what we
- 12:24bring to the table, we I'd I'd like
- 12:25to think that, you know, in an interim
- 12:27capacity, we bring kind of a a national
- 12:29perspective on things. Yeah. Sometimes organizations,
- 12:35need that,
- 12:36outsider kind of view, independent view.
- 12:39And, you know, that's what we bring to
- 12:41the table. Yeah. And it still holds true
- 12:43to exactly what you were talking about just
- 12:45as the, you know, the the permanent CFO
- 12:47role, how that's evolving, interim CFOs, interim roles,
- 12:50they evolve too. You know? Absolutely.
- 12:53Yeah. Well, wonderful. John, it has truly been
- 12:56such a pleasure getting to connect with you,
- 12:58learn a little bit more about you personally
- 13:00and Methodist Livonar Healthcare. So I I appreciate
- 13:03you hopping on the podcast with me, and
- 13:05I look forward to connecting again soon. Absolutely.
- 13:07My pleasure, Madeleine. Thanks for having me.
