Minnesota’s Medicaid program is extending its freeze on new providers for at least six more months as part of a sweeping effort to root out fraud and ensure only legitimate providers remain on the rolls. Since January, officials have reviewed over 5,500 providers, removing more than 3,400 — though only 59 were flagged for possible fraud. Nearly 2,700 have appealed their removal.
