- 0:03Good morning, Good morning, good morning, and welcome to Ask the Doctor
- 0:06with doctor Carrie Williams with Med Club, Bartlesfail. How are we today?
- 0:10We're good? Are you well? You know I'm great because I've got a
- 0:13great doctor. By the way, transparency, Carrie Williams is my doctor direct
- 0:18primary care. Tell us about this particular model. This particular model is somewhat
- 0:24new in the United States. It's a practice model where patients make direct contact
- 0:30contracts with physicians primary care physicians to provide them services at mid Club. We
- 0:37have a monthly charge depending on your age, and what that covers is my
- 0:43time, my consultations care for chronic diseases and things like physicals, but we
- 0:56do midchecks. Extra things are extra labs, prescriptions, and we offer those
- 1:03at a club at a discounted rate. Whis really great. It's kind of
- 1:08like a one stop deal there. And you're not finding yourself. You know
- 1:11it's your choice, but you're not finding yourself, you know, going all
- 1:15over town to get things done. You can get most everything you need done
- 1:19right there at Med Club. Artist. Well, yes, sir, and
- 1:22I genuinely more doing them. You are now the advantage, of course of
- 1:29being a member of a med club. You got plenty of But let's take
- 1:33a look at it. You well, first of all, through transparency,
- 1:37you know how much you're going to pay every month. It's not changing.
- 1:40You know getting unexpected bills. Wait a minute, this is medical stuff and
- 1:45we're not getting unexpected bills. No, and this is usually less expensive.
- 1:49The average patient save it over twenty percent that they use versus traditional healthcare interns
- 1:57as far as they're overall cost according to the statistics, and we cut out
- 2:06the middle man. There's not an insurance company with their hands out m M.
- 2:13And the statistics show that if you're a patient of a direct primary care
- 2:19practice, you have, as a general rule, there is a sixty five
- 2:23percent less chance that you will end up in the emergency room on an emergency
- 2:28room visit. Well, that's great, but those are expensive. They are
- 2:32it's a it's not a day at Disney World and for sure, but it
- 2:37may cost as much. Oh my, yes, yes, And what I
- 2:42like is the accessibility I have to the doctor. Yes, we try to
- 2:47do same day or next day appointments. Some people like to book out further,
- 2:52but we do that. For instance, if you've had an unexpected hospitalization
- 2:57and you need to be seen quickly after you've been dis from the hospital,
- 3:00We'll try to make room for you to be seen. And a lot of
- 3:07times the number one advantages is that you get explanations of what's happened. Because
- 3:15the medical field can be very subservient. You don't know what's going on when
- 3:19you're in the hospital, So having somebody to give you a play by play
- 3:23or they did this procedure for that reason, and that's what that's providing that
- 3:31patient understanding gets through. It gets rid of a lot of the fear,
- 3:38and fear is misery, and so if we can help a patient get comfortable
- 3:45with their medical care or what's been done and what to expect, to've really
- 3:50done our job. One thing I like is that the appointments are waiting in
- 3:54a waiting room for an hour, can be seen for five minutes, well,
- 4:00there's no wait to hear. We try not to make any one way
- 4:03and limit the size of the practice. When I was in a traditional medical
- 4:09practice, I had over three thousand patients. At med Club, we limit
- 4:14our patients just seven hundred, so I have time to get to know you
- 4:19and most of the appointments are at least a half hour, and you know,
- 4:26some patients want that, some patients don't. But the explanation going back,
- 4:31the accessibility is a huge thing. Being accessible to a natural physician is
- 4:40huge. Well, you get more personalized care and for a lot of people
- 4:46that goes a long way because we used to kind of being shuffled around and
- 4:51you might only get to see your doctor three to four minutes and tops,
- 4:56yes, and then it's in out by seaa, here's your bill. Yes,
- 5:00And it's not that way there. As a patient, I get to
- 5:02ask questions about you encourage that. Yes, I encourage my patients to bring
- 5:06a list with them to the appointment written please, because you're not going to
- 5:11remember them. That's exactly right. Hey, it getting their questions answered again
- 5:19takes takes the guesswork out by the patient. You can go and read the
- 5:28computer look it up, but getting through the vocabulary is a lot of time,
- 5:32very confusing, and some of the things mean the same thing and understanding
- 5:41oh they did this that means sad and I have to anticipate this coming at
- 5:48me. Yes, every patient is different, but the disease process is kind
- 5:56of with most act similar. One thing I like is that you'll actually bring
- 6:02out a physical book if needed. Absolutely, I'm shure the patient exactly what
- 6:08is going on and why so that we understand it. And that's something that
- 6:14I've never witnessed is a patient in my entire life until being a member of
- 6:19med Club article well, providing that patient explanation and understanding, understanding how that
- 6:28doctor's thinking about you, Understanding this is where this particular organ lives, and
- 6:33showing the patient a picture and say when you talk to the doctor, they're
- 6:40going to be talking left right, antier post here, and how that doctor
- 6:46determines what that means. Then the vocabulary is not so bad. The way
- 6:53that doctor talks to you is not that confusing, but bringing that knowledge level
- 6:58up. Also, if I send a patient to a specialist, I want
- 7:02them to understand what the specialist is going to be thinking about, oh yeah,
- 7:08and what to anticipate and how not give the any answers, but how
- 7:14to answer the questions that have been asked by the specialist. Therefore, the
- 7:19communication is automatically approved by tenfold. Yes, and my patient will be looked
- 7:28at as a cooperative patient so when they want to have back in their office,
- 7:33and that makes doctors are no different than anyone else. If they like
- 7:39you, they'll spend more time with you. Now, the met Club bartletpill,
- 7:44we talked about that, the monthly fee, it covers the primary care
- 7:47consultation. Now we did kind of mention labs and another thing, tell us
- 7:53more about labs and other things if you okay, yeah, for lab basic
- 7:59labs for our office, bloodwork, during other things. And we have discounted
- 8:05prices in comparison to the national labs. We're not adding on what the insurance
- 8:11company would add or what that middleman for processing is. You know what your
- 8:18lab bill's going to be. And also with the medications. I am in
- 8:22a buying group with other direct primary care practices and we get discounted hostel medicines.
- 8:31Oh yes, you do a lot of times it will cost you less
- 8:35to purchase it at mid club, will be cost you what your insurance will
- 8:39cover. Very good. This is all really pretty cool. Now when we
- 8:46take a look at this for IRS purposes, and I know some people do.
- 8:50I know I have to these days because the I R seems to like
- 8:54me. What is there some kind of coding or something that we have to
- 8:58kind of go through here we talked about, I think called a health plan.
- 9:01A health plan, the IRIS does not allow individuals to detect the monthly
- 9:07costs from their taxes. That should and that's basically the only intervention that the
- 9:16IRIS has in this. Now, when we go through a cost and payments
- 9:20from med Club, there is the monthly fee. And also there's a lot
- 9:24of great things that come along with this, because like you said, it's
- 9:28not like every time you visit it's a monthly feed. You're going to visit
- 9:31as much as you need to visit. Correct, So if I need some
- 9:37screening, I can just come in with an appointment and we get that taken
- 9:41care of it. Like say I got an acre of pain. A week
- 9:43later, I can call up and say, hey Doc, yes, and
- 9:46it's still one monthly fee. Yeshow that here? We also text and email
- 9:56my patients and I communicate a lot. Even though you may not see me
- 10:00every month, there's a lot of communications that goes on behind behind the scenes.
- 10:05Your labs are sent to you, I have looked at them. I'll
- 10:09make comments on them, and or X rays or other tests. Patients have
- 10:16questions. You know, my medicine that I started is doing this is this
- 10:22normal or is this not? And also there's access emergency access to me after
- 10:28hours and on the weekends, So that's that cuts down on a lot of
- 10:35the er visits. It does, yeah, absolutely does we keep you well,
- 10:41We'll try. And I put an enorming amount of time into the patients
- 10:46figuring out where they're at, what needs to be done, what they've done,
- 10:50what they haven't done, and get them caught up on their screenings,
- 10:54figure out their medicines. And sometimes I've got patients that are on medicines that
- 11:01don't agree together and adjust things and figure out more than anything, figure out
- 11:09what that patient's goals are. You know, you said meet your patients where
- 11:13they are. I know a fellow who hadn't been to a doctor for twenty
- 11:16years until he visited you, and he raves about how fast you caught up
- 11:26and how fast everything came into play simply because there was communication and propertystic and
- 11:31know we're a patient, what their goals are, how they plan on living,
- 11:37what their goals are. You know, everyone wants a healthy life,
- 11:39but that's not realistic and setting the realistic expectations. Also, it's a little
- 11:48known fact doctors are not reimbursed for reading past medical records in the office.
- 11:56And there are patients who come with hundreds of pages worth of past medical records
- 12:01and that's where doctors find their discrepancies, but they it takes hours to go
- 12:07through them. And my theory on that if it's important enough to hit my
- 12:11office, this is important enough for me to read and understand and see what's
- 12:16been done. And a lot of the costs of a patient are cut down
- 12:20in that step right there, because patients don't remember if they've had a test,
- 12:26but those records will indicate yay or name that is correct, and usually
- 12:31as the result, yes, that is correct, And in understanding that and
- 12:35being able to document that, it's a big deal. Also for traditional healthcare,
- 12:41Medicare and Medicaid, there's something called medical necessity, which is a big
- 12:46nuisance. From time to time you have to prove as a physician why that
- 12:52patient needs that test, and a lot of times there's a lot of hoops
- 12:56to jump through. If I have old medical records that I am familiar with,
- 13:01sometimes I can find medical necessity in those. Now sometimes you're gonna need
- 13:09health care insurance with the direct primary care sometimes tell us about them. Sometimes
- 13:15they sometimes are on catastrophic events things that you don't anticipate. Surgeries, heart
- 13:22attacks, cancers. These are devastating to patients in their financial well being because
- 13:31it's easy to run up chains of thousands of dollars worth of medical bills.
- 13:37Also, if you've got cancer, some of those drugs are exorbitantly expensive,
- 13:43and so a high deductible health care insurance plan is what we recommend to go
- 13:50with this because in direct primary care, we can catch most of these many
- 13:56of the health care needs ahead of time and possibly ward things off or catch
- 14:03them early before they are a catastrophic event. Very good. How do we
- 14:07become a patient of doctor Cary Williams at bed Club Bartlesville. You have a
- 14:13website that will answer almost every question you may have. Also, if you'd
- 14:16like to become a patient, there's a place to sign up online. Mike
- 14:22Electronic Medical Workers were flagged me and tell me to take a look at this
- 14:28and we will contact you and set up an appointment. If you don't want
- 14:33to become a patient, but you have questions, we have one appointment up
