- 0:00This is Laura Dierda with the Becker's Healthcare
- 0:02podcast. I'm thrilled today to be joined by
- 0:04doctor Todd Karpinski,
- 0:06chief pharmacy officer and system vice president as
- 0:08well as associate dean for health system services
- 0:10at WVU Medicine. Doctor Karpinski, it's a pleasure
- 0:13to have you on the podcast today.
- 0:15Yeah. Thanks for having me.
- 0:17I'm excited for our conversation, and really to
- 0:19dig a little bit deeper into spotlighting some
- 0:21of the things you're doing at WVU Medicine
- 0:23as well as,
- 0:24your perspective on the future. But before we
- 0:26dive in, can you tell us a little
- 0:27bit more about yourself and the health system?
- 0:30Yeah. I've been here at WV Medicine for
- 0:32the past seven years.
- 0:34We've very much been a growing organization.
- 0:37When I first started in 2019, we were
- 0:39at seven hospitals.
- 0:40By the 2026,
- 0:43we'll be up to 30 hospitals in four
- 0:46states.
- 0:47I do oversee all the pharmacy services
- 0:49across,
- 0:51the health system, which would include inpatient, retail,
- 0:54specialty,
- 0:55home infusion,
- 0:56our pharmacy warehouse.
- 0:58And I also serve as the associate dean
- 1:00at the School of Pharmacy
- 1:02here at WVU.
- 1:04That's great to hear. And and definitely, you
- 1:07know, I I think,
- 1:08great that you have that opportunity to provide
- 1:11leadership in such an important,
- 1:13space of the health system. I'm curious when
- 1:15you think back from the last year or
- 1:17so,
- 1:18what do you,
- 1:19think is the most exciting,
- 1:21initiative that you led? What did you do,
- 1:23and what were the
- 1:25Yeah. I think one of our, bigger strategic
- 1:27initiatives last year were to address the growing
- 1:30needs,
- 1:31of our specialty and home infusion pharmacy.
- 1:34So we worked with,
- 1:36internal leadership and our finance committee really side
- 1:39by side to
- 1:40come up with some justification for a new
- 1:42$20,000,000,
- 1:44facility
- 1:45that will be co located where our pharmacy
- 1:47warehouse
- 1:48is, an ability that we call our Innovation
- 1:51Corporation,
- 1:52which is a couple of blocks away from
- 1:53our academic medical center. It's really gonna expand
- 1:57our ability to provide even more specialty pharmacy
- 1:59services to the patients we serve,
- 2:02allow us to grow our home infusion,
- 2:05footprint by about 400%,
- 2:07as well as being able to implement a
- 2:10fully automated mail order pharmacy
- 2:12where we will then be able to centralize
- 2:14all of our mail order operations out of
- 2:16the one location.
- 2:20That's amazing to hear. And, you know, seems
- 2:22like a really great,
- 2:24project to be working on and and provides
- 2:26a ton of value.
- 2:28What was the process for doing that? What
- 2:30were some of the things that you found
- 2:31out during it and, you know, learned along
- 2:33the way?
- 2:34Yeah. Well, we did a lot of data
- 2:36mining to figure out where we had leakage
- 2:39of our specialty in home infusion.
- 2:42Medications being filled by other outside facilities
- 2:46really built our ROI
- 2:48around capturing or recapturing those prescriptions and filling
- 2:51those here internally.
- 2:52That really gave us the dollars to justify
- 2:55the almost $18,000,000
- 2:57project,
- 2:58that it will be.
- 3:00And the other aspect of that is we
- 3:02have 13 retail pharmacies across our system, and
- 3:05most of them do kind of mail order
- 3:07out of the back end
- 3:08or backside of the pharmacy.
- 3:10We wanted to consolidate that,
- 3:12out of one facility, do shipping out of
- 3:14one facility to try to reduce overall logistic
- 3:16costs
- 3:17as well as improve our turnaround inefficiency
- 3:20around filling mail order medications.
- 3:24Now in looking ahead for 2026, what are
- 3:26some of the big priorities and headwinds that
- 3:28you're most focused on? Right?
- 3:30Well, I mean, that biggest headwind, probably like
- 3:32other large academic,
- 3:34medical centers and IDNs is what's gonna happen
- 3:37to the three forty b drug program.
- 3:40We all thought knowing the 2026
- 3:42that will we be moving to the rebate
- 3:43model.
- 3:44And obviously, it's now been put on hold
- 3:47through the courts. And I recently just saw
- 3:49today that HRSA is going to be rethinking
- 3:52how they approach the rebate model. So that
- 3:54was going to be a huge headwind.
- 3:56But we may be backing that down a
- 3:58little bit to make that wind gust not
- 4:00quite as great.
- 4:02But we're also looking at, you know, what
- 4:03is the government doing around Medicare
- 4:06Part d and our Medicare fair pricing and
- 4:08how will that impact our reimbursements
- 4:11on the retail side,
- 4:12as well as what, if anything, they plan
- 4:15to do around
- 4:16the OPPS
- 4:17pricing,
- 4:19survey and how that could potentially impact Medicare
- 4:21Part b reimbursement.
- 4:25And that's fascinating to hear. I know, you
- 4:27know, things, as you mentioned, are are changing
- 4:29quite rapidly, and you never quite know exactly,
- 4:32how things are going to unfold. And so
- 4:35when you're looking at a environment like this,
- 4:37a political one where there's swings in one
- 4:39direction or another,
- 4:40how do you keep your teams focused in
- 4:42in making sure that you have the short
- 4:45term as well as the long term goals
- 4:46in mind with any decision that you're making?
- 4:50Yeah. I think we have to keep our
- 4:52our our finger very close to the pulse
- 4:54on what's going on,
- 4:56both at the federal level and here locally
- 4:59at our state level. So we do a
- 5:00ton of advocacy work, try to stay very
- 5:03connected
- 5:04on which way we may be moving with
- 5:06things like three forty b or,
- 5:09MFP
- 5:11or how are we looking at,
- 5:13our Medicaid days and
- 5:15making sure we,
- 5:16maintained the Medicaid days to continue to qualify
- 5:20for DISH status. And so I think just,
- 5:22you know, we have to stay very close
- 5:24as what's happening. So whether that be through
- 5:26our professional organizations,
- 5:28through our contacts and colleagues across the country,
- 5:31we really must keep really tight,
- 5:34again, your finger on the pulse there to
- 5:36see what where things are headed.
- 5:39That makes a lot of sense and, you
- 5:41know, a very level headed way to, approach
- 5:44approach future planning
- 5:46and how you can, you know, keep focused
- 5:48on the right things for the patients as
- 5:50as well as making sure you're ready for
- 5:52whatever comes our way. Now what do you
- 5:54think the hardest thing you're gonna have to
- 5:56do next year will be?
- 5:58Well, I would have said, three forty b
- 6:00rebate model, but, and that's still very may
- 6:03very well be depending upon, you know, where
- 6:05that ends up landing. But I think, you
- 6:08know, keeping keeping
- 6:10our eye on our retail pharmacy reimbursements, I
- 6:13think, is gonna be key with with MFP
- 6:16and making sure we're able to to see
- 6:18how that money is flowing and and flowing
- 6:20the right way.
- 6:22I think,
- 6:23we continue to experience some some technician staffing
- 6:26shortages.
- 6:27So continually growing the pipeline for our technicians.
- 6:30We actually implemented a program a couple years
- 6:32ago with three of our high schools in
- 6:34our county
- 6:35to get exposure to high school seniors into
- 6:38the pharmacy technician field.
- 6:40They do an internship with us and then
- 6:42by the time they graduate, they can sit
- 6:44for the national certification exam. We've been able
- 6:47to maintain a lot of those technicians on
- 6:49our workforce,
- 6:50following their graduation. So how are we doing
- 6:52things to address some of the staffing issues,
- 6:55that we may anticipate?
- 6:57Then obviously, always financial concerns,
- 6:59making sure that we continue to explore new
- 7:02revenue streams from a pharmacy perspective.
- 7:04We did that a few years ago when
- 7:06we opened up our home infusion
- 7:08business line. And so what are some of
- 7:10the other business lines that we can may
- 7:11look to to to go after?
- 7:15That's fascinating to hear. And I think especially
- 7:18interesting
- 7:19that you you've got such a seems like
- 7:21a great and,
- 7:23innovative partnership to bring that high school program
- 7:26to light to life.
- 7:29You know, when you think about that type
- 7:31of pipeline and leadership and bringing folks into
- 7:34the organization early, what does it take to
- 7:36set up a program like that, and and
- 7:38how have you seen it make a difference,
- 7:41overall as you're looking at staffing and and
- 7:43figuring out where talent is coming from for
- 7:45the future?
- 7:47Yeah. It was actually a really good collaboration,
- 7:49between our,
- 7:51Department of Education here in the county.
- 7:54They have a variety of technical programs that
- 7:57they offer
- 7:58kinda outside the normal high school,
- 8:00work day.
- 8:02And based on that program, we proposed to
- 8:04them as the pharmacy tech program.
- 8:07We take 10 technician or 10 tech students
- 8:10every year.
- 8:11And it's really been a hit and a
- 8:13success, not only for us, but I think
- 8:14also for the students and the opportunities that
- 8:16they get to get, you know, not only
- 8:18exposure to pharmacy, but exposure to to to
- 8:21hospital and health system work.
- 8:23I think, on this on a similar
- 8:25pathway, we're working very closely with our school
- 8:28of pharmacy
- 8:29to get into middle schools and high schools
- 8:31across the con across the state. I would
- 8:33love to do it across the country, but
- 8:35across our state to get, students excited about
- 8:38entering the pharmacy profession
- 8:40knowing that pharmacy school attendance, especially during COVID,
- 8:44really shrunk.
- 8:46And there was a fear of possible impending
- 8:48pharmacist shortage. But fortunately, attendance has rebounded and
- 8:52we're kind of back to what pre COVID
- 8:54numbers were into in terms of pharmacy school
- 8:57attendance. But really just getting the word out
- 8:59there about all the variety of opportunities that
- 9:01there are,
- 9:02in the field of pharmacy, whether it be
- 9:04a pharmacy, a pharmacist, or a pharmacy technician.
- 9:09That's great to hear. Now before we wrap
- 9:11up, I wanted to ask about growth as
- 9:13well. Where do you see some of the
- 9:14big opportunities for organizational growth over the next
- 9:16twelve to twenty four months or so?
- 9:19Yeah. About four years ago, we started up
- 9:21our own health plan called, Peak Health. It
- 9:24does have a pharmacy component
- 9:26in there, which I have somewhat oversight over,
- 9:29in collaboration
- 9:31with the Peak leadership.
- 9:33We have been working very hard to collaborate
- 9:36with our insurance offering in our retail slash
- 9:39specialty
- 9:40in order to improve medication adherence,
- 9:43improve medication access,
- 9:45ensure affordability
- 9:46for our insurance customers,
- 9:48really to make sure that they have access
- 9:49to those medications that will keep them well
- 9:51and keep them out of the hospital, as
- 9:53well as reduce kind of overall employee medication
- 9:56costs
- 9:57that the organization has to bear every year.
- 9:59So that partnership with peak health has been
- 10:02tremendous.
- 10:03And I only see that really continuing to
- 10:06grow into 2026,
- 10:082027.
- 10:10I love that. Doctor. Carpinski, thank you so
- 10:12much for your time today. This has been
- 10:13such a a fun and informative conversation.
- 10:16I look forward to connecting with you again
- 10:17soon and seeing you in April at the,
- 10:20live event. We have the chief pharmacy officer
- 10:22summit, coming up here, and I know you're
- 10:24speaking on a panel, so it'll be fascinating
- 10:26to hear a bit more about how you're
- 10:28thinking about things and dig deeper into some
- 10:29of the themes you talked about today as
- 10:31well. Yep. Yep. Looking forward to it. Thank
- 10:33you for your time.
