- 0:00Hi, everyone. This is Lucas Voss with Becker's
- 0:02Healthcare. Thanks so much for tuning in to
- 0:04the Becker's Healthcare podcast series. It's exciting to
- 0:07have you. And we're talking about an exciting
- 0:10topic that's seen a lot of movement lately,
- 0:12payer compliance
- 0:14and regulatory oversight in health care. And I'm
- 0:16so excited to be joined by Sarisa Horner,
- 0:19director of policy and payment integrity at Saris.
- 0:23Sarisa, thanks so much for being here today
- 0:25and taking the time. Thank you for having
- 0:27me.
- 0:28It's great to have you. And I wanna
- 0:30start off with introductions for our audience. Could
- 0:32you introduce yourself and just share a little
- 0:34bit about your work in health care? Thank
- 0:37you so much. So my name is Theresa
- 0:38Warner. I'm the director of policy and payment
- 0:41integrity here at Saris. I've been with Saris
- 0:43a little bit over eight years, but actually
- 0:46in the health care industry around eighteen years.
- 0:50I started out in health care revenue cycle,
- 0:52working for hospitals and really gaining a deep
- 0:55understanding
- 0:56of what that revenue cycle process is from
- 0:59beginning to end and then transition over to
- 1:02the payment integrity side,
- 1:04working with vendors like Sarris who perform work
- 1:07on behalf of payers.
- 1:10With Sarris, I actually oversee a few departments,
- 1:13between provider relations,
- 1:15recovery,
- 1:16our internal QA,
- 1:18and also policy review and implementation.
- 1:21And, Sarrisa, I feel like we need that
- 1:23deep understanding today because there's a lot of
- 1:26movement as I've mentioned. There's a lot of
- 1:28things are happening,
- 1:30within the payer oversight space. Right? There's CMS
- 1:33ramping up Medicare Advantage audits to to states
- 1:36issuing fines related to reimbursement
- 1:39practices and and mental health parity. There's so
- 1:41much happening right now. And I'm I'm just
- 1:43curious from your perspective,
- 1:45what are some of those most pressing challenges
- 1:47that you're seeing that health plans face and
- 1:49and adapting to that new scrutiny to all
- 1:52of that movement both on the federal and
- 1:54state levels?
- 1:56I'd say consistency.
- 1:59Yeah.
- 2:00You know? And I'd say that that's all
- 2:02the way around,
- 2:03whether that's from state and federal guidance and
- 2:07implementation
- 2:08to their own
- 2:10internal policies.
- 2:12It's very hard
- 2:13to be consistent across different states,
- 2:17when it comes to specific guidelines, especially when
- 2:19you think about,
- 2:21something like mental health purity. Right?
- 2:24I'm not sure quite how many people are
- 2:26familiar with it, but mental health purity basically
- 2:29means that
- 2:30insurance plans have to treat
- 2:32mental health and substance
- 2:35use disorders
- 2:36the same way that they would treat
- 2:39any other condition
- 2:40that a a patient may have.
- 2:43Yeah.
- 2:44You know? And it's it's it's very hard.
- 2:46You know, we would think that it's 2025,
- 2:49so all benefits would be treat be treated
- 2:51equal.
- 2:52But, unfortunately, that's not true. So different states
- 2:55have different guidelines when it comes to that.
- 2:58And they're basically trying to make sure that
- 3:01the co pays
- 3:02and the deductibles,
- 3:05the number of of visits that are available
- 3:07for services,
- 3:09are the same. Right?
- 3:10Even down to pre authorization,
- 3:13making sure that if an authorization is not
- 3:16required
- 3:17for a same or similar,
- 3:19physician health benefit or physician health service, that
- 3:23that mental health benefit
- 3:25is treated in the same way.
- 3:27And you've mentioned,
- 3:29again, consistency and how hard it is to
- 3:31maintain consistency, and it's it's crucial for success,
- 3:35obviously. Now consistency becomes even even harder when
- 3:39things get more complex, right, as we've talked
- 3:42about, and and when things become very dynamic
- 3:44because you have to change constantly. And again,
- 3:46that obviously impacts consistency there. What are some
- 3:49of the strategies that you're seeing payers adopting
- 3:52right now to to keep up? Right? And
- 3:55I was wondering if you'd also have any
- 3:56any sort of example of of tools or
- 3:59or examples of of your your work that
- 4:01stand out to you.
- 4:03Yeah. I would say that,
- 4:05consistency is is is pretty
- 4:08challenging because
- 4:10most payers,
- 4:11even most health systems,
- 4:13and even vendors,
- 4:14you have multiple systems that don't always talk
- 4:17to each other. You know? And they are
- 4:20not always compatible
- 4:22with each other. And so
- 4:24it's really trying to be able to be
- 4:27proactive
- 4:28because that's what, you know, we're looking to
- 4:30do when you are,
- 4:32implementing different federal and state guidelines.
- 4:35And there is a couple of different ways
- 4:38to do that, but it does require everybody
- 4:41working together,
- 4:42and having a central vision,
- 4:45and being
- 4:46very clear
- 4:47about what that end goal,
- 4:50and the outcome is that we want to
- 4:52have. So I would say first,
- 4:56is that everyone kind of talks about AI.
- 4:59And I think AI is very important,
- 5:02when it comes to looking at at payment
- 5:04integrity
- 5:05and how data is viewed
- 5:08and how data is collected
- 5:11across different,
- 5:13patient health groups. And so when you look
- 5:15at something like risk adjustment,
- 5:18CMS has really ramped up their audits on
- 5:21that. And so how do payers actually
- 5:26tackle that issue
- 5:27because
- 5:28negative impacts from those risk adjustment audits can
- 5:32have very significant financial impact on a payer.
- 5:34And sometimes it can be due,
- 5:37to the smallest piece of,
- 5:40documentation
- 5:41either being missing or documented incorrectly.
- 5:44And so one of the things that a
- 5:46lot of payers do is that they work
- 5:49with their internal teams. I like to call
- 5:51them cross functional,
- 5:52where we're really making sure that
- 5:55different departments, whether that's claims or finance,
- 6:00are really inputting the information
- 6:03that's needed to get down to that granular
- 6:05level,
- 6:06that CMS will be looking for.
- 6:09Yeah. And you've touched on a couple of
- 6:10important things here. One, obviously, is being able
- 6:12to eliminate silos and having conversations. I I
- 6:15again, I love the cross functional piece, right,
- 6:17that's so important for success today and being
- 6:20proactive
- 6:21and, again, AI, such a big piece of
- 6:24the conversation as well.
- 6:26What are some of those additional capabilities
- 6:28or approaches that that you're looking towards for
- 6:31the future here for for, obviously, the 2025,
- 6:34but 2026
- 6:35as well that are really critical for payers
- 6:38to look at as they're continuing to to
- 6:40navigate the space? And and what's one thing
- 6:43that that you think everybody should be doing
- 6:45right now to be able to be prepared?
- 6:48I think that everyone should be looking beyond
- 6:50just traditional fraud, waste, and abuse.
- 6:53We all have
- 6:55very comprehensive
- 6:56systems
- 6:57for the most part that are able to
- 6:59track that. Now ideally, we would like for
- 7:02all of that to be done prepay,
- 7:05and that's where the industry is trying to
- 7:07move toward is having a lot of these
- 7:09audits that are conducted post pay
- 7:12transition
- 7:13to prepay reviews. It's a lot cleaner and
- 7:16it's a lot easier for both parties,
- 7:18meaning the payer and also the provider.
- 7:21But we really have to look
- 7:24at our AI discussion and our, software discussion
- 7:30as being able to identify some predictive compliance
- 7:34modeling. Right?
- 7:35There are ways to use NLP
- 7:38and machine learning
- 7:40to make the review of records and policies
- 7:44much more consistent in being able to identify,
- 7:48I say, anomalies in the data or areas
- 7:51to focus on beyond just our traditional FWA.
- 7:55Yeah. As you pointed out earlier in the
- 7:57conversation, this is something that never stands still.
- 7:59There's it's always evolving. There's always something new
- 8:02happening. So it's really becomes critical to, as
- 8:04you've mentioned, be proactive and and looking at
- 8:07the tools that can make this better in
- 8:08the long run. Sarisa, again, thank you so
- 8:10much for your time and insights today. I
- 8:12I wanted to open the floor up to
- 8:14you here as we close out our conversation.
- 8:16Any fun final thoughts or anything that we
- 8:18didn't cover that you'd like to share with
- 8:20the audience?
- 8:21I would say, you know, look. It's just
- 8:23in general.
- 8:24Compliance is no longer really about just checking
- 8:27the box. You know? For decades, we have
- 8:30kinda just checked the box of just the
- 8:32standard things that we're looking for, whether that's
- 8:35timely filing, no authorization,
- 8:37met NEC. And as we are seeing with
- 8:39the changes in federal guidance and even down
- 8:42to state guidance, we are seeing that reimbursement
- 8:46when it comes to health care claims is
- 8:48a bit more fragile than what we have
- 8:50seen in the past. So it not only
- 8:53requires
- 8:54providers to be more agile,
- 8:56but also payers to be very agile.
- 8:59And one of the ways that I know
- 9:01that payers do is that they partner with
- 9:03vendors like Cirrus, where we're able to give
- 9:06them that industry knowledge, and we can actually
- 9:09discuss what does data transparency
- 9:12actually look like. How do you become standard,
- 9:15in the information that you're sending, and then
- 9:17how do you tighten up those internal processes,
- 9:21and make your reimbursement policies most effective and
- 9:24most in line with what your goals are.
- 9:26Yeah. Well, Theresa, again, thank you so much
- 9:29for being here today. It's fantastic to have
- 9:30you. Thanks for your insights and your time.
- 9:33Thank you very much as well. And I
- 9:34feel like we'll have to revisit this at
- 9:36some point with everything that's happening. And we
- 9:38also want to thank our podcast sponsor, Cirrus.
