- 0:40president and CFO of Nemours Children's Health. Caswell,
- 0:43thanks so much for being on with me
- 0:45today.
- 0:46Thank you for having me. Looking forward to
- 0:48the discussion today. Me too. And happy 2026
- 0:51to you as well.
- 0:53Yes. Happy New Year to you as well.
- 0:55Wonderful. So, now we've had you on the
- 0:57podcast before, but for those who, did not
- 1:00have the pleasure of listening to our first
- 1:02discussion, do you mind sharing just a little
- 1:04bit about yourself and Nemours with our with
- 1:06our listeners?
- 1:08Sure. I'll be excited to. Caswell Sams. I'm
- 1:11the executive vice president and chief financial officer
- 1:14for Nemours Children's Health.
- 1:16Been working in health care finance for over
- 1:18twenty five years. Started my health care career
- 1:21consulting.
- 1:23Decided real quickly,
- 1:25that I did not wanna be in consulting.
- 1:27I actually wanted to work for a provider,
- 1:29so I joined the provider side shortly after
- 1:31that, and just continued to grow my career,
- 1:33and I'm excited as of,
- 1:36March
- 1:372025
- 1:38to be a part of the Nemours Children's
- 1:41Health Leadership Team.
- 1:43And do you mind just sharing a little
- 1:45bit more about Nemours with us as well?
- 1:47Sure.
- 1:48Nemours is the only multistate
- 1:52pediatric
- 1:53health system in The United States. We provide
- 1:56a multitude of different,
- 1:58provider based services,
- 2:00primary care, prevention, and health.
- 2:02We have two hospitals, and our main and
- 2:05primary markets that we operate in are in,
- 2:08Delaware and the surrounding states, as well as
- 2:11the state of Florida.
- 2:13We have over 10,000
- 2:15associates,
- 2:161,100
- 2:16employee physicians
- 2:18operating
- 2:1980 different locations,
- 2:20provide
- 2:22more than,
- 2:23you know, 2,200,000
- 2:25patient encounters on an annual basis.
- 2:28And our ultimate goal as we continue to
- 2:32provide for all children
- 2:34is to create the generation
- 2:35of the healthiest children in the world through
- 2:38our whole child health strategy that we have
- 2:40here at Nemours. Yeah.
- 2:42Incredible. Thank you for sharing that background.
- 2:45So let's dive in. I know we just
- 2:47said it's the new year entering a whole
- 2:49new
- 2:50cycle. So as we are now here in
- 2:532026,
- 2:54could you share maybe some financial trends or
- 2:56shifts that you're keeping a close eye on
- 2:58from your perspective
- 3:00and why you're watching me so closely?
- 3:03Yeah. I think when you have an organization
- 3:05like Nemours, especially a pediatric health system
- 3:09that has a large portion of Medicaid services
- 3:13and patient population
- 3:15that it provides.
- 3:16Our our focus is on things that can
- 3:18impact inflation as well as reimbursement
- 3:21growth for the organization,
- 3:23as well as how do we provide access.
- 3:24So I'm quite sure these are top of
- 3:26mind for
- 3:27every organization.
- 3:28Number one, what's gonna happen with the three
- 3:30forty rebate model? Obviously,
- 3:33that continues to be litigated, and hopefully, providers
- 3:36come out on top. Lesser to the case,
- 3:38the ACA subsidies, but we do believe as
- 3:41a pediatric health system, there may be a
- 3:43halo effect
- 3:44on the pediatric services that organizations have. So
- 3:47we're continuing to monitor that as well. And
- 3:50then, how do we
- 3:52mitigate
- 3:53the potential growing impacts of HR one? So
- 3:56we're continuing to see what we can do
- 3:58either from an advocacy perspective or an education
- 4:01perspective to our government officials on providing
- 4:05those type of educations that they need on
- 4:07the implications that it has on pediatric health
- 4:09systems.
- 4:11Yeah. One of the top things that we're
- 4:12focusing on for 2026
- 4:14is,
- 4:16purposeful growth.
- 4:17Not just growing to just grow, but purposeful
- 4:20growth. And, you know, how do we implement
- 4:22new technologies and get more efficient as an
- 4:24organization?
- 4:26Mhmm. And to to kind of follow-up there,
- 4:28when we connected on the podcast in I
- 4:30believe it was 2025,
- 4:32you said Nemours
- 4:34wasn't in, quote, panic mode, you know, despite
- 4:36some of the uncertainties
- 4:38tied to HR one or, you know, more
- 4:40commonly known as the one big beautiful bill
- 4:42act. So, how has that steady approach you
- 4:45and I discussed kind of held up, and
- 4:47what have you maybe learned since then?
- 4:49No. I would probably say we are still
- 4:51not going to take our eye off our
- 4:53strategy. We're gonna still move full steam ahead
- 4:56as an organization.
- 4:58Again, we're gonna monitor. We're gonna advocate. We're
- 5:00gonna continue to do what we believe is
- 5:02best for the organization, partnering with the different
- 5:04hospital associations we have. So that from a
- 5:07strategy perspective has not changed for us. We're
- 5:10gonna continue to do the things that we
- 5:11believe we need to do
- 5:13as it represents our mission from as an
- 5:16organization, as well as our strategy to make
- 5:19sure we create the healthiest generation of children
- 5:22moving forward, and our focus around the whole
- 5:24child health is gonna help us do that.
- 5:27So we understand that, hey. Things are gonna
- 5:29change. We're preparing to do what we need
- 5:32to do internally to mitigate any potential negative
- 5:35impacts on the organization. So we're not gonna
- 5:38continue
- 5:38to rest on our laurels, but we're gonna
- 5:40continue to see what we can do to
- 5:42say what what type of impacts do we
- 5:45need to make or changes we need to
- 5:46make and provide that education to our board,
- 5:48our leadership team, and prepare for potentially negative
- 5:51outcomes in the future. But we believe we
- 5:53have a very solid strategy to help address
- 5:55those issues,
- 5:56but can continue to monitor and advocate as
- 5:59we need to, to make sure that pediatric
- 6:01health systems get the, you know, most appropriate
- 6:04fair reimbursement on the Medicaid population that they're
- 6:07servicing.
- 6:08Right. Essentially, you're ready for anything
- 6:11Very true. That it has to throw you.
- 6:12Well, you also just talked about purposeful growth
- 6:16this year. So could you dive a little
- 6:18deeper into what that looks like and maybe
- 6:19you see where you see some of the
- 6:20biggest opportunities for this type of smart growth?
- 6:24Yeah. I think for us, we're looking at
- 6:25our markets and saying, hey. What do we
- 6:28need to grow? How do we leverage the
- 6:29clinical data we have? How do we understand
- 6:32the community needs?
- 6:34How do we deepen our payer partner ships
- 6:36either through value based and risk aligned arrangements?
- 6:39Number one, in our Delaware market,
- 6:41we have what's called a global budget where
- 6:44essentially
- 6:45we're going to cap our own dollars and
- 6:47take a little bit more risk in the
- 6:48near future with the state of Delaware on
- 6:51the Medicaid and the Medicaid HMO population. So
- 6:53how do we create that value based strategy
- 6:56and make sure we bring more value
- 6:58than just utilization
- 7:00and unnecessary utilization
- 7:02into our Delaware market.
- 7:04On top of that, we're still focusing on
- 7:06those services that need to
- 7:08have a positive impact on the Delaware market
- 7:12as well. So the expansion of our
- 7:15Lisa Dean Mosley Foundation Institute for Cancer and
- 7:18Blood Disorders.
- 7:19We're excited about the opportunity for more patients
- 7:23in this market
- 7:24not to have to leave the market for
- 7:26cancer services. So that is gonna that is
- 7:29a state of the art service that we're
- 7:30expanding
- 7:31in this market. The other expansion that we're
- 7:34doing is our advanced delivery unit in this
- 7:37market, mostly related to maternal fetal medicine interventions
- 7:41that need to have for stable
- 7:43obstetric patients in the Delaware Valley market. I
- 7:46think it's a great opportunity for the patient
- 7:48population not to have seek services out of
- 7:51this immediate area and be able to come
- 7:54to Nemours and not only for us to
- 7:56be able to take care of the neonate
- 7:58perspective of the
- 7:59of the child, but also
- 8:01provide those services that a mother may need,
- 8:04who may be in need of their perinatology
- 8:06services here at Nemours as well. So we're
- 8:08excited about that continue to grow for us.
- 8:11Yeah. And in, 2025,
- 8:13I covered a a good amount of just
- 8:15maternity care closures
- 8:17in general.
- 8:18But, do know that Nemours has been, you
- 8:20know, making major investments in these areas just
- 8:23in terms of child and, you know, mother.
- 8:26So how important is that, to your organization
- 8:29and just the continued growth there?
- 8:31Yeah. For Nemours, I think this is critical.
- 8:34You know? We're not looking at just traditional
- 8:36obstetric expansion.
- 8:38Yep. But we believe that this is gonna
- 8:40be a way for us to stabilize what's
- 8:43happening across the maternity market.
- 8:46You know, there's a lot of safety nets,
- 8:48a lot of,
- 8:49as you pointed out, to have maybe some
- 8:51some fragmented
- 8:53impacts on how they handle high risk pregnancy
- 8:56for us. You know, we're looking at the
- 8:58upstream impact,
- 9:00identifying risk earlier,
- 9:02serving, seeing as a regional anchor for high
- 9:05acuity maternal and neonate care, and then taking
- 9:08an approach that, hey. We're gonna stabilize
- 9:10this pediatric
- 9:11service by creating a
- 9:14more predictable pipeline for our NICU,
- 9:16addressing complex pediatric care by also creating that
- 9:20continuity for pregnancies
- 9:22through,
- 9:23you know, high risk opportunities.
- 9:25And, you know, it's not about chasing volume.
- 9:27It's about delivering high quality of care. So
- 9:29we're not looking at this from a numbers
- 9:31perspective. We're looking at it from more of
- 9:33a holistic service line perspective.
- 9:35Yeah. No. That's great to hear. And, you
- 9:38know, just amid, you know, plans to grow
- 9:40and as we just touched on a little
- 9:42bit earlier, some of the uncertainties that we
- 9:45are seeing in the industry.
- 9:46From your perspective, what leadership mindset will be
- 9:49most important for financial leaders navigating 2026?
- 9:54Yeah. I think it's gonna be critical,
- 9:56for
- 9:57financial leaders to have a different perspective.
- 10:00I don't think we can continue to operate
- 10:03as financial scorekeepers.
- 10:06We're gonna have to be more strategically
- 10:09engaged,
- 10:10very fluent on clinical operations,
- 10:13technology,
- 10:15artificial intelligence,
- 10:16quality, and just workforce dynamics. I think that
- 10:19means, as a financial leader,
- 10:22a little bit of rigor and empathy is
- 10:23gonna create a lot of
- 10:26longevity for your organization.
- 10:28Adding that with, you know, making sure you
- 10:30can make decisive
- 10:31decisions with adaptability
- 10:33as it's needed. Mhmm. You know, I think
- 10:36that's gonna help CFOs who, you know, might
- 10:39need clarity in in uncertain times because we
- 10:42never know what's gonna be the next
- 10:44potential
- 10:46hill we have to climb as we move
- 10:47forward in 2026.
- 10:49I think that helps bring
- 10:51confidence to your the rest of the executive
- 10:53team members as well as the board,
- 10:56because, you know, we're gonna continue to make
- 10:58informed data driven decisions.
- 11:01But, you know, we know that the the
- 11:03path is not perfectly clear moving forward, but,
- 11:06we're gonna continue to make informed decisions as
- 11:08a leadership team and and continue to make
- 11:11sure that whatever we do is just not
- 11:13to grow to grow, but it's for purposeful
- 11:15growth reasons.
- 11:16Yeah. Yeah. And you touched on technology. Is
- 11:19there, you know, a piece technology, a tool
- 11:21right now, or just an area that you
- 11:23are personally keeping an eye on?
- 11:26Yes. I mean, we are continuing to make
- 11:29strategic and investments for us of our advanced
- 11:32care at home program that we're having, which
- 11:34is kinda more of a telehealth medicine model.
- 11:37We believe we've had a number of early
- 11:39successes with it,
- 11:41especially in our Florida market to kinda keep,
- 11:45patients out of the emergency room, keep our
- 11:48patients
- 11:49discharged earlier from our PICCI or our NICU.
- 11:51And, you know, because of the reach we
- 11:54have and people seeking, you know, specialized services,
- 11:57we have, you know, families who may travel
- 11:59seventy, ninety miles. Imagine as a mother or
- 12:03a father, you don't have to travel, and
- 12:05you can kinda just
- 12:06log in to, a telemedicine system and have
- 12:09a discussion with a nurse and be able
- 12:11to, you know, alleviate any concerns you may
- 12:13have instead of having an unnecessary visit in
- 12:15the emergency room. So we're we're significantly invested
- 12:19and can see and that continue to grow,
- 12:20and we actually just implemented that in our
- 12:22Delaware market as well.
- 12:24Wow. Now those I mean, the technologies we're
- 12:26seeing right now are so incredible. So definitely
- 12:28worth keeping an eye on. Final question for
- 12:31you here.
- 12:32Looking at 2025,
- 12:33what's one lesson that you learned that you're
- 12:36looking to carry into the new year?
- 12:39I'd probably say we have to move faster.
- 12:42And then move faster, making informed decisions.
- 12:46You know, I think the volatility that we
- 12:49experience from a, you know,
- 12:51a a a governance or a government perspective
- 12:54is gonna continue to create challenges for us.
- 12:57So, you know, we're looking at our capital
- 12:59allocation.
- 13:00We're making sure we're aligned as a leadership
- 13:02team, but we definitely have to move faster
- 13:04and continue to make
- 13:05informed but better decisions. So we're gonna double
- 13:09down on our fundamentals. You know? We're gonna
- 13:11be as agile as we possibly can.
- 13:14We trust our leadership team to make informed
- 13:17decisions.
- 13:18We're gonna have the appropriate financial durability that
- 13:20we need,
- 13:21you know, which is no longer an option
- 13:23in health care. And, you know, I think
- 13:25that'll continue to help us make sure we
- 13:27serve our mission in a way that we
- 13:29wanna serve, which is all about our whole
- 13:31child health. Yeah. Well, Caswell, as always, it
- 13:35has been such a pleasure getting to chat
- 13:36with you and hear about all the exciting
- 13:38things going on at Nemours.
- 13:41Really looking forward to hopefully connecting again soon.
- 13:43So I appreciate you taking the time.
- 13:46Thank you.
