- 0:11Welcome to Running: A FEVER.
- 0:12My name is Michael Davis
- 0:13and this is a show about fitness, diet,
- 0:16and medicine. My goal is to live a
- 0:18long, healthy,
- 0:19happy, active life right up to the very
- 0:21end. It's all about livin' long and lovin'
- 0:23life.
- 0:24What's a prostate? What does it do?
- 0:29Why is it important, and what do we
- 0:31need to know
- 0:31about how healthy it is?
- 0:34I hope to answer these questions in today's
- 0:36episode.
- 0:37What is the prostate? It's a gland that
- 0:40surrounds the urethra of males at the base
- 0:43of the bladder,
- 0:43comprising a muscular portion and a glandular portion.
- 0:48What does it do? Well, the muscular
- 0:51portion controls the release of urine, and the
- 0:54glandular portion secretes an alkaline fluid
- 0:58that makes up part of the semen and
- 1:01enhances the motility and fertility of sperm.
- 1:06Yuck.
- 1:07I think you can see why it is
- 1:09important. Since females do not have a prostate,
- 1:11I was wondering what they use to control
- 1:13urine flow. Turns out there is no specific
- 1:16organ that
- 1:17does this. It's a combination of muscles and
- 1:19nerve signals. Let's get to the meat of the subject,
- 1:23prostate health. There are several related diseases and disorders.
- 1:28Number one, prostate cancer. The good news about
- 1:32prostate cancer is that though it
- 1:34is one of the most common types of
- 1:36cancer, it is usually found early and it
- 1:39often grows
- 1:40slowly. Most people with prostate cancer are cured.
- 1:45Treatments include surgery,
- 1:46radiation therapy, and careful monitoring.
- 1:51Number two, benign prostatic hyperplasia,
- 1:56BPH. This is also called an enlarged prostate.
- 2:00This causes blockage of the flow of urine
- 2:02out
- 2:03of the bladder, and can also cause bladder,
- 2:06urinary tract or kidney problems.
- 2:09There are many
- 2:10options for treatment depending on your symptoms, the
- 2:13size of your prostate, and other health problems
- 2:15you may have. Symptoms include frequent or urgent
- 2:19need to urinate, urinating more often at night,
- 2:24trouble starting to urinate, weak urine stream, or
- 2:28a stream that starts and stops,
- 2:30dribbling at the end of urination, or not
- 2:33being able to fully empty the
- 2:35bladder. Also, the uncommon symptoms of urinary tract infection,
- 2:42not being able to urinate at all, and blood in the urine.
- 2:48Usually symptoms of BPH tend to slowly get
- 2:51worse, but they can sometimes stay the
- 2:54same
- 2:57or even improve
- 3:00over time. The cause of BPH may be
- 3:03side effects of
- 3:04medicines like powerful pain relievers, opioids, cold and
- 3:08allergy medicines, or older medications for
- 3:10depression called tricyclic antidepressants. The main cause is
- 3:16just aging. Nearly all men
- 3:18experience some prostate growth if they live long
- 3:24enough, with 50% of men over 60
- 3:27showing signs.
- 3:28This is why I'm spending so much time
- 3:31on BPH. This combines with hormonal shifts.
- 3:35As men age,
- 3:36lower testosterone levels coupled with stable estrogen levels
- 3:39may trigger cell growth. You
- 3:42may have genetic predisposition. The prognosis is actually
- 3:48pretty good. It has nothing to do with
- 3:50prostate cancer. It is manageable through lifestyle changes,
- 3:53medication, or minimally invasive procedures,
- 3:57with many cases staying stable.
- 3:59The medication used is often alpha-blockers or
- 4:025-alpha-
- 4:03reductase inhibitors. There is also a surgical procedure
- 4:07called TURP, which is removal of
- 4:11inner portions of the prostate gland through the
- 4:13urethra, requiring no external incisions.
- 4:18Is there any hope for reduction in size
- 4:20of an enlarged prostate? Yes.
- 4:22The most effective
- 4:23long-term reduction is achieved with the aforementioned
- 4:265-alpha-reductase inhibitors
- 4:29like finasteride, which can shrink the prostate by
- 4:3220 to 30 percent over 6 to 12
- 4:35months.
- 4:37There are other treatments, such as prostate artery embolization,
- 4:40which reduces blood flow to the
- 4:42prostate, causing it to shrink.
- 4:45Steam therapy uses water vapor to destroy excess
- 4:48prostate tissue.
- 4:50Laser therapy can destroy or remove overgrown tissue.
- 4:55Microwave therapy can also help.
- 4:57The most accessible, if not the easiest, way
- 5:00to manage BPH is lifestyle change. Exercise.
- 5:05Regular physical activity helps manage weight and supports prostate
- 5:09health.
- 5:11Dietary adjustments. Consuming a diet rich in vegetables
- 5:14and healthy foods can support prostate
- 5:16health. Managing stress can also help control urinary
- 5:21symptoms. Okay, I really wanted to spend
- 5:25a lot of time on BPH.
- 5:27There is one other health issue that I'd
- 5:29like to talk about, prostatitis.
- 5:32This is a condition most often linked with
- 5:35swelling and irritation called inflammation.
- 5:39It can make it painful or hard to
- 5:41urinate. It also may cause pain in the
- 5:43groin,
- 5:44pelvic area, or genitals. It is often caused
- 5:47by bacterial infections. Get medical help immediately
- 5:51if you're unable to pass urine, have a
- 5:54fever and trouble urinating or pain while urinating,
- 5:57have blood in your urine, or have a
- 6:00lot of discomfort and pain in the pelvic
- 6:02area or genitals.
- 6:04Treatment includes antibiotics, alpha-blockers for urinary flow,
- 6:08or anti-inflammatories.
- 6:11As a 60-year-old man, this topic
- 6:13is important to me, and it's good to
- 6:15know there are treatments
- 6:16and hope for men with these problems.
- 6:18I hope this will also be helpful to you or a man in your life.
