- 0:02- There's a small circle of people who truly
- 0:04- understand what it takes to lead a health
- 0:05- system right now. The decisions are enormous.
- 0:08- The variables are constantly shifting, and the stakes
- 0:11- of getting a call wrong don't stay contained.
- 0:14- That's not a complaint. It's a structural reality.
- 0:17- And it's why the most effective c suite
- 0:19- leaders are deliberate about where do they go
- 0:21- to pressure test their thinking.
- 0:22- Becker's fourteenth annual CEO and CFO roundtable
- 0:26- brings together more than 1,500
- 0:28- executive level attendees
- 0:30- and nearly 400 speakers for four days built
- 0:32- around growth, financial sustainability,
- 0:35- workforce strategy, and the leadership questions that don't
- 0:38- have easy answers.

- 0:51- Welcome to the Becker's Healthcare Clinical Leadership podcast.
- 0:54- I'm Mariah Taylor, assistant editor, and I'm thrilled
- 0:57- to be speaking with Mandy Richards,
- 0:59- chief nursing executive and president of children's health
- 1:01- at Intermountain Health. Thank you so much for
- 1:04- joining me today, Mandy. I'm so excited to
- 1:06- learn more about your career ladder for your
- 1:08- nurses. But before we dive into all of
- 1:11- that, I'd love for you to introduce yourself
- 1:12- and tell us a little bit about your
- 1:13- background.
- 1:15- Maria, thank you so much for having us
- 1:17- on today.
- 1:19- I'm just thrilled to be here. I have
- 1:21- been a nurse for over twenty five years,
- 1:23- obviously started in Australia,
- 1:25- and have been in nursing leadership positions for
- 1:27- over twenty years.
- 1:29- And I'm really excited to be here at
- 1:31- Intermountain Health and excited to share some of
- 1:33- our great work today. So thank you again.
- 1:36- Yes. I'm so excited to hear it. Now
- 1:38- some of our readers who have been reading
- 1:41- recent articles here at Becker's might be familiar
- 1:44- with a few of the programs we're about
- 1:46- to talk about because I wrote about it
- 1:47- not that long ago. So if this sounds
- 1:50- familiar to you, that might be why. But
- 1:53- we're gonna do a deep dive into two
- 1:55- of the cool career ladder initiatives you have.
- 1:57- One being I Aspire.
- 1:59- So two years ago, Intermountain launched this initiative
- 2:03- that really helps nurses
- 2:05- figure out how to advance their careers, both
- 2:07- at the bedside and beyond. So can you
- 2:09- tell us a little bit more about I
- 2:10- Aspire and how it works?
- 2:13- Sure can. So I love it when we're
- 2:15- rounding with our nurses
- 2:16- and we hear some of the things that
- 2:19- they're interested in. And so we were rounding
- 2:20- in the ICU,
- 2:22- and some of our nurses said we would
- 2:23- love to stay at the bedside and grow
- 2:25- and develop in our careers,
- 2:27- but there's really not a program to do
- 2:29- that. It feels like we have to move
- 2:31- into
- 2:32- nurse educator roles or nurse manager roles in
- 2:35- order to advance.
- 2:36- And I'm like, that shouldn't be the way.
- 2:38- And so with that, we pulled together some
- 2:41- work groups to say, well, how would we
- 2:43- create a program where nurses can thrive in
- 2:46- our organisation and at the bedside?
- 2:48- So we have nurses that
- 2:51- are in really acute care facilities where there's
- 2:53- a breadth to there's a depth to their
- 2:55- clinical knowledge that they need. But we've also
- 2:58- got nurses in rural facilities where
- 3:00- there is breadth to what they need to
- 3:02- know, and that might be where one moment
- 3:05- they're delivering a baby in labour and delivery,
- 3:07- and then they're running down to the ER
- 3:08- because there's a trauma, and they're kind of
- 3:10- needing that breadth of expertise.
- 3:13- So as we're developing
- 3:15- the program, we're like, how do we accommodate
- 3:18- a program that really is accessible to all
- 3:20- of our nurses, not just those that have
- 3:23- the acute, you know, high ICU
- 3:27- competencies that they need, but for all of
- 3:29- our nurses that spend more than 50% of
- 3:31- the time at the bedside. So and we
- 3:33- actually included our LPNs as well. And so
- 3:35- we actually started by including our nurses and
- 3:38- our nurse educators
- 3:39- to come up with what this looks like.
- 3:41- They looked to the literature, and what we
- 3:43- found is there's a lot of programs out
- 3:45- there that just look at the clinical ladder,
- 3:47- but there's not a lot that look at
- 3:49- the professional development side as well, which is,
- 3:52- what we've talked about before, which is advanced
- 3:54- degrees
- 3:55- that are used at the bedside,
- 3:57- certifications,
- 3:58- evidence based practice projects, and things like that.
- 4:01- And so we came up with IASPIRE,
- 4:04- which has five levels to it, and it
- 4:06- really is accessible to all of our nurses
- 4:08- to continue to grow. And that was what
- 4:10- was really important to us no matter where
- 4:11- they work in our system.
- 4:14- That's amazing.
- 4:16- And since you've been running this program, what
- 4:18- have some been some of the most impactful
- 4:20- results?
- 4:22- Yeah. It's been great to work with our
- 4:24- with our nurses to say, okay. Was it
- 4:26- easy to apply? How how what would you
- 4:28- do to change the program? And so we
- 4:30- have an intake process each year. But some
- 4:32- of the benefits that they found was their
- 4:35- professional growth.
- 4:37- They felt that they were in had more
- 4:39- motivation because they could see the impact of
- 4:41- what they were doing. The quality improvement, obviously,
- 4:44- those projects enhance the the care that we're
- 4:47- delivering. But the thing that I loved is
- 4:49- we did a we did a survey,
- 4:51- of our teams before entering into the program,
- 4:54- and some of the results from the survey
- 4:56- pre and post were really awesome. So let
- 4:58- me share with with you some of those.
- 5:00- So seventy two and a half percent of
- 5:02- our nurses felt rewarded for their professional efforts.
- 5:05- Seventy three percent felt that they reported, they
- 5:08- improved in their evidence based practice knowledge.
- 5:11- 80%
- 5:12- said that they would participate in the program
- 5:14- again, which was huge.
- 5:16- And ninety two percent
- 5:18- intended to remain employed at Intermountain for at
- 5:20- least two years. So to me, that was
- 5:23- outstanding enough. But then there was one more
- 5:25- thing that was so compelling and really
- 5:28- gets to the whole point of why we
- 5:29- did this, and that's to keep our nurses
- 5:30- at the bedside.
- 5:32- So we had an interesting situation where we
- 5:35- had nurses that were eligible. Some of them
- 5:37- participated, early adopters,
- 5:39- and some of them said, you know what?
- 5:41- I'm gonna wait for the first round of
- 5:42- this and see how it goes. So those
- 5:43- that were just sitting back but were eligible.
- 5:46- When we looked at those two cohorts, it
- 5:48- was very nice because we almost had a
- 5:49- study a study group and a control group.
- 5:52- And what we found is that the nurses
- 5:54- that did participate in the program
- 5:56- had an had an almost 8%
- 5:59- decrease in their turnover rate compared to those
- 6:01- that were eligible and did chose not to
- 6:03- participate.
- 6:04- So it just shows us that by being
- 6:06- in this program,
- 6:07- it really does impact the engagement of our
- 6:09- nurses and them wanting to
- 6:11- stay at the bedside.
- 6:12- They feel that their, efforts are being rewarded
- 6:16- and recognized.
- 6:17- And what I'm really excited about, Mariah, for
- 6:19- our next year is to now we'll be
- 6:21- a few years into the program
- 6:23- to see what impact it's having on the
- 6:25- quality outcomes that our nurses are delivering. Because
- 6:28- with all these evidence based practice projects that
- 6:31- we're doing, we anticipate to see an elevation
- 6:35- in all the quality of the clinical care
- 6:37- we're giving.
- 6:38- That's amazing. I love that. Now what makes
- 6:41- this, I think, even more impressive is right
- 6:43- around the same time that you launched I
- 6:46- aspire, you also launched a nurse leadership program.
- 6:49- So can you tell us a little bit
- 6:50- more about the nurse leader academy and what
- 6:52- that entails?
- 6:54- We did. And so part of why we
- 6:55- launched them at the same time is we
- 6:57- really had a desire
- 6:59- to grow all of our nurses, whether they're
- 7:01- nursing leaders or moving into nursing leadership roles,
- 7:03- like nurse manager roles, as well as our
- 7:05- frontline nurses. So this one was for our
- 7:07- nurse managers. So
- 7:09- I remember my first nurse manager role, and
- 7:12- I had been an advanced practice nurse in
- 7:15- an ICU. And I loved being at the
- 7:17- bedside caring for patients and families. And I
- 7:19- got asked the question,
- 7:22- Mandy, would you like to be the nurse
- 7:24- manager, or have you thought about being the
- 7:25- nurse manager? And I remember I'm thinking, no.
- 7:28- I haven't. And and and as I moved
- 7:30- into the role,
- 7:31- I you didn't really know what you didn't
- 7:33- know. And I remember I would always walk
- 7:35- into the unit every morning thinking,
- 7:37- I'm in I am overseeing a $20,000,000
- 7:40- budget, and I have no idea what I'm
- 7:41- doing. Like, I truly was like, do they
- 7:44- know that I had no idea what I'm
- 7:45- doing? But you worked it out as you
- 7:47- went, and I asked lots of questions. And
- 7:49- but it has always stuck with me that
- 7:52- we don't always prepare our nurse lead as
- 7:54- well. Like we have a lot of really
- 7:55- good professional development programs at Intermountain that that
- 7:58- are leadership development, but we didn't have one
- 8:01- that was specific for nursing leaders.
- 8:03- And so that's why we started with that.
- 8:05- And so I'm like, okay, It would be
- 8:08- far better for our nursing leaders to have
- 8:10- a year long
- 8:12- academy which supported their learning and all the
- 8:15- fundamentals of what they needed to know to
- 8:16- be a nurse manager.
- 8:18- And so it's a year long program,
- 8:20- and it is, structured around two six month
- 8:23- sessions, and it focuses on four key domains.
- 8:26- So it's really about their professional development as
- 8:28- a leader,
- 8:30- leading their teams,
- 8:31- leading their teams through change management,
- 8:34- and
- 8:34- achieving operational outcomes. Like, how do they how
- 8:37- do they drive for those results? But what
- 8:39- I really enjoyed about what we set up
- 8:41- was that,
- 8:43- it was really individualised to each individual nurse
- 8:46- manager, and so there was some simulation that
- 8:48- was involved where, you you know, they might
- 8:50- have a simulated environment where they're having a
- 8:52- crucial conversation with one of their caregivers,
- 8:56- and it would help them
- 8:58- by giving them some tips and some feedback.
- 9:02- And then they would have some learning modules
- 9:04- based upon what their needs were in how
- 9:06- to improve. And so that would happen throughout
- 9:08- the year multiple times.
- 9:10- I loved that the as we developed it,
- 9:12- the team,
- 9:13- of our
- 9:14- professional development team
- 9:16- worked around the core competencies that ground us
- 9:18- in the American Organization of Nursing Leaders,
- 9:22- which is fantastic.
- 9:24- And then it also provided a great network
- 9:26- of support for each other. So as new
- 9:28- nurse managers are coming in and more seasoned
- 9:31- nurse managers are in this cohort as well,
- 9:33- There was some really good networking and support
- 9:35- and mentorship.
- 9:37- Amazing.
- 9:38- And that's run through about two cohorts at
- 9:42- this point. So what have some of those
- 9:43- results been?
- 9:45- So the results have been really exciting, and
- 9:48- they the the nurse leaders that have been
- 9:50- through it said that they really did feel
- 9:52- it enhanced their leadership skills and confidence.
- 9:55- They liked that it was personalized to their
- 9:57- own opportunities,
- 9:59- and they felt that they were more empowered
- 10:01- to drive results, and foster a culture of
- 10:04- collaboration on their units. But, of course, I
- 10:06- love the data. And so we did another
- 10:08- survey, and it was an authentic nurse leadership
- 10:11- questionnaire.
- 10:12- One of the questions we asked is pre
- 10:15- and then post the first cohort.
- 10:17- It was the question was, as a leader,
- 10:19- I am confident in my ability to empower
- 10:22- my caregivers in the workplace.
- 10:25- And before going through the Nurse Leader Academy,
- 10:28- the score out of four was 2.75,
- 10:31- and then after, it was three point o
- 10:34- nine. So it had increased
- 10:36- point 34%.
- 10:38- And so what was good about that was
- 10:40- that was only just after the the course
- 10:42- had finished. They're gonna continue to grow and
- 10:44- evolve. So we're we're excited to do this
- 10:46- survey another year out and see how much
- 10:48- more that's improved up to the four point.
- 10:51- But we certainly have seen lower turnover rates
- 10:54- with our nursing leaders,
- 10:55- increased engagement by their by both their teams
- 10:58- and their participants,
- 11:00- and increasing
- 11:01- engagement scores from our caregivers. So we know
- 11:04- that we're still learning. We'll continue to adapt
- 11:07- the program to meet the needs of our
- 11:08- nursing leaders, but now we're starting to move
- 11:11- this across all of the system so all
- 11:14- of our nursing managers have a chance to
- 11:16- participate. And then the goal in mind for
- 11:18- the future would be
- 11:19- we should be putting through some of our
- 11:22- high performing
- 11:23- talent that we anticipate will want to move
- 11:25- into a nurse leadership role in the future.
- 11:27- So they're getting some of these skills even
- 11:29- before they move into these roles. So that
- 11:31- is that is that will be the next
- 11:33- evolution of the program, but we're really excited
- 11:35- to see, the responses of our nursing leaders
- 11:38- who have gone through it.
- 11:40- Incredible.
- 11:41- Well, thank you so much for being here
- 11:42- and sharing so much about your two programs.
- 11:45- This has been such a great discussion.
- 11:47- Thank you so much for having me.
