- 1:18SPEAKER_02: And I can tell you every dermatologist who injects has that book in their office.
- 1:23SPEAKER_02: It's an excellent book.
- 1:25SPEAKER_02: Congratulations on that.
- 1:26SPEAKER_02: That's a labor of love, I know.
- 1:28SPEAKER_02: And it's been translated into five languages, which shows you how popular it is.
- 1:32SPEAKER_02: He's also published many papers and textbook chapters on filler techniques and neuromodulators as well.
- 1:39SPEAKER_02: He's a sought-after speaker.

- 1:55SPEAKER_02: So I'm so excited to have you.
- 1:58SPEAKER_02: Thanks for joining us.
- 1:59SPEAKER_01: It's my pleasure, my dear.
- 2:01SPEAKER_01: Thank you so much for the invitation, Patty.
- 2:03SPEAKER_01: I'm very happy to be with you here today.
- 2:06SPEAKER_01: Thank you so much.
- 2:07SPEAKER_01: You're welcome.
- 2:08SPEAKER_02: So I want to start by talking about, and we're going to ultimately get into the study that you conducted.
- 2:16SPEAKER_02: And this, but the story starts with a product called HA Intensifier, which I'm sure you remember and I remember.
- 2:23SPEAKER_02: It's a random product that Skin Suiticals developed many years ago.
- 2:27SPEAKER_02: Very popular product.
- 2:28SPEAKER_02: But they recently tweaked that formula and now renamed it HA Intensifier Multiglycan.
- 2:37SPEAKER_02: So maybe go through with us, you know, what did they do?
- 2:40SPEAKER_02: How did they change it?
- 2:41SPEAKER_02: How does that affect its benefits and its efficacy?
- 2:45SPEAKER_01: Okay.
- 2:46SPEAKER_01: So uh thank you so much again for the question.
- 2:49SPEAKER_01: So yeah, this product is very interesting because have for the first time a very uh high uh concentration of HA of phylauronic acid, no, because of that intensifier, no, but the the old one is top on this.
- 3:04SPEAKER_01: So the multi-glycan is because now they are worried about the the degradation of phylauronic acid.
- 3:12SPEAKER_01: So the thing we know that you have the gags, no, the the glucosaminoglycans, we have 50% is the AJ non-sulfated, no, and the other 50% is other glycans, we call the sulfate.
- 3:26SPEAKER_01: And then uh generally this first 50%, the sulfate is the reason is the hydrations, the benefit, we know that.
- 3:34SPEAKER_01: And the other one, uh, we have uh uh, and of course, the hydration structure support to the skin, we have the uh plump, a youthful look, everything of that.
- 3:45SPEAKER_01: But the the other one, the non-sulfated, the sulfate, sorry, they have a degradation very fast.
- 3:52SPEAKER_01: And trying to treat this kind of of uh uh issue, no, they add no in the a they intensify multi-glycan uh more ingredients.
- 4:03SPEAKER_01: So they change some ingredients and they add the the postbiotic ferment, the 2% postbiotic ferment.
- 4:12SPEAKER_01: Because with the 12% of proxylin that they had before, but they increase the amount, you know, the concentration.
- 4:19SPEAKER_01: Now they have the the the improve the sustainability and the degradation too.
- 4:26SPEAKER_01: So nowadays we have the the I'm gonna talk about the ingredients because I think it's very interesting.
- 4:33SPEAKER_01: So the AI intensifier multi glycan technology.
- 4:36SPEAKER_01: We have the 1.3% of thealuronic acid, of course, no, because the most important thing here, and that we're gonna call the surface plumping and smoothing, no, and then we have uh to inhibit the A degradation, we have the 2% depotation glycerizate, no, and 02% of purple rice extract, and they inhibit the AJ degradation.
- 5:01SPEAKER_01: And now with the thing about improved the stimulation of multi-glycan and the structural volume, we're gonna have the 12% of proxylant, so 20% more than the first uh uh vial, not the first uh AJ intensifier.
- 5:19SPEAKER_01: And now the the new ingredient, 2% of postbiotic ferment that is new.
- 5:25SPEAKER_01: So, with the all of the technology together, we're gonna have gonna be more fresher, lighter, fill, and improve the sustainability of the product.
- 5:36SPEAKER_01: And because of that, when you use this, we feel more hydration, you know, and uh during the modes that you're gonna use, you're gonna have more of this fresh uh and glow effect to the skin.
- 5:48SPEAKER_01: And because of that, I think it's very, very assertive for the the treatment of the patient and combining with other kinds of treatments too.
- 5:58SPEAKER_02: And I know that they did a number of studies before you started your work with the filler, they looked at it, and I was particularly intrigued by the study that looked at the volumization.
- 6:11SPEAKER_02: It was so interesting.
- 6:12SPEAKER_02: So maybe talk about that for a minute.
- 6:15SPEAKER_01: Yeah, this study is very interesting.
- 6:18SPEAKER_01: It wasn't uh my uh uh the my study, no, but it's very interesting.
- 6:23SPEAKER_01: We're showing the the incus in symposia of skin pseudo in Paris.
- 6:28SPEAKER_01: No, that we are together there for some uh things too.
- 6:32SPEAKER_01: So, and then they they they show that it was the methodology was like two-week uh single-centered clinical study.
- 6:41SPEAKER_01: It was 81 female patients between 35 and 65 years old, and then they the endpoint was uh image with the 3D technology, no, and quantified volume change too.
- 6:56SPEAKER_01: And uh it and it was an amazing result because we can see that you're gonna have uh in 12 weeks in this study, the H intensifier multiplicane will show increase the cheek volume in 0.69, so almost 0.7 cc.
- 7:15SPEAKER_01: No, we know that one syringe is like one cc generally, it was 0.7 cc.
- 7:22SPEAKER_01: That's impressive for a topical, right?
- 7:24SPEAKER_01: Yeah, impressive because they bring to water and also hydrophilic effect, and we can see in the 3D image now, because they pull in the the different colors to show the volumizing effect, and and especially in the difficult areas that if you feel with thealuronic acid injectables, we need to take care a lot about to not have uh volumizing.
- 7:50SPEAKER_01: So as a topical treatment is amazing and very safe, is a very to complement, no, to complete a treatment with injectables is very interesting.
- 8:01SPEAKER_01: I love this.
- 8:02SPEAKER_01: I do too.
- 8:03SPEAKER_02: So with the whole area of integrated skin care, which really skin pseudocals pioneered years ago, has they've done done such great work.
- 8:11SPEAKER_02: So let's let's talk a little bit about the study that you conducted in that study design, looking at the ability of patients having dermal fillers to be able to utilize this product as well.
- 8:24SPEAKER_02: I I love the study design.
- 8:27SPEAKER_01: Thank you so much.
- 8:28SPEAKER_01: Yeah, the idea is because you know, the as we do in our practice, no, like you and me.
- 8:35SPEAKER_01: So the thing is nowadays we know that the patients don't have time, so they they like they like to have the treatments and the skincare very close, no, to to intake the treatments, they are very fast about that to understand the necessity of this kind of treatment.
- 8:53SPEAKER_01: So and then we decide to show how the AJ multiglican, so intensify multiglicing, is important now to complete the result that you have with injectable.
- 9:05SPEAKER_01: And because of that, we'd make a split phase study, no.
- 9:09SPEAKER_01: So the idea is to show the efficacy, of course, and tolerance too of combination of the treatment with AJ intensifier multiglican, the topical, and the alalonic acid dermal filler.
- 9:21SPEAKER_01: We choose the the vol lift for joboderm for allerg and ab, okay, and improve uh in this patient uh after some weeks.
- 9:30SPEAKER_01: So we did a split phase, one side with H intensifier and filler, and the other side a vehicle, like a placebo, uh, with filler.
- 9:40SPEAKER_01: Now we treat the nasolabel fold initially because they used to have another uh studies to more areas and to evaluate too.
- 9:50SPEAKER_01: But then we begin with this.
- 9:52SPEAKER_01: So it was uh split phase 14 weeks.
- 9:56SPEAKER_01: We make 40 patients and female and male too, Fitzpatrick one to six, no age between 38 or 70 years old.
- 10:06SPEAKER_01: So it was very the average was very complete, and then uh with mild to moderate nasolabia folds, global screen wrinkles, pro speed firmness, and elasticity.
- 10:19SPEAKER_01: And then during the 14th day, we see in in different weeks, but then we see again, okay, in 14 uh days.
- 10:29SPEAKER_01: So then we see that uh okay, we completed in 40 patients.
- 10:34SPEAKER_01: It was like the majority of male, you know, that's easy to do.
- 10:38SPEAKER_02: Like it's always the way, just to be clear, you did a pre-treatment for two weeks, right?
- 10:43SPEAKER_02: Yeah, let them use it for 14 days, then they had their injections, so they went off, you did the injections, and then the following day they restarted.
- 10:51SPEAKER_01: Yeah, yeah, okay, and then evaluate after that.
- 10:54SPEAKER_02: You followed them for 12 weeks.
- 10:56SPEAKER_01: Yeah, exactly that 12 weeks.
- 10:59SPEAKER_01: So, and then we see that in the nasolable uh fold area that we inject with fillers, of course, and the the the real product, the benefit was better uh about the about the the the improvement of the skin, the quality reflect more light, this kind of thing compared to the other side.
- 11:23SPEAKER_01: But the most incredible, not the most incredible, what we were was expecting for this, but the the the result very interesting, it was about the at week eight in the post-dermophyll injection.
- 11:38SPEAKER_01: Uh, when you compare, we have an improvement in tempo skin firmness.
- 11:44SPEAKER_01: In the tempo area, the skin firmness, it was much more uh important than before comparing to the other side.
- 11:52SPEAKER_01: No, it was very interesting.
- 11:54SPEAKER_01: And then another thing interesting, 12 weeks after we have a great statistically significant improvement of forehead skin thickness.
- 12:06SPEAKER_01: The the thickness of the skin changed too much in this area, so just comparing, we didn't use aaluronic acid, that just the H intensifier topical comparing the placebo.
- 12:17SPEAKER_01: So it was very interesting the result, and then the it is is we cannot see the pictures here, but the picture are very convincing about the the firmness in temple and thickness of the forehead.
- 12:30SPEAKER_01: You saw this.
- 12:30SPEAKER_02: Yeah, I saw the pictures, the pictures were amazing, and so you looked at for the temples, you looked at it with catometer, and then this was ultrasound for the density on the forehead.
- 12:43SPEAKER_02: Okay, just so everybody listening understands.
- 12:46SPEAKER_02: So they was done with instrumentation, it wasn't just a visual assessment.
- 12:51SPEAKER_02: So it's always nice when you have objective measures, and it was it was impressive.
- 12:56SPEAKER_01: You can see it for sure.
- 12:58SPEAKER_01: And of course, we have the the part of the study showing the the satisfaction of the patient of this, then was very satisfied about that, and again about the firmness, the sickness, and even of the nasal labor fault.
- 13:12SPEAKER_01: But it was more impressive about these other areas that we didn't inject the aloratic acid, no, because of that, we want to do more studies, more in different areas to show because it was very impressive about so these two studies that you showed today about the volumizing effect, and then you can change the thickness of the skin because it's difficult, no?
- 13:38SPEAKER_01: And the firmness too is very interesting.
- 13:40SPEAKER_01: I'm very excited about that.
- 13:42SPEAKER_02: Yeah, getting results uh with ultrasound is it's always difficult.
- 13:47SPEAKER_02: You know, people who run studies will always say, Oh, that's a that's a tough hurdle.
- 13:51SPEAKER_02: So that is impressive, and you can clinically see it in the photographs.
- 13:54SPEAKER_02: They're so impressive.
- 13:57SPEAKER_00: This episode is brought to you by Skin Suticles, the number one medical aesthetic skincare brand worldwide, and the leader in antioxidant science.
- 14:06SPEAKER_00: To learn more, visit www.skinseuticals.com.
- 14:12SPEAKER_02: So, what other areas would do you have further studies planned to look at other areas?
- 14:18SPEAKER_02: I mean, cheek volumizing, temples.
- 14:21SPEAKER_01: Yeah, because we began in Azolebiophot because it was easier now to that, and everybody can, yeah.
- 14:28SPEAKER_01: So, but now we want to have more uh more a bigger area, like zygomatic bone, zygomatic arch, you know, or then the because the jaw line and jaw hummus is more difficult because in Mayo patients have the beard and this kind of so we try to stay here.
- 14:46SPEAKER_01: The tempo injection is very uh a good option because it's a specific area and the superficial injection, subcutaneous layer, of course, because it's easy to evaluate too, because the deep injections don't work here because we want to show the the benefits with the topical product.
- 15:07SPEAKER_01: So I think you're gonna stay on on these areas.
- 15:09SPEAKER_01: Now the tempo and the zygomatic bone of this part here.
- 15:14SPEAKER_02: I I think one of the most important things about what you've done is while we always talk about integrated skincare with lasers, energy-based devices, I don't think we pay as much attention to injectables.
- 15:26SPEAKER_02: But this study really hits the nail on the head, showing, you know, more better patient satisfaction, better outcome, and benefits all over the face.
- 15:37SPEAKER_02: So people are obviously going to be very pleased with their injection, with their injection treatments if the rest of their face gets better as well.
- 15:47SPEAKER_02: And so I think this, I think this is a very important study.
- 15:51SPEAKER_01: Um thank you so much.
- 15:53SPEAKER_02: Congratulations, yes, it's a good one.
- 15:56SPEAKER_01: It was good because you were right.
- 15:58SPEAKER_01: Generally, we used to see like the technology, so or generally after technology, or like microneedling with radio frequency, something like that.
- 16:08SPEAKER_01: No, this is very important too because it's the real life, but with injectable, I inject a lot, so every day.
- 16:15SPEAKER_01: So I just finish now, by the way, after I wear my clinic.
- 16:20SPEAKER_01: So, but uh, and and the patient asks to my okay, may I use which product can I use?
- 16:26SPEAKER_01: Yeah, which product may I use tomorrow?
- 16:28SPEAKER_01: So I'm gonna improve my my skin using the pro this product, or the most common question we're gonna uh the duration are gonna be bigger, the injectable with the skincare with this topical product.
- 16:42SPEAKER_01: That this is the most common question for me, at least, and even with botaline toxin, too.
- 16:47SPEAKER_02: So I was just gonna say, can you imagine this with botulinum toxin in the forehead?
- 16:52SPEAKER_02: Yeah, I mean, that's amazing.
- 16:55SPEAKER_02: That would be such a please do that study for me.
- 16:58SPEAKER_02: That's gotta be a good one.
- 17:00SPEAKER_01: This is one, the idea too, and you know that they have the PTOX too, no, that is compared to the orbicularis area.
- 17:09SPEAKER_01: So the idea is to make with the both one, but of course begin with one and another one because it's so interesting.
- 17:16SPEAKER_01: No, and the pay and and because of that, again, the skin serious is very innovative again, because uh to support this kind of idea is very interesting.
- 17:26SPEAKER_01: Because if you can prove that really a topical product, the skincare can have benefit with the procedures injectable in this case, you'll be amazing because of that.
- 17:38SPEAKER_01: So interesting the study and the idea.
- 17:41SPEAKER_01: So I I I I would like to do much more now, I think, and maybe you can join with me, it would be a pleasure, because I think it's the future, and then the patient can understand how powerful as other topical products, too, especially in the brands that invest on this, never stop to study new ingredients, the science behind.
- 18:06SPEAKER_01: I think is the is the present and the future, no, definitely.
- 18:10SPEAKER_02: I agree, and you know, as I was looking over everything to prepare for this, I it reminds me of how methodical skin suiticals is about testing their products.
- 18:19SPEAKER_02: And this goes way back to the CE for Rulec days.
- 18:22SPEAKER_02: But I mean, in on this product, they've done histologies, they've looked at what gags were upregulated, the different glycosaminoglycans and staining.
- 18:32SPEAKER_02: You know, there's a very it's a good roadmap for companies who want to know how the guys who do it right do it.
- 18:38SPEAKER_02: And they always do it in a very methodical way, evaluating it clinically, evaluating it in all skin types.
- 18:45SPEAKER_02: All the studies had skin types one through six, which is so important.
- 18:49SPEAKER_02: Um, but and then, you know, again, histologies.
- 18:52SPEAKER_02: And then when they did the clinical study, you all did clinical grading, you did photographs, you did objective measures, and that's really what it takes.
- 19:00SPEAKER_02: You know, I have a lot of young companies I work with, and they're like, what is it gonna take to convince the doctors that this skincare product works?
- 19:07SPEAKER_02: And I'm like, a lot.
- 19:10SPEAKER_02: Yeah, we want to see a lot of data, and we want to see it in all skin types, so it, you know, they just don't realize the depth of the studies that go into this.
- 19:19SPEAKER_02: Was this a multi-center study, Andre?
- 19:21SPEAKER_02: Was it multi-centered or do you do all the patients?
- 19:24SPEAKER_01: No, no, it was a multi-center, it was three colleagues here in colleague in Brazil.
- 19:29SPEAKER_01: Yeah, it's what three centers, no, it's not a lot of them, but that we inject at the same time, you know, evaluating and the but the other patient was measured in just one place, you know, the the cotometer, the ultrasound for the same person, no, to have the same analysis and this kind of thing, you know.
- 19:49SPEAKER_01: But I think it's is very important again because then we can understand how the improvement of this together is the future.
- 20:00SPEAKER_01: And you know that as a dermatology, and we are we are so serious about the data, no, we like science to understand is not only the to observe the effect is important, the patient opinion, of course, is amazing, but we need to understand what is going on.
- 20:18SPEAKER_01: No, the thing about the glycans that you said that I said in the beginning, no, non-sufate, sulfate, and this they they uh uh uh uh worry about that, and then they add a new ingredient to this.
- 20:31SPEAKER_01: This is very interesting.
- 20:33SPEAKER_01: It's very we are very safe about that, no, about the skin syrup.
- 20:37SPEAKER_02: Yeah, I I I love the way they thought this through, you know, and not just to boost hyaluronic acid, but also to look at those sulfated gags, which are really the structural uh glycos amino glycans that give the skin probably a lot of the benefits that we're looking at in these pictures and and with these objective measures.
- 20:58SPEAKER_02: Anything interesting that you see on the horizon?
- 21:01SPEAKER_02: Um, you as you're such an expert in injectables, I mean, there's so much going on in that marketplace right now.
- 21:08SPEAKER_01: Yeah, you know that the last the last okay, the yaluronic acid is the most hero, you know, for the last 20 years.
- 21:15SPEAKER_01: It's amazing.
- 21:17SPEAKER_01: Then we're gonna have, we began to have the beau simulators in the last 10 years at least, and very famous in in Europe, for example, more recent, but in Brazil, in Latin America, we have been doing a lot for the body, for the face.
- 21:31SPEAKER_01: So the combination between them, no, it's a very uh uh recent too.
- 21:37SPEAKER_01: We we we uh helped to create a product that the first one, the same Syrincho had yaluronic acid, a calcell uh uh and a caja, calcium hydroxyfoxide, etc.
- 21:50SPEAKER_01: It's a very interesting product that I am they they call me the father of this, but it's not it's because I I I designed since the beginning, uh, it was a startup.
- 22:00SPEAKER_01: In Israel, and then when Anargon bought now, I I develop all the idea, the positioning, and when they approve around the world, I launch, so it's my pleasure.
- 22:12SPEAKER_01: And but you know, a lot of other companies they are trying to mix uh yaluronic acid alone with calcium hydroxypatite and carboctometrosaloids to try to have the same thing.
- 22:23SPEAKER_01: Sometimes not the same thing because have one more ingredient, but but I think I I mean this yaluronic acid and uh biostimulator together or in the same syringe or in the same moment, you know, uh are the present and the future.
- 22:40SPEAKER_01: And I think that much more products with different uh uh reticulated uh agents, you know, like BDDE, like DVS, new things are coming, but it's very ecological and this kind of thing is is is is is our studies now, and another product that is not aaluronic acid, no, but is but came for the sugar and something is coming to because you can inject without feel, you know, but giving more structure and have a stimulation too, but in another way.
- 23:20SPEAKER_01: So I think for the future is this holistic approach, like this 360 approach, because the patient they want not only restore the loss of volume, restore, they want to have the very beautiful quality of the skin at the same moment that you can treat the aging process.
- 23:39SPEAKER_01: So for this, we need to have a more complete approach.
- 23:43SPEAKER_01: I all the big companies they are doing that, even the injectables one, right?
- 23:49SPEAKER_01: To have naturality, more natural results.
- 23:53SPEAKER_01: I think that nowadays the social media is a very difficult moment because it's so bad results and so fake results and overfill.
- 24:03SPEAKER_01: So uh a lot of patients are very afraid, no, of bad results because of the social media's things, and because of that, we need to explain, okay, thealuronic acid, for example, is still amazing, I love, but in the right hands, no, right, the sense of art, the assessment good, just restore, you know, but not make it wide, only if you want to do that, to change a facial shape in in steps, not in just once, you know what I mean.
- 24:35SPEAKER_01: So we need to to have this is the best way, but stimulating all the time the college, the quality of the skin.
- 24:44SPEAKER_01: I think this is this is the future.
- 24:46SPEAKER_01: This combination you'll be uh the dominant for the last five years, the next five years, for example.
- 24:55SPEAKER_01: Uh I think.
- 24:56SPEAKER_02: I think you're I think you're you're right, absolutely right on.
- 25:00SPEAKER_02: And a product like this really helps with that skin quality as well.
- 25:04SPEAKER_02: Yeah, that's so underestimated or undervalued, maybe I should say.
- 25:10SPEAKER_02: People really want their skin to look good, they want that dewy look, um, and you know, this kind of hydrating and structural, this that component of the skin, you know, a lot of times it's harder to get with fillers if you have a product that can do it, skincare.
- 25:27SPEAKER_02: That combination is is magic.
- 25:29SPEAKER_01: And just to add what you I'm sorry for interrupting you, because in the future you is exactly this that you said now, and have been talking during this podcast.
- 25:39SPEAKER_01: So is the the combination more and more of injectables and technologies and the skincare, but then you we can prove that this skincare, not only all of them, of course, like products like skin cellular, like are doing this science behind, can prove that the benefits will be bigger and very easy to notice, no, like and because of that we are doing this kind of studies.
- 26:08SPEAKER_01: If the duration will be bigger, the benefits, improvement of the skin, you'll be together, you know, not only the restore the loss of volume, okay, and one day gonna treat the skin.
- 26:21SPEAKER_01: No, we're gonna do everything together, and then the skincare advanced it.
- 26:26SPEAKER_01: I used to say skincare gonna help a lot, and we can prove that.
- 26:30SPEAKER_01: No, yes, excellent.
- 26:32SPEAKER_02: Well, I want to thank you for your time.
