- 0:00This is Scott Becker with the Becker's Healthcare
- 0:03Podcast.
- 0:04I'm thrilled today to be joined by a
- 0:06brilliant leader in health care and surgery center
- 0:09supply chain.
- 0:11We're joined today by Grant Luke.
- 0:14Grant, could you take a moment and introduce
- 0:16yourself and tell us a little bit about
- 0:18what you do?
- 0:19Yeah. Yeah. Absolutely. Thank you, Scott.
- 0:21So, yeah, Grant Luke based down here in
- 0:23Dallas, Texas,
- 0:25but been around
- 0:27health care for a while, been with, ambulatory
- 0:29surgery centers specifically for last probably nine or
- 0:32so years,
- 0:33and have worked for companies like HST Pathways,
- 0:36did project management for SCA,
- 0:39director of ASC applications for USPI,
- 0:41and now currently,
- 0:43doing startup work where I'm consulting with startups,
- 0:46and working with, right now a company called
- 0:48CapExpert
- 0:49that is seeking to,
- 0:51really do more of the blocking and tackling
- 0:52of asset management across, the ASC world as
- 0:56well as, outpatient imaging. Just trying to make
- 0:59sense of, a really disjointed approach from what
- 1:02we've seen of of asset management. So currently
- 1:04working with CapExpert,
- 1:06a lot in the ASC space and, also
- 1:08outpatient radiology.
- 1:12Thanks. And what are some of the trends
- 1:13you're watching in the surgery center space now?
- 1:16What sort of top of mind? What trends
- 1:18are you watching?
- 1:19Yeah. There's probably three different things that I'm
- 1:22looking at.
- 1:23One is we're seeing higher acuity cases being
- 1:27transferred from the hospital to the ASC.
- 1:30And with that transfer of these higher acuity
- 1:32cases, it it's not just the cases, but
- 1:34it's everything else attached to it. Right? It's
- 1:36the the complex HVAC. It's the sterile processing
- 1:39capacity. It's,
- 1:41it's the equipment itself.
- 1:43And so we're seeing
- 1:44this migration of these procedures. It really requires
- 1:47ASCs to really develop a more sophisticated asset
- 1:50life cycle management capability
- 1:52so they can actually manage these assets, so
- 1:55that they can bring in these surgeries
- 1:57and do so effectively.
- 1:59The other thing I'm looking at, is just
- 2:02really just how much waste there still is,
- 2:04in the ASC world. You know, ASCs, you
- 2:07know, comparatively,
- 2:09to to hospitals are still I still think
- 2:11much more efficient.
- 2:13But we're seeing within ASC space
- 2:15a trend of
- 2:17hyper optimization,
- 2:18if we will. How can we dig into
- 2:20every single
- 2:21bit of workflow,
- 2:23and really optimize what we're looking at? So
- 2:25we're talking about things like, you know, preference
- 2:28card variance,
- 2:29inventory obsolescence,
- 2:31you know, specifically with CapExpert,
- 2:34how much reactive capital procurement
- 2:37is is is a tremendous waste when you're
- 2:39having to
- 2:40react to capital equipment going down that's directly
- 2:43responsible for,
- 2:45case volume, and how that just is a
- 2:48tremendous
- 2:49hit on any type of P and L.
- 2:51So
- 2:52the the waste is still there within ASC
- 2:54space, whether it's supply chain,
- 2:57EHR,
- 2:58practice management revenue cycle.
- 3:00Where there's waste, I think there's also tremendous
- 3:02opportunity.
- 3:03The last thing that we're looking at is,
- 3:06from a capital asset perspective
- 3:08is really a capital
- 3:10really a portfolio optimization.
- 3:13You know, we're seeing, you know, ASC is
- 3:15transitioning
- 3:15from kind of a single item budget review
- 3:18to more of a true
- 3:20what I call systems thinking,
- 3:22capital portfolio optimization approach. You know? So instead
- 3:25of asking,
- 3:26hey. Should we buy a new laser or
- 3:28c arm? It's, hey. What combination
- 3:32of infrastructure upgrades, equipment purchases,
- 3:35facility repairs.
- 3:37What what leads to the maximum return on
- 3:39capital
- 3:40and the maximum return on clinical benefit and
- 3:43surgical outcomes?
- 3:44And so between those three, the the the
- 3:46shift of the higher acuity, the ASCs,
- 3:49the still,
- 3:50you know, hidden waste within the ASC space,
- 3:53and then the capital optimization are are the
- 3:55three biggest things I'm looking at.
- 3:58Thank you. And talk about as you worked
- 4:01towards 2026,
- 4:02it's hard to believe we're almost getting there.
- 4:04What are you most focused on? It yeah.
- 4:06It really is. What are you most focused
- 4:08on and excited about?
- 4:10Yeah. So I I think with CapExpert,
- 4:13you're really seeing the ability
- 4:15to quantify the true cost of ownership
- 4:18of
- 4:19your most important asset, your clinical equipment.
- 4:22And so you're able to not just look
- 4:24at the sticker price or maintenance contract,
- 4:27but you can now calculate what what we
- 4:29call the total economic impact of every major,
- 4:32asset.
- 4:33And so I think being able to take
- 4:35that comprehensive approach as you expand
- 4:38not just one ASC, not just two, but
- 4:40within, you know, several ASCs or ASC management
- 4:42companies, you can now, again, go from that.
- 4:45You know, I'll borrow from, you know, my
- 4:47friend Preston Alexander who's well known.
- 4:49You you go from
- 4:51a,
- 4:51firefighting mentality to a systems mentality
- 4:54of now being able to use that data
- 4:56to your advantage.
- 4:58So that's one thing I'm really excited about
- 4:59is being able to really quantify that that
- 5:01total economic impact of assets.
- 5:04I think the other thing that we're seeing
- 5:06is we've seen the rise of AI tools
- 5:08like ChatGPT
- 5:10and Gemini and Copilot,
- 5:12and now we can start doing that with
- 5:14your assets. So CapExpert is becoming
- 5:18CHAT GPT for medical assets and service contracts
- 5:21where soon you'll be able to prompt just
- 5:23like you would with Gemini or CHAT GPT
- 5:26and be able to ask questions and have
- 5:28your own data within your
- 5:31medical equipment and service contracts respond and be
- 5:33able to highlight where there's
- 5:36ability to save, where there's ability to consolidate.
- 5:39So
- 5:40that that leverage of of becoming a tool
- 5:43that people are used to, they they know
- 5:44how to use it. It's much more intuitive.
- 5:47That's very exciting.
- 5:48And I think for us this year,
- 5:51we got to see
- 5:52an incredible amount of customer success when they
- 5:55leveraged us across their entire platform of about
- 5:5814 different locations. We saw, I think, a
- 6:0026 x return on investment,
- 6:03with them either through our automated procurement tools,
- 6:06our marketplace for reallocation or selling surplus,
- 6:10our equipment planning for de novo facilities,
- 6:13and then our consolidation of service contracts was
- 6:15really able to help, again, go back to
- 6:17the blocking and tackling of eliminating waste.
- 6:20So those are the things I was really
- 6:22excited about for this year.
- 6:26Thank you. And and and and and take
- 6:28a second. You've had this great career. Well,
- 6:30first, I'm gonna ask you. You're in Dallas.
- 6:32Are you a Cowboys fan?
- 6:34Have you watched the Jerry Jones special about
- 6:36the Cowboys that was on you know, I
- 6:38think it was either Netflix or HBO. I
- 6:40forget which one it was. But it was
- 6:41fantastic television with Michael Irvin and Emmitt Smith
- 6:44and more. Are you a Cowboys fan? It's
- 6:46like, I guess, the first next question. So,
- 6:48unfortunately, I am not a Cowboys fan, but,
- 6:52we do have Cowboys watchers in the house.
- 6:55So, Scott, I've got a wife who's also
- 6:57a nurse at a surgery center, so I
- 6:59live and breathe all the problems every day.
- 7:01She is a native Texan,
- 7:03but she we're we're more to cheerleaders around
- 7:05here. I've got,
- 7:07three girls and one on the way as
- 7:08well. So we we see the Dallas cheerleaders
- 7:12documentary, but I'm definitely gonna have to log
- 7:14that into,
- 7:15my next watch for sure. It is fantastic.
- 7:18Now, Grant, I would feel uncomfortable
- 7:21watching the special on the Dallas Cowboys cheerleaders,
- 7:24so I can't watch that. I don't even
- 7:25know what that's about. No. I'm just kidding.
- 7:27The, you know, so talk a little bit
- 7:29about
- 7:30what advice would you get? Because so congratulations
- 7:33on three daughters and one on the way.
- 7:34That's fantastic.
- 7:36That's as good as it gets. Literally Absolutely.
- 7:38Nothing better. That just is so nice. So
- 7:40nice. Yeah.
- 7:41Talk for a second about sort of,
- 7:44as you move into this,
- 7:46next year, what advice would you give to
- 7:48emerging leaders? You know, what do you tell
- 7:51an emerging leader and and so forth?
- 7:54Yeah. So I I would actually approach it
- 7:56from three different aspects.
- 7:58One, I'll give you something from CapExpert related,
- 8:00so we'll stay focused on CapExpert per second.
- 8:03The second one being if you're just an
- 8:05ASCs in general.
- 8:06And then the third is what I would
- 8:08just call life advice.
- 8:09So the first one per cap expert,
- 8:12when you look at your p and l,
- 8:14you're looking at a lagging indicator.
- 8:16We really see that your asset health could
- 8:19and should be a leading indicator.
- 8:22So if you think about, you know, from
- 8:24your asset perspective, you know, the moment you
- 8:27have a critical piece of equipment
- 8:29that's getting, you know, close to the end
- 8:31end of life or as many years beyond
- 8:33useful life, that is a direct
- 8:35measurable threat
- 8:37to your future revenue
- 8:38and your risk profile.
- 8:41And so we think people should be leading
- 8:43those p and l discussions with an asset
- 8:46management data approach.
- 8:48So if you're a leader out there,
- 8:50you can look at your medical assets, and
- 8:53that can literally be a more accurate indicator,
- 8:56than even your p and l.
- 8:59From an ASC perspective,
- 9:00I think ASC leaders, like, you have to
- 9:04become
- 9:05your chief translation officer.
- 9:07You have to be under be able to
- 9:09understand how to speak clinician, how to speak
- 9:11finance,
- 9:12how to speak,
- 9:14physician owners, your board of directors,
- 9:17and vice versa, be able to go between
- 9:19the two. And so I I think anything
- 9:20that you can do
- 9:22from a translation perspective, whether it that'd be
- 9:25go to a Becker's ASC conference, which is
- 9:27one of the best in the entire world.
- 9:29Shameless plug
- 9:31there. God bless you, Greg. Could you just
- 9:33repeat that last part very quickly? No. I'm
- 9:35just kidding. That's fantastic. Thank you. Yeah. So
- 9:37so becoming a so be but but being
- 9:40around people at these conferences to help you
- 9:42understand
- 9:43how to translate finance into clinician,
- 9:46how to do clinician to finance, how to
- 9:48speak to physician owners. So that ability to
- 9:50be a chief translation
- 9:51officer is just so critically important
- 9:54as a leader, within the ASC space. And
- 9:57then lastly, from a life perspective,
- 10:00I think something that people don't realize what
- 10:02they're looking for is peace.
- 10:04I know that's kind of a meta approach
- 10:06to this conversation, but,
- 10:08I think there are there are many ways
- 10:11and many things that you can do in
- 10:13your own life
- 10:14and
- 10:15working with others and and living life with,
- 10:17you know, others, you know, whether it be
- 10:19family or,
- 10:20in the workplace
- 10:22that can give you peace. And I think
- 10:24too many people
- 10:25pursue things that think are gonna give them
- 10:27peace and and and that doesn't. And I
- 10:30think that results in a lot of either
- 10:33bad decisions or a lot of heartache,
- 10:35across the board. But if you can find
- 10:37a way,
- 10:39to
- 10:39to set up for yourself and and then
- 10:41be at peace, whether it be at work,
- 10:44or your personal life,
- 10:46that to me is the end all be
- 10:48all. So if as you're out there being
- 10:50a leader,
- 10:52make decisions for yourself, make decisions for people
- 10:54that you report to that are following your
- 10:57leadership to to provide that piece.
- 11:00Grant, I can't tell you how much I
- 11:02appreciate you taking the time to visit with
- 11:04us today on the Becker's Healthcare podcast. I've
- 11:06watched your career evolve over the years. I
- 11:08can't tell you how impressed I am with
- 11:10what you're doing. Congratulations on the fourth daughter
- 11:12on the way. Nothing better than that, and
- 11:14that's more important than any of this. Thank
- 11:15you so much for joining us today on
- 11:17the Becker's Healthcare Podcast.
- 11:18Thank you so much, Scott. Thank you for
- 11:20having me.
