- 1:16Welcome to the Becker's Health Care podcast. I'm
- 1:18Mariah Taylor, assistant editor, and I'm thrilled to
- 1:21interview with Sachin Gupta, chief medical officer at
- 1:24UNC Physicians Network
- 1:26on the podcast today.
- 1:28Sachin, thank you so much for joining me.
- 1:30I'm excited for our conversation because you're one
- 1:33of our upcoming panelists on the CEO CFO
- 1:35conference, and this is such a fun way
- 1:37to get a sneak peek into some of
- 1:39your thought leadership. But before we dive into
- 1:41the questions, I would love if you could
- 1:43introduce yourself and tell us a little bit
- 1:44about your background.
- 1:47Sure.
- 1:48Well, first, thanks for having me on. I'm
- 1:50really excited to share a little bit about
- 1:52who I am and what we do here
- 1:53at UNC Physicians Network,
- 1:55and really looking forward to the, conference later
- 1:58this year. So as I said, my name
- 2:00is Sachin Gupta. I'm, I'm the chief medical
- 2:02officer for UNC Physicians Network
- 2:05here
- 2:06in Morrisville, North Carolina.
- 2:08We're under the UNC Health Care umbrella.
- 2:10I mean, UNC Physicians Network is the community
- 2:13facing arm of the health care system.
- 2:15Fred, statewide,
- 2:16we have approximately 700 clinicians and about a
- 2:19160,
- 2:20clinics,
- 2:21across the state of North Carolina,
- 2:23helping to really to bring care to to
- 2:26those communities that need it most and trying
- 2:27to keep care
- 2:29local. Our mission is really to serve all
- 2:31North Carolinians.
- 2:33I am a family physician by training. I'm
- 2:36board certified and
- 2:37a diplomat of the, American Academy of Family
- 2:41Practice.
- 2:43I did my training mostly up and down
- 2:46the East Coast. I've been with this organization
- 2:49now
- 2:49for about 14 years. I joined in 2010.
- 2:54Started as a a local medical director in
- 2:56a 2 physician practice.
- 2:58We started kinda de novo, and we were
- 3:01out on the sidewalks trying to recruit patients.
- 3:04And, over time, I moved into a regional
- 3:06medical director role.
- 3:08And then I've been in this chief medical
- 3:10officer role now since the start of the
- 3:11pandemic. So,
- 3:14just over 4 years now.
- 3:17Excellent.
- 3:18Tell me a little bit about one of
- 3:19your most successful projects or initiatives from the
- 3:21last year. What did you do, and what
- 3:23were the results?
- 3:25Yeah. So I'm really excited to talk a
- 3:27little bit about
- 3:28one of the projects that we had,
- 3:30around onboarding,
- 3:32and optimization.
- 3:33So launching an onboarding and optimization team here
- 3:36at UNCPN,
- 3:38that really helped to,
- 3:40tackle a few different,
- 3:42challenges that we had here.
- 3:45It sounds very much like it it is,
- 3:47onboarding and optimization, which we call the o
- 3:50two team. We are giving oxygen and life
- 3:53back to our teammates.
- 3:54And what this is around is all of
- 3:57our new clinics, new clinicians, and teammates that
- 3:59onboard,
- 4:00we have a robust onboarding process for them.
- 4:03So using a lot of standard work and
- 4:04standard processes
- 4:06to get folks ramped up. This includes
- 4:08epic support. So being able
- 4:10to get in front of the clinician ahead
- 4:12of their go live date, understanding what their
- 4:14preference list might look like, what they like
- 4:16their templates no templates to look like, and
- 4:18creating all of those things before they even
- 4:20start. So that way they can hit the
- 4:23ground running, and they're not having to deal
- 4:25with the struggles of,
- 4:26navigating the EMR from day 1.
- 4:29Another part of the o two team is,
- 4:32their work that they've done around scribing.
- 4:34To being able to deploy in person scribes.
- 4:37We're we're also
- 4:38taking the next iterative step around generative AI
- 4:41scribing, so running some pilots
- 4:44within the UNC Health Care System, virtual AI
- 4:46scribing,
- 4:47to decrease administrative burden.
- 4:49And then the last part is, around a
- 4:51centralized in basket team. So being able to
- 4:54remove
- 4:55those administrative
- 4:56tasks from the clinic that don't need to
- 4:58be done there, so things like medication refills
- 5:00and result notes,
- 5:02things that tend to clog up our clinicians,
- 5:04in baskets,
- 5:06those are the things that we're trying to
- 5:08centralize and make the work work life balance
- 5:11a little bit better for our clinicians.
- 5:13The results have really been fantastic. From an
- 5:15onboarding standpoint, we've seen our clinicians,
- 5:18be able to ramp up at a much
- 5:20faster clip. They're getting to their productivity thresholds,
- 5:24much sooner than those clinicians that were not
- 5:26put through the onboarding process. We see greater
- 5:28cohesiveness and cultural alignment in the clinics that
- 5:31have undergone the onboarding and optimization process.
- 5:35We're seeing efficiencies
- 5:36in workflows as well without with scribing as
- 5:39well as the centralized in basket work. So
- 5:41it's been really exciting
- 5:42to see all of these initiatives
- 5:44take flight and seeing how it really impacts
- 5:47the day to day lives of our, clinicians
- 5:49and and teammates.
- 5:51Yeah. That is amazing. As we all know,
- 5:53workforce issues like burnout and shortages have been
- 5:55a really hard part for health care in
- 5:58the last few years. So it's really cool
- 6:00to see what you're doing to help address
- 6:01those.
- 6:02Looking ahead, though, what is the biggest challenge
- 6:05you expect to face in the next year,
- 6:06and how are you preparing?
- 6:09Yeah. Well, I think you actually hit on
- 6:11it,
- 6:13right on the head, to be honest with
- 6:14you.
- 6:16You know, we just recently conducted our med
- 6:18staff survey, and a lot of the feedback
- 6:20that we got from our clinicians,
- 6:21one of the biggest challenges remains,
- 6:24decompression. You know, how can a clinician,
- 6:27shut off,
- 6:29that
- 6:30that that seemingly,
- 6:32unstoppable
- 6:34spigot of clinical care and how can they
- 6:36detach actually from work. So one of the
- 6:38challenges that we that we continue to face
- 6:41is how do we address this decompression
- 6:44and how are we able
- 6:45to really let our clinicians detach from work
- 6:47and and be able to, achieve that work
- 6:50life balance.
- 6:51So what we're doing for that is a
- 6:53lot of the projects that I just spoke
- 6:55about, we're really trying to expand those to,
- 6:57the larger health care system as well, bringing
- 6:59those,
- 7:00onboarding initiatives to the rest of the health
- 7:02care system, being able to expand scribing
- 7:05and expanding centralization
- 7:07of of,
- 7:09administrative work.
- 7:10I think that that's gonna be, a huge
- 7:13focus for us. Our clinicians,
- 7:15really feel this burden of care. They feel
- 7:18obligated to their patients, but what can we
- 7:20be doing,
- 7:21to make their work life balance a little
- 7:23bit better?
- 7:25So that's a huge focus for us and,
- 7:28really, part of the efforts that we're gonna
- 7:30be undertaking in the coming year.
- 7:32Amazing.
- 7:33Now let's look a little bit more about
- 7:34the chief medical role itself.
- 7:37You've been in this role for several years
- 7:39at this point. How do you see the
- 7:41role of chief medical officer evolving?
- 7:45Yeah. That that's a great question. And
- 7:47if I were to,
- 7:49you know, talk to my past self from
- 7:514 years ago
- 7:52to tell myself what kind of work I
- 7:54would be doing now, I don't think I
- 7:56would believe it. But, you know, I think
- 7:58traditionally, the role has been around well, I'm
- 8:00just gonna, focus on, you know, provider behavior
- 8:04issues and, trying to move the needle on
- 8:06quality. But I think it's so much more
- 8:08than that now. There is this greater pressure,
- 8:12to increasingly understand
- 8:14and leverage technology,
- 8:16in health care. I think that includes the
- 8:18electronic medical records, how we use telemedicine and
- 8:21data analytics.
- 8:23I think that there's this shift also this
- 8:25kind of ever evolving shift for value based
- 8:27care. So as we shift from volume to
- 8:30value based care,
- 8:32us as CMOs, we need to be focusing
- 8:34on improved outcomes while controlling costs.
- 8:37And I think part of that
- 8:39also comes with financial acumen. I think with
- 8:42health care costs under greater scrutiny,
- 8:45CMOs need to have a solid understanding of
- 8:48the financial aspects of the health care industry,
- 8:50and that means really partnering very closely with
- 8:53our operational dyad partners, our financial partners,
- 8:57so that we're getting the total picture of,
- 8:59the care that we deliver.
- 9:00I think there's also part of this around
- 9:02crisis management.
- 9:04You know, the recent events such as COVID
- 9:0619 have really highlighted the impact that CMOs
- 9:09can play
- 9:10in crisis management,
- 9:12really
- 9:12trying to model our team so that we
- 9:15can be adaptable, we can be resilient,
- 9:18and capable of leading through uncertainty.
- 9:20So I think that's how the chief medical
- 9:22officer role is is evolving, and it'll continue
- 9:25to evolve.
- 9:27What is your advice for aspiring CMOs?
- 9:32Yeah.
- 9:33Gosh. I I wish I had all the
- 9:34answers for that, but I I would think
- 9:36that one thing that I would certainly recommend,
- 9:39and one thing that I've really been fortunate,
- 9:42to have
- 9:43is being surrounded by
- 9:46people that you, trust,
- 9:49people that you can,
- 9:50really bounce ideas off of, people that are
- 9:52gonna challenge you and challenge the way that
- 9:54you think.
- 9:56I think we have a tendency to to
- 9:57surround people surround ourselves with people that think
- 10:00like us and and approach care like us,
- 10:03but you really need to have a diverse
- 10:04team
- 10:05and a team, again, that you can trust
- 10:07and delegate things to. I don't think
- 10:09any one single person can do all of
- 10:11the work that's required of us. So being
- 10:13able to delegate
- 10:14the folks that you can really trust, and
- 10:16you respect their ideas and you know that
- 10:18they're gonna take the ball and run with
- 10:19it, I think, is huge. And then also
- 10:22being able to,
- 10:23work with those people that are gonna challenge
- 10:25your opinions and thought, so that you can
- 10:27move the needle forward.
- 10:29Excellent.
- 10:30This has been such an informative discussion. Thank
